Description
Due to a disease, often cancer, it may be necessary to surgically remove the larynx (laryngectomy). By removing the larynx, some important functions are lost. The epiglottis and its closing function are lost, so an opening in the trachea, a tracheostoma, must be created on the neck of the patient to establish a direct connection with the trachea. The tracheostomy is sometimes lowered (lowered) relative to the surface of the patient's neck around the stoma and may be of irregular shape, as it is often necessary to remove tissue around the stoma.
It is also known to connect a type of heat and moisture exchanger with the tracheostoma. Therefore, the term "tracheostoma valve" as used in the present specification is intended to encompass any other device associated with the tracheostoma.
Regardless of the type of device connected to the tracheostoma, the most common way to make this connection is by means of an annular flange which is attached to the neck of the patient by means of an adhesive. However, there is also a disposable patch on the market for attaching tracheostoma valves to the neck, this patch being formed from a single coated band attached to a flange. The flange forms a base and a flat flange secured to the band. The patch has a generally flat surface to be attached to the skin surrounding the tracheostoma. This type of patch causes problems with tension and incomplete contact with the skin near the tracheostoma due to the fact that the stoma is usually lowered or lowered.
Other prior art means for securing to the neck of a patient in conjunction with a tracheostomy include an annular flange and a pedestal integrally formed with the flange and projecting on one side thereof at an angle to the axis of the pedestal. The tracheostoma valve is exchangeable in the socket. The flange is made of soft PVC. On the other side of the flange, a double-coated tape is attached as means for attaching the flange to the skin surrounding the tracheostoma. The tape must not extend beyond the edge of the flange as any exposed adhesive of the tape will attract dirt and dust that will adversely affect the tape. The flange is intended for reuse. Thus, if the band is to be replaced, the patient must first remove the double-coated band from the flange, clean the flange with a solvent, allow the flange to dry, and then attach a new, double-coated band to the flange. This procedure is repeated at least once a day.
Although the products of the above type work well in many ways, there are still some problems. The conventional products are usually of the reusable type, which means that laborious cleaning must be done by means of unhealthy solvents. Often, the patient is an elderly person and has difficulty completing these steps. So that the tape adheres to the flange, this is made of plasticized polyvinyl chloride (PVC). This plastic is known to cause health problems and should accordingly be avoided. The band is attached to the entire area of the flange, which means that it can relatively easily detach from the skin due to the fact that a large force is concentrated on the band edge near the tracheostoma. When the tracheostoma valve is used or needs to be removed while speaking or coughing, the pedestal is subjected to large axial forces which put the band edge adjacent to the pedestal under tension. Since the flange is relatively small and the tape does not extend beyond it or at least not far beyond it, it is difficult to obtain sufficient adhesion to patients with a large, irregular tracheostoma. The flange is inclined, however, due to the tracheostoma being frequently lowered or lowered, and the skin around the tracheostoma often relatively flat, the surrounding skin adapts to the unnatural shape, and in this way, tensions in the tissues and thus complaints are generated for the patient. There is also an increased risk that the patch will be easier to detach from the patient's neck.
An object of the present invention is to reduce the functional height at a distance plaster and height of the inserted functional part such. B. HME-speaking valve, etc. Another object is, by a special clamping element, for. B. clamping cone to achieve stabilization and additional sealing of the tracheostomy. The clamping element is connected to the adhesive layer of the plaster and thus points in the direction of the tracheostoma. This achieves a very low functional height. The wearing and operating comfort for the patient are improved.
The invention is explained in more detail by the attached schematic drawings and illustrated visually.
