Dabei werden die Schablonen äußerlich der Thoraxform angepasst und Implantat gegenüber der Schablone um 1–2 cm kürzer gewählt, da dieses im Endeffekt im subcutanen Bereich liegt. Der Pectus Bar wird mittels eines Bandes in den Thorax gezogen, wobei die Enden des C-förmig gebogenen Bügels in ventrale Richtung weisen. Danach wird das Implantat mit speziellen Implantationshilfen um 180° gedreht und seitlich an den Enden des Bügels die Stabilisatorplatten befestigt und mit einer Drahtnaht am Bügel fixiert. Die Stabilisatorplatten werden durch nicht resorbierbare Nähte an der seitlichen Thoraxwandmuskulatur gesichert. Der gebogene Bügel hebt das Brustbein an und zwingt den Brustkorb über den Zeitraum der Implantation, welcher üblicherweise im Bereich von 2–3 Jahren liegt zu seinem normalen Wachstum zurück.
