Description

  • Ungekoppelte Knieendoprothesen bestehen aus einer tibialen und femoralen metallischen Komponente und einem dazwischen positionierten Tibiaplateau. Dieses bildet mit der femoralen Komponente die artikulierenden Flächen des Gelenks. Es besteht in der Regel aus Polyäthylen, in der Zukunft sind auch andere, abriebärmere Werkstoffe denkbar. Bei den üblichen Endoprothesen der verschiedenen Hersteller ist die Höhe des Plateaus im medialen und lateralen Abschnitt stets gleich. Im Idealfall ist also der Spalt zwischen der metallischen femoralen und tibialen Komponente bei ausgeglichener Weichteilspannung und korrekter Positionierung der beiden Komponenten beiderseits gleich weit. Er wird bei komplett eingebrachter Prothese durch das Tibiaplateau ausgefüllt.
  • Das Erreichen einer optimalen Achsausrichtung (Alignment) bei der Implantation von Knieendoprothesen ist eine wichtige Voraussetzung für das Langzeitüberleben der Prothese. Dies gilt vor allem für nicht gekoppelte Prothesen. Varus- oder Valgusfehlimplantationen von über 2 – 3 Grad gehen mit einer Fehlbelastung der implantierten Prothesenteile und meist auch mit Weichteilinstabilitäten (z. B. Kollateralbandinstabilitäten) einher. Die operative Korrektur bestand bisher in der Explantation und korrekten Neuimplantation der kompletten Prothese.
  • Der in den Schutzansprüchen angegebenen Erfindung liegt das Problem zugrunde, eine Achskorrektur vorzunehmen, ohne die im Knochen verankerten metallischen femoralen und tibialen Prothesenkomponenten zu entfernen.
  • Dieses Problem wird mit den in den Schutzansprüchen aufgeführten Merkmalen gelöst.
  • Mit der Erfindung wird erreicht, dass nach irrtümlicher Fehlpositionierung der beiden im Knochen fest verankerten metallischen Komponenten ein normales Alignment unter Belassung beider Komponenten hergestellt werden kann, wenn ein asymmetrisches, dem Ausmaß der Fehlstellung entsprechendes Tibiaplateau eingesetzt wird.
  • Diese Maßnahme kann sowohl bei der Primärimplantation als auch als spätere Achskorrektur erfolgen.
  • Dabei ist es ohne Bedeutung, ob es sich um ein sog. „Mobile Bearing" oder um ein fest mit dem tibialen Teil verankertes Plateau handelt. Durch entsprechende Gestaltung des Plateaus können Achskorrekturen im Varus-, Valgus-, aber auch Extensions- und Flexionssinne durchgeführt werden.
  • Ein Ausführungsbeispiel zur Varus- bzw. Valguskorrektur der Erfindung wird anhand der 1 bis 3 erläutert. Es zeigen:
  • 1 ein normales Tibiaplateau (1) mit beidseits gleicher Höhe (2),
  • 2 eine metallische tibialeKomponente (3) mit einem normalen Tibiaplateau (1),
  • 3 ein 4-Grad asymmetrisches Tibiaplateau (4) mit beiderseits unterschiedlichen Höhen (2,5) zur Varus- bzw. Valguskorrektur.
  • Analog zu diesem Beispiel ist die Höhe des Plateaus zur Korrektur einer Extensions- bzw. Flexionsfehlstellung vorne und hinten unterschiedlich.

Claims (3)

  1. Asymmetrisches Tibiaplateau für ungekoppelte Knieendoprothesen, dadurch gekennzeichnet,
  2. dass für die Korrektur einer Varus- bzw. Valgusfehlstellung das Tibiaplateau (4) auf beiden Seiten entsprechend der zu korrigierenden Fehlstellung unterschiedlich hoch ist, d. h. die Höhe des Plateaus nimmt von der niedrigeren Seite (2) kontinuierlich bis zur anderen Seite (5) hin zu, und
  3. dass für die Korrektur einer Extensions- bzw. Flexionsfehlstellung das Tibiaplateau vorne und hinten entsprechend der zu korrigierenden Fehlstellung unterschiedlich hoch ist, d. h. die Höhe des Plateaus nimmt von vorne nach hinten kontinuierlich ab bzw. zu.
