Abstract

Es wird eine Femurprothese für die Hüftendoprothetik beschrieben mit einem zum Einsetzen in den Markhohlraum des Femurs bestimmten Schaft (10), der sich vom proximalen Ende zum unteren distalen Ende (22) hin verjüngt. Auf der ventralen Seite verläuft der Schaft in zwei in Längsrichtung aneinander anschließenden Bögen (18, 20) und stützt sich mit einem mittleren Bereich (24) an dem ventralen Kortikalisplanum ab. An der dorsalen Seite des Schaftes (10) stützt sich dieser mit einem proximalen Bereich (26) und einem distalen Bereich (28) ab, wobei zwischen diesen Bereichen (26, 28) die dorsale Seite als ebene Fläche oder als nach posterior konkave Fläche (32) ausgebildet ist.

Description

Die Erfindung betrifft eine Femurprothese für die Hüftendoprothetik gemäß dem Oberbegriff des Anspruchs 1.
Es ist bekannt, bei der Hüftendoprothetik Femurprothesen zu verwenden, deren Schaft der anatomischen Form des Markhohlraums des Femurs angepaßt ist. Solche Femurprothesen sind z. B. aus der EP 0 528 284 B1 und in ähnlicher Form aus der GB 2 281 213 A bekannt. Bei diesen bekannten Femurprothese verlaufen die ventrale Seite und die dorsale Seite des Schaftes jeweils in zwei in Längsrichtung aneinander anschließenden Bögen, so daß sich in lateraler Sicht die Form eines langgestreckten S ergibt. Der obere proximale Bogen ist auf der ventralen Seiten nach anterior konkav und auf der dorsalen Seite nach posterior konvex gebogen, während der untere distale Bogen auf der ventralen Seite nach anterior konvex und auf der dorsalen Seite nach posterior konkav gekrümmt ist. Der in das Femur eingesetzte Schaft stützt sich in seinem axial mittleren Bereich ventral an dem Kortikalisplanum ab. Dorsal stützt sich der Schaft im oberen proximalen Bereich und an seinem unteren distalen Ende an der Kortikalis ab. Da der sich abstützende obere proximale Bereich der dorsalen Seite konvex gebogen ist, kann die Zone, in der die Abstützung an der Kortikalis stattfindet, bei Belastung axial wandern, was zu einer Instabilität der Abstützung und im ungünstigsten Falle zu einer Lockerung des Prothesenschaftes führen kann.
Der Erfindung liegt die Aufgabe zugrunde, eine Femurprothese zur Verfügung zu stellen, die einen stabilen Sitz mit längerer Haltbarkeit gewährleistet.
Diese Aufgabe wird erfindungsgemäß gelöst durch eine Femurprothese mit den Merkmalen des Anspruchs 1.
Vorteilhafte Ausführungen der Erfindung sind in den Unteransprüchen angegeben.
Der wesentliche Gedanke der Erfindung besteht darin, die dorsale Seite des Schaftes der Femurprothese so zu formen, daß unabhängig von der Belastung der Prothese zuverlässig nur zwei definierte Abstützflächen wirksam werden. Dadurch ist in Verbindung mit der ventralen Abstützung im Bereich des Kortikalis­ planums sichergestellt, daß der Schaft unabhängig von der Belastung der Femur­ prothese stets in denselben drei definierten Flächenbereichen abgestützt ist und eine stabile Drei-Punkt-Abstützung in der ventral-dorsalen Ebene dauerhaft ge­ währleistet ist.
Um zwei definierte Abstützbereich auf der dorsalen Seite des Schaftes zu erzeu­ gen, ist die dorsale Seite des Schaftes von dem oberen proximalen Bereich zu dem unteren distalen Ende hin in einer ebenen oder einer nach posterior konkav gebogenen Fläche ausgebildet. Der Schaft stützt sich dadurch dorsal nur in dem oberen proximalen Bereich und im Bereich des distalen Endes ab, während in dem dazwischenliegenden Bereich eine im wesentlichen drucklose Berührung zwischen der dorsalen Seite des Schaftes und der Kortikalis vorhanden ist.
Im folgenden wird die Erfindung anhand eines in der Zeichnung dargestellten Ausführungsbeispiels näher erläutert. Es zeigen
Fig. 1 eine ventrale Ansicht einer Femurprothese für das rechte Hüftgelenk,
Fig. 2 einen Querschnitt durch die Prothese entlang der eingezeichneten Schnittlinie,
Fig. 3 eine laterale Ansicht der Femurprothese der Fig. 1 für das rechte Hüftgelenk und
Fig. 4 eine Fig. 3 entsprechende laterale Ansicht der Femurprothese für das linke Hüftgelenk in einer zweiten Ausführungsform.
Die Femurprothese kann zementfrei oder zementiert in den Markhohlraum des Femurs eingesetzt werden. Die Femurprothese besteht vorzugsweise aus einer hochfesten Titanlegierung, wobei die Oberfläche im distalen Bereich durch Grob­ strahlung aufgerauht ist.
Die Femurprothese weist einen Schaft 10 auf, an dessen proximalem oberen Ende eine Schulter 12 ausgebildet ist. An der Schulter 12 ist ein Ansatz 14 ange­ formt, um ein Angreifen an der Femurprothese für das Einsetzen und eventuelle Herausziehen des Schaftes 10 mittels eines geeigneten Instruments zu erleich­ tern. Weiter ist an dem proximalen Ende des Schaftes 10 medial unter einem Antetorsionswinkel ein Hals 16 angeformt, der einen Konus zum Aufstecken eines nicht dargestellten Gelenkkopfes aufweist.
Der Schaft 10 weist einen Querschnitt auf, der sich von dem proximalen Ende zu dem distalen Ende hin verjüngt. Wie Fig. 2 zeigt, hat der Querschnitt die Form eines Polygons mit abgerundeten Ecken, so daß sich auf der Außenfläche des Schaftes 10 in Schaftlängsrichtung verlaufende zum distalen Ende des Schaftes hin schmaler werdende Flächen bilden.
In Fig. 3 ist eine laterale Ansicht der Femurprothese für das rechte Hüftgelenk gezeigt. Es ist deutlich der Antetorsionswinkel des Halses 16 gegenüber der lateral-medialen Ebene erkennbar. Auf der ventralen Seite, die in der Darstellung der Fig. 3 die rechte Seite ist, weist der Schaft 10 zwei axial aneinander an­ schließende Bögen auf. Ein oberer proximaler Bogen 18, der sich etwa über die halbe Schaftlänge erstreckt, ist nach anterior konkav gekrümmt, während der anschließende untere distale Bogen nach anterior konvex gekrümmt ist. Die ventrale Seite des Schaftes 10 weist damit die Form eines langgestreckten fla­ chen S auf. Durch die Krümmung des unteren distalen Bogens 20 läuft das distale Ende 22 des Schaftes 10 mit einer leichten Krümmung nach posterior aus. Im unteren Bereich des proximalen Bogens 18 liegt die ventrale Seite des Schaftes 10 bei eingesetzter Femurprothese an dem ventralen Kortikalisplanum des Fe­ murs an. Dieser abstützende Bereich ist mit 24 bezeichnet. In dem Querschnitt der Fig. 2 ist erkennbar, daß dieser abstützende Bereich 24 abgeflacht ist.
Die in Fig. 3 linke dorsale Seite des Schaftes 10 stützt sich mit einem proximalen oberen Bereich dorsal an der Kortikalis ab. Ebenso stützt sich der Schaft 10 an seiner dorsalen Seite mit dem nach posterior auslaufenden distalen Ende 22 in einem distalen Bereich 28 ab. Zwischen dem proximalen Bereich 26 und dem distalen Bereich 28 verläuft die dorsale Seite in einer geraden ebenen Seiten­ fläche 30.
Durch diese Formgebung des Schaftes 10 ist gewährleistet, daß sich der in dem Markhohlraum des Femurs eingesetzte Schaft 10 in der dorsal-ventralen Ebene d. h. in der Zeichenebene der Fig. 3 gegen Kippbelastungen in dieser Ebene im wesentlichen in drei definierten Bereichen abstützt, nämlich auf der dorsalen Seite im proximalen Bereich 26 und im distalen Endbereich 28 und auf der ventralen Seite in dem an dem Kortikalisplanum anliegenden mittleren Bereich 24. Dadurch ergibt sich eine stabile Drei-Punkt-Abstützung des Schaftes 10.
Fig. 4 zeigt eine Abwandlung der Femurprothese. Da Fig. 4 eine Femurprothe­ se für das linke Hüftgelenk zeigt, ergibt sich eine zu Fig. 3 spiegelsymmtrische Form. Der Schaft 10 ist auf seiner ventralen Seite (in Fig. 4 die linke Seite) bis auf die spiegelbildliche Ausgestaltung mit dem Schaft 10 der Fig. 3 identisch.
Die dorsale Seite des Schaftes 10 der Fig. 4 ist gegenüber dem Schaft der Fig. 3 dahingehend abgewandelt, daß die dorsale Seite in dem Längenbereich des Schaftes 10 zwischen dem abstützenden proximalen Bereich 26 und dem ab­ stützenden distalen Bereich 28 in einer nach posterior konkaven Fläche 32 einge­ wölbt ist. Durch die konkave Einwölbung der Fläche 32 zwischen dem abstützen­ den proximalen Bereich 26 und dem abstützenden distalen Bereich 28 ist verstärkt gewährleistet, daß sich der Schaft 10 dorsal nur in den definierten Bereichen 26 und 28 abstützt, während in dem dazwischenliegenden Bereich der konkaven Fläche 32 keine wesentliche Druckabstützung erfolgt.
Es ist ohne weiteres ersichtlich, daß die Form der Fläche zwischen dem proxima­ len dorsalen Bereich 26 und dem distalen dorsalen Bereich 28 für die Erfindung nicht wesentlich ist, sofern nur sichergestellt ist, daß die dorsale Seite des Schaf­ tes 10 in diesem Bereich nicht nach posterior über eine geradlinige ebene Verbin­ dungsfläche 30 hinausragt. Soweit diese Forderung erfüllt ist, ist gewährleistet, daß keine die Stabilität der Drei-Punkt-Abstützung beeinträchtigende zusätzliche Abstützung der dorsalen Seite im Bereich zwischen dem proximalen Bereich 26 und dem distalen Bereich 28 stattfindet.

Claims (6)

1. Femurprothese für die Hüftendoprothetik, mit einem zum Einsetzen in den Markhohlraum des Femurs bestimmten Schaft, der sich von proximal nach distal hin verjüngt und der auf seiner ventralen Seite in zwei in Schaftlängsrichtung aneinander anschließenden Bögen verläuft, nämlich in einem oberen proximalen nach anterior konkaven Bogen und einem unte­ rem distalen nach anterior konvexen Bogen, dadurch gekennzeichnet, daß die dorsale Seite des Schaftes (10) zwischen einem oberen proximalen Bereich (26) und einem unteren distalen Bereich (28) in einer Fläche (30, 32) verläuft, die nach posterior nicht über eine ebene Verbindungsfläche zwischen dem proximalen Bereich (26) und dem distalen Bereich (28) überragt.
2. Femurprothese nach Anspruch 1, dadurch gekennzeichnet, daß die dorsale Seite des Schaftes (10) von dem oberen proximalen Bereich (26) zu dem unteren distalen Bereich (28) hin in einer ebenen Fläche (30) verläuft.
3. Femurprothese nach Anspruch 1, dadurch gekennzeichnet, daß die dorsale Seite des Schaftes (10) von dem oberen proximalen Bereich (26) zu dem unteren distalen Bereich (28) hin in einer nach posterior konkav gebogenen Fläche (32) verläuft.
4. Femurprothese nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, daß der untere distale Bereich (28) der dorsalen Seite durch das nach posterior auslaufende distale Ende (22) des Schaftes (10) gebildet ist.
5. Femurprothese nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, daß der Querschnitt des Schaftes (10) die Form eines Polygons mit abgerundeten Ecken aufweist, wodurch die Außenfläche des Schaftes (10) in Schaftlängsrichtung verlaufende, sich zum distalen Ende (22) des Schaftes verjüngende Flächen bildet.
6. Femurprothese nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, daß sich der Schaft (10) in der ventralen-dorsalen Ebene in dem mittleren Bereich der Schaftlänge ventral an dem Kortikalisplanum und in seinem proximalen und distalen Bereich dorsal abstützt.
