Problemstellung: Die Hornhaut ist bezüglich ihrer Zusammensetzung
bislang nicht vollständig charakterisiert. Insbesondere fehlten bis vor
kurzem Daten über die Elektrolytzusammensetzung in den verschiedenen
Schichten der Hornhaut. Diese Daten stehen mit einer neu entwickelten
Methode jetzt zur Verfügung (Schrage et al. 1995). In der heutigen
Lokaltherapie am Auge spielen vornehmlich pharmazeutische
Überlegungen der Wirksamkeit, Haltbarkeit und Applizierbarkeit im
Vordergrund der Überlegungen. In Patenten, wie z. B. Pt. Nr. 0437368 B1
und 0424 042 B1 (Merck Sharp Dohme), die neuartige Suspensionen in
Polymerflüssigkeiten beschreiben, ist die Elektrolytzusammensetzung
nicht berücksichtigt. Die Elektrolytzusammensetzung ist allenfalls
isoosmolar und isoton zur Tränenflüssigkeit. Weitere Beispiele von
Phosphatpuffer und Boratpuffer enthaltenden Augenlösungen werden in Pt
Nr. DE 35 07 024 C2 beschrieben. In eigenen Arbeiten zur mineralischen
Zusammensetzung der Cornea konnten wir eine pathologische
Zusammensetzung der Hornhaut nach Verätzung und lokaler Tropf- und
Spültherapie am Auge beweisen. Diese Zusammensetzungsänderung ließ
sich in systematischen Versuchen durch Zusätze verschiedener
Elektrolyte zu den Spülmedien gezielt verändern. Da wir im Verlauf der
Experimente Phosphat als unphysiologisches Element in der Hornhaut
identifizieren konnten und vielfache Belege im Experiment und in der
Anwendung am Patienten für Hornhautverkalkungen durch dauernd

Stand der TechnikState of the art
In Lehrbüchern der Pharmazeutik wird das Thema Augentropfen bezüglich
der Applikation und Herstellung von wirksamen Tropflösungen behandelt
(Dolder, Skinner 1989). Insbesondere wurden Versuche zur Verträglichkeit
von NaCl Lösungen von 0% bis 2% Lösungen durchgeführt und ein
Verträglichkeitsbereich, der ohne brennen vertragen wird, von ca. 04,%
bis 1,4% Lösungen ermittelt. Weiterhin sind Dextrane als entquellende
nichtdissoziierende Zusätze untersucht und ihre entquellenden
Eigenschaften beschrieben worden. Insbesondere in der Organkultur für
Hornhäute wird dieses Verfahren benutzt. Die Möglichkeit, eine Lösung zu
puffern, wird nach Maßgabe des pK Wertes der zu applizierenden
Substanzen vorgeschlagen. Insbesondere in der Applikation von
Spüllösungen und Augenbädern nach Verätzungen wir Phosphatpuffer auf
die Hornhaut appliziert (Isogutt (Winzer pharma) und TimOphthal
(Chibret)). Beide Lösungen enthalten Na-K-phosphatpuffer in ca. 140
mmol/l Konzentration. Diese Pufferung ist wie wir aus den in Tab. 1
