Description
Die Erfindung betrifft eine Stimulationssonde zur passageren transösophagealen
Vorhofstimulation zum Zwecke einer diagnostischen Ischämieprovokation des
Herzmuskels. Diese soll durch eine tachykarde Stimulation des linken Vorhofes erzeugt
werden.
Bei einer koronaren Herzkrankheit wird durch die schnelle Stimulation des Herzens
über einen erhöhten Sauerstoffbedarf eine Ischämie des Herzmuskels erzeugt,
objektivierbar durch EKG-Veränderungen und Wandbewegungsstörungen der linken
Herzkammer in einer simultan durchgeführten transthorakalen Echokardiographie
(Feigenbaum, Stress echocardiography: an overview, Herz 16, 1991, S. 347-354).
Für dieses diagnostische Verfahren soll eine schmerzfreie Stimulation gewährleistet
sein. Hierzu wird eine kardiale Reizschwelle im linken Vorhof unterhalb der
Schmerzschwelle im Ösophagus angestrebt. Mit bisher verfügbaren
Stimulationssonden waren schmerzfreie transösophageale Stimulationen mangels
stabiler Sondenfixation an der Ösophaguswand in Vorhofhöhe problematisch.
Ermöglicht wird eine ausreichend niedrige Reizschwelle mittels möglichst naher
Fixierung der Stimulationselektroden an der Ösophaguswand in Vorhofhöhe: Durch die
anatomisch oval ausgeformte Konfiguration des Sondenballons nach Patentanspruch
1 wird nach dem Aufblasen des Ballons eine vollständige Wandanlegung und stabile
Sondenfixierung im Ösophagus erreicht, ohne eine schmerzhafte Überdehnung der
Ösophaguswand zu bewirken.
Eine orale, nach der Positionierung anzubringende Fixationsplatte nach
Patentanspruch 3 verhindert einen Weitertransport der Stimulationssonde in Richtung
Magen durch die Ösophagusperistaltik. Die korrekte Lage der Stimulationssonde an der
Ösophaguswand in Vorhofhöhe wird vor der Elektrostimulation durch Ableiten eines
EKGs über die Stimulationselektroden nach Patentanspruch 2 dokumentiert. Die
räumliche Orientierung erfolgt über einen Vergleich der möglichst hohen
Vorhofpotentiale im Vergleich zu den Kammerpotentialen im abgeleiteten EKG.
Hierdurch wird sichergestellt, daß die nachfolgende tachykarde Stimulation im Vorhof
und nicht in der Herzkammer erfolgt.
Das Verfahren soll als schmerzlose transösophageale Stimulation bei Patienten mit
koronarer Herzkrankheit Anwendung finden, die sich nicht mehr ausreichend
ergometrisch belasten können und bei denen eine pharmakologische Stimulation des
Herzens zur Ischämiediagnostik nicht in Frage kommt.
Eine vorteilhafte Ausgestaltung der erfindungsgemäßen Stimulationssonde ist in
anliegender Zeichnung angegeben. Für die erfindungsgemäße Stimulationssonde wird
eine Sonde mit zwei distal angebrachten Elektroden (2) im Abstand von 2-4 cm zur
bipolaren Stimulation sowie wahlweisen EKG-Registrierung verwendet. In der Sonde
befindet sich ein Luftkanal mit einem proximalen Lufteinlaß (3), passend für
handelsübliche Dreiwegehähne und Spritzen. Distal befindet sich in Höhe der
Stimulationselektroden ein Ballon (1), der mit ca. 20 ml Luft nach der
transösophagealen Positionierung aufgefüllt wird.
Zur besseren Orientierung ist die Sonde alle 10 cm mit ringförmigen Markierungen
versehen.
Nach der peroralen oder transnasalen Positionierung wird die Stimulationssonde mit
einer Fixationsplatte (5) an der oralen Seite der Sonde gesichert, um eine
Vorwärtsbewegung der Stimulationssonde magenwärts durch die Ösophagusperistaltik
auszuschalten.
EKG-Ableitung und Elektrostimulation erfolgen alternativ über die bipolaren
Stimulationselektroden. Die Sonde wird hierzu an ein externes Registrier- und
Stimulationsgerät über Steckverbindungen (4) angeschlossen.
Claims (3)
1. Stimulationssonde zur passageren transösophagealen Vorhofstimulation zum
Zwecke einer diagnostischen Ischämieprovokation des Herzmuskels, gekennzeichnet
durch einen aufblasbaren, anatomisch oval geformten Ösophagusballon (1) an der
Sondenspitze, der einen direkten und stabilen Wandkontakt im Ösophagus in Höhe des
linken Vorhofs des Herzens gewährleistet.
2. Stimulationssonde nach Patentanspruch 1, gekennzeichnet durch einen Abstand
der Elektroden (2) von 2-4 cm zur wahlweisen Elektrostimulation des linken Vorhofes
des Herzens oder Registrierung von EKG-Ableitungen.
3. Stimulationssonde nach Patentanspruch 1, gekennzeichnet durch eine oral an der
Sonde zu befestigenden Fixationsplatte (5), die nach Positionierung der
Stimulationssonde eine Lageveränderung durch die Ösophagusperistaltik verhindert.
