Description
The present invention relates to a one-hand implantation device for implanting pieces of metal wire in Heme or Lymphangiomas and a cannula a wire piece is contained in their working channel, the cannula in the implantation device according to the invention can be used and the piece of metal wire preferably consists essentially of Magnesium exists. Accordingly, the present invention relates in particular the use of implantation devices, in particular implantation guns and therefor adapted cannulas, the with metal wires, especially loaded with magnesium wire for implantation wires in heme and lymphangiomas.
Childish and adult haem lymphangiomas at large Expansion is a therapeutic entity that is difficult to treat Is hemangioma a benign tumor of the blood vessels. of origin is the endothelium, a single layer of cells that covers all blood vessels to the smallest branches, the capillaries. Everywhere over there, where blood vessels occur can hence hemangiomas occur, e.g. in the skin, in internal organs but also in the bones. Hemangiomas become colloquial also known as "blood sponges". you can have two forms of hemangiomas distinguish: the capillary hemangioma and the cavernous Hemangioma. Moreover there are still special forms, such as the intramuscular, so hemangioma located in the muscle. Hemangiomatosis one speaks when multiple hemangiomas at the same time available. hemangiomas are mainly found in the skin of the face and trunk can but also in different organs, e.g. the liver, the kidneys, in the brain, spleen or lungs.
If there are multiple hemangiomas at the same time, is hemangiomatosis. she are likely to result from an embryonic malformation and are part of a systemic disease with other malformations. Such rare diseases include, for example, Sturge-Weber disease, characterized by hemangiomas of the soft meninges and the face as well as simultaneous eye changes, e.g. a glaucoma; Hippel-Lindau syndrome, with typical hemangiomas of the retina and cerebellum, as well as any cystic changes internal organs, e.g. the kidney, lungs or liver; the Klippel-Trenaunay syndrome with multiple hemangiomas mostly on one extremity, that is, on the arms or legs, connected to a giant growth in the affected Section.
The malformations caused by the growth can Soft tissues and joints enforce what is diagnosed appropriate clinic can be demonstrated. As a classic Examples are lymphangiomas, vein nectasias, but also the naevus flammeus, the spider naevus and the «stork bite». Syndromes such as «Hippel-Lindau», «Sturge-Weber» and «Klippel-Trenaunay» are also popular to find this group. The blood sponge occurs in the first weeks of life in appearance and growing then disproportionately to the growth of the child. It can be infiltrating and destructive to grow.
The causes of the development of hemangiomas have not yet been clarified. However, genetic factors seem to be particularly common among the generalized ones hemangiomas, of greater importance to be.
The goal of the treatment is in primarily the interruption of progression.
As therapy options are in the early Contact cryotherapy is available the application of cold for therapeutic purposes. However, cold therapy is quite painful and leads almost always scars. Laser therapy is used for superficial, small hemangiomas as a pulsed color laser, with deeper than neodymium-yag laser applied. However, laser radiation is also very painful. Moreover the deeper penetrating neodymium-yag laser carries the risk of Scarring in itself. In addition, the purchase of a laser device with high Associated costs. Interferon or systemic may also develop Cortisone therapy used. However, these chemical interventions hide the risk of side effects. X-ray radiation is also a problem size Burden for the body A surgical intervention is strong in extensive cases complicated by the high tendency to bleed.
Since the late 19th century is therefore a reduction through the implantation of magnesium wire performed in the lesion. This metallic magnesium wire is supplied in rolls and is difficult to apply. For the material is autoclaved, small arrows with a length of 10 mm to 20 mm cut out of the magnesium foil and for some Immersed in 10% acid solution for seconds.
The application of this wire is so far about commercial needles carried out, which have an inner diameter of 1.0 mm or 0.5 mm. It is however difficult to cut the wire into pieces through this cannula to push into the tumor.
The invention is based on this based on the task, inexpensive and effective agents for the treatment of heme (lymphangioma) put.
The solution to the task succeeds by providing a magnesium wire application gun, the magnesium pieces by means of handle and standardized ejector drifts. Ready-made "loaded" cannulas are also provided, into the one or more pre-assembled magnesium wires sterile are used and in which only the package is still open and be attached to the reusable pistol-like handle got to. In order to succeeds in a problem-free and standardized application of Magnesium wire pieces in the heme (lymph) angioma.
Further advantages of the invention exist in the light and standardized application of the magnesium wire. Moreover deleted cumbersome procurement of the wire in large Roll and it can be dosed individually. Furthermore, the so-called chemical sterilization, in which the wire before application, for example soaked in acetic acid and has to be neutralized afterwards. Therefore, advantages are one easier and sterile handling even under standardized conditions. Finally allow the implantation device according to the invention and cannula save the attending doctor time and a simplified surgical procedure to guarantee. Also would be Clinics or medical practices are not obliged to invest in expensive laser devices. The magnesium wire gun according to the invention is used associated loaded cannula for example in the clinics and practices of dermatology, neck, Nose, ear and eye medicine.
Goodfellow GmbH, Bad Nauheim, Deutschland. In a first aspect therefore concerns the present invention a one-hand implantation device for implantation of metal wires in hammers and Lymphangioma with a cannula receiving housing, one in the case longitudinal, relative to the cannula oriented ejector, and one, the expulsion in the housing effective, can be operated with one hand Actuator, being after actuation the actuator in the cannula stored piece of wire be introduced into the body can and with the piece of wire preferred consists essentially of magnesium. Although pure pieces of magnesium wire are preferred, can u.U. magnesium alloys or pieces of wire are also used consist only partially of magnesium, for example only in those is the part made of magnesium that comes into contact with the tumor. Metal wires, magnesium wire in particular can be obtained, for example, from from Goodfellow GmbH, Bad Nauheim, Germany.
Implantation devices which are suitable or can be prepared for use according to the invention can be found in the prior art. For example, various so-called “seed” implantation applicators for the use of radionuclides in brachytherapy are described. Seeds are small, 2.5 mm long and 0.8 mm large radioisotope preparations, for example 198 Au or 125 I, for the most permanent interstitial radiation therapy, the implantation of which into the tissue is carried out by means of a cannula and an implant gun, such implant guns are described, for example, in WO97 / 22379 and in WO99 / 20337 The pistol is loaded and then the needle is inserted into the tumor. By pulling on the impression, a stylet is advanced by the length of the specimen, ie in this case 2.5 mm, and a seed is pushed out of the needle into the tissue with each impression.

As already mentioned above, points the housing the implantation device according to the invention preferably in the form of a pistol, the one receiving the cannula Part in the barrel-like upper housing part and the actuating device designed in the form of a trigger in the manner of a handle trained lower housing part is arranged. In a particularly preferred embodiment is the implantation device according to the invention a magnesium wire gun.
As already described above, stands out the implantation device according to the invention characterized by the fact that sterilized, already loaded with pieces of wire needles can be included. Accordingly, in the implantation device according to the invention the one that holds a cannula casing with a recess for interchangeable Adaptiereinsätze Mistake. In a particularly preferred embodiment, the implantation device according to the invention has a Luer or, particularly preferably, a Luer-Lock standard connection for receiving corresponding ones Luer or Luer-Lock cannulas on.
The housing of the implantation device according to the invention can be made of plastic or metal, for example, or both, light metal is preferred.
Act as described above it is preferred in the implantation device according to the invention and advantageously around a magnesium wire gun, i.e. around a Device for Implantation of magnesium wire (spiked) in hem (lymph) angiomas. It is known to have hemangiomas can be treated with magnesium wire. For this, the material is autoclaved, small arrows with a length from 10 to 20 mm cut out of the magnesium foil and for some Immersed in 10% acid solution for seconds. The magnesium is then by means of a cannula through the skin into the hemangioma promoted.
With the implantation device according to the invention for the treatment of heme / lymphangiomas it is now possible Magnesium wire pieces easier and faster to apply under the skin. The manual cutting of the magnesium wire during or before the operation does not apply so that the doctor to be treated needs a shorter preparation time. Further allows the provision of pre-made cannulas and Magazines that use the magnesium wires contain an elevated Degree of Sterility. Therefore, the implantation device according to the invention particularly beneficial for the implantation of magnesium wire for the treatment of angioma, preferably a hammers or lymphangioma. As described above, there is the implantation device or implantation set preferably from a device with a Handle and a standardized ejector, which drive out the magnesium pieces can and a prefabricated cannula and / or a "magazine", which has already contain sterile magnesium parts and only inserted into the pistol Need to become.
In weiteren Ausführungsformen können die erfindungsgemäßen Implantationsvorrichtungen
vorteilhafterweise auch Tiefenmesser besitzen. Dies können beispielsweise
mechanisch und mittels eines Rasters funktionieren. Andere Tiefenmesser
As described above the piece of wire in the cannula or in the working channel of the cannula preferably consists essentially of magnesium and has a length of 2.5 to 40 mm, preferably 5 to 30 mm and particularly preferred from 10 to 20 mm.
As already described above it is particularly advantageous that the cannulas according to the invention are sterile can be offered packaged. This do that earlier necessary preparations such as acid treatment, Neutralization and sterilization of those still to be cut in the operating room Pieces of magnesium wire unnecessary and guarantees fast and reliable application. The sterilization and packaging of the cannulas can be done according to customary procedures carried out be as they are for the sterilization and packaging of cannulas are common in medicine, for example by means of autoclaving.
For easy handling, it is also provided that that, for example, the cannulas according to the invention are provided with a color code who, on the one hand, can specify which implantation device the corresponding cannula is usable and / or notes on the length of the metal wire pieces contained, possible additives, such as Fibrin glue or other properties.
The present invention further relates to a magazine in one of the implantation devices according to the invention described above can be used and metal wire pins, preferably magnesium wire pins contains or at least one of the cannulas described above. Such Magazines can according to magazines from the state of the art for designed and manufactured the so-called seed implant guns become; see for example the magazine in WO99 / 20337. Furthermore The present invention relates to those described above Cartridges in an implantation device according to the invention or in the magazine according to the invention can be used and contain pieces of metal wire, preferably pieces of magnesium wire and optionally a fibrin glue. Subject of the present Invention are also cartridges containing fibrin glue and for use with an implantation device according to the invention, cannula and / or magazine are provided.
