Description
The present invention relates to a surgical instrument for cutting, ablating or aspirating material in an operating area, with a head portion, with the material in the operating area cutting, abrading or suction acting.
Surgical instruments of the generic type are known from the prior art and are used for example in retinal surgery as an instrument for ultrafine cutting and ablation of tissue. Surgical instruments for aspirating material in a surgical field are used, for example, for removing degenerated tissue parts in brain surgery, wherein the surrounding, non-degenerated tissue should remain intact during the suction process. By material is meant in this context primarily any human or animal tissue material, this may also be bone or tooth material.

The present invention has for its object to provide a surgical instrument of the generic type and further develop the improved control and control options over the known from the prior art surgical instruments, so that in particular more precise and reproducible operations are possible.
This object is achieved with an operation instrument of the type mentioned at the beginning, in which the analysis system has a Michelson interferometer for optical coherence tomography.
The object is solved by the
The invention is based on the finding that an improved control and control option of the surgical instrument is possible if the detection means required for an assessment of the material to be processed are integrated into the surgical instrument. Then can the material to be processed is analyzed directly in a particularly advantageous manner and the result used to control the surgical instrument so that, for example, the material of the operating area or field can be selectively processed or removed with regard to its specific properties.
The invention focuses on a non-imaging optical analysis of the material, since it is not about the visual positioning of the surgical instrument, but about an assessment of the material or the change made thereto. A non-imaging optical analysis according to the present invention therefore does not mean imaging optics in the sense of endoscopes.
For this evaluation, the surgical instrument therefore has detection means which operate essentially on an optical basis. Thus, a light source is provided, with the light of the operating area is illuminated. For this purpose, this light passes through an illumination beam path that runs from the light source to the head part of the surgical instrument. The light source or the light of the light source is expediently selected so that a specific detection method can be carried out with the light, for example fluorescence detection. In principle, the light relevant here can comprise not only the visible wavelength range, but also the infrared and / or the ultraviolet wavelength range.
The light used for illumination is reflected and / or scattered at the operating area or on the material. Depending on the detection method, the illumination of the operating area can also excite or induce light, for. B. luminescence or Raman light. The reflected, scattered, excited or induced light passes through the detection beam path to a detector which generates a detection signal. How much of the reflected, scattered, excited or induced light enters the detection beam path depends on the effective numerical aperture of the part of the detection beam path facing the operating area.
The detector detects the light in the detection beam path and generates a detection signal. This detection signal depends on the detected light intensity, for example, a direct proportionality or a binary relationship can be used. Depending on this detection signal, the surgical instrument can now be controlled, it being conceivable that the control automatically intervenes in all operating and operating modes of the surgical instrument. The control according to the invention could also be performed in addition to or as an alternative to an operator-initiated manual control.
The light source and the detector are not located at the head portion of the surgical instrument to keep the surgical instrument small. It is only necessary to ensure that the illumination beam path and the detection beam path extend to the head part of the surgical instrument. The illumination and detection beam path expediently extends at least partially along a supply line for the head part.
The head portion of the surgical instrument can be advantageously made small, so that it is particularly suitable for brain surgery. In this case, the head portion of the surgical instrument will be elongated and in particular made thin, so that as little intact material as possible is damaged by the surgical instrument.
It may be necessary in an operation that the currently performed suction or cutting operation must be abruptly interrupted to preserve intact tissue material. This can be achieved with the surgical instrument according to the invention by a control unit evaluating the detection signal and automatically interrupts the cutting, ablation or suction of material based on this evaluation, off or changed in intensity. If the operator repositions the header and thus wishes to continue the operation elsewhere, reevaluation of the detection signal may indicate that material is being processed or removed at that location. In this case, the control unit can turn on the cutting, removal or suction of the surgical instrument again.
As a result, the duration of the operation can be minimized in a particularly advantageous manner, since the surgical instrument then works with increased power, if this is not critical in the current operating situation. Both the switching on, off and reducing or increasing the cutting or suction of material can be automated according to the invention. Insofar, incorrect decisions in an operation that damages the tissue are reduced in a particularly advantageous manner.
The optical detection method of the surgical device according to the invention, the optical coherence tomography, allows an evaluation or classification of the material to be processed or of the machining process. Of course, a combination with other optical and acoustic detection methods is possible, namely in particular with acousto-optic spectroscopy. A complete waiver of image acquisition on Operating area allows a very small-built surgical instrument; the size of endoscopes is significantly undercut.
Optical coherence tomography enables morphological information on the material to be processed or processed to be obtained, as well as information about its scattering and absorption properties, and is in principle realized with an optical design which essentially corresponds to a Michelson interferometer. Light of a light source has a short coherence length, and one of the two partial beam paths of the Michelson interferometer is used as a measuring arm. The other partial beam path serves as a reference arm. Due to the low coherence length of the light used, an interference signal only occurs when the partial beam path serving as the reference arm and the partial beam path serving as the measuring arm have the same effective optical length. With optical coherence tomography, one can measure distances to an object with an accuracy that is essentially limited only by the coherence length of the light used.
It is therefore envisaged that the surgical instrument according to the invention uses optical coherence tomography, in the form of a Michelson interferometer. The Michelson interferometer has a reference arm and a measuring arm, wherein the measuring arm extends from a beam splitter to the operating area and back to the beam splitter and the beam splitter is arranged in the illumination and detection beam path. The measuring arm of the Michelson interferometer is therefore at the same time a part of the illumination and detection beam path. For this purpose, the light of the light source has a coherence length suitable for optical coherence tomography. On the one hand, this can be achieved by using a light source whose coherence length is correspondingly low, for example a mercury vapor lamp. Of course, a light source can be used which has a high coherence length, z. As a laser when an optical component is introduced in the illumination beam path, that reduces the coherence length, for example, a scattered light plate.
In particular, to determine the depth of cut when cutting the material, it is necessary to detect the morphological depth structure of the material, that is substantially perpendicular to its surface. For this purpose, in the embodiment with the Michelson interferometer, scanning of the operating area along the optical axis of the illumination beam path takes place. The scanning is effected by changing the ratio of the effective lengths of the measuring arm to the reference arm with simultaneous detection of the light of the detection beam path. This mode of operation of an optical coherence tomograph is referred to as analogous to the ultrasound examination as A-scan. The change in the effective length of the measuring arm can be achieved by varying the focal length of the illumination and detection beam path at the operating area or field, for example via a corresponding, along the optical axis of the illumination beam path movably arranged lens system. Alternatively or additionally, a change in the ratio of the effective lengths of the measuring arm to the reference arm can be achieved by adjusting the length of the reference arm. Thus, no focusing optics must be provided on the head part of the surgical instrument.
In a particularly preferred embodiment, it is provided that the Michelson interferometer is used to scan the surgical field transversely to the optical axis of the illumination beam path. This operating mode of an optical coherence tomograph is also referred to as B-scan. It is also conceivable simultaneous scanning along the optical axis of the illumination beam path and a direction orthogonal thereto. Preferably, the scanning takes place transversely to the optical axis of the illumination beam path, that is, for example, in a plane orthogonal to the optical axis. A B-scan provides morphological data of the material along the scanned line or surface, so that this information can advantageously be used to control the surgical instrument in a sheet-like processing of the material. Finally, a combination of an A-scan and a B-scan is conceivable, namely scanning in three different directions, preferably orthogonal to one another.
Preferably, the scanning takes place in at least one direction transverse to the optical axis of the illumination beam path by a rotation or rotation of an optical component, wherein the optical component is located in the measuring arm. The optical component can, for. B. include a deflection prism or a deflection mirror on the head part of the surgical instrument. Alternatively or additionally, the scanning in two different directions by a rotation of a light guide about its longitudinal axis, wherein the measuring arm extends at least partially in the light guide and wherein the light guide is guided in a corresponding guide unit. The rotation of the light guide can be carried out as a continuous or as a periodic back-and-forth rotary motion. A guide unit may comprise a tube or a tube in which the optical fiber rotates (about its longitudinal axis). The inner diameter of the tube or of the tube is greater than the outer diameter of the optical fiber and to be selected so that a movement with as little friction as possible of the light guide can be done so that no possible torsion of the Light guide occurs The classification of tumor-affected material or tissue can be carried out in particular by detection of Raman or luminescence, in particular a fluorescence light detection is possible. With this detection method spectroscopic information about the material is obtained. On the one hand, autofluorescence of the material can be excited, on the other hand, assuming a corresponding label with fluorescent dye, a fluorescent dye can be excited.
The light of the light source is to be selected such that, as desired, Raman light and / or luminescence light, in particular fluorescent light, can be excited.
A marker with fluorescent substances can in a known manner, for. B. by systemic administration. But it is also possible to spray suitable fluorescent marker directly on the surgical field. For this purpose, it is preferable to provide a spraying device for contrast or fluorescent substances on the surgical instrument, in particular on the head part.
Preferably, the light of the light source can cause two or more photon excitation. Such light sources are, for example, short-pulse lasers which emit light in the infrared wavelength range. This light is focused into the material so that the light intensity present in the focus area is high enough to effect the two or more photon excitation process. The use of the multi-photon excitation is particularly advantageous because the excitation light has a wavelength that is in the infrared range and therefore - due to lower scattering processes on the material - can penetrate deeper into the material than that of excitation light for One-photon excitation with shorter wavelengths is the case.
In order to select the light of the light source for the illumination of the surgical field, it is advantageous to provide in the illumination beam path of the analysis system an illumination filter system which transmits a wavelength or a wavelength range. Then, a light source can be used for illumination that emits light over a large spectral range. By changing a filter of the illumination filter system different wavelengths or wavelength ranges are used. The same applies to laser light sources that emit light of several different wavelengths. As a result, an illumination of the surgical field with light of different wavelengths is possible, so that advantageously different tissue types can be evaluated with the same surgical instrument.
Preferably, the illumination filter system may comprise a color filter or tunable filter. The color filter may be a suitably coated interference filter or a color glass filter. A tunable filter can, for. B. in the form of an acousto-optically tunable filter can then be used particularly advantageous when a laser light source is used to illuminate the surgical field, which emits light of several different wavelengths.
Such a filter essentially consists of a birefringent crystal, which is subjected to high-frequency oscillations by a piezoelectric crystal arranged on one side of the crystal, whereby a wave propagates in the crystal. As a result, a Bragg grating is produced, at which the laser light can be diffracted or deflected from a zeroth order into a first order. With a suitable arrangement of the filter in the illumination beam path, one or more wavelengths can thereby be selected - also simultaneously - for the illumination of the surgical field, wherein the light of each selected wavelength can be varied or optimally adjusted with the filter in each case. Furthermore, acousto-optical modulators and / or acousto-optical deflectors can be used in a corresponding manner for the selection of the illumination light.
In the detection beam path, it is advantageous to provide a detection filter system which transmits light of a wavelength or a wavelength range for the detection. The detection filter system may also comprise a color filter, i. H. a suitably coated interference filter or a colored glass filter. Then, with a suitable choice of the illumination and detection filters light from luminescence, ie in particular fluorescence or phosphorescence, but also Raman light can be excited and detected. If the illumination beam path and the detection beam path partially coincide to achieve a compact structure, they can with the aid of a color beam splitter -. As a dichroic beam splitter - be separated again, as is common, for example, in conventional fluorescence microscopy.
The illumination beam path can cause a punctiform or circular illumination of the surgical field. A punctiform moistening is expediently produced by corresponding lenses which are suitably arranged in the illumination beam path. Spot illumination will be chosen primarily for optical coherence tomography.
Another detection method for characterizing the material of the surgical field is provided the acousto-optic spectroscopy. In this case, the operating area is exposed to pulsed light from the light source. The light-exposed material in the operating area absorbs at least a portion of the pulsed light, thereby increasing the temperature of that material. This in turn causes expansion of this material so that a pressure wave propagates in the surrounding material of the surgical field. This pressure wave is detected in a time-resolved manner with a pressure sensor, wherein the detection preferably takes place on the material surface of the surgical area.
An evaluation of the detection signals generated by the pressure sensor makes it possible to draw conclusions about the material properties, in particular, thereby material boundaries can be detected. The acousto-optic spectroscopy is thus in a particularly advantageous manner a helpful addition to the purely optical detection method, especially if the material of the surgical field is highly light-absorbing and a detection of deeper material areas of the surgical field with the purely optical detection method is problematic.
As a light source for the acousto-optic spectroscopy, an Nd: YAG laser can be used, which operates in Q-switched mode operated light pulses with a wavelength of 532-nm (2nd-harmonic). These light pulses have a pulse duration of 5 ns with a power of up to 100 mJ.
Als Drucksensor kann beispielsweise ein PVDF-Film verwendet werden, wie er in dem Artikel ”Depth profiling of absorbing soft materials using photoacoustic methods”, J. A. Viator, S. L. Jacques, S. A. Prahl, IEEE J. Selected Topics Q. E. 5, 989–996 (1999), beschrieben ist Dieser Drucksensor hat eine Sensorfläche von ungefähr 1 mm × 1 mm und könnte beispielsweise am Kopfteil des Operationsinstruments angeordnet sein.As a pressure sensor, for example, a PVDF film can be used, as described in the article "Depth profiling of absorbing soft materials using photoacoustic methods", JA Viator, SL Jacques, SA Prahl, IEEE J Selected
In a particularly preferred embodiment, the illumination beam path and / or the detection beam path has a light guide. As a light guide can serve a single fiber or a glass fiber bundle. If a monochromatic laser is used as the light source, the use of a single-mode optical fiber may be advantageous. In this case, a point-shaped light extraction is present on the light-outcoupling side of the single-mode optical fiber, which can be imaged with a corresponding optics on the operation field to a point-like illumination. In principle, the material properties of the optical waveguide are to be selected such that the optical waveguide for the illumination and in particular for the detection light has the lowest possible attenuation. This is particularly advantageous for the detection of Raman light, which usually has an even lower intensity than fluorescent light.
The illumination beam path and the detection beam path can extend at least in regions in at least one light guide. Thus at least two light guides are to be provided, namely one in which the illumination beam path and another in which the detection beam path runs. It is therefore preferred that the illumination beam path extends in an optical waveguide and that a plurality of light guides are provided for the detection beam path, which collect the light reflected, scattered, excited or induced light at the surgical field at the head part of the surgical instrument. In particular, in the case of detection of fluorescence or Raman light, it is advantageous on account of the low intensity of the fluorescence or Raman light if a plurality of detection channels are provided which collect the light to be detected at the surgical field and supply it to the detector. Also, several measuring points can be sensed in the operating area.
Also, the illumination beam path and the detection beam path can extend at least partially in the same light guide or in the same light guides. Then, in the illumination and detection beam path - preferably between the light source and light guide on the one hand and the detector and light guide on the other hand - to provide a beam splitter, which reflects the light of the light source in the direction of the light guide and allows the light of the detection beam path to pass in the direction of the detector. In a particularly advantageous manner, a plurality of optical fibers may be provided, wherein in each of the optical fibers in each case the illumination beam path and the detection beam path extends. In particular, when these optical fibers are arranged in the outer region of the head part, a plurality of illumination and detection partial beam paths are thereby formed, which in each case illuminate the surgical field and can collect the light reflected, scattered, excited or induced therefrom again. By this illumination or detection, an evaluation of the material at several points of the surgical field can be carried out simultaneously, in a pattern which substantially corresponds to the arrangement of the light guides on the head part of the surgical instrument.
There are several ways in which the light guide or fibers can be arranged on the head part of the surgical instrument. If only one light guide is provided, this may be inside or outside the head part. If a plurality of light guides are provided, they may also be arranged inside and / or outside the head part. In a substantially cylindrically shaped head portion of the surgical instrument, the light guides in the outer region of the head part a take a circular arrangement. Preferably, the longitudinal axis of the light guide or the light guide is arranged substantially parallel to a longitudinal axis of the head part.
For focusing the illumination beam path, advantageously a grin lens can be provided, which is arranged at the end of the light guide which faces the surgical field. A Grin lens is characterized by the use of a material with a location-dependent refractive index, a so-called gradient index - (Grin) glass. There are essentially two groups of gradient index glasses, namely radial and axial. Here, in particular radial grins are provided, they are used in the form of rod lenses and have a base refractive index along the central axis of the glass blank. There are also other changes in refractive index that can be described as a function of the radial distance from the central axis. Grin lenses are far superior in performance to achromats, best form lenses and pressed aspherical lenses. Due to their ability to focus diffraction-limited, their high damage threshold and their low surface scattering, they are particularly suitable for high-quality laser systems. They eliminate the need for additional aberration correction optical elements and can therefore reduce the size, weight, complexity and cost of optical systems. In particular, they can be used at the operating field end facing a light guide for effective focusing of the illumination light in a space-saving design.
In a preferred embodiment, a display device is provided for the analysis system, which is connected to the control unit and provides the surgeon acoustic and / or visual information from the detection signal. For example, an audible indicator may issue an alarm signal if the diagnosis of the material of the surgical field indicates that a cutting or suction operation is to be discontinued, otherwise intact tissue material would be damaged. A display device, which provides visual information to the surgeon, may be implemented as a monitor, for example. Thus, the result of the diagnosis - optionally superimposed on an image of the surgical field - can be displayed, so that the surgeon can use this display to see how he is currently processing the material.
In principle, the surgical instrument can be positioned and controlled in a computer-controlled manner, so that, for example, an operation is completely planned in advance by a surgeon and then carried out automatically. In that regard, corresponding positioning devices for the head part of the surgical instrument are required for this purpose. In a preferred embodiment, the surgical instrument is designed for manual operation by the surgeon. For this purpose, a handle part is provided on the head part, with which the surgeon holds and positions the surgical instrument. The handle part preferably comprises an operating element which is connected to the control unit. With this control element, for example, the surgical instrument can be manually switched on and off. Another arranged on the handle control then serves to control the cutting, Abtrag- or suction.
For the operation of the surgical instrument, a supply unit is usually provided, which is connected to the head part via a supply line. This supply unit provides the surgical instrument with the energy necessary for cutting, aspiration or removal of material. If the surgical instrument is designed to aspirate material or if the surgical instrument has a suction module, the supply unit preferably provides the head part with a necessary negative pressure. In this case, the head part is designed as a suction head for sucking off material.
In a very particularly preferred embodiment, a tubular suction line is provided, can be sucked through the material from the surgical field. In this case, the suction line comprises a suction line material that is transparent for light of the illumination and detection beam path, and the illumination and / or detection beam path runs in the transparent suction line material. In order to avoid light losses, preferably the suction line inner and suction line outer surfaces are mirror-formed, so that light losses at the mirrored suction line surfaces remain as low as possible. By this measure, the suction line itself can be used as a light guide in a particularly advantageous manner, so that the manufacturing cost of the suction line can be reduced and its construction is simplified. The light of the light source is preferably coupled into the suction line material at an end remote from the surgical field of the tubular suction line. In the same way, the detection light of the detection beam path can be coupled out at this end of the tubular suction line.
If the head part as a surgical electrode system, for. B. Pulse Electron Avalance Knife is formed, the supply unit supplies the necessary electrical energy for operation to ignite a plasma at the tip of the head and / or to generate a shock wave. The electrode system comprises a substantially central arranged electrode and a coaxially arranged further electrode, which are electrically coupled to a supply unit. Between the two electrodes, an insulator is provided, which is formed light-conducting and in which the illumination beam path and / or the detection beam path extends at least partially. The insulator is preferably made of glass or of plastic, wherein care should be taken that the glass or the plastic should be transparent to the light of the illumination and / or detection beam path. As a result, the head part of the surgical instrument can be produced in a simple and uncomplicated manner, so that ultimately the material and manufacturing costs can be kept low.
Finally, the supply unit may include a laser that generates light for laser ablation of the material. In that regard, for example, corneal surgery on the human eye with significantly improved control and control options by the measurement capabilities that provides the analysis system for the material to be processed or processed material can be carried out with an appropriately designed for laser ablation surgical instrument. Thus, it is possible to control how much material has been removed from the cornea by means of optical coherence tomography, which improves the precision of the operation in a particularly advantageous manner.
