For surgery in the human body, it may be necessary to remove diseased organs, tissues, tumors or the like. Since the material (20) to be removed is often very large, it is comminuted in the body cavity by HF morcellation. In the case of HF morcellation, the smoke development limits the surgeon's vision. In addition, there is a risk that flue gas gets into the respiratory tract of the surgeon. The present invention provides a medical bag (10), a medical system and a medical method for improving the visibility of the surgeon and for reducing the health risks during the operation. For this purpose, at least one first electrode (19) is arranged in a bag interior (13) for producing an electrostatic field.

In operations in the human body, especially in the abdominal or abdominal area, it is sometimes necessary that diseased organs, tissues, tumors or the like must be removed. For this purpose, for example, during laparoscopic surgery through openings in the abdominal wall on a minimally invasive way various instruments necessary for the operation are introduced into the body. This introduction of instruments is usually via so-called ports, which are connected to a trocar, which allows access to the body cavity. So that there is sufficient space or scope for the surgeon in the body cavity, the body cavity is expanded or inflated by means of a gas.
Through the mostly multiple ports then the most diverse medical or surgical instruments such as scalpels, forceps, Morzellatoren and necessary supply lines are introduced into the body cavity. For controlling or controlling the instruments, an optical unit is additionally introduced into the body cavity through a port, via which the area to be treated can be viewed indirectly by the surgeon directly or via a monitor.
For example, in tumorous diseases, it is usually necessary to remove a large amount of diseased tissue or even whole organs from the body. Since the dimensions of the material to be removed often exceed the diameter of the openings in the abdominal wall or the ports, it is customary to crush this material directly in the body cavity, in order then to dissipate it from the body in a comminuted state.
In known methods, the diseased tissue or organ, after it has been separated from the healthy tissue is comminuted by means of a Morcellators. This morcellation can be done by mechanical morcellators in a "cold" way, or by HF morcellators or bipolar morcellators in a "hot" way. Mechanical Morzellatoren are rod-shaped and have at a distal end portion rotating blades. These rotating blades are guided along or into the material to be removed, whereby it is comminuted in such a way that it can be transported away from the abdominal cavity in a simple manner. In HF morcellation, a ring electrode, which can be acted upon by alternating current, is arranged at a distal end region of a tubular shaft. By a gripper, which is arranged in the tubular shaft movable on a guide, the diseased tissue can be pulled through the shaft of the abdominal cavity, the material is meanwhile cut by the ring electrode. The cut or shredded tissue can be transported away as a round strand through the shaft to the outside.
This Morzellieren the diseased tissue or the tumor carries the great danger that by disintegrating diseased tissue or cancer cells in the abdominal cavity are scattered so that they can not be detected by thorough collection. A renewed illness of the patient is thus possible. Therefore, the Morzellieren recently takes place in a bag that was previously placed in the abdominal cavity. Subsequently, the material to be marcelled is placed in the bag and the opening of the bag is then connected to the port so that from outside the body by means of the Morcellator the material in the bag, which is still in the abdominal cavity, can be crushed , This prevents that diseased tissue or cancer cells from depositing uncontrollably in the abdominal cavity. After morcellation, the bag may be removed from the abdominal cavity along with the remnants of the minced material.
Flue gas, which is generated during HF morcellation, presents a problem in particular with regard to HF morcellation. Due to the not inconsiderable development of smoke, the view necessary for the surgeon is considerably restricted. Thus, the surgeon has in part no choice but to pause, before he "blinded" morcelliert. In addition, there is the danger that despite the sealing effect of the port flue gas or other micro particles, which may also have cancerous cells, enter the respiratory tract of the surgeon.
The object of the present invention is therefore to provide a medical bag, a medical system and a medical method for improving the visibility of the surgeon and for reducing the health risks during surgery for the patient and the surgeon.
A medical bag for achieving this object has the features of claim 1. Accordingly, the invention provides that at least one first electrode is arranged in the interior of the pouch for generating an electrostatic field by which electrically charged particles can be attracted. It is provided in particular that the first electrode is positively charged and the electrically charged particles negative. It is envisaged that the electric field generated by the first electrode will be maintained at least during RF morcellation. As a result, the charged particles are collected or fixed to the bag interior during the entire operation. The fact that the electrically charged particles are pulled away from the interior to the first electrode, the field of view for the surgeon is free.
The present invention may preferably further provide that at least one inner wall of the bag, in particular a partial area of an inner wall, has an electrically conductive layer formed as a first electrode, layer or coating. For example, it can be provided that the inner wall is acted upon or vapor-deposited with a metallic layer. By vapor deposition of the inner wall, a conductive layer is produced, which generates an electrostatic field in the entire bag interior by applying an electrical voltage. As a result, an electric field can be generated that covers the entire bag interior and thus can effectively knock down all charged particles.
In particular, it can be provided that the first electrode is connectable to a DC voltage source outside the body, preferably the first electrode can be contacted by means of an electrical line through the opening, in particular through the port, to the DC voltage source. For this purpose, the bag, in particular the first electrode associated with a cable-like conductor, which has a plug-in connection at an extendable out of the body end. Through this plug-in connection, the first electrode can be connected to the DC voltage source. It can also be provided that the bag or the first electrode is directly connectable to the port, which in turn has an electrical contact for contacting the DC voltage source. Through this contacting of the first electrode by means of connectors, the electrostatic field in the interior of the bag can be produced or controlled particularly easily.
Another particularly advantageous embodiment of the present invention may provide that at least one second electrode, preferably a plurality of second electrodes, is insertable through the opening through the port or into the interior of the bag. This second electrode has a polarity opposite to the polarity of the first electrode. In particular, it is provided according to the invention that the second electrode is negatively charged. The second electrode is connectable to the negative pole of the DC voltage source outside the body. This second electrode can also be a multi-point brush electrode.
Furthermore, it can be inventively provided that the at least one second electrode has a tip, in particular that the second electrode is formed like a needle and at least the tip consists of a material with a low work function. Due to the low work function of the tip material, free electrons are generated by the application of a DC voltage between the first and the second electrode, which discharge from the second electrode. The thus released electrons follow the field lines of the electric field between the first and the second electrode and move on their way to the first electrode through the entire bag contents. In this case, the number of electrons and their kinetic energy can be controlled almost arbitrarily by the DC voltage.
It is provided according to the invention that the free electrons settle on the tissue components produced in the HF morcellation or on the flue gas particles and thus form charged particles. Because now in particular the flue gas particles are charged, they can be deposited due to their electrical charge through the prevailing in the bag electric field in the direction of the inner wall and the first electrode. Preferably, for this purpose, the second electrode is aligned in such a way that the at least one tip points in the direction of the region in which most of the flue gas is developed during the HF morcellation, ie in the direction of the ring electrode of the HF morcellator. This effective deposition of the flue gas particles, the field of view of the optical unit is released to the ring electrode of the RF Morcellators.
In addition, it may also be provided that the second electrode is integrated in the Morzellator, in particular in the HF Morzellator. Thus, the second electrode, in particular the tip of the second electrode, can be brought directly in front of or in the vicinity of the ring electrode of the HF morcellator, so that flue gas particles can be directly exposed to free electrons.
It is preferably provided that the first electrode is formed from a plurality of individual electrodes, in particular from a plurality of electrically conductive strips, on the inner wall of the bag. In this case, the plurality of individual electrodes are interconnected electronically, so that all electrodes can be acted upon by the same voltage. By this division of the first electrode into a plurality of individual first electrodes, the bag can be made more flexible. In addition, the bag points thus a lower weight and is also cheaper to manufacture.
A medical system for the solution of the aforementioned object is described by the claim 9. Accordingly, it is contemplated that the system comprises an RF Morcellator, an optical unit and a medical bag according to claims 1-8. In addition, it may be provided that the system further medical or surgical instruments, such as forceps, scalpels or the like are assigned. This medical system allows tumors and diseased tissue to be removed from the body to be treated in a simple and safe way and under good visual conditions for the surgeon.
A medical method for solving the above-mentioned problem comprises the measures of
Preferably, the present invention further provides that a DC voltage is generated between a first and a second electrode in the bag, in particular that a DC voltage is generated in the bag between the inner wall of the bag acting as first electrode and an electron source acting as second electrode. By this voltage-built electrostatic field between the first and second electrodes, the charged particles are drawn off to the first electrode. By controlling the DC voltage, both the field strength, and the number of charges can be controlled, so that, for example, with increased flue gas development is to use a higher DC voltage, as in a low flue gas development.
A further embodiment of the present invention may provide that the constituents of the material and / or the flue gas particles are exposed to free electrons which emerge from the at least one tip of the second electrode, in particular from an electron source, by applying the DC voltage. The number of emitted electrons can be selected on the one hand by the height of the voltage and on the other hand by the choice of the material of the tip. Thus, for example, with a tip made of a material which has a low work function, many free electrons can be generated. The voltage value can vary from a few volts to a few kilovolts and can be set as required.
Furthermore, it can be provided according to the invention that a large part of the material in the bag is comminuted and removed by the HF morcellator and the components and / or flue gas particles accumulated on the inner wall of the bag are removed from the body together with the bag. By the HF Morzellator the main component of the diseased tissue or the tumor is crushed and discharged directly, so that only a few small components or flue gas particles remain in the bag. It is inventively mainly provided to remove the flue gas particles from the field of view of the surgeon. These residual components and particles remaining in the bag are withdrawn from the abdominal cavity and disposed of together with the bag after completion of Morzellierens through the trocar or through the port.
Further, the present invention may provide that the DC voltage between the first and second electrodes is controlled such that at least the flue gas particles are removed from the field of view of the optical unit.
