Description
Due to the complicated construction of the kinematics chain, the control of the known training device is relatively complex. In particular, it is difficult to map complicated movement patterns. Another disadvantage of the known training device is that the training device is only suitable for training in an upright position. A use in bedridden patients is therefore not possible. However, this is especially desirable in the rehabilitation training of neurologically damaged patients, in particular stroke patients.
An acute stroke (apoplexy) is often associated with hemiplegia (hemiparesis). This degenerates the muscles in the paralyzed extremities. The treatment of such, usually high-grade weakness of the lower extremities is difficult and often unsuccessful. Conventional physiotherapy is only possible after the acute stage of the disease, when the patient can leave the hospital bed. In addition, conventional physiotherapy is laborious and usually aims to relieve spasticity of the muscles and practice pre-sitting exercises both while sitting and standing, for example, to strengthen weight bearing on the affected leg.
Modern, scientific concepts of the rehabilitation of stroke patients on the other hand favor a repeated practice of individual movement sequences of walking with the feet and legs already in the acute phase, ie in the acute care bed. In stroke patients it has been shown that the repeated movement of the affected extremities already in the acute phase is beneficial for the recovery of the motor functions. In addition, it is common knowledge from practice that bilateral training of both the healthy and the weakened limb leads to better results. The co-movement of the non-weakened side, ie the side that is not affected by hemiplegia, positively influences the activation of the brain structures responsible for the use of the paralyzed limb in the parietal lobe.
The positive effect of training the lower extremities in the acute phase has already been recognized and implemented in training devices known from practice. For example, training devices are known which have a couch on which movement devices, such as cranks are attached, so that a training of the lower extremities is made possible while lying down. However, such devices on the one hand have a high space requirement, which leads to additional costs for the provision of equipment in the hospital or in the rehabilitation facility. Moreover, the handling of the equipment is complex, because the patient must be transferred from the acute care bed on the couch. Since stroke patients are subject to restricted mobility in the acute phase, the relocation is usually carried out by nurses, which represents a relatively high level of physical stress for the nursing staff.
The invention has for its object to provide a therapy device for training of lower limbs of a human, which is simple and compact and easy to handle. In particular, should be possible with the therapy device training while lying down. A further object of the invention is to provide a method for operating such a therapy device and a trolley with such a therapy device.
According to the invention this object is achieved with regard to the therapy device by the subject-matter of patent claim 1, with regard to the method by the subject-matter of claim 9 and with regard to the transport carriage by the subject-matter of claim 10.
The invention is based on the idea to provide a therapy device for training lower limbs of a human with a frame and independently movable movement means, each having a holding means for fixing a limb which is rotatably coupled to a carriage. The carriage is slidable along a linear guide attached to the frame. The movement devices each comprise two flexible transmission means, which are guided over deflection rollers mounted on the frame and extend at least in sections parallel to the linear guide. A first transmission means connects the carriage with a linear drive, so that the carriage is movable along the linear guide. A second transmission means couples a rotary roller of the holding means with a rotary drive, so that the holding means is rotatable.
The therapy device according to the invention is particularly simple and compact. This advantageously allows a mobile use of the therapy device. In other words, the therapy device can be used not only in a seated patient, but in particular in the acute phase in bedridden patients. Due to the compact dimensions and the simple mechanism, it is possible to use the therapy device according to the invention directly in the acute care bed or hospital bed. A special additional lounger is therefore not required. Also, the rearrangement of the patient is avoided with the therapy device according to the invention.
In contrast to known systems in which the patient must be brought to the therapy device, the therapy device according to the invention is so compact and mobile that it can be brought to the patient. Due to the comparatively simple mechanism, the maintenance effort is additionally reduced. Despite the simple construction, a multiplicity of different movement sequences can be realized with the therapy device according to the invention. In this way, a successful therapy can be carried out by frequent repetition of exercise procedures. Since the different movement sequences can be set differently for both independently movable movement devices, the therapy device also enables a transmission of learning effects from the brain side responsible for the healthy limb to the brain side responsible for the weakened limb or the respectively responsible brain area. Moreover, the therapy device allows the strengthening of the leg and back muscles, which also brings benefits in the recovery even in the treatment of advanced patients or for the training of healthy people.
Overall, the therapy device according to the invention allows a bilateral or alternating therapeutic movement of the lower limbs of a person sitting or lying, just because of the independently movable movement means without having to mobilize the patient from the wheelchair or from the bed. The course of the movement runs linearly along the predetermined direction of movement, ie along the linear guide in order to realize the flexion and extension in the hip and knee joint. Additionally or alternatively, the holding means, thus also the foot mounted on the holding means, can be rotated in order to realize the plantar and dorsiflexion.
In a preferred embodiment of the therapy device according to the invention, the frame is designed in the form of a right-angled triangle, wherein the movement devices are arranged on the hypotenuse. This particular shape of the frame makes it possible in a particularly simple manner to change between a position of the therapy device in which a patient is treated while sitting, and a position of the therapy device in which a patient is treated lying down.
Specifically, it may be provided that the frame has two legs arranged perpendicular to one another and a cross-member connecting the legs, wherein the linear guides are fastened to the cross-member. The legs may form the footprints of the therapy device, wherein one of the legs rests on a floor or generally a pad when the therapy device is used for the sedentary therapy and another of the both legs rest on the floor or the base when the therapy device is used for lying therapy.
Preferably, the legs have different leg lengths. This ensures that the cross-beam with each of the two legs each includes a different sized angle. In particular, the leg lengths are set such that between the leg, which forms the base for the seated therapy, and the cross-beam is a greater angle than between the cross-beam and the leg, which forms the base for the lying therapy. In this way, comparatively realistic movement patterns can be adjusted.
In a preferred embodiment of the therapy device according to the invention, the transmission means are designed as belts, in particular as toothed belts, or as a chain. Such transmission means allow a particularly simple and lossless power transmission from the linear drive or rotary drive to the carriage or the holding means.
In general, the linear drive and the rotary drive can be controlled independently of each other. In particular, it is provided that different speeds can be set on the linear drive and on the rotary drive, so that the holding means is rotated. Alternatively, same speeds can be set on the linear drive and on the rotary drive, so that the holding means is held level with the same.
In the therapy device according to the invention, the transmission means for the rotational movement of the holding means and for the linear movement of the carriage on the one hand parallel to each other and on the other hand parallel to the linear guide of the carriage. The first transmission means, which is connected to the carriage, causes the linear movement of the carriage along the linear guide. If the second transmission means, which engages in the rotary wheel of the holding means, at the same speed as the first transmission means is moved, takes place between the holding means and the carriage no relative movement, in particular no relative rotation. The holding means is thus kept level and moves together with the carriage linearly along the linear guide. A rotation of the holding means, ie a relative movement between the holding means and the carriage, is effected by a relative movement between the first transfer means and the second transfer means. By different speeds of the linear drive and the rotary drive so both the speed, as well as the degree of rotation of the holding means can be adjusted. In this way, complex movements can be adjusted by relatively simple control parameters, namely only by varying the speed of the linear drive and the rotary drive.
In order to ensure that the second transmission means is in constant engagement with the rotary roller, it is provided in a preferred embodiment of the invention that the rotary roller is arranged between two tension rollers which are rotatably mounted on the carriage and engaged with the second transmission means.
According to a sidelined aspect, the invention is based on the idea to provide a method for operating a previously described therapy device in which the linear drive at a first speed and the rotary drive are driven at a second speed, the speeds for holding the same holding means equal and the same Rotate the holding means are set differently.
Another secondary aspect of the invention relates to a trolley with a previously described therapy device and a lifting device which has an extendable lifting plate for receiving the therapy device. On the lifting plate, a tilting element is attached, which is connected to the frame of the therapy device such that the therapy device from a first position in which a first leg of the frame is arranged horizontally, is tiltable to a second position in which a second leg of the Frame is arranged horizontally. The second leg may be aligned perpendicular to the first leg of the frame.
The lifting device facilitates handling of the therapy device insofar as the therapeutic device can be easily lifted into the acute care bed or general hospital bed by the lifting device, for example, simply and without the physical exertion of an operator. The tilting element makes it possible in a simple manner to bring the therapy device into different positions, wherein the therapy device in the first position is particularly suitable for the therapy of sedentary patients and in the second position for the therapy of recumbent patients.
