Abstract

Oberflächenersatzprothesen sind ein anerkanntes Verfahren für den Ersatz verschlissener oder geschädigter Hüftgelenke (Coxarthrose). Die Verankerung der Hüftkopfkappe auf dem bearbeiteten Hüftkopf erfolgt in den meisten Fällen mit Zement oder aber auch zementfrei. Mit der vorliegenden Erfindung werden beide Verankerungsmöglichkeiten kombiniert, indem z. B. der apikale Teil der Hüftkopfkappe mit Zement verankert wird und der seitliche Teil der Hüftkopfkappe so ausgestaltet ist, dass ein Anwachsen des Knochens (Osteointegration) eine zementfreie Verankerung dieses Teilbereiches ermöglicht.

Description

  • Der Ersatz eines verschlissenen oder zerstörten Hüftgelenkes (Coxarthrose) durch ein Kunstgelenk (Endoprothese) ist ein weltweit anerkanntes Verfahren. Seit Anfang der 90iger Jahre werden wieder Oberflächenersatzprothesen verwendet, bei denen der bearbeitete Hüftkopf mit einer Hüflkopfkappe bedeckt wird, die exakt zu einer Kunstpfanne passt, die im Beckenknochen verankert wird. Beide Gelenkpartner sind aus Metall, man spricht von einer Metall-Metall-Gleitpaarung. Die derzeit in Deutschland am meisten implantierte Oberflächenersatzprothese ist die nach McMinn aber zahlreiche andere ähnliche Prothesen sind auf dem Markt.

  • Die vorliegende Erfindung beschäftigt sich mit dem Problem der Verankerung der Hüftkopfkappe auf dem bearbeiteten Hüftkopf.
  • Derzeit werden Hüftkopfkappen in den allermeisten Fällen mit Knochenzement auf dem bearbeiteten Hüftkopf verankert. Bisher gibt es keine standardisierte Zementiertechnik, so dass die Verankerungsqualität der Hüftkopfkappe sehr wechselhaft ist. Außerdem ist bekannt, dass durch die Geometrie des bearbeiteten Hüftkopfes und der Hüftkopfkappe praktisch keine homogene Zementpenetration in die seitlichen, in der Regel zylindrisch geformten Flächen des bearbeiteten Hüftkopfes erreicht werden kann. Die bisher analysierten Versagensfälle der Hüftkopfkappe beruhen daher in den meisten Fällen auf einer mangelnden Zementqualität.
  • Eine Alternative ist die zementfreie Verankerung der Hüftkopfkappe mit entsprechender Oberflächengestaltung der inneren Verankerungsfläche der Hüftkopfkappe, wie sie ebenfalls auf dem Markt angeboten werden. Diese Technik zeigte in klinischen Untersuchungen aber schlechte Ergebnisse, was vermutlich mit der mangelnden Durchblutung des Hüftkopfes insbesondere im apikalen Bereich durch die Erkrankung selbst (Sklerose, Cystenbildung, Nekrosen) und die Verschlechterung der Durchblutung durch operativ entstandene Gefäßunterbrechungen.
  • Mit der vorliegenden Erfindung sollen die Vorteile der Zementfixierung mit den Vorteilen der zementfreien Fixierung kombiniert werden. Die Hüftkopfkappe wird durch die in den Patentansprüchen aufgeführten Merkmale im schlecht durchbluteten apikalen Bereich mit Zement fixiert und gleichzeitig im gut durchbluteten seitlichen, meist zylindrischen Anteil des Hüftkopfes zementfrei einwachsen (Osteointegration). Die Qualität der Zementfixierung im apikalen Anteil kann durch einen speziellen Zementapplikator und Ausgestaltung der Verankerungsfläche standardisiert werden. Die Osteointegration im seitlichen Anteil kann durch spezielle Oberflächengestaltung und ggf. Beschichtung mit z. B. Hydroxylapatit optimiert werden. Durch die Zementfixierung im apikalen Bereich ist eine sofortige stabile Verankerung der Hüftkopfkappe gewährleistet. Diese primär stabile Verankerung ermöglicht ein sicheres Anwachsen des Knochens (Osteointegration) an die Innenfläche der Hüftkopfkappe im zementfreien Verankerungsbereich. Sollte es nach Jahren zu einer Zementzerrüttung kommen, wie dies nach 10 bis 15 Jahren bekannt ist durch die Zementalterung, übernimmt die knöchern stabile zementfreie Verankerungsregion die sichere Stabilisation der Hüftkopfkappe.

  • Ein Ausführungsbeispiel ist in 1 und 2 dargestellt. 1 zeigt einen schematischen Frontalschnitt durch das Hüftgelenk. Eine Hüftkopfkappe (1) ist auf einen bearbeiteten Hüftkopf (2) aufgebracht ist. Ausschließlich im apikalen Anteil der Hüftkopfkappe (1) befindet sich zwischen Hüftkopf (2) und Hüftkopfkappe (1) der Knochenzement (3). Im seitlichen Anteil der Hüftkopfkappe (1) besteht ein direkter Kontakt zwischen Hüftkopfkappe (1) und Hüftkopf (2) mit dem Bereich der zementfreien Verankerung (4). In diesem Ausführungsbeispiel findet sich zwischen der Verankerungsregion mit Knochenzement (3) und der zementfreien Verankerungsregion (4) eine Trennleiste (5), die eine Ausbreitung des Zementes verhindert. Durch diese Technik wird der Zement (3) beim endgültigen Aufpressen der Hüftkopfkappe (1) auf den Hüftkopf (2) in die offen Spongiosawaben des Knochens eingepresst (6) und sichert die stabile Verankerung.
  • 2 zeigt die Hüftkopfkappe (1) beim Befüllen mit Knochenzement (3). Ein auf die Größe der Hüftkopfkappe (1) abgestimmter Applikator (7) wird in der Innenfläche der Hüftkopfkappe aufgesetzt und eine exakt definierte Zementmenge (3) mit einer Zementspritze (8) in die Hüftkopfkappe (1) eingespritzt. Anschließend wird der Applikator (7) und die Zementspritze (8) entfernt und die mit Knochenzement (3) gefüllte Huftkopfkappe (1) auf den bearbeiteten Hüftkopf (2) aufgebracht.

Claims (14)

  1. Hüftkopfkappe für Oberflächenersatzprothesen des Hüftgelenkes, dadurch gekennzeichnet, dass ein Teilbereich der inneren Verankerungsfläche mit Knochenzement und ein anderer Teilbereich der inneren Verankerungsfläche zementfrei am bearbeiteten Hüftkopf verankert wird.
  2. Hüftkopfkappe für Oberflächenersatzprothesen nach Patentanspruch 1, dadurch gekennzeichnet, dass vorzugsweise der apikale Anteil der inneren Verankerungsfläche mit Knochenzement und der seitliche, meist zylindrische Anteil der inneren Verankerungsfläche zementfrei am bearbeiteten Hüftkopf verankert wird.
  3. Hüftkopfkappe für Oberflächenersatzprothesen nach einem der vorangegangenen Patentansprüche, dadurch gekennzeichnet, dass der apikale Anteil der inneren Verankerungsfläche eine speziell für die Zementverankerung ausgestaltete Form aufweist, wie z. B. aber nicht darauf beschränkt sphärisch, konisch, abgerundet, mit Oberflächenvergrößerungen oder ähnlich.
  4. Hüftkopfkappe für Oberflächenersatzprothesen nach einem der vorangegangenen Patentansprüche, dadurch gekennzeichnet, dass der apikale Anteil der inneren Verankerungsfläche eine speziell für die Zementverankerung ausgestaltete Oberflächenstruktur oder Beschichtung aufweist.
  5. Hüftkopfkappe für Oberflächenersatzprothesen nach einem der vorangegangenen Patentansprüche, dadurch gekennzeichnet, dass der seitliche Anteil der inneren Verankerungsfläche eine speziell für die zementfreie Verankerung ausgestaltete Form aufweist, wie z. B. aber nicht darauf beschränkt zylindrisch, konisch, mit Verankerungsnuten, mit Verankerungsleisten, Oberflächenvergrößerungen unterschiedlicher Konstruktion oder ähnlich.
  6. Hüftkopfkappe für Oberflächenersatzprothesen nach einem der vorangegangenen Patentansprüche, dadurch gekennzeichnet, dass der seitliche Anteil der inneren Verankerungsfläche eine speziell für die zementfreie Verankerung ausgestaltete Oberflächenstruktur aufweist.
  7. Hüftkopfkappe für Oberflächenersatzprothesen nach einem der vorangegangenen Patentansprüche, dadurch gekennzeichnet, dass der seitliche Anteil der inneren Verankerungsfläche eine speziell für die zementfreie Verankerung ausgestaltete Beschichtung aufweist, wie z. B. aber nicht darauf beschränkt Hydroxylapatit.
  8. Hüftkopfkappe für Oberflächenersatzprothesen nach einem der vorangegangenen Patentansprüche, dadurch gekennzeichnet, dass die Bearbeitung des Hüftkopfes mit Standardfräsen erfolgen kann.
  9. Hüftkopfkappe für Oberflächenersatzprothesen nach einem der vorangegangenen Patentansprüche, dadurch gekennzeichnet, dass die Bearbeitung des Hüftkopfes mit speziellen Fräsen erfolgt, die auf den Teilbereich der Zementfixierung und den Teilbereich der zementfreien Fixierung exakt abgestimmt sind.
  10. Hüftkopfkappe für Oberflächenersatzprothesen nach einem der vorangegangenen Patentansprüche, dadurch gekennzeichnet, dass die gewünschte Zementmenge für den Teilbereich der Zementfixierung mit einem speziell abgestimmten Applikator in die Verankerungsfläche der Hüftkopfkappe eingebracht wird.
  11. Hüftkopfkappe für Oberflächenersatzprothesen nach einem der vorangegangenen Patentansprüche, dadurch gekennzeichnet, dass die gewünschte Zementmenge für den Teilbereich der Zementfixierung mit einem speziell abgestimmten Applikator auf den bearbeiteten Hüftkopf aufgebracht wird.
  12. Hüftkopfkappe für Oberflächenersatzprothesen nach einem der vorangegangenen Patentansprüche, dadurch gekennzeichnet, dass der Zementapplikator eine Zementbenetzung des Führungsstiftes der Hüftkopfkappe sicher vermeiden kann.
  13. Hüftkopfkappe für Oberflächenersatzprothesen nach einem der vorangegangenen Patentansprüche, dadurch gekennzeichnet, dass für den Teilbereich der Zementfixierung niedrigvisköser oder hochvisköser Zement verwendet werden kann.
  14. Hüftkopfkappe für Oberflächenersatzprothesen nach einem der vorangegangenen Patentansprüche, dadurch gekennzeichnet, dass zwischen dem Teilbereich der Zementverankerung und dem Teilbereich der zementfreien Verankerung eine Trennleiste oder sonstige Struktur zur Begrenzung der Zementausbreitung an der inneren Verankerungsfläche angebracht werden kann.
