Abstract
Die Erfindung bezieht sich auf ein Implantat, bestehend aus einem kegelfoermigen Stumpf, einem mehrstufigen zylindrischen Mittelteil und einem kalottenfoermigen apikalen Teil, wobei am Uebergang zwischen Stumf und Mittelteil eine Rille vorgesehen ist. Ziel der Erfindung ist ein Implantat, das bei entsprechender Operationstechnik einen festen Primaersitz und eine dauerhafte Verankerung im Kieferknochen und gleichzeitig eine guenstige biomechanische Kraftuebertragung auf den Knochen erlaubt. Erfindungsgemaess ist deshalb die Rille sattelfoermig ausgebildet und am Uebergang zum Kegelstumpf ist ein senkrecht zur Implantatachse liegender elliptischer Rand vorgesehen und am Uebergang vom zylindrischen Mittelteil zum kalottenfoermigen apikalen Teil ist ein zylindrisches Zwischenstueck mit drei bis fuenf parallel zur Implantatachse verlaufenden Stabilisierungsstegen angeordnet, wobei der Durchmesser des Zwischenstueckes gegenueber dem Durchmesser des unteren Mittelteils um 15 bis 25% verringert ist. Bei Wechsellast — wie im Falle eines Kieferimplantates — führt diese mangelnde Verankerung zur Lockerung des Implantates und schließlich zu dessen Abstoßung. Ein weiterer Nachteil dieser Implahtattypen ist die unphysiologische Kraftübertragung vom Implantat auf den Knochen. Während der natürliche Zahn durch ein System von Collagenfasern mit dem Knochen verbunden ist, das im wesentlichen Zugkräfte auf den Knochen überträgt, ist dieser Mechanismus an einem Implantat nicht zu verwirklichen. Erfindungsgemäß wird der Übergang zwischen dem kalottenförmigen apikalen Ende und dem zylindrischen Mittelteil durch ein wesentlich dünneres Zwischenstück (15 bis 25% gegenüber dem schwächsten Stück des Mittelteils) gebildet, das drei bis fünf parallel zur Implantatachse verlaufende Stabilisierungsstege aufweist. Auf diese Weise entstehen große Lakunen, in die der tragende Knochen einwachsen kann, was zur physiologisch tolerierbaren Kraftübertragung und zur Drehstabilität des Implantates beiträgt. Das Implantat ist außerdem so gestaltet, daß das Mittelteil im wesentlichen aus zwei zylindrischen Stücken mit unterschiedlichem Durchmesser besteht. So entsteht eine Stufe senkrecht zum Implantatachse, die während der Operation sehr leicht durch Einsatz zweier verschiedener Bohrerstärken im Implantatbett hergestellt werden kann. Für diese Operationstechnik ist es günstig, wenn der zylindrische Mittelteil in einer Stufe verjüngt ist und der Kugeldurchmesser des kalottenförmigen apikalen Teils gleich dem Durchmesser des unteren Mittelteils ist. Wesentliches Element der Konstruktion ist eine sattelförmige Rille im Durchtritt durch die Schleimhaut, wobei die Rille in ihrem dem Kegelstumpf zugewandten oberen Abschiußbereich in einen elliptischen Rand übergeht, der senkrecht zur Implantatachse liegt. Diese Form entspricht den physiologischen und anatomischen Gegebenheiten und ist damit eine wichtige Voraussetzung für die Bildung eines festen Narben ring es und damit für den Gesamterfoig der-Implantation. Das Implantat hat einen kegelförmigen Kopf, der es ermöglicht, sofort nach der Implantation eine Immediatkrone aufzusetzen, wodurch dem Patienten sofort das Behandlungsergebnis sichtbar wird. Die Ausführung als offenes (primär offenes) Implantat hat außerdem den Vorteil, daß kein Zweiteingriff erforderlich wird. Das Kieferimplantet zum Einzelzahnersatz wird aus einer bioinerten Aluminiumoxidkeramik gefertigt und besteht im wesentlichen aus einem kegeiförmigen Stumpf 1, einem zylindrischen stufenförmigen Mittelteil 2 und einem kalottenförmigen apikalen Teil 3. Der Übergang vom oberen Mittelteil 2.1 zum Stumpf 1 erfolgt über die Rille 4, die wie ein Sattel gemäß Fig. 2 ausgebildet ist. Nach oben geht die Rille in einen elliptischen Rand 5 über, der senkrecht zur Implantatachse liegt.
