The invention relates to a multi-part endoprosthesis of Nuleus pulposus, which is used between adjacent vertebral bodies. The aim of the invention is to provide a possible full-quality endoprosthesis of the Nuleus pulposus, which ensures a distance attitude and / or restoration in the affected spinal column section. The invention has for its object to provide an endoprosthesis of Nuleus pulposus, which has biochemical and biomechanical tolerance, dimensionally stable at pressure pickup, which can be used without expensive processing of the adjacent vertebrae and excludes a mechanical destruction of the adjacent vertebral bodies. This problem is solved by the fact that the intervertebral disc endoprosthesis consists of symmetrical, convex end plates with intervening, at both ends concave shaped cylindrical Distanzstueck. The radii of curvature of the convex and concave surfaces are the same. The Distanzstueck is provided with an outer, stable Huelse. It is possible to provide the convex end plates with a pin and incorporate circular recesses in the concavities of the spacer to limit the maximum range of movement.
Description
Anwendungsgebiet der ErfindungField of application of the invention
The invention relates to a multipart endoprosthesis of the nucleus pulposus, which is used between the base and cover plate of two adjacent vertebral bodies.
Charakteristik der bekannten technischen LösungenCharacteristic of the known technical solutions
There are a number of devices known to replace or at least replace degenerated, damaged or destroyed discs.
There are devices such as described in US-PS 4,401,112, for example, which ensure only a pressure recording without restoring the physiological mobility. It is known to produce a disc replacement in that after the surgical removal of the nucleus pulposus of the resulting cavity is filled with silicone rubber, which polymerized in place. US-PS 4,349,921 describes a dübeiförmige disc endoprosthesis. Also known are multipart disc prostheses made of metal or metal-plastic pairings or plastic, each consisting of an upper and lower part with pushbuttonartigem, discus or spherical intermediate piece (DE-PS 3,023,353 / DE-OS 2,263,842 / CH- PS 624.575 / CH-PS 640.131). Special constructions are US-PS 4,309,777 and SU-PS 895.433.
The prostheses listed above have the disadvantage of not replacing the function of a nucleus pulposus either only partially or only inadequately. The incorporation of bearing pans in the cancellous bone of the vertebral bodies has the disadvantage that z.T. lengthy manipulation of the patient is required. In addition, it is not desirable that the relative to cancellous bone relatively hard outer layer of the vertebral bodies is destroyed or drilled. Due to the high contact loads, the uneven pressure distribution and / or the intransigence of the materials there is not only the risk of necrosis or bone resorption, but also the possibility of mechanical destruction of the vertebral bodies. Further difficulties arise from the surgical techniques to be used and from the fact that the required safety with respect to changes in position in the application of the disc endoprosthesis is not given.
In the end, a multi-part endoprosthesis of the nucleus pulposus has also been described, which consists of symmetrical end plates and an intermediate spacer with a spherical surface. The end plates have a concave central part and a cranked guide edge.
The object of the invention is to provide a possible full endoprosthesis of the nucleus pulposus, which ensures a distance-preservation and physiological mobility in the affected spinal column section. In addition, the functionality must be guaranteed over a long period of use with maximum security against changes in position.
Darlegung des Wesens der ErfindungExplanation of the essence of the invention
The invention has for its object to provide an endoprosthesis of the nucleus pulposus, which has biochemical and biomechanical tolerance, dimensionally stable in pressure recording, which can be used without expensive processing of the adjacent vertebral bodies and excludes a mechanical destruction of the adjacent vertebral bodies. This object is achieved in that the intervertebral disc prosthesis consists of symmetrical convex end plates with intermediate, concave shaped at both ends cylindrical spacer.
The vertebral body or cover plate facing side of the end plates is either flat or slightly convex curved in order to adapt as well as possible to the vertebral body. The end plates may additionally be provided with a peripheral toothing or a planar toothing and / or a bioactive coating or be cemented. It is also possible to provide the end plates with lateral broadening in order to utilize as much as possible the entire available surface of the vertebral body or cover plate for pressure transmission. The radius of curvature of the convex part corresponds exactly to the radius of curvature of the concave recesses of the spacer. It is conceivable that the end plates carry centrally located cranially or caudally pointing pin, which protrude into recesses of the spacer. This limits the maximum flexibility of the intervertebral disc endoprosthesis and prevents the spacer from sliding out. It is also possible to provide the cylindrical spacer, which preferably consists of a medical polyethylene or polyurethane, with an outer, stable sleeve. This ensures the greatest possible dimensional stability of the spacer. The sleeve, which consists of a proven material in implant technology, is kept smaller in height than the spacer to prevent this sleeve gets in contact with the end plates.
The intervertebral disc endoprosthesis can be made variable in height according to the cranio-caudal extent of the intervertebral disk space.
Die dazuoehörigen Zeichnungen zeigen:The ventro-dorsal height difference of the disc endoprosthesis may, for. As the spinal lordosis be adapted. The advantages of the solution according to the invention are that the end plates rest on the Wirbelkörpergrund- or cover plates over the entire surface and that the spacer undergoes additional stabilization with simple means, so that deformations are precluded by cold flow. The increase in the area of the vertebral body or cover plate used for power transmission substantially reduces the pressure load per unit area. This result is particularly desirable for vertebral bodies with reduced load capacity. The invention will be explained in more detail below with reference to two exemplary embodiments. r>ie Bandscheibenendoprothese besteht aus symmetrischen konvex geformten A£schlußplatten 1, die eine Verzahnung 2 ragen. The intervertebral disc prosthesis consists of symmetrical convexly shaped closing plates 1, which project a toothing 2. The cylindrical spacer 4 has two concave ends 3. The radii of curvature of the concave and onvexen surfaces are equal to ensure the widest possible power transmission. The spacer 4 is' give with a sleeve, which consists of a proven in implant technology material. The height of these sleeves is smaller than that of the testückes to exclude contact between the sleeve and end plate.
The endoprosthesis of the nucleus pulposus, as described in Example 1, constructed. In addition, however, the end plates have 1 lateral broadening 6. In addition, in the middle of the convex elevation of the end plates, a pin 7 is additionally present, which projects into a recess 8. The diameter of the recess 8 is greater than the diameter of the pin 7 to allow movement between the end plates 1 and spacer 4. A reverse arrangement of pin 7 and recess 8 is conceivable.
