Abstract
Die Erfindung betrifft einen Vakuumhebeadapter fuer einen Hornhauttrepan zur Herstellung einer Keratoplastik an einem durch eine Schaedigung (Truebung, Verletzung u. a.) der Hornhaut in seiner visuellen Funktion beeintraechtigten Auge. Das Ziel der Erfindung besteht in der Entwicklung eines Vakuumhebeadapters fuer einen Hornhauttrepan zum Durchfuehren einer sauberen und vollstaendigen Hornhauttrepanation bei der Ausfuehrung einer Keratoplastik. Durch einen Vakuumhebestempel in dem Trepan soll ein solches Schneiden durch ein leichtes Ansaugen der Hornhaut erreicht werden. Dieses Ziel ist eine optimale Grundvoraussetzung fuer die Wiederherstellung einer guten bzw. ausreichenden visuellen Funktion des Auges. Die Aufgabe der Erfindung besteht in der Herstellung eines Adapters zum sauberen Heraustrepanieren eines kreisfoermigen Hornhautteiles. Erfindungsgemaess wird diese Aufgabe dadurch geloest, dass zentral im Trepan (1) ein verschiebbarer Niedervakuumhebestempel (4) mit Saugvorrichtung und eingeklebter Kontaktlinse (5) mit Mikrobohrungen zum Vakuumraum (7) angeordnet ist. Zu Beginn des Schneidevorgangs wird der Vakuumhebestempel bis zur Schneidenkante des Trepans vorgeschoben und die Hornhaut durch ein Niedervakuum, das über Mikrobohrungen zwischen Stempel und Hornhaut gelangt, angesaugt und so vor dem sich drehenden Trepanmesser sicher fixiert. Während beim Trepanieren das Messer langsam in die Hornhaut eindringt, wird der Vakuumstempel um den entsprechenden Wert zurückgezogen. Da der Homhaut-„Deckel" beim Weichwerden des Auges unter dem Druck des rotierenden Trepans nicht in das Auge abkippen kann, wird bis zum vollständigen Durchtrennen der Hornhaut gefahrlos weitertrepaniert. Der Schnittwinkel bleibt dadurch über die gesamten 360° konstant. Durch Belüften und Vorschieben des Stempels mit dem trepanierten Homhautteil kann dieses komplikationslos dem Trepan entnommen werden. Durch Verwendung verschiedener Stempel mit unterschiedlichen Krümmungsradien der konkaven Unterseite (eingeklebte afokale Kontaktlinsen mit verschiedenen Krümmungsradien) können Hornhäute auch mit unterschiedlichem Krümmungsradius spannungsfrei trepaniert werden. Bei Nichtübereinstimmung der Krümmungsradien von Hornhaut und Stempelunterseite entstehen unterschiedliche Kantenwinkel am Transplantat und dem Randbereich der Empfängerhornhaut. Nach Figur 1 befindet sich in dem hülsenförmig ausgeführten Trepanmesser 1 der verschiebbare Vakuumhebestempel 4 mit der eingeklebten Kontaktlinse 5 und ihren Vakuummikrobohrungen zum Ansaugen der Hornhaut. Der Vakuumstempel 4 wird während des Trepanierens mit dem Zeigefinger unter gleichzeitigem Verschluß der Vakuumsteuerbohrung 14 bis zur Schneidenvorderkante des Trepans 1 gegen den Druck einer Feder 12 vorgeschoben, bis die Hornhaut 2 durch die Wirkung des Unterdruckes 6/7 am Stempel anhaftet. Beim Trepanieren durch Drehen des Kreismessers 10 zwischen zwei Fingern (bzw. durch Motorantrieb) dringt dieses in die Hornhaut 2 ein, während durch Nachlassen des Fingerdruckes auf den Stempel 4 dieser durch die Wirkung der Federkraft Θ die Hornhaut gleichmäßig gegen das Kreismesser zieht.
