The nail head comprises a first anastomotic end and a second anastomotic end which are oppositely arranged when the auxiliary sheet component and the surgical anastomat are in a closed state. The secondary sheet assembly includes: auxiliary sheets; at least one sucking disc is arranged on the surface of the first anastomosis end, the auxiliary sheet is detachably arranged on the surface of the first anastomosis end through the sucking disc, the mounting operation of the auxiliary sheet is more convenient, the sucking disc is high in adsorption stability, the auxiliary sheet is not easy to fall off in the use process, and when the anastomat is triggered, the auxiliary sheet can be bound on tissues together with anastomat nails, so that the effects of strengthening an anastomosis opening, assisting treatment and the like are achieved.
The structure of the auxiliary sheet assembly and the staple head is specifically described below with reference to the drawings. It is to be understood that the drawings and the following description are only exemplary and are not intended to limit the scope of the application.
The auxiliary sheet component can be applied to the nail heads of various anastomat such as a linear anastomat, an arc-shaped anastomat, a circular tube-shaped anastomat and the like. Fig. 1 to 10 are schematic structural views of a staple head and auxiliary sheet assembly according to an embodiment of the present application. In this embodiment, the stapler is described as a linear stapler. As shown in fig. 1-8, the head includes a first mating end and a second mating end disposed opposite one another. In this embodiment, the first anastomotic end is a cartridge assembly, the second anastomotic end is an anvil 3, and the proximal end of the staple head is connected to the stapler body by a connecting rod 9. The cartridge assembly comprises a cartridge 2 and a cartridge holder 1 accommodating the cartridge 2. The side of the nail bin 2 facing the nail anvil 3 is a nail bin surface. The nail bin surface is provided with a cutter groove 22 extending along the axial direction of the nail head, and two sides of the cutter groove 22 are respectively provided with a plurality of nail holes 21. The staple holes 21 are configured to receive staples (not shown), and the head further includes a firing assembly including a staple pusher 61 and a firing member 8. In the initial state, the firing member 8 is located at the proximal end of the staple cartridge 2, the staple pusher 61 is located at the bottom of the staple, and when the stapler is fired, the firing member 8 is driven by a knife pushing rod (not shown in the figure) to move in the distal direction, and the firing member 8 pushes the staple pusher 61 upward (from the first stapling end toward the second stapling end) to push the staple toward the anvil 3, the staple is stapled on the tissue and is formed on the surface of the anvil 3 in a closed manner, and meanwhile, a cutter provided at the distal end of the knife pushing rod cuts the tissue to complete the stapling operation.
In the present application, the distal end and the proximal end are the proximal end with respect to the operator, the end closer to the operator, the end farther from the operator, i.e., the end closer to the operation site, and the axial direction along the axial center of the nail head, i.e., the direction from the distal end to the proximal end of the nail head, or the direction from the proximal end side to the distal end side of the nail head. For example, in the views of fig. 1 and 2, the distal side is the left side and the proximal side is the right side for cartridge 2. The S direction is the direction from the distal end to the proximal end of the staple head, defining the S direction or the direction opposite to the S direction as the axial direction of the staple head, which coincides with the axial direction of the stapler body when the staple head is not deflected relative to the stapler body.
As shown in fig. 2 to 4, the nail head further comprises an auxiliary sheet 4 and a plurality of suckers 5. The auxiliary sheet 4 is located between the nail bin 2 and the nail anvil 3, the suction cup 5 is arranged between the auxiliary sheet 4 and the nail bin surface of the nail bin 2, the specific structure of the suction cup 5 can be seen in fig. 8 and 10, the suction cup 5 comprises an adsorption end 51 and a fixed end 52, the adsorption end 51 is a bowl-shaped structure, and an air suction cavity 511 capable of exhausting air is formed inside the suction cup. When the auxiliary sheet 4 is fixed to the nail bin 2, the fixed end 52 of the suction cup 5 is fixed to the nail bin 2, then the auxiliary sheet 4 is placed between the nail bin 2 and the nail anvil 3, the lower surface of the auxiliary sheet 4 is attached to the adsorption end 51 of the suction cup 5, and the air in the air suction cavity 511 is discharged, so that the air suction cavity 511 is in a vacuum state, and the adsorption end 51 can be stably adsorbed on the surface of the auxiliary sheet 4 by virtue of external air pressure. Therefore, the mounting operation of the auxiliary sheet 4 is more convenient, and the sucking disc 5 has high sucking stability, so that the auxiliary sheet 4 is not easy to fall off in the use process. When the anastomat is triggered, under the action of the triggering piece 8 and the nail pushing piece 61, the auxiliary piece 4 can be stapled on the tissue together with the anastomat when the anastomat is pushed out towards the nail anvil 3, thereby playing roles of strengthening the anastomat, assisting treatment and the like. In this embodiment, the auxiliary sheet 4 may be an auxiliary sheet 4 coated with an anti-inflammatory drug, or an auxiliary sheet 4 for supporting and reinforcing an anastomotic stoma, or a biological patch, etc.
In this embodiment, the suction cup 5 may be provided with a suction hole connected to the suction chamber 511, and the auxiliary sheet assembly further includes a suction assembly configured to suck the gas in the suction chamber 511 through the suction hole, for example, a suction pump and a suction tube, and the suction tube is detachable from the suction hole. In another embodiment, the air in the air pumping chamber 511 is discharged by pressing the auxiliary sheet 4 onto the cartridge 2 manually without providing an air pumping assembly.
As shown in fig. 3 and 4, a plurality of suction cups 5 are symmetrically disposed on both sides of the cutter slot 22. On each side of the cutter slot 22, the suction cups 5 may be arranged in the axial direction of the head. The present application is not limited thereto, and in other embodiments, the number and arrangement of the suction cups 5 may be set as required. The auxiliary sheet 4 has an adsorption area 41 corresponding to each suction cup 5, and the division of the adsorption areas 41 in the drawing is only an example, and in practical application, the auxiliary sheet 4 may not divide the adsorption areas 41, or the identification of the adsorption areas 41 may be performed on the surface of the auxiliary sheet 4, so that the alignment between the auxiliary sheet 4 and the suction cups 5 is convenient when the auxiliary sheet 4 is mounted.
As shown in fig. 3, in this embodiment, the auxiliary sheet 4 is provided with a plurality of cutting holes 42 at positions corresponding to the cutter grooves 22, the plurality of cutting holes 42 being aligned in the axial direction of the head portion. The distal and proximal ends of the auxiliary sheet 4 are each provided with an opening 43 at a position corresponding to the cutter groove 22, and the opening 43 may be a triangular opening, but the present application is not limited thereto. By providing the cutting hole 42 and the opening 43, the auxiliary sheet 4 is more easily cut off when the cutter cuts the tissue, so that the cutter cuts the tissue and the auxiliary sheet 4 more smoothly.
As shown in fig. 4, the surface of the nail bin 2 is provided with a matching groove 23, and the fixed end 52 of the suction disc 5 is at least partially arranged in the matching groove 23. The outer diameter of the fixed end 52 of the suction cup 5 is smaller than the outer diameter of the suction end 51. The sucking disc 5 can be clamped in the matching groove 23, so that the stability of fixing the sucking disc 5 and the nail bin 2 is maintained. The suction cup 5 may be made of plastic, silica gel or other flexible material, and when the suction cup 5 is installed in the matching groove 23, the suction cup 5 is slightly deformed into the matching groove 23, and the suction cup 5 and the matching groove 23 form interference fit. In order to ensure that the auxiliary sheet 4 can be separated from the suction disc 5 in time when the anastomat is triggered, a driving piece is further arranged in the nail bin 2 and at least partially enters the matching groove 23. The top end of the driving piece is in a spike shape and is used for puncturing the sucking disc 5 when the anastomat is triggered, so that the air suction cavity 511 is communicated with the outside, and after air enters the air suction cavity 511, the sucking disc 5 does not adsorb the auxiliary sheet 4 any more.
As shown in fig. 7 to 9, the driving member includes a first driving member 62, and the tip of the first driving member 62 is spike-shaped. The first connecting part 63 is disposed on one side of the ejector pin 61, and the first driving part 62 is disposed on one side of the first connecting part 63 facing the anvil 3. The first driving member 62 is located below the mating groove 23 and may at least partially enter the interior of the mating groove 23, so that when the first driving member 62 moves upward, the first driving member 62 can contact the suction cup 5 and further pierce the suction cup 5. During the firing process of the anastomat, the firing member 8 is driven to move from the proximal end to the distal end of the staple cartridge 2, the firing member 8 pushes the staple pushing member 61 out towards the anvil 3, the first driving member 62 is driven to move towards the anvil 3 through the first connecting part 63 while the staple pushing member 61 pushes the staples out, the first driving member 62 pierces the suction disc 5, so that the suction cavity 511 is communicated with the outside, after air enters the suction cavity 511, the suction disc 5 does not adsorb the auxiliary sheet 4 any more, and the auxiliary sheet 4 can be more easily separated from the staple cartridge 2 and stapled together with the staples.
As shown in fig. 7, in this embodiment, both sides of the first connection portion 63 are respectively connected to the adjacent two of the push pins 61 so that stability and balance of the first connection portion 63 when the push pins 61 are pushed out are maintained when they move outward, but the present application is not limited thereto, and in another embodiment, only one side of the first connection portion may be connected to the push pins 61 as long as the push pins 61 move upward to move the first connection portion. By adopting the structure, the auxiliary sheet 4 can be separated from the staple cartridge 2 in time when the anastomat is triggered, the driving cooperation of the existing staple pushing piece 61 and the triggering piece 8 is effectively utilized, excessive parts are not required to be added, and the process of puncturing the sucking disc 5 and the process of closing the anastomat can be synchronized. In addition, the embodiment can be designed to ensure that the suction force of the suction disc 5 is larger, so that the stability of the installation of the auxiliary sheet 4 before the percussion of the anastomat is ensured, and the auxiliary sheet 4 cannot be separated from the nail bin 2 in time due to the excessively strong suction force.
In order to enhance the fixation of the distal portion of the auxiliary sheet 4, suction cups 5 may be provided in the distal end of the cartridge 2 in areas where there are no nail holes 21. As shown in fig. 7 and 10, the distal end of the staple cartridge 2 is further provided with a mounting groove and a mounting member 71, the mounting member 71 includes two side walls 711 and a second connecting portion 712 provided between the two side walls 711, the two side walls 711 of the mounting member 71 are at least partially provided in the mounting groove, and the mounting member 71 is movable toward the anvil 3 under the guidance of the mounting groove. The driving member is disposed on a side of the second connecting portion 712 facing the anvil 3. The driving member further comprises a second driving member 72, and the second driving member 72 is disposed below the suction cup 5 at the distal end and can at least partially enter the corresponding matching groove 23. When the firing member 8 is driven to move in the distal direction from the proximal end of the cartridge 2, the firing member 8 pushes the mounting member 71 out toward the anvil 3, and the mounting member 71 drives the second driving member 72 toward the anvil 3 to pierce the distal suction cup 5 so that the distal suction cup 5 is disengaged from the auxiliary sheet 4.
The present application is not limited to the structure employing the driving member shown in this embodiment. In other embodiments, the first driver 62 may be positioned in a similar configuration as the mounting member 71, rather than on the staple pusher 61. Or in other embodiments, a driving member is not required, and the suction cavity 511 is designed to have a smaller adsorption force acting on the auxiliary sheet 4 by the suction disc 5, so that the auxiliary sheet 4 can be driven to be separated from the suction disc 5 under the action of the anastomat when the anastomat is pushed out. Or other ways of driving the suction cup 5 to be separated from the auxiliary sheet 4 can be adopted, which are all within the protection scope of the application.
Other embodiments may be used between the fixed end 52 of the suction cup 5 and the cartridge 2. For example, the bottom surface of the fixed end 52 of the suction cup 5 is a flat surface, and the flat surface is adhered to the surface of the cartridge. The above embodiment is described by taking the case that the fixed end of the suction cup is arranged in the nail bin and the suction end is adsorbed on the auxiliary sheet. In another alternative embodiment, the fixed end of the sucker is fixed on the auxiliary sheet, and the adsorption end of the sucker is adsorbed on the surface of the nail bin. For example, the auxiliary sheet is provided with a matching hole, the fixed end of the sucker is at least partially arranged in the matching hole, and the adsorption end of the sucker is adsorbed to the nail bin. The auxiliary sheet has certain flexibility. When the auxiliary sheet is fixed on the surface of the nail bin, the size of the matching hole is smaller than that of the fixed end of the sucker, so that the sucker is clamped in the matching hole. When the anastomat is triggered, the auxiliary sheet is stressed by the force far away from the nail bin when the anastomat is triggered, and the sucker is firmly adsorbed on the surface of the nail bin, so that the acting force for mutual pulling is arranged between the matching hole of the auxiliary sheet and the fixed end of the sucker, the matching hole is slightly expanded and deformed under the acting force of the sucker, the fixed end of the sucker can be separated from the matching hole, and the adsorption end of the sucker is adsorbed on the nail bin but separated from the auxiliary sheet, so that the separation of the auxiliary sheet and the nail bin is realized. Such means are also within the scope of the present application.
The above embodiments are described using the first anastomotic end as the cartridge component and the second anastomotic end as the anvil. In another alternative embodiment, the first anastomotic end is a nail anvil, the second anastomotic end is a nail bin assembly, the sucker is arranged between the auxiliary sheet and the nail anvil, the sucker is fixed on the surface of the nail anvil or on the auxiliary sheet, and the fixing mode of the sucker and the nail anvil can be a mode of arranging a matching groove on the surface of the nail anvil or pasting the fixed end of the sucker on the nail bin surface, and the fixing mode of the sucker and the auxiliary sheet can be a mode of arranging the sucker in a mounting hole in the auxiliary sheet in a penetrating mode, and the like.
The above embodiments are described taking a surgical stapler as an example of a linear stapler. In another alternative embodiment, the auxiliary sheet assembly may be applied to a nail head of a circular tube type anastomat, the nail head includes a first anastomotic end and a second anastomotic end which are oppositely disposed, the first anastomotic end includes a nail cartridge located at a distal end of the nail cartridge, the second anastomotic end includes a nail anvil and a nail anvil shaft, the first anastomotic end and the second anastomotic end are connected through the nail anvil shaft, the suction cup may be disposed at a distal end surface of the nail cartridge, and the auxiliary sheet may be detachably disposed at a distal end surface of the nail cartridge through the suction cup. Correspondingly, a driving piece is optionally arranged in the nail bin, when the firing piece is driven to move from the proximal end to the distal end, the driving piece is pushed out to the distal end direction by the nail pushing piece to expose the distal end surface of the nail bin, and the tip of the driving piece punctures the sucking disc, so that the auxiliary piece can be separated from the nail bin. Or the first anastomotic end comprises a nail anvil and a nail anvil shaft, the second anastomotic end comprises a nail cartridge positioned at the far end of the nail cartridge, the sucking disc is arranged on the proximal end face of the nail anvil, and the driving piece is optionally arranged inside the nail anvil. When the surgical anastomat is a circular tube type anastomat, the suckers can be uniformly distributed along the circumference of the anastomat. In yet another alternative embodiment, the auxiliary sheet assembly may be applied to a head of a curved stapler, the head comprising a first jaw and a second jaw disposed opposite to each other, the first jaw comprising a cartridge holder and a cartridge, the second jaw comprising an anvil holder and an anvil, the first jaw being provided with the suction cup as the first stapling end or the second jaw being provided with the suction cup as the first stapling end. When the surgical stapler is an arc-shaped stapler, the suckers can be uniformly distributed along the arc extending direction of the first clamp or the second clamp.
The foregoing is a further detailed description of the application in connection with the preferred embodiments, and it is not intended that the application be limited to the specific embodiments described. It will be apparent to those skilled in the art that several simple deductions or substitutions may be made without departing from the spirit of the application, and these should be considered to be within the scope of the application.
