本实用新型公开了一种下颌骨截骨神经保护导板,导板整体贴附于下颌骨骨面,导板的边缘设有具有弧形的引导区,所述引导区的弧形及长度与下牙槽神经管的弧形及长度相匹配。本实用新型可以使截骨手术更容易进行,便于术者操作;经导板指导下截骨可沿边界精确切除下颌骨病变,避免残留,有效降低术后复发的风险;经导板指导下截骨可有效保存下牙槽神经血管束,避免由于切除或误伤而导致的术后患侧颏唇部皮肤麻木、患侧下颌余留牙、牙槽骨等失神经化、缺血、失活甚至坏死等严重并发症,提高患者术后生存质量。

技术领域
本实用新型涉及一种医疗辅助装置,尤其涉及的是一种下颌骨截骨神经保护导板。
背景技术
在下颌骨良性病变的治疗中,传统手术在切除下颌骨病灶的同时往往忽略下牙槽神经血管束的保存,现有的截骨技术是整体截除受累下颌骨骨段,同时截断并结扎下牙槽神经血管束。这种方法势必导致神经损伤,并存在术中大出血的危险,严重者可能威胁患者生命;其次,术中截骨范围的确定往往依靠简单的影像学资料以及手术医师的经验,若截骨不充分,则可能导致病灶残留,增加术后复发的风险;再次,截断下牙槽神经血管束后,术后患者将出现患侧永久性颏唇部皮肤麻木,同时患侧下颌余留牙、牙槽骨等可能出现失神经化、缺血、失活甚至坏死等严重并发症,严重影响患者术后生存质量。
实用新型内容
本实用新型的目的在于克服现有技术的不足,提供了一种下颌骨截骨神经保护导板,在术中能够方便使用,实现精准定位。
本实用新型是通过以下技术方案实现的,本实用新型所述导板整体贴附于下颌骨骨面,导板的边缘设有具有弧形的引导区,所述引导区的弧形及长度与下牙槽神经管的弧形及长度相匹配。
作为本实用新型的优选方式之一,所述导板的厚度为1.5~3mm。
所述引导区为刃状结构。用于由口内入路进行截骨。
所述引导区为肩台结构。用于由口外入路进行截骨。
所述引导区的宽度为2~3mm。导板边缘即为下牙槽神经管骨表面投影位置,截骨后便可准确暴露下牙槽神经血管束,便于术中剥离并保护此重要结构,以避免术后患者出现前文所述的多种并发症。
本实用新型相比现有技术具有以下优点:本实用新型可以使截骨手术更容易进行,便于术者操作;经导板指导下截骨可沿边界精确切除下颌骨病变,避免残留,有效降低术后复发的风险;经导板指导下截骨可有效保存下牙槽神经血管束,避免由于切除或误伤而导致的术后患侧颏唇部皮肤麻木、患侧下颌余留牙、牙槽骨等失神经化、缺血、失活甚至坏死等严重并发症,提高患者术后生存质量。

具体实施方式
下面对本实用新型的实施例作详细说明,本实施例在以本实用新型技术方案为前提下进行实施,给出了详细的实施方式和具体的操作过程,但本实用新型的保护范围不限于下述的实施例。
如图1为患病下颌骨示意图,下牙槽神经管1的下面虚线L1为口外入路时,紧贴神经管骨表面投影下缘设计的截骨线;下牙槽神经管1的上面虚线L2为口内入路时,紧贴神经管骨表面投影上缘设计的截骨线。
实施例1
如图1和图2所示,图2为口外入路截骨导板2设计示意图,本实施例的导板2整体贴附于下颌骨骨面,导板2的边缘设有三个引导区21,两端的引导区21用于指示病变及截骨范围,中间的引导区21的弧形及长度与下牙槽神经管1的弧形及长度相匹配。
本实施例导板2的厚度为2mm。本实施例的引导区21为肩台结构,宽度为2.5mm。引导区21指示截骨线的位置,并可引导口外入路术中截骨及保留神经的操作。
本实施例可用于各种不累及下牙槽神经血管束的下颌骨良性病变的治疗中,如骨纤维结构异常、骨化性纤维瘤、下颌骨骨髓炎等。术前基于患者CT数据,运用三维重建技术,精确定位病变区域及下牙槽神经管位置;利用CAD/CAM技术设计截骨及神经保存导板,并通过三维打印技术制作适合每位患者的个体化截骨导板。导板2整体为透明或乳白色,术中借助导板对术前确定的病变累及范围以及下牙槽神经管加以定位并指导截骨操作,以利于完整切除病变,并解剖保存下牙槽神经血管束。
实施例2
如图1和图3所示,图3为口内入路截骨导板2设计示意图。导板2的边缘有两个引导区21,一条引导区21用于指示病变及截骨范围,另一条引导区21的弧形及长度与下牙槽神经管1的弧形及长度相匹配。导板2的厚度约2mm,与下颌骨外侧紧密贴合;引导区21为刃状结构,指示截骨线位置,并可引导口内入路术中截骨及保留神经的操作。其他实施方式和实施例1相同。

1.一种下颌骨截骨神经保护导板,其特征在于,所述导板整体贴附于下颌骨骨面,导板的边缘设有具有弧形的引导区,所述引导区的弧形及长度与下牙槽神经管的弧形及长度相匹配,所述引导区为肩台结构。 
2.根据权利要求1所述的一种下颌骨截骨神经保护导板,其特征在于,所述导板的厚度为1.5~3mm。 
3.根据权利要求1所述的一种下颌骨截骨神经保护导板,其特征在于,所述引导区的宽度为2~3mm。 
4.一种下颌骨截骨神经保护导板,其特征在于,所述导板整体贴附于下颌骨骨面,导板的边缘设有具有弧形的引导区,所述引导区的弧形及长度与下牙槽神经管的弧形及长度相匹配,所述引导区为刃状结构。
