Abstract
本发明涉及一种人血高密度脂蛋白及其产品,以及其制造方法和应用。所述的人血高密度脂蛋白中,apo AI含量为77-89%。本发明以人血浆为原料,通过分离纯化步骤,从中提取HDL,提供了大规模制备人血浆HDL的新工艺方法,是对宝贵人血资源的充分利用。本发明涉及的人血高密度脂蛋白及其产品在制备治疗多器官功能障碍综合症(MODS)、心血管系统疾病,如动脉粥样硬化(AS)以及中和细菌毒素等急性症状药物方面有着很好的应用。
Description
技术领域
本发明涉及一种人血高密度脂蛋白、其制造方法及其在药剂学上的应用,属于生物化学领域。
背景技术
AS是因动脉壁脂质沉积,纤维及结缔组织增生,而造成动脉管壁增厚变硬,弹性减退并形成糜粥样病灶的常见多发性疾病。AS引起的动脉管腔闭塞,可使心、脑、肾等重要器官供血不足,从而严重威胁人类健康和生命。目前临床上常用的治疗AS药物,一般有扩张血管药物、降血脂药物、抗血小板药物及溶栓药物等。这些药物虽然可通过解除血管运动障碍及调节血脂等作用对抗AS,但均不能安全、有效的预防和消除AS斑块。并且由于AS是一种慢性非炎症性、退行性和增生性的病变,长期使用某些药物治疗,会引起胆石症、肝功能损伤等副作用。
大量流行病学研究表明,血脂升高、肥胖、高血压、糖尿病以及吸烟等是AS主要危险因素。但是,目前对AS的防治并无理想的方法和药物。现阶段防治AS除饮食控制外,在药物治疗上一般多选用降低血脂、扩张血管或针对易患因子的药物。因此发现和制备能够预防,特别是可以促使AS逆转或发生消退的新药就十分必要。
七十年代以来大量流行病学调查已经发现,血浆高密度脂蛋白(HDL)水平与动脉粥样硬化(AS)发病率呈负相关。Framingham调查表明,血浆高密度脂蛋白-胆固醇(HDL-C,可以代表血浆HDL含量的指标)高的家庭或地区冠心病(CHD)发生率明显降低。Rifkind等在调查的人群中发现,血浆HDL-C含量每增加1mg/dL,CHD的危险性就减少2-3%。Helsinki Heart Study(赫尔辛基心脏研究)也证实,如用药物升高血浆HDL-C水平,男性CHD发生率可以下降34%。动物实验还证明,血浆HDL水平较低的动物如家免、猴、猪等均易诱发AS,相反血浆HDL含量较高的动物如貂、北京鸭和树鼩等则不易甚至不产生AS。另外,细胞培养证明,HDL可促进细胞(包括动脉平滑肌细胞)内胆固醇的移出,动物实验也证明HDL有抑制AS斑块形成及促进斑块消退的作用。这些都说明HDL具有对抗AS的作用。而且由于HDL是来源于人血浆的天然药物,在预防和治疗AS的长期过程中具有其它人工合成药物无法比拟的优势。
HDL主要由载脂蛋白和脂质组成,是血浆中密度最高但组成极不均一的一类脂蛋白。HDL是由蛋白质和脂质组成的脂蛋白,它含蛋白质50%~55%,主要由载脂蛋白(apo)AI和AII构成,另含少量的apoAIV、CI、CII、CIII、D、E以及J,HDL含脂质约45%~50%,主要由磷脂(PL,40~60%)、胆固醇及胆固醇酯(总胆固醇TC,30%~40%)及少量甘油三酯(TG,10%左右)构成。在HDL中apoAI的含量最多而且又是HDL主要的功能蛋白质,因此可以用apoAI的含量反映HDL的含量。
目前尚无由血浆直接制备HDL的方法。
发明内容
本发明的目的在于提供一种人血高密度脂蛋白及其产品。
本发明的另一目的是提供人血高密度脂蛋白及其产品的制造方法,该方法直接以血浆为原料,制备HDL。
本发明的再一目的是提供上述人血高密度脂蛋白及其产品在制备治疗多器官功能障碍综合症(MODS)、心血管系统疾病,如AS以及中和细菌毒素等急性症状药物方面的应用。
本发明所述的人血高密度脂蛋白中,apoAI含量为77-89%。
上述的人血高密度脂蛋白中apoAI分子量为27.7KD。
本发明所述的人血高密度脂蛋白产品中可含有0.1-99%的人血高密度脂蛋白。
上述的人血高密度脂蛋白产品可以加入9~11%蔗糖和/或0.5%白蛋白和/或150mg±1mg/L维生素C,制成供静脉输注的人血HDL制剂。
上述的HDL不限于本发明所述的HDL,现有技术中的HDL同样适用前述的组合互配。
本发明所述方法包括以下步骤:
A.取原料血浆,滴加缓冲溶液至混合液的pH为5.0-5.5,加入混合液1/4~1/3体积的95%乙醇,在-5~-2℃下搅拌1-3小时,静置6-8小时,离心得上清;向上清补加45-55%上清体积的注射用水,滴加缓冲溶液,使混合液pH为4.5-6.0,在-6~-3℃下搅拌1-3小时,静置1-3小时,离心得沉淀,弃上清;
B.沉淀用2-3倍重量的0.02~0.1M NaCl溶液溶解,调pH至5.2±0.5后,在0~2℃下搅拌2-6小时,然后离心得沉淀;沉淀用50-150倍重量的0.1~0.2M NaCl溶液溶解,调节溶液pH为7.2±0.5,在2-8℃下搅拌6-20小时,过滤得滤液;
C.上述滤液加入缓冲液,使pH为5.2±0.5,搅拌1-3小时,在2~4℃下,静置6-20小时,离心收集沉淀;沉淀用50~150倍量的0.1~0.2MNaCl溶液溶解,调pH至7.2±0.5,搅拌使沉淀完全溶解,过滤,得滤液;
D.滤液用超滤膜超滤,脱盐并浓缩,得富含HDL的溶液,经后处理,得HDL及其产品。
上述方法中调节pH使用1mol/LNaHCO3溶液。
上述方法中所述的缓冲液为pH=4的HAC/NaAC缓冲体系。步骤C结束后可根据需要重复一遍,以使达到更好的纯度。
步骤D中的超滤膜为8-70KD超滤膜。优选使用30~50KD超滤膜,其具有进一步纯化的作用。
步骤D所述的后处理可以是:超滤浓缩液加9~11%注射用蔗糖或0.5%白蛋白,按每升加入150mg±1mg注射用维生素C,调pH至7.2±0.5,得HDL溶液;上述HDL溶液于微机自控的巴氏灭菌器内60±1℃、10小时灭活病毒;灭活病毒后的HDL溶液,除菌分装,并于2~8℃保存。
本发明方法以低温乙醇法制备人血白蛋白及丙种球蛋白弃去的FIV-1沉淀为原料,在pH5.1±0.5、0~2℃以及0.03M NaCl的条件下,通过洗涤充分除去HDL以外的其它血浆蛋白。HDL粗品溶解后再于pH5.1±0.5(等电点)、2~4℃、0.15M NaCl的条件下重沉淀精制。最后纯化的HDL溶液经超滤、透析、浓缩、配制、60±1℃,10小时灭活病毒及除菌制成pH6.4~7.4、载脂蛋白AI含量≥0.5%、纯度≥95%,含10%±1%蔗糖的人血HDL制品。在HDL制备过程中,在分离各步骤调节适当酒精浓度在6~30%、蛋白浓度在1~6%、pH值在4.5~7.5、离子强度在0.02~2.0kΩ·cm、温度在-6~10℃。
HDL是由蛋白质和脂质组成的脂蛋白,它含蛋白质50%~55%,主要由载脂蛋白(apo)AI(65%~70%)和AII(20%~25%)构成,另含少量的apoAIV、CI、CII、CIII、D、E以及J。HDL含脂质约45%~50%,主要由磷脂(PL,40~60%)、胆固醇及胆固醇酯(总胆固醇TC,30%~40%)及少量甘油三酯(TG,10%左右)构成。在HDL中apoAI的含量最多而且又是HDL主要的功能蛋白质。因此可以用apoAI的含量反映HDL的含量。发明人采用华西医科大学载脂蛋白研究室研制的载脂蛋白AI检测试剂盒(单向免疫扩散法),检测apoAI含量并以制品中apoAI含量代表HDL含量,以apoAI含量≥0.5%为合格。
研究发现,肝细胞、动脉平滑肌细胞、内皮细胞等的细胞膜上存在可特异结合HDL的受体。发明人采用预包被在酶标板上的肝细胞膜HDL受体与HDL发生特异结合反应,再与辣根过氧化物酶-抗体交联物HRP-抗apoAI IgG发生免疫学结合反应,最后通过吸附在酶标板固相上的受体-配体(HDL)-抗配体抗体-酶复合物中酶催化的显色反应检测制品中HDL的生物活性。生物活性测定时,以1μg apo AI作为1个HDL活性单位(U)。由于HDL制品apoAI含量≥0.5%,即相当于含有5×105U HDL(理论值),因此发明人以该值的80%作为判断制品合格的标准,即HDL制品生物活性应≥4×105U。
本发明HDL产品的动物急性毒性试验:试验结果表明HDL用静脉注射及腹腔注射两种途径给药,以提取中最高浓度(1%)作为最大给药浓度,以静脉注射及腹腔注射给药最大体积0.8ml/20g(体重)给药,12h内连续给药3次,给药连续观察7天,动物均未发现异常,7天后动物无一死亡,故HDL静脉注射及腹腔注射给药小鼠最大耐受量为0.024g/20g(体重)。
本发明HDL产品的稳定性试验:根据中华人民共和国卫生部药政局1993年7月编制的“新药(西药)临床前研究指导原则汇编(药学药理学毒理学)”对人血HDL制品进行了影响因素试验、室温(25℃)5个月留样考察和2~8℃14个月留样考察。结果显示,HDL考察样品能够耐受流通领域和使用过程中条件变化的影响,制品的贮存条件应严格控制在2~8℃和避光下保藏,这样即可保证在1年内,HDL液体制剂在流通和使用过程中保持《中国生物制品规程》要求的各项质量标准稳定不变。
本发明HDL产品与治疗作用有关的主要药效学试验:为了验证本发明人血浆HDL制品的抗AS作用,发明人以高脂饲养造成家兔As模型,在高脂饲养的同时(实验性As预防试验)以及在高脂饲养10周造成AS模型后(实验性AS治疗试验),分低(L)、中(M)、高(H)三个剂量组(10、50、100mg以apoAI含量计)静脉注射人血浆HDL制剂(每周一次,共计10周),从受试动物血中脂质和脂质过氧化物水平,动脉壁、肝脏脂质含量,胆囊胆汁脂质含量以及动脉壁AS斑块面积等方面考查和验证HDL制剂的抗AS作用。
由于没有与HDL作用机制完全相同或相似的抗AS药物,因此发明人选用调血脂药菲诺贝特为阳性对照药。在阴性对照的选择上,由于研究已经证实免疫损伤,特别是高脂饲养的模型动物反复输注异种蛋白会因发生免疫反应而加速AS的发生和发展,而人血浆HDL药效学的动物试验不可避免地将会因注射人HDL异种蛋白引起动物的过敏反应及免疫反应,反而加重高脂饲养的模型动物AS发生及发展,为了消除异种蛋白对观察人血浆HDL制剂在兔AS模型动物中抗AS作用的影响,发明人除设置注射生理盐水的高脂对照(C)组外,还设置了与HDL中剂量组剂量相当的人血浆白蛋白阴性对照(A)组,结果显示,用人血浆HDL制剂来防治人类AS疾病不会发生异种蛋白反应。
实验主要结果如下:
(1)血脂分析的结果表明:人血浆HDL制剂并不具有使高脂饲养的AS模型动物(实验性AS预防及治疗试验)血脂(胆固醇TC、甘油三酯(TG)以及磷脂(PL)水平降低的作用。因此,HDL抗AS作用不是通过降低血脂(TC、TG)含量来实现的。
(2)血脂质过氧化物测定(实验性AS预防试验)的结果发现,注射HDL制剂,低、中、高(L、M、H)三个剂量组血脂质过氧化物水平(3.9、2.5、3.2mmol/ml)均比对照组水平(5.4及5.0mmol/ml)显著降低(P<0.05,P<0.01及P<0.001),表明人血浆HDL制剂具有对抗高脂饲养的家兔血清脂质发生氧化的作用。
(3)模型动物胸主动脉脂质及AS斑块面积测定发现,高脂饲养的家兔动脉壁脂质含量及AS斑块面积显著增加,且以阴性对照(A)组动物最为严重。注射HDL的各组动物动脉壁脂质及AS斑块面积减少或显著减少,其中以HDL中、高剂量(M、H)组最为明显。例如:预防试验M、H组动物动脉壁TC、TG含量及AS斑块面积分别为33.8、35.0、28.1、23.7mg/g干组织以及38.4%(M组),均比A组动物54.7、38.5mg/g干组织及57.6%显著减少(P<0.05);治疗试验的动脉壁脂质测定结果与预防试验相似,其L、M、H组AS斑块面积分别为60.2%、58.4%以及53.7%比A组AS斑块面积79.6%显著减少(P<0.01及P<0.001)。
(4)肝脏脂质测定结果与动脉壁脂质测定结果基本相似。预防试验动物肝脏TC、TG含量,H组为135.8及44.0mg/g干组织,与A组192.1及72.8(mg/g干组织)比较明显降低(P<0.05);治疗试验的结果更为突出,L、M、H组动物肝脏TC、TG以及PL含量均比A组明显降低(P<0.05及P<0.01)。
(5)胆汁脂质含量测定结果与动脉壁及肝脏脂质含量相反,无论是AS预防或治疗试验,注射HDL的各组动物与对照组比较其胆汁脂质含量呈增加趋势,且以预防试验的结果最明显。AS预防试验H组TC含量(337.5mg%,即每100ml含337.5mg,下同)显著高于C组(217.2mg%),M及H组PL含量(978.7及1008.8mg%)显著高于A组(688.3mg%,P<0.05)。
上述结果表明注射人血浆HDL具有使采用高脂饲养的As模型动物动脉壁、肝脏脂质沉积减少、胆汁脂质排出增加以及动脉壁AS斑块面积减小的作用。
本发明HDL通过给高脂模型家免注射HDL制剂,直接观察HDL抗AS作用,得到的实验结果表明,无论在高脂造型的同时,或高脂造成家兔AS模型以后,注射兔血HDL均可使造型动物动脉壁及肝脏沉积的脂质、动脉壁AS斑块面积显著减少,充分证实HDL具有抑制AS发生并促使AS逆转或消退(即预防和治疗)的双重作用。因此本发明HDL在制备预防和治疗AS的新药方面,有着很好的应用前景。
细菌毒素主要分为外毒素(exotoxin)和内毒素(endotoxin);一些危害人类健康的重要致病菌如白喉和破伤风等细菌的外毒素,随着其抗毒素和类毒素的相继研制成功和有效应用,已不再使人谈虎色变;如今对细菌毒素的抗争已聚焦在内毒素。
针对内毒素血症的抗内毒素治疗一直困扰着医学界。尽管采用了日臻完善的器官支持和重症监护技术,以及抗内毒素抗体、抗炎性细胞因子(炎症介质)抗体或炎性细胞因子受体拮抗剂等新药,但重症革兰氏阴性菌感染引起的内毒素血症所致全身炎症反应综合征(SIRS)以及由此而来的多脏器功能障碍综合症(MODS),其病死率却一直居高不下。因此,内毒素依然是一大医学难题。
内毒素在机体内的吞噬、降解和清除,有赖于免疫系统的积极参与;此外,机体内完善的解毒机制对内毒素的脱毒亦发挥了重要作用。机体的免疫和解毒系统在很多场合下既相互重叠或相互联系,又相互区别。内毒素进入人体后,血液中的脂蛋白,特别是高密度脂蛋白等参与脂质交换和转运的蛋白成分首当其冲地对内毒素予中和缓冲作用,血液的中性粒细胞释放某些阳离子蛋白(如杀菌通透性增加蛋白)对内毒素予脱毒解聚作用,组织中广泛分布的碱性磷酸酶还可对内毒素予去磷酸化作用,网状内皮系统则对内毒素予吞噬降解作用,由此形成机体对内毒素的解毒过程。所以,脂蛋白,特别是高密度脂蛋白具有保护机体解毒功能的作用,其在制备预防和治疗中和细菌毒素、以及多器官功能障碍综合症MODS新药方面,有着很好的应用前景。
本发明直接以人血浆为原料,在大量实验的基础上,通过分离纯化步骤,从中提取HDL制品,提供了大规模制备人血浆HDL的低温乙醇新工艺方法,并且成功制备了安全、稳定、无毒、无热原、无菌及无病毒污染、pH6.4~7.4、载脂蛋白AI含量≥0.5%、HDL纯度≥95%、含10%±1%蔗糖的供静脉输注的人血HDL制剂,脂质及载脂蛋白组成及含量分析表明,HDL制品的脂质及载脂蛋白组成与含量与天然HDL值基本一致,且不含可以促使动脉粥样硬化发生的低密度脂蛋白(LDL)及极低密度脂蛋白(VLDL)。

具体实施方式
实施例1
取原料血浆1000L,滴加pH=4.0的HAC/NaAC缓冲溶液25L,调整混合液的pH=5.2,加入95%乙醇324L,在-5℃下搅拌2小时,静置6小时,使用142型连续离心机,在14000r/min下离心,得上清1340L,沉淀50Kg;
向上清补加注射用水630L,滴加pH为4.0的HAC/NaAC混充溶液13.7L,使混合液pH为4.5,此时蛋白浓度为1%,离子强度为0.05,在-3℃下搅拌2小时,静置2小时,使用142型连续离心机,在14000r/min下离心,得沉淀9Kg,弃上清;
向沉淀中加入30mmol/L氯化钠溶液19.8L,滴加pH为4.0的HAC/NaAC混充溶液14L,使pH为5.5,在0℃下搅拌4小时,使用142型连续离心机,在14000r/min下离心,弃上清,得沉淀4.65Kg;
向沉淀中加入160mmol/L氯化钠465L,滴加1mol/L NaHCO3溶液378ml,使混合液pH=6.98,在5℃下搅拌6小时,过滤,得滤液577L;
向滤液中滴加pH=4.0的HAC/NaAC缓冲溶液109mL,使混合液pH为5.16,在3℃下搅拌1小时,静置6小时,用142型连续离心机,在14000r/min下离心,弃上清,得沉淀3.86Kg;
向沉淀中加入160mmol/L氯化钠溶液386L,此时溶解液pH为5.15,滴加1mol/LNaHCO3溶液347ml,使溶解液pH为7.20,在5℃下搅拌6小时,过滤,得滤液495L;
使用8-10KD超滤膜超滤,浓缩,得浓缩液350L,浓缩液中加入35Kg蔗糖,蛋白浓度为18.5g/L,乙醇含量为0.02%,电导率为0.4Ms;在微机自控的巴氏灭菌器内60±1℃、10小时灭活病毒;灭活病毒后的HDL,apoAI含量为89%,除菌分装成品50ml×6890瓶,并于2℃保存。
实施例2
取原料血浆1000L,滴加pH=4.0的HAC/NaAC缓冲溶液24.5L,调整混合液的pH=5.15,加入95%乙醇330L,在-4℃下搅拌2小时,静置8小时,使用142型连续离心机,在14000r/min下离心,得上清1270L,沉淀47Kg;
向上清补加注射用水646L,滴加pH为4.0的HAC/NaAC混充溶液14.3L,使混合液pH为5.2,此时蛋白浓度为1.68%,离子强度为0.07,在-6℃下搅拌2小时,静置2小时,使用142型连续离心机,在14000r/min下离心,得沉淀9.5Kg,弃上清;
向沉淀中加入30mmol/L氯化钠溶液19.0L,滴加pH为4.0的HAC/NaAC混充溶液13.7L,使pH为5.7,在1℃下搅拌4小时,使用142型连续离心机,在14000r/min下离心,弃上清,得沉淀4.33Kg;
向沉淀中加入160mmol/L氯化钠433L,滴加1mol/LNaHCO3溶液387ml,使混合液pH=7.08,在6℃下搅拌6小时,过滤,得滤液530L;
向滤液中滴加pH=4.0的HAC/NaAC缓冲溶液115mL,使混合液pH为5.2,在2℃下搅拌1小时,静置6小时,用142型连续离心机,在14000r/min下离心,弃上清,得沉淀3.83Kg;
向沉淀中加入160mmol/L氯化钠溶液383L,此时溶解液pH为5.19,滴加1mol/LNaHCO3溶液368ml,使溶解液pH为7.00,在5.5℃下搅拌5小时,过滤,得滤液480L;
使用8-10KD超滤膜超滤,浓缩,得浓缩液367L,浓缩液中加入36.7Kg蔗糖,蛋白浓度为19g/L,乙醇含量为0.01%,电导率为0.2Ms;在微机自控的巴氏灭菌器内60±1℃、10小时灭活病毒;灭活病毒后的HDL,apoAI含量为85%,除菌分装成品50ml×7250瓶,并于2℃保存。
实施例3
取原料血浆1000L,滴加pH=4.0的HAC/NaAC缓冲溶液24.3L,调整混合液的pH=5.22,加入95%乙醇304L,在-5℃下搅拌2小时,静置6小时,使用142型连续离心机,在14000r/min下离心,得上清1299L,沉淀45Kg;
向上清补加注射用水611L,滴加pH为4.0的HAC/NaAC混充溶液14L,使混合液pH为5.1,此时蛋白浓度为1.68%,离子强度为0.07,在-6℃下搅拌2小时,静置2小时,使用142型连续离心机,在14000r/min下离心,得沉淀8.9Kg,弃上清;
向沉淀中加入30mmol/L氯化钠溶液18.9L,滴加pH为4.0的HAC/NaAC混充溶液13.6L,使pH为4.9,在0℃下搅拌4小时,使用142型连续离心机,在14000r/min下离心,弃上清,得沉淀4.05Kg;
向沉淀中加入160mmol/L氯化钠405L,滴加1mol/LNaHCO3溶液375ml,使混合液pH=6.99,在8℃下搅拌6小时,过滤,得滤液570L;
向滤液中滴加pH=4.0的HAC/NaAC缓冲溶液130mL,使混合液pH为5.22,在2℃下搅拌1小时,静置6小时,用142型连续离心机,在14000r/min下离心,弃上清,得沉淀2.87Kg;
向沉淀中加入160mmol/L氯化钠溶液287L,此时溶解液pH为5.11,滴加1mol/LNaHCO3溶液340ml,使溶解液pH为7.01,在5℃下搅拌8小时,过滤,得滤液500L;
使用8-10KD超滤膜超滤,浓缩,得浓缩液378L,浓缩液中加入37.8Kg蔗糖,蛋白浓度为18g/L,乙醇含量为0.01%,电导率为0.3Ms;在微机自控的巴氏灭菌器内60±1℃、10小时灭活病毒;灭活病毒后的HDL,apoAI含量为84%,除菌分装成品50ml×7460瓶,并于2℃保存。
实施例4
取原料血浆1000L,滴加pH=4.0的HAC/NaAC缓冲溶液25L,调整混合液的pH=5.25,加入95%乙醇320L,在-5℃下搅拌2小时,静置8小时,使用142型连续离心机,在14000r/min下离心,得上清1380L,沉淀50Kg;
向上清补加注射用水648L,滴加pH为4.0的HAC/NaAC混充溶液14.3L,使混合液pH为4.55,此时蛋白浓度为1.88%,离子强度为0.15,在-6℃下搅拌2小时,静置2小时,使用142型连续离心机,在14000r/min下离心,得沉淀9.9Kg,弃上清;
向沉淀中加入30mmol/L氯化钠溶液19.8L,滴加pH为4.0的HAC/NaAC混充溶液15L,使pH为5.0,在2℃下搅拌4小时,使用142型连续离心机,在14000r/min下离心,弃上清,得沉淀4.32Kg;
向沉淀中加入160mmol/L氯化钠432L,滴加1mol/LNaHCO3溶液414ml,使混合液pH=7.12,在7℃下搅拌6小时,过滤,得滤液550L;
向滤液中滴加pH=4.0的HAC/NaAC缓冲溶液116mL,使混合液pH为5.12,在2℃下搅拌1小时,静置6小时,用142型连续离心机,在14000r/min下离心,弃上清,得沉淀3.12Kg;
向沉淀中加入160mmol/L氯化钠溶液312L,此时溶解液pH为5.14,滴加1mol/LNaHCO3溶液362ml,使溶解液pH为7.05,在8℃下搅拌8小时,过滤,得滤液489L;
使用8-10KD超滤膜超滤,浓缩,得浓缩液372L,浓缩液中加入37.2Kg蔗糖,蛋白浓度为19.5g/L,乙醇含量为0.02%,电导率为0.2Ms;在微机自控的巴氏灭菌器内60±1℃、10小时灭活病毒;灭活病毒后的HDL,apoAI含量为79%,除菌分装成品50ml×7240瓶,并于2℃保存。
实施例5
取原料血浆1000L,滴加pH=4.0的HAC/NaAC缓冲溶液24.3L,调整混合液的pH=5.15,加入95%乙醇331L,在-5℃下搅拌2小时,静置7小时,使用142型连续离心机,在14000r/min下离心,得上清1430L,沉淀57Kg;
向上清补加注射用水672L,滴加pH为4.0的HAC/NaAC混充溶液14.3L,使混合液pH为5.7,此时蛋白浓度为2%,离子强度为0.15,在-6℃下搅拌2小时,静置2小时,使用142型连续离心机,在14000r/min下离心,得沉淀10.4Kg,弃上清;
向沉淀中加入30mmol/L氯化钠溶液 20.8L,滴加pH为4.0的HAC/NaAC混充溶液15.1L,使pH为5.39,在2℃下搅拌4小时,使用142型连续离心机,在14000r/min下离心,弃上清,得沉淀3.98Kg;
向沉淀中加入160mmol/L氯化钠398L,滴加1mol/L NaHCO3溶液395ml,使混合液pH=7.1,在8℃下搅拌6小时,过滤,得滤液576L;
向滤液中滴加pH=4.0的HAC/NaAC缓冲溶液128mL,使混合液pH为5.16,在2℃下搅拌1小时,静置6小时,用142型连续离心机,在14000r/min下离心,弃上清,得沉淀3.69Kg;
向沉淀中加入160mmol/L氯化钠溶液369L,此时溶解液pH为5.15,滴加1mol/L NaHCO3溶液365ml,使溶解液pH为7.00,在8℃下搅拌6.5小时,过滤,得滤液560L;
使用8-10KD超滤膜超滤,浓缩,得浓缩液400L,浓缩液中加入40Kg蔗糖,蛋白浓度为19.3g/L,乙醇含量为0.01%,电导率为0.4Ms;在微机自控的巴氏灭菌器内60±1℃、10小时灭活病毒;灭活病毒后的HDL,apoAI含量为77%,除菌分装成品50ml×7910瓶,并于2℃保存。
实验例1
本实验例为本发明产品HDL的检定数据。
1、方法
1.1HDL的鉴定及纯度分析:采用免疫电泳及脂蛋白琼脂糖凝胶电泳鉴定制备的HDL,并根据脂蛋白琼脂糖凝胶电泳结果扫描分析其纯度。
1.2蛋白质含量测定:采用Markwell等的改良Lowry法进行测定。
1.3脂质及载脂蛋白含量测定:HDL中甘油三酯(TG)和总胆固醇(TC)采用北京中生北控生物科技股份有限公司酶法试剂盒测定,磷脂(PL)采用改良抗坏血酸还原法测定。载脂蛋白(apo)AI、AII、B100、CII、CIII、E采用四川大学华西载脂蛋白研究室免疫单向扩散试剂盒测定。
1.4apoAI分子量测定:采用SDS-PAGE法进行测定。
1.5HDL生物活性测定:采用徐燕华等建立的人血HDL生物活性测定法进行测定。按每1μg apoAI为1个HDL活性单位(U)。
1.6无菌检查:按《中华人民共和国药典》2005年版三部附录“无菌检查法”进行。
1.7细菌内毒素检查:按《中华人民共和国药典》2005年版三部附录“细菌内毒素检查法”进行。
1.8热原检查:按《中华人民共和国药典》2005年版三部附录“热原检查法”进行。
1.9异常毒性检查:按《中华人民共和国药典》2005年版三部附录“异常毒性检查法”进行
2、结果
2.1HDL的鉴定及纯度分析
(1)免疫电泳,见图3;由图3可见,HDL制品与正常人血浆在相同位置出现沉淀线。
(2)脂蛋白琼脂糖凝胶电泳,见图4;由图4可见,HDL制品在脂蛋白琼脂糖凝胶电泳中呈现一条区带,Biorad凝胶成像扫描系统扫描分析,其纯度为100%。
2.2HDL中脂质及载脂蛋白组成(%),见下表;由下表可见,本发明HDL制品apoAI含量较高。

实验例2
本实验例为人血HDL抗AS生物活性测定方法的研究,内容如下:
1、材料及试剂
日本大耳白兔;正常人血浆采自献血员;人血浆HDL制剂,按本发明所述方法制备;HRP(RZ=3.0),Sigma公司产品;纯化兔肝细胞膜、羊抗人apoAI IgG、HRP-抗apoAI IgG、人LDL及VLDL,自制;TG、TC酶法测定试剂盒,北京中生北控生物科技股份有限公司;载脂蛋白AI、B100、CII、CIII及E免疫单向扩散试剂盒,四川大学华西载脂蛋白研究室;其余试剂均为国产分析纯。
96孔聚苯乙烯酶标板,Corning公司产品;Model 550型酶标仪,Bio-RAD公司产品;L8-55型超速离心机,Beckman公司;Sebris扫描仪,Sebris公司产品。
包被缓冲液为pH9.6,50mmol/L Na2CO3-NaHCO3缓冲液;洗涤及稀释缓冲液为pH7.4,含0.05%Tween-20的20mmol/L PBS;酶反应基质液为pH5.6,含0.04%邻苯二胺、0.05%H2O2的0.1mol/L柠檬酸-Na2HPO4缓冲液(临用前配制)。
2、方法
2.1兔肝细胞膜的制备
按蔗糖密度梯度超速离心法纯化兔肝细胞膜。收集37%~41%蔗糖密度界面膜成分,用缓冲液(50mmol/L Tris-HCl,100mmol/L NaCl,0.5mmol/L CaCl2,pH7.5)稀释后,于27000×g离心20分钟。收集沉淀,加少量缓冲液,用MSE-150型超声波仪于冰浴中处理2×10秒,振幅14。制备的肝细胞膜分装于-70℃保存。膜产率为1.5~2.0mg/g湿肝组织,与肝匀浆比较,5′-核苷酸酶活力提高6~10倍。
2.2LDL及VLDL制备
按一次性密度梯度离心法分离人血浆脂蛋白收集d=1.030~1.050g/mL的LDL组分及d=0.95~1.006g/mL的VLDL组分,对10mmol/L Tris-HCl、1mmol/L EDTA,50mmol/L NaCl,pH7.4的缓冲液透析脱盐,4℃保存备用。
膜蛋白及脂蛋白均用Markwell等的方法测定蛋白质含量。
2.3HDL(实施例2)的处理
以本发明实施例2得到的人血HDL制剂为原料,超滤浓缩后按Havel等超速离心法制备。制备时HDL制剂用固体NaBr调密度至1.21g/mL,于L8-55型超速离心机50000rpm、10℃离心24小时。收集上层d≤1.21g/mL组分,透析,除菌分装,4℃保存备用。
HDL(实施例2)TG及TC含量采用酶法试剂盒测定;磷脂(PL)采用抗坏血酸还原法测定;apoAI、AII、B100、CII、CIII及E含量采用免疫单向扩散试剂盒测定。
2.4HDL生物活性检测试验
2.4.1测定步骤
按HDL受体酶联免疫检测法程序进行,见图6;
测定时,膜蛋白的包被浓度及HRP-抗apoAI IgG的稀释度,按棋盘滴定法确定分别为25μg膜蛋白/ml及1∶250。
2.4.2 HDL生物活性标准曲线
以制备的HDL(实施例2)为标准,按每1μg apoAI为1个HDL活性单位(U),将HDL参考品用稀释液稀释成80、40、20、10及5(U/mL),再按上述步骤测定,绘出光吸收值与HDL生物活性的标准曲线,并求出相应的双对数回归方程。
2.4.3样品测定
待测样品稀释后,在制作标准曲线的同一酶标板内测定其光吸收值。最后以标准曲线的回归方程及样品稀释倍数计算HDL制剂的活性单位。
3、结果
3.1HDL(实施例2)的鉴定
3.1.1HDL(实施例2)脂质及载脂蛋白组成,见下表:

由上表可见,HDL(实施例2)apoAI含量高于HDL文献值。
3.1.2脂蛋白预染电泳:HDL(实施例2)呈现一条区带,Sebris扫描仪扫描分析,其纯度为99.8%。
3.1.3免疫电泳:见图3。HDL(实施例2)与正常人血清在相同位置出现沉淀线。
3.2HDL生物活性标准曲线:HDL标准曲线结果表明,492nm处光吸收值与HDL活性单位数的对数,在1~16个活性单位范围内呈直线关系,见图7。
3.3特异性试验:由图7可见,用肝细胞膜包被的酶标板,纯化的人LDL及VLDL对酶-抗体交联物无竞争性作用,表明人LDL及VLDL与酶-抗体交联物无交叉反应。
3.4重复性试验:
3.4.1板内重复性:同一HDL制剂,分别稀释250及500倍,测得板内变异系数分别为7.6%及9.5%(n=10)。
3.4.2板间重复性:三块酶标板分别测定同一HDL制剂(250倍稀释),3日内测完,测得板间变异系数为12.3%。
实验例3
本实验例为HDL制剂抗AS生物活性稳定性试验。
在2~8℃、25℃及40℃分别放置实施例1制备的HDL制剂,定期取样,并测定其pH值、apoAI含量及抗AS生物活性,观察存放温度及存放时间对HDL制剂抗AS生物活性的影响。结果见下表:

由上表可见,在2~8℃下放置18个月及25℃下放置5个月,HDL制剂抗AS生物活性均没有明显变化,而在40℃下放置一个月时,其抗AS生物活性即明显降低;表明在低温条件(2~8℃)下,HDL制剂抗AS生物活性可以长时间保持稳定,而在高温(40℃)条件下,HDL制剂抗AS生物活性会明显下降;所以,HDL制剂的适合环境温度为2-8℃。
实验例4
本实验例说明本发明HDL制剂具有对抗内毒素血症的作用。
取正常200只兔,随机分组,每组100只,分为实验组和对照组。
对两组兔注射内毒素(2ng/kg)造成内毒素血症。
在注射内毒素前3.5小时,先给实验组静脉滴注实施例3制备的HDL制剂(10mg/kg,以apoAI含量计),30分钟滴完。
10小时后发现,实验组TNF-α、IL-6等炎性介质的释放较小,且实验组兔感染症状较轻,表明静脉滴注HDL具有对抗内毒素血症的作用,见下表:

本实验例充分证实本发明HDL制剂具有对抗内毒素血症的作用。本发明人研究认为,HDL分子外壳的磷脂层能够与内毒素分子中的类脂A紧密结合,结合后的内毒素不再与CD14受体结合,因而阻断了内毒素刺激单核/巨噬细胞释放炎性介质。
实验例5
本实验例说明HDL治疗能够有效降低多脏器功能障碍综合症(MODS)患者炎性介质。
1、目的:比较HDL治疗、连续性肾脏替代治疗(CRRT)对MODS患者炎性介质(白细胞介素-1β、白细胞介素-6和肿瘤坏死因子-α等)的治疗效果。
2、方法:选取表现为MODS的危重病患者40例,MODS诊断符合美国危重病协会1992年制定的诊断标准。患者按入院先后顺序随机分为实验组和对照组,单号为实验组,20例,实验组患者接受HDL治疗;双号为对照组,20例,接受连续性肾脏替代治疗(CRRT)治疗。两组分别在诊断MODS当时及治疗24小时后抽血待检。
3、测定:(放免药盒购自中国人民解放军总医院科技开发中心放免研究所)白细胞介素-1β和白细胞介素-6(IL-1β、IL-6)测定:取静脉血2ml,注入试管中待凝固后,分离血清,放-20℃低温保存,采用平衡法一次监测;肿瘤坏死因子-α(TNF-α):采静脉血2ml,以10%EDTA二钠作抗凝剂,分离血浆后,放-20℃低温保存,采用液相竞争法一次监测。
4、结果:实验组患者经HDL治疗24小时后,白细胞介素-1β、白细胞介素-6和肿瘤坏死因子-α明显低于对照组。见下表:

全身炎症反应(SIR)是MODS发病机制的基础,当机体遭受感染或创伤等严重打击后细菌/毒素或组织损伤将刺激机体巨噬细胞等炎性细胞,释放炎性介质(肿瘤坏死因子-α、白细胞介素-1β和白细胞介素-6等),肿瘤坏死因子是最早释放的炎性介质之一,可进一步刺激和激活巨噬细胞、粒细胞、淋巴细胞和内皮细胞,释放大量的炎性介质,形成炎性介质介导的“瀑布样”连锁反应,犹如多米诺骨牌逐级放大,使炎性反应失控。若不有效阻断其发展,将导致多脏器功能不全综合症(MODS),甚至多脏器系统功能衰竭(MOSF),而致病人死亡。
本实验例证实HDL治疗可以显著降低多脏器功能障碍综合症患者肿瘤坏死因子-α、白细胞介素-1β和白细胞介素-6等炎性介质浓度,有效降低病人死亡率。
Claims (6)
1、一种人血高密度脂蛋白的制造方法,其特征在于,所述方法包括以下步骤:
A.取原料血浆,滴加缓冲溶液至混合液的pH为5.0-5.5,加入混合液1/4~1/3体积的95%乙醇,在-5~-2℃下搅拌1-3小时,静置6-8小时,离心得上清;向上清补加45-55%上清体积的注射用水,滴加缓冲溶液,使混合液pH为4.5-6.0,在-6~-3℃下搅拌1-3小时,静置1-3小时,离心得沉淀,弃上清;
B.沉淀用2-3倍重量的0.02~0.1MNaCl溶液溶解,调pH至5.2±0.5后,在0~2℃下搅拌2-6小时,然后离心得沉淀;沉淀用50-150倍重量的0.1~0.2M NaCl溶液溶解,调节溶液pH为7.2±0.5,在2~8℃下搅拌6-20小时,过滤得滤液;
C.上述滤液加入缓冲液,使pH为5.2±0.5,搅拌1-3小时,在2~4℃下,静置6-20小时,离心收集沉淀;沉淀用50~150倍量的0.1~0.2M NaCl溶液溶解,调pH至7.2±0.5,搅拌使沉淀完全溶解,过滤,得滤液;
D.滤液用超滤膜超滤,脱盐并浓缩,得富含人血高密度脂蛋白的溶液,经后处理,得人血高密度脂蛋白。
2、根据权利要求1所述的制造方法,其特征在于,所述方法中调节pH使用1mol/LNaHCO3溶液和/或所述的缓冲液为pH=4的HAC/NaAC缓冲体系。
3、根据权利要求1所述的制造方法,其特征在于,所述步骤C结束后重复一遍。
4、根据权利要求1所述的制造方法,其特征在于,所述步骤D中的超滤膜为8-70KD超滤膜。
5、根据权利要求4所述的制造方法,其特征在于,所述步骤D中的超滤膜为30~50KD超滤膜。
6、根据权利要求1所述的制造方法,其特征在于,所述步骤D所述的后处理可以是:超滤浓缩液加9~11%注射用蔗糖或0.5%白蛋白,按每升加入150mg±1mg注射用维生素C,调pH至7.2±0.5,得人血高密度脂蛋白溶液;上述人血高密度脂蛋白溶液于微机自控的巴氏灭菌器内60±1℃、10小时灭活病毒;灭活病毒后的人血高密度脂蛋白溶液,除菌分装,并于2~8℃保存。
