Abstract

本器械用于慢性非化脓性中耳炎的鼓室积液的治疗。由带内芯的金属套管,后端有持管柄构成,内芯径1mm。通气管为哑铃硅胶管,内径为1mm,正嵌合在内芯上,内芯较套管长2mm,硅胶管套于内芯,置入切开的鼓膜间,待抽取内芯时,管便嵌于鼓膜切口间。

Description

鼓膜置管器
鼓膜置管术用于治疗慢性非化脓性中耳炎(慢性鼓室积液)。慢性非化脓性中耳炎是由于咽鼓管功能障碍，鼓室长期负压，而致鼓室积液，初期为渗出液，随着病程延长，鼓室粘膜分泌亢进，积液呈粘性不易排除，如果长期不治疗，积液逐渐呈胶冻状，导致鼓室粘连，听力下降。
治疗方法：初期穿刺抽出渗液，鼓室注入消炎药，用血管收缩剂点鼻，改善咽鼓管功能。连续4次穿刺注药不见好转者，可考虑置管治疗。
目前无锡市程冯新科技开发实验厂生产的置管器械及其系列配套产品可用于临床(中华耳鼻咽喉杂志，1997,4,29卷，2期)，但该安装器前端为镊状，钳夹使用，比稳固，本置管器是用套入置管及退抽内心送入鼓膜切口处，置入稳定，能一次性置入。而且该器械为上颌窦穿刺针稍加改造而成，取材方便，使用便捷，能有效地达到治疗目的。
1、取一根外套管长7.5cm，内芯长7.7cm；外径1mm的不锈钢针管(因外耳道长2.5-3.5cm)；顶部有冠嵌合(见图)。
2、内外管前端均磨平。
3、置管器顶端有呈180的两柄，为操作时手持部。
4、通气管为哑铃管，内径为1mm。
5、使用时将通气管套在置管器的突出端，然后用小棉签将麻醉剂涂于鼓膜表面(或部分外耳道)10分钟左右即可操作(不做棉片贴敷及麻醉剂灌入耳道，以免鼓膜灼伤加速表皮角化脱落)。鼓膜麻醉后，在其前下或后下用鼓膜切开刀做3mm平行鼓环略带弧形切口，将置管器的的通气管插入切口内，左手固定器械柄，右手抽出置管器内芯，将管嵌在鼓膜切口处。
6、优点：
1)置管器内芯采用抽入法，置管较稳固，易一次成功。
2)哑铃管不易内脱或外脱。
3)手握置管柄，操作较容易。

  1. 本发明利用1mm内径金属实芯作为鼓膜通气管的支架，金属外套作为通气管的底托，使用时只要抽出实芯，管便置入鼓膜间。
