Submucosal dissection (Endoscopic submucosal dissection, ESD) via endoscope refers to endoscopic minimally invasive surgery using various endoscopic electrotomes to submucosally dissect early lesions of the digestive tract greater than 2 cm. The technique realizes the whole excision of a larger lesion part and provides accurate case diagnosis stage for the lesion.
Compared with the traditional surgery, the ESD can better maintain the physiological functions of the digestive tract on the basis of tumor resection, thereby obviously improving the postoperative life quality of patients, and is currently the first choice treatment method for early cancers and precancerous lesions of the digestive tract including esophagus.
A common complication after large area esophageal lesions ESD surgery is luminal stenosis, which is also one of the pain spots in the clinic at present. To solve this problem, the methods commonly used at present are: the treatment schemes of hormone oral administration, hormone injection, expansion, stent installation and the like, but the actual effect is not exact, and the repeatability cannot be ensured. The early-stage published clinical research shows that the short-course oral administration of the hormone has better combined hormone soaking effect and can obviously reduce the incidence rate of esophageal lumen stenosis.
In the process of intracavity hormone injection and soaking diagnosis and treatment, firstly, the gas in the gastric cavity and the esophageal cavity is required to be sucked out, and then a certain amount of hormone diluent is injected into the esophageal cavity, but due to peristaltic movement of the lumen itself and choking cough possibly occurring in operation, the retention time of the medicine in the lumen is uncontrollable, and the treatment effect is affected.
Therefore, in order to solve the problems, the application provides a catheter which can seal the lumen in a period of time so as to ensure that the medicine is fully mixed and soaked in the wound surface, in particular to a single balloon catheter with the functions of medicine injection and sealing.
Embodiments of the present application are described in further detail below with reference to FIGS. 1-4.
Referring to fig. 1 and 2, fig. 1 is a schematic partial view of a single balloon catheter with drug injection and blocking functions according to an embodiment of the present application, and fig. 2 is a cross-sectional view of a single balloon catheter with drug injection and blocking functions according to an embodiment of the present application in a direction A-A. An embodiment of the present application provides a single balloon catheter 1 with drug injection and blocking functions, comprising a first tube 11 and a second tube 12 stacked in the first tube 11. The first tube member 11 includes an injection passage 111 extending in the longitudinal direction F1, the injection passage 111 penetrating through an end face of the proximal end of the first tube member 11 to form an insertion port into which the second tube member 12 is inserted, the second tube member 12 being slidably mounted in the injection passage 111 through the insertion port.
In the embodiment of the present application, in order to facilitate the sliding operation of the second pipe 12 in the injection channel 111, an oil film layer is coated on the inner wall of the injection channel 111, so as to reduce the friction resistance between the outer wall of the second pipe and the inner wall of the injection channel 111. The oil film layer may be, for example, glycerol, or other liquid with lubricating effect, which is not limited herein.
In addition, in order to facilitate the single balloon catheter 1 with the functions of drug injection and blocking to be smoothly inserted into the natural cavity of the human body for administration, the first tube member 11 and the second tube member 12 in the embodiment of the application are made of flexible materials, and may be specifically made of polyurethane materials.
Referring to fig. 3, fig. 3 shows a cross-sectional view of a single balloon catheter B-B with drug injection and occlusion functions in accordance with an embodiment of the present application. Specifically, the first tube 11 includes a first through hole 114 disposed on the sidewall, and the first through hole 114 communicates with the injection channel 111 along the radial direction F2, so as to communicate the injection channel 111 with the outside.
The second tube member 12 includes a drug delivery channel 121 and a second through hole 122 provided in a side wall, wherein the drug delivery channel 121 extends in a longitudinal direction F1 and penetrates an end surface of a proximal end of the second tube member 12 to form a drug delivery port through which a drug is injected into the drug delivery channel 121 for delivery. The second through hole 122 communicates with the above-mentioned administration passage 121 in the radial direction F2 for communicating the administration passage 121 with the first through hole 114, thereby releasing the medicine to the outside to perform a therapeutic operation.
In the embodiment of the application, the medicine can be a hormone liquid medicine. It will be appreciated that in other embodiments, the medicament may be other kinds of medicaments, and the medicament is selected adaptively according to actual operation conditions, which is not limited in the present application.
As shown in fig. 2 and 3, in some embodiments, the single balloon catheter 1 with the drug injection and blocking functions includes a closed state and an operating state, and when the drug injection is not needed, the single balloon catheter 1 with the drug injection and blocking functions maintains the closed state, and the inside of the single balloon catheter is isolated from the outside, so that the drug stored in the catheter is not easy to be polluted by air, and the drug effect is ensured.
After the single balloon catheter 1 with the drug injection and blocking functions is inserted into a human body, the single balloon catheter 1 is switched to a working state, and at the moment, the liquid medicine inside the single balloon catheter 1 with the drug injection and blocking functions directly contacts with a lesion part after flowing out of the catheter, so that the possibility that the drug is polluted by the outside is low, the drug input into the human body is ensured to be pollution-free and bacteria-free as far as possible, and the treatment effect is improved.
In the embodiment of the present application, when the single balloon catheter 1 with both drug injection and blocking functions is in a closed state, the distal end of the first through hole 114 contacts with the distal end of the second through hole 122, and the first through hole 114 and the second through hole 122 are not overlapped in the radial direction F2, i.e., the second through hole 122 is blocked by the side wall of the first tube 11, so that the drug solution is isolated in the drug administration channel 121, and is prevented from being polluted due to contact with the outside.
When the single balloon catheter 1 with the functions of drug injection and blocking is in a working state, at least partial coincidence exists between the first through hole 114 and the second through hole 122 in the radial direction F2, so that the drug injected into the drug administration channel 121 can flow to the outside through the second through hole 122 and the first through hole 114 in sequence, and the drug administration operation on the lesion part is completed.
In addition, along with the sliding operation of the second tube 12 in the first tube 11, the area of the overlapping portion between the first through hole 114 and the second through hole 122 is also changed, so as to control the drug flow, so as to adapt to different drug administration requirements of different patients.
With continued reference to fig. 3, in some embodiments, in order to further ensure the smoothness of the switching between the closed state and the working state of the single balloon catheter 1 with the drug injection and the blocking function, the single balloon catheter 1 with the drug injection and the blocking function further includes a limiting member 13, where the limiting member 13 is adhered or integrally formed to the outer wall of the second tube 12, and as shown in fig. 3, the limiting member 13 is connected to the second tube 12 by adopting an adhering manner.
Specifically, the first pipe fitting 11 is internally provided with the limiting groove 115 extending along the longitudinal direction F1, the limiting piece 13 is slidably mounted in the limiting groove 115, and the side wall of the limiting piece 13 is in contact with the groove wall of the limiting groove 115, so that the limitation on the circumferential position of the second pipe fitting 12 is realized, the second pipe fitting 12 is not easy to twist in the first pipe fitting 11, and therefore, the first through hole 114 and the second through hole 122 can at least partially overlap in the circumferential direction in the sliding process of the second pipe fitting 12, and the single balloon catheter 1 with the functions of drug injection and blocking is further ensured to be freely switched between a closed state and a working state.
With continued reference to fig. 2 and 3, the stop 13 includes a closed position and a plurality of open positions spaced from the closed position. Wherein, the closed position is correspondingly arranged at the distal end of the limit groove 115. When the stopper 13 is in the closed position, the first through hole 114 and the second through hole 122 are offset from each other in the radial direction F2, and the drug solution is blocked in the drug delivery channel 121. The plurality of opening positions are arranged on one side of the closed position close to the proximal end of the limiting groove 115, and all the closed positions are distributed from the distal end of the limiting groove 115 to the proximal end of the limiting groove 115.
In the embodiment of the application, the adjacent opening positions are distributed at equal intervals, so that an operator can monitor the medicine injection efficiency corresponding to the current opening in real time according to the opening positions. In other embodiments, the spacing between adjacent opening positions may also be unequal to accommodate different dosing requirements for different treatment procedures.
Further, in order to facilitate the operator to perceive the real-time position of the limiting member 13, in some embodiments, the single balloon catheter 1 with the functions of drug injection and blocking further includes a plurality of opening ribs 14 disposed in the limiting groove 115, and the opening ribs 14 are disposed in one-to-one correspondence with the opening positions.
In the embodiment of the application, three opening ribs 14 are arranged from the far end to the near end of the limiting groove 115, and the three opening ribs 14 are uniformly distributed in the limiting groove 115 and are connected with the groove wall of the side, away from the limiting piece 13, of the limiting groove 115. In the embodiment of the present application, the opening rib 14 may be connected to the first pipe 11 by bonding or integrally forming, as shown in fig. 3, which is only the connection achieved by bonding, it is understood that in other embodiments, the opening rib 14 and the first pipe 11 may be configured as an integral structure.
Specifically, in the radial direction F2 of the first pipe fitting 11, at least a part of the opening rib 14 and the limiting member 13 overlap, that is, in the sliding process of the second pipe fitting 12, the end portion of the limiting member 13 can be abutted against the end portion of the opening rib 14, and in the actual operation process, an operator can determine the size of the communication area between the first through hole 114 and the second through hole 122 by counting the number of times of the withdrawal resistance felt when withdrawing the second pipe fitting 12.
It should be noted that, in the embodiment of the present application, the opening rib 14 and the limiting member 13 are made of flexible materials, so as to ensure that the opening rib can be elastically deformed under the condition that the opening rib and the limiting member are partially overlapped in the radial direction F2, so that the second pipe fitting 12 can slide smoothly.
Referring to fig. 3 specifically, in the embodiment of the present application, the opening ribs 14 are specifically set to three, and the three opening ribs 14 are uniformly distributed along the extending direction of the limiting groove 115, and different openings between the opening ribs 14 and the adjacent opening ribs 14 or the wall of the limiting groove 115 define the first through hole 114 and the second through hole 122.
Specifically, from the distal end to the proximal end of the limiting groove 115, the opening degrees are respectively set to 33%, 66% and 100%, so as to meet different injection requirements of different patients. It will be appreciated that in other embodiments, more opening positions may be provided and more opening ribs 14 may be correspondingly provided, thereby improving the accuracy of the dosing rate adjustment.
Referring to fig. 1, in some embodiments, the single balloon catheter 1 with drug injection and blocking functions further includes a balloon 15 mounted on the first tube 11, and the balloon 15 is closer to the distal end of the first tube 11 than the first through hole 114. In the embodiment of the application, the balloon 15 is arranged to be capable of stretching along the radial direction F2, so as to be clamped and fixed on the lesion part of a patient, thereby realizing the fixation of the single balloon catheter 1 with the functions of drug injection and blocking. Specifically, a filling cavity 151 is disposed in the balloon 15, and a filling channel 112 is further disposed in the first tube 11 along the longitudinal direction F1, and the filling channel 112 is disposed at a distance from the injection channel 111. Wherein the filling channel 112 is connected to the filling cavity 151 for injecting a filling medium into the filling cavity 151, and the diameter of the balloon 15 is changed along with the change of the volume of the injected filling medium.
