Abstract
本发明涉及iPS诱导定向分化的成内皮祖细胞和抗体联合治疗心脑血管疾病。本发明提供了诱导产生的内皮祖细胞与双特异性抗体一起组成的药物组合物。本发明开发了将诱导产生内皮祖细胞与抗体联用后能够有效的诱导VIII因子表达增加,心肌VEGF蛋白表达上调,左室射血分数及左室短轴缩短率明显提高,使得EPCs归巢率增加,EPCs归巢后能使梗死心肌处毛细血管密度明显增加,心功能改善,更有效的促进血管修复与新生,实现心脑血管疾病的治疗。

技术领域
本申请涉及生物领域,更具体的涉及iPS诱导定向分化的成内皮祖细胞和抗体联合治疗心脑血管疾病。
背景技术
内皮祖细胞(EPCs)是一类可以分化为成熟内皮细胞的前体细胞,由中胚层的成血管母细胞分化而来,出生后存在于骨髓、外周血中。在某些缺血因素的刺激下,EPCs开始从骨髓基质中脱离,并移行进入到外周血中,逐渐向组织缺血部位移行,并最终定居于缺血组织,在缺血组织定居的EPCs继续分化、并整合入新的血管,包括原先存在的血管的延伸(血管形成)和原位新血管生成。
大部分的内皮祖细胞存在于造血干细胞和骨髓基质细胞提供的造血微环,与骨髓移植过程中造血干细胞的动员和归巢相似,骨髓中的EPCs可被动员到外周血中,并趋化至缺血部位。在某些缺血因素的刺激下,EPCs开始从骨髓基质中脱离,并移行进入到外周血中,这一过程又称为动员,进入外周血循环的EPCs逐渐向组织缺血部位移行,并最终定居于缺血组织,这一过程称为归巢(Homing);在缺血组织定居的EPCs继续分化、并整合入新的血管,包括原先存在的血管的延伸(血管形成)和原位新血管生成,这一系列的过程中受到多种因素的影响。近年来的研究发现,体内缺血、血管损伤和大量细胞因子,如:VEGF、SDF-1、G-CSF、EPO或药物,如:他汀类降脂药物、雌激素等都可动员骨髓中的EPCs,使其向外周循环迁移,并分化为成熟内皮细胞,促进形成新生血管,缓解组织缺血和修复血管损伤。而各种心血管病的危险因素如吸烟、高半胱氨酸血症、高水平的LDL-C等作用则相反。VEGF作为血管内皮细胞特异性的有丝分裂原和趋化因子,无论在胚胎发育过程中还是在出生后血管新生中均发挥重要调控作用,VEGF能够促进EPCs的增殖、迁移和趋化。在鼠缺血模型中,经VEGF处理后发现EPCs数量明显增加、增殖和迁移活性大大加强。
近年来,干细胞移植治疗心肌梗死是当前热点之一。将体外扩增的EPCs经静脉注射到冠状动脉前降支结扎的心肌缺血模型后,发现EPCs能够增加缺血心肌的血供,减少缺血面积,改善左心室功能。研究发现:CD34+EPC衍生细胞可在体外扩增培养,并达到临床可用的数量。而体内实验提示,在实验性心肌梗死模型,这些细胞可增殖、形成血管结构、并促进心肌梗死后左室功能的恢复。EPCs在促进血管新生上的作用已经被大量实验所证实。Kalka等成功的将体外扩增的人内皮祖细胞(hEPCs)用于裸鼠缺血后肢,发现缺血区血管密度增加、血流改善。研究发现移植体外扩增的hEPCs对缺血心肌模型左室功能的维持有益;静脉输注的hEPCs聚集于心肌缺血区并参与心肌新生血管的形成;超声心动图检查提示移植内皮祖细胞组心室明显小于对照组,缩短率(FS)明显提高,心室瘢痕区更小,心室壁活动更佳。将骨髓单个核细胞注入大鼠心肌梗死区,3周后心肌梗死区血流、毛细血管密度和代偿性微血管数目各自增加了4.6倍、2.8倍和5.7倍,标记的骨髓单个核细胞整合入约31%的新生毛细血管中,心脏功能得以明显改善。干细胞能修复受损心脏,最有说服力的证据来自于临床试验。用自体骨髓单个核细胞移植治疗一名大面积心肌梗死患者获得成功,植入的细胞成功地再造了被破坏的心肌组织和血管组织。
将内皮祖细胞移植于缺血心肌后,明显促进血管生长,改善左室功能。同时发现将内皮祖细胞体外转染VEGF基因后,治疗效果较单纯内皮祖细胞移植明显增强,移植细胞成活率和新生血管数目显著增加,缺血区心肌细胞凋亡数量明显减少,左室舒张末容积和心功能进一步改善。研究发现单核细胞和内皮祖细胞联合移植对心脏超声、血流动力学各项指标均有改善,能刺激小血管生成,抑制纤维化,还发现内皮祖细胞能部分向心肌细胞分化,从而减少心肌梗死面积。也有研究发现内皮细胞灌注和免疫抑制剂环孢霉素同时运用可以降低白细胞介素2,γ-干扰素的表达,增加转化生长因子B的表达,减少肿瘤坏死因子α表达,减轻同种异体大鼠心脏移植术后的急性排斥反应,延长供心存活时间。此外,17β-雌二醇调节骨源性内皮祖细胞(BM-EPCs)对心血管系统的影响。结果发现BM-EPCs移植后25d,小鼠心室扩张程度明显减轻,心肌梗死区域新生血管密度明显升高,左室纤维化面积占整个左室面积的百分比明显低于空白对照组。静脉注射从人脐静脉血分离的内皮祖细胞给予卒中后48h的大鼠,发现其在缺血区能诱导血管新生,并提供神经生成的适宜环境,首次证实了在缺血性脑损害的修复中血管生成与神经生成的直接关系。将神经干细胞来源的神经球移植入大脑中动脉末端梗死7d后的缺血鼠皮质,4周后发现,距离损伤边缘的细胞生存不佳,提示炎性细胞因子对移植的细胞有害,而血管新生加速毒性产物的清除,促进化学活性物与神经营养因子的释放,如白细胞介素8、成纤维细胞生长因子、血小板源性生长因子、脑源性神经营养因子,这些因子都能诱导新神经元形成。此外,采用血管内皮生长因子动员自体骨髓内皮祖细胞,对脑缺血/再灌注损伤小鼠的影响。发现实验组血管内皮生长因子持续增加,在第四天达到高峰,同时梗死灶体积明显缩小。
目前,应用研究内皮祖细胞的难点在于还没有一种比较简便的方法鉴定纯化,本发明提供了一种工程化的采用IPS细胞诱导分化获得内皮祖细胞的方法,该方法可以批量获得大量的内皮祖细胞,解决了内皮祖细胞获得困难的技术难题。同时,本发明将内皮祖细胞与治疗性单克隆抗体一起使用后,能够有效的促进内皮祖细胞发挥功效。
发明内容
本发明开发了将诱导产生内皮祖细胞与抗体联用后能够有效的用于促进心肌血管新生,实现心脑血管疾病的治疗。
具体的,本发明内皮祖细胞是诱导产生的,具体方法参考本发明在前的专利申请(CN202410231405X)中所公开。
如在前专利所述,本发明同样也一种新的GREM1抑制剂,所述抑制剂能够较好的抑制GREM1的活性,进而有效的与GREM1重组蛋白一起配合使用有效的促进IPS细胞诱导分化为内皮祖细胞。
一方面,本发明提供了一种特异性针对GREM1的单克隆抗体。
具体的,所述的特异性针对GREM1的单克隆抗体G-3D8,该单克隆抗体G-3D8通过测序,鉴定获得其重链可变区序列如SEQ ID NO:1所示,轻链可变区序列如SEQ ID NO:2所示。
具体的,通过亲和力测定,本发明的单克隆抗体G-3D8的亲和力为2.13×109M-1 ,亲和特性较好。
进一步的,本发明中涉及的皮肤成纤维细胞制备ips细胞的方法是本公司已经较为成熟的技术。已经在此前的发明专利中公开。进一步的,所述的重编程可以采用本领域较为成熟的重编程试剂盒转染获得。
本发明进一步的,还提供了一种促进ips细胞分化为内皮祖细胞(iEPC)的方法,所述方法包括使用针对GREM1的单克隆抗体G-3D8促进相应的分化效果。
进一步的,本发明还提供了一种促进ips细胞分化为内皮祖细胞(iEPC)的培养基,其中所述的培养基中含有本发明的针对GREM1的单克隆抗体G-3D8。
具体的,所述的培养基可以是DMEM/F12细胞培养基或者mTeSR1培养基。所述培养基均为市售的成熟的培养基。
进一步的本发明的提供了一种促进ips细胞分化为内皮祖细胞(iEPC)的方法,人ips细胞在mTeSR1培养基中维持培养,以80-90%的密度在基质胶包覆的六孔板种板,mTeSR1培养液中加入10μM Y27632。种板后24hr开始诱导分化,Day0在培养液中加入含有1-10μM G-3D8单抗和50-100mg/ml抗坏血酸的DMEM/F12培养基。分化第2天抽吸培养基,加入50-100mg/ml抗坏血酸DMEM/F12培养基。然后每天更换DMEM/F12培养基来维持细胞。在分化第5-8天加入重组蛋白GREM1,GREM1的加入浓度为0.1-1μg/ml,分化后第9天即可收获相应的内皮祖细胞。
进一步的,本发明的ips细胞是通过已经成熟的方法制备获得。具体的,将分离培养的皮肤成纤维细胞调整细胞数,电转缓冲液中加入Epi5TM Episomal iPSC 重编程试剂盒中的重编程因子,采用常规电转染细胞按2ml/孔接种于基质胶包被的6孔板中,置于培养箱中培养。并使用ReproTeSRTM重编程培养基隔天换液,转染后第8天,将细胞消化后重新接种在5代小鼠成纤维细胞制作的饲养层上,第18天,观察iPSCs可见典型的克隆,采用试剂盒检测干细胞多能性基因OCT4、SOX2、NANOG、KLF4和LIN28表达情况,将阳性ips克隆进行筛选后重新接种于基质胶包被的6孔板中,置于培养箱中培养,使用mTeSRTM培养基继续培养,筛选纯化后备用。
一种通过ips细胞分化为内皮祖细胞的方法。更具体的,本发明开发了针对GREM1的单克隆抗体G-3D8,该单克隆抗体能够通过抑制GREM1进而有效的促进ips细胞的分化,并且采用单抗替换CHIR99021,表现出了更好的特异性抑制作用,避免了CHIR99021由于多靶点作用的副作用进而影响相应的分化促效果。
进一步的,本发明提供了一种药物组合,所述药物组合为诱导产生的内皮祖细胞与双特异性抗体一起组成。
进一步的,本发明提供了一种治疗心血管疾病的方法,所述方法包括将诱导产生内皮祖细胞与抗体联用后能够有效的用于促进心肌血管新生,实现心脑血管疾病的治疗。
有益效果
本发明提供了诱导产生的内皮祖细胞与双特异性抗体一起组成的药物组合物。本发明开发了将诱导产生内皮祖细胞与抗体联用后能够有效的诱导VIII因子表达增加,心肌VEGF蛋白表达上调,左室射血分数及左室短轴缩短率明显提高,使得EPCs归巢率增加,EPCs归巢后能使梗死心肌处毛细血管密度明显增加,心功能改善,更有效的促进血管修复与新生,实现心脑血管疾病的治疗。

具体实施方式
下面将参照附图更详细地描述本发明的具体实施例。虽然附图中显示了本发明的具体实施例,然而应当理解,可以以各种形式实现本发明而不应被 这里阐述的实施例所限制。相反,提供这些实施例是为了能够更透彻地理解 本发明,并且能够将本发明的范围完整的传达给本领域的技术人员。

实施例1 皮肤成纤维细胞制备ips细胞
本实施例所采用的方法是本公司已经成熟使用的技术,如在前专利公布的那样。取手术后分离的儿童包皮皮肤约0.3cm×0.3cm, 将其置于含有青霉素(100kU/L)和链霉素(100mg/L)的HBSS中反复漂洗后,去除皮下脂肪。在60mm培养皿中用眼科剪将皮肤剪成1mm2大小,置于含有青霉素(100kU/L)和链霉素(100mg/L)的DMEM/F12(1:1)液3ml 37℃下置于体积分数为5%的CO2、95%空气、饱和湿度孵箱中过夜。次日挑除汗腺腺体,待成纤维细胞生长明显时,吸弃旧培养液,用无Na+、Mg2+的HBSS洗2次,加入含有质量分数为0.25%胰酶和0.02%乙二胺四乙酸(EDTA)的D-Hank消化液约1ml 37℃下置于5%CO2、95%空气、饱和湿度条件下孵育2min,加入含10%胎牛血清的DMEM约2ml终止消化,1000r/min离心6min并收集细胞,用含胎牛血清(10%)、青霉素(100kU/L)和链霉素(100mg/L)的DMEM 分散细胞,细胞以1x104/ml接种4ml至25cm²的培养瓶中,在37℃、5%CO2、95%空气、饱和湿度条件下培养,经过连续5次传代培养后,可得到纯化的成纤维细胞。去少量细胞进行HE染色发现,细胞为长梭形,细胞核为淡蓝色,细胞浆为粉红色,通过染色体核型分析显示为46条染色体,鉴定制备获得了较为纯净的成纤维细胞。
将分离培养的皮肤成纤维细胞调整细胞数为1×106个,100μl电转缓冲液中加入3μl Epi5TM Episomal iPSC 重编程试剂盒中的重编程因子,采用常规电转染细胞按2ml/孔接种于基质胶包被的6孔板中,置于培养箱中培养。并使用ReproTeSRTM重编程培养基隔天换液,转染后第8天,将细胞消化后重新接种在5代小鼠成纤维细胞制作的饲养层上,第18天,观察iPSCs可见典型的克隆,采用试剂盒检测干细胞多能性基因发现OCT4、SOX2、NANOG、KLF4和LIN28均阳性表达,表明制备获得了ips细胞。将阳性ips克隆进行筛选后重新接种于基质胶包被的6孔板中,置于培养箱中培养,使用mTeSRTM培养基继续培养,筛选纯化后备用。
进一步的,为了检验ips细胞的活性,将制备的ips细胞体外悬浮培养形成EB,裂解细胞提取RNA,用于RT-PCR检测EB细胞基因表达量,结果显示,EB细胞的胚层分化基因外胚层Nestin、中胚层Eomes、内胚层AFP的表达水平均显著高于对照组ips细胞5倍以上(P<0.01),实验结果表明所建立的人ips细胞具有体外分化能力。
实施例2 ips细胞向内皮祖细胞(iEPC)的分化
CHIR99021对照组:实施例1制备的经鉴定的人ips细胞在mTeSRTM培养基中维持培养,以80-90%的密度在基质胶包覆的六孔板种板,mTeSRTM培养基中加入10μM Y27632。种板后24hr开始诱导分化,Day0在培养液中加入含有6μM CHIR99021和60mg/ml抗坏血酸的DMEM/F12培养基。分化第2天抽吸培养基,加入60mg/ml抗坏血酸DMEM/F12培养基。然后每天更换DMEM/F12培养基来维持细胞。在分化第5-8天加入重组蛋白GREM1,GREM1的加入浓度为0.25μg/ml,分化后第9天即可收获相应的内皮祖细胞。
单抗实验组:实施例1制备的经鉴定的人ips细胞在mTeSRTM培养基中维持培养,以80-90%的密度在基质胶包覆的六孔板种板,mTeSRTM培养基中加入10μM Y27632。种板后24hr开始诱导分化,Day0在培养液中加入含有6μM G-3D8单抗和60mg/ml抗坏血酸的DMEM/F12培养基。分化第2天抽吸培养基,加入60mg/ml抗坏血酸DMEM/F12培养基。然后每天更换DMEM/F12培养基来维持细胞。在分化第5-8天加入重组蛋白GREM1,GREM1的加入浓度为0.25μg/ml,分化后第9天即可收获相应的内皮祖细胞。
利用流式细胞仪鉴定分化后的内皮祖细胞中CD31/CD34和CD144/VEGFR2的双阳性率,结果如本公司在前专利(CN202410231405X)中所示。单抗实验组相比于CHIR99021对照组,其CD34/CD31双阳性细胞从(33.87±0.45)%提升到了(39.31±0.59)%;其CD144/VEGFR2双阳性细胞从(36.53±0.79)%提升到了(41.25±0.93)%。将两组分化获得内皮祖细胞进行筛选,选取二组都是双阳性的细胞进行增殖后备用。
实施例3 诱导的内皮祖细胞(iEPC)的功效验证
将人Anti-CD34抗体(货号:fnab01469,武汉菲恩生物科技有限公司)和人心肌肌凝蛋白抗体(AMLCA抗体)(产品货号:XG-K98396,西格生物)。采用化学交联制备得到CD34×AMLCA双特异性抗体。具体如下: CD34抗体10mg,溶于50mMNaCl和1mM的EDTA中,调节pH至8.0,然后按1:5摩尔的比例溶于Traut's Reagent试剂,室温下反应1h;AMLCA单克隆抗体10mg,溶于0.1mM 磷酸钠与0.15mM NaCl混合液,调节pH至7.2,然后按1:10摩尔的比例溶于Sulpho-SMCC试剂,室温下反应1h。2种溶解后的单抗立即以等摩尔比例混合,4℃过夜。采用非还原性SDS-PAGE电泳进行分离,将分子量大于29万的双特异性抗体进行分离,调整双特异性抗体浓度为0.5mg/mL备用。
将双特异性抗体以100ng/1x106个/实施例2分化制备的EPCs/mL混合培养,在mTeSRTM培养基中共培养,培养条件是37℃,5%CO2饱和湿度的培养箱中培养8天,每2天半量换液一次。
SD大鼠心肌梗死模型的建立:20%乌拉坦(300mg/kg)联合1%的戊巴比妥钠0.05g/kg腹腔注射麻醉,左前胸备皮3×3cm2,1%活力碘消毒皮肤2次。记录标准I、avL导联心电图。采用气管插管引导灯,经口气管插管,接小动物呼吸机,频率90次/分,保证有效通气量为1.0ml/g.min。左胸前第3、4肋间做横切口,依次分离切断各层组织,避免损伤肋骨和胸廓内动脉,暴露心脏,剪开心包。在左心耳的右缘下1-2mm处、肺动脉圆锥左缘用圆针穿过左冠状动脉前降支(LAD)下方的心肌表层,用6-0丝线结扎。结扎成功后,观察到结扎血管以下心室壁变白,左室前壁局部心肌运动明显减弱,及心电图I、avL导联ST-T段弓背向上抬高确定心肌梗死形成。用盐水纱布覆盖伤口观察1小时,对建立心梗成功的大鼠依分组,分别接受不同的处理。用0号丝线将3、4肋间对位闭合胸腔,确定无开放性气胸后依次对位缝合胸壁各层组织。1%活力碘消毒伤口2次。待大鼠恢复自主呼吸后拔除气管导管,移至37℃温室,复苏2小时后笼养与清洁动物房。
建模三天后的大鼠进行内皮祖细胞治疗。将制备成功的模型大鼠随机分为5组(每组移植剂量均为1ml):(1)对照组,经尾静脉输入0.01mM PBS 液1 ml;(2)单纯双特异性抗体治疗组,经尾静脉输入双特异性抗体100ng 1ml;(3)单纯EPCs移植组,经尾静脉输入实施例2采用单抗诱导分化的EPCs 1ml (浓度为1x106/ml);(4)单抗诱导EPCs+双抗组,经尾静脉输入装配双抗的采用实施例2单抗诱导的EPCs 1ml(浓度为100ng双抗/1x106个EPCs );(5)对照EPCs+双抗组,经尾静脉输入装配双抗的采用实施例2未采用单抗诱导的EPCs 1ml(浓度为100ng双抗/1x106个EPCs )。实验过程中对动物的处置均符合动物伦理学标准。
细胞移植40天后,采用M型超声心动图观察各组大鼠的声像图表现,计算左室射血分数EF和左室短轴缩短率 FS以评价心功能,结果如表1所示。

从表1可以看出,采用双抗处理的EPCs组的EF和FS较没有处理的组,显著增加,这充分说明,采用双抗协同EPCs组能够有效的用于心肌梗死的治疗。从表1的结果也可以看出,采用本发明G-3D8单抗诱导的EPCs比没有采用G-3D8单抗单抗诱导的EPCs具有更好的治疗效果,这也说明,采用G-3D8单抗诱导的EPCs细胞具有更强的细胞活性。
做完心动图后,处死大鼠,取出心脏,找出手术建立模型时大鼠左冠状动脉前降支结扎点,将结扎点水平以下的心肌沿心脏横轴方向切下,收集心肌梗死区边缘组织,4%多聚甲醛固定,石蜡包埋切片,采用免疫组织化学法行VIII 因子检测,显微镜下每张切片癫痕边缘区任取10个视野,计算 VIII 因子阳性的毛细血管平均数作为新生血管数,结果如图1所示。
从图1的结果可以看出,采用双抗处理的EPCs组的微血管密度相比于单纯采用双特异性抗体治疗组效果显著增加(P<0.05),这充分说明,采用双抗协同EPCs组能够有效的用于心肌梗死的修复。从图1的结果也可以看出,采用本发明G-3D8单抗诱导的EPCs联合双抗治疗后其血管密度达到了(78.2±3.1)个,比没有采用G-3D8单抗单抗诱导的EPCs联合双抗具有更好的促进微血管密度提高的治疗效果。这也说明,采用G-3D8单抗诱导的EPCs细胞具有更强的细胞活性,血管修复与再生作用增强。
Western blot检测各组VEGF蛋白的表达称取新鲜心肌组织100mg,加1ml含蛋白酶抑制剂的总蛋白抽提试剂,匀浆后抽提总蛋白。用考马斯亮蓝法测定蛋白浓度,SDS-PAGE电泳分离蛋白,转膜,VEGF抗体孵育,胶片显影分析。结果如图2所示。
从图2可以看出,以β-actin为内参,采用Quantity One软件定量分析各组VEGF蛋白的表达情况显示单抗诱导EPCs+双抗组的蛋白水平显著高于单纯单抗或者单纯EPCs治疗组(P<0.05),采用本发明G-3D8单抗诱导的EPCs比没有采用G-3D8单抗单抗诱导的EPCs具有更好的促进VEGF表达的效果。
基于以上实验结果显示采用单抗诱导EPCs+双抗组能够诱导VIII因子表达增加,心肌VEGF蛋白表达上调,左室射血分数及左室短轴缩短率明显提高,说明了EPCs归巢率增加,EPCs归巢后能使梗死心肌处毛细血管密度明显增加,心功能改善,更有效的促进血管修复与新生,具有较好的治疗效果。

Claims (4)
1.一种用于治疗心脑血管疾病的药物组合,其特征在于由iPS诱导定向分化成的内皮祖细胞和双特异性抗体组成;其中IPS诱导定向分化成的内皮组细胞是采用如下方法制备获得:将皮肤成纤维细胞调整细胞数为1×106个,100μl电转缓冲液中加入 3μl Epi5TMEpisomal iPSC 重编程试剂盒中的重编程因子,采用常规电转染细胞按2ml/孔接种于基质胶包被的6孔板中,置于培养箱中培养;并使用ReproTeSRTM重编程培养基隔天换液,转染后第8天,将细胞消化后重新接种在第5代小鼠成纤维细胞制作的饲养层上,培养18天,观察iPSCs可见典型的克隆,将ips克隆重新接种于基质胶包被的6孔板中,在mTeSRTM培养基中维持培养,以80-90%的密度在基质胶包覆的六孔板种板,mTeSRTM培养基中加入10μM Y27632,种板后24hr开始诱导分化,Day0在培养液中加入含有6μM 单克隆抗体G-3D8和60mg/ml抗坏血酸的DMEM/F12培养基;分化第2天抽吸培养基,加入60mg/ml抗坏血酸DMEM/F12培养基,然后每天更换DMEM/F12培养基来维持细胞,在分化第5-8天加入重组蛋白GREM1,GREM1的加入浓度为0.25μg/ml,分化后第9天即可收获相应的内皮祖细胞;双特异性抗体是抗CD34抗体和抗AMLCA抗体通过化学交联制备而成;所述的单克隆抗体G-3D8其重链可变区序列如SEQID NO:1所示,轻链可变区序列如SEQ ID NO:2所示。

3.如权利要求2所述的用途,其中所述的IPS诱导定向分化成的内皮祖细胞的移植量为1×106个。
