迫切需要新的诊断工具,理想的非侵入性工具以提高PCa诊断并降低不必要的活体组织检查和过度治疗。来自容易获取的样品类型例如血液的更准确的诊断将使得医师和患者对于潜在的PCa病例以及是否需要前列腺活体组织检查做出更明智的决策。
与诊断类似,PCa的治疗和/或预后由于该疾病的异质性而仍然是一大挑战。尽管已经提出了PCa的多种机制,但是缺乏能够对患者进行分层的合适特征并且用于治疗性干预治愈的关键靶蛋白仍然不可获得。
在WO 2009/138392中提出了一种发现用于前列腺癌的合适诊断系统的方法,其中提出测量已知存在于人血液中的24种蛋白质的列表中的至少两种,并且预期其根据对应患者的健康状态而被下调或上调。
已知方法的问题在于其仍然缺乏关于癌症实际存在的灵敏度且特别是特异性,并且缺乏关于避免假阳性结果和假阴性结果的诊断可靠性。另一个问题是相应检测探针的实际可用性,无论是基于抗体的检测还是任何其他类型的检测,使得相应的工具不仅适用于学术目的而且还适用于广泛应用。另一个问题是相应的检测系统应简单并且不需要大量的单独测量。
WO-A-2018011212提出了用于收集关于对象健康状态之信息的方法,其包括在对象的血清、血浆或血液中定量检测THBS1的浓度、游离PSA的比例(%fPSA),优选包括选自CTSD、OLFM4、ICAM1的至少一种蛋白质的浓度。
PCa可通过根治性治疗例如根治性前列腺切除术(Radical Prostatectomy,RP)来管理,其提供了对局部性PCa的极好的癌症控制。约30%的手术治疗的男性将经历生化复发(biochemical recurrence,BCR),从而处于临床癌症进展(转移)的显著风险中,并且需要制定全身性治疗。
已表明临床分期、治疗前PSA水平和前列腺活体组织检查Gleason分级是治疗无效的可靠且独立的预测因子。临床风险概况(治疗前列线图(例如Kattan或CAPRA评分)被设计成鉴定可安全避免侵袭性治疗的患者或选择用于新辅助临床试验的潜在候选者。
然而,当前模型的有用性取决于其预测的准确性。在PSA定期用于筛查的群体中,术前PSA水平可主要反映良性前列腺增生(BPH)而不是PCa的存在。在活体组织检查之后,预测PCa的侵袭性是困难的,即使在其算法中使用了并入临床信息的列线图也是如此。这尤其适用于疾病进展风险低和极低的患者,对于所述患者主动监测正成为广泛采用的策略。
US-A-2020292548公开了用于在对象中诊断前列腺癌生化复发(BCR)的存在的方法,这样的方法包括检测多种诊断BCR的生物标志物的水平。还提供了用于检测本发明生物标志物的以试剂盒和试剂组形式的组合物。
Oguic et al在Patholog Res Int.2014;2014:26219中评价了基质金属蛋白酶2(matrix metalloproteinase 2,MMP-2)和基质金属蛋白酶9(matrix metalloproteinase9,MMP-9)在前列腺癌中在主要肿瘤块和肿瘤细胞中在阳性切缘处的表达以及这些生物标志物对前列腺切除术患者中疾病的生化复发的影响。对120例存档的前列腺癌样品的组织微阵列进行针对MMP-2和MMP-9表达的免疫组织化学评价,并将其与临床病理参数进行比较。与切缘阴性的肿瘤相比,手术切缘阳性的肿瘤显示出显著更高的MMP-9总体表达(P=0.0121)。在来自具有生化复发的患者的肿瘤中,MMP-9表达显著升高(P=0.0207)。在切缘阴性的患者组中,高于截止值的MMP-9表达与复发显著相关(P=0.0065)。多变量分析表明MMP-9是生化复发的良好预测因子(优势比=10.29;P=0.0052)。MMP-2在肿瘤细胞中的表达在阳性切缘处显著高于在主要肿瘤块中(P=0.0301)。该结果强调了MMP-2和MMP-9表达对于预测前列腺切除术之后具有手术切缘阳性和阴性的前列腺肿瘤行为的潜在价值。
Miyata et al在Prostate.2015Jan;75(1):84-91中报道了进行新辅助激素治疗(neoadjuvant hormonal therapy,NHT)以改善器官局部性前列腺癌的结局。然而,关于血管生成与NHT之间的关系的信息很少。该研究的目的是确定合适的方法来评价组织的血管生成状态,并确定该方法对NHT之后经受了根治性前列腺切除术的患者中的生化复发的预后价值。他们分析了来自通过根治性前列腺切除术治疗的患者的108个经福尔马林固定的标本。在48例患者(52.9%)中施用NHT,并选择60例具有相似的Gleason评分和pT分期的患者作为非NHT治疗的对照组。使用抗CD31、抗CD34和抗CD105抗体来测量微血管密度(microvessel density,MVD)。还通过免疫组织化学来评价血管内皮生长因子(VEGF)-A和血小板应答蛋白(TSP)-1的表达。研究CD31-MVD、CD34-MVD和CD105-MVD对生化复发的预后价值。报道了NHT组中CD105-MVD的平均值/SD(13.3/4.7)显著(P<0.001)低于非NHT组中CD105-MVD的平均值/SD(125.8/7.3)。在NHT组中,CD105-MVD与pT分期相关,并且其与VEGF-A表达呈正相关(r=0.56,P<0.001)且与TSP-1表达呈负相关(r=0.42,P=0.003)。CD105-MVD被确定为是用NHT治疗的患者中生化复发(BCR)的重要预测因子(对数秩检验,P<0.001)。尽管CD31-MVD和CD34-MVD在非NHT组中与pT分期或Gleason评分显著相关,但其在NHT组中与病理特征和BCR不相关。其结果表明CD105-MVD反映了用NHT治疗的前列腺癌组织中的血管生成状况。CD105-MVD也被确定为是采用NHT经受根治性前列腺切除术的前列腺癌患者中生化复发的重要且独立的预测因子。
Coulsen-Thomas et al在Experimental Cell Research 319(7)中报道了肿瘤周围的间质反应导致肿瘤特异性微环境的形成,其可在肿瘤的生长和进展中发挥限制性作用或支持性作用。光蛋白聚糖,即胞外基质(extracellular matrix,ECM)的富含亮氨酸的小蛋白聚糖(small leucine-rich proteoglycan,SLRP),调节胶原原纤维生成。最近,还表明光蛋白聚糖在胚胎发育、组织修复和肿瘤进展期间调节细胞行为。光蛋白聚糖在癌症中的作用根据肿瘤的类型变化。在该研究中他们在体内和体外二者中分析了光蛋白聚糖在前列腺癌发病机制中的作用。在通过实时PCR和免疫染色二者分析的原发性肿瘤中观察到光蛋白聚糖的总体上调。在泡状腺周围具有纤维沉积的前列腺原发性肿瘤周围的反应性基质中观察到光蛋白聚糖表达的提高。体外分析表明光蛋白聚糖抑制了从淋巴结、骨和脑中分离的转移性前列腺癌细胞的迁移和侵袭二者。此外,接种在光蛋白聚糖上的前列腺癌细胞表现出细胞突起的形成降低、板状伪足的形成降低(通过ZO-1、角蛋白8/18、整合素β1和MT1-MMP的降低重排而检出)和侵袭性伪足的形成降低(通过α-平滑肌肌动蛋白、皮动蛋白和N-WASP的破坏而检出)。此外,通过光蛋白聚糖敲除小鼠的腹膜观察到前列腺癌细胞侵袭的显著提高,进一步表明了存在于ECM中的光蛋白聚糖对前列腺癌侵袭的限制性作用。总之,存在于前列腺原发性肿瘤周围的反应性基质中的光蛋白聚糖在癌症进展中发挥限制性作用,并因此我们假设光蛋白聚糖可以是前列腺癌分期中有价值的标志物。
Behnsawy et al在BJU Int.2013Jan;111(1):30-37中报道了旨在分析在局部性前列腺癌(prostate cancer,PC)中上皮-间充质转化(epithelial-mesenchymaltransition,EMT)中所涉及的多种分子标志物的表达模式,以阐明这些标志物在经历根治性前列腺切除术(RP)的患者中的意义。通过免疫组织化学染色来评价来自197例连续的具有局部性PC的患者的RP标本中13种EMT标志物的表达水平,所述标志物即E-钙黏着蛋白、N-钙黏着蛋白、β-联蛋白、γ-联蛋白、纤连蛋白、基质金属蛋白酶(MMP)2、MMP-9、Slug、Snail、Twist、波形蛋白、ZEB1和ZEB2。在13种标志物中,E-钙黏着蛋白、Snail、Twist和波形蛋白的表达水平与多种常规预后因子密切相关。单变量分析确定这四种EMT标志物作为生化复发(BR)的重要预测因子,而血清前列腺特异性抗原、Gleason评分、精囊侵犯(seminalvesicle invasion,SVI)、手术切缘状态(surgical margin status,SMS)和肿瘤体积也是重要的。在这些重要因子中,Twist和波形蛋白、SVI和SMS的表达水平在多变量分析中显示出与BR独立相关。根据这四种独立因子的阳性数目,无BR存活存在显著差异。即在90例针对风险因子呈阴性的患者中有4例发生BR(4.4%),在83例针对一种或两种风险因子呈阳性的患者中有21例发生BR(25.3%),以及在24例针对三种或四种风险因子呈阳性的患者中有19例发生BR(79.2%)。除了常规预后参数之外,表明了在RP标本中测量潜在的EMT标志物,特别是Twist和波形蛋白的表达水平,将有助于准确预测RP之后具有局部性PC的患者中的生化结局。
Bair et al.在Prostate.2006Feb 15;66(3):283-93中报道了90K/Mac-2结合蛋白是表达水平与许多不同肿瘤类型中的转移潜力相关的细胞黏附蛋白。该研究的目的是检查前列腺癌中的90K表达并确定90K在癌症进展中的可能作用。通过免疫组织化学来评价前列腺细胞系和组织样品中的90K表达。还通过Western印迹分析和实时RT-PCR来评价在细胞系中的表达。通过ELISA来评价90K对重组基质金属蛋白酶前体(promatrilysin)的诱导。一些所研究的人前列腺细胞系表达90K。90K在38.8%的前列腺癌肿瘤样品、7.14%的PIN病变和18.6%的正常组织中过表达。还表明90K在前列腺细胞系LNCaP中诱导重组基质金属蛋白酶前体表达。这些数据表明90K在大部分恶性肿瘤中过表达。90K可诱导重组基质金属蛋白酶前体表达的事实表明90K在癌症进展和转移中的可能作用。这表明90K过表达可以是用于检查前列腺癌进展的有用标志物。
WO-A-2018011212公开了,提出的用于收集关于对象健康状态之信息的方法,其包括在对象的血清、血浆或血液中定量检测THBS1的浓度、游离PSA的比例(%fPSA),优选包括选自CTSD、OLFM4、ICAM1的至少一种蛋白质的浓度。
发明内容
因此,迫切需要鉴定与生物学侵袭性PCa的存在特异性相关的新的标志物以用于具有PSA水平中度升高的群体中的结局的改进的预测。由于目前主要基于总PSA水平和标准病理肿瘤分级的模型的这些局限性,因此我们研究了替代的PCa相关生物标志物及其与具有临床局部性PCa的患者中的BCR和不良病理状况(adverse pathology,AP)的相关性。
我们使用小鼠模型和蛋白质组学技术对最初在PTEN突变背景下发现的多种蛋白质生物标志物进行了单变量和多变量分析。与前列腺癌风险评估(Cancer of theProstate Risk Assessment,CAPRA)评分相比,评价了多种生物标志物与临床分级组(grade group,GG)和PSA一起组合的临床表现以用于预测RP之后的BCR。另外,还研究了与AP的相关性。
因此,目的是确定新的生物标志物组合在经受根治性前列腺切除术(RP)的PCa患者中的预后效用。
在Martini Clinic(Hamburg,Germany)的PCa临床分期(clinical stage,cT)<3的经受RP的557名男性的血清样品和临床数据用于分析。使用活体组织检查样品来确定GG,同时使用免疫测定来测量血清中的肿瘤标志物浓度。使用Cox比例风险回归和Kaplan-Meier分析来评估所提出的标志物组合的预后效用。将该表现与总组群和低风险患者亚群中的CAPRA评分进行比较。
建立了包含纤连蛋白1(fibronectin 1,FN1)、半乳凝素-3结合蛋白(galectin-3-binding protein,LG3BP)、光蛋白聚糖(lumican,LUM)、基质金属蛋白酶9(matrixmetalloprotease 9,MMP9)、血小板应答蛋白-1(thrombospondin-1,THBS1)和PSA与GG一起的多变量模型。所提出模型是BCR的重要预测因子(HR 1.28/5单位评分,95%CI 1.19至1.38,p<0.001)。Kaplan-Meier分析表明,与CAPRA评分相比,所提出模型对在RP之后的低风险疾病具有更好的预测(分别为4.9%vs.9.1%的BCR可能性)。在预限定的低风险群体亚群中,使用所提出模型的BCR风险低于5.5%,并因此当与CAPRA评分=0至2(9%)、GG<2(7%)和NCCN=低风险(6%)亚群相比时更低。另外,所提出模型可显著(p<0.001)区分在RP时具有AP事件的患者与不具有AP事件的患者。
因此,所提出模型在总组群以及在预限定的低风险患者群体亚群中针对RP之后BCR的预测出乎意料且显著地优于CAPRA评分。其也与在RP时的AP显著相关。
更一般而言,本发明涉及如所附权利要求书中所述的方法。
所要求和描述的是用于收集关于对象健康状态之信息的方法,其包括在对象的血清、血浆或血液中定量检测选自以下的系统(system)中至少四种的浓度:THBS1、LUM、FN1、LG3BP、MMP9以及(总)PSA。

优选具有可见读出的针对相应蛋白质具有特异性的夹心酶联免疫吸附测定和/或优选具有荧光读出的针对相应蛋白质的基于珠的夹心抗体测定可以是通过分别使用人THBS1、LUM、FN1、LG3BP、MMP9的重组蛋白质以及通过用其免疫接种动物产生的小鼠单克隆抗体获得的测定。
对所述各浓度的定量检测可包括相对于外部蛋白质标准品来确定这样的生物标志物的浓度,其包括通过测量数个蛋白质标准品、优选5至7个蛋白质标准品的确定浓度来制作参考标准曲线,所述蛋白质标准品用在对样品进行的同一组测量中测定的蛋白质稀释液相同的缓冲剂稀释。

具体实施方式
研究群体:
回顾性组群包括557名具有局部性PCa的男性。所有对象均在Martini Clinic(Hamburg,Germany)经受了RP且临床分期为cT<3,进行或不进行分期淋巴结切除术。所有的血液样品均在RP之前、在任何前列腺操作(DRE,TRUS引导的活体组织检查)之后八周或更多周抽取,并立即处理和冷冻。没有一个患者经受过任何另外的治疗。
主要结局是RP之后BCR,限定为任何术后PSA>0.2ng/ml。在5年随访时对患者进行删减(censor)。次要结局为RP时的AP,限定为病理GG3或更高,病理分期为pT3a或更高,或者阳性病理结节(pN1)。
测定方法:
CE-IVD免疫测定用于CTSD和THBS1的定量(Proteomedix,Proclarix测定)。根据制造商说明进行测定。所有其他免疫测定均为非IVD免疫测定并且由来自R&D Systems的市售组分(ATRN、ECM1、LG3BP、LRG1、LUM、MMP9、NCAM1、TIMP1、VEGF、ZAG)或Proteomedix专有的试剂(CFH、FN1、HYOU1、ICAM1、OLFM4、POSTN、VTN)构成。使用的形式是ELISA(CTSD、THBS1、CFH、FN1、VTN、POSTN)或Luminex(所有其他标志物)。专有重组蛋白质(HYOU1、ICAM1、OLFM4)和市售重组蛋白质(所有其他标志物)用作免疫测定校准的参考。
统计学方法:
所提出的用于具有BCR的患者预后的生物标志物模型开发如下:对于所有20种标志物,创建了关于BCR的单变量Cox比例风险(Cox proportional hazard,CoxPH)和关于AP的一般线性模型(General Linear Model,GLM)。针对BCR和AP的在相同方向(向上或向下)调节的标志物被保留用于进一步的模型建立。然后使用关于BCR的CoxPH和关于AP的GLM来应用Step Akaike信息标准(Step Akaike Information Criteria,StepAIC)选择。最后,多变量CoxPH模型用于创建新提出模型的算法。使用Schoenfeld方法来评估CoxPH模型的拟合优度(goodness-of-fit)。该测试的非显著性结果表明没有偏离比例风险假设,因此所提出的CoxPH模型将是稳健的。
包含FN1、LG3BP、LUM、MMP9、THBS1和PSA与GG一起的最佳CoxPH模型被选为新提出的模型。
组合的生物标志物模型值优选地使用下式计算:

对于在AP情况下90%的灵敏度,优选选择36,优选30至42的组合评分值阈值。
通过使用Kaplan-Meier时间到事件(time-to-event)方法来评估所提出模型关于BCR的预后效用。将所提出模型的结果与NCCN标准[15]或CAPRA评分[8]进行比较。对于在RP时AP的区分能力,双侧t检验p<0.05被认为是统计学上显著的。所有统计学分析均使用R统计程序包4.0.2版本和GraphPad PRISM 6.0版本进行。
结果
活体组织检查结局
患者特征在表1中示出。在本研究中包括的557名男性中,中位(最小-最大)年龄为65岁(44至78岁)。基于NCCN标准(87%的群体)或CAPRA评分(89%),绝大多数患者具有低至中等风险的PCa。在557名患者中,31%在RP时显示出AP事件。14%的患者在5年内具有BCR。对于没有BCR的患者,中位随访时间是7.0年(IQR 5.0,7.4)。

所提出模型的建立

风险比(Hazard Ratio,HR)和优势比(Odd Ratio,OR)比较排除了年龄、ATRN、OLFM4、POSTN和TIMP1用于进一步模型建立。逐步选择应用于关于BCR的CoxPH和关于AP的GLM,产生了针对BCR的9-plex模型(GG、PSA、ECM1、FN1、LG3BP、LUM、MMP9、THBS1和VTN)和针对AP的5-plex模型(GG;前列腺体积、PSA、LG3BP和LUM)。在这10个不同的变量中,针对区分BCR的低风险vs.中等风险和高风险,对将5至7个变量组合的20个不同的多变量CoxPH模型的性能进行测试。在5年之后,BCR的可接受的低风险分数被设定为低于5%。最后,选择包含FN1、LG3BP、LUM、MMP9、THBS1和PSA与GG一起的最佳CoxPH模型作为新提出的模型。
针对关于BCR的CoxPH所提出模型的多变量分析在表2B中示出。
所提出模型与BCR显著相关(HR 1.28/5单位评分,95%CI 1.19至1.38,p<0.001)。将PSA添加至GG以及在第二步骤中将5种血清标志物添加至GG+PSA提高了BCR的预测,通过将c指数分别提高0.051和0.039。用于测试CoxPH模型的拟合优度的Schoenfeld方法表明在此时观察到的协变量与预期的给定风险集之间没有差异。对于每个协变量(p>0.07)和对于所提出模型(p=0.76,补充数据),测试在统计学上是不显著的。因此,我们可以假设没有偏离比例风险假设。
关于BCR预测的Kaplan-Meier分析
免于BCR的Kaplan-Meier分析在图1中示出。确定所提出模型的阈值,以便对无BCR(<37.8)、低风险(<47.3)、中等风险(47.3至71.1)和高风险(>71.1)的BCR的群体进行分层。对于所提出模型,在5年之后低风险BCR的限定设定为低于5%,而对于高风险BCR的限定设定为高于40%。
作为结果,总组群的Kaplan-Meier分析表明与CAPRA相比,所提出模型对RP之后的低风险BCR具有良好的预测(对于n=194和n=210的患者,BCR的可能性分别为4.9%vs.9.1%)。这些结果表明所提出模型对于区分具有低风险BCR的患者的优异的能力。当通过选择CAPRA<2(n=231),GG<2(n=257)或NCCN=低风险(n=200)的患者,在具有预限定低风险BCR的组群中应用所提出模型时,这些调查结果是类似的。

在此,使用所提出模型的低风险截止值(<37.8)的BCR风险在所有三个亚组中均低于5.5%(n>138名患者)并因此当与CAPRA=0至2(9%)、GG<2(7%)和NCCN=低风险(6%)亚群相比时更低。
不良病理状况的区分:
当应用阈值<36时,所提出模型与在RP时的AP(p<0.001;图2)以及与三个单一AP事件(p<0.001对于GG>2、pT>2和pN1;补充数据)显著相关。对于预测AP的临床性能是不优异的,但仅等同于CAPRA(补充数据):当将阈值CAPRA<2和截止值<36应用于所提出模型时,两个模型之间的灵敏度和特异性结果是没有显著不同(当比较灵敏度时p值为0.090,并且当比较特异性时p值为0.159)。
讨论:
评估PCa预后的能力对于管理经受RP的男性是至关重要的。由多种以下与PCa进展相关的不良结局增强了预测PCa的难度:BCR、AP、转移或死亡。理想的预后模型将需要涵盖所有这些方面以便帮助制定用于可能的术后治疗的决策。目前临床实践中的风险分层仍然是相当差的。已基于病理结局开发了多种免费列线图(即CAPRA、d’Amico评分)。商业上可用的测试例如CCP评分,即31个细胞周期进展基因的基于组织的基因组测试,或GPS评分,即基于17个基因的RNA表达的测试也可对PCa进展的风险进行分层,正如在针对CAPRA、CCP或GPS的多项研究中所示出的。然而,很难确定一个逻辑阈值,采用该逻辑阈值来指导不同组群中的治疗仍然是具有挑战性的。
在本研究中,我们评价了新提出的模型用于评估RP之后BCR以及RP时AP的预后可用性。将所提出模型的性能与CAPRA进行比较。来自本研究群体的所有患者(n=557)的临床分期均低于3。
正如预期的那样,CAPRA对BCR的预后能力在组群中是有限的。大量低风险患者(CAPRA 0至2,9.1% BCR,n=210)和极少量不具有BCR的患者(CAPRA=0,n=2)使其对于患者的安全治疗指导的应用受到限制。
在此,我们首先使用小鼠模型开发了具有最初在PTEN突变背景下发现的蛋白质生物标志物的模型。多变量模型将THBS1、LUM、FN1、MMP9、LG3BP与PSA和临床GG一起组合。尽管在单变量分析中并非所有标志物均与BCR或AP显著相关,但所提出模型可显著(p<0.001)区分在RP时具有AP事件的患者并且是BCR的重要预测因子(HR 1.28/5单位评分,95%CI 1.19至1.38,p<0.001)。这些调查结果是以c指数分析支持的,当将四种生物标志物添加至PSA和GG时,c指数提高。
与CAPRA相比,所提出模型显示出对RP之后BCR的优异预测。其可预测12.6%的群体无BCR风险,其中CAPRA预测低于0.1%,CAPRA=0。在应用低于37.8的低风险阈值(n=194)时,其还可预测4.9%的复发,相比之下低风险CAPRA=0至2(n=210)则为9.1%。低于5%的风险可被认为是相当低的,使患者处于可观的RP之后BCR风险。
在限定为CAPRA=0至2,NCCN=低和GG<2的不同低风险患者群体中,所提出模型具有低于5.3%的BCR风险,再次略优于CAPRA评分(9%的BCR风险),NCCN(6%)和GG(7%)。
仅14%的患者在5年内具有BCR。这是由于选择标准排除了经受新辅助和辅助治疗的患者以及选择cT<3的患者。然而,用于本研究的组群可以被认为是低风险患者群体的代表,其中风险分层仍然是尤其具有挑战性的。该组群与其他研究中使用的组群相当,还评估了关于RP之后BCR风险的多种模型[23]。
本研究具有一些应该注意的限制性。主要限制性是所提出模型是针对单一回顾性组群训练的,受限于一个单一中心,其中主要是高加索人男性(Caucasian men)。因此将该模型推广到更多样化的群体是受限的。另外,另一个限制性是缺乏对模型的适当验证。即使使用Schoenfeld方法评估了CoxPH模型的拟合优度,但所提出模型及其所选择阈值的性能在应用于另一个独立的组群时也无法推测。最后,我们可表明所提出模型仅与BCR和AP显著相关。在该组群中,无法评估与其他相关预后终点(即死亡或转移)的相关性。
总之,所提出模型改善了经受前列腺切除术的男性的BCR风险和AP的临床分层。该模型可潜在更好地指导治疗选择,但验证研究应在独立的组群中进行以便验证该模型。
Claims (11)
1.用于收集关于对象健康状态之信息的方法,其包括在所述对象的血清、血浆或血液中定量检测选自以下的系统中至少四种的浓度:THBS1、LUM、FN1、LG3BP、MMP9以及PSA。
2.根据权利要求1所述的方法,其包括在所述对象的血清、血浆或血液中定量检测THBS1、LUM、FN1、LG3BP、MMP9以及PSA中每一种的浓度,
其中优选地还将至少一个先前活体组织检查的Gleason分级(GG)考虑在内,其表示为1至5的整数。
3.根据权利要求1或2所述的方法,其中所述方法包括:
如下进行的第一步骤:使所述对象的血清、血浆或血液、优选地在其稀释之后与每种蛋白质的至少一种亲和试剂接触、优选与每种蛋白质的两种亲和试剂接触,并检测各蛋白质与所述至少一种亲和试剂之间是否发生结合,并使用各蛋白质之浓度的定量读出,从而允许计算在原始血清、血浆或血液中各自的浓度;
第二步骤:基于在第一步骤中确定的所有所述蛋白质浓度以及PSA浓度来计算组合评分值。
4.根据权利要求3所述的方法,其中在第二步骤之后,在第三步骤中,基于在第二步骤中确定的所述组合评分值来确定所述对象的不良病理状况(AP)和/或者前列腺癌手术之后生化复发(BCR)的风险,其中超过相应组合评分值阈值被视为手术之后阳性前列腺癌生化复发和/或视为需要前列腺切除术。
5.根据前述权利要求3或4中任一项所述的方法,其中所述组合评分值基于所测量的浓度XtPSA、XMMP9、XLG3BP、XTHBS1、XFN1、XLUM和表示为1至5之整数的至少一个先前活体组织检查的Gleason分级(GG),使用下式以及β0、βtPSA、βGG、βMMP9、βLG3BP、βTHBS1、βFN1、βLUM来计算:

7.根据权利要求5或6所述的方法,其中:
对于低BCR可能性,选择低于50或低于47.3,优选40.4至54.1的组合评分值阈值,
对于中等BCR可能性,选择50至75或47.3至71.1,优选40.4至79.5的组合评分值的值,
对于高BCR可能性,选择高于75或高于71.1,优选62.6至79.5的组合评分值阈值。
8.根据权利要求5至7中任一项所述的方法,其中对于在AP情况下的90%灵敏度,选择36,优选30至42的组合评分值阈值。

10.根据权利要求9所述的方法,其中所述优选具有可见读出的针对相应蛋白质具有特异性的夹心酶联免疫吸附测定和/或所述优选具有荧光读出的针对相应蛋白质的基于珠的夹心抗体测定是通过分别使用人THBS1、LUM、FN1、LG3BP、MMP9的重组蛋白质以及通过用其免疫接种小鼠产生的动物单克隆抗体而获得的测定。
11.根据前述权利要求中任一项所述的方法,其中对各浓度的定量检测包括相对于外部蛋白质标准品来确定这样的生物标志物的浓度,其包括通过测量数个蛋白质标准品、优选5至7个蛋白质标准品的确定浓度来制作参考标准曲线,所述蛋白质标准品用在对样品进行的同一组测量中测定的蛋白质稀释液相同的缓冲剂稀释。
