Description

Semi-automatic purse-string suture device
Technical Field
The invention belongs to the technical fields of treatment, first aid and medical treatment, and particularly relates to a semiautomatic purse-string suture device.
Background
The purse-string suture device is a medical instrument which is frequently used when performing surgery on human tissues such as intestinal tracts, soft tissues or skin surgical incisions, and has important functions in improving the working efficiency of surgeons and shortening the surgery time. In the prior art, there are two methods for performing purse string suture operations: one is manual stitching, which is time-consuming and labor-consuming and is easy to operate by mistake; another is the use of an automatic purse-string stapler. However, the existing automatic purse-string suture device is mainly used for suturing intestinal and skin incisions, and the structure of the existing automatic purse-string suture device mainly comprises a handle clamping seat and a nail bin assembly, wherein the two handle clamping seats are crossed to form a scissor-shaped structure or form a gun-shaped structure back and forth, and nails, nail pushing sheets and purse-string wires are arranged in the nail bin assembly; after the handle is clamped, nails in the nail bin are shot out, the purse string is fixed on the outer wall of the tissue, and then the purse string can be tightened to bind the purse. Chinese patent CN101756724a describes an annular pouch stapler, which comprises an annular staple cartridge, the annular staple cartridge has an outer wall and an inner wall, a plurality of staple holes are formed from the outer wall to the inner wall in a penetrating manner, each staple hole is internally provided with a staple pushing sheet and a staple, staple legs of the staple are provided with a pouch wire, the staple pushing sheet can move relative to the annular staple cartridge along the direction from the outer wall to the inner wall, and the staple can be pushed out of the annular staple cartridge and is pricked into tissues.
In the field of peritoneal dialysis catheterization, it is also necessary to suture the peritoneal tissue with a purse-string, but the above-described existing purse-string suture device is not directly applicable to purse-string suturing of the peritoneal tissue. Firstly, when the existing purse-string suture device is used for suturing the purse-string, the clamp seat of the purse-string suture device is required to be opened, the front end of the clamp seat is required to be opened sufficiently so that tissues can enter a suture zone from a notch on one side of the clamp seat without obstruction, the operation zone is required to be sufficiently large so that the opened purse-string suture device can enter the operation zone without obstruction, and therefore the clamp seat is only suitable for suturing esophagus, intestinal tracts or thicker soft tissues, and the space of peritoneal tissues is not large enough to provide enough operation zone for suturing; secondly, the peritoneal tissue is a layer of soft tissue which is thin, and the peritoneal membrane is very easy to tear when being fixed by nails; in addition, in the purse string suture process of the peritoneal tissue, the peritoneal dialysis catheter is placed in the center of the peritoneum tissue of the purse string, and the peritoneum tissue wall can be tightly attached to the peritoneal dialysis catheter by using the purse string, but if the purse string suture device in the prior art is used for suture, the purse string is not easy to pack the peritoneal dialysis catheter due to the existence of a sharp nail, and the postoperative peritoneal dialysis fluid leakage is easy to cause. Therefore, in order to simplify the procedure of purse string suture in the peritoneal operation, shorten the time for surgical purse string suture, improve the operation efficiency, and simultaneously avoid the damage to the peritoneal tissue, a novel purse string suture device needs to be developed so as to facilitate the purse string suture in the peritoneal dialysis tube placement operation field.
Disclosure of Invention
The invention aims to overcome the defects and the shortcomings in the prior art and provide a semi-automatic purse-string suture device.
A second object of the present invention is to provide the use of the semi-automatic purse-string suture device.
The above object of the present invention is achieved by the following technical solutions:
aiming at the defects in the prior art, the invention designs a semiautomatic purse-string suture device which sequentially comprises a chassis, a cutting sleeve and a suture disc from bottom to top. The chassis is round, the edge of the upper surface of the chassis is provided with an annular bulge, and the bulge is provided with a groove in the centrifugal direction of the chassis; the clamping sleeve is a disc, and an annular gap matched with the bulge is formed in the edge of the clamping sleeve; the disk body of the sewing disk is round with the same size as the chassis, the edge of the disk body is provided with an inner wall extending downwards, an annular needle matched with the groove is detachably fixed in the wall, and the tail part of the annular needle far away from the needle point is connected with a charging envelope wire. The chassis, the clamping sleeve and the suture discs are sequentially combined from bottom to top, the clamping sleeve is connected with the protrusions one by one through the gaps, and the suture discs are sleeved on the clamping sleeve.
Further, the chassis is sized to match the surgical area of tissue to be sutured. For example, the chassis has a diameter of 4 to 5cm; the size of the sewing disc is consistent with that of the chassis; the diameter of the cutting sleeve is slightly smaller than that of the chassis, and is equal to the edge of the convex structure on the chassis, and the diameter is 3.5-4.5 cm.
When the semiautomatic purse string suture device is used for suture, tissues to be sutured are placed on the chassis, the cutting sleeve is pressed on the chassis, namely the tissues are placed between the chassis and the cutting sleeve, the displacement of the tissues can be prevented through the combination of the chassis and the cutting sleeve, meanwhile, the tissues are only exposed on the protrusions of the chassis, a plurality of points of the tissues can be accurately perforated, the suture disc is reversely buckled on the cutting sleeve, then the suture disc is rotated to drive the annular needle to rotate, and the annular needle pulls the purse string to pass through the tissues along the grooves on the protrusions of the chassis; after the suturing is finished, the annular needle is disassembled, the suturing disc, the cutting sleeve and the chassis are removed, and the purse string which penetrates into the tissue epidermis is fastened, so that the suturing of the purse is completed.
Further, the chassis is formed by combining two semicircular chassis; preferably, the two semicircular chassis are respectively provided with concave-convex structures at the center shaft, and the chassis is formed by combining the concave-convex structures. This is because the surgical field is smaller when suturing the peritoneum, and the two semicircular chassises can be passed through the smaller peritoneal incision, respectively, and assembled after entering the inside of the peritoneum when in use, thereby enabling circular suturing of the smaller incision.
Further, the center of the chassis, the clamping sleeve and the sewing disc is provided with a through hole, and the chassis, the clamping sleeve and the sewing disc can be connected by sequentially penetrating through the through holes through a connecting rod. The three components are connected through the connecting rod, so that the stability of the stitching instrument in use can be improved.
Further, the bulges are in an inverted concave shape, and grooves are formed in the centrifugal direction of the chassis. During suturing, the annular needle rotates, sequentially through the grooves in each boss, to precisely perforate tissue at that location.
Further, the number of the protrusions is not less than 3.
Preferably, the number of projections is 5.
Further, the radian of the annular needle is consistent with the inner wall of the suture disc, and the length of the annular needle is 0.5-1 time of the circumference of the suture disc.
Further, the suture disc also comprises a buckle fixed on the inner wall, and the annular needle is detachably and fixedly connected with the inner wall through the buckle.
Further, the charged envelope wire is cotton wire.
When used to suture the peritoneum, a small incision can be made in the peritoneum of the film, the chassis is placed under the peritoneum through the incision as described above, and the annular needle in the suture disc can draw the purse string along the raised groove of the chassis through the peritoneum by rotating the suture disc, thereby performing annular suturing. The semiautomatic purse string suture device can make a smaller incision on the peritoneum of a film, then performs annular suture, does not contain any staple, can use the most commonly used cotton thread to perform purse string ligation, has simple structure, safe and convenient technology, low cost and good effect, skillfully solves the problems that manual suture is time-consuming and labor-consuming and is easy to suture intestinal wall or large omentum in the abdominal cavity by mistake, improves the safety of the peritoneal dialysis catheter implantation, and further improves the efficiency of peritoneal surgery.
The semiautomatic purse-string suture device can be used for suturing peritoneum in an open peritoneal dialysis catheter implantation operation, the suture of a peritoneal purse-string can be completed by rotating a suture disc, the required incision is smaller, the made purse-string suture can be tightened and tightly wrapped around the catheter, leakage is not easy, and the semiautomatic purse-string suture device is particularly suitable for the implantation of the peritoneal dialysis catheter.
Further, in the operation of suturing the purse-string of the peritoneal tissue, the peritoneal dialysis catheter is placed first, and then the purse-string is tightened so that the peritoneal tissue tightly surrounds the catheter.
The invention also provides application of the semiautomatic purse-string suture device in suturing tissues.
Further, the tissue is the peritoneum.
Compared with the prior art, the invention has the following beneficial effects:
the semi-automatic purse-string suture device provided by the invention has reasonable structure and convenient use, can complete annular suture of the purse string only by a small incision when suturing tissues, does not contain any staple, improves the safety of surgical suture purse, shortens the surgical time and improves the working efficiency; the annular sutured charged wire can be further tightened to tightly wrap the catheter, and is suitable for the placement of the peritoneal dialysis catheter.
