Abstract
The invention relates to a vertical control device for orthodontics of children in dentition period, which comprises a maxillary vertical control component and a mandibular vertical control component; the maxillary vertical control assembly comprises: a connecting body; casting crowns arranged at two ends of the connecting body and fixed on the breast teeth of the patient; a buccal tube disposed on the buccal side of the cast crown; the traction hook is arranged on the casting crown and extends to the maxillary anterior tooth area; the mandibular vertical control assembly includes: a base which is abutted with the lingual side; the arrow clamps are arranged at the two sides of the base and clamped in the mandibular teeth grinding areas at the two sides, so that the retention is realized; a labial arch arranged in the middle of the base, wherein the labial arch is used for preventing the labial inclination of the anterior mandible; and the adjacent hooks are arranged in the middle of the base, prevent the extension of the anterior mandible and strengthen retention. Compared with the prior art, the invention can depress the upper anterior teeth, control the growth of the lower anterior teeth, improve the deep dental articulations and achieve obvious effects of the reverse dental articulations and the mandibular planes.
Description
Technical Field
The invention relates to the technical field of medical appliances, in particular to a vertical control device for orthodontic treatment of children in dentition period.
Background
The high angle of the mandibular plane angle of children is a misjaw deformity with abnormal development of mandible in the vertical direction, and is often accompanied with a sagittal bone type II misjaw. The high-angle infant has the advantages that the mandible grows excessively in the vertical direction, the front face is higher than the rear face, the mandible plane rotates clockwise to cause overlong lower parts, the appearance of the jaw face is affected due to the fact that the hyoid bone and the tongue body are located at the rear, the upper airway is easy to narrow, sleep breathing disorder is caused, and the general health of the infant is adversely affected.
In orthodontic clinic, if a bone type II patient is accompanied with high angle deformity, the treatment difficulty is obviously increased. For patients with permanent dentition, the vertical control technology of the implant nails is often used for camouflage treatment, or combined treatment of orthodontic-orthognathic surgery is relied on after the adult. For patients in the growth and development peak or the pre-peak period, the treatment is mainly to correct the sagittal imbalance of the jawbone and simultaneously change the growth direction of the mandible, namely, the vertical control technology reduces the severity of high angles and reduces the later treatment difficulty.
However, the prior art has limited orthodontic treatment means and has very little general effect on the group II osseous children with high angles in the dentition period. When the functional appliance is worn, teeth elongation of different degrees can occur at the same time of correcting sagittal development disorder of the jawbone, so that the jawbone rotates clockwise, the angle of the plane of the jawbone is increased, and the angle of the jawbone is worsened, so that serious angle of the jawbone is a relative contraindication of the functional appliance. In addition, the existing dentition period vertical control device is an extraoral device, such as an extraoral arch-head cap, has larger volume and poor comfort, needs to rely on the cooperation of patients, and is difficult to achieve the expected effect if the compliance of the patients is not high.
Disclosure of Invention
The invention aims to overcome at least one of the defects in the prior art and provide a vertical control device for orthodontics for children in dentition period, which can greatly reduce the difficulty of late stage camouflage treatment and reduce the probability of adult orthognathic surgery. After the device is worn on the affected side, the upper jaw front teeth are actively depressed, the first molar teeth are elongated, the upper jaw running-in plane rotates anticlockwise, the lower jaw front teeth are controlled to grow vertically, and the vertical growth of the lower jaw bone is changed.
The aim of the invention can be achieved by the following technical scheme:
a vertical control device for orthodontics of children in dentition period comprises an upper jaw vertical control component and a lower jaw vertical control component;
the maxillary vertical control assembly comprises:
a connecting body;
casting crowns arranged at two ends of the connecting body and fixed on the breast teeth of the patient;
a buccal tube disposed on the buccal side of the cast crown;
the traction hook is arranged on the casting crown and extends to the maxillary anterior tooth area;
the mandibular vertical control assembly includes:
a base which is abutted with the lingual side;
the arrow clamps are arranged at the two sides of the base and clamped in the mandibular teeth grinding areas at the two sides, so that the retention is realized;
a labial arch arranged in the middle of the base, wherein the labial arch is used for preventing the labial inclination of the anterior mandible;
and the adjacent hooks are arranged in the middle of the base, prevent the extension of the anterior mandible and strengthen retention.
In one embodiment of the invention, the connector is a palate support or an expander, which can fix and strengthen the overall resistance of the upper jaw.
Preferably, the expander is a spiral expander.
In one embodiment of the invention, the thickness of the casting crown is 0.3-1mm.
In one embodiment of the invention, the towing hook is a stainless steel wire with a cross-sectional circle diameter of 0.8-2 mm. The traction hook is connected with an arch wire in the dental arch through an elastic rubber chain to have integral depression effect on the upper front teeth, the occlusion is opened due to the thickness of the casting crown in the wearing process, the first molar is elongated, and then the effect of enabling the maxillary dentition plane to rotate anticlockwise is achieved.
In one embodiment of the invention, the cast crown is welded to the buccal tube.
In one embodiment of the invention, the buccal tube is provided with a hollow tube through which the corrective archwire may be secured for ease of passage of the corrective archwire.
More specifically, buccal tubes (brackets) welded to the buccal sides of the double-sided cast crown may allow the corrective archwire to pass through the hollow tube thereof, thereby serving to secure the corrective archwire.
In one embodiment of the invention, the arrow-shaped clip is annular, and the retention is achieved by a clip on a bilateral mandibular molar area.
In one embodiment of the invention, a bending adjusting part is arranged at one end of the labial arch, which is close to the base, and the bending adjusting part is positioned at the outer side of the base.
In one embodiment of the invention, both ends of the adjacent hooks and the arrow cards are embedded in the base, and the hook cards of the adjacent hooks and the hitching parts of the arrow cards are positioned at the outer sides of the corresponding base.
Compared with the prior art, the invention has the following advantages:
(1) The existing dentition period vertical control device is an extraoral device, such as an extraoral arch-head cap, has larger volume and poor comfort, is generally worn only at night, needs to rely on the cooperation of patients, and is difficult to achieve the expected effect if the compliance of the patients is not high. The device designed by the invention is an intraoral device, can be worn all day, can ensure the treatment effect,
(2) The existing vertical control device for the dentition period does not relate to correction of anterior teeth, has no correction effect on deep occlusion of the anterior teeth, and has limited left and right rotation on the upper jaw plane and the lower jaw plane. The invention can depress the upper anterior teeth, control the growth of the lower anterior teeth, improve the deep dental occlusion and achieve obvious effects of contra-rotating dental occlusion surface and mandibular plane.
(3) Serious high angles are relative contraindications of the functional appliance, and a large number of patients needing functional correction can only be treated in a suspending way due to the high angles, wait for the treatment of the permanent dentition period to be disguised by using the implanted nail anchorage technology, and even treat the adult posterior orthognathic surgery. The device designed by the invention is reversely screwed on a plane, so that the growth of the lower jaw is changed, the difficulty of orthodontic treatment in the dentition period can be obviously reduced by the later-stage combined functional appliance treatment, and the risk of orthognatic operation is reduced.

Detailed Description
The invention will now be described in detail with reference to the drawings and specific examples. The present embodiment is implemented on the premise of the technical scheme of the present invention, and a detailed implementation manner and a specific operation process are provided, but the protection scope of the present invention is not limited to the following embodiments.
Examples
A vertical control device for orthodontics of children in dentition period, see fig. 1-8, comprises a maxillary vertical control component and a mandibular vertical control component;
the vertical control assembly of upper jaw includes:
a connector 1; casting crowns 2 arranged at two ends of the connecting body 1 and fixed on the breast teeth of the patient; a buccal tube 3 provided on the buccal side of the cast crown 2; the traction hook 4 is arranged on the casting crown 2 and extends to the maxillary anterior tooth area, and the traction hook 4 is connected with an arch wire in the dental arch through an elastic rubber chain to have integral depression effect on the upper anterior tooth;
the mandibular vertical control assembly includes:
a base 5 abutting the lingual side; the arrow cards 6 are arranged at the two sides of the base 5, and the arrow cards 6 are clamped in the mandibular teeth grinding areas at the two sides so as to realize retention; a labial arch 7 arranged in the middle of the base 5, wherein the labial arch 7 is used for preventing the labial inclination of anterior mandibular teeth; an interproximal hook 8 provided in the middle of the base 5, the interproximal hook 8 preventing extension of the anterior mandibular teeth and reinforcing retention.
Referring in detail to Figs. 2-3, the connector 1 is a palate support or expander, preferably a screw expander. The thickness of the casting crown 2 is 0.3-1mm. The traction hook 4 is a stainless steel wire with the cross-section circle diameter of 0.8-2 mm. The cast crown 2 is welded to the buccal tube 3. The buccal tube 3 is provided with a hollow tube which is convenient for the corrective archwire to pass through, and the corrective archwire can be fixed through the hollow tube.
Referring to fig. 4 and 5 in detail, the arrow card 6 is annular and is retained by being clamped in the mandibular molar area on both sides. One end of the lip bow 7, which is close to the base 5, is provided with a bending adjusting part, and the bending adjusting part is positioned at the outer side of the base 5. Both ends of the inter-adjacent hook 8 and the arrow head card 6 are embedded in the base 5. The hooking part of the adjacent hook and the sleeving part of the arrow head are positioned at the outer side of the corresponding base.
Working principle:
referring to fig. 7 in detail, in this embodiment, the vertical control component of the upper jaw is a fixing device, the casting crown 2 of the bilateral deciduous molar area is fixed on the deciduous molar through an adhesive, and the bilateral casting crown 3 is connected through the connecting body 1 of the palate part, so as to play a role in fixing and enhancing the overall anchorage of the upper jaw; the buccal tube 3 welded to the buccal side of the double sided cast crown 3 allows the corrective archwire to pass through its hollow tube, thereby serving to secure the corrective archwire. The cast crown 3 stretches to two traction hooks 4 of the maxillary anterior tooth area, and is connected with an arch wire in the dental arch through an elastic rubber chain to have integral depression effect on the upper anterior tooth, and the cast crown is opened and occluded due to thickness in the wearing process, so that the first molar is elongated, and further the effect of enabling the maxillary occlusal plane to rotate anticlockwise is achieved.
Referring to fig. 8 in detail, the mandibular vertical control component in this embodiment is a movable device, the arrow cards 6 of the mandibular molar area on both sides thereof play a role in retention, and the interproximal hooks 8 of the anterior teeth mainly play a role in preventing extension of the anterior teeth of the mandible and enhancing retention; the labial arch 7 of the anterior dental area is primarily intended to prevent the lower anterior labial tipping.
After the vertical control device is worn, the upper jaw front teeth are actively depressed, the first molar teeth are elongated, the upper jaw teeth engagement plane rotates anticlockwise, the lower jaw front teeth are vertically controlled to be free from growth, and the vertical growth of the lower jaw bone is changed, so that correction is realized.
Fig. 9 is a diagram showing the mechanism of anterior mandibular plane angle pronation of a prior art functional appliance, with the dashed line indicated as pre-treatment and the solid line indicated as post-treatment. Fig. 10 is a diagram showing a mechanism of the orthodontics vertical control device for children in dentition stage according to the present embodiment, wherein the vertical control device is reversely rotated to the mandibular plane before the functional correction, and the broken line is shown as before the treatment, and the solid line is shown as after the treatment. B1 is a schematic diagram of a pre-treatment anterior occipital plane a and a pre-treatment mandibular plane B; b2 is that in the treatment, the vertical control device presses down the upper front teeth and the lower front teeth, and the occlusal plane is reversely rotated; b3 is the anterior mandible of the functional appliance during treatment; b4 is the mandibular plane reverse rotation after the functional appliance is worn.

a labial arch (7) arranged in the middle of the base (5), wherein the labial arch (7) is used for preventing the labial inclination of the anterior mandibular teeth;
and an interproximal hook (8) arranged in the middle of the base (5), wherein the interproximal hook (8) prevents the extension of mandibular anterior teeth and strengthens retention.
2. The vertical control device for orthodontic treatment of children in dentition period according to claim 1, wherein the connector (1) is a palate bracket or an expander.
3. The orthodontic vertical control device for a dentition-period child according to claim 2, wherein the expander is a screw expander.
4. The vertical control device for orthodontics for a dentition child according to claim 1, wherein the thickness of the cast crown (2) is 0.3-1mm.
