Abstract
The invention discloses an arch-expanding type occlusion pad for a breast-replacing odontoseisis condylar fracture, which comprises a base, a functional surface, a tissue surface and an arch expander, wherein the base wraps an upper dentition, the tissue surface at the upper part of the base is adhered and fixed on a dental crown of a patient, the base is split into two parts at the middle part and consists of a patient side base and a health side base, the patient side base is connected with the health side base through the arch expander, and the functional surface at the lower part of the base is matched with mandibular teeth. The invention aims at solving the problems of irregular width of the upper and lower jawbones and back teeth back biting of children with condylar fracture, and the bite pad is not easy to dislocate, reduces the pressure of the area of the side affected condyle, is matched with the relationship of the cuspid fossa of the opposite teeth, and has good restoration of the bite function. The invention has reasonable design and convenient use, and is suitable for the deciduous teeth and/or the teeth replacement stage patients, especially the deciduous teeth stage cases.
Description
Technical Field
The invention belongs to the field of oral medical appliances, and particularly relates to an arch expanding type bite pad for a breast-substituted odontopathy condylar fracture.
Background
In all mandibular fractures, the incidence rate of mandibular fracture of children is less than 15%; however, in mandibular fracture in children, condylar fractures are common and account for 24% -72%. Children are in the growth and development period, if the condylar fracture is mishandled, the condylar fracture can lead to the rigidity of the temporomandibular joint and the disorder of the occlusion relationship, and even seriously affect the development of the mandible and the maxillofacial region of the child. At present, the common non-arch-expanding bite pad has wide application, simple instrument and low price, but has insufficient treatment individuation for different patient conditions. After the fracture of the condyle of the child, the occlusal muscles pull the jawbone and dentition of the affected side, and the height of the ascending branch of the jawbone of the affected side is reduced and the occlusal process may occur. The existing closing pad has uniform thickness, and can not effectively recover the position of a broken end of a bone when the occlusion height of a patient side is not enough; in the event that the inverse combination cannot be corrected, no additional processing means can be provided. Some patients also adopt removable bite pads, and are fixedly arranged on teeth by virtue of snap rings and gap hooks, but due to the short clinical crowns of children patients in the tooth replacement period, permanent teeth are not erupted yet, and the retention force is weak. The patient has the biting movement during chewing, the masticatory muscle strength is large, and the bite pad dislocation is more easily caused.
Disclosure of Invention
In view of this, it is an object of the present invention to provide an arch expanding bite pad for use in deciduous odontoid fractures. The device is used for reducing the pressure of the affected side condylar area and solving the problem of rapid recovery of the children condylar fracture by adjusting the bow expander, increasing the expansion area, improving the height of the affected side occlusal pad and the like.
In order to achieve the aim of the invention, the invention adopts the following technical scheme: an arch expansion type occluding pad comprises a base, a functional surface, a tissue surface and an arch expander; the base wraps the upper dentition and is fixed on the dental crown of the oral cavity of the patient through the tissue surface at the upper part of the base; the base is split into two parts at the middle part and consists of a base with a side group and a base with a side group; the patient side base is connected with the healthy side base through an arch expander; the functional surface of the lower part of the base is matched with the teeth of the lower jaw. The base plates of the base plates and the health-care base plates are in asymmetric design, and the extension areas are arranged on the base plates of the base plates, so that the base plates of the base plates are thickened and extended, and the extension and support functions of the base plates are achieved. The thickness of the base on the affected side is 2-3mm greater than that of the base on the healthy side. The thickness of the base is 2mm, and the expansion area is determined according to the occlusion height which can be recovered after the mandibular joint is regulated by a clinical manipulation. The connection position of the base and the bow expander is positioned in the rear third area of the central axis of the base. The size of the base covers the palate and the palate side, the occlusal surface and the buccal side of the dentition, and the distal part wraps the distal part of the last molar. The bow expander adopts a spiral bow expander. The functional surface covers four teeth from the side incisors of the base with the side base and the side healthy base to the first molar, and is matched with the cuspid relation of the involution teeth, so that the occlusion function is good. The tissue surface is directly adhered and firmly fixed on the upper dental crown of a patient by adopting a glass ion water portal material, so that the retention is enhanced, and the bite pad is not easy to loose. The base is made of polymethyl methacrylate material.
The beneficial effects obtained by the invention are as follows:
aiming at the problem that after the fracture of the child condyle, occlusion muscles pull the mandible and dentition on the affected side, the healing height of the condyle is possibly insufficient, the contra-engagement of the unilateral rear teeth is caused, and the height of the lifting branch of the mandible on the affected side is reduced, the distance between the left base and the right base can be increased by using the bow expander, so that the width expansion of the base is realized. The thickness of the occlusal surface of the base on the affected side is larger than that of the healthy side, and the opening amount of the occlusal height of the affected side is increased. The occlusal pad is adhered to the teeth of the upper jaw by the adhesive, and the retention is good. The functional surface of the bite pad is matched with the teeth of the lower jaw, so that the bite pad has a better staggered relation of the cuspids, and the chewing efficiency is enhanced. The pressure of the affected side condylar area can be reduced by increasing the height of the affected side bite pad to be larger than that of the healthy side bite pad.
(1) The spiral arch expander is added in the middle of the occlusion pad base, the closed palate middle seam is pulled Zhang Shangwei through the orthopedic force level, new bone deposition in the seam is stimulated, the transverse development of the maxilla is promoted, and the width of the maxilla dental arch is increased. (2) The invention increases the occlusion height of the affected side, is beneficial to correcting the position of the fracture end, relieves the pressure of the condyle, promotes the healing of the condyle and restores the ascending branch height of the mandible of the affected side.
(3) The invention is designed to be adhered to the adhesive tissue surface of the teeth, directly adhered to the dental crowns of patients, enhanced in retention and not easy to loose.
(4) The occlusion surface is used for manufacturing a tooth anatomical function surface, is matched with the relationship of the cusps of the involution teeth, and has good occlusion function.
(5) The invention makes the height of the affected side bite pad larger than that of the healthy side bite pad, and can reduce the pressure of the affected side condyle area.

Detailed Description
The present invention will be described in detail below with reference to specific embodiments thereof in conjunction with the accompanying drawings.
As shown in fig. 1, 2 and 3, in order to increase the distance between the left base and the right base and realize the width expansion of the base, the arch expansion type bite pad for the breast-substituted odontoid fracture comprises a base 2, a functional surface 3, a tissue surface 4 and an arch expander 1; the upper part of the base 11 is a tissue surface 4, and the lower part is provided with a functional surface 3. The base 2 wraps the upper dentition and is fixed on the dental crown of the oral cavity of a patient through a tissue surface 4 at the upper part of the base 2; the base 2 is split into two parts at the middle part and consists of a base 11 on a patient side and a base 12 on a health side; the base 11 on the affected side is connected with the base 12 on the healthy side through the bow expander 1; the functional surface 3 at the lower part of the base is matched with the teeth of the lower jaw.
Further, in one embodiment of the present invention, the opening amount of the occlusal height of the patient side is increased, the patient side base 11 and the health side base 12 are asymmetrically designed, and the extension area 5 is provided on the patient side base 11 to thicken and extend the patient side base 11, so as to play a role of base extension and support.
Further, in one embodiment of the present invention, the pressure of the side affected condyle area can be reduced by making the thickness of the affected side base 11 greater than the thickness of the healthy side base 12 by 2-3mm.
Preferably, the thickness of the base 2 is 2mm, and the extension area 5 is determined according to the bite height that can be restored after the mandibular adjustment by the clinical practice.
Preferably, the connection position of the base 2 and the bow spreader 1 is located in the rear third area of the central axis of the base 2.
Preferably, the base 2 is sized to cover the palate and palate side, occlusal surface, buccal side of the dentition, distal to the distal center of the last molar.
Preferably, the expander 1 is a screw expander.
Preferably, the functional surface 3 covers four teeth from the base of the patient side and the cuspid of the base of the health side to the first molar, and is matched with the cuspid relation of the involution teeth, so that the occlusion function is good.
Preferably, the tissue surface 4 is directly adhered and fixed on the upper dental crown of the patient by adopting glass ion water portal material, so that the retention is enhanced, and the bite pad is not easy to loosen.

the bow-expanding type occluding pad comprises a base (2), a functional surface (3), a tissue surface (4) and a bow expander (1);
the base (2) wraps the upper dentition and is fixed on the dental crown of the oral cavity of the patient through a tissue surface (4) at the upper part of the base (2);
the base (2) is split into two parts at the middle part and consists of a base (11) on the base side and a base (12) on the health side; the patient-side base (11) is connected with the health-side base (12) through the bow expander (1); the functional surface (3) at the lower part of the base is matched with the mandibular teeth, wherein,
the base (11) and the health-care base (12) are in asymmetric design, and an expansion area (5) is arranged on the base (11) to thicken and expand the base (11) and play a role in expanding and supporting the base;
the thickness of the base (11) on the affected side is 2-3mm greater than that of the base (12) on the healthy side;
the functional surface (3) covers four teeth from the side incisors of the base and the health side base to the first molar, and is matched with the cuspid relation of the involution teeth, so that the occlusion function is good.
