Description
技术领域
本发明涉及抗癌治疗领域。具体地,本发明涉及肠道微生物群在基于免疫检查点抑制剂(ICI)的治疗的功效中的作用,并提供用于确定患者是否可能受益于基于ICI的治疗,更准确地说,包括施用针对PD1或PD-L1的抗体的治疗的方法。
本发明提供了一种用于在接受基于ICI的治疗之前根据癌症患者对细菌补偿的潜在需要对癌症患者进行分层的方法。
本发明还提供了一种用于确定个体是否患有肠道生态失调的简单方法。
背景技术
靶向PD-1/PD-L1相互作用的免疫检查点抑制剂(ICI)的开发改变了晚期非小细胞肺癌(NSCLC)患者的治疗格局(Herbst等人,2016;Brahmer等人,2015;Borghaei等人,2015;Gandhi等人,2018;Paz-Ares等人,2018;Reck等人,2016)。对既往接受过治疗的晚期NSCLC患者进行的具有里程碑意义的试验表明,与标准化疗相比,PD-1/PD-L1阻断疗法具有更高的总存活率(OS)(Herbst等人,2016;Brahmer等人,2015;Borghaei等人,2015)。在针对先前未经治疗的晚期NSCLC患者进行的III期随机试验中获得了前所未有的OS结果后,ICI被批准用于一线治疗,既可以作为肿瘤细胞上肿瘤PD-L1表达≥50%的患者的单一疗法,也可以与铂类双重化疗联合使用,无论PD-L1表达如何(Gandhi等人,2018;Paz-Ares等人,2018;Reck等人,2016;Arielle Elkrief等人,2020)。然而,只有少数(35%)的患者受益于对ICI的持续反应(Gadgeel等人,2020)。大多数NSCLC患者出现原发性或继发性耐药,或疾病的偶尔加速进展,称为“超进展”(Ferrara等人,2018)。此外,由于低敏感性和特异性,目前针对ICI反应的生物标志物并不令人满意,因此,迫切需要了解对ICI的耐药机制,以鉴定稳健的耐药生物标志物。
原发性耐药归因于低肿瘤突变负荷和肿瘤细胞的不良内在抗原性(Riaz等人,2017;Rizvi等人,2015)、缺陷的抗原呈递(Spranger,Bao,et Gajewski 2015)、与T细胞耗竭相关的有限瘤内浸润(Smyth等人,2016)和代谢免疫抑制途径(Koyama等人,2016;Young等人,2016)。目前开发了高维组学技术以破译“癌症免疫设定点”的主要调节因子-超过其在肿瘤携带者中可以发生有效的免疫反应的阈值(Chen et Mellman 2017)。
最近的证据表明,肠道微生物群的组成对于影响癌症患者中的外周免疫张力和ICI的有效性具有生物学意义。由1013个微生物组成的人类肠道微生物组调节许多宿主过程,包括新陈代谢、炎症、蠕动、病原体和异生物质的消除、免疫功能成熟以维持对微生物和食物抗原的耐受性以及肠上皮屏障适应性(Routy,Gopalakrishnan等人,2018)。最近,肠道微生物群意外地被证明影响ICI的有效性(Routy,Le Chatelier等人,2018)。首先,在临床前模型中,对无菌或抗生素(ATB)处理的小鼠进行的实验表明,ICI的抗肿瘤活性需要肠道微生物成分的存在(Vétizou等人,2015)。同样,在ICI启动之前的ATB显著降低不同癌症类型(黑素瘤、晚期NSCLC、肾细胞癌(RCC)和尿路上皮癌)之间几个患者队列中的临床获益和存活率(Routy,Le Chatelier等人,2018;Gopalakrishnan等人,2018;L.Derosa等人,2018;Lisa Derosa et Zitvogel 2020)。这一观察结果在前瞻性试验和大型meta分析中得到证实,表明肠道微生物群可能有助于ICI的免疫刺激作用模式(Lurienne等人,2020)。
支持这一论点的是,我们和其他人报道,具有上皮性肿瘤和黑素瘤的患者对抗PD-1/PD-L1抗体的主要反应可能至少部分归因于肠道微生物群的组成(Routy,Le Chatelier等人,2018;Gopalakrishnan等人,2018;Matson等人,2018)。多样化的微生物群和特定细菌(例如嗜粘蛋白阿克曼菌(Akkermansia muciniphila)、瘤胃球菌属(Ruminococcus)或双歧杆菌属(Bifidobacterium))的存在与改善的对ICI的临床反应相关,并与增加的全身免疫张力相关(Routy,Le Chatelier等人,2018;Gopalakrishnan等人,2018;Matson等人,2018)。我们之前的工作报告了接受二线或三线抗PD-1抗体治疗的100名诊断为难治性NSCLC或RCC的患者的基于宏基因组学的微生物组分析,得出的结论是,与处于进行性疾病的患者相比,在出现部分反应或稳定疾病的患者中嗜粘蛋白阿克曼菌(Akk)的流行率增加(分别为34%相对61%,p=0.007)(Routy,Le Chatelier等人,2018)。在分析60名NSCLC患者的亚组时,Akk在无进展生存期(PFS)>3个月的患者中也过度呈现(Routy,Le Chatelier等人,2018)。另一组对37名NSCLC患者粪便进行了16S rRNA基因扩增子测序,证实Akk在对ICI做出反应的患者中富集(Jin等人,2019)。在接受阿比特龙(abiraterone)治疗的前列腺腺癌患者中,肠道Akk也与治疗反应相关(Daisley等人,2020)。Akk在临床样本中与低体重指数、健壮、总体健康和成功衰老有关(如其在无病百岁老人中的存在所表明的)(Santoro等人,2018)。在动物模型中,Akk补充减少肥胖(Zhou等人,2020)及其并发症,缓解神经退行性疾病(Blacher等人,2019)并对抗早衰症(Bárcena等人,2019)。因此,Akk可以被视为宏生物体中稳态的潜在主要调节剂。
总而言之,来自异质地理分布的小队列的这些初步结果提出关于肠道微生物群组成和更具体地诊断时Akk的存在是否可以代表预测患有晚期NSCLC和其他癌症的患者对ICI的反应的潜在生物标志物。为了验证这一假设,我们进行了一项前瞻性多中心研究,并分析了接受抗PD-1抗体治疗的311名晚期NSCLC患者的基于宏基因组学的微生物组特征谱。
发明内容
发明人先前已显示,来自癌症患者的粪便样品中嗜粘蛋白阿克曼菌的不存在指示对PD1阻断的抗性(WO2018115519)。
在下面报道的实验中,发明人令人惊讶地鉴定了一组表现出高水平的嗜粘蛋白阿克曼菌的患者,其与不良结果相关。
因此,他们考虑将患者按三分法分层为Akk-、Akk低(<75th百分位数)和Akk高(>75th百分位数)个体,这比二分法(Akk-相对Akk+)划分产生了总存活率的更准确的独立预测因子。使用这种3组患者分层,嗜粘蛋白阿克曼菌水平代表了接受使用PD-1阻断的免疫治疗的患者的预后的更可靠的生物标志物,甚至推翻了PD-L1作为≥2L NSCLC患者中对ICI反应的预测生物标志物。
然后,他们证明,阿克曼菌SGB9228的行为方式与嗜粘蛋白阿克曼菌相同,即肠道中“正常水平”的嗜粘蛋白阿克曼菌(嗜粘蛋白阿克曼菌低)或阿克曼菌SGB9228(A.SGB9228低)的存在可以被认为是宿主肠道健康的替代物。相反,在宿主肠道菌群中阿克曼菌属的细菌不存在或它们以过高的水平存在(尤其是嗜粘蛋白阿克曼菌和/或阿克曼菌SGB9228的过高水平)时,宿主需要细菌补偿以用于对ICI疗法作出反应。
因此,根据第一方面,本发明涉及一种体外治疗诊断方法,以确定癌症患者是否可能是对基于免疫检查点抑制剂(ICI)的疗法的良好反应者,包括测量来自所述患者的样品中阿克曼菌属的细菌(例如嗜粘蛋白阿克曼菌和/或阿克曼菌SGB9228)的相对丰度,其中低于预定阈值的所述阿克曼菌的存在表明该患者可能是对基于ICI的疗法的良好反应者。
如下面的实验部分所示,肠道中正常水平的嗜粘蛋白阿克曼菌(Akk低)和/或阿克曼菌SGB9228(Akk.SGB9228低)的存在可以被视为宿主肠道健康和免疫细胞侵入微环境的替代物,从而识别可能对ICI治疗有反应的患者,而嗜粘蛋白阿克曼菌和/或阿克曼菌SGB9228在超生理水平下指示不良预后,这可能反映了ATB或其他有害因素诱导的肠道伤口愈合。因此,肠道微生物群中嗜粘蛋白阿克曼菌和/或阿克曼菌SGB9228的缺失和过度呈现是其中患者在开始基于ICI的疗法之前应接受补偿性微生物治疗的情形的标志,以提高他/她对所述基于ICI的治疗做出反应的机会。
因此,本发明还涉及一种用于确定癌症患者在施用基于ICI的疗法之前是否需要细菌补偿的方法,包括测量来自所述患者的样品中阿克曼菌的相对丰度,其中:
(i)如果样品中不存在阿克曼菌,患者在ICI施用前需要用至少阿克曼菌进行细菌补偿;
(ii)如果样品中存在低于预定阈值的阿克曼菌,则患者在ICI施用前不需要任何细菌补偿;和
(iii)如果样品中存在高于预定阈值的阿克曼菌,特别是如果这种过度呈现是在抗生素暴露之后发生和/或与属于γ-变形菌纲和/或脱硫弧菌科的物种的过度呈现相关,则患者需要使用多微生物联合体(尤其是复杂的多微生物联合体,即至少5种、优选至少7种、和例如超过10种不同微生物菌株的联合体)和/或通过来自健康个体或来自对基于ICI的疗法成功反应的癌症患者的粪便微生物移植(FMT)进行细菌补偿。
下面报道的实验还表明,嗜粘蛋白阿克曼菌的肠道驻留是肠道生态系统丰富度的间接标志,如75th百分位数内的嗜粘蛋白阿克曼菌相对丰度与粪便的α多样性(香农多样性指数)之间的关联所示。因此,可以测量嗜粘蛋白阿克曼菌的水平,以快速、轻松地识别肠道生态失调,这对于所有以微生物群为中心的干预措施非常有用。
因此,本发明的另一个方面是用于确定个体是否患有肠道微生物群生态失调的方法,包括测量来自所述个体的样品中阿克曼菌,尤其是嗜粘蛋白阿克曼菌和/或阿克曼菌SGB9228的相对丰度,其中低于预定阈值的所述阿克曼菌的存在表明不存在肠道微生物群生态失调,并且不存在阿克曼菌属的细菌,尤其是嗜粘蛋白阿克曼菌和/或阿克曼菌SGB9228,或者高于预定阈值的它们的存在,表明肠道微生物群生态失调。
本发明还涉及多微生物联合体或FMT的用途,尤其是使用来自健康个体或来自对基于ICI的疗法成功反应的癌症患者的材料,用于治疗在他/她的肠道微生物群中具有过度呈现的阿克曼菌属,尤其是嗜粘蛋白阿克曼菌和/或阿克曼菌SGB9228的癌症患者(在ICI治疗之前和与ICI治疗相结合)。
本发明的另一个方面是包含阿克曼菌属的细菌、尤其是阿克曼菌SGB9228和/或嗜粘蛋白阿克曼菌的细菌组合物,其用于治疗在他/她的肠道微生物群中不具有嗜粘蛋白阿克曼菌和阿克曼菌SGB9228的癌症患者(在ICI治疗之前和与ICI治疗组合)。更具体地,使用阿克曼菌SGB9228的冻干封装菌株(例如Akksp2261)的治疗性补充对未暴露于ATB且缺乏内源性嗜粘蛋白阿克曼菌和内源性阿克曼菌SGB9228的患者有益,特别是当它使微生物群转向有利的健康相关物种(即产丁酸肠杆菌(Intestinimonas butyriciproducens)、内脏臭杆菌(Odoribacter splanchnicus)、排泄物毛螺旋菌(Parasuterrellaexcrementihominis)、粪罗氏菌(Roseburia faecis))时。

肠道微生物群
“肠道微生物群”(以前称为肠道菌群或微生物群落)是指生活在属于动物界(人类、动物、昆虫等)的任何生物体的肠道中的微生物群体。虽然每个个体都有独特的微生物群组成(在总共400-500种不同的细菌物种/个体上超过50%的样本群体共有60到80种细菌物种),但它总是履行相似的主要生理功能,并对个体健康具有直接的影响:
·它有助于消化胃和小肠无法消化的某些食物(主要是不可消化的纤维);
·它有助于某些维生素(B和K)的产生;
·它防止其他微生物的侵袭,保持肠粘膜的完整性;
·它在正常免疫系统的发育中发挥着重要作用;
·健康、多样化和平衡的肠道微生物群是确保肠道正常功能的关键。
考虑到肠道微生物群在身体正常功能及其实现的不同功能中发挥的主要作用,它现在被认为是一个“器官”。然而,它是一个“后天”器官,因为婴儿出生时是无菌的。也就是说,肠道定殖在出生后就开始并随后演变。
肠道微生物群的发育从出生时就开始了。在子宫内处于无菌状态,新生儿的消化道很快被来自母亲(阴道、皮肤、乳房等)、分娩环境、空气等的微生物定殖。从第三天开始,肠道微生物群直接取决于婴儿的喂养方式:例如,与婴儿配方奶粉喂养的婴儿相比,母乳喂养婴儿的肠道微生物群主要以双歧杆菌为主。
肠道微生物群的组成在从出生到老年的整个生命过程中不断演变,并且是不同环境影响的结果。肠道微生物群的平衡可在衰老过程中受到影响,因此,老年人的微生物群与年轻人有很大不同。
虽然大多数健康人肠道主要微生物群的总体组成相似(4个主要门,即厚壁菌门、拟杆菌门、放线菌门和变形菌门),但物种水平上的组成是高度个性化的,并且很大程度上取决于个体的遗传、环境和饮食。肠道微生物群的组成可能暂时或永久地变得适应饮食成分。例如,日本人可以消化海藻(他们日常饮食的一部分),这要归功于他们的微生物群从海洋细菌中获得的特定酶。
生态失调
尽管它能够适应变化并具有较高的恢复能力,但在某些特定情况下可能会出现肠道微生物群组成失去平衡的情况。这被称为“生态失调”,即肠道中潜在“有害”和“有益”细菌之间的不平衡,或者在主要细菌群组成和多样性方面与被认为是“健康”微生物群的任何偏差。生态失调可能与功能性肠道疾病、炎症性肠病、过敏、肥胖和糖尿病等健康问题有关。它也可能是治疗的结果,例如细胞毒性治疗或抗生素治疗。
免疫检查点阻断剂

嗜粘蛋白阿克曼菌和阿克曼菌SGB9228是两种最普遍的阿克曼菌物种并且可以使用例如与所有阿克曼菌共有的基因杂交的引物一起检测。根据另一个实施方案,本发明因此涉及确定癌症患者是否可能是对基于免疫检查点抑制剂(ICI)的疗法的良好反应者的体外治疗诊断方法,包括测量来自所述患者的样品中阿克曼菌属的相对丰度,其中低于预定阈值(“上阈值”)的阿克曼菌属的细菌的存在表明患者可能是对基于ICI的疗法的良好反应者。
在WO2018115519中,发明人已经表明,来自癌症患者的粪便样品中嗜粘蛋白阿克曼菌的不存在指示对PD1阻断的抗性。在下面报道的实验中,他们现在证明,粪便样品中嗜粘蛋白阿克曼菌和/或阿克曼菌SGB9228的过度呈现(即其存在高于上阈值)指示尽管进行了ICI治疗,预后仍然不佳。至少如图2D所示,超生理水平的嗜粘蛋白阿克曼菌的存在与显著低于其粪便缺乏嗜粘蛋白阿克曼菌的患者的总体存活率的总体存活率相关,其本身低于表现出介于下阈值(对应于所报告的实验中的检测限)和上阈值(下文称为“预定阈值”)之间的水平的嗜粘蛋白阿克曼菌的患者的总体存活率。
在本发明的框架中可以用作“预定阈值”的阈值的示例在下面的实验部分中公开。当然,技术人员可以根据用于测量嗜粘蛋白阿克曼菌和/或阿克曼菌SGB9228和/或阿克曼菌属的相对丰度的技术(例如宏基因组学、定量PCR、微阵列杂交或焦磷酸测序)、检测到的阿克曼菌物种、患者的具体病理、患者的饮食习惯、用于治疗的具体ICI以及其他可能的因素调整或改进该阈值。例如,执行上述方法时要考虑的阈值可以通过测量与寻求预后的患者患有相同癌症的个体的代表性队列中嗜粘蛋白阿克曼菌和/或阿克曼菌SGB9228和/或阿克曼菌属的相对丰度并选择第75个百分位数的值作为阈值来预先确定。对于嗜粘蛋白阿克曼菌和对于阿克曼菌SGB9228,该阈值可能不同。
为了说明目的,在健康志愿者(来自现有文献)中和从诊断为患有黑素瘤或肾癌或肺癌的转移性患者的三个队列获得的嗜粘蛋白阿克曼菌的相对丰度值如下所示。

在本文中,“低于预定阈值的嗜粘蛋白阿克曼菌的存在”因此意味着嗜粘蛋白阿克曼菌以两个阈值之间的水平存在:下阈值(接近0,通常<0.001,例如0.0005)和上阈值(如上所述的“预定阈值”)。这同样适用于阿克曼菌SGB9228和阿克曼菌属。
根据本发明的特定实施方案,预定阈值对应于1%至10%之间、例如3%至6.5%之间的相对丰度。
根据另一特定实施方案,选择预定阈值以使得其在第75个百分位数和第77个百分位数之间。其可以通过网格搜索算法来选择。对于下面实验部分中使用的队列,选定的截断值(预定阈值)对应于4.79(第77个百分位数),测量误差为4.75+/-0.1。因此,当执行本发明的方法时,4.75+/-0.1的预定阈值可以用作上阈值。当然,如已经提到的,本领域技术人员可以通过常规实验来改进或调整该阈值。
根据本发明的另一个具体实施方案,基于ICI的疗法是基于抗PD1/PD-L1/PD-L2Ab的疗法(例如但不限于纳武单抗、派姆单抗、阿特珠单抗和德瓦鲁单抗)或基于抗CTLA4Ab的疗法(例如但不限于伊匹木单抗)或其组合。
如下面的实验部分所示,本发明对于患有非小细胞肺癌(NSCLC)或肾癌或患有适合PD1/PDL-1或CTLA4阻断的任何癌症的患者特别有用。
更一般地,本发明的方法可以有利地对患有适合免疫治疗的任何癌症的癌症患者进行,例如但不限于:黑素瘤;肾细胞癌(RCC);非小细胞肺癌(NSCLC);头颈鳞状细胞癌(HNSCC);Merkel细胞癌(MCC);膀胱癌;霍奇金淋巴瘤;鳞状细胞癌;乳腺癌,尤其是三阴性乳腺癌;胃癌;小细胞肺癌;原发性纵隔B细胞淋巴瘤;宫颈癌;肝细胞癌;食道癌;具有微卫星不稳定性的癌症;子宫内膜癌和任何具有高肿瘤突变负荷的癌症(TMB-H癌症)。
如下面的实验部分所示,本发明可用于其中基于ICI的疗法作为一线治疗或二线治疗或以上(第三、第四线)施用的情况。
根据本发明的特定方面,选择预定阈值以使得高于该阈值的嗜粘蛋白阿克曼菌和/或阿克曼菌SGB9228和/或阿克曼菌属的存在指示尽管进行了基于ICI的疗法,但预后不佳。
根据另一个方面,本发明涉及一种用于在施用基于ICI的疗法之前体外确定癌症患者是否需要细菌补偿的方法,并且向医生提供与可以改善患者对治疗产生反应的可能性的补偿的类型相关的信息。
根据具体实施方案,该方法包括测量来自所述患者的样品中嗜粘蛋白阿克曼菌和/或阿克曼菌SGB9228的相对丰度,并为医生提供以下指导:
(i)如果样品中不存在嗜粘蛋白阿克曼菌和/或阿克曼菌SGB9228,则患者在ICI施用之前需要至少用嗜粘蛋白阿克曼菌和/或阿克曼菌SGB9228进行细菌补偿;

根据另一个具体实施方案,用于体外确定癌症患者在施用基于ICI的疗法之前是否需要细菌补偿的方法包括测量来自所述患者的样品中阿克曼菌属的相对丰度,并为医生提供以下指南:
(i)如果样品中不存在阿克曼菌属,则患者在ICI施用前需要用阿克曼菌属的细菌进行细菌补偿,例如用候选物种阿克曼菌SGB9228的细菌,例如以编号CSUR P2261保藏在Collection de souches de l’Unitédes Rickettsies(CSUR)的菌株,或以编号CSUR 4531保藏在CSUR的菌株;
(ii)如果样品中存在低于预定阈值的阿克曼菌属,则患者在ICI施用前不需要任何细菌补偿;和

根据上述方法的具体实施方案,预定阈值对应于在1%与10%之间,例如在3%与6.5%之间或如上所述的任何其他预定阈值的嗜粘蛋白阿克曼菌和/或阿克曼菌SGB9228和/或阿克曼菌属的相对丰度。
根据上述方法的另一个具体实施方案,基于ICI的疗法是基于抗PD1/PD-L1/PD-L2Ab的疗法或基于抗CTLA4 Ab的疗法或其组合(如上所述)。
根据本发明的方法特别适用于体外确定患有非小细胞肺癌(NSCLC)、尤其是非鳞状NSCLC的癌症患者在施用基于ICI的疗法之前是否需要细菌补偿,以及体外确定患有肾癌或患有适合PD1/PD-L1/PD-L2和/或CTLA4阻断的任何癌症的癌症患者在施用基于ICI的疗法之前是否需要细菌补偿。
根据上述方法的特定方面,该方法在作为一线疗法施用的基于ICI的疗法之前进行,以评估患者是否需要细菌补偿来提高他/她对此疗法作出反应的机会。
根据上述方法的另一个具体方面,该方法在作为二线治疗或以上(第三、第四线)施用的基于ICI的疗法之前进行。
发明人令人惊讶地证明,阿克曼菌属的细菌例如嗜粘蛋白阿克曼菌和阿克曼菌SGB9228的肠道驻留是肠道生态系统丰富度的间接标志,如75th百分位数内的嗜粘蛋白阿克曼菌相对丰度与粪便的α多样性(香农多样性指数)之间的关联所示,使得可以测量嗜粘蛋白阿克曼菌和/或阿克曼菌SGB9228和/或阿克曼菌属的水平以快速、轻松地识别肠道生态失调。
因此,特别适用于所有以微生物群为中心的干预措施的本发明的另一方面是一种用于确定个体是否患有肠道微生物群生态失调的方法,包括测量来自所述患者的样品中嗜粘蛋白阿克曼菌和/或阿克曼菌SGB9228和/或阿克曼菌属的相对丰度,其中低于预定阈值的嗜粘蛋白阿克曼菌和/或阿克曼菌SGB9228和/或阿克曼菌属的存在表明不存在肠道微生物群生态失调。

如下面的实验部分所示,发明人可以在经过ATB治疗然后接受来自注定治疗失败的患者的FMT的化身小鼠模型中恢复对PD-1阻断的反应性。他们显示,在无法检测到供体Akk的实验中获得了最佳的有益效果。因此,根据其另一个方面,本发明涉及包含嗜粘蛋白阿克曼菌或阿克曼菌SGB9228的细菌组合物在治疗在他/她的肠道微生物群中不具有嗜粘蛋白阿克曼菌和无阿克曼菌SGB9228的癌症患者中的用途,尤其是用于改善患者对基于ICI的疗法产生反应的机会。
根据本发明的细菌组合物的一个具体实施方案,阿克曼菌细菌来自以编号CSURP2261保藏在Collection de souches de l’Unitédes Rickettsies(CSUR)的菌株。该菌株最初被鉴定为嗜粘蛋白阿克曼菌,最近被重新分类在阿克曼菌SGB9228候选物种中。
根据本发明的细菌组合物的另一个具体实施方案,阿克曼菌细菌来自以编号CSUR4531保藏在Collection de souches de l’Unitédes Rickettsies(CSUR)的菌株。
如果在基于ICI的疗法之前和/或施用根据本发明的粪便微生物组合物或细菌组合物,特别是与使用基于抗PD1/PD-L1 Ab的疗法或基于抗CTLA4 Ab的疗法或其组合的治疗组合,则它们可以是特别有用的。
根据本发明的具体实施方案,上述微生物组合物或细菌组合物用于治疗患有非小细胞肺癌(NSCLC)、尤其是非鳞状NSCLC、或肾癌或适合PD1/PDL-1或CTLA4阻断的任何癌症的患者,如上文所详述的。

实施例
实施例1:肠道嗜粘蛋白阿克曼菌预测晚期非小细胞肺癌患者中对PD1阻断的临床反应
方法
研究设计和治疗
伦理道德问题。辅助研究是根据IRB批准的研究(Oncobiotics*Sponsor ProtocolN:Center for Security and Emerging Technology(CSET)2017/2619,Agence nationalede sécuritédu médicament et des produits de santéID-RCB N:2017-A02010-53https://clinicaltrials.gov/ct2/show/NCT04567446)设计的。该试验是根据良好临床实践指南和赫尔辛基宣言的规定进行的。所有患者均提供了书面知情同意书。已根据ClinicoBiome,Gustave Roussy中已有的Oncobiome H2020模型系统应用了一般数据保护条例程序和匿名规则。所有数据和样品收集均符合监管、道德和欧洲GDPR要求。
患者资格。NCT04567446(一项多中心前瞻性观察研究)旨在评估微生物组组成对接受抗PD-(L)1治疗的晚期NSCLC患者的临床结果的影响。我们在法国的12个学术中心和加拿大的2个学术中心进行招募。患有经病理证实的晚期非鳞状或鳞状NSCLC和0-2的东部肿瘤协作组(ECOG)表现状态评分、适合作为护理标准的ICI并强制提供粪便样品的成年患者是符合资格的。符合资格的患者在铂类化疗方案上进展后接受ICI,无论PD-L1表达如何,使用纳武单抗或阿特珠单抗,或者如果PD-L1≥1%,则使用派姆单抗。鉴于随后在研究累积期间一线环境中一线ICI的批准,接受派姆单抗单药治疗或与铂类化疗联合治疗组合治疗的患者也包括在内,具体取决于PD-L1表达。持续标准护理治疗,直至疾病进展、出现不可接受的不良反应或按照方案完成(2年的ICI)。试验方案(NCT04567446)中列出了完整的资格标准。每个中心在电子病例报告表中输入基线特征,包括ICI启动前最后两个月接受的并行药物的详细清单以及最后一次随访的日期。
假设。样品量计算是根据Routy等人(Routy,Le Chatelier等人,2018)提出的假设(即在肠道微生物群中具有宏基因组学可检测到的Akk(Akk+)的群体中,反应率将高于具有未检测到的Akk(Akk-)的群体中的反应率)中定义为研究者评估的ORR的主要终点进行的。我们认为,有意义的临床差异与ORR增量(从Akk-组中的10%到Akk+组中的20%)相关。考虑到优越性假设,使用程序将功效(power)设置为80%,两侧α水平为5%。因此,我们确定至少需要292名患者才能确认我们的主要目标。
研究终点和评估。在基线和第一年每8-12周以及此后每12-15周直至疾病进展进行计算机断层扫描。使用实体瘤的反应评估标准版本(RECIST)1.1(Eisenhauer等人,2009)评估肿瘤反应。主要终点是研究者评估的客观缓解率(ORR),其定义为具有确认的完全反应(CR)或部分反应(PR)的最佳总体反应(BOR)的患者数量和百分比。最佳总体反应(BOR)被定义为最佳反应指定,在首次治疗剂量的日期和根据RECIST v1.1最初客观记录的肿瘤进展的日期或后续治疗的日期(以先发生者为准)之间记录。对于没有记录的进展或后续治疗的患者,所有可用的反应指定都有助于BOR确定。次要终点包括总体存活率(OS)以及微生物组变量,例如α和β多样性以及属水平上的差异丰度分析。总体存活率定义为从试验纳入到因任何原因死亡的时间。数据库锁中活着的患者的随访被审查到最后一次接触记录的日期。
治疗方式:接受的派姆单抗(每隔21天)或纳武单抗(每隔15天)或阿特珠单抗(每隔21天)注射的次数在8-12周为4+/-2次,在12个月为20+/-4次。
人类粪便样品和宏基因组分析
根据国际人类微生物组标准(IHMS)指南,在每个中心前瞻性收集粪便样品(V1:ICI前,V2:第二次ICI注射前,V3:ICI后3个月,V4:ICI后6个月)。此分析仅考虑基线V1样品,对于无法及时采集的患者,V2样品被视为如(1)中的“基线”。对于宏基因组分析,按照MetaGenoPolis(INRA)France,对粪便进行处理以用于总DNA提取和使用Ion Proton技术测序,如之前报道的(Routy,Le Chatelier等人,2018;Li等人,2014;Nielsen等人,2014)。使用两种不同的管线进行读数的清理、过滤和分类:MetaOMineR和MetaPhlAn 3(Beghini等人,2020,3)。为了确定嗜粘蛋白阿克曼菌是否存在,我们使用了在MetaPhlAn中从四个阿克曼菌候选物种水平基因组箱(SGB)中鉴定出的总共463个遗传标记(SGB9223-38个标记、SGB9224-54个标记、SGB9226-171个标记和SGB9228-200个标记)(Karcher等人,2021)。如表2所示,被描述为SGB9226的嗜粘蛋白阿克曼菌(MucT)菌株类型是我们队列中最常见的物种(>80%的阿克曼菌阳性子集),因此用于计算此文章中主要附图中Akk的相对丰度。

我们发现msp_0025对应于SGB9226,并使用其相对丰度值作为MetaOMineR中嗜粘蛋白阿克曼菌的代表。Derosa等人(Lisa Derosa等人,2020)提供了DNA纯化和宏基因组流程的完整描述。从丰度矩阵开始,仅考虑所有样品的至少2.5%中存在的分类群,然后对原始数据进行标准化和归一化(Sci-Kit-learn版本0.20.3)。
肿瘤RNA测序:
使用之前发表的技术(Fumet等人,2018),从来自主要分析中包括的晚期NSCLC患者以及来自有限阶段的患者的福尔马林固定石蜡包埋(FFPE)的肿瘤中提取总RNA(表3)。按照制造商说明,使用Ribo-Zero(Illumina)从12μl总RNA和TruSeq Stranded Total RNA制备文库。使用BBMAP v38.87修剪测序衔接子并过滤低质量和过短的读长。Kallisto软件(Bray等人,2016)用于针对来自Ensembl数据库第101版的GRCh38 cDNA参考转录组来量化来自RNA-seq数据的转录物丰度。下游分析中仅包含蛋白质编码转录物和基因。每百万的转录物值已用于下游分析。Mann-Whitney测试已根据阿克曼菌群体进行以比较基因表达。使用欧氏距离的PERMANOVA检验已用于评估差异表达基因的子集上各组之间的差异。

临床前研究详情:
小鼠。所有动物实验均符合法国和欧洲法律法规。当地机构动物伦理委员会(Ministère de la Recherche,de l' Superieur er de l'Innovation)批准了所有小鼠实验(许可号:2016-049-4646、2018-020-510263031v3)。小鼠化身研究已获得地方和国家动物监管机构委员会(Ministère de la Recherche,de l'Enseignement Supérieur et de l'Innovation)的批准(Everimmune#13366-2018020510263031v3,APAFIS#17530-201811413352738v2(03/2019-03/2024).APAFIS#21378-201907080848483459)。雌性C57Bl/6和BALB/c分别购自Harlan(France)和Janvier(France)。使用饲养在特定的无病原体条件(SPF)中的8至16周龄的小鼠。所有小鼠实验均在Gustave Roussy Cancer Campus的动物设施中进行,其中动物被饲养在SPF条件下。
细胞培养、试剂和肿瘤细胞系。MC38、MCA-205和B16F10(来自C57BL/6小鼠的同基因的)和4T1细胞系(来自BALB/c小鼠的同基因的)购自ATCC。4T1、MCA-205和MC38细胞在含有10%FCS、2mM L-谷氨酰胺、100UI/ml青霉素/链霉素、1mM丙酮酸钠和MEM非必需氨基酸的RPMI 1640中培养。所有试剂均购自Gibco-Invitrogen(Carlsbad,CA,USA)。B16F10和CT26细胞在含有10%FCS、+100UI/ml青霉素/链霉素+非必需氨基酸的DMEM中培养。所有细胞系均在37℃、5%CO2下培养,并定期检测是否没有支原体污染。
MCA-205、MC38、B16F10和4T1的皮下模型。同基因C57BL/6小鼠分别皮下植入0.8×106个MCA-205、1.0×106个MC38/CT26或3×105个B16F10细胞。同基因BALB/c小鼠皮下植入3×105个4T1细胞。对于肿瘤生长实验,当肿瘤大小达到20至40mm2时,使用抗PD-1mAb(250μg/小鼠;克隆RMP1-14,批号695318A1)或同种型对照(克隆2A3,批号686318F1)对植入肿瘤的小鼠进行腹膜内(i.p.)治疗。小鼠以3天的间隔注射4次抗PD-1mAb。每周用卡尺常规监测肿瘤长度和宽度3次。所有抗体均购自BioXcell,NH,US。
抗生素治疗。用添加到小鼠的饮用水中的含有氨苄西林(1mg/ml)、链霉素(5mg/ml)和粘菌素(1mg/ml)(Sigma-Aldrich)的抗生素溶液(ATB)对小鼠进行治疗。通过将以0.1g/ml重悬于BHI+15%甘油中的粪便颗粒在COS(含5%羊血的Columbia琼脂)平板上在需氧和厌氧条件下于37℃培养48小时证实抗生素活性。简而言之,在粪便微生物移植实验中,小鼠接受3天的ATB,然后在第二天使用动物喂食针通过口服灌胃进行粪便微生物移植。
FMT实验。通过解冻粪便材料进行粪便微生物群转移(FMT)。然后通过口服灌胃将200μL悬浮液转移到ATB预处理的受体中(如上所述)。此外,将另外100μL施加在每只动物的皮毛上。FMT后两周,皮下注射肿瘤细胞,并用抗PD-1mAb或同种型对照处理小鼠,如前所述。我们使用MCA-205纤维肉瘤,因为它通常(在SPF生态平衡小鼠(eubiotic mice)中)对抗PD-1Ab敏感并已在报道于(Routy,Le Chatelier等人,2018)和(Lisa Derosa等人,2020)中的我们之前的化身实验中用作参考小鼠模型,这两篇论文都显示使用MCA-205获得的结果分别在原位TC1肺癌或RENCA模型中得到重复。因此,我们可以相信该MCA-205模型系统在未来的实验中探测FMT或分类学粪便成分的生物学相关性和适用性。

阿克曼菌CSUR p2261的肠道定殖。阿克曼菌CSUR p2261由Institut hospitalo-universitaire Méditerranée Infection,Marseille,France提供。阿克曼菌p2261在使用3个厌氧发生器(BioMerieux)创建的厌氧气氛中、在37℃下在富含5%羊血的Columbia琼脂(COS)平板上生长至少72小时。使用基质辅助激光解吸/电离飞行时间(MALDI-TOF)质谱仪(Microflex LT分析仪,Bruker Daltonics,Germany)对细菌进行鉴定。使用包含PBS中的从使用荧光分光光度计(Eppendorf)在600nm光密度下109CFU/mL的悬浮液中获得的1×108个细菌的100μl悬浮液,通过口服灌胃对ATB预处理的小鼠进行定殖。对每只小鼠进行五次细菌灌胃:第一次注射抗PD-1mAb前的前24小时,和随后在ICI的同一天进行四次。
小鼠粪便DNA提取和微生物群表征。在免疫治疗开始后7至14天之间,收集每只小鼠和每组中的粪便用于宏基因组学。样品储存在-80℃直至处理。小鼠粪便样品的制备和测序在IHU Méditerranée Infection,Marseille,France进行。简而言之,使用两种方案提取DNA。第一个方案包括分别使用玻璃粉和蛋白酶K进行物理和化学裂解,然后使用Macherey-Nagel DNA组织提取试剂盒(Duren,Germany)进行处理。第二个方案与第一个方案相同,增加了糖蛋白裂解和去糖基化步骤。对所得DNA进行测序,靶向16S rRNA基因的V3-V4区域。在工作站DELL T7910(Round Rock,Texas,United States)上使用Mothur管线v.1.39.5分析原始FASTQ文件以用于质量检查和过滤(测序错误、嵌合体)。将原始读数过滤并聚类为操作分类单元(OTU),然后消除数量较少的OTU(直到5个读数),并使用归一化参数和用于比对的SILVA rDNA数据库v.1.19以97%成对同一性从头挑选OTU。使用Python v3.8.2对已识别的OTU进行统计分析时,得到≥2.5%的流行率阈值。每个OTU中最具代表性和最丰富的读数(如使用Mothur v.1.39.5的上一步所证明)使用国家生物技术信息中心(NCBI)Blast软件(ncbi-blast-2.9.0)和最新的NCBI 16S微生物数据库(ftp://ftp.ncbi.nlm.nih.gov/blast/db/)进行核苷酸Blast。在每个分类单元水平(门、纲、目、科、属、种)建立细菌相对丰度矩阵,用于后续的多元统计分析。
统计分析
在人类中,数据矩阵首先使用来自Sci-Kit学习包v0.20.3的QuantileTransformer和StandardScaler方法进行标准化随后归一化。使用output_distribution='normal'选项进行标准化将每个变量转换为严格的高斯分布,而归一化导致每个标准化变量具有零的均值和一的方差。标准化和随后归一化的这两个步骤确保了具有不同动态范围的变量(例如细菌相对丰度)的正确比较。对于微生物群分析,使用SciKit-学习软件包v.0.4.1在物种水平上计算α多样性(样品内多样性)的测量值,例如丰富度和香农指数。β-多样性(样品间多样性)的探索性分析是使用Bray-Curtis相异性度量计算的,并在主坐标分析(PCoA)中表示,以及使用比较多变量样品单位组的方法(相似性分析-ANOSIM,排列多变量方差分析-PERMANOVA)来评估数据点聚类中的显著性。ANOSIM和PERMANOVA在999次排列后自动计算,如SciKit-学习包v0.4.1中实现的那样。我们实施了偏最小二乘判别分析(PLS-DA)和随后的变量重要性图(VIP)作为监督分析,其中VIP值(数量级)用于识别最具判别力的细菌物种。所有分析均在Python v3.8.2环境中进行。通过效应大小的线性判别分析(LEfSe)进行单变量差异丰度分析(Segata等人,2011)。我们使用两种不同的多元差异丰度方法进一步支持差异丰度物种;ANCOM-BC(Lin et Peddada 2020)和MaAsLin2(Mallick等人,2020),其中包括诸如年龄、性别、BMI、队列和测序批次的协变量。法国数据保护立法第8条(国家信息与自由委员会[CNIL])禁止对种族和民族起源进行分析。原始测序计数是根据物种水平MetaPhlAn3相对丰度通过将这些值乘以每个样品的读取总数并将这些值用于默认参数的ANCOM-BC(v.1.0.1)来估算的,其中文库大小截断值为500次读取,并且无结构零检测。Masslin2(v.1.4.0)使用经过总和缩放(TSS)标准化的Logit转换相对丰度和使用感兴趣的变量作为固定效应来运行。
使用Kaplan-Meier方法估计生存曲线,并在单变量分析中与对数秩检验(Mantel-Cox方法)进行比较。使用Cox回归模型进行多变量分析,以确定针对其他临床病理特征进行调整的OS的HR和95%置信区间(CI)。使用cox.zph R函数测试Schoenfeld残差的趋势来检查比例风险假设。当测试对于变量具有统计显著性时,在模型中引入其与时间的交互作用并使用tt(时间变换)函数以不同的函数形式(线性、指数、对数和罚样条)进行测试。使用基于多元Cox模型的网格搜索算法获得了定义不同预后组的每种细菌物种的最佳截断值,以考虑潜在的混杂因素(年龄、性别等)。每个物种的网格是由非零流行率值的分布的百分位数定义的。选择与具有较好Akaike信息准则(AIC,越低越好)的模型对应的截断值作为最优截断值。
所有测试均为双侧测试,统计显著性设置为p值<0.05。使用GraphPad Prism 7和R软件(http://www.R-project.org/)进行统计分析。
在小鼠中,所有肿瘤生长曲线均使用Kroemer实验室开发的软件进行分析:https://kroemerlab.shinyapps.io/TumGrowth/。小鼠的组间比较、全局比较使用Kruskall-Wallis检验进行,使用Dunn检验进行事后多重比较。最后,将源自小鼠实验(每个实验6只小鼠)的天然肿瘤生长数据针对每个时间点(T0至T8)进行平均,然后在第一个时间点进行纵向标准化,以具有1的共同的起始值。然后,所有平均和标准化的肿瘤值都用折叠比表示(FR,图9E),并进行以2为底的对数化,以增强肿瘤生长的增加(灰色代码(越暗越大)上的星号)或减小(灰色代码(越暗越大))。基于Bray-Curtis距离的分层聚类分析(HCA)在纵向FR数据上实施以定义对阿克曼菌p2261的反应者(R)的分支(浅灰色簇),或对阿克曼菌p2261的非反应者(NR)的组(深灰色簇)。所有报告的p值均经过10%的Benjamini-Hochberg两阶段错误发现率(FDR)。
结果
嗜粘蛋白阿克曼菌与临床结果之间的关联
从2015年12月到2019年11月,本研究中共有493名患者接受了入组筛选,其中338名患者符合纳入标准,提供了至少一份基线(V1和/或V2)粪便样品用于特征分析(图4)。使用鸟枪法宏基因组测序,我们确定Akk在131名(39%)患者中可检测到,而在207名(61%)名患者中不存在,其中使用扩展的MetaPhlAn分析以识别主要Akk物种水平基因组箱(SGB9226spp(Karcher等人,2021),表5)。基线特征在可检测到的Akk(Akk+)组和不可检测到的Akk(Akk-)组患者之间在性别、ECOG表现状态、吸烟史、肿瘤组织学、体重指数(BMI)、PD-L1表达和治疗线方面良好平衡(表5A)。然而,Akk+组中对于老年人存在不具统计学意义的趋势(66岁相对64岁,p=0.08)。总共69名患者(20%)在ICI开始前60天接受了抗生素(ATB),Akk+和Akk-组中的频率(p=0.68)和方案相似(表5A)。在ATB类别中,两组中最常使用β-内酰胺(表5B,Akk+中的80%相对Akk-中的64%,p=0.33)。

当考虑我们队列中的Akk+与Akk-组时,客观反应率(ORR)分别为28%和18%(图1A,p=0.04)。对于Akk+组,部分反应(PR)、稳定疾病(SD)和进行性疾病(PD)率分别为28%、28%和44%,对于Akk-组分别为18%、31%和50%。当考虑单独接受免疫治疗作为一线治疗的患者亚组(1L IO,n=86)时,Akk+相对Akk-的ORR分别为41%和19%(图1B,p=0.016)。值得注意的是,对于整个患者队列(无论采用何种治疗方案),Akk+组的中位总体存活率(mOS)为18.8个月,而Akk-组为15.4个月(HR=0.72;(95%CI:0.73-1.62);p=0.03,图1C,表5A)。对于Akk+组,≥2L NSCLC患者的中位OS为18.8个月,而Akk-组中为13.4个月(HR=0.70;(95%CI:0.50-0.98);p=0.04,图1D,表5A)。对于1L IO患者,59%的Akk+患者在12个月后仍然存活(OS≥12),而只有35%的Akk-个体是长期生存者(图1E,p=0.04)。我们在使用MetaOMineR管线(LeChatelier等人,2013)时得出了相同的结论(图5A-B左图)。
为了建立Akk(及其生态系统)的存在与对ICI的反应之间的因果关系,我们回顾性地进行了收集29个实验的临床前meta分析,其中在化身小鼠模型中跟踪肿瘤生长动力学(Routy,Gopalakrishnan等人,2018)。这些接受者接受了ATB治疗,然后接受了来自26名不同NSCLC患者的粪便微生物移植(FMT)(表6)。然后,小鼠被植入同基因原位MCA-205肉瘤(对抗PD-1抗体敏感的代表性肿瘤模型,如前所述(Routy,Le Chatelier等人,2018;LisaDerosa等人,2020)),随后进行PD-1阻断(图5C-E)。如我们的临床队列中所观察到的,小鼠对治疗的耐药性与FMT实验中可检测到的Akk的不存在相关(图5C-D)。
因此,在对338名晚期NSCLC患者进行的第二项独立前瞻性研究中,我们使用两种不同的宏基因组学管线在人类中以及在小鼠中验证Akk的存在与接受ICI的NSCLC患者中的较高ORR和较长OS相关。

Akk、肠道生态系统和肿瘤微环境
鉴于肠道微生物群中的组成差异与肿瘤免疫景观之间已知的相关性(Routy,LeChatelier等人,2018;Gopalakrishnan等人,2018),该相关性可能因组织学类型而异(Meng等人,2019),我们解决了粪便Akk检测和肿瘤组织学(鳞状相对非鳞状NSCLC)之间的相互作用。诊断时粪便中Akk的存在对组织学没有影响(非鳞状NSCLC相对鳞状NSCLC(对于多变量分析,表5A中p=0.556)。
为了揭示Akk驱动的肿瘤内转录组差异,我们使用被诊断为局部晚期或转移性NSCLC后从Akk+(n=22)和Akk-(n=22)患者收集的可用的肿瘤活检在患者子集中进行了肿瘤RNA测序(表3)。Oncomine免疫反应研究测定(Hwang等人,2020)的一组395个免疫相关基因中Akk+和Akk-组之间的显著基因表达差异的监督分析揭示了一组与对于肺癌中的PD-1阻断的反应相关的差异表达基因(图1F),例如具有激活(CD4、CD74、Vcam-1,与肿瘤巢内T淋巴细胞的粘附和转迁移相关(Nakajima等人,s.d.)、与细胞溶解活性相关的颗粒酶H丝氨酸蛋白酶Gzmh)和耗竭(Ctla4、Fas、Tigit、Havcr2)标记以及IFN指纹(Ccr5、Cxcl9、Cxcl10、Tdo2和IFN诱导鸟苷酸结合蛋白1)的CD4+T辅助细胞(图1G-H)。这些结果支持Akk可以促进Th1细胞诱导或再循环进入肿瘤微环境的可能性,如先前在小鼠模型中所显示的(Routy,LeChatelier等人,2018;Vétizou等人,2015)。
接下来,我们检查了Akk+相对Akk-患者的肠道微生物群中的组成分类差异。我们发现与Akk-患者相比,Akk+患者的香农多样性指数显著增加(图6A上图,p<0.0001),并且两组之间的总体微生物群落组成存在差异(图6A下图,p=0.0001)。使用LEfSe(线性判别分析效应大小)分析来调查2组之间物种相对丰度的差异,我们发现了瘤胃球菌科(Ruminococcacae)(布氏瘤胃球菌(Ruminococcus bromii)、双环瘤胃球菌(Ruminococcusbicirculans)、酸奶瘤胃球菌(Ruminococcus lactaris))、毛螺菌科(Lachnospiraceae)(惰性真杆菌(Eubacterium siraeum)、挑剔真杆菌(Eubacterium eligens))家族成员和别样杆菌属的几个种(狭边别样杆菌(Alistipes inops)、芬氏别样杆菌(Alistipesfinegoldii)、苍莽别样杆菌(Alistipes indistinctus)、沙氏别样杆菌(Alistipesshahii))如之前报道的那样(Hakozaki等人,2020)在Akk+粪便中富集,而Akk-患者的粪便在小韦荣菌(Veillonella parvula)、放线菌和梭菌(无害梭菌(Clostridiuminnocuum)、哈氏梭菌(Clostridium hathewayi))方面是过度丰富的(图6B),如在预后不良的NSCLC患者(Tsay等人,2020)以及对ICI耐药的肾癌患者(Lisa Derosa等人,2020)的下气道微生物组中所描述的。
此外,在Akk+组中,我们发现OS≥12个月的患者与OS<12个月的患者之间的总体微生物组成存在显著差异,但在Akk-患者中未观察到这种差异(图2A,p=0.009相对p=0.07)。在OS≥12个月的Akk+组相对OS<12个月的那些中的LEfSe分析显示OS≥12的患者中毛螺菌科家族成员(产甲酸多尔氏菌(Dorea formicigenerans)、长链多尔氏菌(Dorealongicatena)、直肠真杆菌(Eubacterium rectale)、霍氏真杆菌(Eubacterium hallii)、肠道罗氏菌(R.intestinalis)、陪伴粪球菌(Coprococcus comes))的相对丰度增加。相反,属于γ-变形菌纲(大肠杆菌)、梭菌纲(成乳钌细菌(Ruthenibacterium lactatiformans))或杆菌纲(例如格氏乳杆菌(Lactobacillus gasseri)、副格氏乳杆菌(Lactobacillusparagasseri)、口腔乳杆菌(Lactobacillus oris)、阴道乳杆菌(Lactobacillusvaginalis)、副血链球菌(Streptococcus parasanguinis))的物种以及小韦荣菌(Veillonella parvula)在OS<12的患者中占主导地位(图6C)。
总而言之,这些结果表明Akk的存在与NSCLC患者肠道微生物群和肿瘤微环境中重要的、潜在的预后相关的变化有关。
基于Akk的相对丰度的临床结果的分层
我们意外地发现Akk+组中OS<12个月的患者中Akk的过度呈现,这表明Akk的相对丰度可能比其绝对存在或不存在对预后的影响更大。接下来,我们检查序数变量而不是分类变量(Akk+相对Akk-)以分析Akk的临床意义。事实上,Akk+群体中Akk的相对丰度范围为0.035%至66.210%。使用定位OS≥12或OS<12的患者的Akk相对丰度的核密度估计,我们注意到携带Akk高的患者(相对丰度>75th百分位数(4.656%))确实在OS<12个月内聚集(图2B-C左图,p=0.005)。为了确认调整其他风险因素后的这一观察结果,我们使用监督方法来统计上定义Akk的最佳截断值以区分具有不同预后的两个假设的Akk+患者组。我们利用基于多变量Cox模型的网格搜索算法(针对年龄、性别、ECOG表现状态、ATB、组织学、PD-L1和治疗线进行调整),并以Akaike信息标准作为性能指标来确定对应于4.799(该队列的第77个百分位数)的这个最佳截断值(图2C左图)。在338名患者的整个队列中,我们观察到只有9%的患者(Akk+亚组的23%)落入Akk>4.799的“相对过度丰富”类别(下文称为“Akk高”)(图2C右图)。
此外,我们发现与Akk-或Akk高样品相比,Akk低中的香农多样性显著增加(图7A,p=0.00003),其中整体微生物群落显示Akk-相对Akk低之间以及Akk低相对Akk高患者之间的明显分离(图7B-C,分别为p=0.0001和p=0.0003)。对分类学粪便成分的变量重要性图(VIP)判别分析表明,Akk中异常富集的生态系统(Akk高)中梭菌属(共生梭菌(Clostridiumsymbosium)、无害梭菌(Clostridiuminnocuum)、闪烁梭菌(Clostridium scindens)、博尔特梭菌(Clostridium boltae)、梭状梭菌(Clostridium clostridioforme))的物种以健康共生体(普氏栖粪杆菌(Faecalibacter prausntzii),陪伴粪球菌(Coprococcus comes),直肠真杆菌(Eubacterium rectale),产甲酸多尔氏菌(Dorea formicigenerans),长链多尔氏菌,扭链瘤胃球菌(Ruminococcus torques),图7D-E)的代价过度呈现。
根据Akk相对丰度使用三分分层的Kaplan Meier生存曲线出现分歧(对数秩检验p=0.0007,图2D),其中与Akk高(27.2个月相对7.8个月,Cox调整的HR:0.38;95%CI:0.22-0.65,p=0.0005)和Akk-患者(27.2个月相对15.5个月,经调整的HR:0.63;95%CI:0.44-0.91,p=0.0150)相比,Akk低患者的中位OS显著更长。多变量Cox回归分析中考虑的其他风险因素的HR如图2E和表4所示。毫不奇怪,ECOG表现状态≥1是该队列的另一个独立预后因素(Desai等人,2016)。比例风险假设仅对于ATB存在疑问(Schoenfeld残差趋势检验p=0.016),但无法识别与时间的统计显著相互作用。此外,ATB暴露与较短的OS相关(在单变量分析中,p=0.009(表4B、表7),在多变量分析中,p=0.088(表7)。
我们还分析了235名具有肿瘤PD-L1的可用肿瘤表达的晚期NSCLC患者中Akk和PD-L1之间的相互作用(表5A,图4)。我们根据PD-L1表达和Akk粪便检测将队列分为六组。多变量Cox回归分析表明,Akk比PD-L1更能决定NSCLC预后(235名患者中p=0.012,图2F)。
总体而言,我们得出的结论是,考虑将患者分为Akk-、Akk低或Akk高个体的三分分层可能是比二分(Akk-相对Akk+)划分更准确的总生存的独立预后因素(多变量Cox模型的似然比检验:p=0.0009)。肠道中Akk的“正常水平”的存在(Akk低)可被视为宿主肠道健康状况的替代指标。Akk推翻了PD-L1作为NSCLC患者中对ICI的反应的预测生物标志物。

ATB使用对Akk相对丰度和生存结果的影响
Akk高水平和ATB暴露被认为是与接受ICI治疗的NSCLC患者中的较短OS相关的独立变量。鉴于这些观察结果,我们首先将患者关于Akk的二分分类(Akk-相对Akk+)与他们之前的抗生素暴露史结合起来以将患者分为四组。与其他三组相比,没有ATB暴露的Akk+组显示出最强的临床获益(中位OS为23.0个月)(图8A)。下一个预后良好的组落入无ATB暴露的Akk-组,中位OS为16.0个月。相比之下,暴露于ATB显示最短的OS(mOS为约9个月,图8A,p=0.017)。因此,ATB倾向于降低Akk+组的α多样性(图8B-C)。此外,ATB暴露以牺牲在未服用ATB的Akk+组中也过度呈现(未显示)的健康相关的细菌(产气柯林斯菌(Collinsellaaerofaciens)、长链多尔氏菌、产甲酸多尔氏菌、真细菌物种CAG 38)(Routy,Gopalakrishnan等人,2018;Benevides等人,2017)为代价富集了Akk+组中的γ-变形菌纲(大肠杆菌)、梭菌纲(鲍氏梭菌(Clostridium bolteae)、成乳钌细菌)和产生H2S的细菌(沃氏嗜胆菌(Bilophila wadsworthia))(未显示),如前所述(Lisa Derosa等人,2020)。
为了建立ATB使用与Akk相对过度丰富之间的关联,我们比较了使用ATB的NSCLC患者相对未使用ATB的患者中Akk高粪便的百分比(图2G),并根据首次施用ICI之前的ATB暴露情况在两个亚组中使用三分分类(Akk-、Akk低相对Akk高)绘制了Kaplan Meier OS曲线(图2H)。我们发现,ATB使用者相对非使用者中Akk高患者的比例急剧增加(40%相对19%,p=0.023,图2G)。我们证实,暴露于ATB的患者在诊断时的粪便Akk的过度丰富与较低的总体存活率(尽管接受了ICI治疗)相关(图8,图2H下图)。在无ATB的受试者中,与Akk低个体相比,Akk高患者表现出减少的对ICI的获益,但并不比Akkneg亚组更差(图2H上图)。
总而言之,这些结果表明,ATB暴露是对ICI的存活的负面预测因子,与Akk的过度丰富(Akk高)和梭菌属几个种(鲍氏梭菌、Lachnoclostridium)的相对优势有关。

我们使用各种统计方法,例如线性判别分析效应大小(图6)、ANOVA模型的火山图(图3A)和MaAsLin多元统计框架(表8)来比较Akk+相对Akk-粪便之间的分类成分变化以鉴定16种细菌物种,其中14种与Akk粪便流行率呈正相关,2种与Akk粪便流行率呈负相关,这独立于诸如年龄、性别、BMI、治疗线的其他混杂因素(图3A,表8)。接下来,我们进行了Cox逻辑回归多变量分析以利用其各自相对丰度的三分分布(根据上面定义的截断值)来确定每种细菌对患者临床结果的影响。除Akk外,很少有细菌(除了已被描述介导免疫调节功能的霍氏真杆菌、青春双歧杆菌(Bifidobacterium adolescentis)、排泄物毛螺旋菌、产丁酸肠杆菌(Intestinimonas butyriciproducens)或无害梭菌(Elkrief等人,2019;Seo等人,2015;Atarashi等人,2011))也与ICI治疗后延长(图3B、图3D)或减少(图3F、表8)的存活相关,与对于Akk观察到的情况相反,具有“线性剂量”反应。存在一个附加值以考虑Akk以及霍氏真杆菌或青春期双歧杆菌的协调一致的存在,以准确预测338名NSCLC患者的该队列中的ORR和/或mOS(图3C、E)。因此,这些发现表明Akk及其附带共生可能参与患者的ICI介导的临床获益。

Akk通过改变微生物组来挽救对PD-1阻断的反应
为了试图在Akk和/或其附属生态系统与患者临床结果之间建立联系,我们转向了上文描述的我们的携带微生物群人源化身肿瘤的小鼠模型。如上文所述和评论(图5C-E),小鼠对抗PD-1抗体的抗性与缺乏Akk的FMT相关。接下来,我们分析了在接受Akk+相对Akk-FMT的两组动物中口服补充称为阿克曼菌p2261的免疫原性阿克曼菌菌株(Routy,LeChatelier等人,2018)所带来的补偿效应。在特定的无病原体条件(SPF)中饲养(在图9A-B中表示为FMT-)并接种相同的肿瘤细胞系的小鼠用作“小鼠生态平衡”的对照。事实上,只有在Akk-FMT的情况下,我们才能恢复对PD-1阻断的反应性(图9B-C)。接下来,目的是分析口服补充阿克曼菌p2261是否会改变受体小鼠的宿主微生物组,以及宿主生态系统的重塑是否与外源阿克曼菌p2261的临床前益处相关。为此,我们连接了与首先用来自29名个体NSCLC患者的粪便转移(表S3)和然后用抗PD-1抗体治疗(图9A)并在用阿克曼菌p2261口服喂养之前和之后收集受体粪便的BALB/c(CT26、4T1)和C57BL/6(B16F10、MCA-205、MC38)小鼠同基因的所有肿瘤模型。对生态平衡条件下PD-1阻断的效果标准化的具有或不具有外源性阿克曼菌p2261的抗PD-1抗体的肿瘤生长动力学之间的比率(SPF相对FMT)的无监督分层聚类表明外源性阿克曼菌p2261在大约50%的情况下是有效的(图9D、图9E左图)。我们检测到对外源阿克曼菌p2261的反应者(R)相对无反应者(NR)的受体微生物群的分类学差异(图9E右图)。与NR小鼠相比,R小鼠在含有Akk+粪便的人类中具有图3A中注释的宏基因组物种(即产丁酸肠杆菌、排泄物毛螺旋菌)的相对过度呈现,相比之下NR小鼠倾向于富集拟杆菌属的几个种(图9E右图)。
总而言之,用Akk-人类粪便材料转移的化身小鼠表现出对PD-1阻断的肿瘤抗性的表型,但当阿克曼菌p2261可以将微生物组转向有利的Akk相关附属生态系统时,被阿克曼菌p2261拯救。
讨论
在这里,我们报告了一项前瞻性、多中心研究的结果,该研究基于对接受PD-1阻断治疗的晚期NSCLC患者的肠道微生物群的特征分析。Akk的相对丰度与临床获益相关,临床获益定义为ORR和存活的增加,同时考虑到主要的微生物群相关混杂因素(年龄、性别、BMI、治疗线)。这种肠道细菌的预后意义通过表明Akk与ICI治疗的晚期NSCLC的预后显著相关(独立于年龄、性别、ECOGPS、ATB使用和PD-L1)的多变量分析和相互作用研究得到验证。Akk的肠道驻留是肠道生态系统丰富度的代表,如在第77个百分位数内的相对丰度(Akk低<4.799%)下Akk与粪便α多样性(香农多样性指数)的关联所示。这些结果扩展了之前在较小队列的NSCLC患者中进行的观察(Routy,Le Chatelier等人,2018;Hakozaki等人,2020),并提供了肠道微生物群多样性和组成,特别是Akk的相对丰度的证据,提供了相关信息来预测适合ICI的NSCLC患者的存活。
我们的研究是试图验证Akk作为ICI的新预后因素的最大宏基因组学前瞻性分析。我们的研究符合预先指定的统计显著性标准,其在考虑所有(大多数是2L)338名NSCLC患者时设定为Akk-和Akk+患者之间的10% ORR增加(ORR从18.2%增加到28.2%,图1A)。此外,我们还观察到Akk-和Akk+之间的>10%ORR增加(ORR从19%增加到41%,图1B),其中在1LNSCLC患者中具有mOS优势(图1E)。此外,我们的研究将肠道微生物群组成与肿瘤微环境景观联系起来,突出了适应性免疫和IFN指纹的基因产物的增加的转录。最后,我们在小鼠中验证了Akk-粪便赋予对PD-1阻断的抵抗力。
除了在更大且同质的队列中前瞻性地验证该假设之外,我们还报告Akk不仅与增加的α多样性相关,而且与与以瘤胃球菌(普氏栖粪杆菌、酸奶瘤胃球菌)和毛螺菌科(产甲酸多尔氏菌&长链多尔氏菌、直肠真杆菌&霍氏真杆菌、粪罗氏菌&肠道罗氏菌)家族成员以及青春双歧杆菌、产丁酸肠杆菌等为代表的健康或免疫原性状态相关的独特细菌群落相关。这些发现协调了多项研究、地理分布和测序技术的结果,因为此前有报道称,粪杆菌、瘤胃球菌和双歧杆菌在北美、日本和韩国的黑素瘤和NSCLC癌症患者中富集,并且具有良好的结果(Routy,Gopalakrishnan等人,2018;Gopalakrishnan等人,2018;Hakozaki等人,2020),(Lee等人,2021)。此外,即使在化身肠道人源化小鼠模型中,对外源阿克曼菌p2261的反应者也将其微生物组转向属于Akk+生态系统的上述一些物种的过度呈现。
证实与反应相关的细菌多样性和共生体的临床意义,我们的结果验证了将ATB使用与不良临床结果联系起来的越来越多的证据(Elkrief等人,2019)。除了消耗与生存相关的有利菌属(例如瘤胃球菌)(Hakozaki等人,2020)外,ATB的使用倾向于使肠道微生物组组成偏向于先前与促炎或免疫调节途径相关的有害细菌(例如大肠杆菌和鲍氏梭菌)(Seo等人,2015),支持了之前在适合ICI的肾细胞癌中的发现(Lisa Derosa等人,2020)。令人惊讶的是,除了作为独立的负面预后因素外,ATB还促进Akk(Akk高)过度丰富到与不良预后相关的第77个百分位数水平以上。事实上,ATB使用使呈现粪便Akk高表型的个体的比例增加了一倍。Akk>4.799的过度丰富的这种表型特征与属于梭菌属的IV和XIVa簇的梭菌属物种(鲍氏梭菌、无害梭菌、天冬酰胺梭菌(Clostridium asparagiforme)、闪烁梭菌、共生梭菌)(其已知在结肠固有层中维持产生IL-10的Treg(Atarashi等人,2011))的优势相关。
除了ATB的使用之外,Akk>4.799(Akk高)的过度丰富与Akk<4.799的“正常”相对丰度相比与较短总体存活率相关,这可能反映了这些晚期癌症患者中肠道屏障的潜在病理生理学紊乱。生态系统中嗜粘蛋白阿克曼菌的高相对比例或次优势与病理生理学失败有关(例如神经性厌食症(Ruusunen等人,2019)、GVHD(Shono等人,2016)、衰老(van der Lugt等人,2019)、代谢障碍(Depommier等人,2019b)、HIV感染(Ouyang等人,2020)、致病共栖菌谱(pathobionts)(Huck等人,2020)或肝损伤(Wu等人,2017))。因此,在ATB使用导致的或有利于ATB使用的肠道损伤的情况下,Akk可能构成正在进行但不完善的肠道修复的生物标志物。值得注意的是,我们未能观察到与调查其他细菌的相反临床结果相关的类似三分分布(图3)。
因此,我们得出结论,Akk相对丰度可以代表接受使用PD-1阻断的免疫治疗的患者的预后良好或不良的可靠生物标志物。对于基于鸟枪宏基因组学(而不是16S rRNA)测序的风险分层的I-O患者来说最重要的是除了ATB使用之外精确量化Akk的相对丰度以及发现最佳的生物标志物的包括NSCLC患者的前瞻性试验中的PD-L1表达。
目前正在评估调节微生物群的治疗策略(例如FMT或共生体),以增强ICI反应或规避对ICI的原发性耐药性,尽管没有根据患者的生态失调程度进行分层(Baruch等人,2020)。在这里,我们提供的临床前数据表明,Akk可以在治疗上绕过因缺乏内源性Akk的FMT而赋予的对抗PD-1阻断的耐药性(图9和(Routy,Gopalakrishnan等人,2018;Routy,LeChatelier等人,2018))。口服补充外源性阿克曼菌(阿克曼菌p2261)在其使受体微生物组转向健康状态时特别有效(图9D-E)。最近的一项临床前研究表明,Akk可以以Toll样受体2依赖性方式提高重组白细胞介素2的抗癌作用(Shi等人,2020)。在患者中,使用Akk的食物补充是安全的,并且在前驱糖尿病的情况下降低胰岛素抵抗和血脂异常(Depommier等人,2019a)。总而言之,我们推测用冻干封装的阿克曼菌进行治疗性补偿将有益于未暴露于ATB且缺乏内源性Akk的患者亚组。相比之下,复杂的多微生物联合体或粪便微生物移植可能最适合具有先前ATB暴露的患者。
因此,我们的研究为开发评估肠道生态失调的诊断工具以进行常规肿瘤管理提供了强有力的依据,以及提供了用于设计以微生物群为中心的干预措施以避免NSCLC患者中对ICI的主要耐药性的框架。
实施例2:肠阿克曼菌SGB9228预测晚期非小细胞肺癌患者对PD1阻断的临床反应,并增强临床前模型中PD-1阻断的功效
背景:
几个小组最近发表的研究表明,阿克曼菌存在多个分支(Becken等人,mBio 2021)或物种水平(Karcher等人,Genome Biol.2021;Guo等人,BMC Genomics 2017)。事实上,正如Karcher等人所建议的,人类、小鼠和非人类灵长类肠道微生物群中存在总共五种阿克曼菌候选物种,包括嗜粘蛋白阿克曼菌,其中四种尚未得到充分研究和未表征(阿克曼菌SGB9223、SGB9224、SGB9227和SGB9228)。值得注意的是,这些菌株的16S rRNA基因序列表现出高度相似性,不同候选物种菌株的16S rRNA基因序列从未差异超过2%。阿克曼菌候选物种在宿主之间的流行率方面存在很大差异。嗜粘蛋白阿克曼菌是迄今为止所有宿主中最常见的候选物种,在34%的成年人中检测到,并且在实验室喂养的小鼠中达到54%的最高流行率。其他候选物种在所有宿主中以较低的流行率(<25%)被检测到。
阿克曼菌SGB9228物种在推动癌症患者中PD1阻断的临床疗效方面的相关性:
我们能够提取基因组标记并分析我们队列中这5个SGB中的4个,因此可以更好地概括阿克曼菌基因组多样性,如下所示。如实施例1中所述(表2,显示了338名NSCLC癌症患者队列中阿克曼菌各个亚种的流行率),SGB9226(嗜粘蛋白阿克曼菌MucT)是我们队列中最常见的物种(>80%的阿克曼菌阳性子集)。
值得注意的是,对于分支SGB9226(嗜粘蛋白阿克曼菌的代表)和SGB9228报告的流行率在一般人群中与Karcher等人报告的流行率没有差异,证明特定进化枝的存在并不参与NSCLC的病理过程(Karcher等人,Genome Biol.2021)。最后,我们比较了SGB9226相对SGB9228进化枝的存在的预测价值,以推动ICI在NSCLC癌症患者中的临床疗效。考虑到将患者分为嗜粘蛋白阿克曼菌-、嗜粘蛋白阿克曼菌低或嗜粘蛋白阿克曼菌高个体的三分分层可能是比二分法(嗜粘蛋白阿克曼菌-相对嗜粘蛋白阿克曼菌+)划分更准确的总生存独立预后因素,同样的分析使用SGB9228进行。如图10所示,阿克曼菌SGB9228的行为方式与嗜粘蛋白阿克曼菌相同,尽管此分析中包含的SGB9228+患者数量明显较少。总而言之,肠道中“正常水平”的嗜粘蛋白阿克曼菌(嗜粘蛋白阿克曼菌低)或阿克曼菌SGB9228低的存在可被视为宿主肠道健康的替代指标(图10)。

因此,对阿克曼菌p2261进行了深入表征,以阐明阿克曼菌p2261的系统群和各自的表型性状。进行了宏基因组组装的基因组的全基因组系统发生,并确定阿克曼菌p2261属于阿克曼菌SGB9228种,其与嗜粘蛋白阿克曼菌不同。
将美国典型培养物保藏中心(ATCC)提供的嗜粘蛋白阿克曼菌型菌株基因组与阿克曼菌p2261基因组进行比较。使用CheckM的lineage_wf函数(Parks等人,GenomeRes.2015)评估组装的分离株的完整性。然后对组装好的重叠群进行处理,并使用Parsnp作为核心基因组比对器来比对多个微生物基因组的核心基因组。使用MUMmer(Kurtz等人,Genome Biol.2004)评估分离株之间的平均核苷酸同一性(ANIm)。如表9所示,虽然这些菌株在16S rRNA基因序列上表现出高度相似性,但阿克曼菌p2261(基因组G1284)和嗜粘蛋白阿克曼菌ATCC之间的全基因组平均估计核苷酸同一性低于90%,这表明阿克曼菌菌株p2261和嗜粘蛋白阿克曼菌在遗传上是不同的。

阿克曼菌SGB9228菌株p2261和4531在临床前模型中安全地增强PD-1阻断的功效:
对阿克曼菌菌株(包括菌株3284、5126、5801、4531和2261)安全性增强免疫检查点阻断剂的临床前疗效的能力进行了深入评估。研究了阿克曼菌菌株在移植了NR NSCLC癌症患者的粪便材料的MCA205荷瘤小鼠中安全改善PD1阻断的功效的能力(图11)。阿克曼菌菌株2261和4531有效增强PD-1阻断的抗肿瘤功效。
我们还监测了之前用不同的阿克曼菌菌株(包括菌株3284、5126、5801、4531和p2261)进行脉冲处理的骨髓来源的树突细胞(BMDC)的IL-12细胞因子的分泌(图12)。阿克曼菌菌株2261和4531有效促进BMDC的IL-12分泌。
识别阿克曼菌属物种的特异性引物的设计:
设计引物用于特异性鉴定嗜粘蛋白阿克曼菌(SGB9226)和阿克曼菌SGB9228物种(图13)。使用Primer-BLAST(Ye等人,BMC Bioinformatics,2012)设计引物,并使用RefSeq上存放的代表性基因组检查特异性。

Claims (17)
