背景技术

过氧化物酶体增殖激活受体(PPAR)是核受体,存在三种不同的亚型:PPARα、PPARβ/δ和PPARγ。它们与类视黄醇X受体(RXR)形成异二聚体,在配体结合后作为转录调节子发挥作用。依赖于组织配体和辅助因子,PPAR亚型发挥多种功能(Wagner,K.D.&Wagner,N.Peroxisome proliferator-activated receptor beta/delta(PPARbeta/delta)actsas regulator of metabolism linked to multiple cellularfunctions.Pharmacology&therapeutics 125,423-435,doi:10.1016/j.pharmthera.2009.12.001(2010))。PPARα主要在棕色脂肪组织、肠、心脏、肝脏、肾脏中表达,而PPARγ在免疫细胞、肠道、白色和棕色脂肪组织中表达,并且PPARβ/δ普遍表达。PPARβ/δ是PPAR家族的促血管生成成员,在血管细胞中表达(Bishop-Bailey,D.Peroxisomeproliferator-activated receptors in the cardiovascular system.Br J Pharmacol129,823-834,doi:10.1038/sj.bjp.0703149(2000))。缺少细胞质前列环素受体的HEK293细胞中PPARβ/δ激活是促细胞凋亡的(Hatae,T.,Wada,M.,Yokoyama,C.,Shimonishi,M.&Tanabe,T.Prostacyclin-dependent apoptosis mediated by PPAR delta.The Journalof biological chemistry 276,46260-46267,doi:10.1074/jbc.M107180200(2001))。后来的研究报道,内皮细胞中PPARβ/δ的抗细胞凋亡特性是通过内皮细胞14-3-3的激活介导的(Liou,J.Y.,Lee,S.,Ghelani,D.,Matijevic-Aleksic,N.&Wu,K.K.Protection ofendothelial survival by peroxisome proliferator-activated receptor-deltamediated 14-3-3upregulation.Arteriosclerosis,thrombosis,and vascular biology26,1481-1487,doi:10.1161/01.ATV.0000223875.14120.93(2006);Brunelli,L.,Cieslik,K.A.,Alcorn,J.L.,Vatta,M.&Baldini,A.Peroxisome proliferator-activatedreceptor-delta upregulates 14-3-3epsilon in human endothelial cells viaCCAAT/enhancer binding protein-beta.Circ Res 100,e59-71,doi:10.1161/01.RES.0000260805.99076.22(2007))。与这些研究一致,选择性PPARδ激动剂GW501516被证明可以保护心肌母细胞系H9c2免受H2O2诱导的细胞死亡(Pesant,M等人,Peroxisomeproliferator-activated receptor delta(PPARdelta)activation protects H9c2cardiomyoblasts from oxidative stress-induced apoptosis.Cardiovasc Res 69,440-449,doi:10.1016/j.cardiores.2005.10.019(2006))。
在WO2015/164506中也提出了PPARβ/δ激动剂作为干细胞移植增强剂用于治疗各种病变的联合疗法,包括联合施用干细胞和PPARβ/δ激动剂。具体地,虽然本申请公开的主要用途是使用过表达前列环素(PGI2)的干细胞和PGI2联合用于治疗血管相关疾病和/或肌肉疾病,但本申请的实施例6公开了MSC和PPARβ/δ激动剂GW501516在缺血性后肢中的联合应用。MSC在体外用PPARβ/δ激动剂GW501516预处理4天,直接注射到NOD/SCID小鼠的缺血后肢的腓肠肌中,该小鼠每天也接受PPARβ/δ激动剂GW501516的施用。
在WO2014/015318中,还提出PPARβ/δ激动剂用于增加基因递送载体(特别是逆转录病毒载体)在各种细胞群(包括MSC,尽管实验数据集中于造血干细胞-HSC)中的细胞转导效率。

因此,需要改进通过细胞疗法治疗缺血再灌注损伤的方法,特别是在心肌梗塞的情况下,其将允许有效移植具有抗细胞凋亡特性的MSC,而不会引起现有技术中公开的共同施用PPARβ/δ激动剂的有害影响。
发明内容
PPARβ/δ在MSC中高度表达(Luz-Crawford,P.等人,PPARbeta/delta directs thetherapeutic potential of mesenchymal stem cells in arthritis.Annals of therheumatic diseases,doi:10.1136/annrheumdis-2015-208696(2016)),但其对MSC抗细胞凋亡和心脏保护特性的作用还没有得到研究。
在本发明的上下文中,发明人发现通过MSC表达PPARβ/δ对于MSC在治疗心肌缺血再灌注损伤中的治疗效率至关重要。事实上,虽然PPARβ/δ的敲除并没有改变它们的抗炎特性,但却减弱了它们的心脏保护作用(见实施例1,特别是图4B),这表明增强MSC的免疫调节特性以改善它们对AMI的有益作用不应该是克服III期临床试验的不一致性的唯一或主要的方法,而是应该考虑在施用至患者后同时提高MSC的存活率。
在这些初步结果的基础上,本发明人进一步令人惊讶地发现,用PPARβ/δ激动剂对MSC进行短时间预处理(仅24小时)就足以显著改善MSC在心肌缺血再灌注损伤中的存活、移植和抗细胞凋亡特性,在比使用未经预处理的MSC低两倍的剂量下产生相似的治疗效果。在该方法中,用PPARβ/δ激动剂预处理天然(未转导的)MSC,然后将激动剂洗掉,并向需要其的受试者施用包含经预处理的MSC但不包含任何PPARβ/δ激动剂的组合物,从而防止与体内施用PPARβ/δ激动剂相关的任何有害作用。出乎意料的是,如此短的预处理足以显著提高MSC的移植和抗细胞凋亡特性,以及其在治疗缺血再灌注损伤中的治疗效率(参见实施例2)。
因此,在第一方面,本发明涉及一种制备旨在用于治疗缺血再灌注损伤的包含间充质干细胞(MSC)的组合物的体外方法,所述体外方法包括:a)提供MSC;b)在包含PPARβ/δ激动剂的培养基中培养MSC;c)去除培养基并洗涤MSC;以及d)将MSC收集在药学上可接受的载体中,其中药学上可接受的载体不含有PPARβ/δ激动剂。
本发明还涉及包含间充质干细胞(MSC)的组合物,其中所述组合物是通过根据本发明的体外方法获得的。

MSC来源的受试者

MSC的分离

步骤b)
在步骤b)中,在包含PPARβ/δ激动剂的培养基中培养MSC。
MSC的接种
将步骤a)中提供的MSC接种到包含培养基的培养容器中。
由于MSC是贴壁细胞,因此优选将它们接种在适合贴壁细胞培养的培养容器中,例如由Corning或Falcon供应商提供的75cm2烧瓶。
MSC的合适接种密度为60%至80%汇合,即3000至10000个细胞/cm2,优选3000至7000个细胞/cm2,4000至6000个细胞/cm2,更优选约5,000个细胞/cm2(Bouffi C、Bony C等人,IL-6-dependent PGE2 secretion by mesenchymal stem cells inhibits localinflammation in experimental arthritis.PLoS One.2010年12月7日;5(12):e14247)。
培养基
步骤b)中使用的培养基可以是任何适合维持MSC的培养基。此类培养基包括补充有胎牛血清(FBS)、谷氨酰胺、抗生素(通常是青霉素和链霉素)和人碱性成纤维细胞生长因子(bFGF)的基本必需培养基(MEM)-α。
在一个实施方案中,步骤b)的培养基可以是补充有5-15%(优选10%)的胎牛血清、1至5mM(优选2mM)的谷氨酰胺、50至200U/mL(优选100U/mL)的青霉素、50至200mg/mL(优选100mg/mL)的链霉素以及1至5ng/mL(优选2ng/mL)的人碱性成纤维细胞生长因子(bFGF)的基本必需培养基(MEM)-α。
PPARβ/δ激动剂
实施例2和3中提供的数据表明,当用于预处理的操作方案时,几种不同的PPARβ/δ激动剂能够显著改善MSC的心脏保护特性。另外,由于根据本发明的制备方法在步骤c)中通过去除培养基和洗涤经PPARβ/δ激动剂处理的MSC,消除了PPARβ/δ激动剂,因此即使是已被证明在体内施用时会增加有害影响风险(如PPARβ/δ激动剂GW501516的癌症风险)的PPARβ/δ激动剂也可使用,因为PPARβ/δ激动剂不会被施用至受试者。
因此,任何PPARβ/δ激动剂都可以用于根据本发明的制备方法中。
已知的PPARβ/δ激动剂包括下表1中呈现的化合物,如公开在Takada I,MakishimaM.Peroxisome proliferator-activated receptor agonists and antagonists:apatent review(2014年至今)。Expert Opin Ther Pat.2020年1月;30(1):1-13(特别参见该文献图1中的化合物3至5,以及图3中的化合物14至20):

步骤b)中使用的PPARβ/δ激动剂的浓度将根据所选择的具体PPARβ/δ激动剂及其对PPARβ/δ的亲和力进行调整。然而,基于实施例2和3中获得的数据,步骤b)的培养基中PPARβ/δ激动剂(特别是化合物GW0742和GW501516)的浓度优选为0.001至10μM,更优选为0.01μM至10μM,更优选0.05μM至5μM,最优选0.1μM至1μM。
持续时间
实施例2和3中呈现的数据表明,用PPARβ/δ激动剂对MSC进行短暂(24小时)预处理足以显著改善其心脏保护特性。
因此,步骤b)的持续时间优选在12小时至72小时之间,优选在18小时至48小时之间,更优选在20小时至30小时之间,如24小时。
缺少用于转导MSC的病毒载体
与WO2014/015318中公开的相反,使用根据本发明的制备方法制备的MSC优选不使用通过表达异源基因的载体(特别是病毒载体,尤其是逆转录病毒载体)的转导进行遗传修饰。
因此,步骤b)的培养基优选不含有用于转导MSC的病毒载体。
步骤c)
在步骤c)中,除去培养基并洗涤MSC。
此步骤非常重要,因为它可以消除用于预处理MSC的PPARβ/δ激动剂。结果,使用根据本发明的制备方法获得的预处理的MSC组合物不含有或仅含有痕量水平的步骤b)中使用的PPARβ/δ激动剂。这避免了与PPARβ/δ激动剂的体内施用可能相关的任何有害影响。
由于MSC是贴壁细胞,因此可以通过丢弃或吸出培养基来容易地去除培养基。
然后通过添加洗涤溶液洗涤MSC,并用与处理步骤b)的培养基类似的方法除去所述洗涤溶液。洗涤溶液不包含PPARβ/δ激动剂,并且优选选自盐水缓冲液(磷酸盐缓冲盐水,MEM)。洗涤步骤(即,添加洗涤溶液并将其除去)可以任选地重复(即,可以存在多于一个洗涤步骤,例如可以存在2或3个步骤洗涤),但是通常仅一个洗涤步骤就足以获得不包含或仅包含痕量PPARβ/δ激动剂的组合物。
步骤d)
在步骤d)中,将MSC收集在药学上可接受的载体中,其不包含PPARβ/δ激动剂。
所述药学上可接受的载体是与血液等渗的盐水溶液,可以尤其地选自氯化钠在水中的等渗溶液、磷酸盐缓冲盐水(PBS)1×和生理血清。还可以将以下化合物添加到与血液等渗的盐水溶液中:

例如,当预处理的MSC旨在用于静脉内施用时,可以施用多至150mL(特别是30至150mL)的体积,可以使用的药学上可接受的载体中的MSC的浓度为102至108个MSC细胞,优选103至107个MSC细胞/mL,特别是在心肌缺血再灌注损伤的治疗中。
当预处理的MSC旨在用于冠状动脉内施用时,可以施用多至10mL(特别是2至10mL)的体积,可以使用的药学上可接受的载体中的MSC的浓度为106至1010个MSC细胞/mL之间,特别是在心肌缺血再灌注损伤的治疗中。
任选的冷冻步骤e)
根据本发明的制备方法可以任选地还包括步骤e),其中PPARβ/δ引发的MSC在使用前冷冻保存。

本发明还涉及包含PPARβ/δ引发的间充质干细胞(MSC)的组合物,其中所述组合物是通过根据本发明的体外方法获得的。
“PPARβ/δ引发的MSC”在本文中意指已经使用根据本发明的方法通过PPARβ/δ处理的MSC。

根据本发明的组合物优选含有治疗有效量的预处理的MSC。发明人发现,在小鼠模型中,再灌注80至150mL体积的包含预处理的MSC的组合物(浓度为2.5×103个MSC细胞/mL,对应于3.105个细胞PPARβ/δ引发的MSC的总数),其与双倍数量的初始(即非PPARβ/δ引发的)MSC一样有效。因此,PPARβ/δ引发的MSC的治疗有效量应约为初始MSC的治疗有效量的一半。
在人类中,已发现在各种情况下(MSC来源、施用途径……)对治疗缺血再灌注损伤具有一定功效的初始MSC数量为107至1011个细胞。当使用PPARβ/δ引发的MSC时,治疗有效量应较低,尤其可以为5×106至5×1010个细胞。
取决于使用所选施用途径时可施用的体积,将为根据本发明的PPARβ/δ引发的MSC组合物选择合适的PPARβ/δ引发的MSC的体积和浓度,如上文根据本发明的方法的步骤d)中所公开的。
如上文关于根据本发明的制备方法的步骤a)所解释的,虽然组合物的MSC可以是自体的或同种异体的,但其优选是同种异体的,优选从健康受试者获得。

施用时间表

施用途径
特别地,根据所治疗的缺血再灌注损伤的类型,可以使用多种施用途径。
在心脏(特别是心肌)缺血再灌注损伤的情况下,合适的施用途径包括冠状动脉内(通过血管内方式)、静脉内和心肌内(过开胸手术方法)或经心内膜(通过血管内方式)。
使用血管内装置的冠状动脉内施用是特别优选的,因为它可以在冠状动脉造影期间进行,在再灌注时或之后进行,冠状动脉造影是在患有心肌梗塞的患者中定期进行的相对软性干预。

MSC剂量
使用治疗有效剂量的根据本发明的PPARβ/δ引发的MSC组合物。
如上在涉及根据本发明的PPARβ/δ引发的MSC组合物的部分中所解释的,治疗有效剂量的范围将取决于各种参数,包括施用途径。然而,合适的剂量范围是在5×106至5×1010个PPARβ/δ引发的MSC之间,特别是当通过冠状动脉内施用进行施用时。
施用的时间
在缺血再灌注损伤的情况下,快速的治疗干预至关重要。特别是在AMI(急性心肌梗塞)患者中,应在AMI后1周内进行MSC的施用,以使其效率。
因此,在优选的实施方案中,根据本发明的PPARβ/δ引发的MSC组合物在缺血再灌注损伤发生的1周内施用,优选在6天内、5天内、4天内,更优选在3天内、2天内或者甚至在24小时内施用。
在另一个优选的实施方案中,根据本发明的PPARβ/δ引发的MSC组合物在缺血器官的再灌注期间施用,以预防缺血再灌注损伤。
在本发明的背景下,通过短暂的PPARβ/δ引发的MSC,使得对缺血或缺血再灌注损伤的这种快速反应成为可能。

实施例
实施例1:PPARβ/δ是间充质干细胞在心肌缺血再灌注损伤中发挥心脏保护作用所必需的
心肌梗塞在全球死亡率中居首位。由于成人心脏无法再生,纤维化会扩大以补偿梗塞后收缩组织的损失,导致心脏重塑和心力衰竭。成体间充质干细胞(MSC)的再生特性及其安全性和有效性已在临床前模型中得到证实。然而,在临床试验中,它们的有益效果存在争议。在关节炎实验模型中,源自PPARβ/δ敲除(KO MSC)小鼠的MSC已被证明比源自野生型小鼠的MSC提供更强的治疗效果。
本研究的目的是比较KO MSC细胞和野生型MSC相较于未经处理的对照条件(IR)对缺血再灌注损伤的治疗效果。
材料和方法
动物饲养和护理
实验按照1986年11月欧洲共同体理事会指令在C57BL/6J小鼠(Charles Riverlaboratory)上进行,符合美国国立卫生研究院出版的《the Guide for the Care and Useof Laboratory Animals》(NIH出版,第8版,2011)。所有小鼠均在研究所的动物设施中维持在环境控制条件下(22±2℃,12小时光照/12小时黑暗循环)。所有离体操作方案均经动物研究伦理委员会批准(协议#A34-172-41)。
离体实验
雄性小鼠腹膜内注射(IP)氯胺酮(14mg/kg,Merial)、赛拉嗪(14mg/kg,/>Bayer),然后注射戊巴比妥(IP;76.6mg/kg;Sanofi-Aventis)进行麻醉。麻醉小鼠接受250U肝素(IP)以防止血凝块形成。胸骨切开术后,切除心脏,通过升主动脉插管,快速安装在Langendorff系统上。在恒定压力和温度(37℃)下灌注预热的Tyrode溶液(NaCl140mM、KCl 5.4mM、MgCl 1mM、Hepes 5mM、葡萄糖5.5mM、CaCl2 1.8mM、pH 7.4)。
缺血再灌注操作方案(IR)
在Langendorff系统上,心脏用预热的Tyrode溶液灌注15分钟(稳定)。通过在30分钟内停止灌注液流(“无液流”),获得整体缺血。通过恢复液流实现再灌注步骤(60分钟)。再灌注期间应用MSC处理(含有MSC的Tyrode溶液)。仅使用Tyrode获得对照条件(IR)。在再灌注操作方案期间(再灌注后5分钟、30分钟和60分钟),收集冠状动脉流出物,储存在-80℃下以供将来的实验。
梗塞大小测量
在IR操作方案结束时,将心脏解剖出来。将左心室包含在琼脂(4%)中,用振动切片机横向切片(1mm)。为显示出组织活力,将切片与2,3,5-氯化三苯基四唑溶液(TTC染料)在37℃下孵育15分钟。经过固定步骤(4%多聚甲醛,48小时)后,对每个切片的每一片进行拍照。使用ImageJ软件通过平面测量来量化梗塞面积。
MSC培养
小鼠MSC的分离、扩增和表征按照之前所述进行(Scholtysek,C.等人,PPARβ/δgoverns Wnt signaling and bone turnover.NatMed 19,608-613,doi:10.1038/nm.3146(2013))。简而言之,从由Béatrice Desvergne提供的Ppard fl/fl sox2cretg PPARβ/δ缺陷小鼠(称为PPARβ/δ-/-MSC)及其野生型Ppard fl/+同窝小鼠(称为PPARβ/δ+/+MSC)的长骨中冲洗出骨髓(BM)(Scholtysek,C.等人,PPARβ/δgoverns Wnt signaling and boneturnover.Nat Med 19,608-613,doi:10.1038/nm.3146(2013))。细胞以0.5×106个细胞/cm2的密度培养在基本必需培养基(MEM)-α中,其含有10%胎牛血清、2mM谷氨酰胺、100U/mL青霉素、100mg/mL链霉素和2ng/ml人碱性成纤维细胞生长因子(bFGF)。之前已对MSC的表型和功能进行了表征(Luz-Crawford,P.等人,PPARbeta/delta directs the therapeuticpotential of mesenchymal stem cells in arthritis.Annals of the rheumaticdiseases,doi:10.1136/annrheumdis-2015-208696(2016))。PPARβ/δ+/+MSC与5μM的PPARβ/δ拮抗剂GSK0660预孵育24小时。
用CM-DiI标记MSC。
荧光细胞示踪剂CM-DiI(Molecular Probes)的原液以1μg/μL的浓度复溶在二甲基亚砜(DMSO)中。收集MSC,以107个细胞/10μg CM-DiI的浓度悬浮在5mL的PBS中。将细胞在37℃中孵育5分钟,然后在4℃中在黑暗中孵育15分钟。然后通过在300g下离心5分钟,以及在PBS中洗涤2次来去除未掺入的荧光染料。将标记的细胞重悬于PBS中并维持在4℃,然后注射到心肌中。
细胞因子定量。
对于细胞因子定量,在基础条件(t0)下、稳定10分钟后以及在再灌注60分钟后(IR60)的操作方案结束时,收集来自被灌注的心脏的流出物。将不同的流出物储存在-80℃,直到根据制造商的操作方案在“Plateforme de Protéomique Clinique deMontpellier”上使用Meso Scale Discovery(MSD)V-Plex Plus Proinflammatory Panel1(小鼠)试剂盒进行测定。该组包括IFNγ、IL-1β、IL-2、IL-4、IL-5、IL-6、KC/GRO(CXCL1)、IL-10、IL-12p70和TNFα。如果样品中的特定细胞因子浓度低于检测限,即细胞因子浓度与背景噪声无法区分,则认为样品的该细胞因子呈阴性。
免疫化学
离体实验结束时,将左心室(LV)固定在4%-PFA中,并包埋在石蜡中。每个左心室从心尖到基部被切开(每150μm切4μm切片)。将石蜡包埋切片脱蜡,然后通过酒精梯度再水化。LV切片与一抗一起孵育:抗F4/80(1:100;大鼠单克隆;Abcam)、抗CD68单克隆抗体(FA-11;Invitrogen)、CX3CR1抗体(1H14L7;Thermofischer Scientific)和抗α肌动蛋白(1:100,小鼠单克隆;Sigma-Aldrich)。细胞核用赫斯特染料(Life Technologies SAS)染色,内皮细胞用异凝集素B4(FITC偶联物;Sigma-Aldrich)染色。一抗孵育后,将切片在PBS中洗涤,与二抗(1:2,000,Jackson ImmunoRes Laboratories,Inc.)一起孵育(3小时)。将一抗和二抗在含有3%BSA和0.1%Triton X100的PBS中稀释。将染色切片安装在Mowiol(Biovalley)中。使用Zeiss Axioimager Z3荧光显微镜获得图像,并使用ImageJ和AdobePhotoshop进行分析以准备最终图像。
统计分析
使用GraphPad Prism 8软件,使用非参数Mann-Whitney(两组)和Kruskal-Wallis(多重比较)对以平均值±SD值表示的数据进行比较。图表显示平均值±标准差(SD)。P值<0.05(*)、P<0.01(**)、P<0.001(***)和p<0.0001(****)被认为具有统计显著性。使用Graph-Pad PrismTM软件(Graphpad)进行分析和图形表示。
结果
在经历离体缺血再灌注损伤的分离的灌注心脏中诱导促炎反应
在这项研究中,我们开发了一种整体缺血之后再灌注的离体模型(IR操作方案),以评价MSC的治疗效果。将分离的心脏在Langendorff系统上安装并灌注,进行30分钟的整体缺血(无液流),然后进行60分钟的再灌注,在该阶段期间施用细胞(参见图1A中的操作方案)。研究的第一部分致力于描述我们模型的特征。心肌缺血再灌注损伤后心脏的特征是梗塞大小平均值为56.4±10.3,以左心室百分比表示(图1B)。免疫组织学允许证明定位于心肌中的巨噬细胞位于心肌层厚度处(数据未显示)。
因此,我们想知道,IR损伤的诱导和心肌中巨噬细胞的存在是否与稳定期间收集的基础条件(Basal)相比,再灌注开始后60分钟收集的冠状动脉流出物中促炎细胞因子的过量释放有关。在使用Meso Scale Discovery VPlex Plus Proinflammatory定量的10种细胞因子中,其中包括IFNγ、IL-1β、IL-2、IL-4、IL-5、IL-10、IL-12p70的7种细胞因子未纳入统计分析,因为在过多样品中存在检测不到的水平。与基础条件(在10分钟稳定期收集,basal)相比,在IR损伤后(缺血和再灌注60分钟后),在心脏冠状动脉流出物中,三种细胞因子TNFα、KC/GRO(CXCL1)和IL-6表现出非常显著的增加(图1C)。总而言之,这些结果表明,经离体缺血30分钟然后再灌注1小时,C57Bl6小鼠心脏的心肌IR损伤与浸泡(bathe)在发炎心肌中的巨噬细胞的存在有关。
在离体的整体缺血模型的再灌注过程中,MSC发挥了有效的心脏保护作用。
为了减少IR心肌的炎症,从而减少梗塞大小,我们依赖于两种公认的心脏保护策略,我们首次在离体模型中比较了这两种策略:基于缺血后PostC和基于MSC的疗法。为此,对于经历30分钟的整体缺血的分离的心脏,在其再灌注期间施用MSC,如图2A中所示的操作方案所述。测试了不同浓度的MSC的灌注溶液(图2B)。使用不同剂量的MSC建立剂量反应曲线,结果显示5,000个细胞/mL(6.105个细胞/心脏)和20,000个细胞/mL(24.105个细胞/心脏)诱导心脏保护作用(分别是,对于MSC 5,000为17.29±6,84,n=6vs对于IR为56.39±10.33,n=12;p=0.0009,以及对于MSC 20,000是17.04±4.07;n=6vs 56.39±10.33,n=12;p=0.0009),与之相反,2,500个细胞/mL(3.105个细胞/心脏)并没有提供显著的有益效果(对于MSC 2,500为34.1±13.93,n=6vs对于IR为56.39±10.33,n=12;p=0.2238)。我们研究的所有实验均选择5,000个细胞/mL的浓度,因为这是在离体小鼠模型中提供最大心脏保护的最低浓度。更进一步,我们证实了再灌注5000个细胞/mL的MSC(6.105个细胞/心脏)与缺血的PostC(实验中作为阳性对照)在相同程度上减少了梗塞大小(对于PostC为14.04±4.32,n=8vs对于MSC为17.29±6.84,n=6,p>0.99;图2C)。免疫组织学分析显示,再灌注1小时后,离体灌注的MSC位于冠状动脉微血管中(数据未显示)。总而言之,这些结果证实6.105MSC/心脏与缺血后PostC具有相同的治疗效果,缺血后PostC是AMI期间心脏保护的金标准策略。
缺血后PostC和MSC施用的有效心脏保护作用都与受损心肌内促炎细胞因子的减
少有关
为了确定缺血后PostC和MSC施用对IR损伤的有益作用是否与免疫反应的调节相关,我们对再灌注开始后15、30和60分钟收集的冠状动脉流出物中的促炎细胞因子进行了定量(图3A)。使用MSD方法定量的10种细胞因子中,样本中未检测到7种细胞因子,包括IFNγ、IL-1β、IL-2、IL-4、IL-5、IL-10、IL-12p70。然而,在经历缺血后PostC或MSC施用的心脏的冠状动脉流出物中,TNFα(图3B)、CXCL1(KC/GRO)(图3C)和IL-6(图3D)的水平显著降低。该结果表明,根据冠状动脉流出物中促炎细胞因子的定量评估,PostC和MSC的保护作用与有效的抗炎作用相关。
PPARβ/δ参与MSC介导的针对IR损伤的心脏保护作用。
最近,我们在关节炎实验中证明,PPARβ/δ对于MSC免疫调节和治疗功能至关重要(Luz-Crawford,P.等人,PPARbeta/delta directs the therapeutic potential ofmesenchymal stem cells in arthritis.Annals of the Rheumatic Diseases2016;75:2166-2174)。然而,PPARβ/δ对MSC心脏保护活性的作用以及PPARβ/δ对发炎心肌中MSC免疫调节特性的相关性尚未得到解决。为了确定PPARβ/δ是否对MSC心脏保护特性至关重要,我们比较了从PPARβ/δ-/-缺陷小鼠(KO MSC)中分离的MSC与从PPARβ/δ+/+对照同窝小鼠中获得的MSC(MSC)的效果。在再灌注过程中,用含有MSC的溶液以6.105个细胞/心脏的最佳剂量对分离的心脏进行灌注(参见操作方案图4A)。在这些条件下,当在缺血性损伤后分离的心脏中灌注KO MSC时,野生型MSC诱导的梗塞大小的急剧减小(对于MSC为24.1±11.8,n=9,vs对于IR为56.4±10.3,n=12;p**=0.001)消失(对于KO MSC为48.4±25.4,n=13vs对于IR为56.4±10.3,n=12;pns=0.75,以及对于KO MSC为48.4±25.4,n=13vs对于MSC为24.1±11.8,n=9;p*=0.029)(图4B)。为了确定在PPARβ/δ敲除后,MSC失去治疗作用是否与MSC失去减轻梗塞心肌的炎症的能力有关,我们对再灌注开始后60分钟收集的冠状动脉流出物中的促炎细胞因子进行了定量。细胞因子的MSD定量显示,在MSC处理的和未处理的心脏的冠状动脉流出物中,TNFα浓度未有任何变化。然而,我们证明了,在遭受缺血性损伤的心脏中MSC具有抗炎作用,这表现在,在再灌注时收集的冠状动脉流出物中CXCL1和IL-6浓度显著下降(对于CXCL1:IR为16.61pg/ml±10.53,n=10vs对于MSC为2.56pg/ml±3.79,n=10;p**=0.0017(图4D),以及对于IL-6:对于IR为21.83pg/ml±11.08,n=10vs对MSC为5.43pg/ml±3.02,n=10;p**=0.004(图4E))。值得注意的是,MSC中的PPARβ/δ敲除并没有改变其抗炎潜力,如在再灌注时收集的冠状动脉流出物中的CXCL1浓度保持不变所示(对于MSC为2.56pg/ml±3.79,n=10vs对于KO MSC为3.73pg/ml±1.65,n=8,pns=0.28;图4D)和IL-6(对于MSC为5.43pg/ml±3.02,n=10vs对于KO MSC为7.65pg/ml±5.04,n=8,pns>0.99;图4E)。
讨论和结论
证实了该研究中使用的整体缺血(30分钟)、然后60分钟再灌注的离体操作方案诱导了促炎症反应,其评估为经过60分钟再灌注的冠状动脉流出物中的TNFα、IL-6和CXCL1的水平增加,之后我们测试了PPARβ/δ对MSC免疫调节和治疗功能的作用。首先,我们确定了MSC的最佳剂量(6.105个细胞/心脏),其能够以最小浓度的细胞在离体小鼠模型中提供显著的心脏保护作用。值得注意的是,在选择的该剂量的MSC下,我们观察到的心脏保护作用与在我们的研究中作为阳性对照的缺血后调理(PostC)类似。事实上,两种治疗方法都允许减少IR损伤的促炎症反应,并以相同的疗效减少梗塞大小。此外,这项研究显示,MSC的心脏保护特性是PPARβ/δ依赖性的,与其对冠状动脉流出物中TNFα、CXCL1和IL-6释放的抗炎症作用形成鲜明对比。
尽管再灌注疗法对AMI患者具有有益的效果,但会诱发局部和全身性炎症,称为无菌炎症,其会强化在AMI后为清除坏死细胞和修复梗塞心肌而引发的初始炎症反应。为了开发和评价创新的治疗方法,我们设计了一种离体模型,其提供了强烈的IR损伤,其通过如已在体内报道的,梗塞大小达56%(以左心室的百分比表示)以及冠状动脉流出物中释放显著水平的促炎细胞因子来评价。
这种炎症反应是改善心肌梗塞后临床的潜在治疗目标,因为它在确定梗塞大小和随后的左心室重塑中起着至关重要的作用。为了保护心肌,已经测试了多种间接或直接针对AMI后促炎症反应的治疗策略。
缺血后调理(PostC)被认为是针对IR损伤的心脏保护的金标准,据报道可以激活无数的细胞内级联反应,从而抑制调节的细胞死亡和抗炎症反应,以便在再灌注一小时后诱导强烈的心脏保护作用(Roubille,F.等人,Delayed postconditioning in the mouseheart in vivo.Circulation 124,1330-1336,doi:CIRCULATIONAHA.111.031864[pii]10.1161/CIRCULATIONAHA.111.031864(2011))。在本研究中使用的IR损伤的离体模型中,相较于IR,PostC心脏保护策略将梗塞大小减少了75.1%(PostC vs IR,p*),以及将在分离的心脏的冠状动脉流出物中的TNFα、IL-6和CXCL1水平分别减少了71.7%、71.8%和91.4%。总而言之,这些结果表明炎症反应的调节与缺血后PostC介导的IR损伤的心脏保护作用相关。

实施例2:过氧化物酶体增殖物激活的受体β/δ激动剂增强了间充质干细胞在心肌缺血再灌注损伤中的抗细胞凋亡和治疗特性
根据实施例1中使用KO MSC细胞获得的结果,测试了用PPARβ/δ激动剂预处理MSC对于MSC在心肌缺血再灌注损伤中的抗细胞凋亡和治疗特性的影响。
材料和方法
动物实验
所有实验均按照1986年11月欧洲共同体理事会指令在C57BL/6J小鼠(CharlesRiver laboratory)上进行,符合美国国立卫生研究院出版的《Guide for the Care andUse of Laboratory Animals》(NIH出版,第8版,2011)。
Langendorff离体研究
对C57Bl6雄性小鼠进行麻醉,首先肌肉注射麻醉剂混合物,包括氯胺酮(14mg/kg,Merial)和赛拉嗪(14mg/kg,/>Bayer),然后注射戊巴比妥(76.6mg/kg;Sanofi-Aventis)。胸骨切开术后,切除心脏,快速通过主动脉插管,并安装在自制的Langendorff系统上。在压力和温度恒定(37℃)的Tyrode溶液中逆行灌注。
缺血再灌注操作方案
在Langendorff系统上,对心脏进行缺血再灌注(IR)操作方案,包括在15分钟的Tyrode灌注稳定期后,进行30分钟的无液流期以诱导整体缺血。再灌注是通过在60分钟内单独使用Tyrode溶液(IR对照组),或在再灌注期间施用包含MSC细胞的Tyrode溶液以恢复灌注来实现的。在整体缺血结束时,通过应用包括3个周期的1分钟再灌注/1分钟缺血、随后是54分钟的再灌注的调理刺激来诱导缺血后调理。为了对基于MSC的细胞疗法进行离体评价,在60分钟再灌注阶段,在离体心脏中通过主动脉和冠状动脉血管逆行灌注了在80至150mL(平均120mL)的Tyrode溶液中制备的不同浓度(2500、5000和20000个细胞/mL)的MSC细胞。
梗塞大小评估
将LV横向切成1mm厚的切片,在1%的2,3,5-氯化三苯基四氮唑(TTC,Sigma-Aldrich)溶液中于37℃孵育15分钟。在4%多聚甲醛-PBS溶液中固定后,对切片进行称重,并对每一侧进行拍照(Olympus相机)。使用ImageJ软件(美国国立卫生研究院)通过平面测量法测量梗塞面积,以占左心室的百分比表示。
MSC培养
在之前描述的条件下进行小鼠MSC的MSC分离和扩增(Bouffi,C.,Bony,C.,Courties,G.,Jorgensen,C.&Noel,D.IL-6-dependent PGE2 secretion by mesenchymalstem cells inhibits local inflammation in experimental arthritis.PLoS One 5,e14247(2010))。然后,将MSC以0.5×106个细胞/cm2的密度接种在基本必需培养基(MEM)-α中,其补充有10%胎牛血清(FBS)、2mM谷氨酰胺、100U/mL青霉素、100mg/mL链霉素和2ng/ml人碱性成纤维细胞生长因子(bFGF)。MSC的表型和分化潜力如前所述进行(Bouffi,C.,Bony,C.,Courties,G.,Jorgensen,C.&Noel,D.IL-6-dependent PGE2 secretion bymesenchymal stem cells inhibits local inflammation in experimentalarthritis.PLoS One 5,e14247(2010))。简而言之,为了表征表型,将MSC与来自BDBiosciences(Le Pont de Claix,法国)的偶联单克隆抗体在补充有0.1%牛血清白蛋白的PBS中孵育20分钟。关于MSC分化为脂肪细胞、成骨细胞和软骨细胞,我们依赖于已经报道的诱导条件(Bouffi,C.,Bony,C.,Courties,G.,Jorgensen,C.&Noel,D.IL-6-dependentPGE2 secretion by mesenchymal stem cells inhibits local inflammation inexperimental arthritis.PLoS One 5,e14247(2010))。MSC的成脂分化通过油红O染色后的脂滴形成分析和RT-qPCR进行评估。通过RT-qPCR评价MSC软骨分化,通过RT-qPCR和细胞外基质的矿化测量成骨分化。
用PPARβ/δ激动剂和拮抗剂预处理MSC,用CM-DiI标记。
将MSC与PPARβ/δ拮抗剂GSK3787(0.1和1μM)、PPARβ/δ反向激动剂GSK0660(0.1和1μM)或PPARβ/δ激动剂GW0742(0.1、1和5μM)预孵育24小时,然后用PBS洗涤,用于共培养实验或注射到心肌中。当指出时,MSC用荧光细胞示踪剂CM-DiI(Molecular Probes)进行标记。收集MSC,悬浮于5mL的含有CM-DiI的PBS中(107个细胞/10μg),然后在37℃下孵育5分钟,然后在4℃在黑暗中孵育15分钟。将标记的MSC在PBS中洗涤两次,重悬于PBS中并维持在4℃,然后注射到心肌中。
心肌细胞和MSC共培养
H9c2(褐家鼠心肌细胞)在Dulbecco改良的Eagle培养基(DMEM)中培养,其补充有10%胎牛血清(FBS)、100单位/mL的青霉素和100μg/mL的链霉素。为了诱导细胞凋亡,将H9c2在含有350μM的H2O2的基本培养基(不含FBS)中培养4小时。为了评价MSC对心肌细胞的影响,使用Transwell系统设计了共培养测定法,以避免细胞与细胞接触。在350μM的H2O2存在下将15,000个心肌细胞(下部)和/或15,000个MSC(上部)铺板到12孔培养皿的每孔中。进行了两种不同的共培养操作方案。在操作方案1中,H9c2和共培养的MSC暴露于H2O2(不含血清)。在操作方案2中,H9c2在基本培养基中暴露于H2O2中4小时,洗涤后,将细胞在完全培养基中与MSC共培养。在操作方案3中,H9c2在H2O2存在下在不含血清的常规培养基中培养4小时,并再继续培养1小时。然后,将H9c2与MSC在完全培养基中共培养1小时。在所有操作方案结束时,回收H9c2用于细胞凋亡和RT-qPCR分析。
DNA片段化的测定法
使用酶联免疫吸附测定试剂盒(Cell Death ELISA;Roche Diagnostics)对细胞裂解物中的DNA片段化进行体外定量,该试剂盒涉及用于测量细胞凋亡诱导后的细胞的细胞质中的组蛋白复合的DNA片段(单核糖体和寡核糖体)。
RT-qPCR
使用RNeasy微型试剂盒(Qiagen,Courtaboeuf)分离每个样本的总RNA。使用NanoDrop ND-1000分光光度计(NanoDrop ND,Thermo Fisher Scientific)确定提取的RNA的数量和质量。然后,使用SensiFAST cDNA合成试剂盒(Bioline),通过将500ng RNA反转录成cDNA来合成cDNA。使用SensiFASTTMSYBR(Bioline)和480定量检测系统,按照制造商的建议进行PCR。使用Primer3软件设计了angpl4、pdk4、bcl2、hgf、pdgf、nrf2的特异性引物。将数据相对于管家基因核糖体蛋白S9(RPS9)归一化,将值表示为使用2-ΔCt方法计算的特定基因表达的相对mRNA水平。
免疫印迹
在离体IR操作方案结束后,将组织样本(左心室)快速冷冻在液氮中。使用研磨机将样本在RIPA缓冲液中[50mM Tris(Sigma-Aldrich)、150mM NaCl(Sigma-Aldrich)、1%Triton X-100(Sigma-Aldrich)、0.1%十二烷基硫酸钠(Sigma-Aldrich),pH 8.0]均质化,所述缓冲液补充有不含EDTA的蛋白酶和磷酸酶抑制剂(HaltTM蛋白酶和磷酸酶一次性抑制剂Cocktail-100×,Thermoscientific)。离心后,将沉淀重悬于RIPA缓冲液中以进行蛋白质纯化,在4℃下孵育1小时。使用二辛可宁酸(BCA)蛋白质测定试剂盒(Pierce)测定蛋白质浓度。蛋白质的样本(25μg)通过SDS聚丙烯酰胺凝胶电泳(4-20%TGXTM预制凝胶;Biorad)进行解析,并转移至硝基纤维素(/>TurboTM;Biorad)。使用以下抗体和供应商:1)EMD Millipore:抗半胱天冬酶3(Upstate 06-735),2)细胞信号传导技术:抗磷酸ERK1,2;抗ERK1,2;抗磷酸AKT;抗AKT;β-微管蛋白;抗α-肌动蛋白;纽蛋白(vinculin)(E1E9V)、GAPDH,3)Jackson ImmunoResearch:辣根过氧化物酶缀合的抗兔抗体。使用ECL试剂盒(SuperSignalTMWest Pico化学发光底物;Thermo ScientificTM)通过增强化学发光方法对蛋白质条带进行可视化。使用ChemidocTM(Biorad)和ImageJ软件(美国国立卫生研究院)进行密度测定分析。每个蛋白质条带的信号强度用上样对照(微管蛋白、纽蛋白、GAPDH或α-肌动蛋白)获得的相应值进行校正,然后用IR对照条件获得的平均值进行归一化。
免疫化学
在离体实验结束时,将左心室(LV)固定在甲醛中,包埋在石蜡中。每个左心室从心尖到基部被切开(每个150μm切4μm切片)。将石蜡包埋切片脱蜡,然后通过酒精梯度再水化。将切片安装在Mowiol(Biovalley)中。LV切片由Zeiss Axio Scan Z1玻片扫描仪(Zeiss,Oberkochen,德国)进行扫描。使用×20物镜进行扫描。使用ImageJ分析图像。
统计分析
仅对n≥5个的独立实验进行统计分析。确认群体的正态性后,使用ANOVA参数检验对数据(平均值±SD)进行分析(正态分布的Anderson-Darling检验)。如果不一致,则使用非参数Kruskal-Wallis和Dunn事后检验进行多重比较,或使用Mann-Whitney进行两组比较。使用GraphPad Prism(MacO版本8.3.0,GraphPad Software,美国加利福尼亚州圣地亚哥,www.graphpad.com)分析数据。统计显著性记为,ns记为p>0.05,*记为p<0.05,**记为p<0.01,***记为p<0.001以及****记为p<0.0001。
结果
在离体的整体缺血模型中,在再灌注期间施用MSC的心脏保护作用取决于PPARβ/δ
首先,我们对用于研究的MSC进行表型和功能表征,如前所述(Bouffi,C.,Bony,C.,Courties,G.,Jorgensen,C.&Noel,D.IL-6-dependent PGE2 secretion bymesenchymal stem cells inhibits local inflammation in experimentalarthritis.PLoS One5,e14247(2010))。我们表明MSC的CD11b和CD45呈阴性,而MSC上表达的标记物如CD44和Sca-1呈阳性(数据未显示)。在诱导MSC向三个特定谱系分化后,MSC产生:1)软骨细胞,如II型胶原蛋白(Col2)的表达所示;2)脂肪细胞,其特征在于pparγ和lpl的表达以及脂滴的形成,以及3)成骨细胞,其特征在于骨钙素和碱性磷酸酶的表达以及茜素红S染色(数据未显示)。为了评价MSC在我们的IR损伤的离体模型中的保护作用,在经历整体缺血的离体心脏的再灌注阶段,以不同浓度(在Tyrode溶液中2500、5000和10000个细胞/mL)施用MSC(参见操作方案,图5A)。图5B显示了MSC心脏保护作用的剂量-反应获得的U曲线。5,000个细胞/mL的浓度被认为是提供最佳疗效的剂量,与PostC刺激提供的疗效相似,该PostC刺激被认为是我们实验中的阳性对照(对于PostC为28.13±4.69,n=7,vs对于MSC 5,000为30.27±5.89;n=12;p$>0.999)。此外,与IR相比,无论是2,500个细胞/mL的剂量(38.64±5.36,n=11vs对于IR为46.51±6.28,n=11;pns=0.80),还是最大的测试剂量,10,000个细胞/mL(对于MSC 10,000为50.28±11.09,n=6vs对于IR为46.51±6.28,n=11;pns>0.999),都没有产生显著减小的梗塞大小。
因为我们在之前的研究中证明了PPARβ/δ表达对MSC功能至关重要,包括MSC的差异电位和免疫调节特性,我们在这里讨论了PPARβ/δ在MSC心脏保护效果中发挥的作用(Scholtysek,C.等人,PPARβ/δgoverns Wnt signaling and bone turnover.Nat Med 19,608-613,doi:10.1038/nm.3146(2013);Luz-Crawford,P.等人,PPARbeta/delta directsthe therapeutic potential of mesenchymal stem cells in arthritis.Ann RheumDis 75,2166-2174,doi:10.1136/annrheumdis-2015-208696(2016);Contreras-Lopez,R.A.等人,PPARβ/δ-dependent MSC metabolism determines their immunoregulatoryproperties.Sci Rep 10,11423,doi:10.1038/s41598-020-68347-x(2020))。为此,将MSC用PPARβ/δ拮抗剂或激动剂药物试剂进行预处理,然后在IR损伤的离体模型中施用(5,000个细胞/mL)。图5C显示,将MSC(5,000个细胞/mL)用GSK0660(1μM)预处理24小时后,在离体缺血心脏中在再灌注阶段施用时,MSC的心脏保护作用会降低(对于MSC+GSK0660为37.78±5.31,n=9vs对于MSC为30.27±5.89,n=12;p#=0.019)。根据这一结果,我们研究了使用不同剂量(0.1、1和5μM)的GW0742PPARβ/δ激动剂是否与改善以较低剂量(2,500个细胞/ml)施用MSC提供的心脏保护作用相关,较低剂量(2,500个细胞/ml)的MSC本身并没有保护作用(图5D)。首先,我们验证了GW0742 PPARβ/δ激动剂预处理对培养的MSC的作用,发现GW0742的激动作用与angpl4和pdk4靶基因表达水平的增加相关,从而证明了激动剂的功能性(数据未显示)。然后,我们表明用不同浓度的PPARβ/δ激动剂预处理MSC,在1μM浓度下可诱导MSC的心脏保护作用(对于MSC+GW0742 1μM为25.77±4.68,n=7vs对于IR为46.51±6.28,n=11;p****<0.0001),而在0.1和5μM时未观察到有益效果。用GW0472(1μM)预处理细胞,再以2500c/mL施用,可以诱导与使用5000c/mL的初始MSC观察到的心脏保护作用相同的心脏保护作用(图5E)。PPARβ/δ引发的MSC治疗效果的改善与移入梗塞心肌组织中的MSC数量增加有关,如使用DiI标记的MSC进行免疫组织学实验所评估的,显示了经过GW0472预处理将荧光百分比增加了3.5倍(图5F)。
PPARβ/δ引发的MSC对模拟IR的抗性增强
由于组织学实验显示,当MSC用PPARβ/δ激动剂引发时,心肌梗塞区域的MSC数量增加,因此我们首先研究了引发的MSC对IR应激的抗性。为此,使用不同浓度的H2O2(100、250、350、500和750μM;参见图6A中的操作方案),对MSC进行4小时的体外IR模拟应激。通过对不同处理组的特异性DNA片段化测量的平均值,评估了细胞凋亡定量。我们发现,在暴露于100μM的H2O2的MSC中,H2O2诱导特异性DNA片段化,其中评分在100至750μM之间达到平台(图6B)。选择250μM的H2O2浓度,是因为与未经处理的MSC相比,这是提供最大效果的最低剂量(p****)。为了确定PPARβ/δ在MSC对H2O2抗性中的作用,我们在H2O2应激诱导(250μM,4小时)之前,用不同浓度(0.1和1μM)的PPARβ/δ拮抗剂和激动剂预处理MSC 24小时。用0.1μM PPARβ/δ激动剂GW0742预处理MSC,并没有改变MSC抵抗氧化应激的能力(图6C)。然而,与暴露于H2O2的初始MSC相比,用1μM PPARβ/δ激动剂GW0742预处理显著增强了MSC对H2O2的抗性(p*)。用1μM的PPARβ/δ拮抗剂GSK0660预处理损害了MSC对H2O2诱导的氧化的抗性,但未达到显著性(图6C)。经PPARβ/δ激动剂预处理,特别是经0.1μM的GW0742预处理后的MSC对细胞凋亡的抗性增强(p**),这一点通过bcl2基因表达水平的显著增加得到了证实。相反,与对照H2O2处理的MSC相比,用1μM GSK0660预处理MSC显著降低了bcl2基因表达水平(p*;图6D)。总而言之,这些结果表明,PPARβ/δ的激活保护了MSC免受H2O2诱导的模拟IR应激引起的细胞凋亡。
PPARβ/δ-引发增强了MSC对暴露于IR样氧化应激的心肌细胞的保护作用

PPARβ/δ-引发逆转了IR样氧化应激对MSC旁分泌功能的抑制作用

PPARβ/δ-引发增加了在离体时MSC对心脏的保护作用
PPARβ/δ引发的MSC对心肌细胞的有效抗细胞凋亡作用在整个心脏上得到了进一步证实。对离体经受IR损伤的心脏进行蛋白质印迹分析,结果表明,与未受处理的IR条件相比,MSC处理诱导减少了对半胱天冬酶3的激活(图9A)。当用GW0742 PPARβ/δ激动剂(1μM)预处理MSC 24小时时,对半胱天冬酶3切割的抑制作用进一步增加(图9B)。相比之下,用GSK0660 PPARβ/δ拮抗剂预处理MSC,然后在再灌注时将MSC注射到离体IR心脏中,使得半胱天冬酶3的完全激活恢复到与IR对照条件相同的水平。
此外,当用GW0742 PPARβ/δ激动剂(1μM)预处理MSC,然后在再灌注期间施用MSC时,对ERK1/2(图9C)和AKT(图9D)促生存激酶的磷酸化模式的评价显示,MSC处理的IR心脏与未处理的IR心脏相比,显著增加了pAkt/Akt的磷酸化比率。相比之下,当在IR操作方案的缺血后阶段施用初始MSC时,pERK1/2/ERK1/2的比率没有变化。
讨论和结论
我们的研究首次证明PPARβ/δ激活保护了MSC免于细胞凋亡,增强MSC抗细胞凋亡和治疗心肌缺血再灌注损伤的特性。在这项研究中,我们开发了一种整体缺血之后再灌注的离体模型(IR操作方案),以评价和比较PPARβ/δ引发的MSC相对于初始MSC的治疗效果。在接受IR操作方案的心脏中,在再灌注阶段施用MSC允许减少梗塞大小。当MSC在施用前用PPARβ/δ拮抗剂预处理24小时时,其心脏保护作用显著降低。使用PPARβ/δ激动剂预处理显著增加了MSC诱导的心脏保护作用,这与心室壁中检测到的细胞数量显著增加有关。我们在体外证明PPARβ/δ预激活允许增加MSC的存活率和对氧化应激的抗性。当用PPARβ/δ激动剂引发MSC时,MSC对H2O2诱导应激的抗性增加与细胞凋亡率降低相关,并且与bcl2转录本数量的增加有关。此外,相较于初始MSC,PPARβ/δ引发的MSC对H2O2应激的培养的心肌细胞具有更有效的抗细胞凋亡治疗作用。在整个心脏中,我们的结果还表明,如通过半胱天冬酶3激活的蛋白质印迹分析所评估的,PPARβ/δ引发的MSC的抗细胞凋亡作用比初始MSC提供的抗细胞凋亡作用更有效。因此,与初始MSC相比,通过PPARβ/δ引发处理的IR心脏中AKT促生存激酶的磷酸化比率也增加。

总之,我们的研究提供了基于MSC的细胞产品诱导的对IR损伤增强的心脏保护的证据,所述基于MSC的细胞产品具有有效的抗细胞凋亡作用,以及在再灌注开始后一小时在受损心肌中具有增加的存活MSC的数量。
对于临床提高MSC对AMI患者的受损心肌的心脏保护的疗效,这些结果可以具有重大意义。
实施例3:使用PPARβ/δ激动剂预处理MSC的持续时间的影响
材料和方法
体外:
间充质干细胞的培养
如前所述,分离来自C57BL/6的骨髓来源的MSC(Bouffi C,Bony C,Courties G,Jorgensen C,D.IL-6-dependent PGE2 secretion by mesenchymal stem cellsinhibits local inflammation in experimental arthritis.PLoS One.2010年12月7日;5(12):e14247)。简而言之,从长骨中冲洗出骨髓,将细胞悬液(0.5×106个细胞/cm2)铺板在基本必须培养基(MEM)-α中,其补充有10%胎牛血清(FBS)、2mM谷氨酰胺、100U/mL青霉素、100mg/mL链霉素和2ng/ml人碱性成纤维细胞生长因子(bFGF)。在亚汇合时,收集细胞,以5,000个细胞/cm2的密度进行增殖,从第7代开始使用。
药理预调理:

在离体心肌缺血再灌注模型中评价了预调理的MSC的心脏保护作用。在来自C57Bl6小鼠的分离的灌注心脏中进行急性心肌缺血再灌注,该小鼠经历30分钟缺血(无液流),然后再灌注。雄性小鼠腹膜内注射(IP)氯胺酮(14mg/kg,Merial)、赛拉嗪(14mg/kg,/>Bayer),然后注射戊巴比妥(IP;76.6mg/kg;Sanofi-Aventis)进行麻醉。麻醉小鼠接受250U肝素(IP)以防止血凝块形成。胸骨切开术后,切除心脏,通过升主动脉插管,快速安装在Langendorff系统上。在恒定压力和温度(37℃)下,灌注预热的Tyrode溶液(NaCl 140mM、KCl 5.4mM、MgCl 1mM、Hepes 5mM、葡萄糖5.5mM、CaCl2 1.8mM、pH 7.4)。
缺血再灌注操作方案(IR)
在Langendorff系统上,心脏用预热的Tyrode溶液灌注15分钟(稳定)。通过在30分钟内停止灌注液流(“无液流”),获得整体缺血。通过恢复液流实现再灌注步骤(60分钟)。再灌注期间应用使用MSC的治疗(Tyrode溶液中的MSC,5,000个细胞/ml)。仅使用Tyrode获得对照条件(IR)。
梗塞大小测量:
在IR操作方案结束时,将心脏从Langendorff中取出并解剖出来。将左心室包含在琼脂(4%)中,用振动切片机横向切片(1mm)。为显示组织活力,将切片与2,3,5-氯化三苯基四唑溶液(TTC染料)在37℃下孵育15分钟。经过固定步骤(4%多聚甲醛,48小时)后,对每个切片的每一侧进行拍照。使用ImageJ软件通过平面测量来定量梗塞大小,并表示为左心室质量的百分比。
结果:
图10显示,与对照条件(用Tyrode溶液灌注,IR)相比,用含有5,000个细胞/ml(共计4.105个细胞/心脏)的Tyrode溶液再灌注允许梗塞大小减小39.8%。
当在Langendorff系统上以4.105个细胞/心脏的剂量灌注时,与对照条件(单独使用Tyrode溶液再灌注,IR)相比,用GW501516(0.1μM)对MSC进行4天预调理,允许梗塞大小减少54.5%。。
当仅进行24小时GW501516预处理时,MSC(4.105个细胞/心脏)能够以相同程度减少梗塞大小,对于GW501516 1天(n=7)的平均值为37.4(以LV质量百分比计),而GW5015164天的平均值为36.05%(n=8),导致梗塞大小分别减少52.8%和54.5%。
使用另一种PPARβ/δ激动剂GWO742也获得了类似的结果。事实上,与未处理条件(IR)相比,当MSC用GWO742(1μM)预调理24小时,梗塞大小减少了54.6%。
结论:
上述数据表明,用GW501516预处理MSC 24小时所产生的药理刺激作用与96小时预处理所产生的药理刺激作用具有相同的心脏保护作用。
因此,PPARβ/δ激动剂对MSC进行短暂预处理,足以显著改善在心肌缺血再灌注损伤后的MSC的心脏保护作用。
尽管如此,我们的24小时操作方案优于WO2015164506的96小时操作方案(实施例6),有两个主要优点:
-预处理时间缩短4倍(1天与4天相比),
-细胞剂量少1000至100 000倍(5,000个细胞/ml对比5-600×106个细胞/ml,如WO2015164506实施例6中所述)。
Claims (14)
1.一种包含PPARβ/δ引发的间充质干细胞(MSC)的组合物,其中所述组合物是通过体外方法获得的,所述体外方法包括
a)提供MSC;
b)在包含PPARβ/δ激动剂的培养基中培养MSC;
c)去除所述培养基,洗涤所述MSC;和
d)将MSC收集在药学上可接受的载体中,其中所述药学上可接受的载体不包含PPARβ/δ激动剂,
所述组合物用于在需要其的受试者中治疗缺血再灌注损伤,优选心肌或肾脏缺血再灌注损伤的方法中,其中所述治疗方法不包括向所述受试者施用PPARβ/δ激动剂。
2.根据权利要求1所述的用途的组合物,其中所述组合物通过静脉内或冠状动脉内施用向受试者施用。
3.根据权利要求1或权利要求2所述的用途的组合物,其中施用至所述受试者的MSC的总量为5×106至5×1010个PPARβ/δ引发的MSC。
4.根据权利要求1至3中任一项所述的用途的组合物,其中所述组合物在缺血再灌注损伤开始后7天内、或在缺血器官的再灌注期间施用。
5.一种制备旨在用于治疗缺血再灌注损伤的包含间充质干细胞(MSC)的组合物的体外方法,所述体外方法包括:
a)提供MSC;
b)在包含PPARβ/δ激动剂的培养基中培养MSC;
c)去除所述培养基,洗涤所述MSC;和
d)将MSC收集在药学上可接受的载体中,其中所述药学上可接受的载体不包含PPARβ/δ激动剂,
其中步骤b)的持续时间为12小时至72小时,优选18小时至48小时,更优选20小时至30小时,如24小时。
6.根据权利要求5所述的方法,其中所述步骤a)中提供的MSC为骨髓来源的MSC或脂肪组织来源的MSC。
7.根据权利要求5或权利要求6所述的方法,其中在步骤a)中提供的所述MSC是同种异体的,并且已从健康受试者获得。
8.根据权利要求5至7中任一项所述的方法,其中在所述步骤b)的培养基中,所述PPARβ/δ激动剂的浓度为0.01μM至10μM,优选0.05μM至5μM,更优选0.1μM至1μM。
9.根据权利要求5至8中任一项所述的方法,其中所述PPARβ/δ激动剂选自GW0742和GW501516。
10.根据权利要求5至9中任一项所述的方法,其中所述步骤b)的培养基不含有用于转导MSC的病毒载体。
11.根据权利要求5至10中任一项所述的方法,其中所述步骤c)中的洗涤用不包含PPARβ/δ激动剂的溶液进行。
12.根据权利要求5至11中任一项所述的方法,其中在步骤d)中,将MSC以102至108个MSC细胞/mL的浓度,优选以103至107个MSC细胞/mL的浓度收集在药学上可接受的载体中。
13.一种包含PPARβ/δ引发的间充质干细胞(MSC)的组合物,其中所述组合物是通过根据权利要求1至12中任一项所述的体外方法获得的。
14.根据权利要13所述的组合物,其用于治疗方法中,其中所述治疗方法不包括向受试者施用PPARβ/δ激动剂。
