The embodiment of the disclosure discloses a traditional Chinese medicine composition for preventing and treating coronary atherosclerotic heart disease stable angina (coronary heart disease angina) and a preparation method of external application thereof. The traditional Chinese medicine composition comprises the following raw materials in parts by weight: 40-60 parts of ligusticum wallichii, 40-60 parts of cassia twig, 40-60 parts of allium macrostemon and 10-15 parts of borneol. The external application patch can be prepared from the traditional Chinese medicine composition as a raw material, so that coronary heart disease and angina pectoris can be prevented and treated.

Technical Field
The present disclosure relates to the field of traditional Chinese medicine, and in particular relates to a traditional Chinese medicine composition, an external patch and a method for treating coronary heart disease and angina pectoris.
Background
Coronary heart disease is also called coronary atherosclerotic heart disease, and refers to heart disease caused by myocardial ischemia and hypoxia due to stenosis or blockage of a lumen caused by coronary atherosclerosis. The three-volt patch is an important method for preventing and treating various chronic diseases in traditional Chinese medicine, and is used for applying yang-supporting medicines to patients suffering from diseases which are easy to occur or aggravate in winter at the time when the yang qi of the human body is most vigorous in summer according to the principle of nourishing yang in spring and summer in yellow emperor' inner menstruation, so that the effects of supporting yang, dispelling cold, tonifying vital energy and eliminating diseases are exerted.
Therefore, a medicament with clinically proven safety and effectiveness for preventing and treating coronary heart disease and angina pectoris is needed, and a dosage form convenient to use and a proper using method are needed.
Disclosure of Invention
In order to solve the problems in the related art, the embodiment of the disclosure provides a traditional Chinese medicine composition, an external patch and a method for treating coronary heart disease and angina according to the main traditional Chinese medicine syndrome type of coronary heart disease and angina.

With reference to the first aspect, in a first implementation manner of the first aspect, the Chinese medicinal composition further includes radix Angelicae sinensis, and the radix Angelicae sinensis is 10-15 parts by weight.

In a third aspect, embodiments of the present disclosure provide an external application patch for preventing and treating coronary heart disease and angina pectoris, which is characterized in that: the external application patch is prepared from the traditional Chinese medicine composition in the first aspect.
With reference to the third aspect, in a first embodiment of the third aspect, the high-quality traditional Chinese medicine decoction pieces meeting processing standards are taken as raw materials, the traditional Chinese medicine composition is weighed, crushed by adopting a low-temperature high-speed crushing technology, and added with a carrier to prepare paste. The low temperature may be 40 ℃ or lower, and the high speed may be 6000 revolutions or higher. The description of Ben Cao gang mu: treating all wind-cooling rheumatism, bone joint contracture pain, relieving muscle and opening striae, inhibiting liver qi, strengthening spleen and earth, and ironing yin arthralgia. "
Allium macrostemon: xin Wenwei, which is bitter, has the functions of activating yang, dispelling cold, promoting qi and removing stagnation, and is recorded in "true materia medica: xie, wei Xin Zhi san, san Zhi Xiao Zhi Ding from upper cold stagnation; bitter taste can decrease and descending can lead to cold stagnation; the air temperature is then dispersed, and the dispersion can immediately remove the middle-cold stagnation; the body slip is common, and the general one can lead to the immediate resolution of the cold stagnation of the long-term aeipathia, which is healed by the thoracoparalysis and stinging.

Chinese angelica root: sweet and pungent in flavor and warm in nature. It enters liver, heart and spleen meridians. Replenishing blood, promoting blood circulation, regulating menstruation, relieving pain, and loosening bowel to relieve constipation. Ben Cao Zheng (Ben Cao Zheng): dang Gui is sweet and heavy in flavor, so it can enrich blood, light and pungent in qi, so it can promote blood circulation, tonify middle energizer, nourish middle energizer, and hong Sheng Yao also. The external application has reliable curative effect on different channels and collaterals caused by blood stasis.
Preparing an attaching piece: pungent and sweet in flavor and large in heat. It can warm and tonify heart yang, and dispel heat, and is a strong drug for dispelling yin cold, eliminating rheumatism and relieving pain, and has good therapeutic effect on cold-dampness pain.
Point selection meaning:
the Danzhong acupoint belongs to the conception vessel, and is the qi-flowing function of the pericardium, the eight-meeting acupoint, and has the effects of benefiting upper jiao, relieving chest and diaphragm, reducing qi and dredging collaterals. "Ling Shu-Zhong Lun" records: fu chest and abdomen, guo of zang Fu. For those with the smell of the middle energizer, the heart governs the uterus.
The Xinshu acupoint belongs to the bladder meridian of foot taiyang and is the back Shu acupoint of the heart. The "Jiujiuyi Jing" records "cold and hot heart pain, circulating and leading to pain with the back". Both points are important points for treating chest stuffiness and pain, and the combination of any pulse with the bladder channel, one yin and one yang, and both channels play a role in regulating yin and yang of the whole body.
Winter disease summer treatment:

Example 5: acupoint plaster
The preparation method comprises the following steps:
taking all the raw materials according to any one of the proportions in the embodiments 1-4, crushing all the raw materials into traditional Chinese medicine powder, and then mixing the traditional Chinese medicine powder with vegetable oil and stirring uniformly; the paste after being stirred evenly is made into medicine cakes with the diameter of about 2.5cm and the thickness of about 5mm, the weight of the medicine cakes is about 1g, and the size of the sticking cloth is about 5cm multiplied by 5cm.
1. Clinical data
1.1 study subjects: the present disclosure collects 230 cases of coronary heart disease patients meeting the inclusion standard in the clinic of the western radix astragali hospital of the academy of Chinese traditional medicine, and the patients taking a doctor in the period of 201x 1 month to 201y 1 month, and the patients are divided into 116 cases of application groups and 114 cases of control groups according to the willingness of the patients. The application group selects the appropriate Chinese medicinal components according to the disease conditions of patients and applies the acupoint patches prepared according to the method of example 5.
1.2 diagnostic criteria: standards are established with reference to the International Association of cardiology and the society and the clinical naming standardization of the world health organization, united states patent group report, naming and diagnosis of ischemic heart disease.
1.3 inclusion criteria: age 18-80 years; meets the diagnosis standard of the coronary heart disease of Western medicine and has any one of the following: (1) a clear history of stale myocardial infarction, or a history of PCI, or a history of coronary bypass. (2) The coronary angiography or coronary CTA result suggests that at least one coronary stenosis is greater than or equal to 50%. (3) The loaded nuclide myocardial scan examination prompts the patients with coronary heart disease and myocardial ischemia. (4) Exercise flat electrocardiogram positive (limited male patient); with typical symptoms of angina pectoris; the subject is informed and voluntarily signs an informed consent.
2. Research method
2.1 study drug: the "winter disease summer treatment" three-volt patch which is developed by Xu Hao professor, chinese medical college of western radix astragali hospital pharmaceutical factory and produced by processing is adopted. The medicine comprises the following components: ligusticum wallichii, allium macrostemon and borneol. The specification of the medicine: the patch is about 5X5cm, the medicine cake is about 1g, and the diameter is about 2.5cm.
2.2, applying acupoints and the method: the patch is applied on the Shanzhong and two Xinshu acupoints. Apply 3 acupoints every first day, apply two Xinshu acupoints every second and third days. During the application, patients are instructed to fast food, such as cold, greasy, spicy, and seafood, mutton, and mushrooms. The specific locations of the two Xinshu acupoints are shown in FIG. 2.
2.3 applying time: according to the traditional Chinese medicine time medical theory, every first day of three volt days is the moment of opening the acupoint, and the application effect is optimal at the moment. The application is started every first volt day, the application is continuously carried out for 3 days, the application is carried out for 1 time a day, each time for 4-6 hours, and the application time can be properly adjusted according to individual differences. Two volts and three volts are the same.
2.4 evaluation of efficacy
2.4.1 angina pectoris curative effect: the treatment of coronary heart disease, angina pectoris and arrhythmia by combining Western and Chinese medicine in 1979 is referred to in "determination criteria for curative effect of coronary heart disease, angina pectoris and Electrocardiogram".
2.4.2 curative effects of Chinese medicine symptoms: recording the traditional Chinese medicine symptoms before and after observation in winter (11 months-2 months).
Fig. 1 shows a coronary heart disease angina score table of an embodiment of the present disclosure.
The symptoms of coronary heart disease and angina pectoris are classified into four grades of no, light, medium and heavy according to different severity, the classification standards are that no is-0, light-2, medium-4 and heavy-6, and the times, duration, pain degree, nitroglycerin dosage and the like of different severity are shown in figure 1. The curative effect judgment refers to the traditional Chinese medicine syndrome curative effect judgment standard of the traditional Chinese medicine new medicine clinical research guiding principle (trial). The effect is shown: the clinical symptoms and signs are obviously improved, and the integral of symptoms is reduced by more than or equal to 70 percent; the traditional Chinese medicine composition has the advantages that clinical symptoms and signs are improved, and the integral of symptoms is reduced by more than or equal to 30%; the clinical symptoms and physical signs are not obviously improved or even aggravated, and the integral of symptoms is reduced by less than 30 percent; weighting: the clinical symptoms and signs are all aggravated, and the integral of symptoms is reduced by <0%.
2.4.3 patient self-evaluation severity of disease: according to the Likett scale, the interval (0-10) is used to represent different severity of illness (0 is asymptomatic, 10 is the most serious illness), patients are asked to select corresponding numbers according to own illness, and the own illness severity of patients is compared before and after observation.
2.5 statistical treatment: statistical processing was performed using the IBM SPSS Statistics 20.0.20.0 software package. First, data are subjected to descriptive statistical analysis: counting data is described in terms of frequency, percentage or composition ratio; the metering data are described in terms of mean.+ -. Standard deviation (. + -.s). The two sets of comparison counting data are checked by X2; the measurement data is in t test in accordance with normal distribution, is in non-parametric test in accordance with normal distribution, gives test statistics and calculates corresponding P value, and has statistical significance that P is less than 0.05. The relevant factors influencing the re-hospitalization in winter are analyzed by adopting a data statistical method such as single factor analysis, binary logics regression and the like.
3. Results of the study
The present disclosure is actually incorporated into 230 patients. 116 cases of the plaster are applied, 4 cases of the plaster are lost, 1 case of the plaster is misreceived (heart failure is combined), and 1 case of the plaster is withdrawn due to adverse reaction; control group 114, 3, 1 by mistake (age > 80 years). Finally 220 patients with coronary heart disease enter the analysis, 110 patients with application group and 110 patients with control group. Wherein, the ages of 108 men and 112 women are 38-80 years, and the average ages are 64.04 +/-9.73 years. The two groups of patients have consistent baseline data such as age, sex, BMI, smoking history, past history, light and heavy grading of angina pectoris, basic medication and the like, and the difference has no statistical significance (P is more than 0.05).
3.1 angina pectoris curative effect: the total effective rate (67.27%) of angina pectoris attacks of patients in the application group in winter is superior to that of the control group (31.82%), and the difference has statistical significance (P is less than 0.05); the total reduction rate (77.40%) of the nitroglycerin in the application group is superior to that of the control group (28.60%), and the difference is statistically significant (P < 0.05), as shown in tables 1 and 2.

3.2 efficacy of symptoms in TCM: the total integral of the traditional Chinese medicine symptoms of the two groups of patients after observation is reduced compared with the traditional Chinese medicine symptoms of the two groups of patients after observation, and the improvement degree of the total integral of the traditional Chinese medicine symptoms of the application group is more obvious (P is less than 0.05) compared with the traditional Chinese medicine symptoms of the control group, and the details are shown in Table 3.

3.3 comparing the curative effects of angina pectoris of patients with yang deficiency and cold accumulation and non-yang deficiency and cold accumulation: the improvement condition of angina pectoris in winter of the patients with the syndrome of yang deficiency and cold accumulation is superior to that of the patients with the syndrome of non-yang deficiency and cold accumulation, and the difference has statistical significance (P is less than 0.05); the angina pectoris treatment effect of the two groups of patients with the two groups of symptoms of the control group is not obviously different (P is more than 0.05), and the details are shown in Table 4.

Note that: the number of effective cases in the table comprises effective patients; the number of invalid cases includes invalid and aggravated patients.
3.4 binary logistics regression analysis of relevant factors affecting winter re-hospitalization: prior to observation, a total of 57 patients in 220 study subjects were hospitalized for angina in the winter season of the last year (11-2 months). Follow-up after half a year, 18 of them were found to be in hospital again in winter for angina pectoris. The related factors affecting the patients with coronary heart disease and angina pectoris to be hospitalized in winter are analyzed in a single factor way, and the result shows that: the three-volt patch, BMI, blood circulation reconstruction and smoking history are relevant factors for influencing re-hospitalization in winter, P < 0.1 is taken as a screening standard, and the result is taken as a variable for entering binary logics regression analysis, and the three-volt patch is found to be an independent influencing factor [ P <0.01, RR=0.029, 95% CI (0.003,0.312) ] of the re-hospitalization event in winter of coronary heart disease patients, so that the three-volt patch for treating winter diseases and summer has a certain protection effect on reducing the re-hospitalization event in winter caused by angina pectoris of coronary heart disease patients, and the detailed table 5 is shown.

3.5 related investigation: comparing the evaluation scores of the two groups of patients before and after observation on the severity of the self-illness, the results show that: after observation, the disease severity of both groups of patients is improved, the application group reduces 83 patients, the control group reduces 56 patients, the total improvement rate (75.60%) of the application group is better than the total improvement rate (50.90%) of the control group, and the difference has statistical significance (P is less than 0.05), and the details are shown in Table 6.

3.6 safety evaluation: no serious adverse reaction event occurs during the application. The patients in 5 cases complain of mild adverse reaction, wherein 3 cases of patients have skin flushing with mild itching after application, 1 case of patients complain of skin abnormal reaction, and the 4 cases of patients can tolerate the skin abnormal reaction without affecting application; another 1 patient was unable to tolerate withdrawal from observation due to allergy to the adhesive tape.

Claims (15)
1. A Chinese medicinal composition for treating coronary atherosclerotic heart disease and stable angina pectoris is characterized in that,
the traditional Chinese medicine composition comprises the following raw materials in parts by weight:
30-60 parts of ligusticum wallichii, 30-60 parts of cassia twig, 30-60 parts of allium macrostemon and 10-15 parts of borneol.
2. The Chinese medicinal composition according to claim 1, wherein,
the Chinese medicinal composition also comprises 10-15 parts by weight of Chinese angelica.
3. The Chinese medicinal composition according to claim 2, wherein,
the traditional Chinese medicine composition also comprises an accessory preparation, wherein the accessory preparation is 10-15 parts by weight.
4. A medicament comprising the traditional Chinese medicine composition of any one of claims 1-3, and further comprising pharmaceutically acceptable excipients and/or carriers.
5. The medicament of claim 4, wherein the medicament is in a topical dosage form.
6. The medicament according to claim 5, wherein the external dosage form is at least one of the following: powder, ointment, lotion, spray, oil, gel, film, powder or patch.
7. An external application patch for preventing and treating coronary heart disease and angina pectoris, which is characterized in that: the external application patch is prepared from the traditional Chinese medicine composition as claimed in claims 1-3.
8. The method for producing an external application agent according to claim 7, characterized in that: taking high-quality traditional Chinese medicine decoction pieces meeting processing standards as raw materials, weighing the traditional Chinese medicine composition, crushing by adopting a low-temperature high-speed crushing technology, and adding a carrier to prepare paste.
9. An acupoint patch for preventing and treating coronary heart disease and angina pectoris, characterized in that the acupoint patch is prepared from the traditional Chinese medicine composition as set forth in claims 1-3.
10. The method for preparing an acupoint patch according to claim 9, comprising the steps of:
weighing the raw materials in the traditional Chinese medicine composition according to parts by weight;
crushing the raw materials into traditional Chinese medicine powder at a low temperature and high speed, and then uniformly mixing and stirring the traditional Chinese medicine powder and vegetable oil to obtain paste;
and (3) preparing the paste into a medicinal cake, and placing the medicinal cake on a patch cloth to obtain the acupoint patch.
11. The method according to claim 10, wherein,
the diameter of the medicine cake is 2.5-3 cm, the thickness is 5-8 mm, the weight is 1-1.2 g, and the patch is 4.5-7.5 cm multiplied by 4.5-7.5 cm.
12. Use of the traditional Chinese medicine composition according to claims 1-3, the medicament according to claims 4-6, the external application patch according to claim 7, or any one of the acupoint patches according to claim 9 in the preparation of a medicament for improving chest pain symptoms.
13. The use according to claim 12, characterized in that the kanzhong and the two Xinshu acupoints are applied using an acupoint patch.
14. The use according to claim 13, wherein 3 total of said two Xinzhong and two Xinshu acupoints are applied on the first day of each volt, and only said two Xinshu acupoints are applied on the second and third days.
15. The use according to any one of claims 12-14, wherein,
said improving coronary heart disease symptoms includes improving at least one of: the number of times and degree of the coronary heart disease and angina pectoris.
