Abstract
本发明提供与血管正常化相关的血浆蛋白作为标志物在制备肺腺癌早期诊断或预后的试剂或试剂盒中的应用,涉及蛋白组学及肿瘤学技术领域,所述血浆蛋白为GPI和PGD中的一种或其组合。本申请通过验证发现肿瘤组织中GPI的高表达会降低血管正常化水平,从而导致肺腺癌病人血浆中GPI表达水平的增高。同时血浆中PGD蛋白的表达与肿瘤组织血管正常化水平呈负相关,说明血浆中GPI和PGD的表达水平能反应肿瘤组织血管正常化水平,以预测肺腺癌病人的转移风险。在此基础上,构建小鼠荷瘤主动转移模型,发现有肺转移的原位肿瘤组织呈现出异常的血管形态,且GPI和PGD在有肺转移的小鼠的肿瘤组织中表达量最高,进而验证了血浆中GPI和PGD的浓度可以反映肿瘤内血管的正常化水平。

技术领域
本发明涉及蛋白组学及肿瘤学技术领域,尤其涉及与血管正常化相关的血浆蛋白作为肺腺癌早期诊断及预后标志物及其应用。
背景技术
肺癌是我国最常见的恶性肿瘤之一。由于持续增长的烟草消费、严重的环境污染以及生活方式的改变,肺癌已严重威胁到我国居民的生命和健康。非小细胞肺癌约占所有肺癌病例的85%,而肺腺癌是非小细胞肺癌最常见的组织学亚型,约占所有非小细胞肺癌的60%。更重要的是,肺腺癌的发病特征在不同性别间存在差异,近年来女性肺腺癌患者的发病率明显增加。由于肺腺癌早期通常无症状且转移风险较高,同时缺乏有效的筛查项目,因此导致很多肺腺癌患者在晚期才被诊断,预后很差。然而,事实证明在早期就被诊断的肺腺癌患者或接受过手术的患者更有可能获得较好的生存率,这表明现如今迫切需要一个有效的筛查和诊断策略,以便更早的发现和诊断肺腺癌。
尽管在肺腺癌的临床应用中通常推荐使用组织活检和组织生物标志物并且不可替代。但是组织活检的主要临床意义是通过病理学检查对良性和恶性肿瘤进行鉴别,通常是给予患者最终诊断的一步,难以用于肺腺癌的早期筛查和早期诊断。组织生物标志物可以指示治疗效果并预测肺腺癌患者的生存率。然而,组织生物标志物需要在发现肿瘤组织之后才能进行检测,且组织采样具有侵袭性,不仅难以获取,还可能会对患者造成一定的伤害,同时有些患者由于自己的意愿或其他医疗原因无法进行组织采样。有时为了获得足够的样本,患者需要重复活检,多次采样可能加重了患者的身体负担。而液体活检的出现有望弥补这些局限性,可能更适合那些不愿意或不能进行组织活检的肺腺癌患者。
液体活检的样本涉及血液、粘膜、胸腔积液、尿液和脑脊液等。由于血液样本的获取和分析比其他体液更方便、更实惠,血液已成为液体活检分析的关键液体。血液活检是一种有价值的诊断工具,可以支持鉴别诊断、估计预后和早期发现残余或进展中的疾病。血液活检的分析物包括循环肿瘤细胞、循环核酸、cfDNA的肿瘤来源部分、cfRNA、细胞外囊泡、肿瘤诱导血小板、蛋白质和代谢物等。
肿瘤异常血管的特征是内皮连接不连续和基底层缺失,这促使肿瘤细胞及其相关物质进入体内循环。因此,来源于肿瘤细胞的血浆蛋白生物标志物在识别肺腺癌方面可能更敏感和特异,因此需要识别血浆蛋白的来源。
发明内容
本发明的目的是为了解决现有技术中对于早期的肺腺癌缺乏有效的筛查和诊断策略的技术问题。
为了实现上述目的,本发明采用了如下技术方案:
一种与血管正常化相关的血浆蛋白作为标志物在制备肺腺癌早期诊断或预后的试剂或试剂盒中的应用。
优选的,所述血浆蛋白为GPI和PGD中的一种或其组合。
本申请还提供了一种肺腺癌早期诊断的试剂盒,所述试剂盒以血浆蛋白GPI和PGD中的一种或其组合作为标志物。
本申请还提供了GPI和PGD用于肺腺癌早期诊断的候选血浆蛋白生物标志物的验证方法,包含以下步骤:
S1:样本收集和资料整理;
S2:计算ROC曲线和AUC。
优选的,所述样本包括102名肺腺癌病例和102名健康对照者;所述病例组都经由病理组织学的证实,排除了有既往恶性肿瘤病史、其他器官转移及采血前接受过化疗或放疗的病人;健康对照组则是从接受常规健康检查的健康人群中,按照年龄、性别频数匹配的原则,随机抽选出来的,所有对照均是排除了有呼吸系统疾病或恶性肿瘤病史的健康对照。

实施例1:血浆蛋白标志物GPI和PGD在血浆中和组织中一致差异的表达且在转录组和蛋白组水平上一致差异的表达:
如图1所示,经过筛选发现GPI和PGD蛋白在肺腺癌患者的血浆中和肿瘤组织中均上调,且GPI和PGD在组织转录组(mRNA)和蛋白组水平上的差异表达也一致(GPI和PGD的mRNA和蛋白质均在肿瘤组织中表达上调)。
实施例2:验证血浆蛋白标志物GPI和PGD在血浆中的差异表达:
S1:样本收集和资料整理:
在实施方式中,在验证阶段纳入了62名女性肺腺癌病例和62名年龄匹配的健康对照组,并且为了保证实验的准确性,还纳入了40个男性肺腺癌病例和40个按年龄匹配的健康对照组进行候选蛋白的验证。验证阶段的人群来自三家医院。病例组都经由病理组织学的证实,排除了有既往恶性肿瘤病史、其他器官转移及采血前接受过化疗或放疗的病人。健康对照组则是从在某医院接受常规健康检查的健康人群中,按照年龄、性别频数匹配的原则,随机抽选出来的。所有对照均是排除了有呼吸系统疾病或恶性肿瘤病史的健康对照。
S2:通过ELASA实验验证血浆生物标志物在肺腺癌患者血浆中差异表达;
请参阅图2,通过ELASA实验发现GPI和PGD的表达在女性肺腺癌病例和男性肺腺癌病例的血浆中均为上调。合并男性和女性病例后,GPI和PGD在总的肺腺癌病例的血浆中都发现为高表达。
实施例3:GPI和PGD用于肺腺癌早期诊断的候选血浆蛋白生物标志物的验证实验:
为了研究GPI和PGD作为血浆生物标志物在肺腺癌诊断中的潜力,使用验证阶段的受试者,包括102名肺腺癌病例和102名健康对照者,计算ROC曲线和AUC。ROC曲线显示了肺腺癌病例和健康对照之间的强烈区别,GPI的AUC为0.795(95%CI:0.733-0.856)(图3A),PGD的AUC为0.839(95%CI:0.785-0.893)(图3B)。GPI对肺腺癌诊断筛查的敏感性和特异性分别为77.5%和74.5%,而PGD对肺腺癌诊断筛查的敏感性和特异性分别为87.3%和72.5%。通过将GPI和PGD结合,我们生成了一条ROC曲线,AUC为0.873(95%CI:0.824-0.922),敏感性为93.1%,特异性为72.5%(图3C)。
实施例4:GPI和PGD用于肺腺癌预后的候选血浆蛋白生物标志物的验证实验
(1)生存分析
使用Kaplan-Meier Plotter网站,通过Kaplan-Meier分析研究了肺腺癌患者中GPI和PGD的mRNA表达水平与预后的关系。如图4A-4B所示,GPI和PGD的表达与肺腺癌的预后有明显的相关性,GPI和PGD的过度表达会导致预后恶化(P<0.001)。此外,该结果在TCGA数据集中被验证,GPI高表达组的预后比低表达组要差(P=0.023)(图4C)。然而,在TCGA-肺腺癌数据集中没有发现PGD表达与预后之间的关联(P>0.05)(图4D)。
(2)功能分析
对GPI、PGD和相关的前50个基因进行GO和KEGG相关功能的探索,在分子功能方面,"相同的蛋白质结合"是最明显的富集的GO术语;GO分析显示,GPI、PGD和相关基因在"细胞膜"中高度富集;最普遍的生物过程分配是"糖酵解过程"(图5A-5B)。KEGG数据库中的路径作为分子相互作用和反应网络的代表,相关基因高度聚集在几个信号通路中,如"代谢通路"、"糖酵解/葡萄糖生成"、"碳代谢"和"磷酸戊糖通路"(图5C-5D)。
(3)GPI和PGD表达与肿瘤血管正常化之间的关系
尽管GPI和PGD在组织和血浆中都是上调的,但目前还不清楚血浆中GPI和PGD的表达是否与组织中GPI和PGD的表达相关。为此,我们测定了10个肺腺癌病例的相应肿瘤组织中GPI和PGD的蛋白表达,这些病例的血浆在前几步的研究中曾被用于血浆蛋白组学分析。图6A和图6B分别显示了肿瘤组织中GPI和PGD的免疫组织化学染色结果。进行相关分析以评估血浆和肿瘤组织中GPI和PGD表达之间的关系。检测到组织和血浆中GPI的表达有明显的正相关(R=0.93,P=1.2×10-4),而PGD的表达在组织和血浆中没有显示相关(R=0.13,P=0.710)(图6C)。
肿瘤细胞及其相关物质突破血管基底层,进入血管是血液活检的前提条件。而肿瘤微环境中不成熟、不稳定的血管为原发性肿瘤细胞及其相关物质进入血管提供了便利。为了确定GPI和PGD在肿瘤血管正常化中的作用,我们对10个肺腺癌病例的相应肿瘤血管进行了免疫荧光染色,这些病例的血浆先前被用于血浆蛋白组学分析(图6D)。相关分析显示,组织中GPI的表达与肿瘤血管的正常化水平之间存在负相关(R=-0.66,P=0.036)(图6E),表明肿瘤组织中GPI的高表达可能导致肿瘤血管的异常发展。肿瘤血管正常化的程度与血浆中GPI(R=-0.71,P=0.021)和PGD(R=-0.64,P=0.045)的表达都成反比(图6F),表明肿瘤血管的异常可以促进肿瘤组织中的物质渗入血液。
(4)小鼠荷瘤主动转移模型
我们构建了一个肺腺癌主动转移模型,探讨肿瘤组织中GPI和PGD的表达与肺转移之间的关联(图7A)。对无肺转移的原位肿瘤组织(a)和有肺转移的原位肿瘤组织(b)的血管进行了免疫荧光染色检测(图7B)。结果显示,有肺转移的原位肿瘤组织呈现出异常的血管形态(图7C),表明肿瘤血管的异常会增加肺转移的风险。
Western-blot检测显示,GPI和PGD在有肺转移的小鼠的肿瘤组织中表达量最高。有肺转移的小鼠(c)的肿瘤组织中GPI和PGD的表达高于无肺转移的小鼠(a)(图7D,图7E),表明肿瘤组织中GPI和PGD的高表达可能促进小鼠的肺转移。代表血管正常化水平的α-SMA的Western-blot结果与肿瘤血管的免疫荧光染色结果一致。有肺转移的小鼠肿瘤组织中α-SMA的表达(b)低于无肺转移的小鼠(a),且α-SMA的表达在有肺转移的肺肿瘤中最低(c)(图7E),说明血管的异常可能增加肺转移的风险。
(5)血浆GPI和PGD影响肿瘤转移的潜在机制
肿瘤异常血管的特征是内皮连接不连续和基底层缺失,这促进了肿瘤细胞及其相关物质进入体循环。那么,来自肿瘤细胞的血浆蛋白生物标志物在识别肺腺癌方面可能更加敏感和特异,所以最好能确定血浆蛋白的来源。
在本研究中,我们发现肿瘤组织中GPI的高表达可能导致肿瘤血管的异常,而肿瘤血管的异常可能促进癌细胞和相关物质进入循环,从而导致肿瘤转移。血浆中GPI和PGD的表达与肿瘤血管的正常化呈负相关,说明血浆中的GPI和PGD是通过肿瘤组织中的异构血管渗入血液的,这一点得到了支持。肿瘤细胞是高度糖酵解的,而糖酵解抑制了周围细胞的血管支持功能。靶向肿瘤细胞糖酵解能诱导肿瘤血管的正常化,从而减少癌细胞的入侵、内吞和传播。因此,作为糖酵解的关键酶蛋白,监测血浆中GPI和PGD的浓度可以反映肿瘤内血管的正常化水平,推断肿瘤转移的风险和恶性程度(图8)。
本申请中通过在肺腺癌病人肿瘤组织和血浆中差异表达一致的蛋白进行了探索,发现肿瘤组织中GPI蛋白的高表达会降低血管正常化水平,从而导致肺腺癌病人血浆中GPI蛋白表达水平的增高。同时血浆中PGD蛋白的表达与肿瘤组织血管正常化水平呈负相关,说明血浆中GPI和PGD的表达水平能反应肿瘤组织血管正常化水平,从而预测肺腺癌病人的转移风险。在此基础上,构建了小鼠荷瘤主动转移模型,发现有肺转移的原位肿瘤组织呈现出异常的血管形态,且GPI和PGD在有肺转移的小鼠的肿瘤组织中表达量最高,进一步验证了血浆中GPI和PGD的浓度可以反映肿瘤内血管的正常化水平,推断肿瘤转移的风险和恶性程度,为评估肺腺癌病人的预后做出贡献。

1.一种与血管正常化相关的血浆蛋白作为标志物在制备肺腺癌早期诊断或预后的试剂或试剂盒中的应用。
2.根据权利要求1所述的应用,其特征在于:所述血浆蛋白为GPI和PGD中的一种或其组合。
3.一种肺腺癌早期诊断的试剂盒,其特征在于:所述试剂盒以血浆蛋白GPI和PGD中的一种或其组合作为标志物进行检测。
4.GPI和PGD用于肺腺癌早期诊断的候选血浆蛋白生物标志物的验证方法,其特征在于:包含以下步骤:
S1:样本收集和资料整理;
S2:计算ROC曲线和AUC。
5.根据权利要求4所述的GPI和PGD用于肺腺癌早期诊断的候选血浆蛋白生物标志物的验证方法,其特征在于:所述样本包括102名肺腺癌病例和102名健康对照者;所述病例组都经由病理组织学的证实,排除了有既往恶性肿瘤病史、其他器官转移及采血前接受过化疗或放疗的病人;健康对照组则是从接受常规健康检查的健康人群中,按照年龄、性别频数匹配的原则,随机抽选出来的,所有对照均是排除了有呼吸系统疾病或恶性肿瘤病史的健康对照。
6.GPI和PGD用于肺腺癌预后的候选血浆蛋白生物标志物的验证方法,其特征在于:包括以下步骤:
