Detailed Description
The technical solutions of the present invention will be described clearly and completely with reference to the following embodiments, and it should be apparent that the described embodiments are some, but not all, embodiments of the present invention. All other embodiments, which can be obtained by a person skilled in the art without making any creative effort based on the embodiments in the present invention, belong to the protection scope of the present invention.
The clinical path is a programmed and standardized diagnosis and treatment plan with strict working sequence and standard time requirements, so that the aims of standardizing medical service behaviors, reducing resource waste and enabling patients to obtain proper medical care services are fulfilled. Inconsistent with clinical pathway planning is referred to as clinical pathway variation. The management of clinical pathways generally requires that, when a mutation occurs, a doctor needs to fill in the cause of the mutation. In the clinical pathway information system which is the mainstream in the market, generally, a maintenance person enters medical orders in a clinical pathway plan into the clinical pathway system, so that when a doctor selects the medical orders, clinical pathway variation does not occur, when the doctor prescribes medical orders which are inconsistent with the clinical plan, the doctor is prompted to change the clinical pathway, and the doctor is required to fill out the cause of the variation.
Due to the specificity of diseases and the diversity of medical advice dictionaries in a hospital, a clinical pathway information system is difficult to cover all similar medicines or examination medical advice, which causes a problem that even if a doctor prescribes similar medical advice in a clinical pathway, a doctor can be reminded of variation, and the excessive reminding of the variation of the clinical pathway influences the doctor to execute the clinical pathway. For example, a patient needs to replace the same kind of medicine due to allergy, but when the replaced medicine is not in the clinical pathway information system, a variation reminder is generated.
In addition, a hospital may introduce new medicines and examination items at irregular intervals, which means updating of an in-hospital medical order dictionary, but the clinical pathway information system is difficult to update in time, which may also cause incorrect variation reminding, and such excessive reminding of clinical pathway variation may cause troubles for the hospital to execute clinical pathways. Based on this, the embodiment of the invention provides a method, a device and an electronic device for determining clinical pathway variation, and the technology can be applied to a scene in which whether a medical order is determined to be clinical pathway variation or not is required to be determined. For understanding, first, a method for determining clinical pathway variation is provided in an embodiment of the present invention, as shown in fig. 1, the method includes:
step S102, a pre-constructed standardized term dictionary, a clinical path standardized medical advice list corresponding to the target disease species and a target medical advice dictionary aiming at the target disease species, which is obtained from a preset hospital medical advice dictionary, are obtained; and the standardized term dictionary and at least one part of the medical order dictionary in the hospital medical order dictionary are pre-established with a matching relation according to a preset matching rule.
The standardized term dictionary usually maintains keyword information of medicines, inspection items and the like, and can indicate the category to which the keyword corresponds, for example, the keyword of the medicine is Mo Junya, and the category to which the keyword belongs is antibacterial drugs and the like; the clinical pathway standardization order list is usually maintained according to categories; the target disease species can be a disease type determined by a doctor for diagnosing a patient, for example, the target disease species can be chronic obstructive pulmonary disease and the like; the target medical advice dictionary may be a medical advice dictionary for a target disease type selected by a doctor from an existing hospital medical advice dictionary, and may be, for example, a specific drug name, an examination item, or the like; in practical implementation, a matching relationship between the standardized term dictionary and the hospital order dictionary may be pre-established, where the hospital order dictionary with the matching relationship established is usually all the hospital order dictionaries, and may also be a part of the hospital order dictionaries, for example, a part of the order dictionaries are newly added, and the matching relationship between the newly added part of the order dictionaries and the standardized term dictionary is not maintained. The matching relationship is typically a one-to-many relationship, i.e., one term in the standardized term dictionary typically corresponds to multiple terms in the hospital order dictionary, e.g., a term dictionary in the standardized term dictionary is an immunosuppressant, which may include multiple drug keywords, each drug keyword may correspond to multiple dictionary data in the hospital order dictionary containing the drug keyword.
Step S104, if the target medical advice dictionary is matched with the standardized term dictionary, determining a matching result; and determining the order category of the target order dictionary according to the matching result.
The number of the target medical advice dictionaries can be one or more, for example, for a certain disease category, a doctor selects antiviral medicines and antibacterial medicines from the existing hospital medical advice dictionaries at the same time; considering that a doctor generally does not prescribe two similar medical orders for a disease, each target medical order dictionary usually only contains one medical order, for example, for antiviral medicines, only one medicine is usually selected; generally, only one kind of medicine is selected for antibacterial medicines.
The order category is generally associated with functions or effects that can be achieved by the drug or the examination item, for example, the target order dictionary is a methotrexate tablet that matches the keyword methotrexate in the standardized term dictionary, which may be corresponding to the matching result, and the category to which the methotrexate belongs in the standardized term dictionary is an immunosuppressant, which may be determined as the order category to which the methotrexate tablet belongs; for another example, the target medical order dictionary is a cefaclor capsule, which is matched with cephalosporium, a keyword in the standardized term dictionary, the cephalosporium can correspond to the matching result, and in the standardized term dictionary, the category to which the cephalosporium belongs is an antibacterial drug, so that the antibacterial drug can be determined as the medical order category to which the cefaclor capsule belongs.
And S106, if the clinical path standardized medical advice list contains medical advice categories, determining that the target medical advice dictionary does not belong to clinical path variation.
The matching relationship between the clinical pathway standardized order list and the standardized term dictionary is usually pre-established, and since the clinical pathway standardized order list is usually also category information, a one-to-one matching relationship between the clinical pathway standardized order list and the standardized term dictionary can be established, for example, if the clinical pathway standardized order list corresponding to the target disease type maintains the category of immunosuppressants, and the standardized term dictionary also maintains the category of immunosuppressants, the matching relationship between the clinical pathway standardized order list and the standardized term dictionary can be confirmed. Therefore, after the order type to which the target order dictionary belongs is determined, it can be determined whether the order type is included in the clinical pathway standardized order list, and if so, it can be considered that the order type is maintained in the clinical pathway standardized order list, and at this time, the target order dictionary is not recognized as clinical pathway variation.
The method for determining the clinical pathway variation comprises the steps of obtaining a pre-constructed standardized term dictionary, a clinical pathway standardized medical advice list corresponding to a target disease species and a target medical advice dictionary aiming at the target disease species, wherein the target medical advice dictionary is obtained from a preset hospital medical advice dictionary; if the target medical order dictionary is matched with the standardized term dictionary, determining a matching result; and determining the order category to which the target order dictionary belongs according to the matching result. And if the clinical path standardized order list comprises order categories, determining that the target order dictionary does not belong to clinical path variation. According to the method, the standardized term dictionary and the hospital order dictionary form a matching relation, the order type confirmed after matching is provided for the clinical pathway standardized order list, and the problem of excessive reminding of similar orders can be solved by confirming whether clinical pathway variation exists according to the order type.
The embodiment of the invention provides another method for determining clinical pathway variation, which is realized on the basis of the method of the embodiment, and comprises the following steps:
acquiring a pre-constructed standardized term dictionary, a clinical path standardized medical advice list corresponding to a target disease species and a target medical advice dictionary aiming at the target disease species, which is acquired from a preset hospital medical advice dictionary; the standardized term dictionary and at least one part of medical order dictionaries in the hospital are pre-established to have a matching relation according to a preset matching rule.
Firstly, a hospital standardized term dictionary is constructed, specifically, the hospital standardized term dictionary can be constructed according to the categories of medicines, treatments, examinations and the like according to the national related term standard documents and policy documents including national basic medical insurance medicine catalogs, ICD-10 disease codes and the like, and then the standardized term dictionary is stored into a Greenplus database by maintenance personnel. According to different terms, the maintainer can construct a matching rule configuration corresponding to the hospital advice dictionary and store the matching rule configuration into a Greenplus database. The matching rules mainly comprise functions of matching dictionaries, retrieving fields, matching methods, matching keywords, excluding keywords and the like. After the standardized term dictionary and the matching rule are constructed, a batch of hospital order dictionary data which can be matched with the standardized term dictionary is searched according to the matching rule, maintenance personnel can selectively carry out monomer or batch contrast association on the batch of hospital order dictionary data and the standardized term dictionary, and if the searched hospital order dictionary data does not meet the contrast requirement, the matching rule is modified until the contrast matching requirement is met. And after the comparison is completed, the maintenance personnel manually audits the compared information.
Step two, if the target medical advice dictionary is matched with the standardized term dictionary, determining a matching result; and determining the order category to which the target order dictionary belongs according to the matching result.
When a doctor orders a target medical order dictionary, according to the medical order category matched with the hospital standardized term dictionary, the medical order category is compared with the clinical path standardized medical order list of the corresponding disease category, and therefore whether the medical order belongs to clinical path variation or not is judged.
And step three, if the target medical advice dictionary is not matched with the standardized term dictionary, determining the target medical advice dictionary as a newly-added target medical advice dictionary.
And step four, acquiring the dictionary type to which the newly added target medical advice dictionary belongs.
And step five, determining the order categories of the newly added target order dictionaries based on the dictionary types and the standardized term dictionaries.
The dictionary types are generally the broad categories to which the target order dictionary belongs, such as, for example, belonging to medicines, examinations, etc.; when a doctor orders a corresponding target medical order dictionary and cannot be matched with the hospital standardized term dictionary, the target medical order dictionary is generally considered as a new hospital medical order dictionary, the type of the newly added target medical order dictionary can be automatically judged at the moment, and all standardized term dictionaries under the dictionary type are compared with the hospital medical order dictionary again according to corresponding dictionary rules. If the newly added target medical order dictionary is matched with the standardized term dictionary after the new execution, the newly added target medical order dictionary can be considered to be included in the standardized term dictionary, otherwise, the term clinical path variation of the newly added target medical order dictionary is considered. Finally, a complete term dictionary is obtained and can be provided for a clinical pathway system.
The fifth step can be specifically realized by the following steps a to D:
and step A, acquiring a target standardized term dictionary belonging to the dictionary type from the standardized term dictionary according to the dictionary type.
For example, an antibacterial drug-Mo Junya is newly added, and is not maintained in a standardized term dictionary at present, at this time, the system triggers automatic matching, and the system acquires the data type (corresponding to the dictionary type) of the newly added target order dictionary, such as medicine, examination, inspection and the like. The system can be matched that the antibacterial drug-Mo Junya belongs to a drug type dictionary, and at the moment, dictionaries such as dermatologic drug administration, immune system drug administration and the like under the drug type dictionary are the target standardized term dictionary.
And B, canceling the matching relation between the current target standardized term dictionary and the hospital advice dictionary.
And step C, reestablishing the matching relation between the target standardized term dictionary and the hospital advice dictionary according to the matching rule.
After the target standardized term dictionary is determined, because the associated matching relationship is established between the target standardized term dictionary and the hospital order dictionary, the associated matching relationship is cancelled, and the associated matching relationship is reestablished with the hospital order dictionary containing the newly added target order dictionary according to the respective corresponding dictionary rules, so that the newly added target order dictionary is maintained in the standardized term dictionary.
And D, determining the order category to which the newly added target order dictionary belongs according to the matching relation.
After the matching relationship is established, the order category to which the newly added target order dictionary belongs can be determined from the standardized term dictionary according to the matching relationship. For example, an antibacterial drug-Mo Junya (vancomycin hydrochloride for injection) is newly added in hospitals, and is not maintained in a clinical pathway system. Similarly, for a patient with acute exacerbation of chronic obstructive pulmonary disease, a doctor needs to prescribe the antibacterial drugs for treatment, and through the technology of the scheme, the medical order 'Mo Junya (vancomycin hydrochloride for injection)' is firstly matched with a hospital standardized term dictionary, and is judged to be a new medical order because no matching item exists. And then automatically matching the antibacterial drug with a standardized term dictionary according to a keyword matching technology to obtain the antibacterial drug. And then matching with the clinical pathway standardized doctor advice list of the chronic obstructive pulmonary disease, wherein the clinical pathway standardized doctor advice list of the chronic obstructive pulmonary disease contains antibacterial drugs, so that the quinolone drugs are not identified as clinical pathway variation, and the clinical pathway variation accords with the practical clinical application condition.
And E, if the matching rule is adopted, the matching relation between the target standardized term dictionary and the newly-added target medical advice dictionary in the hospital medical advice dictionary cannot be established, and the newly-added target medical advice dictionary is determined to belong to clinical pathway variation.
In practical implementation, the matching rule includes at least one of the following: the method comprises the following steps of searching fields, dictionary types, keywords, matching requirements of the keywords and relations among the keywords. If the matching relationship between the target standardized term dictionary and the target order dictionary cannot be established according to the matching rule, it can be considered that the order category to which the target order dictionary belongs is not maintained in the standardized term dictionary, and at this time, the target order dictionary can be identified as clinical pathway variation.
And step six, if the clinical path standardized order list contains order categories, determining that the target order dictionary does not belong to clinical path variation.
For example, for a patient with acute exacerbation of chronic obstructive pulmonary disease, a doctor needs to prescribe an antibacterial drug to treat the acute exacerbation, but penicillin drugs maintained in a clinical pathway system have poor treatment effect on the patient, and at the moment, the antibacterial drug needs to be replaced and quinolone antibacterial drugs are used for treatment. However, due to the fact that the quinolone antibacterial drugs are rarely used for patients with acute exacerbation of chronic obstructive pulmonary diseases and are not maintained in a clinical pathway system of the chronic obstructive pulmonary diseases, the drugs can generate variation reminding when being taken. Through the technology of the scheme, when a doctor develops a quinolone antibacterial drug, the doctor firstly matches with a hospital standard term dictionary, and the matching result is the antibacterial drug.
And seventhly, if the clinical path standardized order list does not contain order categories, determining that the target order dictionary belongs to clinical path variation.
To further understand the above embodiment, a flowchart of another method for determining clinical pathway variation is provided as shown in fig. 2, where standardized term dictionary data is constructed by constructing standardized term dictionary type data, comparison configuration data, specifically, rules for comparison matching, is constructed by constructing hospital order dictionary type data to import hospital order dictionary data, and according to the comparison configuration data, a maintainer may select a single dictionary or a batch comparison, the purpose of the comparison is to indirectly match the hospital order dictionary through the standardized term dictionary in a later clinical pathway information system, and after the comparison is completed and approved, the clinical pathway order is matched with the hospital standardized term dictionary to establish clinical pathway standardized order lists of different types, specifically, it may be understood that an association relationship between the clinical pathway order and the hospital standardized term dictionary is established. Matching the medical orders ordered by the doctor with the hospital standardized term dictionary, if the matching is successful, comparing the matched medical order categories with the clinical pathway standardized medical order list of the corresponding disease categories, if an incidence relation exists, determining that the medical orders do not belong to clinical pathway variation, and if the incidence relation does not exist, determining that the medical orders belong to clinical pathway variation. If the matching of the doctor's order and the hospital standardized term dictionary fails, the doctor's order can be determined to be the newly added hospital order dictionary, the dictionary type to which the doctor's order belongs is obtained, all standardized term dictionaries under the dictionary type are automatically compared according to the keyword matching rule, the hospital order dictionary is matched again, and the subsequent process is continuously executed.
According to the method for determining the clinical pathway variation, a hospital standardized term dictionary is constructed based on a Greenplus database, and the excessive reminding of the clinical pathway variation can be reduced by taking a keyword matching technology as a core. Firstly, a standardized term dictionary system is established, a contrast relation is formed between the term dictionary system and a hospital medical advice dictionary, medicines are managed according to medicine categories (instead of universal names), and the variation management of a clinical path information system is based on the standardized term dictionary, so that the problem of excessive reminding of similar medical advice is solved; and secondly, for new medicines and examination items, on the basis of a keyword matching technology, when a new hospital order dictionary item appears, the standard term dictionary is automatically matched, and the matched proper term dictionary is automatically brought into a comparison system, so that the problem of excessive variation reminding caused by updating of the hospital order dictionary is reduced.
Through the standardized term dictionary and the keyword matching technology, the clinical system can more accurately judge the clinical pathway variation condition and reduce the excessive reminding of the clinical pathway variation. And the maintenance requirement of a clinical path system can be reduced by an automatic in-hospital dictionary matching mode.
The maintained standardized term dictionary and the hospital order dictionary form correlation contrast, and the contrasted dictionary data are provided for clinical path service, so that the problem of excessive reminding of similar orders can be solved, and the problem of excessive reminding of variation caused by updating of the hospital order dictionary is effectively reduced.
An embodiment of the present invention provides a device for determining clinical pathway variation, as shown in fig. 3, the device includes: the acquisition module 30 is configured to acquire a pre-constructed standardized term dictionary, a clinical pathway standardized medical order list corresponding to the target disease category, and a target medical order dictionary for the target disease category acquired from a preset hospital medical order dictionary; the standardized term dictionary and at least one part of medical advice dictionary in the hospital medical advice dictionary are pre-established with a matching relation according to a preset matching rule; a first determining module 31 for determining a matching result if the target medical order dictionary matches the standardized term dictionary; determining the medical advice category to which the target medical advice dictionary belongs according to the matching result; the second determination module 32 determines that the target order dictionary does not belong to clinical pathway variation if the clinical pathway standardized order list includes order categories.
