Abstract

本发明公开了一种低切迹的股骨颈骨折内固定滑动套筒,包括筒体,筒体为内部中空结构,前后贯通,整体呈圆柱状,筒体的后端为斜切面,斜切面与筒体的主轴呈130°,筒体的后端底部连接下部凸起,下部凸起底部开有螺纹通孔,螺纹通孔与筒体内部连通,筒体顶部前端设有开槽,开槽与筒体内部连通。本套筒兼容目前已有的FNS(Femur Neck System)的动力棒、抗旋螺钉及锁定螺钉,无需单独开模,生产成本更低;具有FNS的防旋、抗剪切力和断端加压的优势的同时,无外置钉板设计解决了股骨外侧接骨板与目前已有的FNS同时使用时不兼容的问题,避免了人股骨差异性带来的钉板‑股骨不贴合问题,降低了钉板的断裂风险。

技术领域
本发明涉及医疗设备技术领域,具体涉及一种低切迹的股骨颈骨折内固定滑动套筒。
背景技术
瑞士Depuy Synthes公司研制了一种新型的股骨颈内固定系统(Femur NeckSystem,FNS),它由滑动套筒、动力棒、抗旋螺钉和锁定螺钉构成,有手术创伤小、防旋、抗剪切力,同时具备断端加压的作用。它在生物力学测试中相较于传统的三枚空心螺钉固定具有更好的稳定性,能够有效的防止外翻塌陷及旋转失效。
但是在临床实践中,FNS仍具有一定的问题。滑动套筒的外侧接骨板与套筒呈130°固定角度,而由于人股骨的解剖变异性,外侧接骨板无法与所有人的股骨外侧皮质贴合,接骨板翘起在临床实践中很常见。接骨板的不贴合会导致置钉困难及应力集中,若强行置入,则会带来螺钉断裂等风险。同时由于外侧接骨板较厚(5mm),在手术后患者软组织极易与接骨板摩擦,长期会导致无菌性炎症,慢性疼痛。对于多发伤病人,股骨颈骨折的同时常伴有股骨干骨折,而外侧接骨板较高的凸起使得术者无法联合应用股骨外侧接骨板来治疗股骨干骨折,兼容性较差。
发明内容
本发明的目的在于提供一种低切迹的股骨颈骨折内固定滑动套筒,以解决背景技术中提到的问题。为实现上述目的,本发明提供如下技术方案:一种低切迹的股骨颈骨折内固定滑动套筒,包括筒体,所述筒体为内部中空结构,前后贯通,整体呈圆柱状,所述筒体的后端为斜切面,所述斜切面与筒体的主轴呈130°,所述筒体的后端底部连接下部凸起,所述下部凸起底部开有螺纹通孔,所述螺纹通孔与筒体内部连通,所述筒体顶部前端设有开槽,所述开槽与筒体内部连通。
优选地,所述下部凸起的后端两侧设有凹槽,两个凹槽之间设有螺纹盲孔。
优选地,所述筒体前端开口两侧设有半圆形限位凸起。
优选地,所述下部凸起为圆柱状凸起。
优选地,所述螺纹通孔内部安装有锁定螺钉,所述锁定螺钉与筒体主轴夹角范围为80°-90°。
本发明的技术效果和优点:本套筒兼容目前已有的FNS的动力棒、抗旋螺钉及锁定螺钉,无需单独开模,生产成本更低;具有FNS的防旋、抗剪切力和断端加压的优势的同时,无外置钉板设计解决了股骨外侧接骨板与目前已有的FNS同时使用时不兼容的问题,避免了人股骨差异性带来的钉板-股骨不贴合问题,降低了钉板的断裂风险;内置螺钉的方案使用了80-90°的定位角度在带来更大抗旋角度的同时,也提供了更大的整体结构稳定性,也避免了传统FNS的锁定螺钉需要从股骨外侧臀大肌附着点进钉的问题,有效避免了破坏股骨外侧皮质高张力区域带来的术后发生股骨粗隆下骨折的风险,生物力学性能更好;套筒的尾部具有低切迹设计,使套筒尾部更贴合股骨外侧皮质,兼容市面上现有的股骨外侧接骨板,可以联合股骨外侧接骨板治疗股骨颈骨折合并股骨干骨折的患者,同时有效减少周围软组织与突出置入物摩擦导致的炎症风险与术后疼痛,利于患者术后康复。

为了使本发明的实现技术手段、创作特征、达成目的与功效易于明白了解,下面结合具体图示,进一步阐述本发明,在本发明的描述中,需要说明的是,除非另有明确的规定和限定,术语“安装”、“连”、“连接”应做广义理解,例如,可以是固定连接,也可以是可拆卸连接,或一体地连接或是机械连接,也可以是电连接;可以是直接相连,也可以通过中间媒介间接相连,可以两个元件内部的连通。
实施例
如图1-图5所示的一种低切迹的股骨颈骨折内固定滑动套筒,包括筒体1,筒体1整体呈圆柱状,筒体1的后端为斜切面,斜切面与筒体1的主轴呈130°,筒体1的后端底部连接圆柱状的下部凸起7,下部凸起7在套筒进入股骨6后起到一定抗旋作用。下部凸起7的后端两侧设有凹槽8,两个凹槽8之间设有螺纹盲孔9,与把持器接合,起到固定把持器的作用。
筒体1为内部中空结构,前后贯通,内径等于动力棒直径,用于插入动力棒2及抗旋螺钉3。筒体1前端开口两侧设有半圆形限位凸起10,限位凸起10可与动力棒2两侧凹槽结合,起到限位防止动力棒2旋转及过伸的作用。筒体1顶部前端设有供抗旋螺钉3滑动的开槽11,开槽11与筒体1内部连通,可使动力棒2及抗旋螺钉3在筒体1内自由滑动。
下部凸起7底部开有螺纹通孔12,螺纹通孔12与筒体1内部连通,可使锁定螺钉4穿过并固定筒体1与股骨6,并与筒体1主轴夹角范围为80°-90°。安装完成后,锁定螺钉尾低于筒体1内壁,不影响动力棒2的滑动。
在骨折处13使用该套筒时,在使用克氏针对动力棒2置入方向进行定位后,使用电钻钻出开孔,选择合适长度的动力棒2,从筒体1尾部置入,将把持器插入下部凸起7后端的凹槽8,紧固筒体1及动力棒2;将动力棒2穿过导针后顺着导针方向推入钻孔的合适位置,透视下确认放置无误后,移除导针;顺着把持器的抗旋螺钉孔使用电钻钻孔并拧入抗旋螺钉3,将抗旋螺钉3与主钉锁定;根据锁定螺钉导向器使用电钻钻孔并拧入锁定螺钉4,移除把持器。

2.根据权利要求1所述的一种低切迹的股骨颈骨折内固定滑动套筒,其特征在于:所述下部凸起的后端两侧设有凹槽,两个凹槽之间设有螺纹盲孔。
