技术领域
本发明涉及脊柱侧弯分析辅助方法。
背景技术
脊柱侧弯(Scoliosis),也为脊柱侧凸,是原发或继发的,以脊柱骨性结构三维变化的疾病。脊柱侧弯患者的脊柱会呈现各节段的侧弯、旋转、前后凸凹的三维变化。脊柱侧弯根据患者发病的早晚、发病的原因、脊柱结构变化特点,分为不同的类型。先开性的脊柱及下肢骨骼畸形、原发或继发的神经肌肉疾病,均可能发生脊柱侧弯症状。其中最多见的是原发性青少年脊柱侧弯(adolescent idiopathic scoliosis, AIS),多在青春期前发病,病因不明;目前研究认为该病脊柱受累可能与神经肌肉控制有关。而继发性脊柱侧弯包括骨骼畸形、帕金森病、脑瘫等;在这些疾病中,维持脊柱稳定的神经、肌肉,或骨骼产生了一定的病理变化,最终导脊柱变形。所以,无论是原发性脊柱侧弯,或者是继发性脊柱侧弯,分析神经肌肉问题是此类患者功能评估、精准治疗的需求。
若能在医生进行脊柱侧弯分析诊断时提供辅助,向医生显示患者的三维骨骼肌肉数字模型,并叠加显示患者在多个体位进行测试时所采集表面肌电信号的分析判断结果,并向显示分析报告,则可提高医生脊柱侧弯分析诊断的准确率和效率。
发明内容
为了辅助医生进行脊柱侧弯分析诊断,本发明提供一种脊柱侧弯分析辅助方法,包括如下步骤:
1)将患者脊柱的CT数据导入融合超声,使用超声定位患者棘突节段并标记;
2)在CT数据融合下,进行患者肌肉形态超声测量,建立患者三维骨骼肌肉数字模型并通过屏幕显示;
3)采集患者的表面肌电信号:
采集点包括:T2棘突平面中线两侧2cm处,T7棘突平面中线两侧2cm及4cm处,T12棘突平面中线两侧2cm及4cm处,L3棘突平面中线两侧2cm及4cm处,髂前上嵴与肋骨垂直距离的中点;
使患者处于多个体位进行测试采集:先在患者处于静止俯卧位、左侧屈曲俯卧位、右侧屈曲俯卧位时,采集患者安静放松的肌电信号;然后在患者处于俯卧位抬头、仰卧位卷腹时,分别采集患者腹肌及背部采集点的表面肌电信号;
通过神经网络计算分析采集的表面肌电信号,判断表面肌电连接情况、信号强弱、数据质量以及重复测量次数;表面肌电信号的分析判断结果通过颜色深浅方式,动态、实时地叠加显示至已建立的三维骨骼肌肉模型上,并可调整视角立体观察;
4)采集完成,显示分析报告:
分析报告包括通过神经网络计算分析得到的如下内容:测试采集过程中的肌电实时信息情况,测试采集结束后的肌电网络分析报告,两次或多次差异分析报告,脊柱侧弯责任肌溯源报告。
优选的,步骤3)中,采集点还包括:T1至L3棘突平面中线两侧2cm及5cm处。
优选的,采用无线表面肌电感应器采集患者的表面肌电信号。
优选的,将无线表面肌电感应器预设在肌电采集背心上,患者穿上肌电采集背心进行表面肌电信号的采集。

下面结合附图和实施例,对本发明的具体实施方式作进一步描述。以下实施例仅用于更加清楚地说明本发明的技术方案,而不能以此来限制本发明的保护范围。
本发明提供一种脊柱侧弯分析辅助方法,包括如下步骤:
1)将患者脊柱的CT数据导入融合超声,使用超声定位患者棘突节段并标记;
2)在CT数据融合下,进行患者肌肉形态超声测量,建立患者三维骨骼肌肉数字模型并通过屏幕显示;
3)采集患者的表面肌电信号:
采用无线表面肌电感应器采集患者的表面肌电信号,采集点包括:T2棘突平面中线两侧2cm处,T7棘突平面中线两侧2cm及4cm处,T12棘突平面中线两侧2cm及4cm处,L3棘突平面中线两侧2cm及4cm处,髂前上嵴与肋骨垂直距离的中点,T1至L3棘突平面中线两侧2cm及5cm处;
采集前,先对患者采集点处的皮肤涂磨砂膏并用无水酒精清洁;

更具体的,本发明还提供一种脊柱两侧椎旁肌肌电信号的采集方法,采用可安装无线表面肌电感应器的横条和可安装横条的背心进行采集;
所述背心包括背心前部和背心后部;背心后部设有可露出患者后背脊柱区域的镂空区域,镂空区域为竖置长方形;背心后部设有下延条,下延条的内侧面底端设有横向魔术贴条,背心前部的外侧面设有可与横向魔术贴条粘接的纵向魔术贴条;
所述横条的中部设有用于与患者脊柱节段对准的镂空定位点,镂空定位点为圆形;横条的正面两端分别设有魔术贴头;横条正面在镂空定位点两侧分别设有用于安装无线表面肌电感应器的安装位,且镂空定位点两侧的安装位以镂空定位点对称设置;横条正面在镂空定位点的同一侧设有两个安装位,该两个安装位与镂空定位点的间距分别为2cm和4cm;

然后按照需要采集的脊柱节段的上下位置,由上至下或由下至上依次安装横条;安装横条前,先对患者背部皮肤涂磨砂膏并用无水酒精清洁;安装横条时:将横条横置,使横条正面朝向患者后背,并将横条的镂空定位点对准相应的脊柱节段;然后将横条面朝两端的魔术贴头分别粘接固定在背心后部外侧面的魔术贴片上;此时,横条的对称安装位上的无线表面肌电感应器分别贴紧在患者后背上,且对称安装位上的无线表面肌电感应器与相应脊柱节段两侧的椎旁肌一一对应;
然后通过无线表面肌电感应器采用对应椎旁肌的肌电信号;
肌电信号采集完成后,将横条从背心后部外侧面上取下,将背心后部的下延条从背心前部外侧面上解开,将背心从患者身上脱下。
本发明提供的脊柱两侧椎旁肌肌电信号的采集方法,采用可安装无线表面肌电感应器的横条和可定位横条的背心,便于安装和定位无线表面肌电感应器,便于采集患者脊柱两侧椎旁肌的肌电信号。

3.根据权利要求1所述的脊柱侧弯分析辅助方法,其特征在于,采用无线表面肌电感应器采集患者的表面肌电信号。
4.根据权利要求3所述的脊柱侧弯分析辅助方法,其特征在于,将无线表面肌电感应器预设在肌电采集背心上,患者穿上肌电采集背心进行表面肌电信号的采集。
5.根据权利要求1所述的脊柱侧弯分析辅助方法,其特征在于,采集患者的表面肌电信号之前,先对患者采集点处的皮肤涂磨砂膏并用无水酒精清洁。
