Abstract
本发明公开了一种锰基放疗增敏剂,其为负载小分子PI3Kγ激酶抑制剂的聚乙二醇化空心氧化锰纳米颗粒材料;其中,所述小分子PI3Kγ激酶抑制剂为IPI549。本发明还提供上述锰基放疗增敏剂的制备方法及其在制备肿瘤治疗剂中的应用。本发明所述的锰基放疗增敏剂具有放疗增敏和免疫调节双重功能,可应用于抑制残余肿瘤复发;二氧化锰纳米颗粒在酸性肿瘤微环境中降解,实现肿瘤部位响应性药物释放,且可实现术后缺氧缓解的放疗;PI3Kγ抑制放疗介导的免疫原性细胞死亡效应协同作用,导致切除后免疫抑制TME重编程为免疫原性表型,同时提高了对免疫检查点封锁治疗的易感性。基于锰基放疗增敏剂的放疗联合PD‑L1阻断可显著抑制局部残留肿瘤和远处转移肿瘤,以及预防肿瘤再接种。
Description
技术领域
本发明涉及生物医药技术领域,尤其涉及一种锰基放疗增敏剂及其制备方法和应用,具体地,所述锰基放疗增敏剂为负载小分子PI3Kγ激酶抑制剂的聚乙二醇化空心氧化锰纳米颗粒材料。
背景技术
随着纳米技术的快速发展,功能化纳米材料可以有效地作为对抗实体肿瘤的氧依赖标准治疗的辅助“药物”。其中,氧化锰基纳米系统(二氧化锰NS),作为一种独特的肿瘤微环境响应剂,得到了广泛的研究。研究表明,二氧化锰纳米粒子可以催化肿瘤微环境内过表达的过氧化氢产生氧气,从而显著缓解肿瘤缺氧状态。同时,二氧化锰纳米粒子还可以进一步分解为水溶性Mn2+离子,实现增强的T1磁共振成像效果,并被肾脏快速的排出。此外,具有大空腔的空心二氧化锰纳米结构已被证明是加载治疗药物的优秀药物传递系统,并且其释放可以通过调节壳涂层或结构来精确控制。
外科手术是大多数实体肿瘤的一线治疗手段。尽管手术技术和器械技术不断进步,仍有高达30%-40%的患者在术后5年复发,这主要是由手术边缘残留的微肿瘤灶造成的。随着免疫治疗的发展,各种免疫治疗策略已经被广泛应用于临床前研究甚至临床试验用于防治肿瘤复发。不幸的是,尽管产生了某些抗原特异性细胞毒性T淋巴细胞,但这种免疫反应在术后常常处于抑制状态。放射疗法,作为免疫疗法的最佳拍档,由于其安全且具有免疫刺激作用,常与免疫疗法协同作用用于抑制肿瘤。然而,常规的放疗方案的疗效通常受到肿瘤缺氧相关放疗耐药性的限制。以往许多临床试验和研究表明,恢复肿瘤的氧合水平可以显著提高外射线的疗效。因此,通过克服乏氧能够有效介导放疗的疗效。
目前已初步设计出一种可有效负载相关药物的空心MnO2复合纳米材料,但其对不同药物的装载有待进一步研究,其结构也仍需进一步地改进,且现有技术中并未涉及将小分子PI3K激酶抑制剂直接负载至空心MnO2复合纳米材料进行应用的报道。
发明内容
为了克服现有技术中存在的问题,本发明提供了一种锰基放疗增敏剂,其为一种负载小分子PI3Kγ激酶抑制剂(IPI549)的智能聚乙二醇化空心氧化锰纳米平台(IPI549@HMP),通过空心二氧化锰颗粒和IPI549的物理吸附形式组装而成,其具有放疗增敏和免疫调节双重功能,可应用于抑制残余肿瘤复发。在上述IPI549@HMP系统中,HMP纳米颗粒在酸性肿瘤微环境中降解,实现肿瘤部位响应性药物释放;同时,HMP纳米壳具有优异的过氧化氢酶活性,可将内源性过氧化氢分解为O2,从而实现术后缺氧缓解的放疗。另一方面,PI3Kγ抑制与众所周知的放疗介导的免疫原性细胞死亡(ICD)效应协同作用,导致切除后免疫抑制TME重编程为免疫原性表型,同时提高了对免疫检查点封锁(ICB)治疗的易感性。相应的实验结果表明,基于IPI549@HMP的放疗联合PD-L1阻断可显著抑制局部残留肿瘤和远处转移肿瘤,以及预防肿瘤再接种。上述联合放疗免疫策略能够全身性的调节术后免疫抑制微环境,其特异性、高效性、低毒性的方法在术后肿瘤治疗方面具有潜在的临床应用。
为实现上述目的,本发明采用如下技术方案:
本发明的第一个方面是提供一种锰基放疗增敏剂,其为负载小分子PI3K激酶抑制剂的聚乙二醇化空心氧化锰纳米颗粒材料。
进一步地,所述小分子PI3K激酶抑制剂选自PI3Kγ、PI3Kd、PI3Ka、PI3Kδ等激酶抑制剂中的一种,更优选为小分子PI3Kγ激酶抑制剂。
进一步地,所述小分子PI3Kγ激酶抑制剂为IPI549。
进一步地,在所述锰基放疗增敏剂中,IPI549的载药率为20~80wt%。
进一步地,所述聚乙二醇化空心二氧化锰纳米颗粒的粒径为140~155nm,载药后获得的锰基放疗增敏剂的粒径为156~170nm。
本发明的第二个方面是提供任一上述的锰基放疗增敏剂的制备方法,其包括:
步骤1)制备聚乙二醇化空心氧化锰纳米颗粒(HMP);
步骤2)将预定浓度的所述聚乙二醇化空心氧化锰纳米颗粒的水溶液与预定投入量的小分子PI3Kγ激酶抑制剂混合,以物理吸附形式组装形成所述锰基放疗增敏剂(IPI549@HMP)。
进一步地,在所述步骤1)中,所述聚乙二醇化空心氧化锰纳米颗粒的制备步骤包括:
步骤1A)将乙醇、水、氨水按照预定的体积比混合搅拌,缓慢滴加正硅酸乙酯反应,随后室温离心,分别采用乙醇、水洗涤,得到实心二氧化硅纳米颗粒;
步骤1B)将步骤1A)中得到的所述实心二氧化硅纳米颗粒分散在水中,在超声条件下采用注射泵按预定速率逐滴加入高锰酸钾水溶液进行还原反应得到实心二氧化硅包覆二氧化锰纳米颗粒,将得到的所述实心二氧化硅包覆二氧化锰纳米颗粒溶解在Na2CO3水溶液中进行蚀刻反应得到空心二氧化锰纳米颗粒;
步骤1C)将步骤1B)中得到的所述空心二氧化锰纳米颗粒的水溶液按照预定比例依次与聚烯丙基胺水溶液、聚丙烯酸、氨基化的聚乙二醇混合反应得到所述聚乙二醇化空心氧化锰纳米颗粒。
进一步地,所述步骤1A)中,乙醇、水、氨水的体积比为26~30:3~5:1,反应时间为1~2h,反应温度为38~42℃;优选,氨水的浓度为30%,乙醇、水、氨水的体积比为28:4:1,反应时间为1.5h,反应温度为40℃。
进一步地,所述步骤1A)中,离心后乙醇洗涤一遍,水洗涤两遍。离心过程中加入乙醇洗涤能够更好的去除多余的有机溶剂。
进一步地,所述步骤1B)中,注射泵的注射速率为55~65mL/h,优选以60mL/h的速率滴加高锰酸钾溶液,这能够使锰包覆得更均匀,其中实心二氧化硅纳米颗粒与高锰酸钾的质量比为1:15~25,优选1:20;蚀刻反应的条件为:50~60℃反应8~12h,优选55℃水浴搅拌下反应10h,其得到的所述空心二氧化锰纳米颗粒的粒径为135~145nm,比表面积为340~360m2·g-1,孔径为3.5~4.5nm,在一具体实施方案中空心二氧化锰纳米颗粒的粒径为140nm左右,比表面积为349.5m2·g-1,孔径为4.1nm。
进一步地,所述步骤1C)中,所述空心二氧化锰纳米颗粒与聚烯丙基胺水溶液、聚丙烯酸、氨基化的聚乙二醇质量比为1:4~6:4~6:4~6,获得的所述聚乙二醇化空心二氧化锰纳米颗粒的粒径为140~155nm;在一具体实施方案中,所述空心二氧化锰纳米颗粒与聚烯丙基胺水溶液、聚丙烯酸、氨基化的聚乙二醇质量比为1:5:5:5,其中与聚烯丙基胺水溶液及聚丙烯酸的反应时间均为2小时,与氨基化的聚乙二醇的反应时间为12小时,获得的所述聚乙二醇化空心二氧化锰纳米颗粒的粒径为146nm左右。
进一步地,所述步骤2)中,所述聚乙二醇化空心氧化锰纳米颗粒的水溶液与小分子PI3Kγ激酶抑制剂的质量比为1:0.5~7(例如:1:1,1:3,1:5),其载药率为20~80%(例如32~70%左右,左右可为±3%,±5%等),载药后的粒径为156~170nm,具体可为162nm左右。在一具体实施方案中,不同质量比的载药率如下:1:1载药率为32.2±2.9%,1:3载药率为53.9±5.2%,1:5载药率为69.8±4.7%。
进一步地,在上述步骤中,除了蚀刻反应,各步骤中反应结束均进行离心操作步骤,操作条件为:室温,10000~12000转离心8~12分钟,优选11000转离心10分钟。
本发明的第三个方面是提供一种锰基放疗增敏剂在制备肿瘤治疗剂中的应用,所述锰基放疗增敏剂为任一上述的锰基放疗增敏剂或由任一上述的制备方法制得的锰基放疗增敏剂。
进一步地,所述肿瘤治疗剂为术后肿瘤治疗剂和/或消融后残癌复发治疗剂。其中,术后是指外科手术治疗后,消融后是指微波消融、射频消融和高强度聚焦超声(HIFU)术治疗后。
进一步地,所述肿瘤治疗剂为锰基放疗增敏剂与X射线照射的联合治疗,所述肿瘤治疗剂为抑制局部残留肿瘤、抑制消融后肿瘤复发的治疗剂;或,所述肿瘤治疗剂为锰基放疗增敏剂的放疗联合PD-L1阻断,所述肿瘤治疗剂为全身性的调节术后免疫抑制微环境、抑制局部残留肿瘤和远处转移肿瘤、抑制消融后肿瘤复发、预防肿瘤再接种的治疗剂。
进一步地,在一具体实施方案中,小鼠经尾静脉注射IPI549@HMP,其中MnO2=7.5mg·kg-1,IPI549=1.5mg·kg-1,其采用的放射治疗剂量为3Gy。在又一具体实施方案中,在接受IPI549@HMP的同时静脉注射采用PD-L1阻断治疗,抗PD-L1注射剂(剂量=3.75mg·kg-1)。
进一步地,在一具体实施方案中,IPI549@HMP血液循环的半衰期为0.97h(e)。
与现有技术相比,本发明采用上述技术方案具有以下有益效果:
(1)传统的放疗使用的剂量高,毒副作用大,本发明采用的放射免疫增敏剂能够在3Gy的剂量下实现增强的放疗疗效。
(2)传统的术后放疗忽视了围术期免疫抑制微环境,导致疗效不佳,本发明采用的放射免疫增敏剂通过降低MDSC,促进M2型巨噬细胞向M1型巨噬细胞极化,促使术后免疫抑制微环境逆转为免疫刺激微环境,有利于实现增强的疗效。
(3)传统的放疗效果受到肿瘤内部乏氧微环境的限制,本发明采用的放射免疫增敏剂通过分解肿瘤部位过剩的过氧化氢原位产生氧气,从而缓解乏氧增强放疗疗效,另一方面,释放的锰离子能够实现T1增强的磁共振成像,有利于肿瘤的诊断。
(4)本发明采用的放射免疫增敏剂联合免疫检查点阻断疗法不仅有效的抑制了原发肿瘤,还诱发了强烈的全身抗肿瘤免疫反应,甚至可以预防肿瘤再接种;同时,该放射免疫增敏剂经实验证实在给药范围内全身毒性可以忽略不计。
(5)与现有的空心MnO2复合纳米材料的制备方法相比,本发明对制备步骤及相关工艺参数进行了适当的优化,合成的空心二氧化锰颗粒粒径在146nm左右,载药以后的粒径在162nm左右,上述步骤优化后制得的空心二氧化锰颗粒复合材料更利于IPI549载药及释放;由于IPI549是小分子物质,其能够更轻易地被空心二氧化锰所具有的大空腔所吸附,且上述空心二氧化锰颗粒复合材料与IPI549的载药量随着IPI549的投入量增多而增多。
(6)与现有的BSA-MnO2-IPI549(采用牛血清白蛋白用作药物载体以封装MnO2纳米颗粒和IPI549)相比,其采用的是吸附氧化法,合成过程需要调节PH值,较为复杂;而本发明涉及的氧化锰是通过高锰酸钾还原法制备而成,具有能够在室温下快速合成,便于合成MnO2基复合材料,且能够基于模板改变自身形态,在水溶液中具有分散性好等优点,其治疗效果达到了50%的治愈率。

下面将结合本发明实施例中的附图,对本发明实施例中的技术方案进行清楚、完整地描述,显然,所描述的实施例仅仅是本发明一部分实施例,而不是全部的实施例。基于本发明中的实施例,本领域普通技术人员在没有作出创造性劳动的前提下所获得的所有其他实施例,都属于本发明保护的范围。下列实施例中未注明具体条件的实验方法,通常按照国家标准测定。下述实施例中未注明出处的实验材料,均为市售原料。下述实施例中的各步骤中采用的设备均为常规设备。若没有相应的国家标准,则按照通用的国际标准、常规条件、或按照制造厂商所建议的条件进行。除非另外说明,否则所有的份数为重量份,所有的百分比为质量百分比。除非另有定义或说明,本发明中所使用的所有专业与科学用语与本领域技术熟练人员所熟悉的意义相同。此外任何与所记载内容相似或均等的方法及材料皆可应用于本发明方法中。
需要说明的是,在不冲突的情况下,本发明中的实施例及实施例中的特征可以相互组合。下面结合附图和具体实施例对本发明作进一步说明,但不作为本发明的限定。
实施例1–锰基放疗增敏剂的制备
本实施例提供一种具有放疗增敏和免疫调节双重功能的用于抑制术后肿瘤复发的锰基放疗增敏剂(IPI549@HMP)的制备方法,如图1所示,其首先采用经典的stober方法制备了单分散硬模板固体二氧化硅球(sSio2),与高锰酸钾溶液反应后,在二氧化硅球表面涂覆均匀的二氧化锰层;然后,用碳酸钠溶液对得到的sSiO2@MnO2进行蚀刻,得到空心的二氧化锰(HMnO2);然后,用聚乙二醇(PEG)对获得的空心二氧化锰纳米颗粒进行改性,获得聚乙二醇化空心氧化锰纳米颗粒(HMP),以提高其生理稳定性;最后,将IPI549加载到HMP的中空结构中,得到IPI549@HMP纳米体系,用于进一步的实验。
上述锰基放疗增敏剂的具体制备步骤包括:
(1)将乙醇、水、氨水(30%)按照体积比28:4:1混合搅拌(转速500)在40℃水浴中,随后缓慢滴加500μL正硅酸乙酯反应1.5h,随后室温离心(11000转,10分钟)乙醇洗一遍、水洗两遍获得实心二氧化硅纳米颗粒;
(2)将步骤(1)中获得的30mg实心二氧化硅纳米颗粒分散在10mL水中,在超声条件下采用注射泵按照60mL/h的速率逐滴加入20mL高锰酸钾水溶液(30mg/mL),随后室温搅拌过夜,离心(11000转,10分钟)水洗三遍获得实心二氧化硅包覆二氧化锰纳米颗粒;
(3)将步骤(2)获得的实心二氧化硅包覆二氧化锰纳米颗粒溶解在20mL Na2CO3(212mg/mL)水溶液中,在55℃水浴搅拌下反应10h,水洗三遍获得空心二氧化锰纳米颗粒(粒径在140nm左右,比表面积在349.5m2·g-1,孔径为4.1nm);
(4)将步骤(3)中得到的空心二氧化锰纳米颗粒的水溶液(2mg/mL)按照体积比1:2与聚烯丙基胺水溶液(PAH,MW:15000,5mg/mL)混合搅拌2小时后室温离心(11000转,10分钟)水洗三遍,得到PAH修饰的空心二氧化锰纳米颗粒;
(5)将步骤(4)中得到的PAH修饰的空心二氧化锰纳米颗粒的水溶液(2mg/mL)按照体积比1:2与聚丙烯酸(PAA,5mg/mL)混合搅拌2小时后室温离心(11000转,10分钟)水洗三遍,得到PAA修饰的空心二氧化锰纳米颗粒;
(6)将步骤(5)中得到的PAA修饰的空心二氧化锰纳米颗粒的水溶液按照质量比1:5与mPEG-5K-NH2混合搅拌12小时,室温离心(11000转,10分钟)水洗三遍获得PEG修饰的空心二氧化锰纳米颗粒(粒径在146nm左右);
(7)将步骤(6)中获得的PEG修饰的空心二氧化锰纳米颗粒的水溶液(1mg/mL)与不同投入量的IPI549混合搅拌12小时,室温离心(11000转,10分钟)水洗三遍获得锰基放疗增敏剂IPI549@HMP(粒径在162nm左右);其中,1:1载药率为32.2±2.9%,1:3载药率为53.9±5.2%,1:5载药率为69.8±4.7%)。
上述制得的的锰基放疗增敏剂的透射电镜(TEM)图及元素映射图如图2所示,透射电镜图像清晰显示HMP的球形形态和中空结构,相应的mapping证实了Mn、O元素的存在。
上述制得的的锰基放疗增敏剂的与氮气吸附与脱附的等温线图如图3的a部分所示,孔径分布如图3的b部分所示,本实施例采用氮气脱吸附方法测量了空心二氧化锰纳米颗粒的孔隙率,利用Brunauer-Emmett-Teller模型计算出空心二氧化锰的比表面积为349.5m2 ˙g-1,平均孔径为4.1nm,这表明具有丰富介孔的二氧化锰空心结构有望成为高效载药的理想纳米载体。
上述制得的的锰基放疗增敏剂的的粒径分布如图4的a部分所示,不同PH值下的分散性能如图4的b部分所示,通过动态光散射技术测定,HMP和IPI549@HMP纳米颗粒的平均粒径分别为146nm和162nm,并且IPI549@HMP纳米颗粒在不同的生理条件下也显示出良好的稳定性,随着时间的推移,也没有明显的聚集。
上述制得的的锰基放疗增敏剂中免疫调节药物IPI549的负载曲线如图5的a部分所示,药物释放曲线如图5的b部分所示,通过调整IPI549和HMP的初始投药量比,发现IPI549的负载是随着IPI549的投入量增多而增多,在5:1质量比的投药量时可以达到最佳载药率为69.8%;同时由于HMP的微环境响应性降解,IPI549@HMP的累计释放动力学在酸性溶液中,尤其是含有过氧化氢的酸性溶液中,药物释放的速度更快、更多。
实施例2–锰基放疗增敏剂的效果验证
本实施例通过体内体外实验进行实施例1中制得的锰基放疗增敏剂IPI549@HMP的效果性能进行验证,具体包括:
一、锰基放疗增敏剂的体内外的磁共振效果
采用配有小鼠线圈的3.0T磁共振临床扫描仪(United Imaging,uMR770)监测IPI549@HMP的T1成像性能。仪器的参数设置为:Freq.FOV=8.0mm2,phase FOV:1.00mm2,Slice thickness:2.0mm,Slices:8,spacing=0.3mm,TR:500ms,TE:21ms。用MR系统(uExeed,R002)测量不同IPI549@HMP浓度(0.64,0.32,0.16,0.08,0.04mM)分别在PBS(pH=7.4),PBS(pH=6.5)以及PBS(pH=6.5)包含H2O2(100μM)下的T1-mapping和T1W1图像。对于肿瘤成像,在静脉注射IPI549@HMP(二氧化锰=7.5mg·kg-1,IPI549=1.5mg·kg-1)前后分别进行T1-MRI扫描。
上述实验的体外磁共振效果如图6的a和b部分所示,体内磁共振效果如图6的c部分所示。体内磁共振效果结果表明,将IPI549@HMP静脉注射到CT26荷瘤小鼠中,进一步证明了HMnO2纳米壳在体内肿瘤特异性磁共振成像的性能,肿瘤区域的T1-磁共振信号明显增强;这一现象证实了IPI549@HMP由于其典型的增强通透性和保留效应(EPR),可以有效地积累到肿瘤区域,使其特别适合于肿瘤特异性磁共振成像。体外磁共振效果表明,Mn2+可以通过促进质子转移和延长纵向弛豫来增强T1加权磁共振成像;进一步研究IPI549@HMP纳米颗粒在不同PH值、含有或不含有过氧化氢溶液中的潜在T1磁共振成像能力,结果表明在PH为6.5的T1磁共振成像图中可以看到IPI549@HMP纳米颗粒呈现浓度依赖性的亮化效应,而在中性溶液(PH7.4)中的信号则似乎要弱的多。
二、锰基放疗增敏剂在体内外的乏氧缓解效果
采用便携式溶解氧分析仪(LeiciJPB-607A)定量检测IPI549@HMP纳米颗粒的体外产氧能力。为了评估体内的产氧能力,将IPI549@HMP纳米颗粒(二氧化锰=7.5mg·kg-1,IPI549=1.5mg·kg-1)通过尾静脉注射到荷瘤小鼠体内。在注射后24小时收集肿瘤组织,用HIF-1α抗体染色。每组随机抽取5个切片视野,用ImageJ软件计算阳性缺氧面积。
上述实验结果图7的a、b和c部分所示。已经证明,实体肿瘤的缺氧肿瘤微环境是氧依赖性放疗疗效有限的原因。基于MONx的纳米系统已经被很好地证明可以在肿瘤微环境中触发内源性过氧化氢的分解以生成O2,这可以在原位提高氧水平,对肿瘤特异性治疗非常有益。本实验研究表明,在没有添加IPI549@HMP纳米颗粒的过氧化氢溶液中,几乎没有观察到氧气泡的产生,而在过氧化氢存在的溶液中,可以观察到O2气泡的快速生成;同时,图7的a部分表明,通过氧探针动态监测的溶解氧变化进一步证实了这一结果,O2的产生高度依赖于Mn离子浓度;同时,对分离的小鼠肿瘤组织进行HIF-1α免疫组化染色,定量分析表明注射IPI549@HMP后显著缓解了肿瘤部位的乏氧程度。
三、锰基放疗增敏剂在体外的放疗增敏效果
将CT26细胞预先接种于6孔板中过夜使其贴壁,随后,加入空白/HMP/IPI549@HMP纳米颗粒(100ppm)共孵育8小时。然后在225KV和8mA下进行X射线照射,共计6Gy。将处理过的细胞用4%甲醛固定和1%BSA阻断后,细胞与γ-H2AX抗体(CST,货号No.80312S)孵育4℃过夜,然后再与二抗孵育1小时。最后在共聚焦显微镜成像前10分钟用DAPI对细胞核进行染色。
本实验首先通过使用特异性标记物-γ-H2AX染色来评估放疗诱导的DNA双链断裂(DDSB),以确认HMP对增强放疗反应的影响。DDSB被公认为由电离辐射引起的最致命的损伤类型,是放疗疗效的主要指标。上述实验结果如图8的a部分所示,研究表明,当与HMP或IPI549@HMP孵育和X射线照射时,在CT26细胞的细胞核中观察到显著的γ-H2AX荧光,表明联合细胞的DNA损伤增加。
为了进一步评估HMP的放疗增效效果,将CT26细胞与HMP或IPI549@HMP孵育12小时,然后暴露于X射线辐射(6Gy,225KV,8mA)。24小时后,用标准CalceinAM/PI染色检测细胞毒性,其实验结果如图8的b部分所示,研究表明,当CT26细胞与HMP或IPI549@HMP共孵育时,放疗的细胞杀伤作用最强,这可能是由于空心氧化锰介导肿瘤细胞内的过氧化氢分解导致了额外的氧气供应。
四、锰基放疗增敏剂的体内外安全性评价效果
本实验在体外水平上研究了HMP作为一种多功能纳米载体的生物安全性,IPI549@HMP的细胞毒性用标准的CCK-8试剂进行评估。
细胞增殖毒性试验按照CCK-8试剂盒(Donjindo Chemical Technology Co.,Ltd)说明书进行。将CT26或B16F10细胞预接种于96孔板中过夜,然后加入不同浓度的IPI549@HMP溶液,并与细胞共孵育24h。随后,每孔中加入CCK-8试剂,37℃孵育1h。在吸光度(OD)为450nm时,用酶标仪(ThermoMultiskanFC)测定细胞活力。
采用溶血实验,检验IPI549@HMP的生物相容性。首先,将从健康的BALB/c小鼠中提取的新鲜红细胞用PBS制备成悬浮液。然后,将稀释后的红细胞悬液与不同浓度(6.25、13.5、25、50、100、200ppm)的IPI549@HMP溶液混合,37℃孵育2h。测定上清液在570nm处的吸光度,并根据公式计算溶血率:溶血(%)=(OD样本-OD阴性)/(OD阳性-OD阴性)×100%。值得注意的是,PBS为阴性对照,去离子水为阳性对照。
图9的a部分的研究结果表明,在浓度为100ppm以内,IPI549@HMP对CT26和B16F10细胞的毒性可以忽略不计。同时,图9的b部分的研究结果表明,即使在HMP浓度高达200ppm的情况下,血细胞溶血率仍低于5%。
此外,本实验同时在体内研究了HMP的生物安全性,将IPI549@HMP纳米颗粒(二氧化锰=7.5mg·kg-1,IPI549=1.5mg·kg-1)通过尾静脉注射到健康Balb/c小鼠体内,随后隔天记录一次小鼠体重,共持续20天。在观察的终点期处死小鼠,采集小鼠器官(心、肝、脾、肺、肾)和血液样本进行评估,包括H&E染色和各种血液指数进行监测。
图9的c部分的研究结果表明,通过对体重、血液指标和主要器官(即心、肺、脾、肾、肝)的组织学切片的综合检测,证实了IPI549@HMP的体内高组织相容性,上述结果证明了HMP基纳米颗粒毒性较低,可以安全地用于静脉给药。
五、锰基放疗增敏剂在术后肿瘤的治疗效果
基于体外的优良效果,推测IPI549@HMP可增强RT治疗,以防止手术切除后的结肠癌复发。本实验采用如下的具体步骤:Balb/c小鼠接种Luc+CT26细胞后第10天,手术部分切除可见肿瘤,建立术后肿瘤模型。每次治疗前将残留肿瘤BALB/c小鼠随机分为5组(n=6),包括1组对照(PBS)、2组IPI549@HMP、3组RT、4组HMP+RT、5组IPI549@HMP+RT(二氧化锰=7.5mg·kg-1和IPI549=1.5mg·kg-1)。术后第1、3组按组分别静脉注射PBS/HMP/IPI549@HMP,注射后8h连续2次3Gy的x射线照射,共2个周期,具体如图10的a部分所示。通过捕获肿瘤细胞的生物发光信号来监测肿瘤的生长情况,其结果如图10的b部分所示。此外,体重的变化也被仔细记录下来,直到第一只老鼠死亡,其小鼠的存活情况如图10的c部分所示。
如生物发光实验的结果所示(图10的b部分),游离IPI549@HMP(第2组)对肿瘤生长没有明显的抑制作用;对于X射线照射小鼠的肿瘤(第3组,单独RT),肿瘤的生长部分延迟;HMP联合X射线治疗具有较强的肿瘤生长抑制作用(第4组),可能是由于TME能有效缓解HMP内的缺氧;值得注意的是,在IPI549@HMP与X射线照射的联合治疗组(第5组)中,残余肿瘤的生长速度最慢,由图10的c部分所示,IPI549@HMP与X射线照射的联合治疗组(第5组)所有小鼠均存活60天,50%的小鼠完全缓解,即其术后治疗肿瘤的效果最佳。
六、锰基放疗增敏剂联合免疫检查阻断点疗法对小鼠远隔肿瘤的治疗效果
目前,术后由癌症引起的死亡大多是由肿瘤转移引起的,如果发生,常规治疗很难有效治疗。因此,人们普遍认为,理想的术后治疗不仅应局部抑制残留肿瘤,还应识别、抑制和切除转移瘤。近年来见证了癌症免疫治疗的广泛发展,其中包括PD-1/PD-L1检查点封锁方法在肿瘤治疗方面展现了较高的临床反应。免疫检查点阻断与包括放疗在内的其他治疗方案的结合已成为了研究热点。然而,与未治疗的对照组相比,术后肿瘤组织中与临床免疫检查点阻滞耐药性高度相关的基因表达量更高,这表明手术触发的复杂炎症反应促进了对免疫检查点阻滞治疗的耐药性的产生。
本实验采用锰基放疗增敏剂IPI549@HMP联合免疫检查阻断点(PD-L1阻断)疗法进行小鼠远隔肿瘤的治疗,以验证IPI@549增强的放疗是否能进一步提高对PD-L1阻断的敏感性,其具体的实验步骤如下:采用了双侧CT26荷瘤小鼠模型;将原发肿瘤接种后5天后在对侧接种第二个肿瘤定为模拟的远处肿瘤,随后将右侧原发肿瘤部分切除,每次静脉注射后IPI549@HMP(剂量分别为二氧化锰=7.5mg·kg-1和IPI549=1.5mg·kg-1)后的8h和24h分别行X射线照射2次,第11、13天静脉注射采用PD-L1阻断治疗,抗PD-L1注射剂(剂量=3.75mg·kg-1),具体操作如图11的a部分所示。此外,体重的变化也被仔细记录下来,直到第一只老鼠死亡,其小鼠的存活情况如图11的c部分所示。
治疗结果如图11的b部分所示,单独的抗PD-L1对原发肿瘤和远处肿瘤的抑制确实没有明显的作用;虽然IPI549@HMP增强的RT单独可在一定程度上抑制原发肿瘤的生长,但其未能对模拟的远处肿瘤产生影响;值得注意的是,IPI549@HMP增强的RT联合PD-L1阻断不仅根除了原发性残留的CT26肿瘤,而且还显著抑制了远处肿瘤的生长,且如图11的c部分所示,该组所有的老鼠均存活超过50天。
七、锰基放疗增敏剂联合免疫检查阻断点疗法对小鼠再接种肿瘤治疗效果
免疫记忆反应作为适应性免疫的标志,可以为生物体提供长期的保护,以抵御第二次病原体的攻击,对肿瘤的预防至关重要。因此,有必要评估IPI549@HMP增强的RT+PD-L1阻断所产生的免疫记忆效应。
在本实验中,在IPI549@HMP增强的RT+PD-L1阻断治疗后46天接种继发性CT26肿瘤,并在年龄和性别匹配的naive小鼠接种相同数量的癌细胞作为对照,通过捕获肿瘤细胞的生物发光信号来监测肿瘤的生长情况,并记录小鼠的体重变化,直到第一只老鼠死亡,具体操作如图12的a部分所示。
与预期一致,如图12的b部分所示,所有的小鼠接种CT26细胞后,均观察到快速的肿瘤进展;而与之形成鲜明对比的是,采用RT+PD-L1阻断治疗治愈肿瘤再接种肿瘤细胞时没有肿瘤生长,且如图12的c部分所示,RT+PD-L1阻断治疗组所有的老鼠均存活超过100天,这展现出杰出的免疫记忆效应。
八、锰基放疗增敏剂的体内分布和药代动力学
本实验采用下述方法:对CT26荷瘤小鼠(n=3)进行静脉注射。当肿瘤体积达到200mm3左右时,注射ICG标记的IPI549@HMP。使用IVIS光谱成像系统(VISQUE InvivoSmart-LF)在预先设定的时间点拍摄荧光图像。利用活体图像软件对荧光强度(averageradiance,photos s-1cm-2sr-1)进行定量分析。在评估药代动力学时,在静脉注射IPI549@HMP纳米颗粒后的不同时间间隔(0.08、0.16、0、5、1、2、4、8、12、24h)从小鼠尾静脉中采集15μL血样。根据ICP-MS测定的血液样本中的Mn的含量,计算IPI549@HMP的半衰期。
上述实验结果通过使用IVIS光谱成像系统跟踪ICG标记的IPI549@HMP的荧光,评估IPI549@HMP在CT26荷瘤小鼠中的分布和肿瘤积累。如图图13的a和b部分所示,肿瘤区域的ICG荧光强度随时间增加,并在注射后8h达到峰值水平。根据主要器官分离得到的半定量生物分布表明,IPI549@HMP的肿瘤摄取和保留率较高(图13的c和d部分)。值得注意的是,肝脏和肾脏中出现了明显的荧光,正如预期的那样,说明纳米材料开始降解并被排出。同时,计算出血液循环的半衰期为0.97h(e),这表明IPI549@HMP很容易从中央腔室中清除,如肾脏和肝脏。
九、将锰基放疗增敏剂应用于消融后残癌复发模型
本实验采用下述方法:为建立微波消融肿瘤模型,给小鼠皮下种植CT26肿瘤后第十天,采用经皮插入0.5cm针尖的冷却尖电极进行微波消融。在5W下消融持续1~2min,导致肿瘤组织部分坏死。随后将小鼠随机分为两组,包括对照组和IPI549@HMP+RT组。进行放疗前8小时,不同组小鼠经尾静脉注射PBS/IPI549@HMP(二氧化锰=7.5mg·kg-1,IPI549=1.5mg·kg-1)。X射线辐照参数设置为225KV和8mA,并在注射后8h连续两次3Gy照射,连续两个周期。
在上述实验中,考虑到在肿瘤消融治疗后,如微波消融、射频消融和高强度聚焦超声(HIFU)术后也可能发生复杂的炎症反应。本实验构建了一个微波消融后肿瘤模型来阐明IPI549@HMP联合放射免疫治疗策略是否适用于接受消融治疗的实体肿瘤。微波消融于肿瘤接种后第10天进行,造成肿瘤部分坏死。然后将残留肿瘤随机分为对照组PBS组和IPI549@HMP+RT组(二氧化锰=7.5mg·kg-1,IPI549=1.5mg·kg-1),治疗方法与之前相同,具体如图14的a部分所示。对照组所有小鼠均在30天内死亡,而IPI549@HMP+RT组小鼠存活40天以上,甚至50%(3/6)的小鼠无肿瘤(如图14的b-f部分所示)。值得注意的是,在治疗期间,小鼠的体重变化没有显著差异(如图14的g部分所示),这进一步证明了IPI549@HMP联合放射免疫治疗策略的有效性和安全性,以及它对消融后肿瘤的适用性。
实施例3–锰基放疗增敏剂应用于动物的放射免疫治疗方法
本发明实施例1中步骤(7)中制得的锰基放疗增敏剂IPI549@HMP可用于放射免疫治疗,其具体实施步骤为:Balb/c小鼠经尾静脉注射IPI549@HMP(MnO2=7.5mg·kg-1,IPI549=1.5mg·kg-1);注射药物后在8小时,24小时分别接受两次剂量为3Gy的放射治疗。
上述放射免疫治疗的具体方法为:采用1%的戊巴比妥钠按照15g/100μL的量麻醉小鼠;随后将麻醉后的小鼠单独置于独立的铅盒中,使肿瘤区域暴露而身体的其他部分被完全屏蔽;采用X射线辐照仪(RadSourceRS2000)进行X射线照射,参数设置为225KV和8mA;整个治疗周期内IPI549@HMP共注射2次,分别在第一天和第三天,放射治疗总共进行4次分别在第一次和第二次注药后的8小时和第24小时。
上述锰基放疗增敏剂IPI549@HMP的X射线照射放疗还可联合PD-L1阻断,其在接受IPI549@HMP的同时静脉注射采用PD-L1阻断治疗,抗PD-L1注射剂(剂量=3.75mg·kg-1)。
对比例1–不同空心二氧化锰纳米复合材料的性能能力对比
采用专利CN201710618202实施例1制备的空心二氧化锰纳米复合材料(对比组),其在pH值为7.4的溶液中,Ce6和DOX这两种药物的释放都十分的缓慢,分别为23%和21%。然而在弱酸性的pH值下(pH值为6.5和pH值为5.5),Ce6(49%和73%)和DOX(62%和95%)都会快速的释放,这主要是由于H-MnO2-PEG纳米颗粒在酸性条件下会分解成Mn2+离子,因此可以促进药物的释放。而本发明实施例1制备的锰基放疗增敏剂IPI549@HMP,其在pH值为7.4的溶液中释放率约为21.8±3.1%,在pH值为6.5的溶液中释放率约为40.5±3.5%,在pH值为6.5包含H2O2(100μM)溶液中释放率约为73.4±3.5%,达到了响应性释放和缓释的效果。
另外,对比组并没有详述所制备的氧化锰纳米粒子在体外的磁共振成像效能,本发明实施例1所制备的氧化锰纳米粒子在体外具有较强的磁共振T1弛豫能力,在pH值为7.4的溶液中弛豫率约为0.44mM-1s-1,在pH值为6.5的溶液中弛豫率约为1.35mM-1s-1,在pH值为6.5包含H2O2(100μM)溶液中弛豫率约为7.10mM-1s-1,达到了增强的T1磁共振成像效果。
综上,与对比组相比,本发明实施例1所制备的氧化锰纳米粒子的产氧能力提升,在Mn离子浓度约为40ppm时,5分钟内的产氧速率能达到8mgL-1,其对小鼠肿瘤内的缺氧阳性面积降低了一半以上。
对比例2–不同类型的MnO2-IPI549纳米材料的效果对比
将现有技术中的BSA-MnO2-IPI549(采用牛血清白蛋白用作药物载体以封装MnO2纳米颗粒和IPI549)、本发明实施例1步骤(7)制得的锰基放疗增敏剂IPI549@HMP对术后肿瘤的治疗效果进行对比,其对比结果如下:
BSA-MnO2-IPI549的治疗方式为:每隔两天注射一次,持续18天,在观察期30天内显著抑制了肿瘤的生长,而本发明实施例1制备的IPI549@HMP的治疗方式为:只需注射药物两次,实施放疗4次,就可以达到50%的小鼠治愈率,实现增强的效果。当进一步联合免疫检查点阻断治疗时,该方案不仅可以抑制原发肿瘤生长,还可以显著抑制远隔肿瘤的生长,并诱发强大的免疫记忆效应以抵御肿瘤再接种。同时,经研究表明,本发明实施例1制备的IPI549@HMP还适用于消融后肿瘤复发模型的治疗,能够实现50%的治愈率。
由上对比可知,本发明实施例1制得的IPI549@HMP相对于现有技术具有增强的抑制肿瘤效果,且具有更高的治愈率。
以上所述仅为本发明较佳的实施例,并非因此限制本发明的实施方式及保护范围,对于本领域技术人员而言,应当能够意识到凡运用本发明说明书及图示内容所作出的等同替换和显而易见的变化所得到的方案,均应当包含在本发明的保护范围。
Claims (4)
1.一种锰基放疗增敏剂在制备肿瘤治疗剂中的应用,其特征在于,所述肿瘤治疗剂为术后肿瘤治疗剂和/或消融后残癌复发治疗剂;以及,所述锰基放疗增敏剂为负载小分子PI3Kγ激酶抑制剂的聚乙二醇化空心氧化锰纳米颗粒材料,所述小分子PI3Kγ激酶抑制剂为IPI549;
其中,所述聚乙二醇化空心氧化锰纳米颗粒的水溶液与小分子PI3Kγ激酶抑制剂的质量比为1:0.5~7,其载药率为20~80%,所述聚乙二醇化空心氧化锰纳米颗粒的平均粒径为146 nm,平均孔径为4.1 nm,载药后获得的所述锰基放疗增敏剂的粒径为156~170 nm;
以及,所述肿瘤治疗剂为锰基放疗增敏剂与X射线照射的联合治疗,所述肿瘤治疗剂为抑制局部残留肿瘤、抑制消融后肿瘤复发的治疗剂;或,所述肿瘤治疗剂为锰基放疗增敏剂的放疗联合PD-L1阻断,所述肿瘤治疗剂为全身性的调节术后免疫抑制微环境、抑制局部残留肿瘤和远处转移肿瘤、抑制消融后肿瘤复发、预防肿瘤再接种的治疗剂。
2.根据权利要求1所述的应用,其特征在于,所述锰基放疗增敏剂的制备方法包括:
步骤1)制备聚乙二醇化空心氧化锰纳米颗粒;
步骤2)将预定浓度的所述聚乙二醇化空心氧化锰纳米颗粒的水溶液与预定投入量的小分子PI3Kγ激酶抑制剂混合,以物理吸附形式组装形成所述锰基放疗增敏剂。
3.根据权利要求2所述的应用,其特征在于,所述步骤1)中,所述聚乙二醇化空心氧化锰纳米颗粒的制备步骤包括:
步骤1A)将乙醇、水、氨水按照预定的体积比混合搅拌,缓慢滴加正硅酸乙酯反应,随后室温离心,分别采用乙醇、水洗涤,得到实心二氧化硅纳米颗粒;
步骤1B)将步骤1A)中得到的所述实心二氧化硅纳米颗粒分散在水中,在超声条件下采用注射泵按预定速率逐滴加入高锰酸钾水溶液进行还原反应得到实心二氧化硅包覆二氧化锰纳米颗粒,将得到的所述实心二氧化硅包覆二氧化锰纳米颗粒溶解在Na2CO3水溶液中进行蚀刻反应得到空心二氧化锰纳米颗粒;
步骤1C)将步骤1B)中得到的所述空心二氧化锰纳米颗粒的水溶液按照预定比例依次与聚烯丙基胺水溶液、聚丙烯酸、氨基化的聚乙二醇混合反应得到所述聚乙二醇化空心氧化锰纳米颗粒。
4. 根据权利要求3所述的应用,其特征在于,所述步骤1A)中,乙醇、水、氨水的体积比为26~30:3~5:1,反应时间为1~2 h,反应温度为38~42 ℃;和/或,
所述步骤1B)中,注射泵的注射速率为55~65 mL/h,蚀刻反应的条件为:50~60 ℃下反应8~12 h,其得到的所述空心二氧化锰纳米颗粒的粒径为135~145 nm,比表面积为340~360m2 ˙g-1;和/或,
所述步骤1C)中,所述空心二氧化锰纳米颗粒与聚烯丙基胺水溶液、聚丙烯酸、氨基化的聚乙二醇质量比为1:4~6:4~6:4~6;和/或,
在上述步骤中,除了蚀刻反应,各步骤中反应结束均进行离心操作步骤,操作条件为:室温,10000~12000转离心8~12分钟。
