发明背景
大麻素是在结构上或药理学上与大麻属植物(cannabis plant)的成分或者与大麻素受体CB1或CB2的内源性激动剂(内源性大麻素(endocannabinoid))相关的天然化合物和合成化合物。在自然界中产生这些化合物的唯一方式是通过大麻属植物。大麻属(Cannabis)是大麻科(Cannabaceae)中的开花植物的属,其包括物种大麻(Cannabissativa)、印度大麻(Cannabis indica)和莠草大麻(Cannabis ruderalis)(有时被认为是大麻的一部分)。

大麻二酚(CBD)是大麻属物种诸如工业大麻植物(hemp plant)(大麻)的主要大麻素成分。与其他大麻素诸如THC不同,大麻二酚不结合CB1或CB2,或者其与受体的结合在诱发药理学作用方面是可忽略的。因此,大麻二酚不引起由CB1受体或CB2受体介导的中枢神经系统作用或外周神经系统作用。CBD具有小的精神药物(拟大麻(cannabimimetic))活性或不具有该活性,并且其分子结构和性质与其他大麻素的分子结构和性质显著不同。

实施例1:α6-羟基大麻二酚(6-OHCBD)的合成产生方法
化合物6-OH CBD是大麻二酚的已知的代谢物。
下文描述的合成途径详细说明了可以被使用以便产生大麻素α6-OH CBD的方法。
在方案中,R=C5H11
向在无水吡啶(20mL)中的大麻二酚(5.00g,15.8mmol)中加入乙酸酐(5.13g,4.75mL,50.2mmol),并且将溶液搅拌持续4h。加入二氯甲烷(300mL),并且将溶液用水(200mL)、1M盐酸(200mL)、饱和碳酸氢钠水溶液(200mL)洗涤,干燥(MgSO4)并且浓缩,以给出作为草黄色油的大麻二酚二乙酸酯(5.72g,定量),其在不经进一步纯化的情况下使用。

实施例2:使用小鼠的超大电休克癫痫发作(SUPRAMAXIMAL ELECTROSHOCKSEIZURE)(MES)测试来评价6-羟基大麻二酚(6-OH CBD)的抗痉挛活性
6-OH CBD的效力在癫痫发作的小鼠模型中进行测试,即超大电休克癫痫发作(MES)测试。
超大电休克癫痫发作(MES)测试在临床前被广泛地用于评价分子和标准抗癫痫药物的抗痉挛性质(Loscher等人,1991)。
MES测试是非常严格的模型,在该模型中,小鼠接受预先确定的足够强度的高水平电刺激,以在100%的对照动物中可靠地产生强直性后肢伸肌癫痫发作。因此,MES测试是对抗痉挛活性的严格评价(Swinyard,1985)。
方法
使首次用于实验的小鼠适应它们的居住笼(home cage)中的操作室,其中食物和水可自由摄取。
动物根据治疗组被腹膜内(i.p.)给药。
媒介物(10ml/kg,腹膜内,60min预处理时间)是1∶1∶18的媒介物5%乙醇、5%kolliphor EL、90%盐水。
根据实施例1中描述的方法制备测试化合物α6-OH CBD。
测试化合物6-OH CBD以60min预处理时间10ml/kg腹膜内给予的3mg/kg、30mg/kg、100mg/kg和200mg/kg的剂量来施用。
此外,在120分钟预处理时间以10ml/kg腹膜内给予100mg/kg剂量的CBD,以评估药物在较长时间过程内的作用。
阳性对照丙戊酸盐以250mg/kg(10ml/kg,腹膜内,30min预处理时间)使用。
在预先确定的高水平的(30mA:50Hz)、角膜递送的、足以在100%的对照动物中可靠地产生强直性后肢癫痫发作的强度的电休克(0.2秒持续时间)之后,单独地评估小鼠的强直性后肢伸肌癫痫发作的产生。

结果

在阳性对照丙戊酸盐(250mg/kg)治疗的组中,所述丙戊酸盐在测试之前30分钟腹膜内施用,所有动物都被评分为没有癫痫发作。与媒介物对照相比,该结果是统计学上显著的(p<0.001)。
在6-OH CBD治疗组中,所述6-OH CBD在测试之前120分钟腹膜内施用,3mg/kg剂量的6-OH CBD是无效的。然而,30mg/kg、100mg/kg和200mg/kg剂量的6-OH CBD使在该组内所有小鼠均能够承受癫痫发作,并且与媒介物相比产生统计学上显著的作用(p<0.001)。
此外,与媒介物对照相比,120分钟给予的6-OH CBD的剂量(100mg/kg)还产生了癫痫发作的统计学显著的减少。

实施例3:使用小鼠的最大电休克癫痫发作阈值(MEST)测试来评价6-羟基大麻二酚(6-OH CBD)的抗痉挛活性
6-OH CBD的效力在全身性癫痫发作的小鼠模型中进行测试,即最大电休克癫痫发作阈值(MEST)测试。
最大电休克癫痫发作阈值(MEST)测试在临床前被广泛地用于评价测试化合物的促痉挛或抗痉挛性质(Loscher等人,1991)。
在MEST测试中,药物改变诱发后肢强直性伸肌痉挛(hind limb tonic extensorconvulsion)所需的癫痫发作阈值电流的能力根据电击调整(shock titration)的“上下(up and down)”方法(Kimball等人,1957)来测量。癫痫发作阈值的增加指示抗痉挛作用。包括钠通道阻滞剂(例如,拉莫三嗪(lamotrigine))的抗癫痫药物在小鼠的该测试中全部呈现出抗痉挛性质,所述抗癫痫药物具有针对全身性强直-阵挛性癫痫发作的临床上被证明的效力。
相反,癫痫发作阈值的降低指示促痉挛作用,如使用已知的痉挛剂诸如印防己毒素(picrotoxin)所观察到的。
测试化合物改变诱发强直性后肢伸肌痉挛的存在所需的、以电流(mA)表示的刺激强度的能力在MEST中被评估。从在治疗组的50%的动物中产生强直性后肢伸展的电流中观察到存在(+)或不存在(0)强直性后肢伸肌痉挛的结果(CC50)确定了治疗组的癫痫发作阈值,并且然后将该作用与媒介物对照组的CC50进行比较。
方法
研究细节:
使首次用于实验的小鼠适应它们的居住笼中的操作室持续多达7天,其中食物和水可自由摄取。
在研究开始时对所有动物称重,并且基于各组间体重的平均分布随机分配到治疗组。采用媒介物、3mg/kg、10mg/kg或30mg/kg的测试化合物或2.5mg/kg的地西泮,经由腹膜内(i.p.)注射,以10mL/kg对所有动物进行给药。
对于媒介物在给药后30min,对于3mg/kg、10mg/kg和30mg/kg的6-OH CBD分别在给药后30min、15min和60min,以及对于地西泮在给药后30min,单独地评估动物从单次电休克中的强直性后肢伸肌痉挛的产生。
治疗组中的第一只动物以预期或估计的CC50电流被给予电击。对于随后的动物,根据来自前一只动物的痉挛结果来降低或升高电流。
将从每个治疗组产生的数据用于计算治疗组的CC50±SEM值。
测试化合物:
媒介物(5%乙醇,5%solutol,90%盐水)如下制备:在36mL的盐水中,将2mL的乙醇、2mL的solutol升温至60℃(1:1:18)。
阳性对照:地西泮以2.5mg/kg使用。
根据实施例1中描述的方法来制备测试化合物α6-OH CBD。6-OH CBD在1:1:18的乙醇:solutol:0.9%盐水制剂中以3mg/kg、10mg/kg和30mg/kg(i.p.)施用。
样品收集:
根据1986年动物(科学程序)法案的附表1的动物人道处死,每只动物在产生痉挛之后立即通过以下被人道地处死:通过撞击颅骨而破坏脑,然后确认由于斩首的循环的永久停止。斩首之后进行末梢血(terminal blood)和脑的收集。
将血液收集在锂-肝素管中并且在4℃以1500×g离心持续10分钟。将所得到的血浆取出(>100μL)并且分成2个等分试样的0.5mL Eppendorf管,Eppendorf管包含100μL的抗坏血酸(100mg/mL)用于稳定化。将脑取出,在盐水中洗涤并且分成两半。将每一半放入单独的2mL螺旋盖冷冻管中,称重并且在干冰(cardice)上冷冻。
统计学分析
每个治疗组的数据被记录为在所使用的每个电流水平时的+和0的数,并且然后该信息被用于计算CC50值(50%的动物表现出癫痫发作行为所需的电流)±标准误差。
6-OH CBD的作用还被计算为CC50与媒介物对照组的百分比变化。
根据Litchfield和Wilcoxon(1949)来评估药物治疗的动物和对照之间的显著差异。
结果
下文的表2展示了在本实验中产生的数据,并且图1图示了这些结果。
在媒介物组中,CC50值计算为24.0mA。
在阳性对照地西泮(2.5mg/kg)治疗的组中,所述地西泮在测试之前30分钟腹膜内施用,CC50值为48.8mA。与媒介物对照相比,该结果是统计学上显著的(p<0.001)。
在6-OH CBD治疗组中,所述6-OH CBD在测试之前30min、15min和60分钟腹膜内施用,3mg/kg、10mg/kg和30mg/kg剂量的6-OH CBD与处于化合物所有三种剂量的媒介物相比产生了统计学上显著的CC50值。
这样的数据指示,该化合物将具有治疗益处。

6-OH CBD在MEST中产生了剂量相关的增加,这提供了证据,即该化合物表现出抗痉挛性质。当与媒介物相比时,在3mg/kg、10mg/kg和30mg/kg观察到显著的作用。

实施例4:6-羟基大麻二酚(6-OHCBD)的药代动力学
本研究的目的是在将6-OH CBD单次静脉内或腹膜内施用于小鼠之后确定6-OHCBD的脑和血浆药代动力学参数;以及确定6-OH CBD的脑:血浆浓度比率。
方法
99只雄性小鼠各自接受6-OH CBD的单次静脉内或腹膜内给药,如下文表3中详述的。
表3:给药的细节
对于每种制剂,6-OH CBD以所需浓度在乙醇:Kolliphor EL(Cremophor EL):0.9%(w/v)盐水(1:1:18,v/v/v)中配制。
每只动物接受以2mL/kg的标称剂量体积的、经由尾静脉的单次静脉内施用,或者接受以5mL/kg的标称剂量体积的单次腹膜内施用。
在给药之后,经由心脏穿刺从每只动物收集末梢血样品,并且在以下时间点中的每一个切除脑:
·静脉内组:7分钟、15分钟和30分钟,1小时、2小时、4小时、8小时、12小时和24小时;
·腹膜内组:15分钟和30分钟,1小时、2小时、4小时、8小时、12小时和24小时。
将血液收集在包含锂肝素抗凝血剂的管内,并且离心以制备血浆。脑样品在干冰上被冷冻,然后称重并且在生物分析之前深冻(deep frozen)储存。剩下的尸体被丢弃。将血浆用等体积的抗坏血酸溶液来稳定,然后在生物分析之前深冻储存。
使用有资格的LC-MS/MS方法对稳定的血浆样品进行分析,以确定6-OH CBD的浓度。对这些分析的结果进行评价,以确定非房室药代动力学参数(non-compartmentalpharmacokinetic parameter)。
结果
表4和表5展示了在本研究中获得的结果,并且图2图示了结果。
确定了以下参数:
C最大 观察到的最大浓度。
T最大 观察到最大浓度的时间。
AUC0-t 从0小时至最后可定量浓度的浓度-时间曲线下的面积,通过线性梯形规则(linear trapezoidal rule)来估计。
AUC0-24 从0小时至24小时的浓度-时间曲线下的面积,使用线性梯形规则来估计。
t1/2 消除半衰期,被确定为ln(2)/λz。
Cl 清除率(Clearance),以剂量/AUC0-无限来计算(仅对于静脉内给药)。
Vss 分布的体积,基于末端消除阶段(terminal elimination phase),以Cl/λz来计算(仅对于静脉内给药)。
F绝对 腹膜内给药的绝对生物利用度。

如从表4可以看出的,在以1mg/kg的剂量水平静脉内(IV)施用6-OH CBD之后,T最大在给药后0.117小时(7分钟)被观察到。在达到C最大之后,6-OH CBD浓度下降,其中半衰期(t1/2)为0.451小时。总血浆清除率为48.7mL/min/kg,并且分布的体积为1.90L/kg。
在以3mg/kg、10mg/kg或30mg/kg腹膜内(IP)施用之后,6-OH CBD被吸收,其中在每个剂量水平的T最大值为0.250小时(15分钟)。在达到C最大之后,血浆浓度下降,其中半衰期(t1/2)值在0.993小时和1.08小时之间的范围内,并且在评估的每种剂量水平上相似。
在腹膜内施用之后,在血浆中的6-OH CBD暴露(如通过C最大和AUC0-24值评估的)随着在3mg/kg和30mg/kg之间的剂量水平的每次增加而增加。观察到的暴露的增加大于剂量比例,其中对于剂量水平的10倍增加,C最大增加16.7倍并且AUC0-24增加33.2倍。
6-OH CBD在腹膜内施用之后的绝对生物利用度在3mg/kg、10mg/kg和30mg/kg分别为52.1%、75.8%和173%。
在全身施用之后,在脑中出现6-OH CBD,其中T最大值在静脉内给药之后为0.117小时,并且在从0.250小时至1小时的范围内。在达到C最大之后,6-OH CBD浓度下降,其中半衰期(t1/2)值在静脉内施用之后为0.312小时,并且在0.569小时和0.919小时之间的范围内,并且在腹膜内施用之后具有随剂量水平的增加而增加的趋势。
如从表5可以看出的,在腹膜内施用之后,在脑中的6-OH CBD暴露(如通过C最大和AUC0-24值评估的)随着剂量水平在3mg/kg和30mg/kg之间的每次增加而增加。观察到的暴露的增加大于剂量比例,其中对于剂量水平的10倍增加,C最大增加15.8倍并且AUC0-24增加32.6倍。
6-OH CBD的C最大和AUC0-24的脑:血浆比率对于C最大在从1.23至2.03的范围内并且对于AUC0-24在从1.41至2.01的范围内。在腹膜内施用之后,对于这两个参数的比率在所评估的每种剂量水平上都是相似的。然而,对于这两个参数的比率,在静脉内施用之后相比于腹膜内施用是较高的。
结论
在腹膜内施用之后,6-OH CBD被吸收,其中对于所有腹膜内给药的T最大值为15min;并且对于1mg/kg静脉内给药的T最大值为7min。
本研究的结果提供了化合物6-OH CBD的良好的药代动力学参数(生物利用度、清除率等)的证据。
