Description
Technical Field
The invention relates to the technical field of medical instruments, in particular to a tunnel flushing disinfection catheter for dialysis and a using method thereof.
Background
The global prevalence of Chronic Kidney Disease (CKD) is 11-13%, and end-stage renal disease (ESRD) is entered in most patients. Peritoneal dialysis is an important treatment means of ESRD, compared with hemodialysis, peritoneal dialysis has the advantages of small hemodynamic influence, home operation and the like, is low in medical cost and flexible in treatment time, reduces economic burden for the country and the society, is greatly supported by the country and the government, and is rapidly developed. With the increase of peritoneal dialysis patients and the improvement of the management level of each peritoneal dialysis center, the life span, dialysis age and life quality of the patients with the tubes are all improved. However, as the tube-carrying time is prolonged, in addition to acute complications such as peritonitis and the like, chronic complications such as malnutrition, peritoneal dialysis tunnel infection and the like are more and more generated, wherein the peritoneal dialysis tunnel infection is particularly common, severe cases can cause tunnel granulation, peritonitis and the like, and even the peritoneal dialysis tube needs to be pulled out. The treatment such as incision drainage, large risk of damaging the peritoneal dialysis catheter of silica gel, tunnel mouth dressing change and the like cannot solve the problems of limited tunnel cleaning and disinfection, incapability of reaching tunnel inflammation by antibacterial drugs and the like, and therefore effective treatment is lacked.
CN 214911360U discloses a flushing device for hemodialysis long-term catheter subcutaneous tunnel infection, which comprises a puncture component, an infusion component and a liquid discharge component, wherein the infusion component and the liquid discharge component are respectively communicated with the puncture component; the puncture assembly comprises a puncture needle, an extension hose slides and is nested in the puncture needle, and a plurality of liquid holes are arranged on the side wall of the extension hose in a penetrating mode. The working principle is as follows: firstly, the injector filled with the normal saline is communicated with the liquid inlet pipe through the installation joint, then the liquid valve is closed, and then the injector is used for inputting the normal saline into the subcutaneous tunnel for flushing through the liquid inlet pipe, the three-way joint, the catheter, the puncture needle and the extension hose. Although this method enables rinsing, it still has the following drawbacks: (1) the structure is complex, and the puncture needle needs to be sleeved outside the hose and is in sliding connection with the hose; and during the operation, need insert the pjncture needle in the subcutaneous tunnel, then promote the extension hose and slide in the pjncture needle and extend outside the pjncture needle, make the extension hose further extend to the subcutaneous tunnel inside, input sufficient normal saline in the subcutaneous tunnel through infusion assembly afterwards, normal saline just can flow out from a plurality of liquid hole of hose like this. Therefore, the puncture needle only plays a role of facilitating medical staff to insert the hose into a subcutaneous tunnel, and the normal saline cannot flow out of the puncture needle and the hose must be moved out, so that on one hand, the operation is complicated, on the other hand, the puncture needle only arranges a plurality of liquid holes on the side surface of the hose, cannot ensure that the flushing medicine probes into the deepest part of the tunnel and cannot ensure that the flushing with larger effective area can be obtained, and under the pushing speed action of the injector, the hose can easily generate larger pressure locally even if the liquid holes are arranged, so that the risk of flushing away of local inflammation parts or falling of bacteria is increased; (2) this prior art can only realize disinfecting and washing, can not realize the tunnel portal problem of changing dressings.
Disclosure of Invention
The invention aims to overcome the defects of the prior art and provide a tunnel flushing disinfection catheter for dialysis and a use method thereof, wherein the tunnel flushing disinfection catheter for dialysis has a simple structure, is simple and convenient to operate and has an obvious effect, can detect the depth of tunnel inflammation, can effectively reach the tunnel inflammation part by externally applied medicaments in two different modes of flushing and smearing, and effectively solves the problems that the tunnel is limited in cleaning and disinfection, and antibacterial medicaments cannot reach the tunnel inflammation part.
The technical scheme of the invention is as follows:
the invention relates to a tunnel washing and disinfecting catheter for dialysis, which is provided with an inclined port at one end inserted into a tunnel port to form a main hole at the tip end; a plurality of side holes are formed in the section of the pipe body of the catheter extending into the target position of the tunnel; the hardness of the end head is greater than that of the pipe body; the catheter is provided with a joint for connecting an aspirator.
Further, the inclination angle of the tip main hole is 30-45 degrees.
Furthermore, the outer surface of the side hole is covered with an elastic film, the elastic film is positioned right above the side hole, and the width of the elastic film is half of that of the side hole; the other position of the conduit is a hose. The elastic film covers the outer surface of the side hole, so that the elastic film has three benefits, one is that the elastic film cannot hurt the body when in use, the other is that the elastic film can divide the fluid flowing out from one hole into two parts and aims at different directions, so that the cleaning is more comprehensive, and the third is that the elastic film can adjust the speed of the fluid, but the fluid is more moderate when being ejected.
Further, the aperture of the side hole is 1-2 mm.
Furthermore, the back of the main hole of the tip end and the other surface of the catheter opposite to the side hole are provided with rough surfaces for attaching and smearing medicines.
Further, the position of the rough surface is arranged opposite to the peritoneal dialysis catheter stored in the tunnel.
The invention also discloses a tunnel washing and disinfecting catheter for dialysis, which comprises a first catheter and a second catheter, wherein the first catheter is provided with an inclined port at one end inserted into a tunnel port to form a main hole at the tip end; the tube body of the first catheter is provided with a plurality of side holes, and the inclined ports and the side holes are used for conveying the liquid medicine in the first catheter to a target position of the tunnel; the end head inserted into the tunnel opening and the tube body of the second catheter are provided with rough surfaces for attaching and coating medicines.
Furthermore, the second catheter is also provided with a main pointed end hole at the end inserted into the tunnel opening, and the main pointed end hole is arranged on the back of the rough surface.
Further, the first conduit and the second conduit are provided with joints for connecting an aspirator; or only the first conduit is provided with a fitting for connecting an aspirator.
The invention relates to a use method of a tunnel washing disinfection catheter for dialysis, which comprises the following steps:
when the tunnel needs to be washed, the end of the catheter with the tip main hole and the side hole is conveyed to the inflammation position of the tunnel through the tunnel opening, liquid medicine is filled into the aspirator and communicated with the catheter, the liquid medicine is pushed into the catheter through the aspirator, and the medicine enters the target position of the tunnel through the tip main hole and the liquid hole under the thrust action of the injector to wash and disinfect the tunnel;
when the medicine needs to be applied, the paste or gel medicine is smeared on the rough surface of the catheter with the rough surface, and then the catheter is conveyed to the position of the tunnel inflammation, so that the medicine is smeared on the infected part of the tunnel.
The invention has the beneficial effects that:
(1) on one hand, the catheter can be extended into the deep part of the tunnel to effectively probe the inflammation part of the tunnel for timely flushing, disinfection and antibiosis, and the patient can be flushed at home, so that the cleaning efficiency is high, and the deep tunnel inflammation or granulation can be effectively treated in time; on the other hand, the two different modes of treatment, namely flushing (liquid medicine) and smearing (gel or paste), can be realized; the injection speed is moderate, so that the problem that the infection focus falls off and is attached to other parts of the tunnel due to quick washing can be avoided;
(2) on one hand, the structure can be simplified and the convenience of operation can be improved no matter one catheter is designed to realize washing and medicine applying or two catheters are arranged to respectively wash and apply the medicine; on the other hand, only the end of the catheter inserted into the tunnel mouth needs to be designed into a structure with high hardness, so that a structure of needle puncture and hose sliding nesting does not need to be arranged like the prior art, the structure is greatly simplified, the end of the catheter is provided with a main tip hole and matched with a side hole, the end of the catheter can be better inserted into the deepest part of the tunnel through the main tip hole, the tunnel mouth can be flushed by a larger effective area, and under the condition of the same pushing speed, the pressure generated locally is small, and the risk of scattering of a local inflammation part or bacteria falling off is reduced; and the liquid outflow speed of the main hole at the tip end in unit time can be reduced through the side holes, so that the liquid inlet speed is limited during flushing, and the infected stove can not fall off and be attached to other parts of the tunnel due to quick flushing.

Detailed Description
The invention will be described in further detail below with reference to the drawings and specific examples.
Example 1
As shown in fig. 1: a tunnel washing and disinfecting catheter for dialysis, wherein an inclined port is arranged at one end (hereinafter referred to as a front end head 11) of a catheter 1 inserted into a tunnel port to form a main hole 2 at the tip end; and a plurality of side holes 3 are arranged on the section of the tube body extending into the tunnel; the end of the catheter 1 away from the tunnel entrance (hereinafter referred to as the rear end) is provided with a connector 4 for connecting a syringe.
Specifically, the front end 11 of the catheter 1 has a certain hardness, the remaining part of the catheter except the front end is a hose 12, the hardness of the front end is greater than that of the hose 12, for example, a plastic head is adopted, so as to smoothly enter the deep part of the tunnel, and if the front end 11 is too hard, the soft tissue of the tunnel and the peritoneal dialysis catheter running in the tunnel are easily damaged or scratched; if the front end 11 is too soft, it will not enter the tunnel deep easily. The plastic head and the hose are fixed by gluing or directly integrated into one piece.
