Abstract

本发明公开了一种双层纤维环补片、补片夹钳及纤维环修复方法,通过设置内层可折叠补片和外层可折叠补片,并通过交叉的一对柔性连接件分别连接两个补片的两端;进一步设置一对辅助连接件于外层可折叠补片的两侧,可用于配合夹钳展开外层可折叠补片并实现补片固定。在两个补片的折叠状态下,夹钳将内层可折叠补片穿过破口进入纤维环内,使得内层可折叠补片展开至纤维环内层;外层可折叠补片展开至纤维环外层,并通过两个辅助连接件与缝合线配合固定在纤维环外层上;内层可折叠补片被柔性连接件限位在纤维环内层。修复过程仅需夹钳置入内层可折叠补片和缝合辅助连接件与纤维环,操作简便,解决了现有纤维环修复手术操作复杂及纤维环再破坏的问题。

Description

一种双层纤维环补片、补片夹钳及纤维环修复方法
技术领域
本发明属于纤维环修复技术领域,尤其涉及一种双层纤维环补片、补片夹钳及纤维环修复方法。
背景技术
人体椎间盘由软骨终板、纤维环和髓核三个部分组成,纤维环是个多层结构,纤维环纤维几乎呈同心圆排列,其外周纤维方向较垂直,而越到中心倾斜度越大。髓核位于椎间盘的中央,水分含量可占髓核总量的75-90%。
腰椎间盘突出症是较为常见的疾患之一,主要是因为腰椎间盘各部分尤其是髓核,在不同程度的退行性改变后,在力因素的作用下,椎间盘的纤维环破裂,形成纤维环缺口,髓核组织从破裂之处突出至后方或椎管内,导致相邻脊神经根遭受刺激或压迫,从而产生腰部疼痛,一侧下肢或双下肢麻木、疼痛等一系列临床症状。腰椎间盘突出症以腰4-5、腰5-骶1发病率最高,约占95%。
目前主要采用开放式手术,将压迫神经的突出椎间盘组织及增生肥厚的组织致狭窄的骨性结构切除(手术减压),其手术创伤大,患者恢复时间长。
髓核摘除术是针对腰椎间盘突出保守和传统治疗无效患者进行的一种微创手术,效果显著,尤其适合腰椎间盘突出反复发作的患者。该治疗不使用螺钉、金属棒固定,在进行髓核摘除的同时还能去除黄韧带肥厚以及解决小关节增生引起的神经根管狭窄的问题,而且费用相对低,住院时间短,恢复快,创伤小。然而,目前椎间盘摘除术仅对突出的髓核做切摘,在切除髓核的同时,对椎间盘纤维环进行了更大范围的破坏,即纤维环缺口更大,但一般并不对椎间盘纤维环进行修补,术后有较高的二次复发率,给患者带来较大的痛苦,同时一次手术后纤维环的疤痕愈合也给二次治疗带来很大的挑战。因此,在进行椎间盘切除术时,应该对纤维环加以修复,降低术后复发及术后并发症的发生率。
纤维环修复主要技术包括:外科手术直接缝合纤维环、组织工程支架技术、细胞和基因治疗与支架联合应用等。
下面以一些现有技术进行举例:
一、椎间盘的纤维环用修补器(授权公告号:CN 105796165 B)
修补器组成:用于插入下椎体终板下侧的锚定件,以固定椎间盘的纤维环用修补器;封堵件,一端与锚定件连接,另一端用于由纤维环缺口处插入椎间盘,以使封堵件围绕髓核后上部以封堵纤维环缺口,避免髓核再突出。
操作方法:取出突出物后,在第一椎体终板骨靠近第一椎体的表面上开槽,并在锚定件与封堵件的连接处安装递送器。然后将锚定件放置在合适位置,利用木锤轻轻锤击递送器手柄,从而使得锚定件插入下椎体终板下侧。最后将递送器从修补器上取下,将封堵部通过破口塞进纤维环内,完成封堵。
装置不足:
1)需要在椎体上开槽,对骨有额外的破坏;
2)采用钛金属制作锚定件,表面锋利(不然很难实现骨与锚定件的固定),容易破坏骨质;而且骨与金属钛材料属性相差大,容易引起应力遮挡等问题;
3)封堵部材料是医用涤纶编织件,一部分在破口外,一部分在破口内,阻断了破口再愈合的机会;
4)封堵部的一侧与锚定件链接固定在椎体上,但是另一侧并没有固定,有再突出的风险。
二、纤维环缝合装置
(1)用于腰椎间盘突出髓核微创摘除术后的纤维环缝合装置(授权公告号:CN106063714 B)
装置组成:包括缝针体和缝线,所述的缝针体包括导针丝和导针套筒,所述的导针套筒套设于所述导针丝上,并由导针丝上的限位装置限位;所述的缝线通过缝线连接装置与所述的导针丝连为整体。导针丝包括针杆和针钩,所述的针杆和针钩的连接处设有一周套筒限位体,所述的套筒限位体为绕所述导针丝一周的隆起的弧形结构,所述的导针套筒的下端设为尖体,所述的导针套筒与所述导针丝的相对位置通过所述套筒限位体与所述导针套筒下端的尖体限制,其中针杆、针钩以及套筒限位体为一体化成型结构。缝线固定在所述缝线连接装置的锥尖处,当采用手术持针器向外拔缝线连接装置,可将缝线连接装置带线拔出。
操作方法:缝线连接在针钩上,并将导针套筒套接在导针丝上,下推导针筒,针钩带着缝线穿进破口的一侧,手持固定针的位置,取出导针套筒,用持针器从针钩上将带线的缝线连接装置拔掉,取出导针丝,再将另一缝线接在针钩上,并将导针套筒套接在导针丝上,下推导针筒,针钩带着缝线穿进破口的另一侧,将缝线的两端打结并减掉多余部分,缝合完毕。
装置不足:
1)这样来回放入取出导针套筒套和导针丝,椎间盘缝合操作相当复杂,且手术时间较长;
2)常用术式使得纤维环开口的地方位于神经根后方,一旦操作不恰当,很容易导致神经根和硬膜的损伤,这对医生的技术要求很高;
3)术中当完成将线穿过破口的一侧后,采用手持的方式固定针,再将另一线带入进行另一侧的操作,这样的固定方式很不稳定,一不小心手动了可能会导致二次损伤纤维环;
4)这样的缝合方式只能完成单针缝合,破口的封闭性很差。
(2)椎间盘突出纤维环单针定位缝合装置(授权公告号:CN 112401957 B)
装置组成:装置包括通道管,通道管内设置气动组件,气动组件内端设置缝合组件;缝合组件包括圆弧形的弧形筒,弧形筒直径不大于通道管内径,弧形筒远端内部设置缝合针,缝合针呈圆弧形,其弧度与弧形筒的弧度一致,缝合针配合在弧形筒内,缝合针针头朝外设置,缝合针的尾端固定弧形活塞。
操作方法:推动推杆A,带动活塞将空气推入C形通道,进而推动活塞而推动针,针与线通过凝胶粘贴在一起,线的前端部分事先打好结,针带着线从纤维环破口的一侧穿入,从破口另一侧穿出进入C形通道的另一端,这个部分设有条形缺口和若干卡口,针与线到达这个位置后,结会挂在卡口上,通过拉推杆A带动活塞抽空气使得针复位,但是由于结卡在卡口处,所以针与线分离,再打结,完成一针缝合。
装置不足:
1)该装置采用气动驱动针进行缝合,这对C形装置的密闭性要求很高;
2)缝合线与针采用凝胶粘贴在一起的,有脱落的风险,后面通过提前打好的结挂在侧壁卡口上实现针与线的分离,这更让人怀疑针与线之间的相接关系,因为当针穿过纤维环的时候也会有一定的阻力,不能确保此时针与线再纤维环的阻力下发生分离,因此这一装置操作上是比较复杂的。
(3)一种用于椎间盘微创手术的亲水润滑扩张球囊(申请公布号:CN112190822A)
装置组成:包括连接头,所述连接头的一侧外壁上设置有通道导管,所述通道导管内设置有内导管,且内导管的一端与球囊连接,所述球囊由亲水性润滑涂层、热塑性聚氨酯层、软质聚氯乙烯层和尼龙层组成,所述球囊的外壁置有旋切针,内壁上间隔设置有纵向筋。
操作方法:将未充气的球囊置入纤维环内,通过通道导管给球囊充气。在干燥状态下,球囊有良好的韧性,涂层在球囊表面均匀附着,无色透明,肉眼不易观察到;在润湿状态下,涂层被水激活,形成无色透明的水凝胶,该水凝胶涂层高度润滑且可以承受反复摩擦,提高球囊与人体之间的生物相容性,避免人体受到损害,有效提升球囊的机械强度和稳定性,并且抗腐蚀性、耐疲劳性和抗老化性都比较突出。
装置不足:
1)置入球囊体积较大,影响椎间盘的生物力学分布;
2)球囊通过充气完成破口的封堵,由于手术视野有限,充气程度对医生的经验要求比较高;
3)髓核是接近于水的物质,球囊在这样的环境中可能会发生游走,难以稳定在破口周围。
因此,现有产品存在着下述缺陷:
1)因为是微创手术,都是在镜下操作,但是操作空间又小,为了实现稳定缝合,缝合装置设计一般操作起来比较复杂,手术时间延长;
2)因为手术难度大,所以研发成本高,装置定价也高;
3)因为操作难,所以破口一般缝合的不是很密,髓核是液态的,所以还是会有髓核再突出的风险;
4)缝合会破坏椎间盘的生物力学特征,存在缝合位置二次破坏的可能;
5)现有的补破口的产品,为了达到固定补片,设置一个金属的装置将补片固定在骨头上,但这样会破坏一部分骨头。
发明内容
本发明要解决的技术问题是提供一种双层纤维环补片、补片夹钳及纤维环修复方法,以解决现有纤维环修复手术操作复杂的问题。
为解决上述问题,本发明的技术方案为:
本发明的一种双层纤维环补片,包括:
内层可折叠补片;
外层可折叠补片;
至少一对交叉设置的柔性连接件;所述内层可折叠补片和所述外层可折叠补片在折叠状态下的折叠开口相对设置,且两个所述柔性连接件的两端分别连接所述内层可折叠补片和所述外层可折叠补片的对角端;
至少一对辅助连接件,两个所述辅助连接件分别相对设置在所述外层可折叠补片的两侧;
展开状态下,所述内层可折叠补片和所述外层可折叠补片展平,两个所述柔性连接件交叉并限位所述内层可折叠补片和所述外层可折叠补片分别位于纤维环的内侧和外侧。
本发明的双层纤维环补片,所述内层可折叠补片包括堆叠设置的第一亲水层和第一支撑层,且折叠状态下所述第一亲水层位于所述第一纤维层的内侧;
所述外层可折叠补片包括堆叠设置的第二亲水层和第二支撑层,且折叠状态下所述第二亲水层位于所述第二支撑层的内侧。
本发明的双层纤维环补片,所述第一亲水层的纤维方向与纤维环最内层的纤维方向相同;
所述第二亲水层的纤维方向与纤维环最外层的纤维方向相同。
本发明的双层纤维环补片,所述第一亲水层和所述第二亲水层上均设有多孔结构。
本发明的双层纤维环补片,还包括一记忆金属丝,穿设于所述内层可折叠补片,用于在遇到髓核后展开所述内层可折叠补片。
本发明的双层纤维环补片,所述柔性连接件为连接线,且两根所述连接线的长度大于等于展开状态下所述内层可折叠补片与所述外层可折叠补片相对的两端之间的距离。
本发明的双层纤维环补片,所述辅助连接件为线扣环。
本发明的一种补片夹钳,应用于上述任意一项所述的双层纤维环补片,包括:
夹钳本体;
至少一对卡勾,与至少一对所述辅助连接件相对应,用于使所述辅助连接件挂设在对应的所述卡勾上。
本发明的补片夹钳,所述夹钳本体上开设有用于使记忆金属丝穿过的通孔。
本发明的一种纤维环修复方法,应用于上述任意一项所述的双层纤维环补片和上述任意一项所述的补片夹钳,步骤如下:
步骤S1:所述补片夹钳夹持折叠后的所述外层可折叠补片,所述辅助连接件挂设于对应的所述卡勾,且记忆金属丝穿设所述内层可折叠补片;
步骤S2:所述补片夹钳经微创手术通道将所述双层纤维环补片中的所述内层可折叠补片通过破口放入椎间盘内,直至所述记忆金属丝遇到髓核后撑开并带动所述内层可折叠补片展开,展开后抽出所述记忆金属丝;
步骤S3:所述补片夹钳带动所述外层可折叠补片退出所述破口,带动展开后的所述内层可折叠补片贴合至纤维环的内层;
步骤S4:所述补片夹钳逐步松开并通过所述卡勾展开所述外层可折叠补片,完全松开后所述补片夹钳脱离所述外层可折叠补片,并通过所述补片夹钳的头部将所述外层可折叠补片展平贴合纤维环的外层;
步骤S5:采用缝合针将鱼骨线依次穿过位于所述外层可折叠补片一侧的纤维环和位于同侧的所述辅助连接件;并将鱼骨线依次穿过位于所述外层可折叠补片另一侧的纤维环和位于同侧的另一所述辅助连接件,完成一针固定。
本发明的纤维环修复方法,在步骤S5中,所述缝合针为半圆形,且在缝合前,所述缝合针的尖端放置于纤维环上,所述缝合针的尾端放置于所述辅助连接件处。
本发明的纤维环修复方法,所述缝合针的半径为4~6mm。
本发明的纤维环修复方法,所述鱼骨线上的鱼骨刺分别设置在第一针穿出位置、第二针穿入位置和第二针穿出位置。
本发明的纤维环修复方法,所述鱼骨线末端打结,距线结1~3mm处均匀设置若干鱼骨刺,用于固定第一针穿出位置,再间隔7~15mm设置若干鱼骨刺,用于固定第二针穿入位置和第二针穿出位置。
本发明的纤维环修复方法,所述鱼骨线上的鱼骨刺的夹角呈100°且方向与所述缝合针的前进方向一致。
本发明由于采用以上技术方案,使其与现有技术相比具有以下的优点和积极效果:
1、本发明一实施例通过设置内层可折叠补片和外层可折叠补片,并通过交叉的一对柔性连接件分别连接两个补片的两端,形成X形的交叉连接;进一步设置了一对辅助连接件,设置在外层可折叠补片的两侧,可用于配合夹钳展开外层可折叠补片,也可配合缝合线将外层可折叠补片固定在纤维环上。在内层可折叠补片和外层可折叠补片的折叠状态下,通过夹钳将内层可折叠补片穿过破口进入纤维环内,使得内层可折叠补片可展开并贴合至纤维环内层;而外层可折叠补片则可贴合至纤维环的外层,通过两个辅助连接件与缝合线配合将外层可折叠补片固定在纤维环的外层上,此时内层可折叠补片在两个交叉的柔性连接件的作用下,被限位在纤维环的内层,从而实现了对纤维环的修复。整个修复过程仅需夹钳置入内层可折叠补片和缝合辅助连接件与纤维环两步,操作简便,不需要来回穿针打结,解决了现有纤维环修复手术操作复杂的问题。
2、本发明一实施例采用内层可折叠补片和外层可折叠补片的双层补片设计,可完全堵住破口,并且内外层补片之间可以植入细胞或注入修复材料,促进破口的愈合,降低髓核再突出的风险。
3、本发明一实施例通过将内层可折叠补片和外层可折叠补片均设置为亲水层和支撑层的堆叠,亲水层用来贴合纤维环,有利于细胞的生长以及纤维环的愈合;支撑层则用于提供所需的力学强度,能更好的承受椎间盘的压力。同时,两个补片均没有金属成分,生物相容性更好,更安全。
4、本发明一实施例通过将植入后的两层补片的亲水层设置为相对,分布于破口的两侧,贴合性更好;并将两个亲水层设置为粗糙的多孔结构,更有利于细胞的生长,更有利于纤维环的愈合。
5、本发明一实施例将两个亲水层的纤维方向与对应接触的纤维环的纤维方向设置为一致,可更好保留纤维环自身结构和受力方式。
6、本发明一实施例通过设置两个线扣环,更便于补片和纤维环的缝合固定,使得外层可折叠补片的固定仅需要在纤维环上缝两针,因为缝针很小,基本不损伤纤维环,不额外破坏锥体。
7、本发明一实施例将缝合针设计为直径为5mm的半圆形针,可以围绕圆心旋转,实现在小空间中穿过纤维环和线扣环,完成缝合。整个缝合过程只接触到纤维环和线扣环,缝合阻力不大,缝合操作更容易实现。
8、本发明一实施例将鱼骨线上的鱼骨刺设置在第一针穿出位置、第二针穿入位置和第二针穿出位置,可更好地固定鱼骨线、外层可折叠补片和纤维环。
9、本发明一实施例通过将鱼骨刺夹角设置为100°,方向与缝合针前进方向一致,有利于穿入并再受到后侧拉力时鱼骨刺卡在组织上,可牢牢锁住组织,防止出现滑线现象,具有免打结的特点。同时,可将鱼骨刺设置为末端圆钝,防止在纤维环受力变形后对划伤/破坏纤维环。

以下结合附图和具体实施例对本发明提出的一种双层纤维环补片、补片夹钳及纤维环修复方法作进一步详细说明。根据下面说明和权利要求书,本发明的优点和特征将更清楚。
实施例一
参看图1至图3,在一个实施例中,一种双层纤维环补片,包括内层可折叠补片1、外层可折叠补片2、至少一对交叉设置的柔性连接件和至少一对辅助连接件。
其中,内层可折叠补片1和外层可折叠补片2在折叠状态下的折叠开口相对设置,且两个柔性连接件的两端分别连接内层可折叠补片1和外层可折叠补片2的对角端。两个辅助连接件分别相对设置在外层可折叠补片2的两侧。展开状态下,内层可折叠补片1和外层可折叠补片2展平,两个柔性连接件交叉并限位内层可折叠补片1和外层可折叠补片2分别位于纤维环3的内侧和外侧。
本实施例通过设置内层可折叠补片1和外层可折叠补片2,并通过交叉的一对柔性连接件分别连接两个补片的两端,形成X形的交叉连接;进一步设置了一对辅助连接件,设置在外层可折叠补片2的两侧,可用于配合夹钳展开外层可折叠补片2,也可配合缝合线将外层可折叠补片2固定在纤维环3上。在内层可折叠补片1和外层可折叠补片2的折叠状态下,通过夹钳将内层可折叠补片1穿过破口进入纤维环3内,使得内层可折叠补片1可展开并贴合至纤维环3内层;而外层可折叠补片2则可贴合至纤维环3的外层,通过两个辅助连接件与缝合线配合将外层可折叠补片2固定在纤维环3的外层上,此时内层可折叠补片1在两个交叉的柔性连接件的作用下,被限位在纤维环3的内层,从而实现了对纤维环3的修复。整个修复过程仅需夹钳置入内层可折叠补片1和缝合辅助连接件与纤维环3两步,操作简便,不需要来回穿针打结,解决了现有纤维环修复手术操作复杂及纤维环再破坏的问题。
同时,采用内层可折叠补片1和外层可折叠补片2的双层补片设计,可完全堵住破口,保证修复效果,并且内外层补片之间可以植入细胞或注入修复材料,促进破口的愈合,降低髓核再突出的风险。
下面对本实施例的双层纤维环补片的具体结构进行进一步说明:
在本实施例中,内层可折叠补片1包括堆叠设置的第一亲水层和第一支撑层,且折叠状态下第一亲水层位于第一支撑层的内侧。外层可折叠补片2包括堆叠设置的第二亲水层和第二支撑层,且折叠状态下第二亲水层位于第二支撑层的内侧。通过将内层可折叠补片1和外层可折叠补片2均设置为亲水层和支撑层的堆叠,亲水层用来贴合纤维环3,有利于细胞的生长以及纤维环3的愈合;支撑层则用于提供所需的力学强度,能更好的承受椎间盘的压力。同时,两个补片均没有金属成分,生物相容性更好,更安全。
进一步地,第一亲水层的纤维方向与纤维环3最内层的纤维方向相同,第二亲水层的纤维方向与纤维环3最外层的纤维方向相同。将两个亲水层的纤维方向与对应接触的纤维环3的纤维方向设置为一致,可更好保留纤维环3自身结构和受力方式。
进一步地,第一亲水层和第二亲水层上均可设有多孔结构。将植入后的两层补片的亲水层设置为相对,分布于破口的两侧,贴合性更好;并将两个亲水层设置为粗糙的多孔结构,更有利于细胞的生长,更有利于纤维环3的愈合。
其中,亲水层具体可为由纤维编织而成的多孔结构,亲水性能好,易贴合。而支撑层作为承重结构,需要一定的强度,故材质可采用高分子材料。
参看图4,在本实施例中,双层纤维环补片还可包括一记忆金属丝6,穿设于内层可折叠补片1,用于在遇到髓核后展开内层可折叠补片1。具体地,记忆金属丝6的布置方式可为折叠后的折叠端伸入折叠状态下的内层可折叠补片1,当伸入破口后,记忆金属丝6的折叠端即可恢复原有形状,从而撑开内层可折叠补片1。
在本实施例中,柔性连接件为连接线5,且两根连接线5的长度均大于展开状态下内层可折叠补片1与外层可折叠补片2相对的两端之间的距离。连接线5的长度宜长不宜短,需要保证两层补片均可完全展开,连接线5长度少量超出的部分并不会影响愈合。
在本实施例中,辅助连接件具体可为线扣环4,两个线扣环4分别布置于外层可折叠补片2的两端。设置两个线扣环4,更便于补片和纤维环3的缝合固定,使得外层可折叠补片2的固定仅需要在纤维环3上缝两针,因为缝针很小,基本不损伤纤维环3,不额外破坏锥体。
实施例二
参看图5至图7,一种补片夹钳,应用于实施例一中的双层纤维环补片,包括夹钳本体7和至少一对卡勾8。
其中,卡勾8与上述实施例一中的线扣环4相对应,线扣环4可挂设在对应的卡勾8上。夹钳本体7上则可开设有用于使记忆金属丝6穿过的通孔9,由于记忆金属丝6的两端是开放的,故在夹钳本体7上设置通孔9,具体可为两个,可对记忆金属丝6的两端进行限位,避免记忆金属丝6在撑开时对周边组织造成损伤;同时,记忆金属丝6在通孔9内也可便于后续的抽出。
下面对本实施例的补片夹钳如何展开外层可折叠补片2进行说明:当夹钳本体7夹紧外层可折叠补片2时,卡勾8卡住线扣环4;当夹钳本体7逐渐松开时,卡勾8逐渐放开线扣环4,再用夹钳本体7圆钝的头部将外层可折叠补片2展平,贴在纤维环3外侧。
实施例三
参看图8,本实施例提供了一种应用于上述实施例一和实施例二的缝合针10和缝合线。
其中,缝合针10的形状设置为半圆形,直径为4~6mm,较优值为5mm,使用时缝合针10的尖端放置于纤维环3上,缝合针10的尾端放置于线扣环4中,便于定位。操作时,围绕圆心旋转,实现在小空间中穿过纤维环3和线扣环4,完成缝合。整个缝合过程只接触到纤维环3和线扣环4,缝合阻力不大,缝合操作更容易实现。
缝合线则是采用间断设置若干鱼骨刺12的鱼骨线11,其中,鱼骨刺12之间的间距较小。鱼骨线11末端打结,距线结1~3mm处均匀设置三排鱼骨刺12,用于固定第一针,再隔7~15mm再设置八排鱼骨刺12,用于固定第二针,可根据需求保留鱼骨刺12数量,其余为不设置鱼骨刺12的光滑段。将鱼骨线11上的鱼骨刺12设置在第一针穿出位置、第二针穿入位置和第二针穿出位置,可更好地固定鱼骨线11、外层可折叠补片2和纤维环3。
鱼骨刺12夹角呈100°,方向与缝针前进方向一致,有利于穿入并再受到后侧拉力时鱼骨刺12卡在组织上,可牢牢锁住组织,防止出现滑线现象,具有免打结的特点;鱼骨刺12末端圆钝,防止在纤维环3受力变形后对划伤/破坏纤维环3。
实施例四
本发明的一种纤维环修复方法,应用于上述实施例一中的双层纤维环补片、上述实施例二中的补片夹钳和上述实施例三中的缝合针10与缝合线,具体步骤如下:
步骤S1:补片夹钳夹持折叠后的外层可折叠补片2(夹持位置是外层可折叠补片2折叠后形成的折叠端),线扣环4挂设于对应的卡勾8,且记忆金属丝6穿过夹钳本体7上的通孔9并穿设至内层可折叠补片1。
步骤S2:通过常规手术建立的微创手术通道,由补片夹钳将双层纤维环补片通过破口放入椎间盘内,直至记忆金属丝6遇到髓核后撑开并带动内层可折叠补片1展开,展开后顺着夹钳本体7上的通孔9抽出记忆金属丝6。
步骤S3:补片夹钳带动外层可折叠补片2退出破口,带动展开后的内层可折叠补片1贴合至纤维环3的内层(即第一亲水层贴合至纤维环3的内层)。
步骤S4:逐步松开补片夹钳连带着卡勾8配合线扣环4逐步展开外层可折叠补片2,完全松开后补片夹钳脱离外层可折叠补片2(即卡勾8也与线扣环4分离),并通过补片夹钳圆钝的头部将外层可折叠补片2展平至纤维环3的外层(即第二亲水层贴合至纤维环3的外层)。
步骤S5:退出补片夹钳,将缝合针10固定在补片夹钳的夹持机构上,缝合针10预先穿好鱼骨线11。将缝合针10的尖端一侧放置在纤维环3上,针尾侧放置在外层可折叠补片2一侧的线扣环4内固定好缝合的位置,旋转缝合针10,缝合针10带着鱼骨线11穿入补片前沿的纤维环3,再从同一侧线扣环4中穿出;然后缝合针10带着鱼骨线11在另一侧用相同的缝合方法完成一针固定,依靠鱼骨刺12固定,减去多余的鱼骨线11,完成外层可折叠补片2与纤维环3的固定(即通过鱼骨线11配合两个线扣环4将外层可折叠补片2压在纤维环3上,同时线扣环4可对外层可折叠补片2进行限位)。
本实施例的整个缝合过程只接触到纤维环3和线扣环4,缝合阻力不大,缝合操作更容易实现;鱼骨刺12设置在第一针穿出后、第二针穿入前、第二针穿出后三个位置,更好地固定缝线、补片和纤维环3,并且整个缝合更平整,可避免因为鱼骨刺12而导致对周围组织或神经的损伤。
在其他实施例中,缝合线也可以采用一般的缝线,缝合针10预先穿好缝合线,并且缝合线末端打结。将缝合针10的尖端侧放置在纤维环3上,针尾侧放置在外层可折叠补片2一侧的线扣环4内固定好缝合的位置,旋转缝合针10,缝合针10带着缝合线穿入补片前沿的纤维环3,再从同一侧的线扣环4中穿出,再穿入纤维环3再穿出线扣环4完成一侧的两针固定,然后缝合针10带着缝合线在另一侧用相同的缝合方法完成两针固定,最后在线扣环4上完成打结,减去多余的缝线,完成外层可折叠补片2与纤维环3的固定。另外需要根据纤维环3的缺口尺寸,选择相应规格大小的补片。具体的,利用试模检测确定纤维环3的缺口尺寸,从而可以选择相应规格大小的补片。本实施例中,缝合可以单针缝合也可以多针缝合,可以根据补片大小选择。只要是满足纤维方向与贴近的纤维环3的纤维方向一致、有一定支撑作用的补片都可满足本发明,包括可吸收或者不可吸收的补片。
上面结合附图对本发明的实施方式作了详细说明,但是本发明并不限于上述实施方式。即使对本发明作出各种变化,倘若这些变化属于本发明权利要求及其等同技术的范围之内,则仍落入在本发明的保护范围之中。

Claims (15)

1.一种双层纤维环补片,其特征在于,包括:
内层可折叠补片;
外层可折叠补片;
至少一对交叉设置的柔性连接件;所述内层可折叠补片和所述外层可折叠补片在折叠状态下的折叠开口相对设置,且两个所述柔性连接件的两端分别连接所述内层可折叠补片和所述外层可折叠补片的对角端;
至少一对辅助连接件,两个所述辅助连接件分别相对设置在所述外层可折叠补片的两侧;
展开状态下,所述内层可折叠补片和所述外层可折叠补片展平,两个所述柔性连接件交叉并限位所述内层可折叠补片和所述外层可折叠补片分别位于纤维环的内侧和外侧。
2.如权利要求1所述的双层纤维环补片,其特征在于,所述内层可折叠补片包括堆叠设置的第一亲水层和第一支撑层,且折叠状态下所述第一亲水层位于所述第一支撑层的内侧;
所述外层可折叠补片包括堆叠设置的第二亲水层和第二支撑层,且折叠状态下所述第二亲水层位于所述第二支撑层的内侧。
3.如权利要求2所述的双层纤维环补片,其特征在于,所述第一亲水层的纤维方向与纤维环最内层的纤维方向相同;
所述第二亲水层的纤维方向与纤维环最外层的纤维方向相同。
4.如权利要求2所述的双层纤维环补片,其特征在于,所述第一亲水层和所述第二亲水层上均设有多孔结构。
5.如权利要求1所述的双层纤维环补片,其特征在于,还包括一记忆金属丝,穿设于所述内层可折叠补片,用于在遇到髓核后展开所述内层可折叠补片。
6.如权利要求1所述的双层纤维环补片,其特征在于,所述柔性连接件为连接线,且两根所述连接线的长度大于等于展开状态下所述内层可折叠补片与所述外层可折叠补片相对的两端之间的距离。
7.如权利要求1所述的双层纤维环补片,其特征在于,所述辅助连接件为线扣环。
8.一种补片夹钳,其特征在于,应用于如权利要求1至7任意一项所述的双层纤维环补片,包括:
夹钳本体;
至少一对卡勾,与至少一对所述辅助连接件相对应,用于使所述辅助连接件挂设在对应的所述卡勾上。
9.如权利要求8所述的补片夹钳,其特征在于,所述夹钳本体上开设有用于使记忆金属丝穿过的通孔。
10.一种纤维环修复方法,其特征在于,应用于如权利要求1-7任意一项所述的双层纤维环补片和如权利要求8或9任意一项所述的补片夹钳,步骤如下:
步骤S1:所述补片夹钳夹持折叠后的所述外层可折叠补片,所述辅助连接件挂设于对应的所述卡勾,且记忆金属丝穿设所述内层可折叠补片;
步骤S2:所述补片夹钳经微创手术通道将所述双层纤维环补片中的所述内层可折叠补片通过破口放入椎间盘内,直至所述记忆金属丝遇到髓核后撑开并带动所述内层可折叠补片展开,展开后抽出所述记忆金属丝;
步骤S3:所述补片夹钳带动所述外层可折叠补片退出所述破口,带动展开后的所述内层可折叠补片贴合至纤维环的内层;
步骤S4:所述补片夹钳逐步松开并通过所述卡勾展开所述外层可折叠补片,完全松开后所述补片夹钳脱离所述外层可折叠补片,并通过所述补片夹钳的头部将所述外层可折叠补片展平贴合纤维环的外层;
步骤S5:采用缝合针将鱼骨线依次穿过位于所述外层可折叠补片一侧的纤维环和位于同侧的所述辅助连接件;并将鱼骨线依次穿过位于所述外层可折叠补片另一侧的纤维环和位于同侧的另一所述辅助连接件,完成一针固定。
11.如权利要求10所述的纤维环修复方法,其特征在于,在步骤S5中,所述缝合针为半圆形,且在缝合前,所述缝合针的尖端放置于纤维环上,所述缝合针的尾端放置于所述辅助连接件处。
12.如权利要求11所述的纤维环修复方法,其特征在于,所述缝合针的半径为4~6mm。
13.如权利要求10所述的纤维环修复方法,其特征在于,所述鱼骨线上的鱼骨刺分别设置在第一针穿出位置、第二针穿入位置和第二针穿出位置。
14.如权利要求13所述的纤维环修复方法,其特征在于,所述鱼骨线末端打结,距线结1~3mm处均匀设置若干鱼骨刺,用于固定第一针穿出位置,再间隔7~15mm设置若干鱼骨刺,用于固定第二针穿入位置和第二针穿出位置。
15.如权利要求10所述的纤维环修复方法,其特征在于,所述鱼骨线上的鱼骨刺的夹角呈100°且方向与所述缝合针的前进方向一致。
