Abstract

本发明涉及一种能植入人体的人工腱,由于采用了梯次促腱化处理的人发作人工腱的腱体,令人工腱在被吸收并转化为人体自体腱过程中,人工腱能有效地维持患部的腱功能。本发明的人工腱由于最终能完全转化为人体自体腱,因此克服了以住人体代组织所具有的缺陷;具有很广阔的应用前景。

Description

一种人工腱及其制法和应用
本发明涉及一种能植入人体代替人体自体腱的人工腱。
在1935~1959年间,Teneff,Fanda Bunell研究“同种异体腱”。经实验后发现有坏死阶段,仅起支架作用并有排斥反应。自1977年起,Jenkens研究成“碳纤维腱”,曾在国外及我国广泛应用,碳纤维并非蛋白质,在人体内无排斥反应,但经进一步试验发现18个月后在人体内仍为碳纤维,并已破碎,只形成缔组织外膜,不腱化。至1980年我国戴克戎以硅胶代腱,虽然,硅胶腱不粘连易滑动,但是衔接部不能融为一体,不被体内吸收,易折,并一经感染便成为异物。在1988年我国黄风鸣以人头发代腱,它的拉应力不易衰减,但不能被肌体吸收,不能腱化成机体自身腱,一经感染便成为异物,与组织发生粘连并只能用于跟腱等大动力的肌腱,不能用于手、足屈伸腱。
本发明的目的是提供一种能完全转变为人体机体自体腱的人工腱及其制法和应用。
本发明是这样实现的:人工腱体(1)是由2种以上的,经促腱化梯次处理的人发束结在一起形成,腱体(1)由人体胎盘膜(2)将其表面包裹,腱体(1)和胎盘膜(2)共同形成人工腱。2种以上(例如三种)经不同梯次促腱化处理的人发,在植入人体后,在被人体吸收并重新腱化的时间上呈梯次变化,即第一种人发处于被吸收及形成自体腱的过程时,其腱的作用呈减退→消失→重建的变化;这时,第二、三种人发处于未变化状况,从而有效地起着腱的作用;当第二种人发进入上述变化之时,第一种人发已正在形成自体腱;第三种人发仍处于未变化状态;从而第一、三两种人发共同执行腱的作用;当第三种人发进入上述变化状态之时;第一种人发已完全被吸收并变成个体自体腱,第二种人发进入被吸收及形成人体自体腱的过程,从而,第一、二种人发转化为自体腱执行腱的作用。最终三种人发都完全被吸收并完全转化为人体自腱。2种以上的人发可混合在一起编织成索或用可降解医用胶粘合成素。腱体(1)外的胎盘膜(2)是用从人体胎盘中剥离的胎儿面之膜层;可采用可降解吸收线缝裹紧在腱体(1)的表面,或者采用能降解吸收的医用胶将胎盘膜(2)粘在腱体(1)的表面。胎盘膜(2)的作用,是将腱体(1)与周围组织隔开,使腱体(1)在被吸收→自体腱化过程中不与周围组织粘连、融合在一起。
本发明的促腱化梯次处理做法是:将人发脱脂后清洗,用R+OH-药物进行处理,其温度总范围是7℃~40℃;一个梯次处理的跨级为5℃~15℃;其浓度总范围是0.005~10%,一个梯次处理跨级为0.005~4.5%;其处理时间为2小时以上,一个梯级处理跨级为1小时以上;将部分经以上处理的人发放入氧化剂中处理;将上述处理的人发清洗后放入抗排斥反应药物中固定,最后清洗并高温高压灭菌。其中,R+OH-药物中R代表有机或无机原子团也可代表一价金属,例如K+或Na+。其中梯次处理的参数范围为较佳的范围。抗排斥反应药物可采用常规的专用药物如R-(CHO)n类药品;其中的氧化剂可采用ROOH(R可以是有机原子团,也可指锌、钠、钾、锂)。另外,各梯级处理的跨级可以是等级的,也可以是不等级的。
本发明的人工腱植入方法是:将腱体(1)消毒取出后,浸于生理盐水中15分钟至2小时,将胎盘膜(2)从无菌安瓶中取出在生理盐水中浸泡15~30分钟;将经处理的胎盘膜(2)包裹于经处理的腱体(1)表面作定位固定处理,形成人工腱;将人工腱的两端与患部肌腱残端相接。生理盐水浸泡的作用是消除保存液对人体的影响及令腱体(1)和胎盘膜(2)的湿润合符植入的生理要求。
本发明的植入方法还可以采用将人工腱的两端与患部的肌腱起止点过骨洞固定的方法。
本发明还可以用患者身体的自体膜(2),例如肌膜、腱膜、胸膜、腹膜等来包裹腱体(1),从而共同组成人工腱。
本发明的包裹腱体(1)的膜还可以是非生物膜。由于生物体的生物膜,除患者自体膜和胎盘的胎儿面膜以外,其余的生物体异体膜,由于患者体内生物免疫系统的排异反应;无法充当人工腱的腱体(1)包裹膜;而非生物膜,例如无毒物食品包装塑料薄膜,作为腱体(1)的包裹膜不会引起排异反应;因此,可用它来代替胎盘膜。
本发明包裹腱体(1)的非生物膜,采用可降解的用淀粉为主要原料制成的可降解薄膜。由于上述的塑料薄膜无法降解,故会长期滞留人体内,时间长了对人体内由人工腱转变为自体腱的腱组织会产生不利影响,或者影响功能正常发挥,或者造成患者的不适感。因此,采用可降解的薄膜,例如:如由60~70%以上的淀粉,配合30~40%的塑料组分制成可降解薄膜作为腱体(1)的包裹膜可令人工腱在植入后,腱体(1)被完全吸收、转化为自体腱之后,包裹的薄膜才逐渐降解消失;这样,既达到在腱体(1)未完全自体化之前由它保护腱体(1)不与周围组织粘连或融合为一起,又在腱体(1)自体化之后,逐渐令膜(2)降解消失,从而完全免除膜(2)由于不能自体化而造成的种种缺陷。
图1是本发明人工腱的结构示意图。
本发明由于采用了经过促腱化梯次处理的2种以上人发及胎盘膜做成人工腱;所以,在人工腱体被吸收并转化为人体自体腱的过程中,二种以上的人发在呈现出腱作用的减弱—消失—重建变化的时间上,呈现先后顺次梯级变化之互补作用;使人工腱完全转化为人体自体腱过程中,患者患部的腱功能始终保持完整。从而实现了植入人工腱代替人体原先自体腱的医疗目的。
实施例1
见图1,将人发束用氯仿脱脂后,用蒸馏水清洗;然后将人发分成三等分,用烧碱溶液分别进行浸泡处理;第一种处理:温度5℃,浓度0.005%,处理时间3小时;第二种处理:温度10℃,浓度0.01%,处理时间6小时;第三种处理:温度15℃,浓度0.15%,处理时间为8小时。将经上述第三种处理的人发放入浓度为5%的过氧化钾溶液中常温下浸泡2小时;再将上述三种经处理将人发用蒸馏水清洗后放入戊二醛溶液中液泡8小时,最后将这样处理的三种人发取出均匀混合后部分编织成腱体索(1),一部分编织成腱体套(3),留出一部分作固定胎盘膜的缝合线(4)用。将腱体索(1)穿入腱体套(3)中用蒸馏水清洗后经高温高压进行常规灭菌后无菌封存。将健康人的新鲜的人体胎盘放入浓度为0.001%的氨水溶液中在常温下浸泡20分钟,令人体胎盘各层分离后用含防腐剂的生理盐水清洗20次共30分钟后,取出胎儿面之胎盘膜(2)备用。
上述的胎盘膜(2)如不及时使用可用如下保存液保存,溶液成份为:0.005%的氯芳香烃基胍烷,62.75%二锅头,其余为水。
在需要植入人体患部之时,分别将套有腱体套(3)的腱体索(1)与胎盘膜(2)取出,分别浸入生理盐水中30分钟后取出,将胎盘膜(2)包裹于带腱体套(3)的腱体素(1)外面,用缝合线(4)从外面将其缝合固定形成人工腱,将人工腱的两端与患者手指屈肌腱的残端衔接。
实施例2
腱体(1)所用的人发经促腱化梯次处理后,均匀混合,在需要植入时,再用可降解的医用胶将其粘合或束形成腱体(1),再将胎盘膜(2)用上述医用胶粘在腱体(1)的表面;做成人工腱,将人工腱的两端分别与患者屈指肌腱起止点处过骨洞进行固定。其余的与实施例1相同。
实施例3
用成份为70%的明胶、30%可降解塑料组分制成的可降解塑料薄膜(2)代替胎盘膜(2),其余的与实施例1相同。

Claims (7)

1、一种能植入人体的人工腱,其特征在于:人工腱体(1)是由2种以上的,经促腱化梯次处理的人发束合在一起形成,腱体(1)由人体胎盘膜(2)将其表面包裹,腱体(1)和胎盘膜(2)共同形成人工腱。
2、根据权利要求1所述的能植入人体的人工腱,其特征在于:包裹腱体(1)的胎盘膜(2)用经促腱化处理的人发制成的可最终被吸收的缝合线(4)将其缝合固定。
3、根据权利要求1所述的能植入人体的人工腱,其特征在于:腱体(1)与胎盘膜(2)之间有一经促腱化处理的人发腱体套(3)将腱体(1)套在其中。
4、根据权利要求1或2或3所述的能植入人体的人工腱,其特征在于:腱体(1)由人体自体膜(2)将其包裹,腱体(1)与人体自体膜(2)共同形成人工健。
5、根据权利要求1或2或3所述的能植入人体的人工腱,其特征在于:腱体(1)由非生物膜(2)将其包裹,腱体(1)与非生物膜(2)共同形成人工健。
6、根据权利要求5所述的能植入人体的人工腱,其特征在于:非生物膜(2)是可降解的薄膜。
7、一种权利要求1或2或3中所述的能植入人体的人工腱的人发促腱化梯次处理方法,其特征在于:将人发脱脂后清洗,用含OH根的有机或无机原子团或一价金属与OH根结合的碱进行处理,其处理温度总范围是7℃~40℃;一个梯级处理的跨度为5℃~15℃;其浓度总范围是0.005~10%,一个梯级处理的跨度为0.005~4.5%;其处理时间为两小时以上,一个梯级处理的跨度为一小时以上;将上述处理后的人发清洗后放入抗排斥反应药物中固定,最后清洗并高温高压灭菌。
