In hip joint surgery, the case that the acetabulum part has large-area defect is often encountered, because the acetabulum prosthesis needs multi-point support of peripheral bone to obtain initial stability, the reconstruction of the acetabulum bone defect is the premise of long-term stability of the acetabulum prosthesis, and the acetabulum bone defect is the part of the patients with difficult, long time-consuming, much bleeding and high cost in the primary or revision of the hip joint.

Preferably, the outer surface and the inner surface are arc surfaces protruding in the same direction, and the axes of the inner surface and the outer surface are coincident, and most preferably, the outer surface is a hemisphere surface (as shown in fig. 1 to 4). When the surface of the acetabulum bone defect is not a spherical defect, the acetabulum bone defect can be contused by using the acetabulum contusion to remove a small amount of bone to form the spherical defect suitable for implantation, the loss of the bone is small, the acetabulum defect can be completed by using the conventional acetabulum contusion, the operation is simple, and the universality is strong. The outer surface and the inner surface are arc surfaces protruding towards the same direction, the automatic adaptation effect of the acetabular bone defect block and the acetabular cup is best when the axes of the inner surface and the outer surface are coincided, and the acetabular bone defect block which is larger than one half of the arc surface to the whole arc surface (not including the whole arc surface) is independently arranged between the inner surface and the outer surface of the acetabular bone defect block, so that the risk of overturning the acetabular bone defect block when the acetabular cup or the acetabular cup test mold is driven in and the defect that the acetabular bone defect block is not easily and stably fixed on the bone surface can be avoided; in addition, the outer surface and the inner surface of the acetabular bone defect block are arc surfaces protruding towards the same direction, when the axis of the inner surface is coincident with the axis of the outer surface, the position of the defect block and the acetabular cup cavity are accurately found and the defect block is buckled on the surface of the acetabular cup after the test mold is replaced by a prosthesis, bone cement can be injected into the position of the combined surface of the acetabular cup and the defect block through the hole of the defect block, the combined body of the defect block and the acetabular cup can be easily punched into the manufactured cavity due to no overturning risk during buckling, the implantation of the acetabular cup and the defect block is completed, and the stability of the operation is high.
The acetabulum bone defect block and the defect block in the invention refer to the same.
Furthermore, in order to further reduce acetabular bone grinding and improve the universality of the acetabular bone defect block, the acetabular bone defect block can be designed into different sizes and models, can be suitable for acetabular bone defects with larger areas, can also be suitable for local acetabular bone defects with smaller areas, can also be used for acetabular bone defects penetrating acetabular fossae, has stronger universality, and is preferably used for acetabular bone defects with smaller areas.
Preferably, the difference between the diameter of the ball with the outer arc surface (i.e. the outer surface 3) and the diameter of the ball with the inner arc surface (i.e. the inner surface 2) is not more than 4mm, such as 1mm, 2mm, 2.3mm, 3.5mm or 3.8 mm. Within this range, the difference in diameter ensures that the loss of bone mass at the location of the acetabular bone defect is minimized.
The distance between the inner surface 2 and the outer surface 3 of the acetabular bone defect block (i.e., the thickness of the defect block) is no greater than 20mm, such as 5mm, 8mm, 12mm, 15mm, 18mm, or 19.5 mm. Preferably, the thickness of the defect block body is gradually reduced from the center of the cambered surface to the edge. The design is convenient for filling the acetabulum defect, ensures that the outer surface is attached to the surface of the acetabulum defect, ensures that the inner surface is attached to the outer surface of the acetabular cup, and is convenient for the automatic adaptation of the acetabulum defect block and the outer surface of the acetabular cup.
As a preferred technical solution, the diameter range of the sphere where the outer arc surface (i.e. the outer surface 3) of the acetabular bone defect block is located is 4.0cm-7.0cm, such as 4.2cm, 4.5cm, 5.8cm, 6.0cm, 6.2cm or 6.7cm, and the like, and the diameter range can be adjusted by those skilled in the art as required.
Further, the edge of the inner surface and the edge of the outer surface may be directly connected, but the connection part is relatively sharp when the edges are directly connected, so as to facilitate the postoperative recovery of the patient, and therefore, the edge of the inner surface and the edge of the outer surface are connected through a transition section, which is preferably a circular arc section. The transition section functions to connect the edges of the inner and outer surfaces such that the edges of the inner surface smoothly transition to the edges of the outer surface. As will be appreciated by those skilled in the art, the size of the transition section should be significantly smaller than the size of the inner or outer surface, e.g., the area of the transition section is much smaller than the area of the inner or outer surface, e.g., the area of the transition section may be 1/100-1/10 of the area of the inner or outer surface, e.g., 1/10, 1/20, 1/50 or 1/80. As shown in fig. 1 to 3, the edge of the inner surface 2 and the edge of the outer surface 3 are connected through a circular arc section 5, so that the acetabular bone fracture implant is conveniently implanted into any acetabular bone defect position, interference to acetabular bone and acetabular cup is avoided, and postoperative recovery is facilitated. Preferably, the transition section has a thickness in the range of 1.5mm to 2.5 mm, such as 1.8mm, 2.0mm, 2.2mm or 2.4mm, etc.
Further, as shown in fig. 3, the defective block body 1 includes a body layer 7 and a junction layer 8 covering the outer surface 3 side of the body layer 7, the junction layer 8 is in contact with the acetabulum bone defect surface, the junction layer 8 is a biological porous surface or a coating surface, and is made of a material convenient for bone ingrowth and beneficial for biological bone ingrowth or bone ingrowth. The body layer 7 is made of material which can be implanted into human body. Preferably, the joining layer 8 is made of titanium alloy or hydroxyapatite or tantalum metal trabecular bone, and the body layer 7 is made of titanium alloy. Further, the thickness of the attachment layer 8 is 250 μm to 1500 μm, such as 280 μm, 300 μm, 500 μm, 800 μm, 1000 μm or 1200 μm, etc., in which range the biological bone ingrowth or bone ingrowth is facilitated.
Furthermore, through holes 6 extending from the inner surface 2 to the outer surface 3 are formed in the defect block body 1, the outer surface 3 of the defect block body 1 penetrates through at least one through hole 6 through a fastener to be fixedly connected with the acetabulum, and the through holes 6 are also used for injecting bone cement between the inner surface of the acetabulum bone defect block and the outer surface of the acetabulum cup. In order to fix the acetabular cup with the acetabular bone defect block and the acetabulum, a plurality of avoiding grooves 11 can be further arranged on the edge of the acetabular bone defect block, namely the acetabular bone defect block is starfish-shaped (as shown in fig. 8 to 9). The through-holes 6 may be provided as screw holes or unthreaded holes. When the acetabulum bone defect block is installed, an acetabulum screw of a standard acetabulum cup can be used for penetrating through the through hole 6 and the acetabulum to fix the defect block body 1 on the bone surface of the acetabulum bone defect position, and the acetabulum screw is adopted as a fastener, so that the types of spare and accessory parts can be reduced, the manufacturing cost is reduced, and the operation is simplified. Furthermore, the through hole 6 can adopt a counter bore, so that the head of the fastener is not higher than the surface of the inner surface 2, and the inner surface 2 is uniformly and firmly connected with the surface of the acetabular cup.
Furthermore, the through holes 6 are provided in a plurality, such as 2, 3, 5 or 7, which are uniformly distributed on the defect block body 1. The through holes 6 are provided with a plurality of holes, so that the acetabulum bone defect blocks can be conveniently fixed on the surface of the acetabulum bone by selecting the proper through holes 6 according to needs during operation; the through holes 6 are uniformly distributed on the defect block body 1, which brings convenience for implanting the acetabular bone defect block in an operation. Preferably, the number of the through holes 6 is 5; in addition, the through hole 6 facilitates the injection of bone cement between the inner surface of the acetabular bone defect block and the outer surface of the acetabular cup.
Preferably, the through hole 6 is formed in the center of the defect block body 1, and the through hole 6 is a screw hole, so that an implantation instrument can be connected conveniently, and the acetabulum bone defect block can be moved accurately. Further, when there are a plurality of through holes, it is preferable that 1 through hole is provided in the center of the acetabular bone defect block, and the remaining through holes are uniformly distributed around the centrally provided through hole.
Further, it is preferable that at least one of the through holes 6 is adapted to an acetabular cup mounting hole so as to fixedly connect the acetabular bone defect block with the acetabular cup by using a fastener through the acetabular cup mounting hole on the acetabular cup and the through hole 6.
Further, as shown in fig. 1 to 4, the inner surface 2 is provided with a bone cement fixing structure 4, so that the bone cement can be better combined when the bone cement is coated on the inner surface 2, the acetabular bone defect block can be better fixed with the acetabular cup, and the processing and manufacturing of the inner surface 2 in the defect block body 1 are simplified.
Further, as shown in fig. 1 to 4, the bone cement fixing structure 4 may be a bone cement groove, and preferably, the bone cement groove is uniformly distributed on the inner surface 2 at a plurality of positions with uniform depth. The bone cement grooves are arranged at a plurality of positions, so that the bonding area of the inner surface 2 and the bone cement is large, and the acetabular bone defect block is firmly combined with the acetabular cup; a plurality of bone cement grooves are uniformly distributed on the inner surface 2, so that the inner surface 2 is uniformly connected with bone cement; the depth of the bone cement groove is consistent, so that the processing of the bone cement groove is convenient. The bone cement groove can be in the form of a circular groove, a strip-shaped groove and the like. In order to evenly distribute the injected cement between the inner surface of the acetabular bone defect and the outer surface of the acetabular cup, the cement grooves may be interconnected by connecting grooves (the connecting grooves are not shown in fig. 1-4).
The embodiment also provides a hip joint replacement system, which comprises at least one acetabular bone defect block as described above, an acetabular cup and a femoral stem required by the operation, and the like, and the required implant or implant instrument can be selected according to the operation requirement.
Because the hip joint replacement system comprises the acetabular bone defect block in the technical scheme, the hip joint replacement system has the beneficial effects of the acetabular bone defect block, and the details are not repeated here. The at least one reference may include two (as shown in fig. 10 and 11), three or four equal acetabular bone defect blocks which are stacked or combined to meet the needs of different patients, and the number of stacked or combined reference is determined according to different needs. The overlapped or combined acetabular defect blocks can rotate randomly relative to each other, and are not limited to the angles shown in fig. 10 or fig. 11, and the actual rotation angle is subject to the surgical requirements.
A surgical method of using an acetabular bone defect block as described above, when only one acetabular defect block is applicable, the method comprising:
(1) checking the shape of the acetabular bone defect, and if the bone surface of the acetabular bone defect is not a spherical defect, treating the acetabular bone defect into a spherical defect; if the bone surface of the acetabular bone defect is a spherical defect, the bone surface does not need to be adjusted;
(2) selecting an acetabulum bone defect block or an acetabulum bone defect block trial model with a proper model, and implanting the acetabulum bone defect block or the acetabulum bone defect block trial model into the spherical defect;
(3) selecting an acetabulum cup or an acetabulum cup test mold with a proper model, driving the acetabulum cup or the acetabulum cup test mold into the acetabulum, and automatically adapting the acetabulum bone defect block to the acetabulum cup at the same time;
(4) fixing the acetabular bone defect block and the acetabular cup as required; or replacing the acetabulum bone defect block trial mold and/or the acetabulum cup trial mold with an acetabulum bone defect block and/or an acetabulum cup, and fixing the acetabulum bone defect block and the acetabulum cup.
The fixation is using fastener fixation and/or bone cement fixation.
When two acetabular bone defect blocks are used, the procedure is similar to the procedure described above, including:
(1) checking the shape of the acetabular bone defect, and if the bone surface of the acetabular bone defect is not a spherical defect, treating the acetabular bone defect into a spherical defect; if the bone surface of the acetabular bone defect is a spherical defect, the bone surface does not need to be adjusted;
(2) selecting a first acetabular bone defect block with a proper model, implanting the first acetabular bone defect block into a spherical defect, and fixing the first acetabular bone defect block by using a fastener;
(3) selecting a second acetabulum bone defect block or a second acetabulum bone defect block trial mold with a proper model to be matched with the first acetabulum bone defect block;
(4) the acetabular cup or the acetabular cup trial mold is driven into the acetabulum, and the second acetabular bone defect block or the second acetabular bone defect block trial mold is automatically adapted to the acetabular cup or the acetabular cup trial mold while the acetabular cup or the acetabular cup trial mold is driven into the acetabular cup or the acetabular cup trial mold;
(5) fixing the acetabular bone defect block and the acetabular cup as required; or replacing the acetabulum bone defect block trial mold and/or the acetabulum cup trial mold with an acetabulum bone defect block and/or an acetabulum cup, and fixing the acetabulum bone defect block and the acetabulum cup.
The fixation is using fastener fixation and/or bone cement fixation.
According to the acetabular bone defect block and the hip joint replacement system provided by the invention, as the arc surfaces of the inner surface and the outer surface in the used acetabular bone defect block are arranged and are respectively adaptive to the outer surface of the acetabular cup and the defect surface of the acetabular bone, the inner surface and the outer surface are independently surfaces which are more than half of the arc surfaces, the automatic adaptation of an acetabular bone defect block or an acetabular bone defect block trial mold and an acetabular cup or an acetabular cup trial mold can be ensured, the overturning risk is small during adaptation, the acetabular bone defect block and the hip joint replacement system can be stably fixed at the acetabular bone defect, compared with the manual marking adaptation of the traditional operation method, the operation and difficulty are greatly simplified, and the combination of the acetabular bone defect block and the fixation required by the combination are avoided; the acetabular bone defect block is particularly suitable for acetabular medial defects; in addition, the defect block and the acetabulum cup are convenient to implant into the acetabulum after being combined.
According to the acetabulum bone defect block and the hip joint replacement system provided by the invention, the acetabulum bone defect block is directly filled in the acetabulum bone defect position, so that not only can the stability of the acetabulum bone defect block be ensured, but also the stability of an implanted acetabulum cup can be ensured, namely the implanted acetabulum bone defect block can be more stably positioned in the acetabulum bone defect position, and the implanted acetabulum cup can be more stably positioned in an acetabulum socket.
According to the acetabulum bone defect block and the hip joint replacement system, the outer surface of the acetabulum bone defect block is matched with the surface of the acetabulum bone defect, the defect block with a proper model can be selected according to the size of the acetabulum bone defect, the universality is high, and the bone loss of a patient is small during operation; the difference in diameter between the extrados and intrados of the acetabular bone defect block is not greater than 4mm, so that bone mass loss is minimized.
According to the acetabular bone defect block and the hip joint replacement system provided by the invention, the edge of the inner surface and the edge of the outer surface can be connected through the transition section, so that the implantation of the acetabular bone defect block is facilitated without interfering with an acetabular bone or an acetabular cup.
According to the acetabulum bone defect block and the hip joint replacement system provided by the invention, the junction layer is in contact with the surface of the acetabulum bone defect, so that the growth of biological bones into or on the bone is facilitated.
According to the acetabulum bone defect block and the hip joint replacement system provided by the invention, the defect block body is provided with the through hole, so that the fastener can conveniently penetrate through the through hole to fixedly connect the acetabulum bone defect block and the acetabulum, and bone cement can be conveniently injected between the defect block and the acetabulum cup.
According to the acetabulum bone defect block and the hip joint replacement system, bone cement can be filled in the bone cement groove on the inner surface, so that the acetabulum bone defect block is combined and fastened with the acetabulum cup, and alignment and corresponding operation of a mounting hole of the acetabulum cup and a through hole arranged on the acetabulum bone defect block are avoided; the bone cement grooves are communicated through the connecting grooves, so that the bone cement is uniformly distributed.
The design of the avoidance groove in the acetabulum bone defect block and the hip joint replacement system is beneficial to avoiding the influence of the defect block on the fixation of the acetabulum cup.
Experiments prove that for hip joint replacement surgery, the acetabular bone replacement device has good use effect and strong universality for the cases of primary or revision acetabular bone defects, has stable performance from initial stability to long-term bone ingrowth, does not need to additionally increase implantation instruments, and is convenient to operate.

Claims (10)

1. The acetabulum bone defect block is characterized by comprising a defect block body (1), wherein the defect block body (1) comprises an inner surface (2) and an outer surface (3) which protrude towards the same direction, the inner surface (2) is adapted to the outer surface of an acetabulum cup, the outer surface (3) is adapted to the defect surface of the acetabulum bone, and the inner surface (2) and the outer surface (3) are surfaces which are more than half of a cambered surface independently.
2. The acetabular bone defect block according to claim 1, characterized in that the inner surface (2) and the outer surface (3) are independently circular arc surfaces, and the axes of the inner surface (2) and the outer surface (3) coincide.
3. The acetabular bone defect block of claim 2, wherein the difference between the diameter of the ball on the outer surface and the diameter of the ball on the inner surface is no greater than 4 mm;
preferably, the distance between the inner and outer surfaces is no greater than 20 mm.
4. The acetabular bone defect block according to claim 1, characterized in that the thickness of the defect block body (1) decreases gradually from the centre of the arc to the edge.
5. The acetabular bone defect block of claim 1, wherein the inner surface (2) and outer surface (3) edges are connected by a transition segment.
6. The acetabular bone defect block of claim 1, wherein the defect block body (1) comprises a body layer (7) and a junction layer (8) covering the outer surface (3) side of the body layer (7).
7. The acetabular bone defect block of claim 1, wherein the defect block body (1) is provided with through holes (6) extending from the inner surface (2) to the outer surface (3), and the defect block body (1) is fixedly connected with the acetabulum through at least one of the through holes (6) by fasteners.
8. The acetabular bone defect block according to claim 1, characterized in that a bone cement fixing structure (4) is provided on the inner surface (2); and/or the presence of a gas in the gas,
the edge of the defect block body is provided with a plurality of avoiding grooves (11).
9. The acetabular bone defect block according to claim 8, wherein the bone cement fixing structures (4) are bone cement grooves which are uniformly distributed on the inner surface and have a plurality of uniform depths;
preferably, the cement grooves are communicated through the connection grooves.
10. A hip replacement system comprising at least one acetabular bone defect block as claimed in any one of claims 1 to 9.
