背景

发明概述
本发明部分基于以下发现:测定实体瘤微环境TME的组成并对位于TME内的免疫细胞和癌细胞进行单细胞表型和功能分析是可能的。更具体地,本文所述的方法和组合物有助于免疫抑制表型以及可靶向的免疫调节(例如,检查点)受体及其配体在TME内的细胞表达的定量表征。该方法和组合物可用于确定患有实体瘤的受试者是否可能响应特定的免疫调节剂。该方法和组合物还通过帮助选择适用于治疗受试者实体瘤的免疫调节剂来促进给定受试者中实体瘤的治疗。

I.细胞类型
应理解TME包括本文所述的肿瘤细胞和免疫细胞。
(a)肿瘤细胞
术语“肿瘤细胞”在本文中可与“癌细胞”互换使用。可以使用本文所述的方法和组合物探查的肿瘤细胞类型包括上皮衍生的细胞、内皮衍生的细胞和间充质衍生的细胞。

II.样品加工和细胞计数
实体瘤样品可以从受试者获得并通过本领域已知的任何方式加工成细胞的单细胞悬液。如本文所用的,“单细胞悬液”是一种或多种细胞在液体样品中的悬浮液,其中细胞主要呈单细胞形式而不是细胞簇或聚集体的形式。在某些实施方案中,单细胞代表细胞悬液中60%、70%、80%、90%或95%的细胞。
在某些实施方案中,通过人工和/或酶消化实体瘤。例如,可以将实体瘤样品切成较小的块并使用胰蛋白酶、胶原蛋白酶、DNAse、分散酶和/或透明质酸酶进行酶消化。在某些实施方案中,将消化的组织进行过滤,以除去较大的、未消化的块。
除上述之外或替代于上述,可以使用自动组织匀浆器,如gentOctoDissociator(Miltenyi Biotec GmbH,Bergish Gladbach,德国)进行组织解离。可以过滤细胞以去除未消化的组织,例如,使用70μM滤器。
所得到的单细胞悬液可包含存在于肿瘤微环境中的所有细胞类型,包括上皮细胞、内皮细胞、间充质细胞、基质细胞、肿瘤/癌细胞和免疫细胞。
解离后,可以将细胞洗涤、沉淀、重悬和/或计数。可以使用本领域已知的任何方式对细胞进行计数,例如使用手动细胞计数器或自动细胞计数器。例如,对于实体瘤,可以使用自动细胞计数器获得有核细胞计数,所述细胞计数器使用例如明场成像和/或荧光成像(例如双荧光成像)。示例性自动细胞计数器包括Nexcelom Cellometer 2000(Nexcelom,Lawrence,MA)、Countess II FL自动细胞计数器(ThermoFisher,Waltham,MA)和自动细胞计数器(BioRad,Hercules,CA)。对于全血、骨髓、PBMC或BMMC,可以使用自动细胞计数器,如coulter计数器,例如Beckman Coulter Act2 Diff(Beckman Coulter,Inc.,Brea,CA)。

III.细胞分析
为了分析最初与TME一起处置的细胞,将一旦转化为单细胞悬液的细胞与多种(例如,5、6、7、8、9、10、11、20、13、14、15、16、17、18、19、20、25、30或更多种)结合以选择细胞表面标志物的标记试剂,从而确定细胞是癌细胞还是免疫细胞或亚型,和/或此类细胞或亚型是否表达例如IMR、IMR-L或细胞激活标志物。
具体地,可以使用活体染料(例如,胺反应性染料Alexa750)对细胞进行涂布和染色以区分活细胞与死细胞。细胞可以在染色缓冲液(例如,1X PBS+0.5%BSA)中洗涤。可以使用例如Biotek ELx450深孔板清洗机,使清洗步骤自动化。可以使用本领域的标准方法固定和渗透细胞,例如使用来自eBioscience(ThermoFisher,Waltham,MA)的FOXP3/Transcription Staining Buffer Kit。然后可以在染色缓冲液(例如,1X PBS+0.5%BSA)中洗涤细胞并用一种或多种标记试剂染色来检测一种或多种细胞标志物。染色细胞的方法取决于所用的标记试剂。
(a)细胞表面标志物
本文公开的方法可以用于检测任何细胞表面标志物,如癌细胞标志物、激活标志物或IMR或IMR-L标志物。
细胞类型标志物是存在于特定细胞类型(例如,癌细胞或免疫细胞)之中或之上的蛋白质或其他分子。在某些实施方案中,特定的癌细胞标志物的存在可以指示癌症的类型。在某些实施方案中,特定的癌细胞标志物的存在提供了用于癌症治疗的靶标。示例性癌细胞标志物包括CD44、CD47、CD49f、CD271、CD326、细胞角蛋白(胞内)、E-钙粘蛋白和/或波形蛋白。在某些实施方案中,特定的免疫细胞标志物的存在将免疫细胞鉴定为特定的免疫细胞类型或亚型。示例性免疫细胞标志物提供于以下的表1中。

(e)细胞计量术
(i)流式细胞术
在某些实施方案中,使用流式细胞术分析标记的细胞。一旦使用例如本文所述的染色程序获得标记的细胞,通过流式细胞术分析标记的细胞。可用于本文方法的示例性流式细胞仪包括例如能够同时测量多种(例如,16种)荧光染料的Attune NxT流式细胞仪(ThermoFisher,Waltham,MA)、CytoFLEX流式细胞仪(Beckman Coulter,Indianapolis,IN)、FACSVerse或FACSCanto II流式细胞仪(Becton Dickinson,San Jose,CA)或Aurora流式细胞仪(Cytek,Oakland,CA)。可以使用例如流式细胞仪的标准设置来分析来自流式细胞仪的数据捕获。
在某些实施方案中,在获取流式细胞术数据之前,使用来自Qognit,Inc.(Qognit,San Carlos,CA)的Ryvett软件将诸如样品ID、标记试剂以及流式细胞仪仪器设置和采集参数之类的信息并入板布局文件(plate layout file)中。这个板布局文件可以在采集之前导入到流式细胞术软件中。流式细胞术数据生成后,可以使用所需的门控程序在Ryvett软件中对流式细胞术标准(FCS)文件进行分析和门控。在手动审阅自动门控后,可以使用预定义的标准和数据提取程序将原始数据和计算出的指标导出到CSV文件中。
在某些实施方案中,流式细胞术数据可以使用WinList(Software House,Topsham,ME)、Ryvett(Qognit,Redwood City,CA)、FlowJo(FloJo LLC,Ashland,OR)或Kaluza(Beckman Coulter,Indianapolis,IN)中的门控进行分析。
使用这种软件,可以探查TME中的细胞群,以确定TME中存在什么癌细胞和免疫细胞、它们的状态(例如,非活化、活化的或耗竭的),以及细胞是否表达一种或多种IMR或IMR-L。

在某些实施方案中,可以使用互信息的条件密度重采样估计(DREMI)分析质谱细胞术数据,它任选可以与条件密度重缩放可视化(DREVI)结合使用(Krishnaswamy等(2014)SCIENCE 346(6213):1250689)。
因此,与流式细胞术一样,质谱细胞术可以用于探查TME中的细胞群,以确定TME中存在哪些癌细胞和免疫细胞、它们的状态(例如,非活化、活化的或耗竭的),以及细胞是否表达一种或多种IMR或IMR-L。此外,质谱细胞术可用于执行单细胞基因组测序,以揭示例如单细胞(例如免疫细胞或癌细胞)中的突变(例如,体细胞突变)。
(iii)图像细胞术

(iv)单细胞技术(SCT)
根据本文公开的方法也可以使用单细胞技术(SCT)来评估TME中的单个细胞以及它们在不同条件下(例如,在存在免疫调节和/或癌症药物的情况下)与其他细胞的相互作用。来自TME的单细胞可以使用多种不同的方法进行分离和操作,包括基于流体的、基于物理的、电场驱动的(例如,介电泳(DEP)、光电镊子(OET)和光学技术,如光镊。(参见,例如,Skelley等(2009)NAT.METHODS 6:147-152;Thieleche等(1999)IEEEENG.MED.BIOL.MAG.18:48-52;Taff等(2005)ANAL CHEM.77:7976-7983;Juan等(2011)NAT.PHOTONICS 5:349-356;和Mirsaidov等(2008)LAB CHIP 8:2174-2181。
在某些实施方案中,使用微流控芯片进行SCT。例如,SCT可以使用基于流体动力学的微流控芯片进行。在其他实施方案中,介电泳数字分选方法使用微流控芯片中的半导体控制电极阵列以在介电泳(DEP)笼中捕获单细胞。

V.免疫调节剂
上述系统还可用于确定从TME分离的细胞是否响应单独的或与抗癌药物组合的免疫调节剂的添加。
适用于本文中的免疫调节剂包括能够调节免疫细胞的任何物质,例如,通过激活免疫系统来杀灭肿瘤细胞或通过从肿瘤细胞除去免疫细胞抑制细胞。在某些实施方案中,与标记试剂组合前,将细胞暴露于免疫调节剂。
在某些实施方案中,免疫调节剂是检查点抑制剂。检查点抑制剂可以例如选自PD-1拮抗剂、PD-L1拮抗剂、CTLA-4拮抗剂、腺苷A2A受体拮抗剂、B7-H3拮抗剂、B7-H4拮抗剂、BTLA拮抗剂、KIR拮抗剂、LAG3拮抗剂、TIM-3拮抗剂、VISTA拮抗剂或TIGIT拮抗剂。

IX.药物组合物和免疫调节剂的给药
对于治疗用途,免疫调节剂优选与药学上可接受的载体组合。如本文所用的,术语“药学上可接受的”是指在合理医学判断范围内,适用于与人和动物组织接触而无过度毒性、刺激、过敏反应或其他问题或并发症,与合理的收益/风险比相称的那些化合物、材料、组合物和/或剂型。

在某些实施方案中,药物组合物可以含有持续递送或受控递送制剂。用于配制持续递送或受控递送方式(如脂质体载体、生物可侵蚀微粒或多孔珠和贮库注射剂)的技术也是本领域技术人员已知的。缓释制剂可以包括例如多孔聚合物微粒或半渗透聚合物基质,其为成形制品形式,例如薄膜或微胶囊。缓释基质可以包括聚酯、水凝胶、聚乳酸、L-谷氨酸和γ-乙基-L-谷氨酸的共聚物、聚(2-羟基乙基-乙基丙烯酸酯)、乙烯醋酸乙烯酯或聚-D(-)-3-羟基丁酸。缓释组合物还可以包括可以通过本领域已知的几种方法中的任一种制备的脂质体。
含有本文公开的免疫调节剂的药物组合物可以以剂量单位形式存在并且可以通过任何合适的方法制备。药物组合物应配制成与其预期给药途径相容。给药途径的实例是静脉内(IV)、皮内、吸入、经皮、局部、经粘膜、鞘内和直肠给药。优选的给药途径是IV输注。有用的制剂可以通过制药领域已知的方法来制备。例如,参见Remington’sPharmaceutical Sciences,第18版(Mack Publishing Company,1990年)。适用于肠胃外给药的制剂成分包括无菌稀释剂,如注射用水、盐水溶液、不挥发油、聚乙二醇、甘油、丙二醇或其他合成溶剂;抗细菌剂,如苯甲醇或对羟基苯甲酸甲酯;抗氧化剂,如抗坏血酸或亚硫酸氢钠;螯合剂,如EDTA;缓冲剂,例,如醋酸盐、柠檬酸盐或磷酸盐;以及用于调节张力的物质,如氯化钠或葡萄糖。
对于静脉内给药,合适的载体包括生理盐水、抑菌水、Cremophor ELTM(BASF,Parsippany,NJ)或磷酸盐缓冲盐水(PBS)。载体在制造和储存条件下应当是稳定的,并且应当针对微生物进行保存。载体可以是含有例如水、乙醇、多元醇(例如甘油、丙二醇和液体聚乙二醇)及其合适混合物的溶剂或分散介质。
药物制剂优选是无菌的。灭菌可以通过任何合适的方法来完成,例如通过无菌过滤膜过滤。在组合物被冻干的情况中,可以在冻干和重构之前或之后进行过滤灭菌。
本文所述的组合物可以局部或全身给药。给药通常是胃肠外给药。在优选实施方案中,药物组合物皮下给药,且在至更优选的实施方案中,静脉给药。胃肠外给药的制剂包括无菌水溶液或非水溶液、悬浮液和乳液。
通常,活性成分(例如免疫调节剂)的治疗有效量在0.1mg/kg至100mg/kg的范围内,例如1mg/kg至100mg/kg、1mg/kg至10mg/kg。给药量将取决于变量,如待治疗的疾病或适应症的类型和程度、患者的整体健康状况、抗体的体内效力、药物制剂和给药途径。初始剂量可以增加到超过上限以快速达到所需的血液水平或组织水平。或者,初始剂量可以小于最佳剂量,并且可以在治疗过程中逐渐增加日剂量。人剂量可以优化,例如,在设计为从0.5mg/kg运行到20mg/kg的常规I期剂量递增研究中进行优化。给药频率可能会有所不同,这取决于给药途径、剂量、免疫调节剂的血清半衰期和所治疗的疾病等因素。示例性的给药频率是每天一次、每周一次和每两周一次。优选的给药途径是胃肠外,例如静脉内输注。在某些实施方案中,免疫调节剂是冻干的,并且随后在施用时在缓冲盐水中重构。
VI.疗法
一旦为给定的受试者选择了合适的免疫调节剂,无论是单独的还是与抗癌药物组合的,都可以根据常规的医疗保健实践来治疗该受试者。特别地,可以给受试者施用有效量的免疫调节剂,单独或与有效量的另一种抗癌药物组合。如本文所用的,术语“有效量”是指足以产生有益或所需结果的活性剂(例如,免疫调节剂)的量。有效量可以在一次或多次给药、应用或剂量中给药并且不旨在限于特定的制剂或给药途径。
如本文使用的,“治疗(treat)”、“治疗(treating)”和“治疗(treatment)”是指治疗受试者(例如人)的疾病。这包括:(a)抑制疾病,即阻止其发展;(b)缓解疾病,即引起疾病状态的消退。如本文所用的,术语“受试者”和“患者”是指将通过本文所述的方法和组合物治疗的生物体。此类生物优选包括但不限于哺乳动物(例如,鼠、猿、马、牛、猪、犬、猫等),且更优选包括人。
可以通过本文所述的方法治疗的癌症的实例描述于部分I中。在某些实施方案中,癌症是转移性癌。在某些实施方案中,癌症是难治性癌。
如上所述,预期免疫调节剂可以单独或与另一种癌症药物或治疗剂联合施用。本文所用的术语“联合”被理解为是指在受试者患有疾病的过程中向受试者递送两种(或更多)不同的治疗,使得治疗对患者的作用在某个时间点重叠。在某些实施方案中,当第二种治疗的递送开始时,一种治疗的递送仍在发生,从而在给药方面存在重叠。这有时在本文中被称为“同时”或“并发递送”。在其他实施方案中,一种治疗的递送在另一种治疗的递送开始之前结束。在任一情况的某些实施方案中,由于联合给药,治疗更有效。例如,与第一种治疗不存在下施用第二种治疗看到的相比,第二种治疗更有效,例如,使用较少的第二种治疗看到了相等的效果,或第二次治疗在更大程度上减轻了症状,或使用第一种治疗看到类似的情况。在某些实施方案中,递送使得与病症相关的症状或其他参数的减少大于在不存在另一种治疗的情况下递送一种治疗所观察到的。两种治疗的效果可以是部分相加的、完全相加的或大于相加的。递送可以使得在递送第二种治疗时仍可检测到递送的第一种治疗的效果。
在某些实施方案中,免疫调节剂与一种或多种另外的疗法(例如手术、放疗)或另一种治疗制剂的给药联合施用。在某些实施方案中,另外的疗法可以包括化疗,例如细胞毒剂。在某些实施方案中,另外的治疗可以包括靶向治疗,例如,酪氨酸激酶抑制剂、蛋白酶体抑制剂或蛋白酶抑制剂。在某些实施方案中,其他的疗法可以包括抗炎、抗血管生成、抗纤维化或抗增殖化合物,例如类固醇、生物免疫调节剂、单克隆抗体、抗体片段、适体、siRNA、反义分子、融合蛋白、细胞因子、细胞因子受体、支气管扩张剂、他汀、抗炎剂(例如甲氨蝶呤)或NSAID。在某些实施方案中,另外的疗法可以包括不同类别的疗法的组合。

实施例1-测定肿瘤微环境的组成
这个实施例显示了要求保护的方法利用表面和胞内标志物确定来自肺、乳腺和肾实体瘤的肿瘤微环境的组成的能力。显示升高水平的CD4+CD25hiFoxP+Treg的肿瘤表明具有肿瘤扩散的免疫抑制特征,因此预后和客观响应较差,这有助于为患者制定治疗策略。
在这个实施例中,获得肺、乳腺和肾实体瘤样品,并在维持在2-8℃的Therapak受控速率托运装置中的Miltenyi组织运输缓冲液中运送到Pierian Biosciences。收到后,使用无菌一次性手术刀将肿瘤切成2-3mm碎片并放入肿瘤解离缓冲液(RPMI1640+青霉素/链霉素)中。然后使用来自Miltenyi的人肿瘤解离试剂盒的酶和Miltenyi的带有加热器的gentOcto分离器,将肿瘤碎片机械解离成单细胞悬液。随后将单细胞悬液通过70μM滤器过滤并计数。然后将细胞悬液离心并以所需的细胞浓度重悬于RPMI1640+10%FBS或1XPBS+0.5%BSA中。

实施例2-通过基础和诱导状态下靶标的表面和胞内染色评估来自分离的肿瘤的细胞的功能能力
这个实施例显示了要求保护的方法能通过利用表面和胞内标志物测量来自乳腺、肺和肾实体瘤的已鉴定免疫细胞中功能读数的基础和诱导水平两者来确定功能能力。

IFNγ和TNFα是指示具有更大免疫应答潜力的“发炎”肿瘤的细胞因子,在初始免疫疗法治疗和随后的随访中,升高水平的IFNγ和TNFα分别与初始和持久应答相关。颗粒酶B是在CD8+细胞中发现的效应分子,并且是颗粒酶B-穿孔素复合物的一部分,CD8+细胞利用它来杀灭靶肿瘤细胞。降低水平的颗粒酶B与降低的CD8+细胞毒性潜力有关,并且是T细胞耗竭的指标。确定肿瘤中表达颗粒酶B的CD8+细胞的百分比和颗粒酶B表达的定量水平对于预测对免疫疗法的响应和为患者制定治疗策略是有用的。
实施例3-评估来自实体瘤的免疫检查点和耗竭标志物及其同源配体
在这个实施例中,测量了TIL和肿瘤上皮细胞上的免疫检查点/耗竭标志物及其同源配体的表达水平。免疫疗法治疗目前依赖于肿瘤细胞上IMR配体PD-L1表达的单一IHC测量作为治疗指导,但这并不能充分识别肿瘤微环境内任何其他细胞成分上的IMR和IMR-L表达。本文所述的整体方法可以改进患者分层和治疗策略。

如图7中所示的,肿瘤微环境内检测到CD45+白细胞、CD326+上皮细胞、CD14+单核细胞/巨噬细胞以及CD4+和CD8+T细胞的存在。抑制检查点受体表达的实例显示于图8中,其中在肿瘤微环境中的细胞子集上检测到抑制性IMR TIGIT。具体地,在CD4+、CD8+和CD14+白细胞上检测到TIGIT,但在CD326+肿瘤细胞上未检测到。
测量了IMR/耗竭标志物及其配体(CD279/CD274[PD1/PD-L1]、TIGIT/CD112-CD155、CD366[TIM-3]/Galectin-9、CD172a[SIRPa]/CD47、CD73)的水平,以概括TIL的功能状态。如图9-11中所示的,观察到IMR/IMR-L阳性TIL与阴性TIL以及IMR/IMR-L阳性肿瘤细胞与阴性肿瘤细胞的可靠分离,具有明显的异质性,跨越了细胞和肿瘤类型(乳腺、肺和肾)。这包括TIL上的几种IMR-L以及上皮细胞和基质细胞上的IMR的表达升高,表明调节检查点相互作用的肿瘤-和TIL-内在机制。

实施例4-用免疫调节剂治疗患者
