背景技术
由于癌症的位置、疾病的程度和与它们的手术相关的副发病变(morbidities),存在几种被认为是手术切除的不良候选者的皮肤恶性肿瘤。这些皮肤恶性肿瘤包括乳腺癌皮肤转移、乳腺外佩吉特病和皮肤黑色素瘤(尤其是原位病变和恶性雀斑样痣)1-3。全身性治疗和放射已被用于治疗这些不可切除的癌症,具有仅适度的益处和明显的副作用1-3。
发明内容
如本文所示,一种或多种T细胞/胸腺基质淋巴细胞生成素(TSLP)激活剂(即卡泊三醇、视黄酸)与一种或多种先天性免疫细胞激活剂(即咪喹莫特、STING激动剂)的组合导致协同作用,针对癌症、例如包括非角质形成细胞皮肤恶性肿瘤(例如,乳腺外佩吉特病和乳腺癌转移)以及黑色素瘤的皮肤的癌症提供稳健的(robust)抗肿瘤免疫激活。特别是,与卡泊三醇、视黄酸、咪喹莫特或DMXAA单一疗法相比,局部卡泊三醇加咪喹莫特、视黄酸加咪喹莫特和卡泊三醇加DMXAA的治疗在阻断乳腺癌和黑色素瘤生长方面显著地更有效。此外,短期的局部卡泊三醇加咪喹莫特治疗对于患者的乳腺外佩吉特病的治疗是有效的。因此,本文提供了先天性免疫激活剂和通过胸腺基质淋巴细胞生成素(TSLP)诱导直接激活T细胞的药剂的组合,以及它们在增殖性病症、特别是皮肤恶性肿瘤中的用途。

本研究结果表明,通过TSLP诱导激活T细胞的药剂(例如卡泊三醇或视黄酸)与先天性免疫细胞激活剂(例如咪喹莫特、TLR激动剂、STING激动剂或溶瘤病毒)的组合可以针对癌症产生协同功效。这些组合导致针对恶性肿瘤的适应性免疫(adaptive immunity)的最佳激活,所述恶性肿瘤包括乳腺癌皮肤病变、乳腺外佩吉特病、黑色素瘤和目前正在用咪喹莫特治疗的其它病变。这些发现还提供通过将骨髓细胞中的固有信号传导与借助TSLP的直接T细胞刺激组合以诱导针对癌症的稳健免疫力的将冷(非免疫原性)癌症转变为热(发炎(inflamed))癌症的治疗模式。这些组合疗法通过诱导借助树突细胞和巨噬细胞的最佳抗原呈递(咪喹莫特/DMXAA的作用)和在检测肿瘤相关抗原后诱导完全的T细胞激活(卡泊三醇/视黄酸的作用)来破坏癌细胞(靶标)。靶向信号1、信号2和信号3用于完全的T细胞激活的这些组合导致稳健的抗肿瘤适应性免疫和有效的癌症治疗。尽管可以将这些组合开发为通过将它们递送至肿瘤中来治疗体内癌症,但是以局部组合递送这些药剂的能力产生了有效治疗皮肤恶性肿瘤如黑色素瘤、乳腺外佩吉特病和来自乳腺和其它体内癌症的皮肤转移的独特机会。在这些恶性肿瘤中,乳腺外佩吉特病是一种罕见的携带顶泌腺的皮肤的腺癌(具有与乳腺/癌症相似的发育起源)2。众所周知,这种癌症是难以管理的2。在这些患者中进行的手术具有30%的高复发率,并且由于病变的位置大多在腹股沟和生殖器而导致显著的发病率13,14。此外,关于化学疗法和辐射对这种癌症的功效的数据有限15。因此,本发明的组合可以用作乳腺外佩吉特病的有效治疗方法,这会彻底改变患有该衰弱性疾病的患者的护理。
治疗方法
本文所述的方法包括用于治疗增殖性病症、特别是皮肤恶性肿瘤的方法。通常,该方法包括将治疗有效量的本文所述的联合疗法给予至需要此类治疗或已确定需要此类治疗的受试者,所述联合疗法包括通过TSLP诱导激活T细胞的药剂(例如卡泊三醇或视黄酸)和先天性免疫细胞激活剂(例如咪喹莫特、TLR激动剂、STING激动剂或溶瘤病毒)。
一般而言,本发明的组合物和方法不包括使用细胞毒性剂,例如,如US2017/0246299中所述。
如该上下文中所用,“治疗”是指改善增殖性病症的至少一种症状。给予治疗有效量的本文所述的用于治疗增殖性病症的化合物将导致降低的病灶或肿瘤生长速率、病灶或肿瘤消退、和/或减小的病灶或肿瘤尺寸,并且可以增加寿命、降低复发的风险、和/或减少对手术干预的需要。
本文所述的组合在例如通过产生主动免疫来治疗与异常细胞凋亡或分化过程相关的病症例如细胞增殖性病症或细胞分化性病症(例如癌症)中是有用的。细胞增殖性病症和/或分化性病症的实例包括癌症,例如癌(carcinoma)、肉瘤、转移性病症或造血肿瘤性疾病(例如白血病)。转移性肿瘤可以源自多种原发性肿瘤类型,包括但不限于前列腺、结肠、肺、乳腺和肝来源的那些肿瘤。
如本文所用,术语“癌症”、“过度增殖(hyperproliferative)”和“肿瘤性(neoplastic)”是指具有自主生长能力的细胞,即,以快速增殖的细胞生长为特征的异常状态或状况。过度增殖疾病状态和肿瘤性疾病状态可以分类为病理性的,即表征或构成疾病状态,或者可以分类为非病理性的,即偏离正常状态但不与疾病状态相关。该术语旨在包括所有类型的癌性生长或致癌过程、转移性组织或恶性转化的细胞、组织或器官,而与组织病理学类型或浸润阶段无关。“病理性过度增殖”细胞出现在以恶性肿瘤生长为特征的疾病状态中。非病理性过度增殖细胞的实例包括与伤口修复相关的细胞增殖。
术语“癌症(cancer)”或“瘤(neoplasms)”包括各种器官系统的恶性肿瘤,例如影响肺、乳腺、甲状腺、淋巴、胃肠道和泌尿生殖道的恶性肿瘤,以及包括例如大多数结肠癌、肾细胞癌、前列腺癌和/或睾丸肿瘤、肺非小细胞癌、小肠癌和食管癌等恶性肿瘤的腺癌。

在一些实施方案中,该病症为皮肤非角质形成细胞癌,例如乳腺外佩吉特病、乳腺癌皮肤转移和皮肤黑色素瘤(例如原位病变和恶性雀斑样痣)。
在一些实施方案中,该病症不是角质形成细胞癌,即不是基底细胞癌或皮肤鳞状细胞癌(SCC)。
增殖性病症的其它实例包括造血肿瘤性病症。如本文所用,术语“造血肿瘤性病症”包括涉及造血起源的增生细胞/肿瘤细胞的疾病,例如由骨髓系、淋巴系或红细胞系、或者其前体细胞引起的疾病。优选地,该疾病是皮肤T细胞淋巴瘤(CTCL)。
在一些实施方案中,增殖性病症包括前癌性皮肤病症。该皮肤病症可涉及真皮层、表皮层或下皮层中的细胞或一组细胞或层的异常活性,或者真皮-表皮交界处的异常。
胸腺基质淋巴细胞生成素(TSLP)诱导剂
本组合包括使用TSLP诱导剂。如本文所用,“TSLP诱导剂”是能够诱导胸腺基质淋巴细胞生成素(TSLP)的任意化合物。TSLP是源于上皮的细胞因子,属于白介素7(IL-7)细胞因子家族。许多这样的药剂是本领域已知的。确定化合物是否诱导TSLP的方法是本领域已知的。例如,可以如US2017/0246299(以其整体并入本文)中更详细地描述的来测量TSLP核酸表达、TSLP蛋白表达或TSLP活性。TSLP诱导剂的非限制性实例包括视黄酸、维生素D类似物、聚肌苷酸-聚胞苷酸(poly(I:C)和其它TLR3配体)、FSL-1(和其它TLR2-TLR6配体)、鞭毛蛋白(和其它TLR配体)、β2-肾上腺素受体激动剂、cAMP升高剂(例如佛司可林(Forskolin))、脂肪酸(庚酸、辛酸、壬酸和癸酸)、二甲苯、1,2,4-三甲基苯、12-O-十四烷酰基佛波醇-13-乙酸酯(TPA、十四烷酰基佛波醇乙酸酯(tetradecanoylphorbol acetate)、十四烷酰基佛波醇乙酸酯(tetradecanoyl phorbol acetate)和佛波醇12-肉豆蔻酸酯13-乙酸酯(PMA))、邻苯二甲酸二丁酯(DBP)和邻苯二甲酸二异壬酯(DINP)。
视黄酸是维生素A(视黄醇)的代谢产物和TSLP的诱导剂。它已知为维甲酸、维生素A酸、ATRA和全反式视黄酸。
在一些实施方案中,TSLP诱导剂可以为维生素D或其类似物。维生素D是指一类脂溶性开环甾体(secosteroids)。除了典型的四个甾环的两个B环碳原子在开环甾体中没有连接而在甾体中连接以外,开环甾体在结构上与甾体非常相似。实例包括活性维生素D(或骨化三醇)和维生素D3,维生素D3也已知为胆钙化醇(当光能被前体分子7-脱氢胆固醇吸收时,在动物皮肤中生成的维生素D的一种形式)。

在一些实施方案中,维生素D类似物为卡泊三醇(也已知为钙泊三醇或calcitrene)。卡泊三醇是骨化三醇(维生素D的一种形式)的合成衍生物。它被用于牛皮癣的治疗。对于卡泊三醇局部用软膏剂,Leo Pharma于1993年获得FDA批准。仿制形式(GlenMark和Tolmar)已获批准。源于表皮的细胞因子胸腺基质淋巴细胞生成素(TSLP)的诱导可防止皮肤癌和乳腺癌的发展7-9。这种保护作用由直接响应于TSLP的T细胞介导7,8。卡泊三醇(一种低钙血维生素D类似物和TSLP的诱导剂)是一种已经用于乳腺癌治疗的免疫疗法8。
先天性免疫激活剂
本发明的组合还包括使用激活抗原呈递细胞的先天性免疫激活剂。先天性免疫系统的激活剂包括toll样受体(TLR)的激动剂;STING激动剂;或溶瘤病毒。

如本文所用,“STING激动剂”是合成的环二核苷酸(CDN)和干扰素基因刺激蛋白(STING)的其它激动剂,其可以激活细胞内的cGAS-STING胞质DNA识别通路(cGAS-STINGcytosolic DNA sensing pathway),导致如树突细胞等先天性免疫细胞的激活(例如DMXAA、MK-1454和ADU-S100)。DMXAA(5,6-二甲基呫吨酮-4-乙酸、ASA404或瓦帝莫泽(Vadimezan))是一种STING激动剂,用作血管破坏剂。在一些实施方案中,在使用STING激动剂的情况下,例如如本领域已知的,例如通过病灶内/肿瘤内注射来局部给药。

药物组合物和给药方法
本文所述的方法包括使用包含通过TSLP诱导激活T细胞的药剂和先天性免疫细胞激活剂中的一者或二者作为活性成分的药物组合物。该方法可以包括使用包含两种药剂的单一组合物、或者各自包含一种药剂的两种组合物。在一些实施方案中,这些是仅有的活性剂,即该组合物或方法中不包含其它活性剂。
药物组合物通常包含药学上可接受的载体。如本文所用,术语“药学上可接受的载体”包括与药物给药相容的盐水、溶剂、分散介质、包衣、抗细菌剂和抗真菌剂、等渗剂和吸收延迟剂等。也可以将补充的活性化合物引入组合物中;然而,在一些实施方案中,不使用其它活性化合物。
本发明的组合物和方法不包括使用细胞毒性剂,例如如US2017/0246299中所述,和/或不包括使用抗炎药,例如如双氯芬酸等NSAIDS,和/或不包括使用甾体化合物,例如氢化可的松戊酸酯。
通常将药物组合物配制成与其预期的给药途径相容。给药途径的实例包括肠胃外,例如静脉内、皮内、病灶内/肿瘤内、皮下;经皮或局部;经粘膜;或者直肠或阴道给药。

为了通过吸入给药,化合物可以以气雾喷雾剂的形式从含有合适的抛射剂(例如,如二氧化碳等气体)的加压容器或分配器、或者喷雾器递送。此类方法包括在美国专利No.6,468,798中描述的那些。
如本文所述的治疗化合物的全身给药也可以通过经粘膜或经皮方式进行。对于经粘膜或经皮给药,在制剂中使用适合于要渗透的屏障的渗透剂。此类渗透剂在本领域中通常是已知的,并且包括例如用于经粘膜给药的去污剂、胆汁盐和夫西地酸衍生物。经粘膜给药可以通过使用鼻喷雾剂或栓剂来完成。对于局部或经皮给药,将活性化合物配制成本领域通常已知的软膏剂、油膏剂、凝胶剂或乳膏剂。

该组合物还可以以栓剂(例如,用常规栓剂基质如可可脂和其它甘油酯)或保留灌肠剂的形式制备,用于直肠或阴道递送。此类栓剂可以特别用于治疗与生殖道或胃肠道中存在的病变或肿瘤相关的病况。
药物组合物可以与给药说明书一起包括在容器、包装或分配器中。
包含通过TSLP诱导激活T细胞的药剂和先天性免疫细胞激活剂中的一者或二者、例如咪喹莫特和/或卡泊三醇和/或视黄酸的局部用组合物适合在一定浓度范围内安全且有效地使用。因此,本文所述的组合物中的局部活性剂的量在5μg/mL和2000μg/mL之间,例如25μg/mL、100μg/mL、500μg/mL、1000μg/mL、1500μg/mL或2000μg/mL。在具体的实施方案中,组合使用卡泊三醇(0.005%μg/mL)和约5%μg/mL至约3.75%μg/mL的咪喹莫特。在其它具体的实施方案中,以乳膏剂或凝胶剂形式以0.01%、0.025%、0.04%、0.05%或0.1%的剂量局部给予视黄酸。用于该用途的剂量时间表、即给药方案将取决于多种因素,包括疾病或病况的阶段、疾病或病况的严重程度、患者健康的总体状态、患者的身体状况和年龄等。至少,应当持续给药至少1个月。
局部用组合物还应当适合在一定pH范围内安全且有效地使用。因此,本文所述的局部用组合物的pH在6.0和7.5之间。
在一些实施方案中,本文所述的制剂适合于局部给药。本文所述的方法理想地适合于增加受试者的皮肤区域内的表皮、或表皮和真皮中至少一种药剂的量。本文所述的局部用组合物可以以适合进入血流的剂量给药。因此,可以在直接进入血流的全身给药之前、或与其同时将该药剂作为佐剂局部给药。
该制剂可以方便地以单位剂型存在并且可以通过药学领域中众所周知的任意方法来制备。可以将制剂与适合制造的无毒的药学上可接受的赋形剂混合。制剂可以包含一种或多种稀释剂、乳化剂、防腐剂、缓冲剂、赋形剂等,并且可以以适合于向患者局部给药的如下形式提供:液体、软膏剂、糊剂、乳剂、喷雾剂、乳膏剂、洗剂、浆剂(slurries)、混悬剂、泡沫剂(foams)、控释制剂、凝胶剂、贴剂、植入剂、水油双层组合物、水油粉三层组合物、精华液、散剂、毛滋剂(mousses)、水凝胶剂和一次性涂抹器等。
本文所述的制剂可以包含用于该组合物的皮肤病学上可接受的载体(本文中也简称为“载体”)。如本文所用的,短语“皮肤病学上可接受的载体”是指该载体适合局部施用于头发/头皮,具有良好的美学特性,与组合物中的ETA相容,并且不会引起任何不合理的安全性或毒性问题。选择合适的载体以得到期望的产品形式。此外,组分的溶解性或分散性可以决定载体的形式和特性。在一些实施方案中,载体以相对于组合物的重量为约50重量%至约99重量%、约60重量%至约98重量%、约70重量%至约98重量%、或可选地约80重量%至约95重量%的水平存在。
载体可以为多种多样的形式。非限制性实例包括简单溶液(例如,水溶液、有机溶剂或油基溶液)、乳液和固体形式(例如,凝胶、棒、可流动的固体或无定形材料)。在某些实施方案中,皮肤病学上可接受的载体为乳液形式。乳液通常可以分为具有连续的水相(例如,水包油和水包油包水(water-in-oil-in-water))或具有连续的油相(例如,油包水和油包水包油(oil-in-water-in-oil))。本发明的油相可以包含硅油,非硅油例如烃油、酯和醚等,及其混合物。
例如,水相包含水,例如软化水或蒸馏水。可以在水性载体中使用的其它可接受的载体包括但不限于醇化合物,例如乙醇。根据一些实施方案,该组合物包含醇、二丙二醇和/或水。
乳液可以进一步包含乳化剂。该组合物可以包含任意合适百分比的乳化剂以使载体充分地乳化。乳化剂的合适的重量范围包括基于组合物的重量为约0.1重量%至约10重量%或约0.2重量%至约5重量%。乳化剂可以是非离子乳化剂、阴离子乳化剂或阳离子乳化剂。例如,在美国专利No.3,755,560和4,421,769,以及McCutcheon's Detergents andEmulsifiers,北美版,第317-324页(1986)(通过引用以其整体并入本文)中公开了合适的乳化剂。合适的乳液可以具有宽范围的粘度,这取决于期望的产品形式。乳化剂的非限制性实例包括硬脂酸甘油酯、聚山梨酯60、和由Gattefosse以商品名出售的PEG-6/PEG-32/甘油硬脂酸酯混合物。乳液可以包含脂肪相,该脂肪相的范围可以为组合物的约5重量%和约80重量%之间(例如,约5重量%至约50重量%之间)。本文所述的任意乳液可以包含一种或多种选自油、蜡、乳化剂和辅助乳化剂(coemulsifiers)的组的试剂。制剂中使用的油、蜡、乳化剂和辅助乳化剂的实例是本领域众所周知的。乳化剂和辅助乳化剂可以以组合物的0.3重量%至约30重量%(例如,约0.5重量%至约20重量%之间)的比例存在于组合物中。乳液可以包含脂质囊泡。
可以将局部用组合物与其它药剂一起配制,所述其它药剂包括但不限于用于治疗皮肤癌、炎性疾病、良性瘤(例如血管瘤)、色素性病症的其它药剂,诊断剂和美容剂。可以将此类制剂配置成依序或同时地控制多种药剂以不同速率的释放。
可以一起给药或随后给药的其它药剂包括但不限于抗微生物剂、抗菌剂(antiseptic agent)和止痛剂。此类药剂可以例如在双重释放剂量制剂中依序(例如,在局部用组合物给药之后或之前)或者同时地给药。
在一些实施方案中,该方法包括在手术切除例如在治疗7、10、12、14、21或28天后,例如7-28、14-21天后的剩余病灶之前,每天(例如每天一次或两次)给予本文所述的治疗,持续至少2、3、4、5、6、7、8、9、10、11、12、13或14天,例如持续4天。在一些实施方案中,该方法包括在任选的手术干预之前重复治疗方案至少两次、三次、四次、五次、六次或更多次。在一些实施方案中,治疗方案包括治疗间隔,所述治疗间隔包括:每天(例如每天一次或两次)给予本文所述的治疗,持续至少2、3、4、5、6、7、8、9、10、11、12、13或14天,例如持续4天,然后在治疗间隔之间存在例如至少7、10、12、14、21或28天、例如7-28、14-21天的休息间隔。

实施例
以下实施例说明了本发明的一些实施方案和方面。对于相关领域的那些技术人员显而易见的是,可以在不改变本发明的精神或范围的情况下进行各种修改、增加和替换等,并且这样的修改和变化涵盖在所附权利要求书所限定的本发明的范围内。以下实施例不以任何方式限制本发明。
实施例1.包含卡泊三醇和咪喹莫特的组合物显示出抗肿瘤功效
本发明人假设T细胞激活剂(例如卡泊三醇和视黄酸10)与先天性免疫细胞激活剂(例如,如咪喹莫特等TLR激动剂和如DMXAA等STING激动剂11)的组合可导致针对皮肤恶性肿瘤的稳健的抗肿瘤免疫力。为了验证该假设,研究了卡泊三醇加咪喹莫特、卡泊三醇加DMXAA和视黄酸加咪喹莫特在抑制皮肤乳腺癌和黑色素瘤生长方面的功效。
为了验证T细胞激活剂(即卡泊三醇)与先天性免疫细胞激活剂(即咪喹莫特)的组合可导致针对皮肤恶性肿瘤的稳健的抗肿瘤免疫力的假设,研究了卡泊三醇加咪喹莫特在小鼠中抑制皮肤乳腺癌生长方面的功效。
从发生类似于人类的管腔型乳腺癌的自发性乳腺癌的PyMTtg小鼠获得乳腺癌细胞12。在肿瘤直径达到2cm之前,收集PyMTtg小鼠的乳腺癌。切除癌症并且使其离解成单细胞悬浮液。将乳腺癌细胞以每100μl 1:1比例的RPMI和Matrigel膜基质(Corning)为1×106个细胞的浓度再次悬浮。在癌细胞注射之前将受体小鼠的腹股沟区域剃毛。在第0天皮下注射1×106个细胞。当肿瘤尺寸达到直径5mm时,施加局部治疗。将小鼠分为4组,并且直接在肿瘤部位用卡泊三醇(80nmol)或作为载体的乙醇(EtOH)治疗。在卡泊三醇或EtOH的治疗后,还局部施用5%咪喹莫特乳膏或保湿乳(对照乳膏)来治疗肿瘤部位。3天后再一次施用卡泊三醇/EtOH和咪喹莫特/对照乳膏治疗(图1A)。每天监测动物,并且随时间测量肿瘤,以确定局部治疗对乳腺癌生长的影响。在收集时,将肿瘤称重,并且使用流式细胞术分析肿瘤浸润T细胞。
与EtOH加对照乳膏治疗的乳腺肿瘤相比,卡泊三醇加咪喹莫特组合的两次施用导致皮肤乳腺肿瘤生长的显著抑制(p=0.011,图1B-D)。单独施用的卡泊三醇或咪喹莫特在两次施用后均未显示出对乳腺癌生长的显著影响(图1B-D)。出乎意料的是,卡泊三醇加咪喹莫特组合引起所治疗的肿瘤内T细胞(CD3+CD19-)浸润的显著诱导(图1E),以及CD4+和CD8+肿瘤浸润T淋巴细胞的增殖(Ki67+)(图1F)。这些发现突出显示了抗肿瘤免疫在介导卡泊三醇加咪喹莫特治疗对乳腺癌的效果中的作用。
实施例2.卡泊三醇加DMXAA阻断了黑色素瘤的进展。
在第0天,将黑色素瘤细胞(B16-F10,ATCC,Manassas,VA)以每100μl 1:1比例的DMEM(Corning)2×105个细胞皮下注射至小鼠的胁腹部。在癌细胞注射之前将受体小鼠的胁腹部区域剃毛。当肿瘤尺寸达到直径5mm时,开始用卡泊三醇或EtOH连同腹膜内(IP)注射DMXAA(鼠STING激动剂,5,6-二甲基呫吨酮-4-乙酸、ASA404、或瓦帝莫泽)或生理盐水一起进行局部治疗。将小鼠分为4组,并且直接在肿瘤部位用卡泊三醇(40nmol)或作为载体的乙醇(EtOH)局部治疗。在卡泊三醇或EtOH(对照)治疗后,对小鼠腹膜内注射在100μl生理盐水中的400μg DMXAA或单独的生理盐水(对照)。再次施用卡泊三醇/EtOH和DMXAA/盐水治疗两次(图2A)。每天监测动物,并且随时间测量肿瘤以确定治疗对黑色素瘤生长的影响。在收集时,将肿瘤称重,并且使用流式细胞术分析肿瘤浸润T细胞。
与EtOH加盐水治疗的黑色素瘤相比,卡泊三醇加DMXAA组合的三次施用导致皮肤黑色素瘤生长的显著抑制(p=0.019,图2B-D)。单独的卡泊三醇在三次施用后显示出抑制黑色素瘤生长的功效,但单独的DMXAA在三次施用后并未显示出对黑色素瘤发展的显著影响(图2B-D)。卡泊三醇加DMXAA组合引起所治疗的肿瘤内T细胞(CD3+CD19-)浸润的显著诱导(图2E),以及CD4+和CD8+肿瘤浸润T淋巴细胞的增殖(Ki67+)(图2F),在单独的卡泊三醇组、单独的DMXAA组和EtOH加生理盐水对照组中则不存在该效果。这些发现表明,抗肿瘤免疫介导了卡泊三醇加DMXAA治疗对黑色素瘤的作用。
实施例3.卡泊三醇加咪喹莫特和视黄酸加咪喹莫特抑制黑色素瘤生长
在一组补充实验中,将B16-F10黑色素瘤细胞皮下注射至小鼠的胁腹部。当肿瘤变得明显时施加局部治疗。将小鼠分为5组,并且直接在肿瘤部位用卡泊三醇(20nmol)、视黄酸(20nmol)或作为载体的乙醇(EtOH)进行治疗。在卡泊三醇、视黄酸或EtOH治疗后,还通过局部施用5%咪喹莫特乳膏或保湿乳(对照乳膏)来治疗肿瘤部位。相隔3天,再次施用卡泊三醇/视黄酸/EtOH和咪喹莫特/对照乳膏治疗另外两次(图3A)。每天监测动物并且随时间测量肿瘤以确定局部治疗对黑色素瘤生长的影响。在收集时,对肿瘤拍照,并且记录其重量。
如由宏观图像、随时间的肿瘤体积和收集时的肿瘤重量所示,与使用卡泊三醇、视黄酸或咪喹莫特的单一疗法相比,卡泊三醇加咪喹莫特和视黄酸加咪喹莫特组合的三次施用导致黑色素瘤肿瘤生长的显著抑制(p<0.05,图3B-D)。这些发现突出显示了抗肿瘤免疫在介导卡泊三醇加咪喹莫特和视黄酸加咪喹莫特治疗对黑色素瘤的效果中的作用。
实施例4.卡泊三醇和咪喹莫特组合改善了患者的EMPD病灶
为了确定卡泊三醇加咪喹莫特组合治疗患者的EMPD的功效,在手术切除治疗14-21天后的剩余病灶之前,发明人在早上用0.005%卡泊三醇软膏并且在晚上用5%咪喹莫特乳膏经4天局部治疗两名男性EMPD患者(图4A)。在治疗前和手术时通过临床图像记录疾病的程度,有趣的是,使用卡泊三醇加咪喹莫特组合的4天的治疗在EMPD病灶部位引起明显的炎症(即免疫激活),并且炎症在治疗后经两周消退。重要的是,该治疗导致手术时病灶尺寸的显著减小,这在所切除的减小的病灶的阴性手术切缘上得到了证实(黄色虚线,图4B)。

Claims (21)
