Detailed Description
The technical solutions in the embodiments of the present application will be described below with reference to the drawings in the embodiments of the present application.
The application provides a visit information synchronization method and an on-line follow-up appointment method, by synchronizing the visit information of a specific patient in a data center of a physical hospital to a data center of an internet hospital, the visit information of the patient in the internet hospital and the off-line physical hospital is integrated and linked, and a set of full-visit diagnosis and treatment data for the patient is established in the whole hospital, so that the internet hospital data center can conveniently identify the follow-up patient, the problem of risk potential caused by medical data fault is avoided, and the medical safety is improved.
Fig. 1 shows a simple schematic diagram of a visit information synchronization system in an embodiment of the present application, including: the entity hospital data center 10 and the internet hospital data center 20, and the entity hospital data center 10 and the internet hospital data center 20 may be deployed with an information software system, which is used for recording the treatment information of the patient, and may further include a series of information services such as patient management, doctor management, performance assessment, internal communication, and the like, wherein the entity hospital data center 10 is deployed in an internal hospital network, and the internet hospital data center 20 is deployed in an external network. The doctor terminal 30 connected to the intranet can retrieve the data on the outpatient service of the patient from the data center 10 of the physical hospital when the offline outpatient service is performed, and the doctor terminal 30 connected to the extranet can retrieve the data on the outpatient service of the patient from the data center 20 of the internet hospital when the online outpatient service is performed.
Fig. 2 shows a flowchart of a visit information synchronization method provided in an embodiment of the present application, where the visit information synchronization method is explained based on a physical hospital data center, and as shown in fig. 2, the method includes the following steps:
step 110: and receiving a mark which is added by the doctor through the doctor terminal and corresponds to the doctor room for the current visit and can be used for on-line re-diagnosis.
When a patient is in offline consultation, a doctor calls out the information of the patient from the data center of the entity hospital through the doctor terminal, then issues related medical advice according to the state of an illness of the patient, after the patient is in attendance, the doctor adds an online double-diagnosis mark to the patient through the doctor terminal, and the doctor terminal records the added online double-diagnosis mark in the information of the patient, and uploads the mark to the data center of the entity hospital together for storage. Wherein, the mark for on-line double diagnosis comprises an on-line double diagnosis mark and a mark for non-on-line double diagnosis. In a specific application scenario, when a doctor determines that a disease diagnosis of a patient is within a permitted range of an internet hospital corresponding to the entity hospital and the doctor considers that the patient's condition has a condition for online re-diagnosis, the doctor can add an online re-diagnosis mark to the patient through a doctor terminal to represent that online appointment re-diagnosis can be performed when the patient visits the next time.
The on-line re-diagnosis mark corresponds to the department of the patient who is going to be diagnosed at this time, which means that the patient can go to the department for on-line re-diagnosis, for example, after the patient goes to the department a, a corresponding on-line re-diagnosis mark is added, so the patient can make an appointment for on-line re-diagnosis in department a, but not in department B, because the patient does not have the on-line re-diagnosis mark corresponding to department B.
Each entity hospital corresponds to an internet hospital, and each entity hospital can define various common diseases and chronic diseases which can be treated by the related internet hospital. When the Internet hospital obtains the approval permission of the Internet hospital, the developed common diseases and chronic diseases which can be subjected to the re-diagnosis are reported, the health committee issues corresponding approval licenses according to the types of the common diseases and the chronic diseases reported by the Internet hospital, and the Internet hospital develops the re-diagnosis business based on the approval permission range in the license.
Step 120: according to an appointed synchronization period, synchronizing the treatment information of the patients meeting the preset requirements in the patients who have completed treatment to an Internet hospital data center.
In a specific embodiment, the preset requirements include: the disease diagnosis code of the patient is in the authorized scope of the Internet hospital corresponding to the entity hospital, and the patient is added with an online re-diagnosis mark. Wherein the disease diagnosis code is a code in a specific code system of disease diagnosis preliminarily made by a doctor when a patient is on-line.
After the offline patient visit of the patient is finished, the patient visit information of the patient at the current time is obtained, wherein the patient visit information comprises a disease diagnosis code of the disease suffered by the patient. And the entity hospital data center synchronizes the diagnosis information of the patient, which is in the approval permission range of the Internet hospital and added with the online re-diagnosis mark, of the patient who finishes the diagnosis to the data center of the associated Internet hospital according to the appointed synchronization period.
In a specific embodiment, the patient treatment information of the patient who finishes treatment on the same day in the entity hospital data center, has a disease diagnosis code within the approval permission range of the internet hospital and is added with the online re-diagnosis mark can be automatically synchronized into the database of the internet hospital data center every night, and after the internet hospital data center receives the synchronous treatment information of the entity hospital data center, an online re-diagnosis patient list is formed in the internet hospital data center, so that a related data base is laid for the identification of the re-diagnosis data lattice of the patient in the internet hospital. The night synchronous operation can avoid influencing the business of the entity hospital to a great extent.
In one example, the synchronized visit information may include all the visit information generated when the patient visits off-line, including patient basic information, clinic visit number, clinic date, clinic department, doctor name, doctor job number, disease diagnosis code, on-line reviewer flag, medical history information, etc., which details the patient's chief complaints, symptoms, diagnoses, prescriptions, examination reports, etc. Considering that a part of patients do not go back to the internet hospital after going offline, for example, a part of older elderly people may tend to go to the entity hospital for a back-visit, the entity hospital data center synchronizes all the information of the patients going to the internet hospital data center, which may cause waste of resources.
Therefore, in a specific embodiment, the medical information of the patient synchronized by the entity hospital data center does not include medical information of the patient, and after the patient to be reviewed is successfully scheduled for review in the internet hospital data center, the internet hospital data center requests the entity hospital data center to synchronize the relevant medical information. For this embodiment, the specific implementation process of the entity hospital data center is shown in fig. 3, and includes:
step 210: receiving a synchronization request of an internet hospital, wherein the synchronization request comprises the re-diagnosis reservation information of a patient to be re-diagnosed, and the re-diagnosis reservation information comprises the identity ID of the patient to be re-diagnosed and a reservation department.
The identity ID of the patient to be reviewed is used to uniquely characterize the patient to be reviewed, which may be the identification number of the patient to be reviewed, or a unique code number assigned to the patient to be reviewed when the patient to be reviewed first visits the physical hospital.
Step 220: and according to the re-diagnosis reservation information, extracting medical record information of the patient to be re-diagnosed corresponding to the reserved department, and synchronizing the medical record information to an internet hospital data center.
After receiving the synchronization request and the re-diagnosis reservation information of the internet hospital data center, the entity hospital data center extracts the medical record information of the patient to be re-diagnosed corresponding to the reserved department from the database according to the identity ID and the reserved department, and synchronizes the medical record information to the internet hospital data center, so that a doctor of the internet hospital can conveniently call and read the medical record information during on-line re-diagnosis.
The synchronization mode is an on-demand synchronization mode, namely, the entity hospital data center only synchronizes some basic information of the doctor to the interconnected hospital data center every day, such as basic information of the patient, the clinic number, the clinic date, the clinic department, the name of the doctor, the work number of the doctor, the disease diagnosis code, the mark for on-line re-diagnosis, and the like. For patients who successfully reserve a review at the internet hospital data center, and who definitely go on-line review at a certain day, the internet hospital data center requests the entity hospital data center to synchronize detailed medical record information of the patients.
In addition, when the entity hospital data center synchronizes the visit information to the internet hospital data center according to the agreed synchronization period, the internet hospital data center also automatically synchronizes the visit information corresponding to the patients who have completed the online follow-up visit to the entity hospital data center at regular time according to the agreed synchronization period, and the entity hospital data center receives the visit information synchronized with the internet hospital data center and stores the visit information in the database, so that doctors in the entity hospital can make a call and review when in an outpatient service.
Further, fig. 4 shows a flowchart of an online review appointment method provided in the embodiment of the present application, where the online review appointment method is described based on an internet hospital data center, and as shown in fig. 4, the online review appointment method includes the following steps:
step 310: and receiving the re-diagnosis reservation information of the patient to be re-diagnosed, wherein the re-diagnosis reservation information comprises the identity ID of the patient to be re-diagnosed and a reservation department.
The patient to be rescued can input the rescued appointment information through the user terminal, and the rescued appointment information is uploaded to the internet hospital data center through the user terminal. The identity ID of the patient to be reviewed is used to uniquely characterize the patient to be reviewed, which may be the identification number of the patient to be reviewed or a unique code number assigned to the patient to be reviewed when the patient to be reviewed visits the physical hospital for the first time. Optionally, the review appointment information may further include a review date and a review time for the patient to be reviewed, a selected review doctor, and the like.
Step 320: and matching in the online re-diagnosis patient list according to the re-diagnosis appointment information, and determining whether the patient to be re-diagnosed meets the qualification of online re-diagnosis according to the matching result.
The online re-diagnosis patient list is established and formed according to the clinic information of the patients which are synchronized by the entity hospital data center and meet the preset requirements. The entity hospital data center synchronizes the clinic information of the patients with the online clinic reviewable marks, wherein the disease diagnosis codes of the patients who finish off-line clinic visit are within the allowable range of the Internet hospital practice according to the appointed synchronization period, so that the online clinic patient list is formed.
After receiving the re-diagnosis appointment information of the patient to be re-diagnosed, the internet hospital data center identifies the re-diagnosis qualification of the patient to be re-diagnosed so as to determine whether the on-line re-diagnosis qualification can be met. Specifically, one implementation of step 320 is shown in fig. 5, and includes:
step 3201: and judging whether the on-line re-diagnosis patient list has the diagnosis information of the patient to be re-diagnosed in the pre-diagnosis department or not according to the re-diagnosis reservation information.
The Internet hospital data center inquires whether the same identity ID and the treatment record of the same department exist in an online re-treatment patient list according to the identity ID of the patient to be re-treated and the appointment department, if so, the patient to be re-treated is shown to be subjected to an offline outpatient service in the appointment department before, and if not, prompt information is sent to a user terminal of the patient to be re-treated so as to indicate the patient to be re-treated to go to the entity hospital for treatment.
Step 3202: if the patient to be re-diagnosed is in the re-diagnosis information of the appointed department, judging whether the re-diagnosis date appointed by the patient to be re-diagnosed is within the re-diagnosis validity period according to the re-diagnosis date in the re-diagnosis information.
The visit information of the patient to be re-diagnosed in the appointed department comprises the date of the patient to be re-diagnosed, the internet hospital data center judges whether the date of the re-diagnosis appointed by the patient to be re-diagnosed is within the effective period of the re-diagnosis according to the date of the re-diagnosis, and the effective period of the re-diagnosis can be flexibly set according to different departments and disease types. When the information of a plurality of times of visits of the patient to be rescued in the appointment department exists in the visit information, the date of the visit with the most recent time is selected as the date of the visit. One specific way of determining the scheduled review date is to determine whether the review date is within 21 days from the review date (or the most recent review date among the plurality of review dates) in the review message if the set review validity period is 21 days, and if so, determine that the review date is within the review validity period, and if not, determine that the review date is not within the review validity period.
And if the scheduled re-diagnosis date is not within the re-diagnosis validity period, sending prompt information to the user terminal of the patient to be re-diagnosed so as to instruct the patient to be re-diagnosed to go to the entity hospital for diagnosis.
Step 3203: if the patient is in the effective period of the re-diagnosis, judging whether the patient to be re-diagnosed is added with an online re-diagnosis mark corresponding to the appointed department according to the re-diagnosis information.
If the scheduled review date is within the review validity period, the next judgment is carried out, namely whether the online review mark corresponding to the scheduled department is added to the patient to be reviewed is judged. When a patient to be subjected to a re-diagnosis is in an offline diagnosis, a doctor adds a corresponding on-line re-diagnosis mark through a doctor terminal, and the added on-line re-diagnosis mark corresponds to a department at the time of the next diagnosis.
For example, after the patient is in the department B, the doctor considers that the patient does not meet the online re-diagnosis condition, adds the non-online re-diagnosis mark, then after the patient is in the department A, the doctor adds a corresponding online re-diagnosis mark for the patient through the doctor terminal, and in the appointed synchronization period, the entity hospital data center synchronizes the diagnosis information of the patient in the department A and the department B to the internet hospital data center, but the patient only has the online re-diagnosis qualification in the department A and does not have the online re-diagnosis qualification in the department B. Therefore, when the patient to be reviewed makes an on-line review appointment in department B, it is determined through step 3201 that the review information of the patient to be reviewed in department B exists in the list of the on-line review patients, and it is determined through step 3202 that the scheduled review date is also within the review validity period, it is determined through step 3203 that the patient to be reviewed does not have the on-line review flag corresponding to department B, and thus, the following steps are not performed.
And if the on-line re-diagnosis mark corresponding to the reserved department is not added to the patient to be re-diagnosed, sending prompt information to the user terminal of the patient to be re-diagnosed so as to instruct the patient to be re-diagnosed to go to the entity hospital for diagnosis.
Step 3204: if the on-line re-diagnosis mark exists, the patient to be re-diagnosed is determined to meet the qualification of on-line re-diagnosis.
It can be understood that, in an actual scenario, the confirmation of the review qualification of the patient to be reviewed by the internet hospital data center may also be determined in a different order from the above-mentioned steps 3201 and 3204, for example, after the step 3201, it may be determined whether the patient to be reviewed is added with an online review mark corresponding to the appointment department according to the review information, and if the online review mark is added, it may be determined whether the review date appointed by the patient to be reviewed is within the review validity period; obviously, after step 3201, it may also be determined whether the patient to be reviewed is added with the on-line review mark corresponding to the appointed department, and whether the review date appointed by the patient to be reviewed is within the review effective period, and when the review date is within the review effective period and the on-line review mark is added, it is determined that the patient to be reviewed satisfies the qualification of on-line review. The order in which the above embodiments are shown is merely one implementation.
After step 320, continue with step 330: and when the qualification of on-line re-diagnosis is met, allocating on-line re-diagnosis resources for the patient to be re-diagnosed.
The online re-diagnosis resources distributed to the patient to be re-diagnosed comprise one or more of the re-diagnosis date, the re-diagnosis time and the re-diagnosis doctor when the patient to be re-diagnosed carries out online re-diagnosis. When a patient to be subjected to a follow-up diagnosis appointment, a request can be sent to the Internet hospital data center by selecting the date of the follow-up diagnosis, the time of the follow-up diagnosis and a follow-up doctor, and after the Internet hospital data center determines that the patient to be subjected to the follow-up diagnosis meets the qualification of the on-line follow-up diagnosis, the patient to be subjected to the follow-up diagnosis is determined to be distributed to the selected date of the follow-up diagnosis, the time of the follow-up diagnosis and the follow-up doctor. Or, in the case that the patient to be diagnosed does not select the information such as the date of the re-diagnosis, the internet hospital data center may reasonably allocate resources to the patient to be diagnosed according to the idle time period and the doctor who is in the idle state in the reserved department. And after the on-line review resource allocation is completed, the follow-up appointment success of the patient to be reviewed is shown.
Alternatively, the patient visit information synchronized by the physical hospital data center to the internet hospital data center may include all the visit information generated when the patient visits offline, the system comprises basic information of patients, clinic numbers, clinic dates, clinic departments, names of doctors who receive the doctor, the numbers of the doctors who receive the doctor, disease diagnosis codes, on-line re-diagnosis marks, medical record information and the like, and considers that part of patients can not be subjected to on-line re-diagnosis in an internet hospital after on-line diagnosis, if all the information of the treatment is synchronized to the data center of the internet hospital, the waste of resources is caused, thus, the visit information synchronized by the physical hospital data center to the internet hospital data center does not include medical history information of the patient, and, then, in the internet hospital data center, the on-line follow-up patient list does not contain the medical record information of the patient.
The internet hospital data center adopts an on-demand synchronization mode, and after a patient to be subjected to follow-up diagnosis appointment successfully in the internet hospital data center, the internet hospital data center requests the entity hospital data center for synchronizing the related medical record information so as to facilitate the retrieval of doctors of the internet hospital during diagnosis and treatment.
Optionally, after the online review resources are allocated to the patient to be reviewed, it may be determined that the review appointment of the patient to be reviewed is successful, and the internet hospital data center sends a prompt message indicating that the appointment is successful to the user terminal of the patient to be reviewed, and reminds the patient to be reviewed to perform the online review in the allocated review time in the prompt message. After the follow-up appointment of the patient to be subjected to follow-up examination is successful, the internet hospital data center further executes: initiating a synchronous request to a data center of the entity hospital according to the follow-up appointment information of the patient to be subjected to follow-up examination; the synchronization request is used for requesting the entity hospital data center to synchronize the medical record information of the patient to be diagnosed corresponding to the appointment department to the internet hospital data center.
After receiving the synchronization request and the re-diagnosis reservation information of the internet hospital data center, the entity hospital data center extracts the medical record information of the patient to be re-diagnosed corresponding to the reserved department from the database according to the identity ID and the reserved department, and synchronizes the medical record information to the internet hospital data center.
Wherein, the medical record information can include: the allergic history mark, the past medical history, the chief complaint (description of the main symptoms related to the disease and the duration time of the disease), the present medical history, the diagnosis, the physical examination, the examination report, the inspection report and the like.
It should be noted that there are two possible application scenarios in the above embodiment:
1. advance reservation
The patient to be retested initiates a retest appointment request to the internet hospital data center at least one day in advance, the internet hospital data center receives the retest appointment information input by the patient to be retested when initiating the retest appointment request, the retest qualification is determined through the steps 3201 and 3204, and if the patient to be retested meets the retest qualification, the on-line retest resource can be distributed to the patient to be retested to complete the retest appointment.
For a plurality of patients to be diagnosed who are successfully subjected to the follow-up appointment on the same day, the Internet hospital data center automatically pushes the identity ID of each patient to be diagnosed and the appointed department to the entity hospital data center at regular time in the same day, the entity hospital data center searches in the database according to the received information, and the searched medical record information of each patient to be diagnosed is synchronized to the Internet hospital data center in batch.
2. Instant reservation
For example, the patient is scheduled for the re-diagnosis in the morning of the day and is scheduled for the re-diagnosis in the morning or afternoon, the internet hospital data center firstly determines the qualification of the re-diagnosis according to the re-diagnosis scheduling information of the patient to be re-diagnosed, and the determination step is the same as the determination mode in the advance scheduling. For the patient to be rescued who is successfully ordered immediately, the Internet hospital data center immediately initiates a synchronization request to the entity hospital data center, and pushes the identity ID and the ordered department to the entity hospital data center, and the entity hospital data center immediately synchronizes corresponding medical record information to the Internet hospital data center according to the received identity ID and the ordered department.
In order to facilitate the patient to be diagnosed to effectively grasp the current progress and status, as shown in fig. 6, after the internet hospital data center issues a synchronization request to the entity hospital data center, the method further includes:
step 410: and marking the current state of the patient to be diagnosed as the waiting state.
The waiting state indicates that the internet hospital data center is synchronizing the medical record information of the patient to be diagnosed.
Step 420: after receiving medical record information of the patient to be diagnosed synchronized by the entity hospital data center, which corresponds to the appointment department, the current state of the patient to be diagnosed is marked as a state to be diagnosed.
After the medical record information of the patient to be diagnosed is synchronously finished, the current state is changed from the waiting state to the treatment state, the treatment state indicates that the medical record information is synchronously finished, and the on-line re-diagnosis can be started in the scheduled re-diagnosis time.
Optionally, the internet hospital data center synchronizes the review information corresponding to the patient who completes the online review to the entity hospital data center according to an agreed synchronization period. In a specific embodiment, the internet hospital data center is arranged to automatically synchronize the review information (including medical order information) of the patients who complete online review on the same day of the internet hospital to the entity hospital data center at night. In this way, a set of patient-centered treatment information can be established in the entity hospital data center, and the integration of all treatment information of patients is realized.
Optionally, when the patient is in an offline consultation, the doctor adds an online review mark corresponding to the current consultation department to the patient through the doctor terminal, and simultaneously, the doctor terminal can record the recommended online review time for the patient. At every night, the online review time is recommended, and the online review mark and other review information of the patient are synchronized to the internet hospital data center. In a specific embodiment, the internet hospital data center sends an online follow-up reminding to a user terminal of a target patient before the recommended online follow-up time corresponding to the target patient arrives for any target patient in an online follow-up patient list; the on-line double-visit reminder is used for reminding the target patient to carry out on-line double-visit at the recommended on-line double-visit time.
According to the technical scheme provided by the embodiment of the application, the patient treatment information (including the information during off-line treatment and the information during on-line re-treatment) can be integrated and linked, when a doctor in an entity hospital performs off-line treatment, the patient treatment information during off-line treatment can be called from the entity hospital data center through the doctor terminal, and meanwhile, the re-treatment information during on-line re-treatment of the patient can be called. The on-line review information retrieval method is specifically realized in a way that a retrieval button of on-line review information is embedded in a page displayed by a doctor terminal, the doctor terminal receives the trigger of a doctor when clicking the retrieval button, retrieves the review information from a data center of an entity hospital and displays the review information on the page for the doctor to consult and comprehensively judge, and medical errors caused by information splitting are reduced.
Meanwhile, when the doctor of the Internet hospital carries out on-line re-diagnosis on the patient, the doctor terminal can be used for calling the historical re-diagnosis information of the patient from the Internet hospital data center, and meanwhile, the information of the patient when the patient is in off-line treatment can be called. The online examination information retrieval method is specifically realized in the way that an examination button of offline examination information is embedded into a page displayed by a doctor terminal, the doctor terminal receives the trigger of a doctor when clicking the examination button, the offline examination information is retrieved from a data center of an internet hospital and displayed on the page for a doctor to consult, the doctor can conveniently make the doctor in the internet hospital have sufficient recognition and sufficient diagnosis and treatment basis on the condition that the patient cannot be examined, and the medical risk is reduced.
