Background
Along with the development of society, the improvement of technology, the continuous emergence of a large number of antimicrobial drugs, the clinical application is increasingly wide, and the phenomenon of abuse of the antimicrobial drugs is rare. Due to unreasonable application, not only is huge waste of resources caused, and social burden and adverse drug reactions are increased, but also bacterial drug resistance frequently occurs, and great confusion is brought to treatment of infectious diseases. Whether the antimicrobial drugs are reasonably and directly related to effective treatment of diseases, reasonable distribution of medical resources, safety and health of patients and the like is the key for guaranteeing medical quality, controlling unreasonable medical expenses and maintaining rights and interests of patients.
An ideal antimicrobial drug for clinical use would be one that has a high selectivity of action against pathogenic microorganisms that cause disease in the patient, without other undesirable or minor damage to the patient. The selective action on pathogenic microorganisms is very important for clinical safe medication. In recent years, many new and effective antimicrobial drugs and their rational application rules have been mastered and successfully applied to clinical application through pharmacodynamic, pharmacokinetic and clinical pharmacological studies of various antimicrobial drugs.
When a doctor selects an antimicrobial drug for a patient, the antimicrobial drug sensitive to pathogenic microorganisms is selected based on information of the pathogenic microorganisms, and the antimicrobial drug effective for the patient is selected by considering factors such as administration routes, administration sites, contraindications, adverse reactions, potential safety hazards and use limitations of various antimicrobial drugs and the like in combination with individualized information of the patient, so that the antimicrobial drug possibly harmful to the patient is excluded. Patient medical insurance conditions and economic payment capabilities should also be considered, as well as the suitability of antimicrobial drugs. Only by taking the above factors closely related to the antimicrobial drugs into consideration, the optimal target can be achieved, and the benefit of the patient can be fully secured.
In the process of actually selecting and analyzing antimicrobial drugs, doctors and pharmacists are usually based on the information such as drug indications and clinical experience of the drug specifications, and personalized drug selection and evaluation are difficult to be performed according to the specific actual conditions of each patient due to factors such as information acquisition limitation, workload and time limitation. Meanwhile, the patient is influenced by a plurality of factors in the process of using the medicine, and the phenomena of low efficiency, uncontrollable error rate and the like are inevitably generated only by the memory and manual analysis of professionals. Particularly in time critical treatments where the patient is not fully informed of the full extent of the infection, measures are required to control the condition or prevent the infection at the first time, often based on the clinical experience of the physician, and risks associated with antimicrobial drug use are more likely to arise. The rationality analysis of the conventional medicine use only compares the indications of the medicine, and deep analysis is not carried out according to the clinical practical situation of a patient to judge whether the medicine meets the practical use condition, so that errors in medicine selection can be ignored. Therefore, an intelligent auditing method for antimicrobial drug use is urgently needed, and the method is an important method for guaranteeing the reasonable compliance of the antimicrobial drug use of patients by comprehensively analyzing and auditing the safety, effectiveness, economy, suitability and other aspects of the antimicrobial drug in the whole process based on technical data such as a drug specification, a prescription set, relevant guidelines, relevant regulations and the like. The microorganism information needed to be treated by the patient is obtained by analyzing the microorganism needed to be treated by the patient according to the disease information of the patient, such as relevant indexes, relevant symptoms, relevant diagnosis, relevant operations, population information, drug use, infection environment/season, diet/work/study/exercise/fertility information and relevant examination/inspection/detection results. The infection caused by different microorganisms may have certain differences in the infected people, seasons, environment, infected parts, symptoms, manifestations, various indexes of patients, detection/inspection results, etc., and the microorganisms possibly causing the infection of patients can be analyzed according to the data and rules of the antimicrobial drug use database through related information.
The information of the microorganisms needed to be treated by the patient can be obtained based on the analysis of the antimicrobial drug use database, or can be directly obtained from the already-defined information of the microorganisms needed to be treated by the patient, such as the result of the microorganism-related examination. The microbes infected by the patient may be analyzed to obtain a certain microbe or only the range/combination of microbes possibly infected, and the microbes causing infection or the range of microbes possibly infected may be confirmed or narrowed by the relevant indexes, relevant symptoms, relevant diagnoses, relevant operations, crowd information, drug use, infection environment/season, diet/work/study/exercise/fertility information and relevant examination/test/detection results of the patient. For example, when a patient is in hospital and suffers from nosocomial infection, the distribution of microorganisms detected by the hospital, department, ward and equipment or the analysis result of pathogenic microorganisms of nosocomial infection of other patients can be used as an important reference to indicate the microorganisms possibly infected by the patient. Different areas/hospitals/departments/wards/communities and the like, relevant data in the self antimicrobial drug use database can be maintained according to the monitoring and analyzing results. At the same time, under different seasonal/climatic conditions, the relevant target microorganisms from which the patient acquires an infection can also be maintained according to the monitoring and analysis results. The relationship between environmental, climatic, epidemiological, etc. factors and infectious/preventive microorganisms in the database of antimicrobial drug use is based on various statistical analyses and clinical studies.
When the microorganism information of possible infection of the patient is obtained by analyzing the patient based on the antimicrobial drug use database, the relevant information of the patient is obtained by comparing the relevant information of the patient with the antimicrobial drug use database. Or directly acquiring the existing microorganism information of possible infection of the patient, such as the result of microorganism-related examination.
The microorganism information to be prevented of the patient is acquired according to the relevant information of prevention such as age, diagnosis, symptom, contact history, exposure history, physical condition, past medical history, life habits, environment, operation, trauma, examination/inspection/detection result, epidemic disease, season, climate condition and the like of the patient.
Patients often need preventive medication against microbial infections when there is a certain risk of microbial infections or when the occurrence of infections would be a major hazard. Under different conditions, different microorganisms may threaten infection of patients, and different preventive measures are needed to be analyzed according to the relevant information of prevention.
The information of the microorganism to be prevented of the patient can be obtained by analyzing based on an antimicrobial drug use database, or can be directly obtained by the existing information of the microorganism to be prevented of the patient. Likewise, the microorganisms analyzed to be prevented in a patient may also be analyzed for a defined microorganism or only a range of microorganisms/combination of microorganisms.
The disease information/prevention-related information of the patient may include: relevant diagnosis/symptom/index/feeling of the patient and adjustment, examination/test/detection result of the relevant diagnosis/symptom/index/feeling, population information, genetic-related information, medical history information, medication history information, physiological development information, marriage and childbirth information, physiological status, psychological/intellectual status, life/work/study/sport/entertainment information, environmental information, medicine/medical instrument/health product/cosmetic use-related information, and the like of the patient.
The sources from which the disease information/prevention-related information of the patient is obtained may be: advice prescribed by medical staff; patient medical history, electronic medical history, diagnostic reports, examination/test/detection results, monitoring results, assessment reports; patient-related symptoms, sensations, states, physiological functions self-statement; the state and the requirement of the patient for learning, working, living, sports, etc. are self-describing; preset time, season, time of day, interval, period and other information; and the preset ways of monitoring the change of various physiological and pathological indexes and the like.
The intelligent antimicrobial drug recommending method further comprises the following steps: analysis of target microorganism ranking for use of antimicrobial drugs is required under various disease information/prophylactic information conditions. According to factors including but not limited to the possibility of infection caused by various microorganisms under the condition, the degree of harm of infection caused by various microorganisms to patients under the condition, the emergency degree of infection caused by various microorganisms under the condition in terms of time and the like, the grade/score corresponding to the factors such as the possibility of infection caused by various microorganisms under the condition, the degree of harm, the emergency degree and the like is respectively set, and the relative weight of the factors such as the possibility, the degree of harm, the emergency degree and the like under the condition can be set according to the actual situation, so that when the microorganism needing to be treated/prevented of a patient is analyzed, the comprehensive grade/score of the relevant microorganism needing to be treated/required to be prevented under the condition can be calculated according to the grade/score corresponding to the factors such as the possibility of infection caused by the relevant microorganism, the degree of harm, the emergency degree and the like and the relative weight of each factor, in order to analyze the level of necessity or the overall prioritization of the various microorganisms/types that need to be treated/prevented under the conditions. Depending on the actual condition of the patient, disease, drug, other analytical items besides infection likelihood, degree of harm, degree of urgency may be introduced, taking into account the overall level and score of microbial treatment/prevention. The ranking results in a list of the range of microorganisms/combination of microorganisms that need to be treated/prevented. If the microorganism infected by the patient is directly obtained by a laboratory test or the like, the relevant information is directly used. Data on the possibility of infection by various/various microorganisms under the relevant conditions, the degree of harm to the patient and the degree of urgency in terms of time under the conditions, and the like, of infection by various/various microorganisms may be acquired from an antimicrobial drug use database, or may be manually set.
According to the necessity level or comprehensive priority ranking of each microorganism to be treated/prevented under the condition, whether the patient needs to be treated/prevented by using the antimicrobial drug and the range/combination of the microorganisms to be treated/prevented can be judged manually, and whether the patient needs to be treated/prevented by using the antimicrobial drug and the range/combination of the microorganisms to be treated/prevented can be judged intelligently according to the application system of the preset necessity level range or priority range, so that the treatment/prevention of the microorganisms needing to be treated/prevented by using the antimicrobial drug can be carried out according to the judgment. The related necessity level range or priority level range may be set according to related regulations, policies, guidelines, textbooks, clinical routes, national prescriptions, and the like, or may be obtained by analyzing medical big data.
The precondition for the patient to use the antimicrobial drug is that the level of necessity of the microorganism to be treated/prevented reaches or exceeds a set level under the condition of the patient's disease information or prevention-related information. The list of microorganism ranges/microorganism combinations that need to be treated/prevented can be optimized according to different levels of necessity/overall prioritization of the analysis results, including: measures such as exclusion, backward adjustment of the ranking and the like are taken for microorganisms with low necessity level/comprehensive priority ranking in the list of microorganism ranges/microorganism combinations, and measures such as forward adjustment of the ranking and the like can be taken for microorganisms with high comprehensive level/score. The level/score setting can be continuously optimized and adjusted during use.
When the information of the use condition of the relevant medicine of the patient is incomplete, and the relevant element actual information required by an analysis item for treating/preventing microorganisms is lacked, the treatment measures such as ignoring, reminding and supplementing, forced supplementing and the like can be taken according to the weight of the analysis item and the specific situation of the importance of the lacked information.
And 4, step 4: analyzing the condition that the antimicrobial medicament used by the patient covers the target microorganism needing to be treated/prevented of the patient through the information of the use of the antimicrobial medicament by the patient and the information of the target microorganism needing to be treated/prevented of the patient;
based on the patient antimicrobial drug usage information and the database of antimicrobial drug usage, a range of therapeutic/prophylactic microorganisms covered by the antimicrobial drug used by the patient can be obtained. By comparing the range with the target microorganism to be treated/prevented, it can be analyzed that the antimicrobial drug used by the patient covers the target microorganism to be treated/prevented. The antimicrobial agent used by the patient herein may be a single agent/single agent set or a combination of multiple agents/agent sets.
The results of an analysis of antimicrobial drug use by a patient to cover a target microorganism for which the patient is in need of treatment/prevention may include: the drug cannot cover the target microorganism; the drug can cover the target microorganism and has better contact ratio; drugs that can cover the target microorganism but that have a drug coverage that greatly exceeds the target microorganism range may suggest that the antimicrobial drug regimen will be inconsistent, in line with, excessive, etc. the therapeutic/prophylactic objectives of the patient, respectively.
And 5: sensitivity analysis was performed on antimicrobial drugs used by the patients.
The rationality of the use of the drug was reviewed by performing a sensitivity analysis on the antimicrobial drug used by the patient.
The sensitivity analysis is to analyze the sensitivity of the target microorganism to the antimicrobial drug/drug group based on the sensitivity data of the target microorganism to the antimicrobial drug under the current conditions of use of the patient, including the results of the drug susceptibility test. With the increasing use of antimicrobial drugs, microorganisms exhibit varying degrees of resistance to antimicrobial drugs. For those antimicrobial drugs that are used for longer periods of time and over a wider range of applications, the sensitivity of the microorganism tends to be lower and the therapeutic effect of the antimicrobial drug is poorer. To maintain the effectiveness of the antimicrobial drug, a highly sensitive antimicrobial drug should preferably be selected. If a patient needs to be treated by an antibacterial drug because of being infected by a certain bacterium, the sensitivity of the drug used by the patient can be analyzed through the drug resistance data of the bacterium and each symptomatic antibacterial drug in the hospital, and the drug has no use value if the drug resistance is higher under the current condition. When the antimicrobial drug used by the patient is a combination of two or more antimicrobial drugs/drug groups, if the results of the sensitivity analysis of a portion of the antimicrobial drugs/drug groups do not meet the criteria, the results that do not meet the requirements for the sensitivity of the target microbial drug may be present in their entirety. The sensitivity data of the microorganism to the antimicrobial drug can include a drug-resistant microorganism detection rate of the target microorganism to the relevant drug in the patient treatment environment.
And analyzing and comparing the result of the drug sensitivity test and the sensitivity related rule of the antimicrobial drugs in the antimicrobial drug use database under the current use condition to obtain the condition that the target microorganism is sensitive to the antimicrobial drugs used by the patient, and judging the rationality of the drug use.
Step 6: other reasonable compliance analyses were performed for antimicrobial drug use.
