Abstract
本发明公开了一种用于于内瘘穿刺后修复的活血通络组合物,由红花,三七,桃仁,没药,乳香,莪术制备而成。该组合物可制备成药酒或者其他外用制剂如凝胶剂,软膏剂,酊剂等。本发明的组合物根据中医理论和西医理论相结合的原理,科学配伍,使各药相互配合,取长补短,具有活血祛瘀,消肿止痛,收敛生肌等作用。对于动静脉内瘘术前、术后局部气血凝滞,静脉不通,脉络瘀阻者均可使用。使动静脉内瘘血管得以濡养,气充血足,脉道通畅,减少血管狭窄以及钙化、血肿以及血栓形成等并发症,促进内瘘成熟,保障维持性血液透析患者动静脉内瘘的长期有效使用。具有起效快,局部给药副作用小,安全性高,使用方便等优点。

技术领域
本发明涉及中医药领域,具体涉及一种用于内瘘穿刺后修复的活血通络组合物及制备方法及制剂。
背景技术
血液透析是慢性肾脏病尿毒症期患者肾脏替代的主要方法,80%以上血液透析患者的血管通路是动静脉内瘘,动静脉内瘘是维持性血液透析患者的生命线,一个功能良好的动静脉内瘘对于血液透析患者的生存有重要意义。动静脉内瘘术是医生采用外科手术将患者前臂相邻近的动静脉吻合,使之成为血流通道,动静脉内瘘要求选用的动静脉表浅,手术容易操作,内瘘成熟后,静脉发生动脉化改变,沿原静脉走行可选取的不同穿刺部位,不仅可减轻患者痛苦,而且不影响患者的日常生活,对患者全身血流无明显影响。但在动静脉内瘘手术后会有一些并发症出现,如血管狭窄、血栓形成等,这些并发症的出现会直接影响动静脉内瘘手术的术后护理,对患者的术后康复和内瘘的成熟造成不利影响。
其次,内瘘使用过程中发生的血肿严重影响其使用。对于内瘘血肿的处理,目前临床上多采用拔针24h后使用毛巾湿热敷,于热敷后在血肿局部涂抹多磺酸粘多糖乳膏并轻轻按摩5-10min,直至药膏全部经皮肤吸收,每日进行3-4次,血肿消散及疼痛减轻的效果不是很理想,尤其对血肿面积较大者,见效甚微。
发明内容
本发明的目的在于克服现有技术存在的缺陷,提供一种用于内瘘穿刺后修复的活血通络组合物。
为了实现上述目的,本发明采用的技术方案如下:
本发明的于内瘘穿刺后修复的活血通络组合物,由如下重量份的中药原料制成:红花5-15份,三七2-7份,桃仁5-15份,没药5-15份,乳香5-15份,莪术5-15份。
优选地,由如下重量份的中药原料制成:红花10份,三七5份,桃仁10份,没药10份,乳香10份,莪术10份。
以下是本发明的药材来源:
红花,为菊科植物红花Carthamus tinctorius L.的干燥花。归心、肝经。活血通经,散瘀止痛。用于经闭,痛经,恶露不行,症瘕痞块,跌扑损伤,疮疡肿痛。
三七,为五加科植物三七Panaxnotoginseng(Burk.)F.H.Chen的干燥根。甘、微苦,温。用于咯血,吐血,衄血,便血,崩漏,外伤出血,胸腹刺痛,跌扑肿痛。本品主要含皂苷、黄酮苷、氨基酸等。
桃仁,为蔷薇科植物桃Prunus persica(L.)Batsch或山桃Prunus davidiana(Carr.)Franch.的干燥成熟种子。苦、甘,平。活血祛瘀,润肠通便。用于经闭,痛经,癓瘕痞块,跌扑损伤,肠燥便秘。
没药,属植物没药树Commiphoramyrrha Engl.[C.molmol Engl.]及同属他种植物的树干皮部渗出的油胶树脂。苦,平。活血止痛,外用消肿生肌。用于跌打淤血肿痛,痈疽肿痛,胸腹诸痛;外用治疮口久不收敛。
乳香,为橄榄科植物乳香树、鲍达乳香树、野乳香树等皮部渗出的油胶树脂。辛、苦,温。活血止痛。用于心腹诸痛,筋脉拘挛,跌打损伤,疮痈肿痛;外用消肿生肌。
莪术,为姜科姜黄属植物莪术Curcuma aeruginosa Roxb.[C.zedoaria nonRosc.]、广西莪术Curcuma kwang siensis S.G.Lee et C.F.Liang和温郁金CurcumawenyujinY.H.Chen et C.Ling的根茎。辛、苦,温。破血行气,消积止痛。用于血瘀腹痛、肝脾肿大、心腹胀痛,积聚,妇女血瘀经闭,跌打损伤作痛饮食积滞。
本发明的另一目的是提供本发明活血通络组合物中药原料活性成分的提取方法,具体如下:
(a1)取上述中药原料,粉碎成粒径为200-500μm的粉体,置于高压罐中并加入中药原料总重量2-10倍的无水乙醇,在22-24℃、10-12MPa下保压2-3min,然后将高压罐在10-60s内泄压至常压,分离高压罐的药渣和药液;
(a2)取药渣,加水减压提取1-2次,每次加水量为药渣总重量的3-6倍,每次提取时间为1-3h,合并提取液,过滤,在4000-5000rpm的转速下离心4-10min,待滤液分层,取上层提取液与步骤(a1)的药液合并,得挥发油提取物液,蒸发乙醇,得挥发油成分;
(a3)将步骤(a2)的下层溶液浓缩至60-65℃时相对密度为1.10-1.20的清膏,得到水溶性活性成分。
本发明还提供另一种提取方法,具体如下:
(b1)将没药和乳香粉碎成粗粉,用体积分数为50-70%的乙醇作溶剂,浸渍36-50h后以1-3ml/分钟流速进行渗漉,收集漉液10倍量,离心,静置分层,上层溶液密封,得到挥发油成分,下层溶液备用;
(b2)将步骤(b1)的药渣与其他原料混合后加水浸渍1h煎煮,共煎煮3次,每次2-3h,合并煎液,过滤,与步骤(b1)的下层溶液合并,浓缩至75℃下相对密度为1.05-1.10的浓缩液,冷却至室温,加无水乙醇使含醇量50%,搅匀,静置24h,过滤,减压回收乙醇,然后继续浓缩至60℃下相对密度为1.10-1.20的清膏,得到水溶性活性成分。
此外,上述的活性成分还可以与医药学上可接受的载体配合,制成各种外用制剂,如凝胶剂、酊剂、软膏剂等。
本发明还提供一种利用上述中药原料制成的用于内瘘穿刺后修复的活血通络药酒,具体制备方法如下:
将中药原料粉碎,按1:1-2的体积比加入50-60度的米酒,密封浸泡至少30天,过滤,即得。
此外,还提供另一种制备方法获得用于内瘘穿刺后修复的活血通络药酒,具体制备方法如下:
(c1)将没药和乳香粉碎成粗粉,用无水乙醇作溶剂,浸渍12-18h后以1-3ml/分钟流速进行渗漉,收集漉液10倍量,离心,静置分层,取上清液密封,得到挥发油醇溶液;
(c2)将步骤(c1)的药渣与其他原料混合后加入3-5倍量的水,在75-90℃下回流提取2-3次,每次1-4h,合并提取液,过滤,浓缩,冷却至室温,与步骤(c1)的挥发油醇溶液混合均匀,即可。所述活血通络药酒的含醇量为50-60%。
现代药理和毒理学研究表明,红花中的红花黄色素能够扩张周围毛细血管,抑制血小板聚集,增加纤维蛋白溶解,从而起到活血化瘀,小志志天,抗血栓形成的作用,可加快水肿和皮下淤血的吸收,同时还能一定程度上缓解局部疼痛。红花黄色素为水溶性成分,因此本发明采用水来提取红花中的红花黄色素。三七中的三七皂甙可抗血小板聚集,促进纤维蛋白溶解,从而抑制血栓形成,达到活血功能,并促进多功能造血干细胞的增值,化瘀血而不伤新血。桃仁含苦杏仁甙约3.6%,挥发油0.4%,脂肪油45%,提取物有舒张血管、抑制血液凝固和溶血作用。乳香和没药含挥发油成分较多,具有清热解毒、消肿止痛、活血生肌等作用。挥发油在空气中易树脂化,因此采用醇提后密封保存。莪术提取物可抑制血小板聚集和抗血栓形成,挥发油试管内抑制金黄色葡萄球菌、β-溶血性链球菌、大肠杆菌、伤寒杆菌、霍乱弧菌具有活血、抗炎作用。
本发明由于采用了上述技术方案,具有以下有益效果:
1、本发明的组合物根据中医理论和西医理论相结合的原理,优选了所述的中药原材料,科学配伍,使各药相互配合,取长补短,具有活血祛瘀,消肿止痛,收敛生肌等作用。对于动静脉内瘘术前、术后局部气血凝滞,静脉不通,脉络瘀阻者均可使用。使动静脉内瘘血管得以濡养,气充血足,脉道通畅,减少血管狭窄以及钙化、血肿以及血栓形成等并发症,促进内瘘成熟,保障维持性血液透析患者动静脉内瘘的长期有效使用。并且可改善内瘘周围组织血液循环,增强新陈代谢,促进肿胀消退及皮肤颜色的恢复。具有起效快,局部给药副作用小,安全性高,使用方便等优点。
2、本发明对于中药原材料活性成分的提取,首先将中药材颗粒浸泡在乙醇内进行增压保压,在极短的时间内泄压至常压,有效破坏中药材的表面和内部结构,使其结构变得疏松,泄压过程即可用无水乙醇提取中药材中的挥发油成分,避免挥发油损失或被氧化。其次,由于结构变得疏松,可在低温或常温下快速提取其中的天然活性成分,避免了热效应对植物天然活性成分的破坏。
3、本发明的组合物除了可制成软膏剂、酊剂、凝胶剂外外用制剂外,还可直接将组合物粉碎后浸泡于米酒中,制成药酒使用,制备方法简单,可长期保存。
4、经过临床观察,使用本发明的制剂后,在穿刺点周围血管外观清楚,充盈充实,内瘘触摸搏动有力,震颤感强,说明本发明制剂对内瘘的修复保护具有较好的效果。
5、本发明的制剂可制备成多种剂型,携带、使用方便安全,减少或避免口服药或涂抹激素制剂带来的危险性、痛苦性和副作用,减轻患者痛苦。制备方法简单,制备成本低,疗程短,见效快,适合人群广泛,具有广阔的应用前景。

实施例1
一种用于动静脉内瘘穿刺后修复的活血通络药酒,由如下中药原料制成:红花10kg,三七5kg,桃仁10kg,没药10kg,乳香10kg,莪术10kg。将上述中药原料粉碎,按1:1的体积比加入50度的米酒,密封浸泡30天,过滤,即得。
实施例2
一种用于动静脉内瘘穿刺后修复的活血通络药酒,由如下中药原料制成:红花5kg,三七2kg,桃仁5kg,没药5kg,乳香15kg,莪术15kg。
具体制备方法如下:
(c1)将没药和乳香粉碎成粗粉,用无水乙醇作溶剂,浸渍18h后以2ml/分钟流速进行渗漉,收集漉液10倍量,离心,静置分层,取上清液密封,得到挥发油醇溶液;
(c2)将步骤(c1)的药渣与其他原料混合后加入3倍量的水,在75℃下回流提取3次,每次时间分布为4h,2h,1h,合并提取液,过滤,在50℃下浓缩,冷却至室温,与步骤(c1)的挥发油醇溶液混合均匀,即可,且控制提取液的浓缩进程,使浓缩后的提取液与挥发油醇溶液混合后,混合溶液的含醇量为50%。
实施例3
一种用于动静脉内瘘穿刺后修复的活血通络软膏剂,由如下中药原料制成:红花5kg,三七2kg,桃仁5kg,没药5kg,乳香15kg,莪术15kg。
具体制备方法如下:
(a1)取上述中药原料,粉碎成粒径为500μm的粉体,置于高压罐中并加入中药原料总重量3倍的无水乙醇,在22℃、12MPa下保压3min,然后将高压罐在35s内泄压至常压,分离高压罐的药渣和药液;
(a2)取药渣,加水减压提取2次,每次加水量分别为药渣总重量的6倍,3倍,每次提取时间分别为3h,2h,合并提取液,过滤,在5000rpm的转速下离心5min,待滤液分层,取上层提取液与步骤(a1)的药液合并,得挥发油提取物液,蒸发乙醇,得挥发油成分;
(a3)将步骤(a2)的下层溶液浓缩至65℃时相对密度为1.10的清膏,得到水溶性活性成分;
(a4)取凡士林、硬脂酸、蜂蜡、动物油以及十六醇,混合加热至80℃,搅匀,作为油相,备用;领取十二烷基硫酸钠、薄荷醇以及月桂醇加入有效成分中,再加入蒸馏水,搅匀作为水相,加热至85℃;
(a5)分别将挥发油成分加入油相中,将水溶性活性成分加入水相中,待各相中的有效成分分散后,恒温至70℃,搅匀,将油相缓缓加入水相,置于均浆机中搅拌,搅拌,冷却,凝固,分装,即得。
实施例4
一种用于动静脉内瘘穿刺后修复的活血通络凝胶剂,由如下中药原料制成:红花15kg,三七7kg,桃仁15kg,没药12kg,乳香5kg,莪术5kg。
具体制备方法如下:
(a1)取上述中药原料,粉碎成粒径为300μm的粉体,置于高压罐中并加入中药原料总重量8倍的无水乙醇,在24℃、10MPa下保压3min,然后将高压罐在60s内泄压至常压,分离高压罐的药渣和药液;
(a2)取药渣,加水减压提取2次,每次加水量为药渣总重量的6倍,4倍,每次提取时间为2h,1h,合并提取液,过滤,在4000rpm的转速下离心10min,待滤液分层,取上层提取液与步骤(a1)的药液合并,得挥发油提取物液,蒸发乙醇,得挥发油成分;
(a3)将步骤(a2)的下层溶液浓缩至65℃时相对密度为1.20的清膏,得到水溶性活性成分;
(a4)将适量卡波姆撒于蒸馏水中,使卡波姆充分溶胀,加入丙二醇、月桂氮卓酮研磨使润湿,水浴至65℃,取丙二醇、吐温-80混合加热溶解后,与水溶性有效成分、挥发油成分混匀,冷却,即得本发明凝胶剂。
实施例5
一种用于动静脉内瘘穿刺后修复的活血通络酊剂,由如下中药原料制成:红花8kg,三七3kg,桃仁12kg,没药15kg,乳香9kg,莪术11kg。
具体制备方法如下:
(b1)将没药和乳香粉碎成粗粉,用体积分数为60%的乙醇作溶剂,浸渍36h后以1ml/分钟流速进行渗漉,收集漉液10倍量,离心,静置分层,上层溶液密封,得到挥发油成分,下层溶液备用;
(b2)将步骤(b1)的药渣与其他原料混合后加水浸渍1h煎煮,共煎煮3次,每次2h,合并煎液,过滤,与步骤(b1)的下层溶液合并,浓缩至75℃下相对密度为1.10的浓缩液,冷却至室温,加无水乙醇使含醇量50%,搅匀,静置24h,过滤,减压回收乙醇,然后继续浓缩至60℃下相对密度为1.10的清膏,得到水溶性活性成分;
(b3)在挥发油成分中加入聚乙二醇,混匀后再加入水溶性活性成分,加入0.05%硫脲,混匀,分装,即得本发明酊剂;聚乙二醇的用量为挥发油成分和水溶性活性成分总重量的2倍。
本发明所列举的各原料,以及本发明各原料的上下限、区间取值,以及工艺参数的上下限、区间取值都能实现本发明,在此不一一列举实施例。
临床试验观察
选择200例维持性血液透析治疗动静脉内瘘患者,按照随机数表法分为观察组和对照组各100例,两组患者性别、年龄、病程、文化程度比较,差异无统计学意义(P>0.05),全部患者均为自体内瘘血管通路,均采用内瘘针按无菌原则穿刺,透析规律3次/周,4h/次。观察时间8周。
对照组:采用传统内瘘止血包扎方法,无菌纱布棉球加压包扎内瘘穿刺部位的方法处理。
观察组:将实施例1的药酒2ml倒于无菌纱布棉球上加压包扎于内瘘穿刺部位,待穿刺部位止血后,挤黄豆粒大小的实施例2的软膏剂于食指腹,按螺旋形轻轻涂抹与针眼部位处,1-2次/d。
观察指标:
(1)刺点止血时间:30min内止血为优,大于30min止血为差。
(2)穿刺点周围血管弹性:血管外观清楚,充盈充实,内瘘触摸搏动有力,震颤感强为优良;血管外观不清,呈条索状,周围触之有硬结,血管塌陷,内瘘触摸搏动弱,无震颤为差。
(3)硬结发生:触摸无血管硬结为优,反之为差。
(4)拔针2h后疼痛程度:采用数字评分法(NRS)评估,0分为无痛,1-3分为轻度疼痛,4-6分为中度疼痛,7-10分为重度疼痛。
观察结果见下表。

如上表所示,使用本发明的制剂的患者,内瘘止血快,几乎无硬结发生,拔针后疼痛感轻,说明本发明制剂对内瘘修复具有较好的效果。

1.一种用于内瘘穿刺后修复的活血通络组合物,其特征在于,由如下重量份的中药原料制成:红花5-15份,三七2-7份,桃仁5-15份,没药5-15份,乳香5-15份,莪术5-15份。

(a1)取上述中药原料,粉碎成粒径为200-500μm的粉体,置于高压罐中并加入中药原料总重量2-10倍的无水乙醇,在22-24℃、10-12MPa下保压2-3min,然后将高压罐在10-60s内泄压至常压,分离高压罐的药渣和药液;
(a2)取药渣,加水减压提取1-2次,每次加水量为药渣总重量的3-6倍,每次提取时间为1-3h,合并提取液,过滤,在4000-5000rpm的转速下离心4-10min,待滤液分层,取上层提取液与步骤(a1)的药液合并,得挥发油提取物液,蒸发乙醇,得挥发油成分;
(a3)将步骤(a2)的下层溶液浓缩至60-65℃时相对密度为1.10-1.20的清膏,得到水溶性活性成分。
3.根据权利要求1所述的用于内瘘穿刺后修复的活血通络组合物,其特征在于,所述中药原料的活性成分的提取方法如下:
(b1)将没药和乳香粉碎成粗粉,用体积分数为50-70%的乙醇作溶剂,浸渍36-50h后以1-3ml/分钟流速进行渗漉,收集漉液10倍量,离心,静置分层,上层溶液密封,得到挥发油成分,下层溶液备用;
(b2)将步骤(b1)的药渣与其他原料混合后加水浸渍1h煎煮,共煎煮3次,每次2-3h,合并煎液,过滤,与步骤(b1)的下层溶液合并,浓缩至75℃下相对密度为1.05-1.10的浓缩液,冷却至室温,加无水乙醇使含醇量50%,搅匀,静置24h,过滤,减压回收乙醇,然后继续浓缩至60℃下相对密度为1.10-1.20的清膏,得到水溶性活性成分。
4.一种根据权利要求2或3所述的用于内瘘穿刺后修复的活血通络组合物的制备方法,其特征在于,将所述挥发油成分和水溶性活性成分共同制成外用制剂使用。
5.根据权利要求4所述的用于内瘘穿刺后修复的活血通络组合物的制备方法,其特征在于,所述外用制剂为凝胶剂、酊剂、软膏剂中的任意一种。
6.一种用于内瘘穿刺后修复的活血通络药酒的制备方法,其特征在于,根据权利要求1所述中药原料制备而成,具体制备方法如下:
将中药原料粉碎,按1:1-2的体积比加入50-60度的米酒,密封浸泡至少30天,过滤,即得。
7.一种用于内瘘穿刺后修复的活血通络药酒的制备方法,其特征在于,根据权利要求1所述中药原料制备而成,具体制备方法如下:
(c1)将没药和乳香粉碎成粗粉,用无水乙醇作溶剂,浸渍12-18h后以1-3ml/分钟流速进行渗漉,收集漉液10倍量,离心,静置分层,取上清液密封,得到挥发油醇溶液;
(c2)将步骤(c1)的药渣与其他原料混合后加入3-5倍量的水,在75-90℃下回流提取2-3次,每次1-4h,合并提取液,过滤,浓缩,冷却至室温,与步骤(c1)的挥发油醇溶液混合均匀,即可。
8.根据权利要求7所述用于内瘘穿刺后修复的活血通络药酒的制备方法,其特征在于,所述活血通络药酒的含醇量为50-60%。
9.根据权利要求1所述的用于内瘘穿刺后修复的活血通络组合物,其特征在于,由如下重量份的中药原料制成:红花10份,三七5份,桃仁10份,没药10份,乳香10份,莪术10份。
