Abstract

Description
技术领域
本发明涉及健体塑身、负重训练以及医疗领域中的血流控制的活动。在优选的形式中,本发明包括智能手机应用程序(app)操作的健身装置,该健身装置用于限制静脉血流以提升肌肉力量并促进增肌。
背景技术
虽然本发明涉及一种改进的独立式袖带和止血带,其不需要目前与此类设备相关联的线路、管道或装备,但是研究此类设备目前的用途和现有技术的状态是有益的。
血流限制训练
根据常规理论,为了刺激增肌,需要进行一次反复最大重量(1RM)测试的最少70%的举重。公认的是,低于该阈值的强度缺乏在肌肉组织的肌浆和肌原纤维中产生肌肉适应的基本刺激(Peterson等人,2011年)。然而,最近的研究表明使用低至预测1RM的20%的相对较低负荷结合血流限制(BFR)已经有效地刺激增肌和力量提升,类似于使用较高负荷所实现的那样(Abe等人,2005年)。
现有技术血流限制训练是一种增肌和力量提升刺激,其利用将特殊适合的压力袖带施加到四肢的顶部,例如大腿和/或手臂上部,所述特殊适合的压力袖带在整个锻炼期间被充气到预先确定的压力(Abe等人,2005年;Loenneke,2010年)。施加到四肢上的压力强到足以部分地限制从外周肌肉床到心脏的静脉血液回流,但是还不足以阻断向肌肉区域的动脉血液供应(Abe等人,2005年;Loenneke,2010年)。
血流限制(BFR)训练不应与缺血条件下的训练混淆。BFR不会诱发骨骼肌内的局部缺血,而是改善四肢肌肉组织内毛细血管中的血液蓄积状态,因为在整个训练负荷或锻炼方案中,静脉回流仅轻微受损,平均动脉压下的肌床灌注维持不变(Abe等人,2005年)。
医疗止血带
医疗止血带的使用已被广泛记载。在门诊中,止血带不那么关键,通常包括弹性带或其他手动操作的带。止血带无法建立准确的压力水平,仅用于阻断静脉回流或在极端情况下阻断向四肢的动脉血液供应以控制失血。经常连接到通常为便携的模拟计量器的这些简单阻断袖带包括给空气袖带充气的手动操作式泵。虽然这是基本的、便携的且低成本的设备,但是该阻断袖带不能用于准确地监测肌肉床的灌注。较简单的设备诸如血流限制(BFR)带通常包括弹性材料以限制血流。这些设备价格便宜且用途广泛,但无法测量或保持压力,并且始终不与监测系统相关联。
在患者的强化治疗中,其中对循环系统的控制是至关重要的,连接到数字压力监测器和泵的机器操作式止血带允许通过对阻碍血流的袖带进行受控充气来准确地施加压力。这在需要周期性地发生受损四肢的再灌注的情况下尤其重要。此类止血带必须被编程为定期放气以允许动脉流入到达受影响区域中,从而帮助减轻缺氧造成组织损伤的风险。此类现有技术再灌注止血带通常与在医院的急诊室(A&E)或重症监护室(ICU)中更常见的单独的泵和控制模块相关联。
相关的现有技术医疗止血带包括在日本广泛使用的Kaatsu止血带,该Kaatsu止血带包括具有经由塑料管道连接到空气袖带的电子监测器系统的机器。虽然该止血带具有包括相对便携和准确的电子监测器的优点,但它具有外部管道,因此对于动态锻炼或移动使用而言并不理想。Delphi血流限制袖带是一种数字机器,其通过连接到空气袖带的塑料管道连接到静脉(IV)柱。Delphi血流限制袖带可以准确地监测施加到四肢上的压力,但其价格昂贵、体积笨重、不便携带,并且需要连接到主电源上。Delphi血流限制袖带还需要使用外部管道,这限制了用户的移动性和/或运动自由。
发明内容
因此,在一个方面,本发明在于一种远程控制的血流限制袖带,该血流限制袖带包括:
被构造成围绕四肢定位的空气囊;
位于袖带上用于对空气囊加压的小型空气泵;
用于感测空气囊气压的压力传感器;
用于控制空气泵的操作的电子控制器,该控制器可针对预先确定的压力或压力范围进行调节,并接收和响应于来自压力传感器的信号;
位于袖带上用于向空气泵、控制器和压力传感器提供电力的电源,并且其中可以使用与控制器通信的远程设备来控制袖带的充气和放气。

g)袖带充气至期望的压力;
h)袖带利用响应于压力传感器的电子控制器控制空气泵,根据四肢运动所需对袖带进行充气和放气,从而将压力维持在袖带压力的指定范围内;
i)一旦计时器超时或用户停止程序,则袖带就会放气,并且
j)智能手机应用程序将袖带压力和持续时间记录在用户资料中,其中所存储的数据允许用户基于生理数据和所存储的袖带压力数据生成训练程序,并且使用户能够记录设置和重复次数,由此用户可以使用app用于其他健康参数跟踪。
在另一方面,本发明在于一种使用本文所述的远程控制的血流限制袖带作为医疗止血带的方法,该方法包括以下步骤:
a)将袖带施加到受伤四肢上;
b)通过远程设备或蓝牙协议智能手机app初始化袖带操作;
c)通过多普勒超声监测动脉流入;
d)通过设备或app将袖带压力增加到多普勒信号指示的动脉阻断压力;
e)自动维持由压力传感器测量的袖带动脉阻断压力,并响应于多普勒信号对袖带进行充气或放气;
f)通过远程设备或app对四肢的创伤性损伤进行评级,其中选择再灌注程序来自动允许短暂的袖带放气和再充气以用于四肢的再灌注,从而减轻不必要的缺氧性组织损伤的风险;
g)通过多普勒超声监测动脉流入,其中仅响应于多普勒信号对袖带进行放气和再充气,以根据再灌注程序减少失血量,其中
h)一旦可以提供完全的医疗或医院干预,便可以在医疗监督下对袖带进行放气并移除。
适当地,在可用或需要的情况下,可以使用脉搏血氧计替代或结合多普勒超声来监测动脉流入。
因此,有利的是提供一种用途广泛的静脉限制或阻断袖带而无需任何附接的管道或电缆以用于肌肉力量和增肌训练,但是该袖带也可以用作动脉再灌注止血带以在袖带压力增加时阻止创伤性失血而不会使组织缺氧。
同样有利的是,本发明是一种独立且相对小的装置,其允许在训练时具有更大的用户移动性并且减少了在存储或运输中占用的空间量。
此外,本发明可以用作独立式无线医疗止血带,以在紧急情况和创伤性生命危险情况下防止严重失血和缺氧性组织损伤。
本发明的改进涉及消除与现有技术袖带和止血带相关联的各种充气管和电缆的必要性,现有技术袖带和止血带不仅限制训练期间的运动,而且需要辅助装备诸如外部空气泵和操作这些空气泵所需的监测设备。
重要的是,本发明寻求为公众提供一种相对现有技术的商业替代方案和有益改进。

具体实施方式
下面结合附图对本发明的技术方案作进一步说明。
图1至图4示出了根据本发明的优选的远程控制血流限制袖带10的视图。袖带10被构造成围绕四肢(未示出)定位。模块15中的小型空气泵14通过连接到阀17并通过空气入口12向空气囊(由于缝合在袖带内部而不可见)供应空气的管13对空气囊加压。PCB板19上的压力传感器16感测到空气囊(由于缝合在袖带内部而不可见)中的气压。具有复位按钮21和复位开关21a的PCB板19上的基于微处理器的电子控制器18控制空气泵14的操作,并且对于预先确定的压力或压力范围是可调节的。微处理器控制器18接收并响应于从压力传感器16接收的信号。
电源20定位于袖带上,以向空气泵14和控制器18供电,其中使用与控制器18通信的远程设诸如智能手机(未示出)来控制空气囊的充气和放气。
如前文所述,优选地,控制器18使袖带能够充气至并保持在高达350mmHg的压力下。该充气压力高于大多数用户收缩压,有助于将袖带用作医疗止血带以阻止动脉失血。空气泵14为电池操作式,电源20为可充电电池电源。
如图所示,空气泵14、电源20、压力传感器16和控制器18容纳在袖带10上的模块22中。
在优选的实施方案中,袖带10的操作通过与智能计算机或手机应用程序或其他等效无线系统(例如插入USB端口23中的USB编程记忆棒(未示出))相关联的无线协议诸如蓝牙来远程控制。
优选地,压力传感器16检测精确至1mmHg的压力,其中通过控制器18响应于来自压力传感器16的信号来操作空气泵14,而将空气囊中的压力维持在预先确定的压力水平。

图5示出了图1至图4的实施方案,其用作训练袖带10以通过举抬哑铃60、62来塑造和加强(在这种情况下)上臂30、32的肌肉。
袖带10被施加到上臂30、32。用户40通过具有在蓝牙协议上运行的app 52的智能手机50初始化袖带的控制器并且与其连接。
连接后,用户40响应于锻炼前问卷,通过app 52输入生理数据(年龄、身高、病史、性别、四肢围度尺寸、体脂水平、心率和血压)以及所需的任何其他必要的锻炼前信息。
然后,用户40选择最合适的训练程序和袖带压力范围,并为所选择的训练程序设置计时器。用户按下由app显示的开始按钮53。
袖带10充气至期望压力,并且利用响应于压力传感器的电子控制器控制空气泵,该空气泵根据任何四肢运动对袖带进行充气和放气来维持压力。
一旦计时器超时或用户停止程序,则袖带就会放气,并且智能手机app 52将袖带压力和持续时间记录在用户资料中。
随着智能手机应用程序通过蓝牙持续且无线地监测生理数据(动脉流入、脉搏率和血氧饱和度),该生理数据通过与年龄、体重、四肢围度和去脂体重相结合的算法来处理。此信息用于创建个性化的用户资料,以建立最安全的压力参数(以“四肢阻断压力”的百分比表示),并根据每个用户的需求定制为增肌/力量诱导血流限制训练设备或救生医疗止血带设备(参见下文)。在训练环境中,智能手机应用程序将存储每个会话的数据、使用的压力、持续时间和生理数据,这些数据可用于创建定制的用户血流限制锻炼程序。
图6示出了图1至图4的用作医疗止血带的实施方案。
将袖带10施加到具有被绷带67包扎的伤口的受伤四肢65上。用户通过具有在蓝牙协议上运行的app 72的智能手机70初始化袖带的控制器并且与其连接。通过多普勒超声拾取器74监测到四肢65的动脉流入。然后,用户通过该app将袖带压力增加到多普勒信号衰减指示的动脉阻断压力。控制器通过响应于多普勒信号对袖带进行充气或放气来自动维持由压力传感器测量的袖带动脉阻断压力。
用户通过该app对四肢的创伤性损伤进行评级,其中选择再灌注程序来指示控制器短暂的袖带放气和再充气以用于四肢的再灌注,从而减轻不必要的缺氧性组织损伤的风险。
控制器监测动脉流入,其中仅响应于多普勒信号对袖带进行放气和再充气,以根据再灌注程序减少失血量。数字脉搏血氧计76也可用于监测血氧饱和度和心率。
一旦可以提供完全的医疗或医院干预,便可以在医疗监督下对袖带进行放气并移除。
医疗止血带袖带和应用程序利用智能手机或无线界面显示单元驱动的血流限制系统,通过使用完全独立的数字气动式袖带以采用自调节压力传感器阻止静脉动脉回流来实现血流限制,包括通过连续波多普勒超声和/或脉搏血氧计所实现的血流限制或阻断监测。
连续波多普勒超声具有以下主要功能:
通过位于动脉或心脏上方的皮肤发送和接收超声波,当这些超声波从运动物体诸如穿过动脉的血液处反射时,反射频率发生轻微变化。然后由多普勒超声单元的电子器件分析该变化,并将其转换成心输出量和心率的数字显示。
脉搏血氧计具有两项主要功能,如下:
提供类似于多普勒超声的脉搏血流的可听信号。以及当压力袖带阻断动脉时,血氧计会丢失信号,此时测量血液或组织中血红蛋白的氧饱和度。
连续波多普勒超声和脉搏血氧计用于测量在四肢上袖带位置远侧的脉搏波传播时间。脉搏波传播时间定义为动脉脉搏波沿着固定路径传播所需的时间,用于确定“四肢阻断压力”。可以通过监测止血带施加点远侧的脉搏来进行此测量。不存在远侧脉搏频率的最小动脉压力可确定为“四肢阻断压力”。
以下包括在使用本发明时可以采用的流程的实例。
首次使用并创建个性化的智能手机应用程序用户资料。
向患者解释流程;
确保他/她以半卧位舒适地躺卧。
阶段1-使用脉搏血氧计测量上肢压力。
将合适尺寸的血压袖带围绕患者的上臂固定;
将脉搏血氧计传感器放置在任一手指上(图1a)。将传感器放置在手指上时,血氧计将显示两个数字:第一个表示患者的心律,第二个表示循环的含氧血红蛋白的百分比。脉搏血流也可以通过波形或灯带在血氧计上显示;
记录基线读数。将袖带充气至60mmHg,然后以10mmHg的增量充气,每次充气之间间隔大约10秒。一旦压力达到100mmHg后,增量可以增加到20mmHg;
记录低于脉搏血氧计上可听或脉搏信号丢失点的压力读数;例如,如果信号在180mmHg下丢失,则记录160mmHg的压力;
在另一只手臂上重复测量,然后使用两个读数中的较高者来计算“四肢阻断压力”。
阶段2-使用脉搏血氧计测量下肢压力。
将合适尺寸的袖带围绕最大的最近侧大腿区域固定。
将血氧计传感器放置在前三趾中的一个上(图1b)。
按照阶段1的说明对袖带进行充气,并记录信号丢失时的压力。
在另一条腿上重复测量,然后使用两个读数中的较高者来计算“四肢阻断压力”指数。
动脉脉搏停止时的压力对应于当时阻断动脉的最小止血带袖带压力或“四肢阻断压力”。

该袖带穿戴在上肢(例如上臂)和下肢(例如大腿)上。优选地,该袖带是5cm或10cm宽的气动式袖带,包括皮革或硅树脂的外部袖带材料,并且在材料层之间具有气囊。袖带优选地使用棘轮或带扣或维可牢尼龙搭扣环紧固系统来施加。上肢的气压式袖带长度为25cm至50cm,下肢的气动式袖带长度为50至75cm。
气囊通过双向阀连接到模块单元,该双向阀可以关闭以允许将压力保持并维持在期望的压力水平,且压力传感器和控制器通过对空气压力进行小的调整以允许维持期望的压力,而不管四肢运动如何。袖带模块包含容纳在塑料外壳中的内置电池、压力传感器和空气泵,该塑料外壳可以允许气囊充气并增加气囊中的压力(mmHg)。内置传感器介导的压力增加将压力增加到0-350mmHg之间的预先确定的水平,并在使用期间维持设定压力。

显而易见的是,使用血氧计的优点是容易获得脉搏率和血氧饱和度水平。
本发明是一种无外部导线或管道的数字止血带设备,并且是一种完全自封闭的气动空气袖带。这些设备被设计成在创伤事件中用作医疗止血带来阻止血流,并在血流限制训练中用于健身。
这些设备是数字、无线和气动袖带式的血流限制设备,用于穿戴在上肢和下肢上。本发明的血流限制或阻断设备为电子的、完全无线的和无管的包括集成电源的设计。该设备是独立式的,但与适于穿戴在用户四肢上的自由控制器互动。
该设备测量用户的基于个性化生理数据的个性化压力区间,并维持实时且持续施加在四肢上的限制压力,并且考虑四肢的运动。这意味着该设备会根据用户的个性化压力区间进行调整来增加/减小压力,从而维持预选压力。
生理数据可以包括血流、脉搏率、血压、四肢阻断压力(LOP)和动脉阻断压力(AOP)。血流和脉搏率的生理数据可以通过集成和现成的设备以及智能手机app来确定,并通过以下方法收集:

动脉阻断压力(AOP)是维持无血术野所需的袖带压力的量度。
本发明的设备执行以下计算:-

四肢阻断压力
四肢阻断压力(LOP)可定义为阻止动脉血液流向四肢中袖带远侧所需的最小压力。通过逐渐增加止血带压力直至远侧血流中断来确定LOP。
通过测量LOP并相应地选择袖带充气压力,可以明显降低止血带相关并发症的风险。最佳实践建议,最佳袖带压力应取决于患者的LOP。应尽可能降低止血带压力,较低的压力被认为能防止正常组织的损伤。
本发明的设备根据以下步骤计算LOP:-
1.用户打开控制器
2.用户将设备放置在四肢上
3.该设备使用以下方法来监测用户四肢中的血流:
■光电体积描记
■生物阻抗
■近红外光谱(NIRS)
■多普勒超声
4.该设备以5-10mmHg的增量充气,直到脉搏停止
5.该设备使用前述测量血流和脉搏的方法来感测在什么压力水平下不再检测到脉搏率
6.阻止血液流向四肢端部所需的最小压力称为四肢阻断压力(LOP)
7.血流限制参数取决于LOP
8.根据用户的选择,袖带将充气至LOP的期望百分比(50-80%)
9.用户开始使用具有所选择的LOP压力的设备
10.然后将袖带充气并维持在所选择的AOP百分比,设备使压力在该区间保持恒定,从而允许随四肢/身体运动进行调整。
LOP测量
将通过(a)触诊、(b)多普勒超声、(c)使用远侧光电体积描记传感器、(d)NIRS和(e)生物阻抗手动测量个体四肢阻断压力。
止血带袖带自动充气,并且利用前述探针在袖带水平处和止血带袖带远侧处检测四肢的动脉搏动。
计算LOP的益处
在BFR中应用LOP时,使用外科级自动化设备正确测量LOP,可通过定制老年人的康复、改善受伤运动员的体能以及患者从重大外科手术(诸如膝关节镜)中恢复的血流限制训练,从而辅助达到最佳的康复结果。
将本发明的设备充气至0-350mmHg之间的压力。存在容纳电池操作式空气泵的内置模块。内置压力传感器或监测器检测精确至1mmHg的期望压力。压力在所需的持续时间内维持在期望的压力水平,并且针对四肢运动的压力变化而进行校正。然后用户释放压力。该设备优选地通过蓝牙或无线控制器以及随附的智能手机应用程序控制。与以较高负荷的锻炼相比,该设备将与低负荷阻力训练结合使用(也可不与之一起使用)以增加肌肉尺寸和力量。
在必要的强度下锻炼可能导致受伤,并且将从锻炼中受益最多的人通常不能参与,例如伤患或病患。通常,为了获得积极的肌肉适应,个体必须在其最大水平的80%以上进行锻炼。然而,根据文献,当最大负荷10-30%的低负荷锻炼与血流限制结合时,在相近的时间范围内可以获得与非血流限制训练相同的结果。
目前市场上的现有技术存在两个极端版本。其中包括不便携带且不易使用的体积笨重、有线且重型的医疗设备,以及无法提供准确的生理数据、容易因过度收紧而使用不当且价格昂贵的弹性止血带。
总之,本发明可被描述为在血流限制训练中具有双重作用的无线操作的无管气动式袖带系统,以及用于医疗紧急情况的便携式止血带。作为无线操作的止血带,其能够通过内置的压力传感器和控制器将施加在四肢上的压力维持在期望水平。该设备的主要优点在于完全无线、便携且独立,而无需将多余的管道连接到辅助装备。所有的电子器件和空气囊都被包裹在袖带内。该设备通过用作数字医疗止血带以在较高袖带压力下阻止创伤性失血而应用于医疗领域,以及结合低负荷阻力训练以提升肌肉力量和增肌而应用于健身行业。
该设备通过智能手机应用程序方便地操作,该智能手机应用程序响应于包括动脉脉搏率和血氧饱和度水平的生理数据而创建用户资料,而后用户资料可被编程为根据定制的训练协议自动地控制阻断压力水平。

Claims (16)
1.一种远程控制的血流限制袖带,包括:
被构造成围绕四肢定位的空气囊;
位于所述袖带上用于对所述空气囊加压的小型空气泵;
用于感测空气囊气压的压力传感器;
用于控制所述空气泵的操作的电子控制器,所述控制器可针对预先确定的压力或压力范围进行调节,并接收和响应于来自所述压力传感器的信号;
位于所述袖带上用于向所述空气泵、所述控制器和所述压力传感器提供电力的电源,并且其中可以使用与所述控制器通信的远程设备来控制所述袖带的充气和放气。
2.根据权利要求1所述的限制袖带,其中所述控制器使所述袖带能够充气至并保持在高达350mmHg的压力下。
3.根据权利要求1所述的限制袖带,其中所述空气泵是电池操作式空气泵。
4.根据权利要求1所述的限制袖带,其中所述电源是可充电电池电源。
5.根据权利要求1所述的限制袖带,其中所述空气泵、所述电源、所述压力传感器和所述控制器容纳在所述空气囊近侧的模块中且均位于所述袖带上。
6.根据权利要求1所述的限制袖带,其中所述袖带的操作通过与智能计算机或手机应用程序相关联的无线协议诸如蓝牙来远程控制。
7.根据权利要求1所述的限制袖带,其中所述压力传感器检测所述袖带中的压力,所述压力通过所述控制器响应于来自所述压力传感器的信号来操作所述空气泵而被维持在预先确定的压力水平。
8.根据权利要求1所述的限制袖带,其中接收来自所述压力传感器的信号的所述控制器考虑由于四肢运动而引起的压力变化。
9.根据权利要求1所述的限制袖带,其中所述袖带通过可释放的紧固装置围绕四肢定位。
10.根据权利要求1所述的限制袖带,其中所述袖带的所述操作由智能手机应用程序控制,并且其中所述应用程序包括用于肌肉塑造和肌肉力量的定制程序、计时器、压力控制、紧急停止,以及存储锻炼变量数据诸如重复次数、设置和压力历史的相关生理数据跟踪。
11.根据权利要求1所述的限制袖带,所述限制袖带被用作医疗止血带,允许通过智能手机应用程序以电子方式增加和减小压力,其中压力设置在期望水平,并且所述压力传感器和所述空气泵进行微调节以保持所述袖带压力恒定在所述期望水平。
12.根据权利要求1所述的限制袖带,所述限制袖带被穿戴在近侧上肢和近侧下肢上。
13.根据权利要求1所述的限制袖带,其中所述袖带是5cm或10cm的皮革或尼龙或硅树脂袖带,具有棘轮或带扣或维可牢搭扣环紧固系统。
14.一种使用远程控制的血流限制袖带来塑造和/或增强肌肉的方法,所述方法包括以下步骤:
a)将袖带施加到四肢上;
b)通过远程设备或蓝牙协议智能手机应用程序初始化并连接袖带;
c)连接后,用户响应于锻炼前问卷,通过所述应用程序输入生理数据,年龄、身高、病史、性别、四肢围度尺寸、体脂水平、心率和血压以及任何其他必要的锻炼前信息;
d)所述用户选择最合适的训练程序和袖带压力范围,所述压力单位为mmHg;
e)所述用户为所述选择的训练程序设置计时器;
f)所述用户按下所述远程设备或所述应用程序的开始按钮;
g)所述袖带充气至期望的压力;
h)所述袖带利用响应于压力传感器的电子控制器控制空气泵,根据四肢运动所需对所述袖带进行充气和放气,从而将压力维持在袖带压力的指定范围内;
i)一旦所述计时器超时或所述用户停止所述程序,则所述袖带就会放气,并且
j)所述智能手机应用程序将袖带压力和持续时间记录在用户资料中,其中所存储的数据允许所述用户基于所述生理数据和所存储的袖带压力数据生成训练程序,并且使所述用户能够记录设置和重复次数,由此所述用户可以使用所述应用程序用于其他健康参数跟踪。
15.一种使用远程控制的血流限制袖带作为医疗止血带的方法,所述方法包括以下步骤:
a)将所述袖带施加到受伤四肢上;
b)通过远程设备或蓝牙协议智能手机应用程序初始化袖带操作;
c)通过多普勒超声监测动脉流入;
d)通过所述设备或所述应用程序将袖带压力增加到所述多普勒信号指示的动脉阻断压力;
e)自动维持由压力传感器测量的袖带动脉阻断压力,并响应于所述多普勒信号对所述袖带进行充气或放气;
f)通过所述远程设备或所述应用程序对四肢的创伤性损伤进行评级,其中选择再灌注程序来自动允许短暂的袖带放气和再充气以用于四肢的再灌注,从而减轻不必要的缺氧性组织损伤的风险;
g)所述通过多普勒超声监测动脉流入,其中仅响应于所述多普勒信号对所述袖带进行放气和再充气,以根据所述再灌注程序减少失血量,其中
h)一旦可以提供完全的医疗或医院干预,便可以在医疗监督下对所述袖带进行放气并移除。
16.根据权利要求15所述的方法,包括以下步骤:使用脉搏血氧计替代或结合所述多普勒超声来监测所述动脉流入。
