背景技术
纤毛是基于微管的细胞表面突起,其源于基体、膜对接中心粒。初级纤毛是在大多数生长受到遏制的或分化的哺乳动物细胞的表面上以单拷贝存在的非运动感觉细胞器。纤毛感觉流改变并介导发育和组织稳态期间必不可少的信号传导途径,诸如Hedgehog、Wnt/PCP 和cAMP/PKA信号传导。鞭毛内运输(IFT)选择纤毛基部处的货物并且运输纤毛组装所需的轴丝组分以及参与纤毛信号传导的蛋白质。一旦纤毛形成,对纤毛膜组成的控制就依赖于离散的分子机器,包括阻止膜蛋白在纤毛基部的一个特殊区域(称为过渡区)处进入纤毛的屏障,以及控制G蛋白偶联受体(GPCR)在纤毛上的定位的转运衔接子,其被称为BBSome(Bardet–Biedl综合征(BBS)蛋白与作为基体的组分并参与将货物转运到初级纤毛的其他蛋白质的复合物)。纤毛发生需要许多过程的配合。复杂的细胞周期调控、膜泡转运和纤毛延伸的协调必须以精确的时序发生才能产生纤毛。在胚胎发育期间和在成人生理中产生和维持适当分化的纤毛的重要性通过大量与纤毛病相关联的人类疾病而被最佳地阐明。
纤毛病是一组由纤毛形成或功能中的缺陷直接导致的人类病症。缺陷性初级纤毛导致多向性和高度可变的异常,这与初级纤毛的广泛组织分布以及它们的范围广泛的功能一致。患有原发性纤毛病的个体表现出以下的组合:肾和视网膜异常、可能导致智力低下的中枢神经系统缺陷、肝缺陷(包括囊肿)、肥胖症以及各种骨骼缺陷(包括肢体长度的异常、趾数的异常(多趾症)、左/右轴组织的异常(内脏逆位(situs inversus)和颅面型的异常)。对于连接纤毛的光感受器具有特异性的异常还可能导致视网膜变性和失明。原发性纤毛病的实例包括肾耗竭 (NPHP)、Senior Loken综合征(SLS)、Joubert综合征(JBTS)、BardetBiedl综合征(BBS)、Meckel Gruber综合征(MKS)、口面指综合征(OFD) 和热纳综合征(Jeune syndrome,JATD)。

发明内容

NPHP患者

编码NPHP模块蛋白的一个或多个基因中的突变和/或失活可能会不利地影响纤毛发生和/或上皮化,从而导致NPHP患者的纤维化和囊肿发展。IFT机器选择纤毛基部处的货物并且运输纤毛组装所需的轴丝组分和参与纤毛信号传导的蛋白质。由16种不同的蛋白质组成的IFT-B复合物通过与驱动蛋白II(kinesin II)缔和来介导顺行运输。逆行运输由动力蛋白2和IFT-A复合物的6个亚基介导。已经在NPHP 相关纤毛病中鉴定出6种编码IFT-A亚基的基因的突变,仅3个IFT-B 亚基与肾耗竭相关联(IFT172,IFT54)(Halbritter,J.等人,Am.J.Hum. 关于NPHP模块,Nphp4突变体小鼠发展视网膜变性,但不发展肾囊肿,也不发展严重的纤毛发生缺陷;雄性不能生殖并且呈递运动减弱的精子(Won,J.等人,Hum.Mol.Genet.,20:482-96,2011)。类似地,小鼠中Nphp1的靶向性破坏(最后C-末端外显子20的缺失)不产生肾耗竭,但表现出在P14-P21处开始的快速视网膜变性(Jiang,S.等人,Hum.Mol.Genet.,17:3368-79,2008)并导致雄性不能生殖(Jiang,S.等人,Hum.Mol.Genet.,18:1566-77,2009)。Cep290敲除小鼠缺乏光感受器中的连接纤毛并且无法使运动室管膜纤毛成熟,这与其视网膜变性和脑积水表型一致(Rachel,R.等人,Hum.Mol.Genet.,24:3775-91, 2015)。
NPHP1中的突变是NPHP的最常见原因。在一大群成年发病 ESRD患者(未针对病因学进行选择)中,由NPHP1纯合全基因缺失 (NPHP(del))引起的NPHP在所有成年发病ESRD中具有1个/200个患者的流行率(0.5%)(Snoek,R.等人,J.Am.Soc.Nephrol.,29:772-9,2018)。虽然在18岁与50岁之间的ESRD发病患者中发病率明显更高(0.9%的流行率),但是NPHP的发病年龄可高达61岁。因为他们使用的方法低估了病因突变的总数,所以他们得出结论,NPHP是在当前的日常实践中可能未得到全面诊断的成人发病ESRD的相对频繁的单基因病因。
在来自移植网络中的国际遗传学与翻译研究(International Genetics andTranslational Research in Transplantation Network, iGenTRAiN)联盟的肾移植受体和(对应供体)对照的群组中,在ESRD (18至50岁)成人中鉴定了大约0.5%的相对频率(5606个中有26个) 的纯合NPHP1缺失患者。在这些中,仅13%(26个中有3个)被正确诊断为NPHP,并且大约一半(26%个中有11个)被诊断为病因不明的 CKD患者。这些结果显示,高达1/200(0.5%)的ESRD成人是 NPHP1del基因型;当ESRD发病年龄在18岁和50岁之间时,此数字增加至0.9%(Abstract.ASN2017&Nephr Dial Trans,第32卷, 2017)。

本文描述了用于鉴定可用于治疗纤毛病相关疾病或病症例如 NPHP或NPHP1(del)相关疾病或病症的治疗剂的材料和方法。此类方法可以包括使用来源于患者的细胞系。如所开发的此类细胞系还可以用于其他相关方法,包括例如监测给定治疗对纤毛病相关疾病或病症或者NPHP1(del)相关疾病或病症的功效。
为了鉴定用于治疗与纤毛病相关联的疾病(诸如,例如NPHP)的化合物,获得NPHP患者来源的细胞并且建立细胞系。简而言之,通过SV40 T抗原的逆转录病毒基因转移来使剥离的肾上皮细胞永生化,所述肾上皮细胞主要是从NPHP1缺陷患者的尿液回收的近端肾小管细胞(tbc)。将细胞固定,并且用Hoechst(针对核染色)、抗γ微管蛋白抗体(针对基体染色)和抗ARL13B抗体(针对纤毛染色)进行荧光标记,以使用免疫荧光显微镜进行检测。与每个细胞上具有单个纤毛(图1A)的大多数正常的尿液来源的肾上皮细胞(UREC)相反,大多数NPHP患者来源的细胞不具有纤毛(图1B)。进一步通过RT-PCR(图 1C)和免疫印迹(图1D)证实了这些NPHP患者来源的细胞中NPHP表达的缺乏,这两种方法分别没有显示NPHP患者来源的细胞中NPHP RNA和NPHP蛋白表达的可检测水平。
图2示出了可以用于定量感兴趣的细胞中的纤毛发生的自动体外测定。简而言之,将NPHP患者来源的细胞和对照细胞在完全培养基中在39C(SV40表达的非允许温度)下培养,接着使用免疫荧光显微镜进行自动纤毛分析来测量纤毛发生,例如以%纤毛计。可以使用旋转盘平台进行药物筛选(图5,A-J)和G3多组学数据集的纤毛发生分析(图29,A-E)。可以使用Opera Phenix平台进行其他表型分析(例如,前列地尔和CP-544326纤毛发生滴定、基于纤毛发生的其他EP 激动剂筛选、使用其他NPHP1患者来源的细胞系的纤毛发生、a微管蛋白乙酰化分析)。
图3显示,来自NPHP患者的纤毛细胞(PT1)的百分比显著低于在对照细胞(CTRL)中发现的百分比(p=0.0065)。
药物筛选
上面所述的基于纤毛的测定可以用于鉴定恢复纤毛发生的化合物。图4示出了基于纤毛的测定的过程,其中在第0天可以将细胞接种在细胞培养物(例如,96孔板)中,在第3天将细胞与药物候选物一起温育,并且在例如第5天固定细胞并用Hoechst、抗y微管蛋白抗体和抗ARL13B抗体进行荧光标记。例如,可以执行35个图像/孔的自动随机采集。每个图像可以具有以<1μm间隔拍摄的10个图像的z 堆叠。可以获得核(Hoechst,在461nm处)、基体(γ微管蛋白,在555 nm处)和纤毛(ARL13b,在647nm处)的连续成像。
使用图4中所示的过程,将NPHP患者来源的细胞用若干药物候选物处理以鉴定可以恢复纤毛发生的药物。图5(组A-J)显示与DMSO 相比,氟替卡松、非尼拉敏、维拉帕米、ML-141、米托蒽醌、托烷司琼、爱普杷嗪、赛庚啶、紫杉醇和辛伐他汀在各种测试浓度下对纤毛发生没有显著的影响。意外地,图6显示与DMSO相比,前列地尔通过增加纤毛细胞的百分比而显著恢复了NPHP患者来源的细胞中的纤毛发生。
前列地尔,即前列腺素E1(PGE1)具有化学结构
其表现出血管舒张、抑制血小板凝集以及刺激肠和子宫平滑肌的活性以用于治疗心脏病和勃起功能障碍。前列地尔可能通过结合作为 G蛋白偶联受体(GPCR)的E型前列腺素(EP)受体而起到激动剂的作用,针对EP1、EP2、EP3和EP4的IC50值分别为36、10、1.1和2.1nM。GPCR刺激腺苷酸环化酶并且随后在细胞内cAMP中升高。
如本文所用,“GPCR激动剂”包括活化GPCR来模拟对此受体具有特异性的内源性信号传导分子的作用的组合物。“GPCR拮抗剂”包括抑制GPCR活性的组合物。GPCR活性可以通过结合效应信号传导分子(诸如G蛋白)的能力来测量。“经活化GPCR”是能够与G蛋白相互作用并活化G蛋白的经活化GPCR。受抑制受体结合细胞外配体和 /或有成效地与G蛋白相互作用并活化G蛋白的能力可能会降低。
例如用taprenepag异丙基进行的GPCR激动剂处理可以在例如约 0.1mg/kg至约20mg/kg、约0.5mg/kg至约20mg/kg、约1mg/kg至约20mg/kg、约2mg/kg至约20mg/kg;约3mg/kg至约20mg/kg;约4mg/kg至约20mg/kg;约5mg/kg至约20mg/kg;约6mg/kg至约20mg/kg、约7mg/kg至约20mg/kg、约8mg/kg至约20mg/kg、约9mg/kg至约20mg/kg、约10mg/kg至约20mg/kg、约12mg/kg 至约20mg/kg、约14mg/kg至约20mg/kg、约16mg/kg至约20mg/kg 或约18mg/kg至约20mg/kg的浓度下,以例如每天、每2天、每3 天、每4天、每5天、每6天、每7天、每8天、每9天、每10天、每周一次、每2周一次,每3周一次或每月一次的频率进行。
为了确定用于恢复纤毛发生的前列地尔有效浓度,用1nM至2 μM(图7A)和100pM至2μM(图7B)的前列地尔滴度进行自动纤毛分析。GPCR激动剂,例如前列地尔的有效浓度可以是约1pM至约 10μM、约10pM至约5μM、约50pM至约5μMμM、约100pM至约5μM、约1nM至约5μM、约1nM至约4μM、约1nM至约3μM、约1nM至约2.5μM、约1nM至约2μM、约10nM至约2μM、约 100nM至约2μM、约500nM至约2μM或约1μM至约2μM。图 7C示出了用于确定IC50的对应半对数表示,其指示前列地尔以剂量依赖性方式显著增加了NPHP患者来源的细胞中的%纤毛细胞。
图8A-8C和图9(组A-D)示出了元分析,其指示与对照(DMSO 0.04%)相比,前列地尔处理(2μM)没有显著地影响对照正常上皮细胞 (CTRL)中的纤毛发生(图8A)。相比之下,与对照(DMSO 0.04%)相比,前列地尔处理显著增加NPHP患者来源的细胞(PT1)中的纤毛发生(图 8B)。图8C示出了相对于未接受前列地尔处理的对照细胞,即对照 (DMSO 0.04%),前列地尔对NPHP患者来源的细胞中的纤毛发生的影响增加了约两倍。
元分析还示出了前列地尔剂量对纤毛发生具有近似线性的影响。图9(组A和B)例如显示,关于前列地尔对对照正常上皮细胞中的纤毛发生的影响的R2值为0.9194。类似地,图9(组C和D)显示,关于前列地尔对NPHP患者来源的细胞中的纤毛发生的影响的R2值为0.8489。
为了确定前列地尔(PGE1)的稳定性,从暴露于不同浓度的前列地尔24小时和48小时之后的尿液来源的肾上皮细胞(UREC)获得上清液。然后提取样品并且分为等份以用于在LC/MS/MS和极性LC平台上进行分析。图10示出了PGE1在实验条件下是稳定的。
除PGE1之外,还测试了其他EP激动剂,诸如具有如下化学结构的前列腺素E2(PGE2或地诺前列酮):

恢复纤毛发生的能力。图11A显示,PGE2和dmPGE2在NPHP 患者来源的细胞中具有与前列地尔类似的纤毛发生恢复作用,而在对照正常细胞中未观察到显著作用。在最高浓度(40μM地诺前列酮和 20μM dmPGE2)下观察到NPHP患者来源的细胞中纤毛发生恢复轻微降低,这可能是由于细胞毒性所致。
为了测试前列地尔(PGE1)对NPHP1缺失细胞的影响,用前列地尔(2μM)或DMSO处理来源于NPHPI(del)患者的细胞系,例如PT1、 1-03-P、1-06-P1、1-06-P2、1-09-P、1-10-P和1-12-P。图11B示出了前列地尔显著增加了NPHP1缺失细胞中的纤毛发生速率,而前列地尔对正常对照细胞中的纤毛发生速率没有显著影响,表明前列地尔对恢复NPHP1缺失患者中的纤毛发生是有效的。
图11C中的元分析示出了先前数据的线性回归分析,其中斜率反映了前列地尔对对照细胞和多种NPHP患者来源的细胞系的纤毛发生的影响,每个符号代表独立的实验并且每种颜色代表患者细胞系 (被命名为1-09-P L4、1-06-P1、1-06-P2、PT-1)。对照正常上皮细胞数据的线性回归显示斜率值在0,7665与0,9974之间,表明前列地尔对纤毛发生没有影响。相比之下,多种NPHP患者来源的细胞的线性回归显示汇集斜率值为1.414,或者斜率值范围为1.333至1.506,指示前列地尔对纤毛发生的刺激影响。
前列腺素存在于大多数人类组织中,并且由必需脂肪酸合成。各种前列腺素之间的结构差异是不同生物活性的原因。包括前列腺素在内的类前列腺素(Prostanoid)在肾中丰富地产生。类前列腺素起源于通过磷脂酶A2从膜磷脂释放的花生四烯酸(AA)。花生四烯酸经受环氧化酶(或前列腺素G/H合成酶)的双加氧酶和过氧化物酶活性以形成前列环素G2(PGG2),并且然后形成前列腺素H2(PGH2)。PGH2是合成酶(包括PGE2合成酶、PGD2合成酶、前列环素合成酶、PGF2α合成酶(PGF2α也可以直接由PGE2合成)和血栓素合成酶)的底物,用于合成各个类别的包括PGE2在内的类前列腺素。这些类别全部具有包括EP1-4在内的离散的受体亚型,它们通过所述受体亚型启动它们的作用。环氧化酶1和2(COX1和COX2)是非类固醇抗炎药物(NSAID) 的主要靶,但是这些药物可能对一种同种型或另一种同种型具有特异性,具有选择性或非选择性。通过COX抑制阻断PGH2的产生可以降低所有的下游类前列腺素的水平。
在纤毛发生中的PGE
PGE2是肾病理生理学中得到最佳地表征的类前列腺素。PGE2 由COX1和COX2合成,并且通过细胞膜上的Lkt/ABCC4运输体被输出。释放的PGE2结合纤毛上的EP4受体,导致GPCR(G)和腺苷酸环化酶(AC)的活化以增加cAMP,从而增加顺行IFT并增强纤毛发生。
纤毛的形成和伸长需要COX-Lkt/ABCC4-EP4信号传导级联(在小鼠肾集合管细胞IMCD3中以及在斑马鱼模型中)。已知cAMP依赖性激酶信号传导能在纤毛发生期间增加顺行IFT。Lkt/ABCC4介导的PGE2信号传导影响cAMP水平并且通过IFT的顺行速度的增加来促进纤毛发生。PGE2处理导致在IMCD3细胞的纤毛发生期间细胞内 cAMP增加,但Ca2+不增加。PGE2以自分泌和/或旁分泌方式起作用,因为细胞可以对其本身或其周边物释放的PGE2作出应答。在人类癌细胞中,PGE2与EP4受体的相互作用诱导Wnt/p-连环蛋白信号传导,导致COX2表达,并且从而建立引起进一步的PGE2合成的正反馈环路。
图12示出了外源性PGE2的添加增加了对照细胞中的纤毛长度和纤毛细胞的百分比,但在EP4缺失细胞中未能如此,这指示EP4 在纤毛发生期间在PGE2信号传导的下游起作用。
PGE2由PGE合成酶(PGES)产生并且通过与其GPCR:EP1-4的结合来进行信号传导。EP1(与Gq偶联)的活化通过PLC增加细胞内 Ca2+。EP3(与Gi偶联)的活化通过PLC增加细胞内Ca2+,和/或通过腺苷酸环化酶(AC)抑制cAMP产生。EP2或EP4(均与Gs偶联)的活化通过AC刺激cAMP产生。
大约有800种人GPCR,分为五个主要的系统发育家族:视紫红质、分泌素、粘合素(adhesion)、谷氨酸盐和Frizzled/Taste2。GPCRS 是重组蛋白、小分子化合物、变构配体或抗体的有吸引力的靶。46 种GPCR已经充当针对高血压、疼痛、溃疡、过敏、酒精中毒、肥胖症、青光眼、精神障碍和HIV的药物靶。在许多障碍中,一个主要障碍是普遍缺乏关于推定的GPCR与准确的生理功能或疾病状况的关联的知识。
图13示出了EP1-4在肾和视网膜中表达-这两种器官均受到 NPHP和NPHP-RC的影响。在肾中,EP受体沿肾单位差异地表达,突出了活化肾中每种EP受体亚型的不同功能后果。EP受体调节传入小动脉中的血管张力,其中EP1/EP3充当血管收缩剂并且EP2/EP4 充当血管扩张剂。EP1/EP4调节近端肾小管运输。EP3和EP4调节升支粗段和远端肾小管运输。EP4刺激致密斑释放肾素。EP2/EP4使直小血管发生血管扩张。EP受体调节集合管运输,由此EP1抑制Na+再吸收,EP3抑制H2O再吸收,并且EP4刺激H2O再吸收。
通过qRT-PCR确定包括EP受体在内的PG途径成分在URECS 中的表达。图14A显示,EP2和EP4在mRNA水平下表达,并且EP2 主要在imRNA水平下表达。图14B示出了URECS中的EP2蛋白表达。
PGE2调节剂(EP2)
EP2受体的选择性激动剂和拮抗剂在例如Markovic,T. “Structural featuresof subtype-selective EP receptor modulators”Drug Discovery Today.2017;22(1):57-71中示出,该文献以引用的方式并入。第一类激动剂包含在结构上类似于内源性配体PGE2,但是在ω-亲油链中并入了促进增强的效力和选择性的主要修饰的配体。第二类激动剂是非类前列腺素系列的吡啶基磺酰胺衍生物,其中最强效的是 taprenepag异丙基(PF04217329,CP 544326的前药)。Taprenepag具有如下的非类前列腺素类结构

PF-04418948是一种氮杂环丁烷-3-羧酸衍生物,是第一个选择性 EP2拮抗剂,它的IC50为16nM(Kb=1.8nM),它对EP2受体的选择性相对于其他类前列腺素受体增加了>10,000倍。
Markovic的图5(其以引用的方式并入)示出了EP4受体的选择性激动剂:(a)基于功能化环戊烷核的衍生物,(b)携带羟基环戊酮核的内酰胺对应物的衍生物,和(c)结构上不同的EP4激动剂。将四唑特征引入到a-链中替换末端羧酸官能团,意图改善生物可利用性,这使得发现了L902,688,EP4受体的次纳摩尔激动剂(EC50=0.2nM)。 L902,688具有如下的类前列腺素结构:
KAG-308(一种低纳摩尔EP4激动剂)的结构
在EP4激动剂领域中有些独特之处,因为它是唯一一个基于 7,7-二氟前列环素支架的结构。
Markovic的图6(其以引用的方式并入)示出了EP4受体的选择性拮抗剂和由于微小的结构变化而产生的功能应答的切换:(a)EP4受体的选择性拮抗剂,和(b)在EP4受体处激动和拮抗的切换。PG-1531 是一种三取代呋喃衍生物,是具有优异的选择性谱和增强的水溶解性的纳摩尔EP4拮抗剂。通过引入对分子的微小修饰,可以在EP4受体处对所述分子的固有活性进行微调(实例在图17中示出)。例如,已经证明所述固有活性(激动相对于拮抗)仅依赖于图17(组b)的化合物的苄基基团上的三氟甲基取代基的取代模式。极大的功能变化可以通过配体结构的微小变化来实现。
图15示出了进行了对纤毛发生的影响的测试的PG调节剂(激动剂和拮抗剂)。
图16A显示,CP-544326(一种非类前列腺素EP2激动剂)将纤毛发生恢复至与前列地尔类似的水平。图16B显示,与DMSO相比, CP-544326以剂量依赖性方式恢复纤毛发生。图16C是图16B的结果的半对数表示,其中CP-544326滴定指示对于EP2,EC50=11nM。非类前列腺素CP-544326的纤毛发生的恢复证实了其作用机制的特异性。相比之下,图17A显示,L-902.688(一种类前列腺素EP4激动剂)没有显著影响纤毛发生。这些结果指示EP2在纤毛发生中的作用比EP4更为重要。
图17C显示,与前列地尔类似,CP-544326处理与用DMSO处理相比增加了多种来源于NPHPI(del)患者的细胞系,例如1-09-P、 1-06-P1和1-06-P2中的纤毛发生。图17D中的元分析示出了图17D 的线性回归分析,其中斜率反映了CP-544326对对照细胞和多种 NPHP患者来源的细胞系的纤毛发生的影响,每个符号代表独立的实验并且每种颜色代表患者细胞系(被命名为1-09-P L4、1-06-P1、 1-06-P2、PT-1)。对照正常上皮细胞数据的线性回归显示斜率值在 0.6369与1.03之间,表明CP-544326不影响纤毛发生。相比之下,多种NPHP患者来源的细胞的线性回归显示汇集斜率值为1.36,或者斜率值范围为1.245至1.532,指示前列地尔对纤毛发生的刺激影响。
差异展示分析
进行微阵列分析来鉴定负责前列地尔介导的纤毛发生恢复的表达基因。在96孔板中培养UREC,并且用不同浓度的前列地尔处理,接着使用RLT或Qiazol方法进行RNA萃取,如图18中所总结的。
图19示出了通过层次聚类分析的样品的微阵列数据。首先根据提取类型(Qiazol与RLT)对数据进行聚类。然后根据条件(例如对照与前列地尔处理)对Qiazol样品进行聚类,并且然后通过重复对RLT样品进行聚类。
图20示出了通过层次聚类分析的样品的微阵列数据。从Qiazol 提取样品中获得的数据按条件聚类,然后按重复进行聚类,而不按处理内的剂量或对照内的培养基/DMSO进行聚类。
图21示出了通过层次聚类分析的样品的微阵列数据。从RLT提取样品中获得的数据按重复进行聚类,然后按条件(对照与前列地尔处理)进行聚类,而不是按处理内的剂量或对照内的培养基/DMSO 进行聚类。
对于从RLT提取样品获得的微阵列数据,在DMSO与培养基之间没有显著差异,例如,只有四种差异表达的基因而没有受调节的外显子/模式。然而,图22示出了对照(DMSO)与前列地尔处理(0.2μM, 2μM和10μM)的比较,在三种前列地尔浓度的比较中,表达的和受调节的基因的数量几乎相同。前三位受调节的基因也几乎相同并且共享相同的信号传导途径,例如细胞粘附和细胞外基质的下调。
对于从Qiazol提取样品获得的微阵列数据,在DMSO与培养基之间没有显著差异,例如33种差异表达的基因没有调节外显子/模式。然而,如图26所示,将对照(DMSO)与前列地尔处理(0.2μM,2μM 和10μM)进行比较,在三个前列地尔浓度比较中,表达的和受调节的基因的数量几乎相同。前三位受调节的基因也几乎相同并且共享相同的信号传导途径,例如细胞粘附和细胞外基质的下调以及干扰素信号传导的上调。
此外,图24A和24B显示两个聚类被定义,聚集了总共310种基因,即,“聚类1”=120种下调基因,并且“聚类2”=190种上调基因。这指示在从各种剂量获得的微阵列数据之间没有检测到显著差异。
进一步地,通过将接受或未接受前列地尔处理的患者的微阵列数据交叉并且将该数据同对照与患者的RNAseq的微阵列数据交叉而进行的途径分析揭示了,与对照细胞相比,前列地尔可以逆转在NPHP 患者来源的细胞中观察到的基因表达的改变。
多组学分析
图25示出了有关药物对纤毛发生的影响的多组学分析的过程。图26A-26E示出了例如有关在五个独立的实验中前列地尔对纤毛发生,例如%纤毛细胞的影响的表型分析。这些结果显示前列地尔以n= 1-5部分恢复了纤毛发生,其中倍数比率相似,没有剂量依赖性应答。
图27示出了从多组学数据(在DMSO 0.04%中的NPHP患者来源的细胞与用前列地尔2μM处理的NPHP患者来源的细胞)的蛋白质差异表达分析鉴定的已被药物靶向的和可被药物靶向的基因的总结,已被药物靶向的基因根据所述蛋白质差异表达分析来命名。

体内模型
图29示出了来自斑马鱼NPHP4吗啉代(MO)模型的结果,其中向处于单细胞期的野生型斑马鱼胚胎注射吗啉代(例如,NPHP4 ATG MO),其阻断NPHP4 mRNA的起始位点进行核糖体结合。吗啉代特异性地抑制NPHP4 mRNA的翻译。斑马鱼NPHP4 MO表现出经典的纤毛病相关表型,包括身体弯曲、前肾囊肿、偏侧性(心脏环化)缺陷和泄殖腔扩张(堵塞)。

图31,组A示出了DMSO(0.04%)在野生型斑马鱼胚胎中没有诱导致死性、身体弯曲或前肾囊肿。此外,注射了不影响NPHP4表达的对照吗啉代的斑马鱼也没有表现出身体弯曲(图31,组B)或前肾囊肿(图31,组C)。相比之下,注射了NPHP4 MO的斑马鱼以剂量依赖性方式表现出经典的纤毛病相关表型,包括例如身体弯曲(图31,组B)和前肾囊肿(图31,组C)。
图32,组A示出了四个类别的斑马鱼的代表性身体轴线弯曲:正常、I类、II类和III类。图35,组B显示,与DMSO处理相比,前列地尔处理(0.5μM和5μM)对斑马鱼NPHP4 MO的身体轴线弯曲没有显著地影响(p>0.05,Fischer精确检验)。类似地,使用身体弯曲作为自动定量参数,图32,组C显示,与DMSO处理相比,前列地尔处理(0.5μM和5μM)对斑马鱼NPHP4 MO的背侧弯曲没有显著影响。

为了测试地诺前列酮(PGE2)对纤毛病的影响,用地诺前列酮(50 μM)或DMSO处理斑马鱼NPHP4 MO。图34,组A显示,与DMSO 处理相比,地诺前列酮处理显著地增加斑马鱼NPHP4 MO的%正常身体轴线弯曲(p=0.0066,Fischer精确检验)。然而,图34,组B显示,与DMSO处理相比,地诺前列酮处理对斑马鱼NPHP4 MO的背侧弯曲没有显著影响(p=0.0577,t-检验)。图34,组C显示,与DMSO 处理相比,地诺前列酮处理显著地降低斑马鱼NPHP4 MO的%严重和轻度前肾囊肿并显著地增加斑马鱼NPHP4 MO的%正常前肾囊肿 (p<0.008,Fischer精确检验)。

为了检查taprenepag异丙基(PF 04217329,CP-544326的前药)和 taprenepag(CP-544326)的体内稳定性,在野生型C57BL/6J小鼠中进行药代动力学(PK)研究。图36示出了PK研究设计。在腹膜内注射 taprenepag异丙基(1mg/kg或8mg/kg)或taprenepag(8mg/kg)之后,在不同时间点确定这些化合物在各种器官中的浓度。结果显示,一般而言,taprenepag在血浆(图37A)、肾(图37B)、睾丸(图37C)、视网膜(图37D)和玻璃体液(图37E)中比taprenepag异丙基更稳定。
NPHP1的纯合缺失是少年型肾耗竭1的最常见病因。NPHP1中的纯合或复合杂合突变也与例如Joubert综合征4(大脑异常)和 Senior-Loken综合征1(视网膜病变)相关联。产生NPHP1 KO动物以测试taprenepag是否可以用于治疗这些疾病。为了建立CRISPR/Cas9 工程化的Nphp1-/-小鼠模型,在C57BL/6J胚胎中注射单向导RNA,并且产生在Nphp1的外显子1中涵盖ATG的76bp缺失。为了表征 Nphp1-/-小鼠模型的自然历史,对来自Nphp1+/+和Nphp1-/-小鼠的肾和视网膜切片进行组织化学染色。Nphp1-/-小鼠模型没有表现出肾表型。相比之下,P14年龄的Nphp1-/-小鼠开始表现出光感受器层(例如,内部区段(IS)、外部区段(OS)和外部核层(ONL))的厚度降低,直至它们在P28被处死,这指示此模型中的快速视网膜变性与纤毛病相关表现相对应。
为了评估视网膜变性,开发了一种半自动工具,用于在视网膜切片上手工标记的五个远平面上对每个视网膜层进行检测和定量厚度测量(图38B)。半自动定量分析证实了光感受器层ONL、IS和OS的厚度明显降低(图38C)。
为了评估Nphp1缺失对此模型中的光感受器的结构组织的影响,对来自Nphp1+/+和Nphp1-/-小鼠的视网膜切片进行免疫组织化学(IH) 分析(图39A和39B)。将组织固定,并且用DAPI(针对核染色)、抗视紫红质抗体(针对OS染色)和抗Cep290抗体(针对连接纤毛染色)或 PNA(针对OS和IS染色)进行荧光标记,使用免疫荧光显微镜进行检测。图39A显示,Nphp1-/-小鼠模型表现出组织良好的光感受器结构,其中视紫红质沿OS定位,在连接纤毛处以Cep290点状分布为界。这表明连接纤毛具有允许视紫红质从IS运输至光感受OS的功能。相比之下,Nphp1-/-小鼠无法形成连接纤毛,并且在IS和OS中表现出明显的视紫红质错误定位,这表明视紫红质的运输需要连接纤毛的正确形成/维护。相应地,图39B显示,相比于Nphp1+/+小鼠,Nphp1-/-小鼠在IS/OS以及ONL中表现出明显的视紫红质错误定位。
为了评估Nphp1缺失对光感受器的功能的影响,在不同强度的光刺激下,对Nphp1+/+和Nphp1-/-小鼠进行视网膜电图(ERG)检查(图 40A-C)。图40A和40B示出了在给定的光强度下,在P21处从相同动物记录的ERG a波和b波。相比于Nphp1+/+小鼠,Nphp1-/-小鼠在给定强度的光刺激下表现出明显更低的ERG振幅。图40C是ERG a 波在不同强度的光刺激下的放大倍数,因为a波反映了光感受器功能。
在测试CP-544326对纤毛病相关表型的影响之前,通过免疫组织化学研究潜在靶EP2的表达。荧光显微镜揭示了,EP2在P21年龄的 Nphp1+/+和Nphp1-/-小鼠的光感受器层IS和ONL中在蛋白质水平下良好地表达,尽管OS/IS/ONL边界在Nphp1-/-小鼠中难以辨别。
图42示出了用于评估CP-544326对Nphp1-/-小鼠模型中发生的视网膜变性的影响的实验设计。简而言之,从P6至P21,每3或4天,向动物注射(i.p.)媒介物或在媒介物中的CP-544326(18mg/kg)。表型读出涵盖如先前针对Nphp1-/-小鼠模型的表征所述的结构和功能参数。
图43示出了CP-544326对由ONL/OPL比率表示的光感受器层 ONL厚度的影响,所述比率由IHC切片上的视网膜层的半自动定量计算。与媒介物处理相比,CP-544326处理(18mg/kg)显著地防止 Nphp1-/-小鼠中ONL/OPL比率的降低(p<0.05,Mann-Whitney检验)。类似地,CP-544326处理(18mg/kg)显著防止了Nphp1-/-小鼠中的视紫红质错误定位(p<0.05,非配对t-检验),所述错误定位表示为以半自动方式通过荧光显微镜在IHC切片上定量的参数“ONL中的平均绿色强度”(图44)。
为了评定CP-544326对光感受器应答性的影响,在不同强度的光刺激下,对用CP-544326(18mg/kg)或媒介物处理的Nphp1+/+或 Nphp1-/-小鼠进行视网膜电图(ERG)检查。ERGa波的放大倍数(图45) 显示,与媒介物处理的Nphp1-/-小鼠相比,CP-544326(18mg/kg)触发光感受器应答的振幅的轻微改善。

Claims (30)
1.一种治疗受试者的至少一种纤毛病相关疾病的方法,其包括向所述受试者施用治疗有效量的至少一种靶向至少一种G蛋白偶联受体(GPCR)的剂。
2.如权利要求1所述的方法,其中所述纤毛病相关疾病由NPHP1基因座的纯合缺失引起。
3.如权利要求1所述的方法,其中所述纤毛病相关疾病由所述NPHP1基因座的杂合缺失和第二基因座处杂合或纯合功能丧失引起。
4.如权利要求1所述的方法,其中所述纤毛病相关疾病由NPHP1的一个等位基因中的杂合缺失和所述第二等位基因中的功能丧失突变引起。

(b)将所治疗的动物或细胞模型的所述可测量表型与未治疗动物或细胞模型的所述可测量表型进行比较,以及
(c)当所治疗的动物或细胞模型的所述可测量表型与所述未治疗动物或细胞模型的所述可测量表型相比得到改善时,将所述测试剂鉴定为治疗纤毛病相关疾病的治疗剂。
15.如权利要求14所述的方法,其中所述动物模型是Danio rerio(斑马鱼)。
16.如权利要求15所述的方法,其中所述动物模型通过施用一种或多种破坏剂来产生。
