背景技术
上皮是由紧密相连的细胞组成的组织,细胞间质较少。上皮细胞来源于两种胚胎生殖层:外胚层和内胚层,在结构上构成了大部分的器官。
内胚层衍生出表皮和角膜的上皮细胞。腺部件,如汗腺、皮脂腺和乳腺。
内胚层衍生出覆盖消化管的组织,也起源于内胚层。所有与消化器官相连的腺体,如肝、胰腺和胃以及肠的腺体。
中胚层提供起源给肝脏、男性和女性的生殖器官。
上皮细胞的一个基本特性是,它们倾向于在彼此之间保持非常大的接触,形成覆盖表面和填充空腔的连贯层。
呼吸道上皮有双重起源,即喉部、气管、支气管和肺泡的上皮细胞由内胚层起源,而软骨、肌肉结构和血管系统由中胚层起源。

在肺组织中,肺泡巨噬细胞负责吞噬和清除退化的肺组织,包括I型和II型肺泡壁细胞和表面活性物质。
当巨噬细胞由于其特性不能消化这些颗粒时,颗粒就会在巨噬细胞的吞噬体中随着巨噬细胞迁移,直到粘液纤毛运输开始,在它们流动起来直到口咽后,与痰一起脱粘或清除。
COPD(慢性阻塞性肺疾病)是全球范围内患病率和死亡率最高的呼吸系统疾病之一。它是仅次于心血管疾病和癌症的第三大死亡因素,并且发达国家近10%的成年人受其影响。

发明内容
本发明的作者测定了第一种检测循环肺细胞(CPCs)技术的在体外的敏感性和特异性,并证明了在COPD患者外周血中分离CPCs是可能的。
尽管,到目前为止,没有发现证据表明在血流中存在分化良好型的上皮细胞,但它们的存在暗示存在强烈的组织侵袭过程,超出了巨噬细胞消除“废物”的能力,以及其他生理机制,这在上皮组织(如肺和腺结构)中是特别重要的事实。

具体实施方式
退化的上皮细胞具有消除的生理机制,主要通过巨噬细胞的吞噬作用。因此,在肺组织中,肺泡巨噬细胞负责吞噬和清除退化的肺组织,包括I型和II型肺泡壁细胞和表面活性物质。
由于其性质,颗粒无法被巨噬细胞消化时,颗粒就会在巨噬细胞的吞噬体中随着巨噬细胞迁移,直到粘液纤毛运输开始,在它们流动起来直到口咽后,与痰一起脱粘或清除。
因此,可能引起循环上皮细胞存在的疾病是那些影响肝、胰腺、肠、胃和肠腺以及生殖器官的疾病。
本发明公开了一种检测上皮细胞的方法,该方法令人惊讶地可以在血液中检测上皮细胞。
具体地,本发明的方法公开了一种从上皮组织中分离、检测以及表型和基因表征细胞的自动化方法,并且可以在血液中对其进行检测。外周血中所述细胞的检测是由会引起破坏现象和组织破坏的生理病理过程的存在决定的,例如,但不限于慢性阻塞性肺疾病(COPD)或存在肿瘤疾病。因此,在第一种情况下,我们可以谈论循环肺细胞(CPCs),在第二种情况下,我们可以谈论循环肿瘤细胞(CTCs)。
肿瘤细胞远程产生转移的能力是众所周知的,并且许多临床研究和基础研究集中在其检测和表征上。然而,迄今为止,在血流中尚未检测到循环上皮细胞。本发明方法的临床应用重点在于检测非肿瘤循环上皮细胞作为组织损伤特异性生物标志物的可能性。这项技术可以应用于检测任何上皮细胞,使用特定的标记来识别它们。此外,它还允许使用胞质内的和细胞核的标记物进行自动细胞分离。
本发明的方法
因此,发明的第一个方面涉及一种识别在外周血中循环的上皮细胞的方法,以下简称本发明的第一种方法,包括以下步骤:根据循环上皮细胞,用细胞质内、核和/或表面标记物培养外周血样本,和他们的组织起源,并通过免疫细胞化学、分子和/或细胞遗传学技术来检测和识别细胞。
优选地,本发明的第一种方法还包括对个体的外周血样本进行梯度密度分离的优先步骤。
在另一个优选实施方案中,在梯度密度分离之后,执行包括收集中间相并用盐水缓冲液清洗的步骤,在用标记物培育样品之前。
本发明第一种方法识别的细胞原则上优选为非肿瘤细胞。更优选的,它们是受任何组织侵袭过程影响的身体组织中的非肿瘤细胞。在另一个更优选的实施例中,它们是来自肺、肝脏、胰腺的上皮细胞,以及来自胃腺和肠腺、肝脏、生殖器官或它们的任何组合的细胞。

在另一个更优选的实施方案中,标记的是来自中枢神经系统的细胞,标记物选自:巢蛋白、SOX10、Notch1、HES1、HES2、闭合蛋白、PAX6、HES5、GABAB受体1和2、GAD65、GAD67、GFAP、GLAST、BLBP、TN-C、N-钙粘蛋白、巢蛋白和SOX2。
可以通过亲和分离技术对存在目标表型的群体进行分离,包括但不限于:磁分离(使用包覆特异性抗体的磁性颗粒),亲和层析法,与单克隆抗体结合或与单克隆抗体一起使用的细胞毒素剂,与固相支持物结合的抗体“平移”,以及通过其他合适的技术手段。可以通过流式细胞技术获得更准确的分离,该技术可以根据染色强度以及其他参数(例如细胞大小和细胞复杂性)分离细胞群体。
因此,本发明的第二方面涉及一种分离在外周血中循环的上皮细胞的方法,以下简称本发明的第二种方法,包括本发明的第一种方法的步骤,并且进一步包括通过亲和分离技术分离经识别的细胞。

在本发明该方面的另一个更优选的实施方案中,亲和分离技术是通过包覆特异性抗体的磁性颗粒(或微球)进行的。换句话说,通过阳性免疫磁选择进行分离。更优选地,使用具有以下特征的多个磁性颗粒或微磁性球:

诊断试剂盒或设备

具体实施方式
材料和方法
H820肺细胞培养方法的特异性和敏感性测定,作为研究CPCs的模型
1.细胞培养:肺癌细胞株获自American Type Culture Collection(ATCC,马纳萨斯,VA,USA)(15)。将H820细胞保存在添加10%胎牛血清和100U/ml青霉素和100ng/ml链霉素的RPMI 1640培养基中,置于37℃的培养箱中,以5%CO2加湿。
2.用于检测CPCs的抗体:
-细胞角蛋白:泛细胞角蛋白(AE1/AE3克隆)(Sigma Aldrich)。细胞角蛋白是正常和肿瘤上皮细胞中都存在的细胞骨架蛋白。
-抗CD44v6:兔体内产生的抗CD44v6多克隆抗体(AB2080)(Merckmillipore)。在呼吸器官中,CD44v6的表达可以在正常的肺上皮细胞(优选II型肺细胞)中得到证实,这似乎在维持组织结构中起着重要作用。
3.全血实验:使用基于16%W/V Ficoll PM400为119g/ml(Histopaque 1119)和1.058g/ml的Ficoll1的双梯度离心法从血液中分离II型肺细胞。由于Ficoll的密度为1.058g/ml,血液大约为1.060g/ml,为了避免混合,血液需要事先用19%PBS1X稀释(4ml血液/17.3ml PBS1X)。
将10ml的Ficoll 1119放在50ml的锥形底管的下部,然后加入5ml Ficoll1058,在Ficoll双梯度溶液上小心地加入稀释的血液。
在700G下连续离心45min(Allegra X-12R离心机(Beckman Coulter)),然后在两个Ficoll层之间回收单核细胞相,根据用MACS技术的细胞癌富集检测试剂盒(MiltenyiBiotec,BergischGladbach,德国),对单核细胞相进行渗透性固定。然后,这些细胞与多种细胞角蛋白的特异性抗体(CK3-11D5)(MiltenyiBiotec,BergischGladbach,德国)一起培养,该抗体识别细胞质的细胞角蛋白7、8、18和19,随后与FITC抗细胞角蛋白抗体(克隆号:CK3-6H5;MiltenyiBiotec)培养。最后,通过MACS细胞(MiltenyiBiotec)的磁分离柱获得细胞角蛋白阳性细胞,并在涂覆聚赖氨酸的玻璃板上离心。

接下来,用抗-CD44v6多克隆抗体、外显子v6兔(AB2080 Merckmillipore)结合山羊抗兔IgG(H+L)(A-21070ThermoFisher)的二级抗体Alexa Fluor 633对样品进行培养,并使用含有DAPI封片剂(Vector Labs)的VECTASHIELD进行封片。最后,在蔡司LSM 710共焦/多光子激光扫描显微镜下观察载玻片[Ortega,F.G.et al.miRNA in situ hybridizationin circulating tumor cells-MishCTC.Sci.Rep.5,9207;DOI:10.1038/srep09207(2015)]。
4.健康无吸烟史的受试者的CPCs测定:在测定COPD患者的CPCs后,用此方法测定10名既往无呼吸系统疾病、既往无烟草接触史的健康受试者的CPCs。
5.检测的灵敏度和特异性测定:COPD患者的CPCs测定。
研究设计
2016年10月至2017年1月期间,横断面观察研究在两个呼吸科的门诊服务模式下进行。
研究人群
该研究纳入了40岁以上的,根据国际参考(4)事先诊断出COPD的成年患者,该参考基于累积至少10包-年的消费量以及支气管扩张后的系数和FEV 1/FVC<0.70,并且至少需要在医生手术中进行3年的事先监测。排除标准包括就诊前4周内COPD恶化,前一个月因任何原因进行外科手术或进行活检的先例,存在α-1抗胰蛋白酶缺乏症或除COPD外的其他慢性呼吸道疾病和先前有肿瘤存在,拒绝参与研究或无法进行补充性研究或回答问卷。
研究变量
对于每一个病人,收集了关于他们的社会人口资料、不良习惯和伴随疾病的信息。所有患者均根据现行参考进行支气管扩张术后肺活量测定,并对其呼吸道疾病进行有效问卷调查(呼吸困难测量使用mMRC量表、COPD-评分、COPD评估测试),并使用BODEx指数对疾病严重程度进行多维度评估。此外,收集每一位病人的肺功能的年度下降。为了测定CPCs,在EDTA管(EDTA管)(BD Biosciences,富兰克林湖,新泽西州,美国)中提取10mL的头静脉外周血(丢弃前10毫升以避免被表皮的上皮细胞污染)和所述样品随后在环境温度下处理不到2小时,避免样品暴露在自然光下。
统计分析
定性变量表示为绝对频率和相对频率,定性变量取决于它们是否服从正常的正态分布(应用Kolmogorov-Smirnov或Shapiro-Wilk检验后),分别表示为MD±SD(均值,标准偏差)和范围(最小和最大)P50[P25-P75](中位数,四分位范围)。采用独立样本方差分析或Kruskal-Wallis H检验(取决于是否服从正态分布),对研究组的定量变量进行比较。统计显着性水平确定为p<0.05。采用社会科学统计软件包20.0版(SPSS公司,芝加哥,伊利诺伊州,美国)进行统计分析。
伦理方面
该项目获得了格拉纳达大学伦理与临床研究委员会的批准。在人类研究项目中遵循了《赫尔辛基宣言》的原则。通过签署知情同意书,所有参与者都被告知研究的性质及其目标,并准许他们参与研究。
测定结果
本研究对17例COPD患者进行CPC测定。参与研究的COPD患者的基本特征见表1。简言之,他们是60岁的病人,主要是男性,烟草消费量很高,25%以上是现在的吸烟者。他们有中度的支气管阻塞,绝大多数被纳入全球阻塞性肺病(GOLD)分类的B组(占总样本的58.8%)。

在6名患者中(占总数的35.29%),已经证明在外周血中存在CPC,检出率为1CPC/106个造血细胞。图1显示了COPD患者的CPC。表2显示了分离出CPC和未分离出CPC的患者的特征。

CPC患者显示出肺功能恶化的趋势,相较上一年增加了中度和重度加重的趋势,CAT问卷显示的症状强度更大,肺功能的年度下降趋势更大,尽管这些结果均无统计学意义(图2)。使用BODEx多维指数评估的外周血CPC患者显示出更高的疾病严重度(图3)。
非肿瘤肺组织免疫荧光染色
使用气胸患者的非肿瘤肺组织的冷冻切片,并将其用作肺细胞CD44v6的表达的特异性对照。简短地说,切片解冻,然后在10分钟内用100%冷丙酮(-20℃)固定。将它们用PBS1X 0.5%吐温洗涤两次,用3%牛血清白蛋白封闭1小时,然后将其与CD44v6-FITC(MCA1730 BioRad)在4℃下培养过夜。将切片再次洗涤三遍,将它们与DAPI培养5分钟,它们用PBS1X洗涤,并用Slowfade Antifade试剂盒(S2828 ThermoFisher)进行封片,以进行免疫荧光显示。
还使用相同的方法分析了其他上皮组织(例如前列腺和肝组织)的阴性对照。在这些情况下,未观察到CD44v6的染色。
Claims (21)
1.一种用于识别个体在外周血中循环的非肿瘤上皮细胞的方法,包括以下步骤:根据循环上皮细胞用胞质内、核和/或表面标记物培养外周血样本,并且通过免疫细胞化学、分子和/或细胞遗传学技术来检测和识别细胞。

11.根据权利要求9或10所述的分离在外周血中循环的上皮细胞的方法,其中,所述亲和分离技术选自:磁性分离,亲和层析法,与单克隆抗体结合或与单克隆抗体一起使用的细胞毒性剂,与固相支持物结合的抗体“平移”,或它们的任何组合。
12.根据权利要求9-11中任一项所述的分离在外周血中循环的上皮细胞的方法,其中,所述亲和分离技术是通过包覆特异性抗体的磁性颗粒来实现的。
13.根据权利要求所述的分离在外周血中循环的上皮细胞的方法,其中,所述包覆特异性抗体的磁性颗粒选自:
a)直径在20nm至500nm之间的一组颗粒,
b)直径在4000nm到6000nm之间,优选5000nm的一组颗粒,或
c)上述两组颗粒组成一组颗粒。
14.一种获取用于诊断、预后和分类患有表现出组织破坏和上皮细胞释放的疾病的个体的数据的方法,包括使用如权利要求1-13所述的任何一种方法的步骤,还包括量化分离细胞的数量的步骤。
15.根据前述权利要求所述的获取用于诊断、预后和分类患有表现出组织破坏和上皮细胞释放的疾病的个体的数据的方法,进一步包括将识别出的细胞数量与参考样品进行比较。
16.一种对表现出组织破坏和上皮细胞释放的疾病的个体进行诊断、预后和分类的方法,包括如权利要求14或15所述的方法,进一步包括当在外周循环血中检测到循环上皮细胞的存在时,将该个体划分为具有患表现出组织破坏和上皮细胞释放疾病风险的个体。
17.根据前述权利要求所述的对表现出组织破坏和上皮细胞释放的疾病患者进行诊断、预后和分类的方法,其中,所述表现出组织破坏和上皮细胞释放的疾病是肺气肿,并且循环的肺细胞在外周血中被识别出来。
18.一种用于监测患有表现出组织破坏和上皮细胞释放的疾病的个体对治疗的反应的方法,包括以非同时的方式实施如权利要求1-15中任一项所述的方法,其中,至少一个量化循环上皮细胞的步骤是在给予个体治疗后进行的。
19.一种包括选自β2-受体激动剂、抗胆碱能类药物、皮质类固醇激素化合物、磷酸二酯酶抑制剂和免疫系统抑制剂的活性成分的药物组合物在制备用于治疗患有通过如权利要求17至18中任一项权利要求所述的方法识别的肺气肿的个体的药品中的应用。
20.一种包含选自铂配合物(顺铂或卡铂)、吉西他滨、紫杉醇、多西他赛、依托泊苷、长春瑞滨、培美曲塞、吉非替尼、厄洛替尼、贝伐单抗或它们的组合的活性成分的药物组合物在制备用于治疗患有通过如权利要求17至18中任一项权利要求所述的方法识别的与COPD相关或不相关的腺癌和鳞癌的个体的药品中的应用。
21.一种用于识别、分离和/或诊断的试剂盒或设备,包括分析外周血中循环的上皮细胞的数量的必要元件,并且包括:
a)直径在20nm至500nm之间,优选为50nm的磁性颗粒,
b)直径在4000nm至6000nm之间,优选为5000nm的磁性颗粒,或
c)上述两种颗粒的组合。
