药物组合物
本文提供的实施方案涉及用于治疗肝脏病症的药物组合物。在一些实施方案中,将药物组合物提供或引入输注系统中。在一些实施方案中,药物组合物包含激活的调节性T细胞。在一些实施方案中,药物组合物包含激活的同种异体调节性T细胞。在一些实施方案中,激活的调节性T细胞产生肝细胞生长因子。在一些实施方案中,激活的调节性T细胞增强肝卵圆细胞产生。在一些实施方案中,激活的调节性T细胞以约5×105至2×106个细胞的量存在。在一些实施方案中,激活的调节性T细胞以约1×106个细胞/毫升的量存在。在一些实施方案中,该组合物降低受试者的血清丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)、白蛋白、或胆红素水平至健康受试者的水平的5-20%、10-30%、20-40%、30-50%、40-60%或50-70%。在一些实施方案中,肝脏病症是肝炎、硬化、慢性肝脏疾病、急性肝脏疾病或肝衰竭。在一些实施方案中,药物组合物还包含用于治疗肝脏疾病的化合物。

输注系统
本公开内容涉及输注系统,其包括用于治疗肝脏病症的药物组合物。在一些实施方案中,输注系统包括容纳药物组合物的容器和递送装置。
在一些实施方案中,容器是袋、注射器、烧瓶或任何用于容纳药物组合物以便随后分配的合适容器。容器可以是适用于储存组合物的任何容器。在一些实施方案中,容器可以是例如预填充的注射器、预填充的药筒、小瓶、安瓿瓶等。在其他实施方案中,该容器可以是具有柔性壁的容器,诸如例如气囊。在一些实施方案中,容器保持无菌。在一些实施方案中,容器包括一个或多个端口,包括例如药物端口、施用端口、入口端口、出口端口。在一些实施方案中,施用端口是药物组合物可流经的出口端口。在一些实施方案中,施用端口流体地连接到药物组合物可流经的管。如本文所述,管可以是任何适合的管,并且可以具有标准大小、尺寸和适合于递送药物组合物的材料。药物组合物从容器流出经管流到递送装置。
在一些实施方案中,递送装置是针、注射器或套管。递送装置使得能够通过肠胃外施用递送药物组合物。如本文所用,肠胃外施用药物组合物包括以受试者组织的物理破坏和通过组织中的缺口来施用药物组合物为特征的任何施用途径。因此,肠胃外施用包括但不限于通过注射组合物、通过经过手术切口施加组合物、通过经过穿透组织的非手术伤口施加组合物等来施用药物组合物。特别地,预期肠胃外施用包括但不限于静脉内、皮下、腹腔内、肌内、胸骨内注射或团注注射。
在一些实施方案中,输注系统还包括滴注室、后止回阀、注入口、滑动夹、辊夹、螺纹锁、盖、过滤器或其他用于调节和控制药物组合物通过递送装置流向受试者的组件。此外,本文所述的输注系统可包括使得能够自动递送药物组合物的特征件,并因此还可包括泵、致动器、阀、电路或计算机处理器和存储器。
在一些实施方案中,可以将输注系统配置为自动递送本文所述的组合物。在一些实施方案中,输注系统在被用户致动后,可以自动产生力来递送组合物。以这种方式,递送组合物的力在期望的范围内,并且可以在不同的设备、使用者等之间重复。在一些实施方案中,输注系统包括被配置成产生力以递送组合物的储能构件。

在一些实施方案中,储能构件可以是机械储能构件,诸如弹簧、含有压缩气体的装置、含有基于蒸气压的推进剂的装置等。在其他实施方案中,储能构件可以是电储能构件,诸如电池、电容器、磁储能构件等。在其他实施方案中,储能构件可以是化学储能构件,诸如含有当混合时反应以产生能量的两种物质的容器。
在一些实施方案中,储能构件可以相对于容器处于任何位置和/或取向。例如,在一些实施方案中,可以将储能构件定位在与容器间隔开的壳体内。此外,在一些实施方案中,可以将储能构件定位成使得储能构件的纵轴偏离容器。在其他实施方案中,储能构件可以基本上围绕容器。
在一些实施方案中,储能构件可以可操作地耦合至容器和/或其中所含的组合物,以使得力递送组合物。例如,在一些实施方案中,力可以经由活塞或柱塞传递给组合物。在其他实施方案中,力可以经由液力或气动耦合传递给组合物。在其他实施方案中,力可以电传递至组合物。在仍然其他实施方案中,力可以经由以上任一种的组合传递至组合物。
在一些实施方案中,容器可包括弹性体构件,使得由储能构件产生的力通过弹性体构件转移至组合物。在一些实施方案中,输注系统包括壳体、容器、弹性体构件和储能构件。该容器设置在壳体内,并含有组合物。该组合物可以是本文所述的任何组合物。储能构件设置在壳体内,并且被配置成产生力以递送如本文所述的组合物。
在一些实施方案中,输注系统可以是任何适用于自动递送本文所述的任何组合物的装置。在一些实施方案中,输注系统可以是配置成自动递送组合物的医用注射器。在一些实施方案中,医用注射器包括壳体、递送机构、含有用于治疗肝脏病症的药物组合物的容器、盖子、安全锁和/或系统致动器组件。医用注射器可以类似于2006年11月21日提交的名称为“Devices,Systems and Methods for Medicament Delivery”的美国专利No.7,648,482(其通过引用整体并入本文)中所述的自动注射器。
在一些实施方案中,递送装置耦合至容器并且至少部分地限定了流动路径,通过该流动路径可以将组合物递送至体内。在一些实施方案中,输送装置可以直接耦合至容器的远端部分。在其他实施方案中,递送装置可以间接耦合至容器。
在一些实施方案中,在致动储能构件之前,可以将递送装置耦合至容器,但与容器流体隔离。以这种方式,可以在延长的时间段储存输注系统,同时保持容器内所含组合物的无菌性,从而减少(或消除)组合物从容器中的任何泄漏。该布置还减少和/或消除了在可以使用输注系统施用组合物之前的组装操作。以这种方式,输注系统产生了用于递送组合物的快速且精确的机构。在使用之前减少和/或消除组装操作减少了输注系统和/或递送装置的性能受到损害(如,由于不适当的耦合、泄漏等)的可能性。
在一些实施方案中,递送装置可以经由耦合构件耦合至容器。在此类实施方案中,当储能构件被致动时,容器和/或递送装置可被配置为相对于耦合构件移动。此类移动可以使递送装置和容器流体地耦合,从而限定了可以将组合物递送到患者的流动路径。
在一些实施方案中,递送装置可以增强组合物的递送,从而提高组合物的功效。在一些实施方案中,递送装置可以产生具有所需特征的组合物流,以增强组合物的吸收速率,以使组合物向其中该递送不太有效的身体区域的递送最小化,等等。
例如,在一些实施方案中,递送装置可以产生受控流速的组合物。在此类实施方案中,递送装置可以包括一个或多个流动孔、弯曲的流动路径等,以产生期望的压降和/或控制通过递送装置的流动。例如,在一些实施方案中,可以配置递送装置以最小化组合物的过量递送。
例如,在一些实施方案中,可以将递送装置和储能构件进行协作配置,以便在致动储能构件时,输注系统产生治疗有效范围内的一定量的组合物。在一些实施方案中,储能构件被配置为“匹配”递送装置,使得储能构件被配置为产生预定范围内的力以确保递送装置的期望功能。因此,储能构件可以是任何适合的装置或机构,其在被致动时产生期望的力以递送如本文所述的组合物。通过采用储能构件产生力,而不是依靠使用者手动产生递送力,可以将组合物以期望的压力和/或流速递送至体内。
在一些实施方案中,递送装置可以在容器致动之前(如,通过手动压下柱塞、挤压扳机等)耦合至容器,但与容器流体隔离。
在一些实施方案中,递送装置可以经由耦合构件耦合至容器。在此类实施方案中,可以将容器和/或递送装置配置为在致动容器时相对于耦合构件移动。例如,在使用中,在压下柱塞以致动容器时,耦合构件可以相对于容器移动,之后将通过移动柱塞产生的大部分能量施加到组合物上。此类移动可以使递送装置和容器流体地耦合,从而限定了可以将组合物递送至患者的流动路径。
本文公开的容器和/或输注系统可含有任何适合量的用于治疗本文公开的肝脏病症的任何药物组合物。例如,在一些实施方案中,输注系统可以是含有单剂量的量的待递送组合物的单剂量装置。在其他实施方案中,输注系统可以是含有多个剂量的量的待递送组合物的多剂量设备。
尽管已经将各种实施方案描述为具有特定特征和/或组分组合,但是在适当的情况下,也可以是来自任何实施方案的任何特征和/或组分的组合的其他实施方案。本文所述的任何容器可含有本文所述的任何组合物和/或制剂。

调节性T细胞

12.如实施方案11所述的方法,其中所述间充质干细胞源自包括选自以下的组的组织:a)华通氏胶质;b)骨髓;c)外周血;d)经动员的外周血;e)子宫内膜;f)毛囊;g)乳牙;h)睾丸;i)脂肪组织;j)皮肤;k)羊水;l)脐带血;m)网膜;n)肌肉;o)羊膜;o)脑室周流;和p)胎盘组织。
13.如实施方案12所述的方法,其中所述间充质干细胞表达选自由以下组成的组的一种标记物或多种标记物:STRO-1、CD90、CD73、CD105、CD54、CD106、HLA-I标记物、波形蛋白、ASMA、胶原蛋白-1、纤连蛋白、LFA-3、ICAM-1、PECAM-1、P-选择素、L-选择素、CD49b/CD29、CD49c/CD29、CD49d/CD29、CD61、CD18、CD29、凝血调节蛋白、端粒酶、CD10、CD13、STRO-2、VCAM-1、CD146和THY-1。
14.如实施方案13所述的方法,其中所述间充质干细胞不表达显著水平的HLA-DR、CD117和CD45。
15.如实施方案11所述的方法,其中所述间充质干细胞由多能干细胞生成。
16.如实施方案15所述的方法,其中所述多能干细胞选自由以下组成的组:a)胚胎干细胞;b)诱导型多能干细胞;c)单性生殖干细胞;和d)体细胞核转移来源的干细胞。
17.如实施方案16所述的方法,其中所述胚胎干细胞群表达选自由以下组成的组的基因:阶段-特异性胚胎抗原(SSEA)3、SSEA 4、Tra-1-60和Tra-1-81、Oct-3/4、Cripto、胃泌素释放肽(GRP)受体、足糖萼蛋白(PODXL)、Rex-1、GCTM-2、Nanog和人端粒酶逆转录酶(hTERT)。

Claims (26)
1.用于治疗肝脏病症的药物组合物,其包含激活的调节性T细胞,其中所述组合物经配制用于施用给患有肝脏病症的受试者。
2.如权利要求1所述的药物组合物,其中所述激活的调节性T细胞是同种异体调节性T细胞。
3.如权利要求1-2中任一项所述的药物组合物,其中所述激活的调节性T细胞产生肝细胞生长因子。
4.如权利要求1-3中任一项所述的药物组合物,其中所述激活的调节性T细胞增强肝卵圆细胞产生。
5.如权利要求1-4中任一项所述的药物组合物,其中所述组合物经配制用于静脉内施用。
6.如权利要求1-5中任一项所述的药物组合物,其中所述激活的调节性T细胞以约5×105至2×106个细胞的量存在。
7.如权利要求1-6中任一项所述的药物组合物,其中所述激活的调节性T细胞以约1×106个细胞/毫升的量存在。
8.如权利要求1-7中任一项所述的药物组合物,其中所述组合物将受试者的血清丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)、白蛋白或胆红素水平降低至健康受试者的水平的5-20%、10-30%、20-40%、30-50%、40-60%或50-70%。
9.如权利要求1-8中任一项所述的药物组合物,其中所述肝脏病症是肝炎、硬化、慢性肝脏疾病、急性肝脏疾病或肝衰竭。
10.如权利要求1-9中任一项所述的药物组合物,其还包含用于治疗肝脏疾病的化合物。
11.如权利要求1-10中任一项所述的药物组合物,其还包含药学上可接受的载体。
12.一种输注系统,其包括:
袋,其包含如权利要求1-11所述的包含激活的调节性T细胞的用于治疗肝脏病症的药物组合物,其中所述组合物经配制用于施用给患有肝脏病症的受试者;和
注射器或递送装置。
13.治疗肝衰竭的方法,其包括施用如权利要求1-11中任一项所述的组合物,其中所述组合物包含通过与同种异体间充质干细胞一起培养被激活以具有增强的肝脏再生因子产生的调节性T细胞群。
14.如权利要求13所述的方法,其中所述调节性T细胞群被激活或已呈现为激活的以通过增强的产生肝细胞生长因子的能力来诱导肝增殖,以及增强肝卵圆细胞增殖。
15.如权利要求13-14中任一项所述的方法,其中所述间充质干细胞处于有丝分裂失活状态。
16.如权利要求13-15中任一项所述的方法,其中所述调节性T细胞或所述间充质干细胞在施用前用免疫调节剂处理。
17.如权利要求16所述的方法,其中用所述免疫调节剂的所述培养诱导调节性T细胞增殖或诱导调节性T细胞产生白血病抑制因子。
18.如权利要求16所述的方法,其中所述免疫调节剂是IL-4、IL-10、IL-13、IL-20、TGF-β、CXCL12、VEGF、PGE-2或抑制素,或以上的组合。
19.如权利要求13-18中任一项所述的方法,其中将所述调节性T细胞与2型单核细胞、CD5阳性B细胞、2型NKT细胞、致耐受性树突细胞、γδT细胞、具有免疫调节特性的T细胞、CD34细胞、极小胚胎样干细胞或塞托利细胞共培养。
20.如权利要求13-19中任一项所述的方法,其中治疗肝衰竭包括减少肝纤维化。
21.如权利要求13-20中任一项所述的方法,其中治疗肝衰竭包括刺激损失后肝组织生长的增殖。
22.如权利要求13-21中任一项所述的方法,其中所述调节性T细胞源自外周血单核细胞、经动员的外周血单核细胞、脐带血、经血、或脂肪基质血管组分细胞。
23.如权利要求13-22中任一项所述的方法,其中所述激活的调节性T细胞抑制用激活增殖的信号刺激的天然T细胞的增殖。
24.如权利要求13-23中任一项所述的方法,其中所述激活的调节性T细胞抑制树突细胞的成熟。
25.如权利要求13-24中任一项所述的方法,其中所述激活的调节性T细胞表达神经毡蛋白-1、CTLA-4、CD25、CD39、CD73、CD105、CD127、FoxP3、GARP、GITR配体、IL-10或膜结合的TGF-β。
26.如权利要求13-25中任一项所述的方法,其中所述激活的调节性T细胞通过暴露于血管活性肠肽、IL-10、TGF-β、间充质干细胞条件培养基、间充质干细胞来源的外来体、BDNF、人绒毛膜促性腺激素、VEGF、CD3或CD28抗体、缺氧条件、雷帕霉素或促血管生成素被激活。
