Description

Head throwing suppression tester
Technical Field
The invention relates to the technical field of image acquisition for testing eyeball motion information in a short distance, in particular to a head-throwing inhibition tester.
Background
The vestibule is an important system responsible for maintaining the balance of the human body and is mainly composed of a peripheral vestibule and a central vestibule. The peripheral vestibule comprises three pairs of semicircular canals and an otolith device, and once any semicircular canal of the three pairs of semicircular canals of a human body has dysfunction, the balance dysfunction can be caused, and symptoms such as vertigo are generated. The quantitative and qualitative test of the semicircular canal dysfunction is an important basis for doctors of departments related to vertigo diseases such as otolaryngology departments, neurology departments and the like to provide final disease diagnosis for patients.
According to the frequency characteristics of semicircular canal function, currently, clinical examination of semicircular canal function mainly includes ultralow frequency hot-cold test examination, low frequency to medium frequency swivel chair test examination, high frequency vestibular self-rotation test, head impact test (HIMP), head impact inhibition test (shiimp), and the like. Wherein, the cold and hot test and the swivel chair test can only check the function of the horizontal semicircular canal, and can not test the functions of the two remaining semicircular canals, and the vestibule self-rotation test, the head-swinging test and the head-swinging inhibition test can all test the functions of 6 semicircular canals. The most important functional frequency band of human body in life is 2-6Hz high frequency, and the high frequency inspection has greater reference value than the low frequency inspection.
High frequency hemicycle functional tests rely primarily on the measurement of the Vestibulo-ocular reflex (VOR). In order to have a clear visual field during the rotation of the head, the human eyeball must compensate the angle of the head rotation within a very short time (within 15ms, generally within 8 ms) during the high-speed rotation of the head, i.e. rotate at the same speed in the opposite direction of the head rotation direction by the same angle as the head rotation, so that the eyeball can still firmly look at the object watched before the head rotation within a very short time after the head rotation, thereby maintaining the visual stability. This process is achieved by vestibulo-ocular reflex. The function of the semicircular canal plays an extremely important role in vestibulo-ocular movement reflex, and the strength of the function of the semicircular canal can be inferred by measuring the vestibulo-ocular movement reflex function. In a high-frequency vestibular self-rotation test, a head-swinging test and a head-swinging inhibition test, whether eyeballs of a testee can move in opposite directions and at opposite speeds in time is analyzed through rapid head rotation of the testee and software recording, and then corresponding semicircular canal function data is obtained. The vestibular autogyration test is also called as an active head swinging test, a subject actively and rapidly rotates the head of the subject according to the sound provided by the system, and in the process, besides the vestibular-eye movement reflection, other stimulation given by a central center during subjective rotation is mixed, so the result of the test is still controversial in the school. In contrast, the head swing test and the head swing suppression test are realized by operating the head of the subject to be swiped quickly, unpredictably and at a small angle by a doctor, the eyeball movement of the subject is only participated by the vestibular-ocular movement reflection in a very short time, and the test result is not easily interfered by other factors and is very reliable.
The swing test is applied to clinical examination in 2009, and due to the fact that the swing test is convenient to operate, inexpensive in equipment price, comfortable in subject experience (compared with a cold-hot test, nausea and vomiting of a patient are easily induced), and three pairs of semicircular regulatory quantitative test results can be provided, the swing test is rapidly popularized all over the world and becomes a reliable supplement or even a substitute for examination after a semicircular regulatory examination gold standard cold-hot test. In the head-flicking test, a subject is required to stare at a fixed visual target right ahead tightly, due to the existence of vestibulo-ocular movement reflex, a patient with normal function of the semicircular canal can stare at the visual target firmly in the passive head-flicking process of any semicircular canal plane, a patient with abnormal function cannot generate eyeball movement capable of offsetting the head movement through the vestibulo-ocular movement reflex, so that the visual target cannot be stared firmly, and then the visual signal is processed by the brain (the time is more than 70 ms) at the second time to find that the visual target is not stared and carry out corrective second eyeball movement, which is the corrective saccade recorded in the examination of the patient with abnormal function of the semicircular canal.
In 2016, researchers such as MacDougall proposed a swing suppression test based on the swing test. The operation flow of the swing head inhibition test is basically consistent with that of the swing head test, and the only difference is that in the swing head inhibition test, a subject needs to stare at a visual target which rotates together with the head, and in the swing head test, the subject only needs to stare at a fixed visual target. Due to the vestibulo-ocular movement reflex, when a subject is passively flicked, the eyeball still stares at the instant position of the visual target at the beginning of the flick within 15ms, then the eye finds that the visual target rotates, and corrective saccades are performed at the second time (after 70 ms), so that a patient who is different from a flick test and only has semicircular canal dysfunction can observe the corrective saccades, and in a flick suppression test, a normal person test can record the corrective saccades. The swing test and the swing suppression test are based on the same inspection principle, and the inspection results can be mutually complemented and verified.
Since the shmpi panning usually occurs after the head movement is over (the delay is greater than that of the HIMP panning), the influence caused by the implicit panning (convert saccades) can be eliminated, and the accuracy of calculation of the VOR gain value can be improved compared with that of the HIMP. Especially for patients in the acute stage of vestibular neuritis, shmpi removes the interference from spontaneous eye shock. Therefore, the SHAPI swing inhibition test has higher clinical application value.
The experimental target of restraint of getting rid of head is followed the head and is rotated together, design mode in the present commercial product lets the testee wear laser, the head of testee is followed the laser target and can be followed and rotate together when rotated like this, however, because the difference of three pairs of semicircular canal planes place direction, present laser only can inspect a pair of horizontal semicircular canal, and prior art scheme's projection system requires highly to the service environment, need to set up suitable projection distance, otherwise can seriously influence the use of patients such as myopia, amblyopia, need not to guarantee in the projection environment that shelter from the thing and disturb, otherwise can produce the shadow so that unable normal use. Therefore, the spread of the swing suppression test is greatly limited.

Fourthly, compared with the technical scheme of the head-mounted equipment for measuring the eyeball movement of the user, the high energy consumption and heat dissipation problems of the projection system in the prior art severely restrict the performance index of the head-mounted equipment, and the user experience is poor. The invention directly arranges a watching target in the eyeshade body, provides the visual targets with different visual angles which are consistent with three pairs of semicircular canal planes, and enables the patient to watch the visual targets so as to carry out the swing suppression test. The fixation target comprises led lamps, images, patterns, figures, points or other stable visual targets, and the fixation target has low energy consumption and small heat dissipation.
Fifthly, the swing suppression tester can be used in the existing technical solutions such as head-mountable equipment for measuring the eye movement of a user, and the eye movement camera system can be configured to acquire a first group of images of a first eyeball and/or a second group of images of a second eyeball of the user. The eyeshade body is worn in the upper and lower symmetry upset, can directly wear in user's head with overturning, can select to gather the eye picture of user's left eye or right eye according to clinical demand, need not the configuration, and it is more convenient to use. The foregoing is only a preferred embodiment of the present invention, and it should be noted that, for those skilled in the art, various modifications and decorations can be made without departing from the principle of the present invention, and these modifications and decorations should also be regarded as the protection scope of the present invention.
