Description
Technical Field
The invention belongs to the technical field of medical appliances, and particularly relates to an infusion device with a non-return function for ultra-slow infusion.
Background
In the traditional infusion apparatus, after the liquid infusion of an infusion bag or an infusion bottle is finished, the needle needs to be pulled out in time, otherwise, the condition of blood backflow, namely the phenomenon of blood return, can occur. Once the blood returns, the blood flowing back into the transfusion system is coagulated into blocks, if the blood blocks enter the human body, the blood vessels can be blocked by thrombus, even serious medical accidents such as iatrogenic infection, septicemia and the like occur, and the human body health is endangered.
The disposable transfusion system is one common medical consumable, and through aseptic treatment, it establishes the passage between vein and medicine liquid for intravenous transfusion, and is composed of eight parts including vein needle or injection needle, needle head protecting cap, transfusion hose, medicine liquid filter, flow speed regulator, drip cup, bottle stopper puncture outfit, air filter, etc. and some transfusion systems include injection part, medicine adding port, etc. The working principle of the disposable transfusion system is as follows: under the action of atmospheric pressure, the liquid in the bottle flows into the drip chamber along the thinner infusion hose, and when the pressure of the water column of the drip chamber is greater than the venous pressure, the liquid in the bottle flows into veins along the hose. Before the medical staff uses, checking the disinfection validity period of the disposable infusion set, packaging the disposable infusion set without air leakage and other anomalies, taking out the infusion set, inserting the bottle inserting needle part into an infusion bottle (such as an infusion bag, unless the infusion set is an infusion set with an air inlet pipe and the bottle inserting needle integrated, the air inlet device is not required to be opened, namely the air inlet needle is inserted or the air inlet hole is opened), hanging the infusion bottle on an infusion rod, exhausting until liquid medicine flows to the lower end of the infusion tube, closing the regulator, then penetrating the vein needle into a disinfected vein, and after the vein needle is fixed, unlocking the tourniquet, and adjusting the flow rate.
The core components of the venous indwelling needle, also called venous trocar for short, comprise a soft catheter/sleeve which can be indwelled in a blood vessel and a stainless steel puncture guiding needle core. When in use, the catheter and the needle core are punctured into the blood vessel together, and after the catheter completely enters the blood vessel, the needle core is withdrawn, and only the soft catheter is remained in the blood vessel, so that intermittent, continuous or daily intravenous infusion treatment is carried out. The use of scalp steel needles should be limited to short-term or single dose administration to collect blood samples or infuse blood, blood products, hemodynamic monitoring infusion fluids at or near isotonic, normal PH range (irritant drugs: only intermittent bolus).
The use method of the indwelling needle comprises the following steps:
1. Preparing articles: infusion bottle (glass bottle, plastic bag, plastic bottle), transfusion device, fixed dressing (adhesive tape, transparent film), connecting fitting (heparin cap, needleless connection), flow rate control device (infusion pump, micropump), indwelling needle, sterilized iodophor, cotton swab, tourniquet;
2. Selecting blood vessels: the selection is thick and straight, the elasticity is good, the blood flow is rich, and venous valves and joints are avoided;
3. Disinfecting skin in a disinfecting range of 8cm;
4. bundling a tourniquet: 10cm above the sterilization range, and is not too tight and not more than 2 minutes;
5. Removing the needle protection cap and loosening the needle core: removing the needle protection cap vertically upwards, and loosening the needle core left and right;
6. Connecting scalp needle and exhausting;
7. Puncturing: taking up skin, directly puncturing vein, inserting needle at 15-30 deg. and slowly inserting needle 0.2cm after blood return;
8. withdrawing the needle core: a hand-held needle seat and a hand-withdrawn needle core;
9. Delivery tube: introducing the catheter entirely into the vein;
10. Loosening the tourniquet and adjusting the dripping speed;
11. sealing and nursing: and (3) fixing the puncture point by using a sterile transparent patch. Keeping the puncture point clean and dry, replacing the transparent paste for 3-5 days, and replacing the transparent paste for 2 days in summer, and replacing the transparent paste in time when the transparent paste is not sticky or polluted;
12. Recording: a label is attached to the venipuncture site, and the date of the signed puncture, the name of the operator, and the like. And record the catheter indwelling condition, the name, the dosage and the usage of the special drug every day.
The indwelling needle is classified into an open type and a closed type, the open type is classified into a general type and a safe type (needle penetration prevention), and the closed type is classified into a general type and a safe type (needle penetration prevention).
Routine care of indwelling needle:
First, seal tube and nursing
1. The purpose is as follows: flushing the residual irritant liquid medicine into the blood flow to avoid stimulating local blood vessels to keep a venous access; the blood at the head end of the hose is prevented from being coagulated to cause blockage when the hose is connected with transfusion next time;
2. The positive pressure tube sealing method comprises the following steps: pulling out the injection needle by pushing and injecting the liquid medicine and withdrawing the needle;
3. sealing liquid: isotonic saline: 5-10ml,6-8 hour wash tube once, dilute heparin solution: the heparin content is 10-100 units per milliliter, and the dosage is 2-5ml. Anticoagulation lasts for more than 12 hours;
4. And (3) a correct pipe punching and sealing step: SASH S normal saline-A administration-S normal saline-H diluted heparin solution;
(II) washpipe and nursing
1. Before re-transfusion, sterilizing the heparin cap, and connecting the syringe needle after flushing;
2. the purpose is as follows: the residual liquid medicine in the catheter is infused into the blood vessel by using the normal saline, so that the chemical phlebitis caused by local vascular irritation is avoided, and the incompatibility among medicines is reduced. The method is applied between two drug infusions or before tube sealing;
3. the method comprises the following steps: the injector is pushed and injected, and a pulse flushing method of pushing and stopping is adopted, so that the physiological saline forms small eddies in the catheter, and residual medicines in the catheter are flushed cleanly;
4. The minimum volume of flushing fluid should be 2 times the volume of the catheter and the additional device.
The existing injector matched with the micro-infusion device has small capacity, the injector needs to be replaced for a plurality of times for long-time infusion, so that the continuous infusion and curative effect are affected by the interruption of the infusion, the infusion device is not suitable for patients with large injection dosage, the chemotherapeutic drugs of tumor patients need to be continuously administered intravenously for a plurality of times, the intermittent administration is needed, the administration time needs to be set for a plurality of times, the nursing workload is increased, and the infusion device which is provided with programmed control and continuous constant-speed administration and is set for one time is lacking, so that the nursing workload is reduced, and the clinical demands of patients are met.

When the transfusion is performed under the condition, the interval between two adjacent dropping liquids in the dropping funnel of the transfusion device is about tens of seconds, tens of seconds or even longer, and the total transfusion time is longer; during this period, both the intravenous infusion directly using the injection needle of the infusion set and the intravenous infusion using the indwelling needle have a long dripping time interval, and in this time interval, blood return occurs due to individual differences of patients and other factors, which may cause blockage of the injection needle or the indwelling needle catheter, affecting infusion treatment.
In the embodiment, the specific part of the infusion set or the indwelling needle is sealed by adopting the clamping device at the interval of two adjacent infusion drops so as to block the pressure balance of the infusion set, the indwelling needle and the outside air, and the liquid medicine in the infusion set and the indwelling needle which face one side of the clamping device to a patient is kept still so as to prevent venous blood of the patient from flowing back into the infusion set or the indwelling needle and form a thrombus blocking pipeline.
The clamping part of the clamping device is of a square structure, the clamping part is divided into two components of a fixed clamping part and a movable clamping part, the fixed clamping part is taken as a basic component, the fixed clamping part is fixed on the positions of an infusion support, a patient body and the like by adopting structures such as a binding band, a buckle, a hanging ring and the like or the like, and then an infusion pipeline (a hose or an extension tube) is placed in a tube groove of the fixed clamping part;

The clamping part is provided with a pair of clamping wheels, namely, the fixed clamping part and the movable clamping part are respectively provided with one clamping wheel, the clamping wheel is connected with the clamping part, the clamping wheels on the two clamping parts are symmetrically arranged on two sides of the tube slot, after the two clamping parts are buckled, the two clamping wheels are correspondingly attached, and in the process that the two clamping wheels are close to each other, the infusion pipeline in the tube slot is extruded and sealed.
As described above, when a patient infuses at a low rate, the interval between two adjacent drops in the drip chamber of the infusion set is about ten or more, several tens of seconds, or even longer, and the patient's blood return also occurs at the interval between the two drops. The clamping device is adopted, a specific execution part is a clamping wheel, so that a hose of an infusion apparatus or an extension tube of an indwelling needle is extruded and closed, and venous blood of a patient is prevented from flowing back into an infusion pipeline. When the next drip is needed, the clamping wheel is opened again to restore the smoothness of the infusion pipeline, and the infusion is implemented. The operation of opening (separating two wheels, not extruding and closing the infusion pipeline) and closing (approaching two wheels, extruding and closing the infusion pipeline) of the clamping wheel can be automatically completed by adopting an electronic control device, can be combined in an electronic and mechanical control mode, can be a purely mechanical driving structure or can be completed by adopting a purely manual mode. A specific implementation method of the opening and closing operation of the clamping wheel will be described in the following embodiments.
Further, regarding the selection of the setting position of the pinching device, the pinching device in the present embodiment is set at the following position: a hose between the flow speed regulator of the infusion set at the first position and the liquid medicine filter, a hose between the liquid medicine filter of the infusion set at the second position and the injection needle, and an extension tube between the water stop clamp of the retention needle and the transparent three-way inclined branch pipe at the third position. When the infusion device is directly used for infusion, the first and second positions are mainly the first and second positions, and are preferably arranged at the second position; when the infusion set is used for matching with the indwelling needle for infusion, the position (III) is the main position, and only one clamping device is arranged at the position (III) of the indwelling needle. The above arrangement is adopted because the clamping device should be positioned on the soft alternate pipeline closest to the injection point of the patient, so as to reduce the control range of the clamping device for closing the infusion pipeline as much as possible, reduce the connection joint point of the infusion device to the minimum, strengthen the tightness of the closed infusion pipeline and reduce the possibility of blood return.

The conveyor belt, the guide wheel and the driving wheel on the movable clamping part are symmetrically arranged with the conveyor belt, the guide wheel and the driving wheel on the fixed clamping part;
The transmission part comprises a plurality of groups of gears and a chain or belt for transmission among the gears; specifically, the upper parts of the fixed clamping part and the movable clamping part are respectively provided with a positioning wheel, and the positioning wheels are used for connecting the driving wheel of the conveyor belt with the transmission part in a transmission way; a reversing wheel is arranged between the two positioning wheels and is used for connecting the two positioning wheels in a transmission way, and enabling the two positioning wheels to reversely rotate, and when the movable clamping part is opened from the fixed clamping part along the hinge joint, the positioning wheels on the movable clamping part are always in transmission connection with the positioning wheels on the fixed clamping part; at least one positioning wheel or reversing wheel of the two positioning wheels and the two reversing wheels is in transmission connection with the driving part;
the positioning wheel and the reversing wheel on the movable clamping part are symmetrically arranged with the positioning wheel and the changing wheel on the fixed clamping part;
And a conversion wheel is arranged at the connection part of the transmission part and the driving part and is used for establishing a transmission connection relation between the driving part and the transmission part.
The motor is a speed reducing motor or a speed reducer is assembled on a conventional motor.
The motor is provided with a frequency converter for adjusting the rotation speed of the motor.
It should be noted that: referring to fig. 7-9, the flow direction of the liquid medicine in the infusion line is shown from top to bottom. The application method of the embodiment is as follows:
Firstly, starting a motor, wherein a power output shaft of the motor is in phase transmission connection with a conversion wheel by adopting a reversing transmission device, and the reversing transmission device is as follows: according to the direction of the power output shaft of the motor, the torque of the motor is transmitted to the conversion wheel through a bevel gear or a similar structure with a reversing transmission function, and then the conversion wheel is transmitted to the positioning wheel or the reversing wheel; for convenience of description, the working principle and process of the transmission part will be described by taking the conversion wheel driving positioning wheel as an example, of course, the conversion wheel driving reversing wheel and the clamping part are driven to work, the principle is the same, and the like;
The reversing transmission device adopts a small wheel to drive a large wheel and has a speed reduction function; meanwhile, the conversion wheel also adopts a mode of driving the large wheel by the small wheel, the rotating speed is reduced and then is transmitted to the positioning wheel, the positioning wheel transmits power to the positioning wheel of the movable clamping part through the reversing wheel, the positioning wheel drives the driving wheel on the conveying belt, the driving wheel drives the conveying belt to do circular motion, and finally the clamping wheel is made to do circular motion around the conveying belt. After the speed reduction, the rotating speed transmitted to the driving wheel is slow, and the rotating speed of the motor and the driving wheel can be further regulated by arranging a frequency converter (or using a frequency conversion motor) on the motor, so that the conveyor belt can do circular motion according to the time period required by medical staff. That is, by controlling the rotational speed, the time of the conveyor belt around a circle is equal to the interval time between two adjacent drops in the dropping funnel of the infusion apparatus. Further, the conveyor belt is in a side-standing trapezoid, the trapezoid is composed of an upper bottom, a lower bottom and a waist, after liquid drops in the dropping funnel drop down, the two clamping wheels move to the upper bottom of the trapezoid through the waist above the trapezoid, and at the moment, the clamping wheels squeeze and seal the infusion pipeline to stop infusion; in the conventional transfusion, the blood return is likely to occur at the moment, and the blood return causes may be that the transfusion pipeline is not tightly sealed, or venous blood is diffused into the transfusion pipeline, and the blood return causes also vary from person to person; however, even after the infusion pipeline is closed, the conventional infusion still has blood return. In this embodiment, after the clamping wheel seals the infusion line, the clamping wheel is driven by the motor and will continue to move downwards along the trapezoid upper bottom, at this time, the clamping wheel rolls the infusion line, and the travelling speed is slower, so that the pressure of the sealed infusion line end (the clamping device to the injection point position of the patient) is slightly greater than the venous pressure of the patient under the condition that the liquid medicine is pressed into the vein as little as possible, and the venous blood is prevented from flowing back and diffusing, thereby avoiding blood return and thrombus formation to block the infusion line (including the catheter of the injection needle and the indwelling needle). When a drop of liquid medicine is needed to be dropped, the clamping wheel returns to the position of the driving wheel through the waist and the lower bottom below the trapezoid, and enters a lower surrounding period.
[ Example 3]
As shown in fig. 10, on the basis of the above embodiment, the conveyor belt of this embodiment is provided with a slideway, and a foldable baffle is provided on a side wall of the slideway, and the baffle can fall down along the surrounding direction of the conveyor belt;
the clamping wheel is assembled in a slideway of the conveyor belt by adopting a sliding block, and the sliding block is limited in the slideway;
the section of the conveyor belt positioned at the lower bottom of the trapezoid is provided with a return conveyor belt, the return conveyor belt is in conveying connection with a positioning wheel, a reversing wheel or a driving wheel, and the rotating speed of the return conveyor belt is larger than that of the conveyor belt.
It should be noted that: when the embodiment is used, the clamping wheel is started at the trapezoid waist-up position, the sliding block of the clamping wheel is limited by the baffle plate in the slideway of the conveyor belt, and the clamping wheel is pushed to enter the trapezoid upper bottom section so as to seal and squeeze the infusion pipeline; when a surrounding period (namely the interval time between two drops in the dropping funnel) passes through the trapezoid lower waist and reaches the trapezoid lower bottom starting position, at the moment, the sliding block of the clamping wheel is contacted with the return conveying belt, and the rotating speed of the return conveying belt is greater than that of the conveying belt, and is preferably far greater than that of the conveying belt, so that the clamping wheel is dragged to roll over the baffle plate of the section corresponding to the return conveying belt on the slideway, and the clamping wheel is enabled to quickly return to the trapezoid upper waist starting position and enter the next period.
It should be finally noted that the above-mentioned embodiments are only for illustrating the technical solutions of the present invention and not for limiting the same, and although the present invention has been described in detail with reference to examples, it should be understood by those skilled in the art that modifications and equivalents may be made to the technical solutions of the present invention without departing from the spirit and scope of the technical solutions of the present invention, and they are all encompassed in the scope of the claims of the present invention.
