Detailed Description
In order to make the objects, technical solutions and advantages of the present application more apparent, the present application will be further described in detail with reference to the accompanying drawings and examples. It should be understood that the specific embodiments described herein are for purposes of illustration only and are not intended to limit the present application.
Aiming at the problem of low safety in the mode of manually inputting a medical scanning safety threshold by medical staff in the prior art, the application provides a method for automatically determining imaging scanning parameters of medical equipment.
It will be appreciated that the method proposed in the present application may be applied to various medical scanning imaging apparatuses, and may specifically be MRI (Magnetic Resonance Imaging ) apparatuses, and may also be multi-modality imaging apparatuses including positron emission tomography-magnetic resonance (PET-MR), magnetic resonance radiotherapy (MR Radiation) and the like, which are not limited herein. In one embodiment, as shown in fig. 1, there is provided an imaging scan parameter determining method, which is explained by taking as an example an application of the method to a processor that can determine imaging scan parameters, the method including the steps of:
Step S110, obtaining type information of an implant of a target object.
To determine imaging scan parameters of a target object, a processor first obtains type information of an implant within the target object. Nowadays, with the development of medical technology, patients wearing implants are increasing, and the safety influence degree of different implants on patients needing medical scanning is different, so that a processor needs to acquire type information of the implants, and the type information can be the type of the implants, the size of the implants, the position of the implants in a target object, the presence of ferromagnetism and the like. Alternatively, the implant may be of the type of one or more of arterial clamps, vascular prostheses, venous filters, cardiac pacemakers, cochlear prostheses, drug pumps, contraceptive rings, artificial joints, prosthetic valves, stents for various ducts, dentures, foreign metallic materials due to trauma, and the like. Taking a magnetic resonance scan as an example, when a patient removes an external implant (i.e., not in a magnetic resonance examination room), the patient can perform MR examination safely, however, in many cases, the implant existing in the patient cannot be removed, such as a cardiac pacemaker, a cochlear implant, a defibrillator, etc., so that it is more difficult to ensure the patient's safety. According to relevant regulations, within the magnetic resonance environment, the safety classes of implant devices can be divided into three main classes, MR Safe (MR Safe), MR Conditional Safe (MR Conditional) and MR Unsafe (MR unafe). Among these, the class of MR safe implant devices can be safely used within a magnetic resonance controlled access zone, such as implants made of non-conductive, non-metallic, non-magnetic materials such as plastics. Implant devices of the MR-conditional safety class can only be safely used within the magnetic resonance controlled access zone if certain magnetic resonance scanning conditions meet the requirements, i.e. no danger occurs under specific use conditions. Implant devices of the MR unsafe class cannot then be used safely in the region of the magnetic resonance controlled access, i.e. constitute a hazard in all MR environments. Thus, pre-acquisition of implant type information is of great importance for the safety of medical examinations.
The type information of the implant can be obtained through the logging information registered at the time of the target object visit, which can be stored in the hospital management information system (all Hospital Information System). The type information of the implant can be obtained from the historical diagnosis information of the visiting hospital, or from the information management systems or the historical diagnosis information of other hospitals in the regional or telemedicine. The type information of the implant can also be detected by means of an implant-specific detection device.
Step S120, determining a scan parameter safety threshold corresponding to the target object according to the type information of the implant and a preset corresponding relation between the implant type and the scan parameter safety threshold.
There is a certain difference in the corresponding safe scanning conditions for different types of implants, so that the corresponding safe scanning conditions need to be determined according to the specific type of implant. In this embodiment, the processor may obtain, in advance, a correspondence between different types of implants and a safety scanning condition (i.e., a scanning parameter safety threshold), where the correspondence may be established according to a relevant safety standard. After the processor acquires the type information of the implant of the target object, the safety scanning condition corresponding to the implant can be determined according to the corresponding relation, so that the medical scanning safety of the target object is ensured.
Optionally, the processor may also determine the safety class of the implant based on the implant type information before determining the scan parameter safety threshold based on the preset correspondence. For example, taking magnetic resonance scanning as an example, (1) when the processor determines that the implant of the target object is MR safe according to relevant safety standards or regulations, the MR scan can be directly performed without defining a scan parameter safety threshold of the target object. (2) When the processor determines that the implant of the target object is MR-condition safe according to the related safety standard or rule, the step of determining the scan parameter safety threshold according to the type information of the implant and the preset correspondence in the present embodiment is performed. (3) When the processor determines that the implant of the target object is MR-unsafe according to relevant safety criteria or regulations, it is directly determined that the target object cannot be MR-scanned.
Step S130, determining imaging scanning parameters corresponding to the target object according to the scanning parameter safety threshold corresponding to the target object.
Imaging scan parameters may include repetition Time (TR), echo Time (TE), inversion Time (TI), field of view (FOV), matrix (Matrix), inversion Angle (FA), flow compensation (flow compensation, FC), saturation technique (SAT), no phase wrap artifact (NPW), number of Scan Layers (NSL), slice thickness (slice gap), number of excitations (number of excitation, NEX), echo chain length (echo train length, ETL), sampling bandwidth (th), ratio of phase encoding direction to field of view in the read direction, etc. The imaging scan parameter-formed sequences may include Spin Echo (SE), fast Spin Echo (FSE), gradient echo (gradient recalled echo, GRE), phase-disturbing gradient echo (spoiled gradient recalled echo, SPGR), short-time inversion recovery, and the like. After determining the scan parameter safety threshold corresponding to the target object, the processor can determine the corresponding specific imaging scan parameter so as to facilitate the subsequent medical imaging scan processing of the target object according to the determined imaging scan parameter. For example, when the upper safety threshold of a certain scan parameter is M1 and the lower safety threshold is M3, a certain value M2 or a certain range between M1 and M3 may be taken as a specific value of the scan parameter. Optionally, the upper safety threshold may be a highest SAR threshold, a highest B1 field, a maximum gradient switching rate, a maximum PNS threshold, a maximum rate of change of magnetic field time, a maximum spatial magnetic field gradient threshold (spatial field gradient, SFG) that secures the target object. The lower security threshold may be the lowest SAR threshold, the minimum B1 field, the minimum gradient switching rate, the minimum PNS threshold, the minimum magnetic field time rate of change, the minimum SFG threshold employed to meet the minimum image quality.
Optionally, the processor may further combine other scan requirements in determining the imaging scan parameters, for example, requirements for image quality, requirements for scan speed, etc., and may combine one or more scan requirements to determine the imaging scan parameters, which may be specifically adjusted according to the actual situation.
The embodiment provides an imaging scanning parameter determining method, when medical imaging scanning is performed on a target object, only type information of an implant is required to be acquired, and a corresponding scanning parameter safety threshold can be determined according to a preset corresponding relation, so that a parameter setting process is simpler and more convenient, processing efficiency is improved, and the safety of imaging scanning can be greatly improved due to the fact that error recording or missing recording does not occur.
In one embodiment, obtaining information of a type of implant of a target object includes: identification information of the implant is obtained, and type information of the implant is determined according to the identification information.
Specifically, the processor may determine the type information of the implant according to the implant identification information, and the implant identification information may specifically be in the form of text, an icon, a code, etc., such as a bar code, a two-dimensional code, etc. After the processor acquires the identification information of the implant, the processor may search the local database for the implant type information corresponding to the identification information, or may search the cloud (server) for the implant type information corresponding to the identification information through network access, which is not limited herein.
In one embodiment, type information of an implant of a target object is acquired: a medical imaging scan record of the target object is acquired, and type information of the implant is acquired from the medical imaging scan record.
The processor may determine the type information of the implant according to a medical imaging scan record of the target object, and the medical imaging scan record of the target object may be obtained according to past medical record data of the target object or data stored in a medical service system. In addition, if the target object is not medical scanning on the current medical scanning equipment for the first time, historical scanning data matched with the target object can be searched from the historical scanning record of the current medical scanning equipment, and then implant type information of the target object is obtained from the historical scanning data.
In one embodiment, determining the scan parameter safety threshold corresponding to the target object according to the type information of the implant and the preset correspondence between the implant type and the scan parameter safety threshold includes: when the number of the implants is one, determining a scanning parameter safety threshold corresponding to the implants according to a preset corresponding relation between the types of the implants and the scanning parameter safety threshold; and taking the scan parameter safety threshold corresponding to the implant as the scan parameter safety threshold corresponding to the target object. That is, when only one implant is included in the target object, the scan parameter safety threshold corresponding to the target object may be determined according to the type information of the implant.
In one embodiment, determining the scan parameter safety threshold corresponding to the target object according to the type information of the implant and the preset correspondence between the implant type and the scan parameter safety threshold includes: when the implant at least comprises a first implant and a second implant, determining a first scanning parameter safety threshold corresponding to the first implant and a second scanning parameter safety threshold corresponding to the second implant according to a preset corresponding relation between the implant type and the scanning parameter safety threshold; and taking the first scanning parameter safety threshold and the second scanning parameter safety threshold as scanning parameter safety thresholds corresponding to the target object. That is, when a plurality of implants are included in the target object, it is necessary to determine a scan parameter safety threshold corresponding to the target object according to the type information of the plurality of implants, thereby further improving the safety of medical scanning.
Optionally, the processor may also determine the safety class of the plurality of implants based on the plurality of implant type information before determining the scan parameter safety threshold based on the plurality of implants. For example, taking a magnetic resonance scan, including implant a and implant B within the target subject, (1) when the processor determines that both implants A, B of the target subject are MR safe according to relevant safety standards or regulations, the MR scan can be performed directly without defining a scan parameter safety threshold for the target subject. (2) When the processor determines that the implant a of the target object is MR safe, B is MR safe, or B is MR safe, a is MR safe, or A, B is MR safe according to the relevant safety standard or rule, the step of determining the scan parameter safety threshold in this embodiment is performed. (3) When the processor determines that at least one of the implants A, B of the target object is MR unsafe according to relevant safety criteria or regulations, it may be directly determined that MR scanning of the target object is not possible.
It should be noted that, in the present embodiment, the case where the target object includes two implants is described, it can be understood that when the target object includes more than three implants, the principle is similar to that in the present embodiment, and will not be described herein.
In one embodiment, when the implants include at least a first implant and a second implant, determining imaging scan parameters corresponding to the target object according to scan parameter safety thresholds corresponding to the target object includes: determining a final scanning parameter safety threshold corresponding to the target object according to the first scanning parameter safety threshold and the second scanning parameter safety threshold; and determining imaging scanning parameters corresponding to the target object according to the final scanning parameter safety threshold.
In particular, the processor may take the intersection of the first scan parameter safety threshold and the second scan parameter safety threshold as the final scan parameter safety threshold. For example, for a certain scan parameter, the first scan parameter safety threshold corresponding to the first implant a is [ X1, X3], the second scan parameter safety threshold corresponding to the second implant B is [ X2, X4], and X1< X2< X3< X4, at this time, the intersection [ X2, X3] of the first scan parameter safety threshold and the second scan parameter safety threshold may be taken as the final scan parameter safety threshold.
It will be appreciated that in the actual process, instead of determining the final scan parameter safety threshold by taking the intersection, other methods may be used to determine the final scan parameter safety threshold, which is not limited herein.
In one embodiment, the imaging scan parameter safety threshold comprises: at least one of magnetic field strength, rate of change of magnetic field strength, specific absorption rate, maximum field spatial gradient. When performing magnetic resonance scanning on a target object with an implant in the body, imaging scanning parameters need to be defined in order to ensure the safety of the target object. Wherein, the magnetic field strength (B0) may refer to static magnetic field strength and gradient magnetic field strength; the magnetic field strength change rate (dB/dt) refers to the change rate of the magnetic field strength with time, and when dB/dt >0, the magnetic field is gradually increased; specific absorption rate (Specific Absorption Ratio, SAR) refers to the electromagnetic radiation energy absorbed by a mass of matter per unit time; in addition, since strong magnetic fields have an attractive force to metallic implants, which may cause implant rotation or torque, the effect is often measured using the product of the maximum field spatial gradient (SFG) and the magnetic field strength, and therefore, the maximum field spatial gradient needs to be defined.
In one embodiment, as shown in fig. 2, a magnetic resonance imaging scanning method is provided, the method comprising the steps of:
step S210, obtaining type information of an implant of a target object.
Step S220, determining a scan parameter safety threshold corresponding to the target object according to the type information of the implant and a preset corresponding relation between the implant type and the scan parameter safety threshold.
Step S230, determining imaging scanning parameters corresponding to the target object according to the scanning parameter safety threshold corresponding to the target object.
Step S240, performing magnetic resonance imaging scanning on the target object according to imaging scanning parameters corresponding to the target object.
The embodiment provides a magnetic resonance imaging scanning method, when performing magnetic resonance imaging scanning on a target object, only type information of an implant is required to be acquired, and a corresponding magnetic resonance scanning parameter safety threshold can be determined according to a preset corresponding relation, so that the setting process of the magnetic resonance scanning imaging parameter is simpler and more convenient, the processing efficiency is improved, and the safety of the magnetic resonance imaging scanning can be greatly improved because the condition of error recording or missing recording cannot occur.
In one embodiment, after determining the imaging scan parameters corresponding to the target object in step S230, the magnetic resonance imaging scan method further includes the following steps before step S240:
Acquiring scanning position information of a target object and imaging scanning parameters related to the scanning position information;
it is determined whether the implant will have an effect on the magnetic resonance scan based on the implant type information, the scan site information, and the imaging scan parameters.
Specifically, one or more of the following information may be determined by imaging scan parameters: the patient moves in the sickbed, such as advanced head, advanced foot, etc.; imaging a field of view; radio frequency field excitation range, gradient field range, etc. When the type information of the implant is MR condition safety, the scanning part is the head, the scanning body position is the head advanced, the implant of the patient is the fixing nail of the leg of the patient, the scanning part and the implant of the target object are not in the gradient field or the radio frequency field range, and are not interfered by magnetic resonance scanning or generate heat, and at the moment, the imaging scanning parameters can be increased to obtain the optimal imaging quality.
Optionally, determining whether the implant will affect the magnetic resonance scan may be performed, for example, by calculating the distance between the implant and the center of the FOV, by determining the sequence in which the scan site and the implant pass through the center of the magnetic field, by calculating the interaction force between the implant and the magnetic field, or by calculating the range of influence of the gradient field or the radio frequency field. In one embodiment, where the implant type information is MR conditional safety, the scan site is also the head and the scan position is the foot advanced, the patient's implant is a staple of the patient's leg, however during the movement of the head to the magnet center, the patient's leg implant is first moved to the magnet center. At this time, the implant can also affect the magnetic resonance scanning, so that the operator can stop the movement of the sickbed, readjust the moving state of the patient on the sickbed to be advanced, namely, adjust the scanning position of the imaging scanning parameters.
In this embodiment, after determining the imaging scan parameters, before performing the magnetic resonance scan, the doctor may also adjust the specific operation method of the magnetic resonance scan of the patient according to the actual situation, so as to avoid the implant from affecting the magnetic resonance scan result and improve the accuracy of the scan result.
In one embodiment, as shown in fig. 3, the magnetic resonance imaging scanning method further comprises: step S250, determining a parameter overrun scanning area according to imaging scanning parameters corresponding to a target object;
in step S260, during the scanning process, when the target object is detected in the parameter overrun scanning area, alarm information is output.
Taking the maximum field-space gradient (SFG) as an example, since a strong magnetic field has an attractive force to a metal implant, this attractive force may cause the implant to rotate or torque, for which case when a specific value (range) of SFG is set, the corresponding parameter overscan region can be determined. Specifically, as shown in fig. 4, a cross-sectional view of the magnetic resonance scanning apparatus in three directions is shown, wherein the diagram (a) in fig. 4 is a front view, and the parameter overrun scanning area determined according to SFG is an area S1; fig. (b) is a longitudinal view, and the parameter overrun scan area determined according to SFG is an area S2; fig. (c) is a plan view of the region S3, which is the parameter overrun scan region determined by SFG.
In this embodiment, the scanning object may be detected during the scanning process to determine whether the limb of the scanning object enters the parameter overrun scanning area (at least one of S1, S2, S3), and when it is detected that the limb of the scanning object enters the parameter overrun scanning area, alarm information may be output to remind the scanning object and related healthcare staff.
Alternatively, various monitoring devices, such as cameras, sensors, etc., may be employed to monitor the scanned object in order to ensure the safety of the target object.
Optionally, when the alarm information is output, the modes such as sound alarm, light alarm, message push alarm and the like can be adopted, so that the scanning object and related medical staff can know dangerous situations in time, and the safety of the target object is further ensured.
In one embodiment, during an actual magnetic resonance imaging scan, there may be a plurality of shifts in the patient bed position, where the scan result includes magnetic resonance images of multiple beds, i.e., the relative positional relationship of the implant to the scan site is different from one magnetic resonance image to another. Therefore, in this embodiment, the magnetic resonance imaging scanning method further includes the steps of:
Acquiring type information of an implant of a target object, specifically, the implant is a cardiac pacemaker;
acquiring a plurality of bed information related to target object scanning, such as a first bed, a second bed, a third bed and the like, wherein each bed information corresponds to one scanning area, such as a first scanning area, a second scanning area adjacent to the first scanning area, a third scanning area adjacent to the second scanning area and the like; in this embodiment, since the implant is a cardiac pacemaker, the rf field peak is defined to be less than 30uT.
Further, as shown in fig. 5, a schematic diagram is shown for dividing a scanning area of a target object, a scanning part of the target object is a lower limb blood vessel, a T1 area in the diagram can be determined to be an absolute forbidden scanning area according to the position relationship between the scanning part and an implant, and three bed positions T2, T3 and T4 capable of covering the lower limb blood vessel can be respectively determined according to the size of FOV. Further, according to the positional relationship between the scanning position and the implant, the moving direction of the patient's bed can be determined to be advanced, so that the region T4 is determined to be a first bed, the region T3 is determined to be a second bed, and the region T2 is determined to be a third bed.
For each bed, determining a scanning parameter safety threshold corresponding to a target object corresponding to each bed according to the type information of the implant and a preset corresponding relation between the implant type and the scanning parameter safety threshold; determining imaging scanning parameters corresponding to the target objects for each bed according to the scanning parameter safety threshold corresponding to the target objects corresponding to each bed; moving the sickbed to a first bed according to the bed information, and exciting a first scanning area by adopting imaging scanning parameters corresponding to a target object of the current bed to acquire a first image under the current bed; moving the sickbed to a second bed, and exciting a second scanning area by adopting imaging scanning parameters corresponding to a target object of the second bed to acquire a second image under the second bed; and moving the sickbed to a third bed, and exciting a third scanning area by adopting imaging scanning parameters corresponding to a target object of the third bed to acquire a third image under the third bed.
Optionally, the first image, the second image and the third image may be stitched to obtain a wide-range image of the target object.
It should be understood that, under reasonable conditions, although the steps in the flowcharts referred to in the foregoing embodiments are shown in order as indicated by the arrows, these steps are not necessarily performed in order as indicated by the arrows. The steps are not strictly limited to the order of execution unless explicitly recited herein, and the steps may be executed in other orders. Moreover, at least some of the steps in the flowcharts may include a plurality of sub-steps or stages that are not necessarily performed at the same time, but may be performed at different times, and the order of execution of the sub-steps or stages is not necessarily sequential, but may be performed in rotation or alternately with at least a portion of the sub-steps or stages of other steps or other steps.
In one embodiment, as shown in fig. 6, there is provided an imaging scan parameter determination apparatus including: a type acquisition module 110, a security threshold determination module 120, and a parameter determination module 130.
