Technical Field

Background
At present, according to statistics, the wound of oral cavity maxillofacial region accounts for about 10% -20% of the wound of the whole body, the wound of human skeleton caused by traffic accident or other accidents generally exists, wherein, the patient of human maxillofacial bone fracture is on the rising trend, the clinical treatment of maxillofacial bone fracture usually adopts incision reduction internal fixation, which mainly shows that the soft tissue outside the skeleton is firstly incised, and then bone nails or steel plates, steel wires and other devices are adopted to carry out bone internal fixation in tissues to treat maxillofacial bone fracture, and the following complications may occur in the operation mode: wound surface infection, retention of foreign bodies in the jaw bone, rejection of foreign bodies by human bodies, and particularly, the development of the jaw bone of children in the future can be influenced by internal fixation of children.
But the clinical internal fixation is widely applied, which not only produces negative psychological effect to patients, but also needs to wound the patients again to take out internal fixation objects sometimes after the fracture condition is recovered, which causes secondary wound to the patients to a certain extent, and brings difficulty to be overcome to medical care personnel and the patients;
external fixator among the prior art, though also can carry out the bolt reinforcement to the mandible, but not support the lower extreme of human mandible, lead to the recovery effect of disease injury department relatively poor.
Disclosure of Invention
The invention aims to provide an external mandible fracture fixing bracket with simple structure, good fixing stability and good restoration effect and a fixing method thereof aiming at the defects and the defects of the prior art.
In order to achieve the purpose, the invention provides the following scheme: the invention provides a mandible fracture external fixation support which comprises an external fixation rod assembly, an adjusting support assembly and a locking assembly, wherein the external fixation rod assembly comprises a first external fixation rod and a second external fixation rod, through grooves are formed in the first external fixation rod and the second external fixation rod, through holes are formed in the lower ends of the first external fixation rod and the second external fixation rod, the through grooves are not communicated with the through holes, the adjusting support assembly comprises an adjusting screw rod, a support assembly arranged on the adjusting screw rod, a first adjusting nut, a second adjusting nut and a first compression nut, an annular bulge is arranged at one end of a rod body of the adjusting screw rod, the diameter of the annular bulge is larger than that of the through holes, the diameter of the through holes is not smaller than that of the rod body, and the support assembly comprises a front end, a rear end and a locking assembly which are sequentially connected, The support structure comprises a first support rod, a support piece, a second support rod and a rear end, wherein inner holes for the rod body to pass through are formed in the front end and the rear end, the diameter of each inner hole is not smaller than that of the rod body, two ends of the first support rod are hinged with the front end and the support piece respectively, two ends of the second support rod are hinged with the rear end and the support piece respectively, and the rod body sequentially passes through a first outer fixing rod, a first adjusting nut, the front end, the rear end, a second adjusting nut, a second outer fixing rod and a first compression nut; locking assembly includes locking screw, sets up locking screw is last and is located link up last lock nut and lower lock nut of groove both sides, link up on the groove with go up lock nut with be provided with the bank of cells chamfer on the edge that lower lock nut contacted to form bank of cells chamfer inclined plane, go up lock nut with down lock nut with the one end of linking up the groove and contacting all is provided with the nut chamfer to form nut chamfer inclined plane, nut chamfer inclined plane can with both sides the bank of cells chamfer inclined plane contacts, locking screw is including the first external screw thread section, the second external screw thread section and the pointed end that connect gradually, go up lock nut with the lower lock nut interval sets up on the first external screw thread section.
Preferably, the nut chamfers are arranged at two ends of the upper locking nut and the lower locking nut; or the nut chamfers are arranged at both ends of the upper locking nut, and the nut chamfers are not arranged at the other end of the lower locking nut; or the other ends of the upper locking nut and the lower locking nut are not provided with the nut chamfers.
Preferably, the chamfer angles of the groove edges on the upper and lower surfaces of the first and second outer fixing rods are both first chamfer angles and both form first chamfer angle inclined surfaces, the nut chamfer angles arranged at one ends of the upper and lower locking nuts are both third chamfer angles and both form first nut chamfer angle inclined surfaces, and the first nut chamfer angle inclined surfaces can be in contact with the first chamfer angle inclined surfaces on both sides; or the groove edge chamfers on the upper surface and the lower surface of the first outer fixing rod and the second outer fixing rod are respectively a first chamfer and a second chamfer and respectively form a first groove edge chamfer inclined plane and a second groove edge chamfer inclined plane, the upper locking nut and one end of the lower locking nut are provided with the nut chamfers which are respectively a fourth chamfer and a fifth chamfer and respectively form a first nut chamfer inclined plane and a second nut chamfer inclined plane, the first nut chamfer inclined plane can be in contact with the first groove edge chamfer inclined plane, and the second nut chamfer inclined plane can be in contact with the second groove edge chamfer inclined plane.
Preferably, the lower lock nut is fixedly connected or threadedly connected with the first external thread section, wherein when the lower lock nut is fixedly connected with the first external thread section, the lower lock nut is welded or integrally formed with the first external thread section.
Preferably, a second compression nut is arranged between the first adjusting nut and the first outer fixing rod, and the second compression nut compresses and fixes the first outer fixing rod on the annular bulge; and/or a second compression nut is arranged between the second adjusting nut and the second external fixing rod, and the first compression nut and the second compression nut compress and fix the second external fixing rod.

Preferably, the first supporting rods are arranged in pairs between the front end and the supporting plates, and the two first supporting rods are symmetrically arranged; and/or the second support rods are arranged between the rear end and the support plate in pairs, and the two second support rods are symmetrically arranged.

The invention also provides a mandible fracture external fixation method, which comprises the following steps:
medical personnel respectively fix the second external thread sections of the paired locking screws in the non-fractured sections of the mandible of the patient, wherein the fixing direction of the locking screws is transverse fixing, when the locking screws are transversely fixed, the second external thread sections are arranged in the non-fractured sections of the mandible, the length of the second external thread sections extending into the mandible is opposite bone, and the fixing work of the locking screws is completed;
then, the medical staff fixes the locking screws arranged in pairs on the first outer fixing rod and the second outer fixing rod through an upper locking nut and a lower locking nut, wherein nut chamfer inclined planes on the upper locking nut and the lower locking nut can be in contact with groove edge chamfer inclined planes on two sides of a through groove on the first outer fixing rod and the second outer fixing rod, and the locking screw connection work is completed;
secondly, the medical staff connects an adjusting screw rod in an adjusting support assembly with the lower ends of the first outer fixing rod and the second outer fixing rod, and then pushes the front end and the rear end to move by adjusting a first adjusting nut and a second adjusting nut which are arranged on the adjusting screw rod;
and finally, fixing the lower end of the second outer fixing rod on the adjusting screw rod by medical personnel through a first compression nut and/or a second compression nut, and screwing the first compression nut and/or the second compression nut to complete the fixing work of the first outer fixing rod and the second outer fixing rod.
Compared with the prior art, the invention has the following beneficial effects:
1. the mandible fracture external fixation bracket and the fixation method thereof provided by the invention adopt the mode that the first external fixation rod and the second external fixation rod are provided with the through grooves for accommodating the locking screws, and the groove edges of the through grooves and the upper locking nut and the lower locking nut are respectively provided with the matched chamfer inclined planes, so that the external fixation rod and the locking screws can be separated, the operation is convenient to carry out, the contact surfaces of the upper locking nut and the lower locking nut and the external fixation rod are increased, and the technical purpose of firmly fixing the first external fixation rod, the second external fixation rod and the locking screws is realized.
2. The mandible fracture external fixation bracket and the fixation method thereof provided by the invention adopt the mode that the adjusting support component is arranged on the fixation bracket, so that after the operation (especially within a period of time after the operation is just finished), the mandible is not only subjected to the opposite tensioning force of the first external fixation rod and the second external fixation rod, but also subjected to the upward supporting force of the supporting sheet, the damaged part can be more attached, and the postoperative recovery effect is improved.

As shown in figures 1-2, the invention provides an external fixing bracket for mandibular fracture, which comprises an external fixing rod assembly, an adjusting and supporting assembly and a locking assembly, wherein the external fixing rod assembly comprises a first external fixing rod 1 and a second external fixing rod 2, the first external fixing rod 1 and the second external fixing rod 2 are both provided with through grooves 3, the lower ends of the first external fixing rod 1 and the second external fixing rod 2 are both provided with through holes, the through grooves 3 are not communicated with the through holes, the adjusting and supporting assembly comprises an adjusting screw 12, a supporting assembly arranged on the adjusting screw 12, a first adjusting nut 13, a second adjusting nut 14 and a first compression nut 15, one end of a rod body of the adjusting screw 12 is provided with an annular bulge 16, the diameter of the annular bulge 16 is larger than that of the through holes, the diameter of the through holes is not smaller than that of the rod body, and the supporting assembly comprises a front end 17, The support structure comprises a first support rod 18, a support sheet 19, a second support rod 20 and a rear end 21, wherein inner holes for a rod body to pass through are formed in the front end 17 and the rear end 21 respectively, the diameter of each inner hole is not smaller than that of the rod body, two ends of the first support rod 18 are hinged with the front end 17 and the support sheet 19 respectively, two ends of the second support rod 20 are hinged with the rear end 21 and the support sheet 19 respectively, and the rod body sequentially passes through a first external fixing rod 1, a first adjusting nut 13, the front end 17, the rear end 21, a second adjusting nut 14, a second external fixing rod 2 and a first compression nut 15; locking assembly includes locking screw 4, the setting is on locking screw 4 and is located and link up last lock nut 5 and lower lock nut 6 of 3 both sides in groove, link up and be provided with the bank chamfer on the edge that contacts with last lock nut 5 and lower lock nut 6 on the groove 3, and form bank chamfer inclined plane 7, the one end that goes up lock nut 5 and lower lock nut 6 and link up groove 3 and contact all is provided with the nut chamfer, and form nut chamfer inclined plane 9, nut chamfer inclined plane 9 can contact with both sides bank chamfer inclined plane 7, locking screw 4 is including the first external screw thread section 8 that connects gradually, second external screw thread section 10 and pointed end 11, it sets up on first external screw thread section 8 to go up lock nut 5 and lower lock nut 6 interval.
The inclination angles of the nut chamfer inclined plane 9 and the groove edge chamfer inclined plane 7 are the same or similar, so that the two inclined planes can be attached to each other, and the connection stability of the upper locking nut 5 and the lower locking nut 6 with the first outer fixing rod 1 and the second outer fixing rod 2 is enhanced; the chamfering angle can be determined according to specific conditions, and is 45 degrees, 60 degrees and the like; fillets are arranged on the outer side edges of the fixator, so that human body injury is avoided; in addition, the first outer fixing rod 1 and the second outer fixing rod 2 are provided with the through grooves 3 for accommodating the locking screws 4, and the groove edges of the through grooves 3 and the upper and lower locking nuts 6 are respectively provided with the matched chamfer inclined surfaces, so that the first outer fixing rod 1, the second outer fixing rod 2 and the locking screws 4 can be separated, the operation is convenient to carry out, the contact surfaces of the upper and lower locking nuts 6 and the first outer fixing rod 1 and the second outer fixing rod 2 are increased, and the technical purpose of firmly fixing the first outer fixing rod 1, the second outer fixing rod 2 and the locking screws 4 is realized; the rod body of the adjusting screw rod 12 is provided with external threads, and the diameter of the rod body comprises the length of the main body and the length of the threads; the mode of adjusting the supporting component is arranged on the fixing support, so that after the operation (especially in a period of time after the operation is just finished), the mandible not only receives the opposite tensioning force of the first outer fixing rod 1 and the second outer fixing rod 2, but also receives the upward supporting force of the supporting sheet 19, so that the injury part can be more attached, and the treatment effect is improved.
For convenience of use, nut chamfers are arranged at two ends of the upper locking nut 5 and the lower locking nut 6; or in order to reduce the manufacturing cost, both ends of the upper locking nut 5 are provided with nut chamfers, and the other end of the lower locking nut 6 is not provided with a nut chamfer; or the other ends of the upper locking nut 5 and the lower locking nut 6 are not provided with nut chamfers.
In order to enrich the variety of nuts, the structure is not limited, in the invention, the chamfer angles of the upper and lower sides of the first external fixing rod 1 and the second external fixing rod 2 are all first chamfer angles and form a first chamfer angle inclined plane 7, the nut chamfer angles arranged at one ends of the upper locking nut 5 and the lower locking nut 6 are all third chamfer angles and form a first nut chamfer angle inclined plane 9, and the first nut chamfer angle inclined plane 9 can be contacted with the first chamfer angle inclined planes 7 at the two sides; or the upper and lower groove edge chamfers of the first and second outer fixing rods 1 and 2 are respectively a first chamfer and a second chamfer, and respectively form a first groove edge chamfer inclined plane 7 and a second groove edge chamfer inclined plane 7, the nut chamfers arranged at one ends of the upper and lower locking nuts 5 and 6 are respectively a fourth chamfer and a fifth chamfer, and respectively form a first nut chamfer inclined plane 9 and a second nut chamfer inclined plane 9, the first nut chamfer inclined plane 9 can be contacted with the first groove edge chamfer inclined planes 7 at both sides, and the second nut chamfer inclined plane 9 can be contacted with the second groove edge chamfer inclined planes 7 at both sides.
In order to enrich the fastening form of the lower union nut 6, the invention provides that the lower union nut 6 is fixedly or threadedly connected to the first external thread section 8, wherein the lower union nut 6 is welded or integrally formed with the first external thread section 8 when the lower union nut 6 is fixedly connected to the first external thread section 8.
In order to facilitate the fixation of the first external fixation rod 1 and the second external fixation rod 2, a second compression nut 22 is arranged between the first adjusting nut 13 and the first external fixation rod 1, and the first external fixation rod 1 is compressed and fixed on the annular bulge 16 by the second compression nut 22; and/or a second compression nut 22 is arranged between the second adjusting nut 14 and the second external fixing rod 2, the second external fixing rod 2 is compressed and fixed by the first compression nut 15 and the second compression nut 22, namely the positions of the first external fixing rod 1 and the second external fixing rod 2 on the rod body of the adjusting screw rod 12 are kept unchanged by additionally arranging the second compression nut 22; furthermore, the through hole and the first and second compression nuts 15 and 22 are provided with a matching connection structure of the nut chamfered inclined surface 9 and the groove edge chamfered inclined surface 7 in the locking screw 4 assembly, thereby further improving the compression fixing effect.
In order to facilitate the support sheet 19 to be attached to the lower end of the mandible of a human body and improve the support effectiveness, the support sheet 19 is of a convex arc structure; further, in order to improve the comfort of the support sheet 19 contacting the lower jaw of the human body, a cushion is provided on the outer surface of the support sheet 19.
In order to improve the supporting stability, the first supporting rods 18 are arranged in pairs between the front end 17 and the supporting plate 19, and the two first supporting rods 18 are symmetrically arranged; and/or the second support bars 20 are arranged in pairs between the rear end 21 and the support plate 19, and the two second support bars 20 are symmetrically arranged.
In the invention, the other end of the rod body of the adjusting screw 12 is sleeved with a protective cover 23.
The invention also provides a mandible fracture external fixation method, which comprises the following steps:
medical staff respectively fix the second external thread sections 10 of the paired locking screws 4 in the non-fractured sections of the mandible of a patient, wherein the fixing direction of the locking screws 4 is transverse fixing, when the locking screws are transversely fixed, the second external thread sections 10 are arranged in the non-fractured sections of the mandible, the length of the second external thread sections 10 extending into the mandible is opposite bone, and the fixing work of the locking screws 4 is completed;
then, the medical staff fixes the locking screws 4 arranged in pairs on the first outer fixing rod 1 and the second outer fixing rod 2 through the upper locking nut 5 and the lower locking nut 6, wherein the nut chamfer inclined planes 9 on the upper locking nut 5 and the lower locking nut 6 can be in contact with the groove edge chamfer inclined planes 7 on two sides of the through groove 3 on the first outer fixing rod 1 and the second outer fixing rod 2, and the connection work of the locking screws 4 is completed;
secondly, the medical staff connects the adjusting screw 12 in the adjusting support assembly with the lower ends of the first and second external fixing rods 1 and 2, and then the medical staff pushes the front end 17 and the rear end 21 to move by adjusting the first and second adjusting nuts 13 and 14 arranged on the adjusting screw 12, and because the two ends of the first and second support rods 18 and 20 are hinged, the first and second support rods 18 and 20 will rotate along with the movement of the front end 17 and the rear end 21, and support the support sheet 19 upwards to contact with the lower part of the mandible of the human body, so as to complete the support work of the adjusting support assembly on the mandible;
finally, the medical staff fixes the lower end of the second external fixation rod 2 on the adjusting screw 12 through the first compression nut 15 and/or the second compression nut 22, and screws the first compression nut 15 and/or the second compression nut 22, thereby completing the fixation work of the first external fixation rod 1 and the second external fixation rod 2.
